ABSTRACT Background Hemato-oncological diseases have been described as a common cause of secondary immune thrombocytopenia (ITP). So far, studies on patients with active hemato-oncological disease and concurrent ITP are limited, making it difficult to provide clear treatment recommendations. Due to the underlying disease and cancer treatment, this patient population is especially vulnerable. Research design and methods This retrospective, single center pilot study investigated treatment response and time to treatment response in patients with newly diagnosed ITP, comparing those with active hemato-oncological disease, those in remission after malignancy, to those without any underlying malignancy. Results Comparable response rates to ITP treatment were observed across these groups, including the achievement of platelet counts above 30 G/L and complete response (platelet counts ≥ 100 G/L). However, patients with an active hematologic-oncological malignancy exhibited a significantly longer time to platelet recovery after initiating steroid therapy. Conclusion Our preliminary data suggest that standard treatment recommendations for primary ITP may be effective in patients with an active hematologic – oncologic malignancy. However, therapy responses could be significantly delayed, warranting closer monitoring. Given the pilot nature of this study and the limited sample size, these findings should be considered hypothesis-generating and require confirmation in larger studies.

This study aimed to identify serum biomarkers that distinguish patients with acute calcium pyrophosphate (CPP) crystal arthritis from those with rheumatoid arthritis (RA). This study included patients with acute CPP crystal arthritis and those with RA treated at a single centre. The screening set included 18 patients with acute CPP crystal arthritis and 12 patients with RA. Serum samples were collected from all patients. Additionally, paired samples were obtained from five patients with CPP crystal arthritis after the resolution of their arthritis. The validation set included 11 patients with CPP crystal arthritis and 13 with RA. Serum metabolites were profiled using gas chromatography-mass spectrometry (GC-MS). Orthogonal partial least squares discriminant analysis revealed good separation between patients with acute CPP crystal arthritis and those with RA and between the acute CPP crystal arthritis phase and the resolution phase in the same patients. A total of 101 metabolites were identified. β-Hydroxybutyrate (BHB) levels were significantly higher in patients with acute CPP crystal arthritis than in those with RA and were higher in the acute CPP crystal arthritis phase than in the resolution phase. In the validation cohort, BHB consistently distinguished acute CPP crystal arthritis from RA, with an area under the receiver operating characteristic curve of 0.748, sensitivity of 90.9%, and specificity of 69.2%. BHB is a potential diagnostic biomarker for distinguishing acute CPP crystal arthritis from RA. These findings highlight the potential of metabolomic analysis as a non-invasive diagnostic approach for CPP crystal deposition disease. Key Points • Serum metabolomic profiling identified distinct metabolic signatures distinguishing acute CPP crystal arthritis from rheumatoid arthritis. • β-Hydroxybutyrate was a potential differential biomarker between acute CPP crystal arthritis and RA. • Alterations in ketone body metabolism may contribute to crystal-induced inflammation.

Thrombotic complications cause morbidity after total joint arthroplasty (TJA). Historically, some surgeons used intraoperative intravenous heparin to reduce thrombosis, but the evidence is outdated. We hypothesized that in modern TJA, intravenous heparin would not be associated with fewer thrombotic complications. Patients who underwent primary total hip and total knee arthroplasty (THA and TKA, respectively) at a single center from 2017 to 2024 were included in this retrospective cohort study. The primary outcome was composite thrombotic complications at 90 days, including deep vein thrombosis, pulmonary embolism, myocardial infarction, and cerebrovascular accident. The secondary outcomes included 30-day complications, intraoperative blood losses, and red blood cell transfusions. The outcome was analyzed with propensity score-matched multivariable logistic regressions based on intravenous heparin administration as a binary variable. The model was controlled for clinically relevant covariates. A total of 3,132 THA and 3,407 TKA patients were identified. After propensity score matching, we identified nine of 921 heparin (1%) and five of 900 (0.6%) no-heparin THA patients and 14 of 1,397 heparin (1%) and 15 of 1,319 (1%) no-heparin TKA patients who experienced a thrombotic event. There was no association between heparin and 90-day thrombotic complications for THA (odds ratio 1.873; 95% confidence interval, 0.604 to 5.812; P = 0.277) or TKA (odds ratio 0.775; 95% confidence interval, 0.357 to 1.684; P = 0.520). Intraoperative blood losses were greater in the no-heparin groups for both THA and TKA cohorts; patients in the no-heparin group in the TKA cohort received more transfused units of packed red blood cells than those in the heparin group, but there was no difference between groups in the THA cohort. Intraoperative intravenous heparin was not associated with a reduction in thrombotic complications, and its routine use in primary TJA is not supported by this study.

Neonatal acute kidney injury (AKI) is common in infants born extremely preterm (EPT, < 28weeks' gestation). Neonatal AKI is independently associated with increased mortality and is likely to be associated with kidney dysfunction later in life. This study aimed to describe the epidemiology of AKI in EPT infants in a single tertiary neonatal centre. Retrospective study of EPT infants admitted to a tertiary neonatal centre in Australia between August 2020 and December 2022. AKI was defined using the Kidney Disease Improving Global Outcomes (KDIGO) definition (including creatinine and oliguria criteria). Comparisons between AKI and non-AKI cohorts used linear, logistic, or quantile regression. Of 204 infants admitted during the study period, 193 were included in the study (11 excluded due to death or medical transfer within 48h after birth). Sixty infants (31%) had an episode of AKI during their inpatient admission: 31 (51.7%) had AKI stage 1, 11 (18.3%) had stage 2, and 18 (30%) had stage 3. Among these, 27 (45%) were recognised by the treating team, 18 (30%) had the diagnosis documented on their discharge summary, and three (5%) were referred for nephrology follow-up. Compared with the non-AKI cohort, infants with AKI were of lower gestational age and lower birth weight and had greater exposure to nephrotoxic medication. Infants with AKI had more episodes of sepsis, more days on respiratory support, and more days admitted to a tertiary centre than those without AKI. AKI in EPT infants is common and associates with multiple comorbidities and treatments in the NICU. These data highlight risk factors on which AKI screening and stewardship programmes can be focused.
