Detection of synovial inflammation in the sacroiliac joint space through intravoxel incoherent motion imaging: an alternative to contrast agents.

We investigated the diagnostic accuracy of simplified intravoxel incoherent motion (IVIM) imaging for detecting synovial inflammation in the sacroiliac joint (SIJ) in a population with active sacroiliitis. In accordance with the Assessment of Spondyloarthritis International Society criteria, 86 SIJs of 46 patients with active sacroiliitis were included in this retrospective study conducted between November 2020 and January 2022. Based on T1-weighted post-gadolinium images, the SIJs were divided into two groups: synovial inflammation positive (SIP) (n = 28) and synovial inflammation negative (SIN) (n = 58). Synovial areas in the SIJ space were independently and blindly reviewed for the presence of inflammation by two radiologists with differing levels of expertise in radiology. Using four b values, apparent diffusion coefficient (ADC)= ADC (0, 800) and the simplified 3T IVIM method parameters true diffusion coefficient (D1)= ADC (50, 800), D= ADC (400, 800), f1= f (0, 50, 800), f2= f (0, 400, 800), pseudodiffusion coefficient (D*)= D* (0, 50, 400, 800), ADClow = ADC (0, 50), and ADCdiff= ADClow - D were generated voxel by voxel for each patient. The IVIM and ADC parameters at the SIN and SIP joints were compared. The D parameter was significantly increased in SIP areas (1.23 ± 0.34 × 10-3 mm2/s) compared with SIN areas (1.02 ± 0.16 × 10-3 mm2/s) (P = 0.004). Conversely, the D* parameter was significantly decreased in SIP areas (21.78 ± 3.77 × 10-3 mm2/s) compared with SIN areas (16.19 ± 4.58 × 10-3 mm2/s) (P < 0.001). When the optimal cut-off value of 1.11 × 10-3 mm2/s was selected, the sensitivity for the D value was 71% and the specificity was 72% [area under the curve (AUC): 0.716)]. When the optimal cut-off value of 21.06 × 10-3 mm2/s was selected, the sensitivity for the D* value was 78.6%, and the specificity was 79.3% (AUC: 0.829). The interclass correlation coefficient was excellent for f1, f2 D*, D, and ADCdiff, good for ADClow and D1, but reasonable for ADC. The presence of synovial inflammation in the SIJ can be evaluated with high sensitivity and specificity using only four b values through the simplified IVIM method without the need for a contrast agent. IVIM imaging is a technique that allows us to gain insights into tissue perfusion without the administration of contrast agents, utilizing diffusion-weighted images. In this study, for the first time, we demonstrated the potential of detecting synovial inflammation in the SIJ using IVIM, specifically through the pseudodiffusion (D*) parameter, without the need for contrast agents.

Evaluation of Local Vascular Perfusion in the Lower Extremities on Intravoxel Incoherent Motion Imaging before and after Endovascular Therapy.

To evaluate improvement in local vascular perfusion of the lower limbs on intravoxel incoherent motion (IVIM) imaging after endovascular therapy (EVT). IVIM imaging was performed on 20 lower limbs of 16 patients with lower extremity arterial diseases before and after EVT. To estimate IVIM, diffusion-weighted lower-limb axial images (number of slices = 25 and slice thickness = 3.5mm) were acquired using different b values (0, 300, and 1000s/mm2). IVIM imaging with the simplified IVIM techniques was performed. The perfusion-related coefficient (D* [10-3mm2/s]), perfusion fraction (f [%]), and D*f product (10-3mm2/s %) were calculated before and 2-3days after EVT. The ankle brachial index (ABI), mean D* (10-3mm2/s), mean f (%), and mean D*f product (10-3mm2/s %) before and after EVT were compared. Successful revascularization was achieved in all cases. After EVT, the mean ABI significantly increased from 0.59 ± 0.19 to 0.87 ± 0.15 (p < 0.001, paired t test). The mean D* (10-3mm2/s) (22.08 ± 3.26 versus 24.87 ± 2.65, p = 0.005, paired t test), and D*f product (10-3mm2/s%) (551.03 ± 79.02 versus 634.55 ± 76.96, p = 0.002, paired t-test) of the lower limbs significantly increased after EVT, whereas f (%) (25.00 ± 1.28 versus 25.52 ± 1.61, p = 0.261, paired t-test) did not significantly increased after EVT. D* (10-3mm2/s) and D*f product (10-3mm2/s %) on IVIM imaging could evaluate improvement in local vascular perfusion of the lower limbs after EVT. Level 4, Case Series.

Diffusion-Derived Vessel Density Computed From a Simplified Intravoxel Incoherent Motion Imaging Protocol in Pregnancies Complicated by Early Preeclampsia: A Novel Biomarker of Placental Dysfunction.

Early preeclampsia is associated with significant placental hypoperfusion. We explore the diagnostic value of placental diffusion-derived vessel density (DDVD), a biomarker derived from diffusion-weighted magnetic resonance imaging, which measures in vivo vessel microperfusion, in the differential diagnosis of normal and early preeclampsia pregnancies. This was a prospective study involving 29 controls and 17 singleton pregnancies affected by early preeclampsia. Nineteen pregnancies from 28 to 34 weeks of gestational age were included from the normal group for a comparison with the early preeclampsia group. Using a 3.0 T magnetic resonance imaging scanner, diffusion-weighted images were obtained with the diffusion weighting b values of 0, 20, and 40 s/mm2. DDVDmean was the mean of DDVDb0b20 and DDVDb0b40, while DDVDb0b20 and DDVDb0b40 refer to the diffusion-derived vessel density values computed from b=0 and 20 s/mm2 images, and from b=0 and 40 s/mm2 images, respectively. The correlation between DDVDmean and gestational age was examined using a linear regression model. The area under the curve of the DDVDmean for early preeclampsia pregnancies detection was calculated by the receiver operating characteristic analysis. As gestational age increased, DDVDmean linearly decreased. DDVDmean was significantly decreased in the early preeclampsia pregnancies compared with the normal pregnancies (52.72±46.73 versus 213.34±93.50 au/pixel; P<0.001). The area under the curve (DDVDmean) for discriminating between normal and early preeclampsia pregnancies regardless of fetal growth restriction was 0.954, and the area under the curve was 1.000 when early preeclampsia pregnancies without fetal growth restriction were excluded. DDVDmean, an in vivo vessel microperfusion measure, allowed total separation of normal and early preeclampsia pregnancies.

Simplified intravoxel incoherent motion DWI for differentiating malignant from benign breast lesions

BackgroundTo evaluate simplified intravoxel incoherent motion (IVIM) diffusion-weighted imaging (DWI) for differentiating malignant versus benign breast lesions as (i) stand-alone tool and (ii) add-on to dynamic contrast-enhanced magnetic resonance imaging.Methods1.5-T DWI data (b = 0, 50, 250, 800 s/mm2) were retrospectively analysed for 126 patients with malignant or benign breast lesions. Apparent diffusion coefficient (ADC) ADC (0, 800) and IVIM-based parameters D1′ = ADC (50, 800), D2′ = ADC (250, 800), f1′ = f (0, 50, 800), f2′ = f (0, 250, 800) and D*′ = D* (0, 50, 250, 800) were voxel-wise calculated without fitting procedures. Regions of interest were analysed in vital tumour and perfusion hot spots. Beside the single parameters, the combined use of D1′ with f1′ and D2′ with f2′ was evaluated. Lesion differentiation was investigated for lesions (i) with hyperintensity on DWI with b = 800 s/mm2 (n = 191) and (ii) with suspicious contrast-enhancement (n = 135).ResultsAll lesions with suspicious contrast-enhancement appeared also hyperintense on DWI with b = 800 s/mm2. For task (i), best discrimination was reached for the combination of D1′ and f1′ using perfusion hot spot regions-of-interest (accuracy 93.7%), which was higher than that of ADC (86.9%, p = 0.003) and single IVIM parameters D1′ (88.0%) and f1′ (87.4%). For task (ii), best discrimination was reached for single parameter D1′ using perfusion hot spot regions-of-interest (92.6%), which were slightly but not significantly better than that of ADC (91.1%) and D2′ (88.1%). Adding f1′ to D1′ did not improve discrimination.ConclusionsIVIM analysis yielded a higher accuracy than ADC. If stand-alone DWI is used, perfusion analysis is of special relevance.

Comparison of different ROI analysis methods for liver lesion characterization with simplified intravoxel incoherent motion (IVIM)

This study investigated the impact of different ROI placement and analysis methods on the diagnostic performance of simplified IVIM-DWI for differentiating liver lesions. 1.5/3.0-T DWI data from a respiratory-gated MRI sequence (b = 0, 50, 250, 800 s/mm2) were analyzed in patients with malignant (n = 74/54) and benign (n = 35/19) lesions. Apparent diffusion coefficient ADC = ADC(0,800) and IVIM parameters D1′ = ADC(50,800), D2′ = ADC(250,800), f1′ = f(0,50,800), f2′ = f(0,250,800), and D*' = D*(0,50,250,800) were calculated voxel-wise. For each lesion, a representative 2D-ROI, a 3D-ROI whole lesion, and a 3D-ROI from “good” slices were placed, including and excluding centrally deviating areas (CDA) if present, and analyzed with various histogram metrics. The diagnostic performance of 2D- and 3D-ROIs was not significantly different; e.g. AUC (ADC/D1′/f1′) were 0.958/0.902/0.622 for 2D- and 0.942/0.892/0.712 for whole lesion 3D-ROIs excluding CDA at 1.5 T (p > 0.05). For 2D- and 3D-ROIs, AUC (ADC/D1′/D2′) were significantly higher, when CDA were excluded. With CDA included, AUC (ADC/D1′/D2′/f1′/D*') improved when low percentiles were used instead of averages, and was then comparable to the results of average ROI analysis excluding CDA. For lesion differentiation the use of a representative 2D-ROI is sufficient. CDA should be excluded from ROIs by hand or automatically using low percentiles of diffusion coefficients.

Perfusion parameters derived from MRI for preoperative prediction of IDH mutation and MGMT promoter methylation status in glioblastomas

PurposeTo investigate the feasibility for preoperative prediction of IDH mutation and MGMT promoter methylation status in glioblastomas(GBMs) by intravoxel incoherent motion(IVIM) and dynamic susceptibility contrast(DSC). MethodsPreoperative IVIM and DSC images of 71 patients(IDH mutation:45, IDH wildtype: 26; MGMT methylation: 31, MGMT unmethylation:40) with glioblastomas were analyzed retrospectively. Perfusion parameters including microcirculation perfusion coefficient(D*), perfusion fraction(f), cerebral blood volume(CBV) and cerebral blood flow(CBF) were measured. Corrected perfusion parameters containing corrected perfusion coefficient(ADCperf) and simplified perfusion fraction(SPF) were from the simplified IVIM with 3 b values. Correlations among parameters were analyzed by Spearman correlation. All parameters were compared with Mann-Whitney U test. Univariate and multivariate logistic regression models were constructed. The receiver operating characteristic(ROC) curve was analyzed. ResultsThe IVIM parameters showed merely moderate correlations with CBV and showed no correlation with CBF. IDH mutation GBMs showed lower D*, ADCperf, SPF, CBV and higher f than IDH wildtype GBMs(all p < 0.05). D* was the independent predictor for IDH mutation with the highest AUC of 0.912(95%CI: 0.821–0.966). The D*, ADCperf, SPF and CBV of MGMT promoter methylation GBMs were lower than unmethylation GBMs while f was higher(all p < 0.05). Multivariate model showed the highest prediction efficacy for MGMT promoter methylation with an AUC of 0.915(95%CI: 0.824–0.968). The CBF was not useful in distinguishing IDH mutation and MGMT promoter methylation status(p = 0.055, 0.215). ConclusionIDH mutation and MGMT promoter methylation status in GBMs can be assessed effectively by IVIM and DSC. Besides, D* was the independent predictor of IDH mutation status.

Simplified intravoxel incoherent motion diffusion-weighted MRI of liver lesions: feasibility of combined two-colour index maps

BackgroundTo evaluate the feasibility of two-colour index maps containing combined diffusion and perfusion information from simplified intravoxel incoherent motion (IVIM) for liver lesion malignancy assessment.MethodsDiffusion-weighted data from a respiratory-gated 1.5-T magnetic resonance sequence were analysed in 109 patients with liver lesions. With three b values (0, 50, 800 s/mm2) estimated diffusion coefficient D′, perfusion fraction f′, and apparent diffusion coefficient (ADC) maps were calculated and analysed for regions of interest (ROIs). D′ and f′ cutoff values were determined by differentiating haemangiomas from other lesions and focal nodular hyperplasias from other lesions, respectively. Combined IDf index maps were generated with a voxel value set to 100, if both D′ and f′ voxel values were lower than their cutoff values (1,529.4 × 10-6 mm2/s and 114.4 × 10-3, respectively), otherwise to 0. Moreover, IADC index maps were generated from ADC cutoff value (1,338.5 × 10-6 mm2/s) obtained by differentiating benign from malignant lesions. Discriminatory power was assessed for both IDf and IADC. Index maps were displayed as two-colour overlays to b-800 images and visually assessed within the translucent hyperintense areas.ResultsFor IDf, the same diagnostic accuracy was achieved as for the combined use of parameters D′ and f′ (93.6%). Compared to IADC, IDf showed a higher diagnostic accuracy. Visual judgment of IDf yielded an accuracy (95.4%) similar to that of quantitative analysis (93.6%).ConclusionVoxel-wise combined two-colour index maps IDf provide similar diagnostic accuracy as ROI-based combination of estimated IVIM parameters D′ and f′ and are suitable for visual assessment of liver lesion malignancy.

Intravoxel incoherent motion (IVIM) modeling of diffusion MRI during chemoradiation predicts therapeutic response in IDH wildtype glioblastoma

BackgroundPrediction of early progression in glioblastoma may provide an opportunity to personalize treatment. Simplified intravoxel incoherent motion (IVIM) MRI offers quantitative estimates of diffusion and perfusion metrics. We investigated whether these metrics, during chemoradiation, could predict treatment outcome. Methods38 patients with newly diagnosed IDH-wildtype glioblastoma undergoing 6-week/30-fraction chemoradiation had standardized post-operative MRIs at baseline (radiation planning), and at the 10th and 20th fractions. Non-overlapping T1-enhancing (T1C) and non-enhancing T2-FLAIR hyperintense regions were independently segmented. Apparent diffusion coefficient (ADCT1C, ADCT2-FLAIR) and perfusion fraction (fT1C, fT2-FLAIR) maps were generated with simplified IVIM modelling. Parameters associated with progression before or after 6.9 months (early vs late progression, respectively), overall survival (OS) and progression-free survival (PFS) were investigated. ResultsHigher ADCT2-FLAIR at baseline [Odds Ratio (OR) = 1.06, 95% CI 1.01–1.15, p = 0.025], lower fT2-FLAIR at fraction 10 (OR = 2.11, 95% CI 1.04–4.27, p = 0.018), and lack of increase in ADCT2-FLAIR at fraction 20 compared to baseline (OR = 1.12, 95% CI 1.02–1.22, p = 0.02) were associated with early progression. Combining ADCT2-FLAIR at baseline, fT2-FLAIR at fraction 10, ECOG and MGMT promoter methylation status significantly improved AUC to 90.3% compared to a model with only ECOG and MGMT promoter methylation status (p = 0.001). Using multivariable analysis, neither IVIM metrics were associated with OS but higher fT2-FLAIR at fraction 10 (HR = 0.72, 95% CI 0.56–0.95, p = 0.018) was associated with longer PFS. ConclusionADCT2-FLAIR at baseline, its lack of increase from baseline to fraction 20, or fT2-FLAIR at fraction 10 significantly predicted early progression. fT2-FLAIR at fraction 10 was associated with PFS.

B-Value Optimization in the Estimation of Intravoxel Incoherent Motion Parameters in Patients with Cervical Cancer.

ObjectiveThis study aimed to find the optimal number of b-values for intravoxel incoherent motion (IVIM) imaging analysis, using simulated and in vivo data from cervical cancer patients.Materials and MethodsSimulated data were generated using literature pooled means, which served as reference values for simulations. In vivo data from 100 treatment-naïve cervical cancer patients with IVIM imaging (13 b-values, scan time, 436 seconds) were retrospectively reviewed. A stepwise b-value fitting algorithm calculated optimal thresholds. Feed forward selection determined the optimal subsampled b-value distribution for biexponential IVIM fitting, and simplified IVIM modeling using monoexponential fitting was attempted. IVIM parameters computed using all b-values served as reference values for in vivo data.ResultsIn simulations, parameters were accurately estimated with six b-values, or three b-values for simplified IVIM, respectively. In vivo data showed that the optimal threshold was 40 s/mm2 for patients with squamous cell carcinoma and a subsampled acquisition of six b-values (scan time, 198 seconds) estimated parameters were not significantly different from reference parameters (individual parameter error rates of less than 5%). In patients with adenocarcinoma, the optimal threshold was 100 s/mm2, but an optimal subsample could not be identified. Irrespective of the histological subtype, only three b-values were needed for simplified IVIM, but these parameters did not retain their discriminative ability.ConclusionSubsampling of six b-values halved the IVIM scan time without significant losses in accuracy and discriminative ability. Simplified IVIM is possible with only three b-values, at the risk of losing diagnostic information.

Clinical efficacy of simplified intravoxel incoherent motion imaging using three b-values for differentiating high- and low-grade gliomas.

In this study, we evaluated the efficacy of intravoxel incoherent motion (IVIM)-derived parameters calculated with three b-values in differentiating high-grade gliomas (HGGs) from low-grade gliomas (LGGs) by comparing those calculated with multiple b-values. Ten patients with LGG (ages 35.1±12.1 yrs; 4 males, 6 females) and 21 patients with HGG (ages 60.6±19.1 yrs; 10 males, 11 females) who underwent subsequent surgical resections were examined with both IVIM imaging and histopathological analysis. The IVIM diffusion-weighted imaging was conducted using a single-shot echo planar sequence with 13 b-factors (0, 10, 20, 30, 50, 80, 100, 200, 300, 400, 600, 800, and 1000 sec/mm2) at 3T. In the conventional IVIM analysis, the perfusion fraction (f) and true diffusion coefficient (D) were calculated by biexponential fitting model with 13 b-values. In the simplified method with the selected three b-values (0, 300, and 1000 sec/mm2), D simply corresponds to the slope of a straight line passing through two logarithmic signal intensities (SIs) at the b-values of 300 and 1000 s/mm2, and f corresponds to the difference between the intercept of this line and SI at the b-value of 0 sec/mm2. The maximum f (f-max) and minimum D (D-min) was measured in each tumor. The f-max values calculated with three b-values (12.8±5.9%) were significantly lower than those with 13 b-values (17.3±7.5%, p<0.0001), but a good correlation and agreement were observed between these sets of f-max values (r = 0.79, ICC = 0.87). In the IVIM imaging with both three and 13 b-values, the HGGs showed significantly higher f-max values compared to the LGGs (p<0.001, respectively). The D-min values calculated with three b-values (1.06±0.31 ×10−3 mm2/sec) was not different from those with 13 b-values (1.07±0.33 ×10−3 mm2/sec), and an excellent correlation and agreement were found between them (r = 0.99, ICC = 0.99). The simplified IVIM imaging using three b-values can efficiently differentiate HGGs and LGGs.

Short-Term Measurement Repeatability of a Simplified Intravoxel Incoherent Motion (IVIM) Analysis for Routine Clinical Diffusion-Weighted Imaging in Malignant Liver Lesions and Liver Parenchyma at 1.5 T

To evaluate measurement repeatability of parameters derived from simplified intravoxel incoherent motion (IVIM) analysis of diffusion-weighted imaging (DWI) using 3b-values. 24 patients (16 male, 8 female, mean age: 67 years) with hepatic malignancy (HCC: 10, metastases: 14) underwent 29 liver MRI examinations at 1.5 T. Respiratory-triggered DWI (b = 0, 50, 800 s/mm2) was acquired twice. Parameter maps of the apparent diffusion coefficient ADC(0,800), estimated diffusion coefficient D' and perfusion fraction f' were calculated. Measurement repeatability for a region of interest (ROI) placed in one lesion and liver parenchyma per lobe was assessed by intra-session variation coefficients (CV). 86 ROIs (43 lesions, 43 parenchymas) were analyzed. Parameters did not significantly differ between measurements. Repeatability was excellent for ADC(0,800) and D' and good for f' in parenchyma (CVs: 7.3 %, 9.8 %, 13.0 %) and lesions (CVs: 7.5 %, 8.5 %, 11.0 %). Differences in CV-values between liver and lesions were not significant. Repeatability was better for the right than for the left lobe by tendency, for parenchyma (CVs: 6.4 % vs 8.4 %, 8.8 % vs 10.9 %, 10.5 % vs 16.0 %) and for lesions (CVs: 6.9 % vs 8.1 %, 7.5 % vs 9.5 %, 9.5 % vs 12.7 %). Measurement repeatability is excellent for ADC(0,800) and D' values and good for f' values using the simplified IVIM approach, both in lesions and liver parenchyma. Accurate IVIM model-based liver lesion characterisation can be achieved with only three b-value DWI.

The objective of this study was to evaluate a simplified intravoxel incoherent motion (IVIM) approach of diffusion-weighted imaging (DWI) with four b-values for liver lesion characterisation at 1.5 T. DWI data from a respiratory-gated MRI sequence with b = 0, 50, 250, 800 s/mm2 were retrospectively analysed in 173 lesions and 40 healthy livers. The apparent diffusion coefficient ADC = ADC(0,800) and IVIM-based parameters D1' = ADC(50,800), D2' =ADC(250,800), f1', f2', D*', ADClow = ADC(0,50), and ADCdiff=ADClow-D2' were calculated voxel-wise without fitting procedures. Differences between lesion groups were investigated. Focal nodular hyperplasias were best discriminated from all other lesions by f1' with an area under the curve (AUC) of 0.989. Haemangiomas were best discriminated by D1' (AUC of 0.994). For discrimination between malignant and benign lesions, ADC(0,800) and D1' were best suited (AUC of 0.915 and 0.858, respectively). Discriminatory power was further increased by using a combination of D1' and f1'. IVIM parameters D and f approximated from three b-values provided more discriminatory power between liver lesions than ADC determined from two b-values. The use of b = 0, 50, 800 s/mm2 was superior to that of b = 0, 250, 800 s/mm2. The acquisition of four instead of three b-values has no further benefit for lesion characterisation. • Diffusion and perfusion characteristics are assessable with only three b-values. • Association of b = 0, 50, 800 s/mm2is superior to b = 0, 250, 800 s/mm2. • A fourth acquired b-value has no benefit for differential diagnosis. • For liver lesion characterisation, simplified IVIM analysis is superior to ADC determination. • Simplified IVIM approach guarantees numerically stable, voxel-wise results and short acquisition times.

Intravoxel incoherent motion model-based analysis of diffusion-weighted magnetic resonance imaging with 3 b-values for response assessment in locoregional therapy of hepatocellular carcinoma.

PurposeThe aim of this study was to evaluate an intravoxel incoherent motion (IVIM) model–based analysis of diffusion-weighted imaging (DWI) for assessing the response of hepatocellular carcinoma (HCC) to locoregional therapy.Patients and methodsRespiratory-gated DWI (b=0, 50, and 800 s/mm2) was retrospectively analyzed in 25 patients who underwent magnetic resonance imaging at 1.5 T before and 6 weeks following the first cycle of transarterial chemoembolization therapy, transarterial ethanol-lipiodol embolization therapy, and transarterial radioembolization therapy. In addition to the determination of apparent diffusion coefficient, ADC(0,800), an estimation of the diffusion coefficient, D′, and the perfusion fraction, f′, was performed by using a simplified IVIM approach. Parameters were analyzed voxel-wise. Tumor response was assessed in a central slice by using a region of interest (ROI) covering the whole tumor. HCCs were categorized into two groups, responders and nonresponders, according to tumor size changes on first and second follow ups (if available) and changes of contrast-enhanced region on the first follow up.ResultsIn total, 31 HCCs were analyzed: 17 lesions were assigned to responders and 14 were to nonresponders. In responders, ADC(0,800) and D′ were increased after therapy by ~30% (P=0.00004) and ~42% (P=0.00001), respectively, whereas f′ was decreased by ~37% (P=0.00094). No significant changes were found in nonresponders. Responders and nonresponders were better differentiated by changes in D′ than by changes in ADC(0,800) (area under the curve =0.878 vs 0.819 or 0.714, respectively).ConclusionIn patients with HCCs undergoing embolization therapy, diffusion changes were better reflected by D′ than by conventional ADC(0,800), which is influenced by counteracting perfusion changes as assessed by f′.

A Simplified Model for Intravoxel Incoherent Motion Perfusion Imaging of the Brain.

Despite a recent resurgence, intravoxel incoherent motion MRI faces practical challenges, including limited SNR and demanding acquisition and postprocessing requirements. A simplified approach using linear fitting of a subset of higher b-values has seen success in other organ systems. We sought to validate this method for evaluation of brain pathology by comparing perfusion measurements using simplified linear fitting to conventional biexponential fitting. Forty-nine patients with gliomas and 17 with acute strokes underwent 3T MRI, including DWI with 16 b-values (range, 0-900 s/mm2). Conventional intravoxel incoherent motion was performed using nonlinear fitting of the standard biexponential equation. Simplified intravoxel incoherent motion was performed using linear fitting of the log-normalized signal curves for subsets of b-values >200 s/mm2. Comparisons between ROIs (tumors, strokes, contralateral brain) and between models (biexponential and simplified linear) were performed by using 2-way ANOVA. The root mean square error and coefficient of determination (R2) were computed for the simplified model, with biexponential fitting as the reference standard. Perfusion maps using simplified linear fitting were qualitatively similar to conventional biexponential fitting. The perfusion fraction was elevated in high-grade (n = 33) compared to low-grade (n = 16) gliomas and was reduced in strokes compared to the contralateral brain (P < .001 for both main effects). Decreasing the number of b-values used for linear fitting resulted in reduced accuracy (higher root mean square error and lower R2) compared with full biexponential fitting. Intravoxel incoherent motion perfusion imaging of common brain pathology can be performed by using simplified linear fitting, with preservation of clinically relevant perfusion information.

Quantitative diffusion-weighted imaging and dynamic contrast-enhanced characterization of the index lesion with multiparametric MRI in prostate cancer patients.

To compare a simplified intravoxel incoherent motion (sIVIM) model to commonly used monoexponential and biexponential models in the characterization of prostate cancer (PCa) and noncancerous prostate tissues, and to investigate combinations of diffusion-weighted imaging (DWI) measures with dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI)-derived parameters in MRI-visible index lesions, to facilitate PCa risk stratification. In this retrospective, Institutional Review Board (IRB)-approved study, 43 consecutive patients with PCa who had 3T MRI exams followed by radical prostatectomy were included. DWI and DCE parameters were measured from one index lesion per patient, and noncancerous central gland and peripheral zone. Logistic regression modeling was performed to select the optimal combination of DWI and DCE measurements for tumor risk assessment. All diffusion models showed the lowest diffusion coefficients in tumors, intermediate values in noncancerous central gland, and highest values in noncancerous peripheral zone (all P < 0.001). Ktrans and kep were higher in tumors compared to central gland (P < 0.005) and peripheral zone (P < 0.001). The initial area under the contrast concentration curve was higher in tumor than the peripheral zone (P < 0.001). The area under the receiver operating characteristic curve of the combined DWI and DCE parameters (0.78) was higher than its individual components (0.73 and 0.63, respectively) for discriminating low- and intermediate-to-high-risk tumors. The sIVIM model provided comparable results with fewer b-values and shorter image acquisition time. The combination of DWI and DCE measurements of MRI-visible index lesions improved the preoperative prostate cancer risk characterization compared to the individual parameters from either technique alone. 3 J. Evaluation of a Simplified Intravoxel Incoherent Motion (IVIM) Analysis of Diffusion-Weighted Imaging for Prediction of Tumor Size Changes and Imaging Response in Breast Cancer Liver Metastases Undergoing Radioembolization: A Retrospective Single Center Analysis.

To investigate the value of a simplified intravoxel incoherent motion (IVIM) analysis for evaluation of therapy-induced tumor changes and response of breast cancer liver metastases (mBRC) undergoing radioembolization.In 21 females (mean age 54 years, range 43-72) with mBRC tumor size changes and response evaluation criteria in solid tumors (RECIST) response to 26 primary radioembolization procedures were analyzed. Standard 1.5-T liver magnetic resonance imaging including respiratory-gated diffusion-weighted imaging (DWI) with b0 = 0 s/mm, b1 = 50 s/mm, b2 = 800 s/mm before and 6 weeks after each treatment was performed. In addition to the apparent diffusion coefficient (ADC)(0,800), the estimated diffusion coefficient D' and the perfusion fraction f' were determined using a simplified IVIM approach. For each radioembolization, the 2 largest treated metastases (if available) were analyzed. Lesions were categorized according to size changes into group A (reduction of longest diameter [LD]) and group B (LD increase) after 3 months. Radioembolization procedures were further categorized into "response" (partial response and stable disease) and "nonresponse" (progressive disease) according to RECIST after 3 months. ADC and D' are given in 10 mm/s.Forty-five metastases were analyzed. Thirty-two lesions were categorized as A; 13 as B. Before therapy, group A lesions showed significantly larger f'-values than B (P = 0.001), but ADC(0,800) and D' did not differ. After therapy, in group A lesions the ADC(0,800)- and D'-values increased and f' decreased (P < 0.0001); in contrast in group B lesions f' increased (P = 0.001). Groups could be differentiated by preinterventional f' and by changes of D' and f' between pre and postinterventional imaging (area under the curve [AUC] of 0.903, 0.747 and 1.0, respectively).Preinterventional parameters did not differ between responders and nonresponders according to RECIST. ADC(0,800)- and D'-values showed a larger increase in responders compared with nonresponders (P = 0.013 and P = 0.001, respectively). After therapy f'-values decreased significantly in responders (P = 0.001). Good to excellent prediction of long-term RECIST response was possible by therapy-induced changes in LD, D', and f' (AUC 0.903, 0.879, and 0.867, respectively).A simplified IVIM model-based analysis of early post-treatment DWI can deliver additional information on tumor size changes and long-term RECIST response after radioembolization of mBRC. The estimated perfusion fraction f' is better suited for response assessment than the conventional ADC(0,800) or D'. This can be useful to guide further treatment strategy.
