A NOVEL USE OF FINESSE FLEX LOOP FOR PROLIFERATIVE VITREORETINOPATHY (FINESSE LOOP- ASSISTED PHOTOCOAGULATION)

Purpose: To describe a novel use of the flexible nitinol loop membrane scraper (Finesse Flex Loop; Alcon, Forth Worth, TX) for applying an effective endolaser photocoagulation in proliferative vitreoretinopathy (PVR) cases. Methods: A 27- gauge nitinol flex loop was used to assist applying endolaser photocoagulation to enhance its effectiveness in a case with PVR. Results: A 66-year-old female was presented with PVR after an unsuccessful pars plana vitrectomy and silicone oil injection for rhegmatogenous retinal detachment in her right eye. The patient underwent 25- gauge pars plana vitrectomy, preretinal membrane removal, inferior retinectomy, the nitinol flex loop assisted endolaser photocoagulation, and silicone oil reinjection. After the silicone oil extraction, the retina was reattached and her visual acuity had improved from 0.1 (20/200) to 0.3 (20/60). Conclusion: The nitinol flex loop can be used to assist endolaser photocoagulation and enhance its effectiveness in PVR cases and reduce the retinal excision area which results unnecessary large retinal defects that may lead to redetachment of the retina.

INTRAVITREAL METHOTREXATE INJECTION FOR THE TREATMENT AND PREVENTION OF PROLIFERATIVE VITREORETINOPATHY.

To report on our experience using intravitreal methotrexate (MTX) in patients with retinal detachment associated with proliferative vitreoretinopathy and/or open globe injury. This study performed a retrospective chart review of a consecutive series of 21 eyes of 21 patients who underwent serial intravitreal MTX injection for treatment and/or prevention of proliferative vitreoretinopathy from December 2021 to January 2024. Twenty-one patients underwent pars plana vitrectomy, membrane peeling, laser photocoagulation, silicone oil infusion, and intravitreal MTX injection. Postoperatively, all eyes received a series of intravitreal MTX (400 μg/0.1 mL) injections. Optimally, injections were administered weekly for 8 weeks and every 2 weeks for four weeks for a total of 13 injections, beginning intraoperatively at the conclusion of retinal reattachment surgery. Mean baseline preoperative and postoperative visual acuity was logarithm of the minimum angle of resolution 3.2 (approximately hand motions vision) and 2.5 (between CF and hand motions vision), respectively, yielding an average improvement in visual acuity of 0.7 logarithm of the minimum angle of resolution units (0 ETDRS lines/letters). These 21 patients received an average of 10.5 injections. With a single operation, detachments in 19 (90%) of 21 eyes were successfully reattached. Corneal epithelial defects were noted in 7 (33%) of 21 patients. Serial intravitreal MTX injection was associated with 90% single operation retinal reattachment rate in the setting of retinal detachment with proliferative vitreoretinopathy or retinal detachment at high risk of proliferative vitreoretinopathy.

Objective: To present the cases with a history of vitrectomy, that underwent penetrating keratoplasty (PKP) combined with pars plana infusion in a closed. Material and Methods: The medical records and surgical videos of the patients, with a history of pars plana vitrectomy (PPV), who underwent PKP were reviewed retrospectively. Demographic data, including age at the time of the PKP, gender; indication for the PPV, the time between PPV and PKP, indication for PKP, best-corrected visual acuity (BCVA, LogMAR), lens status and additional ocular diseases preoperatively; intraoperative complications and BCVA, graft status and complications postoperatively were recorded. Results: PKP with manually assisted pars plana infusion was performed in 16 vitrectomized eyes of 16 (7 female and 9 males) patients in this study. Silicone oil extraction was performed in 7 (43.7%) patients (anteriorly in 4, posteriorly in 3). The mean BCVA was improved statistically significantly at the last follow-up visit (2.23±0.3 vs 1.93±0.7, LogMAR, p=0.040). Optic atrophy was found in 11 (68.7%) patients. Choroidal detachment occurred in the early postoperative stage in 1 (6.2%) patient and managed with oral steroids. Graft rejection was seen in 6 (37.5%) patients. Four (25%) patients underwent repeated PKP and 2 (12.5%) patients were listed on the transplant list. At the last follow-up visit, 11 (68.7%) grafts were clear and 5 (31.2%) were edematous. Conclusion: PKP in vitrectomized eyes, pars plana infusion is a cheap and effective method that may prevent hypotony in these patients and the procedure may result in good visual outcomes where no additional retinal problems are accompanied.

A Multidisciplinary Experiment to Characterize Antifouling Biocompatible Interfaces via Quantification of Surface Protein Adsorption.

Novel biomaterial development is a rapidly growing field that is crucial because biomaterial fouling, due to rapid and irreversible protein adsorption, leads to cellular responses and potentially detrimental consequences such as surface thrombosis, biofilm formation, or inflammation. Therefore, biomaterial technology's fundamentals, like material biocompatibility, are critical in undergraduate education. Exposing undergraduate students to biomaterials and biomedical engineering through interdisciplinary experiments allows them to integrate knowledge from different fields to analyze multidisciplinary results. In this practical laboratory experiment, undergraduate students will characterize surface properties (contact and sliding angle measurements) for the antifouling polydimethylsiloxane (PDMS) polymer using a goniometer and a smartphone, as well as quantify protein adsorption on antifouling surfaces via a colorimetric assay kit to develop their understanding of antifouling surface characteristics, UV-vis spectroscopy, and colorimetric assays. The antifouling PDMS polymer is prepared by silicone oil infusion and compared to untreated control PDMS. The polymer hydrophobicity was demonstrated by static water contact angles of ~99° and 102° for control and antifouling PDMS surfaces, respectively. The control PDMS sliding angle (>90°) was significantly reduced to 9° after antifouling preparation. After 24 h incubation of polymer samples in a 200 mg/mL bovine serum albumin (BSA) solution, the surface adsorbed BSA was quantified using a colorimetric assay. The adsorbed protein on the fouling PDMS controls (29.1 ± 7.0 μg/cm2) was reduced by ~79% on the antifouling PDMS surface (6.2 ± 0.9 μg/cm2). Students will gain experience in materials science, biomedical engineering, chemistry, and biology concepts and better understand the influence of material properties on biological responses for biomaterial interfaces.

Clinical effect of peripheral capsule preservation in eyes with silicone oil tamponade.

BACKGROUNDAt present, silicone oil has been widely used in vitrectomy to deal with complex fundus diseases. Usually, cataract extraction is combined with vitrectomy. However, reducing the complications of silicone oil tamponade and facilitating the secondary implantation of intraocular lens (IOL) are still an urgent problem.AIMTo evaluate the clinical effect of vitrectomy combined with peripheral capsule preservation (PCP) in eyes with silicone oil tamponade.METHODSThis single-center retrospective analysis included 70 patients (73 eyes) who underwent vitrectomy and silicone oil tamponade combined with cataract surgery (stage I) between January 2015 and July 2019. All patients underwent selective reoperation for silicone oil extraction and IOL implantation (stage II) more than 3 mo after stage I. These patients were divided into three groups according to the different lens capsule preservation methods: 28 patients (31 eyes) in a whole capsule preserved (WCP) group, 17 (17 eyes) in a capsule absent (CA) group, and 25 (25 eyes) in a peripheral capsule preserved (PCP) group. Intraocular pressure (IOP), best-corrected visual acuity, surgery time, and other complications were recorded at each time point (1 d, 1 wk, and 1 mo after stages I and II).RESULTSThe IOP values were 14.9 ± 8.2 mmHg in the WCP group, 20.3 ± 13.0 mmHg in the CA group, and 14.2 ± 9.7 mmHg in the PCP group (P < 0.05) at 1 mo after stage I operation. Five eyes had IOP higher than 30 mmHg, and one eye in the WCP group appeared to have silicone oil entering the anterior chamber. There was no significant difference in IOP among the three groups at any other time point (P > 0.05). With IOL implantation, visual acuity improved significantly compared to stage I. The incidence rate of posterior capsule opacity was higher in the WCP group than in the other groups (P < 0.001). In the CA group, IOL deviation due to suture relaxation occurred in one case. There was no significant difference in the surgery time among the three groups in stage I (P = 0.618). In stage II, the surgery time of the PCP group and WCP group was significantly shorter than that of the AC group (P = 0.031).CONCLUSIONPreservation of the peripheral capsule in vitrectomy combined with lens removal is a better option. This method has significant advantages in reducing intraoperative and postoperative complications.

Self-Propelled and Electrobraking Synergetic Liquid Manipulator toward Microsampling and Bioanalysis.

Droplet manipulation is of paramount significance for microfluidics-based biochips, especially for bioanalytical chips. Despite great progresses made on droplet manipulation, the existing bioanalytical methods face challenges in terms of capturing minute doses toward hard-to-obtain samples and analyzing biological samples at low temperatures immediately. To circumvent these limitations, a self-propelled and electric stimuli synergetic droplet manipulator (SES-SDM) was developed by a femtosecond laser microfabrication strategy followed by post-treatment. Combining the inspiration from cactus and Nepenthes pitcher plants, the wedge structure with the microbowl array and silicone oil infusion was endowed cooperatively with the SES-SDM. With the synergy of the ultralow voltage (4.0 V) stimuli, these bioinspired features enable the SES-SDM to transport the droplet spontaneously and controllably, showing the maximum fast motion (15.7 mm/s) and long distance (96.2 mm). Remarkably, the SES-SDM can function at -5 °C without the freezing of the droplets, where the self-propelled motion and electric-responsive pinning can realize the accurate capture and real-time analysis of the microdroplets of the tested samples. More importantly, the SES-SDM can realize real-time diagnosis of excessive heavy metal in water by the cooperation of self-propulsion and electro-brake. This work opens an avenue to design a microsampling (5-20 μL) manipulator toward producing the minute samples for efficient bioanalysis and offers a strategy for microanalysis using the synergistic droplet manipulation.

Synthesis and Characterization of Hydrophobic Polystyrene Microspheres Film

A facile strategy for hydrophobic films based on oil-infused polystyrene microspheres with the unit of γ-methacryloxypropyltrimethoxysilane is demonstrated. Specifically, the rough polystyrene microspheres with unit of γ-methacryloxypropyltrimethoxysilane are synthesized by suspension polymerization. These hydrophobic films were used, for the first time, as rough substrates for loading low surface tension silicone oils as lubricating liquids for the fabrication of polystyrene films. The properties of the films can be well tuned by adjusting the toluene and γ-methacryloxypropyltrimethoxysilane balance of the polystyrene microspheres and the mass ratio of toluene to γ-methacryloxypropyltrimethoxysilane reached 7 : 1.5 was found to be the most effective. The hydrophobic films could easily repel water and the water contact angle of the film was 150° before infusing oil and reached 139° after infusing oil. The morphology, structure and composition of polystyrene microspheres were characterized by SEM and FTIR. The content of trapped lubricant was quantified by silicone oil decomposition and measured by thermogravimetric analysis. Bacterial attachment tests showed that silicone oil infusion inhibited the attachment and growth of E. coli. The resulting films maintain water repellency and slipperiness after freezing, water flowing, and friction.

Intraocular pressure changes during and after silicone oil endotamponade (Review).

Silicone oil (SIO) has rapidly become an indispensable adjunct in vitreoretinal surgery. Constant improvements in purity and also in viscosity have not totally prevented specific complications that may occur during endotamponade. Results of in vitro studies that suggested that higher viscosity silicone oil might be superior in terms of stability and safety are confirmed in real life only if endotamponade lasts for more than 6 months. Intraocular pressure changes induced by the silicone oil endotamponade or oil extraction are documented from its very first use and are potentially threatening vision. The purpose of this review is to update current knowledge on the incidence, risk factors, pathogenesis, and management of secondary silicone oil glaucoma. Also, in a retrospective evaluation on cases with complex retinal detachments that underwent 23G vitrectomy and high viscosity SIO endotamponade, we have noticed that a considerable number of cases developed significant intraocular pressure changes during SIO endotamponade and after SIO removal, especially in early postoperative period.

Autologous neurosensory retinal transplantation for recurrent macular hole retinal detachment in highly myopic eyes.

To investigate the morphological and functional reconstruction of the macular fovea after autologous neurosensory retinal transplantation for recurrent macular hole retinal detachment (MHRD) in highly myopic eyes. Ten consecutive cases of recurrent MHRD with high myopia were retrospectively reviewed. All eyes underwent pars plana vitrectomy combined with autologous neurosensory retinal transplantation and were followed up for at least 3months after silicone oil extraction. The main outcomes were whether or not the retina was reattached and the macular hole (MH) was closed, morphological changes in the retinal graft, best-corrected visual acuity (BCVA), the sensitivity threshold and blood flow signal in the macula. At the one month postoperative visit, there was an obvious boundary between the graft and the surrounding retinal tissue, and some retinal structural layers could be seen in the graft on optical coherence tomography scans. At the final follow-up, eight eyes (80%) showed retinal reattachment and closure of the MH. Optical coherence tomography revealed blurring of the boundary between the graft and surrounding retinal tissue and that the retinal structure in the graft was disordered. The MH was not closed in two eyes (20%), in one case because of partial displacement of the graft and in the other because of incomplete coverage of the MH as a result of a smaller graft. The post-BCVA was significantly better than the pre-BCVA (1.32±0.33 versus 2.01±0.29 logMAR; p=0.000, paired t-test). Autologous neurosensory retinal transplantation can be an effective treatment for recurrent MHRD in highly myopic eyes. 'Fusion' between the neurosensory retinal graft and the original retinal tissue may be the mechanism involved in the closure of the MH and reconstruction of the macular fovea.

Intravitreal Conbercept Injection as an Adjuvant in Vitrectomy with Silicone Oil Infusion for Severe Proliferative Diabetic Retinopathy.

Purpose: To assess the clinical effects of preoperative, intraoperative, or preoperative combined with intraoperative intravitreal conbercept (IVC) injection in vitrectomy with silicone oil tamponade for severe proliferative diabetic retinopathy (PDR). Methods: Ninety-eight eyes of 98 severe PDR patients undergoing vitrectomy with silicone oil tamponade were randomly assigned to 3 groups: Group 1 (34 eyes) received IVC injections 3 to 5 days before surgery; Group 2 (35 eyes) received IVC injections at the end of surgery; and Group 3 (29 eyes) received IVC injections 3 to 5 days before and at the end of operation. Follow-up examinations were performed for 6 months. Results: The incidence and severity of intraoperative bleeding were not significantly different (P = 0.233). However, the duration of surgery was significantly shorter in Group 1 and Group 3 compared with Group 2 (P < 0.001). The incidences of early and late recurrent vitreous hemorrhage (VH) were 32.35%, 28.57%, and 13.80%, respectively. At 6-month follow-up, mean best-corrected visual acuity had significantly increased to 1.25 ± 0.45 logMAR in Group 1, 1.29 ± 0.46 logMAR in Group 2, 1.16 ± 0.44 logMAR in Group 3 (all P < 0.001). The incidence of postoperative VH, neovascular glaucoma, and retinal detachment in Group 3 was slightly lower, however, no significant differences were observed (all P > 0.05). In young patients, similar results were observed and Group 3 had better visual improvements (P = 0.037). Conclusions: Preoperative IVC injection could be a safe and effective adjunct in pars plana vitrectomy with silicone oil tamponade for severe PDR. Preoperative combined with intraoperative IVC are promising, especially in young patients.

Amniotic membrane for retinal detachment due to paravascular retinal breaks over patchy chorioretinal atrophy in pathologic myopia.

To describe a new surgical technique, using a human amniotic membrane patch, in two cases of retinal detachment with paravascular retinal breaks over patchy chorioretinal atrophy in pathologic myopia, already underwent pars plana vitrectomy with the internal limiting membrane peeling for myopic foveoschisis. Surgical technique description with surgical video. A 23-gauge pars plana vitrectomy was performed. A human amniotic membrane patch was implanted under the neuroretina to seal the posterior retinal break. Standard silicone oil tamponade was performed at the end of the surgery. The patients were positioned face down after the operation for the first week. Optical coherence tomography scans were used in the follow-ups. The 2 weeks postoperative optical coherence tomography showed a new tissue growth over the human amniotic membrane patch. The 3 months optical coherence tomography showed the new tissue entirely covering the human amniotic membrane plug. Visual acuity improved from 20/2000 (2 LogMAR) to 20/250 (1.1 LogMAR) 3 months after the operation in both patients. The silicone oil was extracted 2 months after surgery, and no recurrences were observed. The patient's visual acuity remained stable at 20/250 after the silicone oil extraction. In these complex cases, human amniotic membrane transplantation can be a valid option, when internal limiting membrane has already been peeled in previous surgeries, to seal the posterior retinal breaks over high myopic chorioretinal atrophy, with encouraging results.

Conbercept and Ranibizumab Pretreatments in Vitrectomy with Silicone Oil Infusion for Severe Diabetic Retinopathy.

We compared the efficacies of intravitreal ranibizumab (IVR) and intravitreal conbercept (IVC) as the adjuvant pretreatments for vitrectomy with silicone oil infusion for tractional retinal detachment (TRD) secondary to proliferative diabetic retinopathy. This retrospective study comprised 74 patients (79 eyes) who underwent vitrectomy with silicone oil tamponade for diabetic TRD. They received IVC (37 eyes) or IVR (42 eyes) at standard doses 3-5 days preoperatively and were followed up for ∼6 months. Anatomic success rate, intra- and postoperative complications, and visual outcomes were compared between both groups. Initial (IVC vs. IVR: 97% vs. 98%) and final anatomic success rates (100% in each group) and mean visual acuity changes were not significantly different (P = 0.46). Intraoperative complications [iatrogenic retinal breaks (P = 0.58) and intraoperative bleeding (P = 0.66)], postoperative complications [fibrin formation (P = 0.51), postoperative preretinal bleeding (P = 0.88), progressing or persistent neovascular glaucoma (P = 0.63), progressive fibrovascular proliferation (P = 0.93), and recurrent retinal detachment (P = 0.93)], and surgical variables [surgical time (P = 0.53)] were similar between both groups. Conbercept and ranibizumab are equally effective surgical adjuvants for vitrectomy with silicone oil infusion in patients with diabetic TRD.

A late-onset interface fluid syndrome post laser-assisted in situ keratomileusis: Diagnostic and therapeutic challenge

A 43-year-old male underwent laser-assisted in situ keratomileusis and maintained good vision for 9 years. Later, he developed interface fluid syndrome secondary to steroid-induced glaucoma as a consequence of the use of steroids postoperatively for vitrectomy with silicone oil infusion for retinal detachment and cataract surgery.

Active silicone oil removal with 23 gauge transconjunctival system; ‘'Doganay silicone oil extraction system''

Aim: To describe a new and simple silicone oil removal method with 23-gauge (G) transconjunctival vitrectomy system. Material and Methods: This is a prospective, single center, interventional clinical trial. A hundred thirty-three eyes of 133 patients were enrolled in this study. 1000 centistoke (cSt) silicone oil was removed with our new method. Main outcome measurements were silicone oil removal time, number of sutured 23 G sclerotomy sites, intraoperative and postoperative complications, preoperative and postoperative intraocular pressure (IOP) alterations, and preoperative and postoperative visual acuity changes. Results: The mean time between pars plana vitrectomy surgery with silicone oil endotamponade and silicone oil removal was 8.5±3.5 months. Inverted internal limiting membrane insertion combined with air tamponade in the treatment of macular hole retinal detachment in high myopia: study protocol for a randomized controlled clinical trial

BackgroundMacular hole retinal detachment (MHRD) occurs most commonly in high myopia and causes severe visual impairment and greatly reduces the quality of life. The aim of this study is to evaluate the efficacy and safety of inverted internal limiting membrane insertion combined with air tamponade in the treatment of MHRD in high myopia, and also to compare the treatment efficacy with that of the conventional “vitrectomy plus internal limiting membrane peeling plus silicone oil tamponade” method for high myopia-associated MHRD.Methods/designIn this clinical trial, 38 patients with MHRD in high myopia will be randomly assigned to two groups (Group 1: standard 3-port 23-gauge pars plana vitrectomy plus internal limiting membrane peeling plus air-fluid exchange plus silicone oil infusion; Group 2: standard 3-port 23-gauge pars plana vitrectomy plus internal limiting membrane peeling plus inverted internal limiting membrane insertion plus air-fluid exchange). The primary outcome is macular hole closure rate in 3 months after the initial surgery. Novel and easy techniques for 27-gauge silicone oil infusion and removal.

To introduce novel and easy techniques for 27-gauge silicone oil (SO) infusion and removal. Consecutive patients treated with 27-gauge pars plana vitrectomy (PPV) plus SO infusion (Infusion Group) and scheduled to have SO removal (Removal Group) were prospectively included, respectively. Patients in Infusion Group underwent 27-gauge PPV plus SO infusion. SO infusion was performed with a 24-gauge intravenous catheter connected with the SO syringe. Patients in Removal Group underwent machine-independent SO removal using a short section of infusion tube connected with a 10-mL syringe. Main outcomes were best-corrected visual acuity, intraocular pressure, surgical time and intra- and postoperative complications. There were thirty-five eyes (35 patients) and forty eyes (40 patients) included in Infusion and Removal Groups, respectively. Mean surgical time of complete SO infusion and removal was 5.5 ± 0.9 and 9.6 ± 2.1min, respectively. In both groups, no patient experienced postoperative vision deterioration or hypotony. No obvious intra- and postoperative complications were observed. We recommend the use of the 24-gauge catheter method for 27-gauge silicone oil infusion when commercial infusion cannula is unavailable. The machine-independent method using easily available plastic infusion tube and syringe would be an ideal option when 27-gauge surgery is anticipated.

Outcomes of transconjunctival sutureless 27-gauge vitrectomy for vitreoretinal diseases.

To evaluate the safety and efficacy profile of 27-gauge (27G) pars plana vitrectomy (PPV) for the treatment of various vitreoretinal diseases. The clinical outcomes of 61 eyes (58 patients) with various vitreoretinal diseases following 27G PPV were retrospectively reviewed. Surgical indications included rhegmatogenous retinal detachment (n=24), full-thickness macular hole (n=12), diabetic retinopathy (n=11), vitreous hemorrhage (n=6), Eales disease (n=4), pathological myopia-related vitreous floater (n=2), and macular epiretinal membrane (n=2). The mean follow-up was 166.4±61.3d (range 98-339d). The mean logMAR best-corrected visual acuity (BCVA) improved from 1.7±1.1 [0.02 decimal visual acuity (VA) equivalent] preoperatively to 1.2±1.0 (0.06 decimal VA equivalent) at the last postoperative visit (P<0.001). The mean operative time was 49.9min. With the exception of complicated cataract in one eye, no intraoperative complications were encountered. No case required conversion to conventional 20-, 23- or 25G instrumentation in all surgical maneuvers except for silicone oil infusion, which required a 25G oil injection syringe. Postoperative complications included transient ocular hypertension, vitreous hemorrhage, persistent intraocular pressure elevation, subconjunctival oil leakage, and recurrent retinal detachment. No cases of hypotony, endophthalmitis, and sclerotomy-related tears were observed. The current results suggest that 27G PPV system is a safe and effective treatment for various vitreoretinal diseases. When learning to perform 27G PPV, surgeons may encounter a learning curve and should gradually expand surgical indications from easy to pathologically complicated cases.

Marine Antifouling Behavior of Lubricant-Infused Nanowrinkled Polymeric Surfaces.

A new family of polymeric, lubricant-infused, nanostructured wrinkled surfaces was designed that effectively retains inert nontoxic silicone oil, after draining by spin-coating and vigorous shear for 2 weeks. The wrinkled surfaces were fabricated using three different polymers (Teflon AF, polystyrene, and poly(4-vinylpyridine)) and two shrinkable substrates (Polyshrink and shrinkwrap), and Teflon on Polyshrink was found to be the most effective system. The volume of trapped lubricant was quantified by adding Nile red to the silicone oil before infusion and then extracting the oil and Nile red from the surfaces in heptane and measuring by fluorimetry. Higher volumes of lubricant induced lower roll-off angles for water droplets, and in turn induced better antifouling performance. The infused surfaces displayed stability in seawater and inhibited growth of Pseudoalteromonas spp. bacteria up to 99%, with as little as 0.9 μL cm-2 of the silicone oil infused. Field tests in the waters of Sydney Harbor over 7 weeks showed that silicone oil infusion inhibited the attachment of algae, but the algal attachment increased as the silicone oil was slowly depleted over time. The infused wrinkled surfaces have high transparency and are moldable, making them suited to protect the windows of underwater sensors and cameras.
