Examining prognostic scores for short-term major adverse cardiovascular events prediction following primary PCI: a comparative analysis

Abstract Background Accurately predicting short-term MACE (major adverse cardiac events) following primary PCI remains a clinical challenge. This study aims to assess the effectiveness of four established risk scores in predicting short-term MACE after primary PCI (percutaneous coronary intervention). Methodology We enrolled a cohort of consecutive adult patients diagnosed with STEMI (ST elevation myocardial infarction) undergoing primary PCI in this prospective observational study. Patients were followed at the interval of 3 months up for up to 12 months, and MACE events were recorded. Additionally, we obtained TIMI, PAMI, CADILLAC, and GRACE scores. Results A total of 2839 patients (79.3% male, mean age 55.6 ± 11.2 years) were included. Over a median follow-up of 244 days, the composite MACE rate was 18.3% (519). All-cause mortality was 13.5% (384), re-infarction requiring revascularization was 4.3% (121), heart failure-related re-hospitalization was 2.6% (74), stent thrombosis occurred in 5.6% (160), and CVA (cerebrovascular accident) events were documented in 1% (28). The AUC (area under the curve) for TIMI, PAMI, CADILLAC, and GRACE scores were 0.658 [0.630-0.685], 0.658 [0.632-0.684], 0.669 [0.644-0.695], and 0.675 [0.649-0.701], respectively, for the prediction of MACE. On multivariable Cox regression, medium and high-risk categories based on GRACE score were independent predictors of MACE with adjusted HR (hazard ratio) of 1.52 [1.21-1.92]; p<0.001 and 1.92 [1.34-2.75]; p<0.001, respectively. Conclusion A significant proportion of patients experienced short-term MACE after primary PCI. While none of the assessed scores demonstrated significant predictive power, the GRACE score exhibited comparatively better predictive ability than TIMI, PAMI, and CADILLAC scores

Immediate and Short Term Outcomes for Using Drug Coated Balloons in Treating Coronary Bifurcation Lesions

Abstract Background lesions at the bifurcation of coronary arteries pose a substantial difficulty due to their intricate anatomical structure and the increased risk of complications during or after the procedures. Drug-coated balloons (DCBs) have recently been recognized as a promising therapeutic strategy for managing bifurcation lesions. Objective to compare the use of drug-coated balloons versus ordinary balloons in provisional stenting of coronary artery bifurcation lesions and its impact on the procedural success rate & short-term MACE Patients and Methods The current study is a prospective, longitudinal cohort, interventional two arm study that included a total of 66 patients. 40 in Control group (group A) and 26 in case group (group B). every patient is assigned to a particular group based on the operator’s preference. In both groups provisional stenting was employed, with dilation of the side branch using a standard balloon in group “A” and a drug-coated balloon in group “B”. The evaluation of the outcomes of the indexed procedure and a six-month follow-up were conducted to monitor the incidence of Major Adverse Cardiac Events (MACE). Results The mean age of patients assigned to the Group A was 57.18 ± 9.84 years, while patients assigned to the Group B had a mean age of 55.50 ± 7.74years, both groups were similar in demographic data, baseline clinical characteristics and Echocardiographic Data. The study’s primary endpoint was the occurrence of major adverse cardiac events (MACEs), at a 6-month follow-up. A significant difference was observed between the case and control groups in terms of MACE after 6 months. The incidence of MACE was 23.7% (9 out of 38 patients who continued the study after 2 cases of in- hospital mortality) in the control group, compared to only 3.9% (1 out of 26 patients) in the case group, with a p-value of 0.039. the secondary endpoints of the study, which encompassed the attainment of lesion success and procedure success, did not show a statistically significant difference between the case and control groups despite the observed variation in the complication rat Conclusion The application of drug-coated balloons in provisional stenting for coronary artery bifurcation lesions demonstrated superiority over conventional balloons in terms of short-term major adverse cardiac events (MACE), and exhibited non-inferiority in relation to lesion or procedural success.

CLINICAL AND ANGIOGRAPHIC OUTCOMES OF PERCUTANEOUS CORONARY INTERVENTION IN CHRONIC TOTAL OCCLUSION PATIENTS: A RETROSPECTIVE STUDY AT A TERTIARY CARE HOSPITAL

Chronic total occlusion (CTO) presents a unique challenge in percutaneous coronary intervention (PCI), requiring specialised techniques to achieve successful revascularisation. Despite the complexity, PCI in CTO cases has evolved significantly, showing promising outcomes. Objectives: To assess percutaneous coronary intervention's clinical and angiographic outcomes in chronic total occlusion patients. Methods: This retrospective observational study was conducted at NICVD Karachi, Pakistan, following approval from the hospital's ethical committee. It reviewed the medical records of 150 patients who underwent percutaneous coronary intervention (PCI) for chronic total occlusion (CTO) from January 2024 to July 2024. Data was collected retrospectively from electronic medical records, including demographic details (age, gender, cardiovascular risk factors, and comorbidities), baseline clinical characteristics (presentation, symptoms, angina status, and prior cardiac history), and angiographic data (pre- and post-procedural findings, including the Thrombolysis in Myocardial Infarction (TIMI) flow grade). Clinical outcomes were assessed for both short-term and long-term results, including major adverse cardiac events (MACE) such as myocardial infarction, repeat revascularisation, target lesion revascularisation, and all-cause mortality. Statistical analysis was performed using SPSS Version 25. Results: 150 patients were enrolled, with a mean age of 54.81±8.97 years. The cohort included 54.7% males (82 patients) and 45.3% females (68 patients). Before PCI, 87.3% of patients (131) had no blood flow in the occluded artery, while 12.7% (19) had minimal flow. Post-PCI, 91.3% (137 patients) achieved full blood flow (TIMI Flow Grade 3), and 8.7% (13 patients) had partial flow. The procedural success rate was 91.3%. In terms of short-term major adverse cardiac events (MACE), 116(77.3%) patients had no events, while 22.7% (34 patients) experienced a myocardial infarction (MI). For long-term MACE, 84.0% (126 patients) had no adverse events, 10.0% (15 patients) required target lesion revascularisation, 4.7% (7 patients) underwent repeat revascularisation, and 1.3% (2 patients) experienced mortality. Conclusion: This study confirms the efficacy and safety of PCI for CTO patients, demonstrating high procedural success rates, significant clinical improvements, and relatively low long-term adverse events. However, advancing procedural techniques and fostering specialised training are essential to enhance patient outcomes due to the risks and complexities involved. Further research, including large-scale randomised controlled trials, is needed to validate these results and refine patient selection and management strategies for CTO PCI.

Predictors of long-term survival in immune checkpoint inhibitor (ICI)-associated myocarditis.

e24026 Background: ICI-associated myocarditis carries a high morbidity and mortality, with close to half of patients experiencing major adverse cardiac events (MACE). Risk factors previously associated with short-term MACE include troponin T (TnT), neutrophil/lymphocyte ratio (NLR), and absolute lymphocyte count (ALC). In this study, we aimed to analyze the predictors of long-term survival. Methods: This single-center retrospective study included 37 patients diagnosed with ICI-associated myocarditis at Massachusetts General Hospital between March 2016 and March 2022. We collected demographics, diagnostic results, and myocarditis treatments from the electronic medical record. Patients were grouped by survival time: short-term ( < 30 day), intermediate (30 to 360 day), or long-term ( > 360 days) survival. We used chi-square and Kruskal-Wallis tests for descriptive statistics. Cut-off points of continuous variables were determined by ROC curve. Predictors of survival were analyzed by logistic regression models. Results: The cohort consisted of 29 males and 8 females with a median age of 75 years (IQR 68-79), and median follow-up of 5.6 months (IQR 0.5-14.9). The median time from ICI start to myocarditis onset was 50 days (IQR 36-91). Ten, 14, and 13 patients were in the short-term, intermediate, and long-term survival groups, respectively. By univariate analysis, predictors of long-term survival included initial NT-proBNP ≤ 450, NLR ≤ 4.44, ALC ≥ 1122, normal EKG QTc interval (≤ 450 ms in male or ≤ 460 ms in female), and TnT decrement ≥ 42% by day 8 of immunosuppression (Table 1). All patients with long-term survival presented with a normal left ventricular ejection fraction (LVEF ≥ 55%), which made it uninterpretable using logistic regression models. Normal LVEF was significantly associated with long-term survival by using chi-square test (p = 0.002). While TnT elevation > 75x the upper limit of normal (ULN) was strongly associated with short-term survival (p = 0.001), it was not associated with long-term survival (p = 0.23). The use of second-line immunosuppression in addition to corticosteroids did not have a significant impact on long-term survival (p = 0.69). Conclusions: NLR, ALC, NT-proBNP, QTc interval, and a substantial decrease in TnT at day 8 after immunosuppression are all associated with long-term survival in ICI myocarditis. [Table: see text]

Influence of Trichosanthes pericarpium extract on improving microcirculation and outcomes of patients with acute myocardial infarction after percutaneous coronary intervention.

Microcirculatory dysfunction is an independent risk factor for a poor prognosis after percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). Trichosanthes pericarpium is a well-known Chinese traditional herb described with the effect of activating blood circulation to dissipate blood stasis and improve blood circulation. However, its effects on microcirculation in patients with AMI after primary PCI remain unknown. This study aimed to investigate the effect of Trichosanthes pericarpium extract (TPE) on improving microcirculation and outcomes in patients with AMI after PCI. This study included 155 patients with a history of emergency PCI treatment. In this cohort, 31 patients received a course of TPE, defined as intravenous TPE infusion (12 ml TPE dissolved in 250 ml 5% Glucose Injection) once daily for 7 days on the background of standard treatment after PCI; 124 who did not receive TPE were regarded as the control group and received standard treatment after PCI. The corrected thrombolysis in myocardial infarction frame count (CTFC) and index of microcirculatory resistance (IMR) were used to evaluate myocardial microcirculation. Cardiac function was measured by echocardiography during hospitalization and follow-up. Major adverse cardiac events (MACEs) were recorded for prognostic analysis. At the 6-month follow-up, AMI patients who received TPE after primary PCI had significantly lower levels of CTCF (24.27 ± 2.40 vs. 21.88 ± 1.92, P < 0.001) and IMR (20.02 ± 2.20 vs. 17.80 ± 2.11, P < 0.001) than patients in the control group. Left ventricular ejection fraction and left ventricular internal dimension at systolic at 6-month follow-up in the TPE group significantly improved than in the control group (56.6 ± 4.5 vs. 62.1 ± 3.5, P < 0.001; 32.5 ± 1.5 vs. 30.2 ± 1.8, P < 0.001). Kaplan-Meier curve analysis indicated that patients with AMI who received TPE had significantly lower rates of MACEs than the control group at 6-month follow-up (P = 0.042). In the context of standard treatment, Trichosanthes pericarpium further improved coronary microcirculation, increased cardiac function, and reduced short-term MACEs rate. Our data suggest that TPE could be used in combination therapy for patients with AMI after primary PCI.

Introduction: Major adverse cardiac events (MACE) are the primary cause of morbidity and mortality in patients undergoing vascular surgical procedures. This is why it is essential to understand the relationship between coronary artery disease and peripheral arterial disease. In this paper, we aimed to determine predictors and risk factors related to the occurrence of MACE in patients undergoing vascular surgery for peripheral arterial disease. Material and Methods: This is a retrospective study of 310 in-hospital patients treated at a high-volume vascular centre. The outcome was classified through the three-point MACE (myocardial infarction, stroke and cardiovascular mortality). Results: We found that the majority of initial MACE happened in patients with aortobifemoral bypass surgery compared to other types of reconstruction (p=0.009). No significant correlation was established between the procedures performed and the short-term primary outcome. In the initial period, MACE was detected in 14 (5.8%) patients. Among them, six (1.9%) experienced a myocardial infarction (MI), three (1%) had a stroke, and five (1.6%) died within the first 30 days. All deaths were linked to cardiac ischemia, which ultimately culminated in cardiogenic shock and subsequent death. Short term MACE was observed in 2 (0.6%) patients both of whom had an MI. Conclusion: Major vascular surgery in patients with coronary artery disease is a highly morbid procedure and perioperative MACE places them at a significantly high risk of mortality. Early detection of coronary heart disease and preoperative optimization can play a major role in reducing the risk of MACE.

Predictors of Adverse Outcome in High-Risk Percutaneous Coronary Interventions Patients.

Identifying high-risk percutaneous coronary intervention (PCI) patients is challenging. We aimed to evaluate which high-risk patients are prone to adverse events. We performed a retrospective study including consecutive high-risk PCIs from 2005 to 2018 in a large tertiary medical centre. Patients with unprotected left main (LM) disease, last patent coronary vessel, or 3-vessel coronary artery disease with left ventricular ejection fraction < 35% were included. A predictive 30-day major adverse cardiac events (MACE) score consisting of any myocardial infarction, all-cause death, or target-vessel revascularisation was constructed. From 2005 to 2018, a total of 1890 patients who underwent PCI met the predefined high-risk PCI criteria. Mortality rate was 8.8% at 30 days and 20.7% at 1 year, and 30-day MACE rate was 14.2% and 33.5% at 1 year. Predictors of short-term MACE were New York Heart Association functional class (NYHA) 4 (hazard ratio [HR] 6.65; P<0.001), systolic blood pressure (SBP) < 90 mm Hg (HR 4.93; P<0.001), creatinine > 1.3 mg/dL (HR 3.57; P< 0.001), hemoglobin< 11.0 g/dL (HR 3.07; P < 0.001), pulmonary artery systolic pressure> 50 mm Hg (HR 2.06; P<0.001), atrial fibrillation (HR 1.74; P<0.001), and LM disease (HR 2.04; P<0.001) or last patent vessel (HR 1.70; P= 0.002). A score constructed from these parameters reached a sensitivity of 90% and a specificity of 81% with areas under the receiver operating characteristic curve of 0.92 for MACE and 0.94 with 89% sensitivity and 87% specificity for all-cause mortality. Specific features such as LM lesion or last patent conduit, pulmonary hypertension, atrial fibrillation, anemia, and renal failure, along with low SBP and NYHA 4, aid risk stratification and consideration of further treatment measures.

Early emergency department discharge for intermediate heart score patients presenting for chest pain.

The use of the HEART score to risk stratify patients for short-term major adverse cardiac events in the emergency department (ED) setting is well established. Although discharge to home for low-risk HEART score patients is widely accepted as safe practice, there are limited outcomes data on moderate-risk HEART score patients discharged to home. We investigated the safety of discharging moderate-risk HEART score patients to home from the ED with established early cardiology follow-up. We performed a retrospective cohort analysis of patients presenting to the ED with chest pain from April 2020 through December 2020. Patients were evaluated in the ED and underwent serial conventional troponin testing and electrocardiogram (ECG). Clinicians calculated a HEART score and employed shared decision-making with moderate-risk patients (score 4-6), offering hospital admission versus discharge home with a formalized process for rapid cardiology follow-up (within 2 business days). We assessed the frequency of acute myocardial infarction or death at 30 days and before cardiology follow-up. During our study period, 2939 patient encounters were screened for chest pain. Of these, 333 of 547 eligible moderate-risk HEART score patients were referred for rapid follow-up. The median time to follow-up appointment was 2.9 business days (interquartile range 1.3, 6.5), and 264 (79%) of patients kept their follow-up appointment. One patient (0.3%) suffered death within 30 days, before cardiology follow-up. There were no myocardial infarctions. These results suggest that moderate-risk HEART score patients may be considered for discharge from the ED with rapid cardiology follow-up. Formalizing processes to facilitate these early evaluations may represent a viable alternative to hospital admission, without diminishing patient outcomes.

Assessment of C-Reactive Protein/Serum Albumin Ratio in Relation to Acute Presentation and Early Outcome of Patients with Acute Coronary Syndrome

Background: Acute coronary syndrome (ACS) is the leading cardiovascular (CV) cause of mortality. C reactive protein (CRP) has linked with long-term risk of recurrent cardiovascular events or death. Albumin, in contrast to CRP known as a negative acute-phase protein. Thus a newly introduced marker assessed relation of CRP to albumin ratio (CAR), which may provide better results than the use of either marker alone. The aim of the study is to assess the association of C-reactive protein to albumin ratio (CAR) with in-hospital short-term major adverse cardiac events (MACEs) in acute coronary syndrome (ACS) patients. Patients & Methods: A multi-centers prospective cohort study was conducted at coronary intensive care units (CICU) in Baghdad during the period from March to October 2021 that included a total of 132 patients who were diagnosed as a case of ACS. They were assessed for major adverse cardiac events (MACEs) like cardiogenic shock, arrhythmias, post-MI angina, and acute heart failure while inside the ward, in addition to need for early interventional therapeutic approach in relation to (CAR) immediately at time of admission to hospital. Results: High values of CAR, whether using hs-CRP or CRP, were identified as an independent predictor for in-hospital MACEs (P value < 0.001 and 0.002 respectively. A cut-off value of CAR (using hs-CRP) is 3.18 mg/L in context of discrimination between medically treated ACS patients and death outcome in term of high CAR. A cut-off value of CAR (using CRP) as 9.13 mg/L suggests the usefulness in discrimination of outcome in relation to medically managed patients, at presentation. CAR had a positive significant correlation with hospital stay (r = 0.210, P = 0.036). Conclusion: The CAR was independently correlated with in-hospital short-term MACEs and can be used for risk stratification in patients with ACS.

Interpretable Predictive Modeling for Major Adverse Cardiac Events Following Lung Cancer Radiotherapy

<h3>Purpose/Objective(s)</h3> Patients with locally advanced non-small cell lung cancer (NSCLC) are at increased risk of developing major adverse cardiac events (MACE) following radiotherapy. Our group previously identified cardiac substructure dose constraints associated with increased risk of MACE. We employ a machine learning (ML) approach to further identify predictors of MACE and evaluate ML capacity for personalized MACE risk-stratification. <h3>Materials/Methods</h3> Retrospective analysis of 701 patients with locally advanced NSCLC treated with thoracic radiotherapy between 2003-2014. MACE included unstable angina, heart failure, myocardial infarction, coronary revascularization, and cardiac death. We used extreme gradient boosting for MACE prediction. Input features included 195 demographic and 240 radiation dose and/or anatomic. Cardiac substructures were manually delineated; heart and chamber volumes were indexed to body surface area (BSA). We split data 70/30 into training/testing, balanced by MACE. Hyperparameters were bootstrap-tuned with 50-round grid search. Area under the receiver operator characteristic curve (AUC) evaluated model performance. We trained our model on any MACE regardless of timeframe and tested performance at varying timepoints (90-day, 180-day, etc.). Shapley values measured feature importance and were used to construct personalized risk profiles for each patient. <h3>Results</h3> Among 701 patients, 70 developed ≥1 MACE. The median age was 65 and median time to first MACE was 20.6 months. Training AUC for any MACE: 0.67; testing AUC: 0.73. Our model displayed time dependent performance improvement, with high accuracy for MACE closer to radiation start. Testing AUC for 90-day MACE: 0.97; 180-day MACE: 0.94. Table 1 displays additional AUCs. The top 10 predictive features for MACE were coronary heart disease history, right atrial volume, left circumflex coronary artery (CA) volume (V) receiving 15 Gy (V15Gy), heart volume, lung V55Gy and V70Gy, hypertension, left ventricle V15Gy, left main CA volume, and left anterior descending CA V15Gy. Based on patients' Shapley-derived risk profile, radiation dose variables generally became less predictive of MACE as time from radiation increased, while demographics remained generally predictive. <h3>Conclusion</h3> ML modeling enabled high precision for predicting short-term MACE in locally advanced NSCLC patients who received radiotherapy, though long-term MACE was less accurate. ML techniques show promise for identifying patients at high risk of short-term MACE. Our Shapley-derived individual risk profiles may assist with baseline cardiac risk assessment and identify opportunities for risk mitigation. Prospective validation may help implement these tools into clinical practice.

The Burden of Short-term Major Adverse Cardiac Events and its Determinants after Emergency Percutaneous Coronary Revascularization: A Prospective Follow-up Study.

ObjectivesPrimary percutaneous coronary intervention (PCI) remains recommended reperfusion therapy for patients with acute ST-elevation myocardial infarction. This study aimed to evaluate the short-term major adverse cardiac events (MACE) and their determinants among patients who underwent primary PCI at a tertiary care cardiac center of Karachi, Pakistan.MethodsA cohort of patients who underwent primary PCI were followed for the MACE. Multivariable Cox-regression analysis was performed with backward conditional variable selection and hazard ratio (HR) along with 95% confidence interval (CI) were obtained.ResultsA total of 1150 patients were included, of which follow-up was successful in 95.8% (1102) and median follow-up duration was 6.1 [6.9–5.1] months. MACE were observed in 210 (19.1%) patients with 14.2% (157) all-cause mortality, 5.4% (60) cardiac mortality, 0.7% (8) stroke, 3.6% (40) re-hospitalization due to heart failure, and 6.1% (67) myocardial infarction requiring revascularization. Independent predictors of short-term MACE were found to be admission glucose ≥200 mg/dL (1.66 [1.25–2.21]), serum creatinine ≥1.5 mg/dL (1.52 [1.02–2.27]), intubation (2.81 [1.98–4.00]), history of PCI (2.06 [1.45–2.93]), history of cerebrovascular accident (2.64 [1.34–5.2]), left ventricular end-diastolic pressure ≥20 mmHg (1.81 [1.3–2.51]), triple vessel diseases (1.43 [1.08–1.9]), culprit left main or proximal left anterior descending artery (1.77 [1.32–2.35]), pre-ballooning (2.14 [1.2–3.82]), and thrombus grade ≥4 (2.21 [1.51–3.24]).ConclusionsA significant number of individuals undergone primary PCI are still vulnerable to subsequent short-term MACE, hence, systematic follow-up and early risk stratification should be considered as an integral part of STEMI management protocol specially for patients with high-risk features as highlighted herein.

Rotational Atherectomy in Acute Coronary Syndrome: A Meta-Analysis

BackgroundThe use of rotational atherectomy (RA) in percutaneous coronary intervention (PCI) of acute coronary syndrome (ACS) is considered relatively contraindicated. There have been several observational studies showing RA use in ACS, however, no systemic studies have been undertaken. We sought to evaluate the feasibility and outcomes of RA PCI in ACS by performing a meta-analysis. MethodsWe searched PUBMED, EMBASE, CINAHL, and Cochrane Central Register of Clinical Trials for any studies that evaluated the role of RA PCI in ACS. The outcomes analyzed were all-cause mortality, cardiac mortality, short and long-term major adverse cardiac events (MACE), procedural complications and cardiac perforations. ResultsThere was a total of 8 retrospective studies with a total population of 1237 with a median follow up of 23 months. The median age of the included patient was 73. Angiographic success rate was 97.4%. The rate of all-cause mortality and cardiac mortality were 5% (range 1–12%, p < 0.001, I2 = 92%) and 2% (range 0–5%, P = 0.03, I2 = 58%) respectively. In-hospital MACE and long-term MACE were 7% (range 3–13%, p < 0.001, I2 = 87%) and 29% (range 21–37%, p = 0.21, I2 = 34%) respectively. The incidence of total procedural complications was noted to be 7% (range 2–14%, p < 0.001, I2 = 90%). Rate of perforation was 1% (range 0–1%, p = 0.9, I2 = 0%). ConclusionOur results show that RA PCI is feasible in ACS with comparable procedural complications and short-term MACE, but with a higher long-term MACE rate compared to RA PCI in routine cases.

Validation and modification of HEART score components for patients with chest pain in the emergency department.

ObjectiveThis study aimed to clarify the relative prognostic value of each History, Electrocardiography, Age, Risk Factors, and Troponin (HEART) score component for major adverse cardiac events (MACE) within 3 months and validate the modified HEART (mHEART) score.MethodsThis study evaluated the HEART score components for patients with chest symptoms visiting the emergency department from November 19, 2018 to November 19, 2019. All components were evaluated using logistic regression analysis and the scores for HEART, mHEART, and Thrombolysis in Myocardial Infarction (TIMI) were determined using the receiver operating characteristics curve.ResultsThe patients were divided into a derivation (809 patients) and a validation group (298 patients). In multivariate analysis, age did not show statistical significance in the detection of MACE within 3 months and the mHEART score was calculated after omitting the age component. The areas under the receiver operating characteristics curves for HEART, mHEART and TIMI scores in the prediction of MACE within 3 months were 0.88, 0.91, and 0.83, respectively, in the derivation group; and 0.88, 0.91, and 0.81, respectively, in the validation group. When the cutoff value for each scoring system was determined for the maintenance of a negative predictive value for a MACE rate >99%, the mHEART score showed the highest sensitivity, specificity, positive predictive value, and negative predictive value (97.4%, 54.2%, 23.7%, and 99.3%, respectively).ConclusionOur study showed that the mHEART score better detects short-term MACE in high-risk patients and ensures the safe disposition of low-risk patients than the HEART and TIMI scores.

The association between 24-h blood pressure variability and major adverse cardiac events (MACE) in hospitalized patients with acute myocardial infarction: a retrospective cohort study

BackgroundAcute myocardial infarction (AMI) is major cardiovascular disease that causes high morbidity and mortality. In AMI, ischemia and necrosis affected some cardiomyocytes leading to a decrease in myocardial contractility which is followed by an acute proinflammation reaction and increased sympathetic tone. Meanwhile, high blood pressure variability (BPV) causing an increased left ventricular workload, heart rate, myocardial oxygen demand and induces proinflamations and endothelial dysfunction. Therefore a high BPV and its associated pathological effects are likely to aggravate the physiological function of the heart and affect the emergence of acute cardiac complications in AMI patients. This study aims to investigate the association’s between short-term BPV and major adverse cardiac events (MACE) in AMI patients. This retrospective cohort study used simple random sampling to identify AMI patients who were hospitalized at Cipto Mangunkusumo National Hospital between January 2018 and December 2019. Mann Withney was performed to investigate the association between BPV and MACE.ResultsThe average systolic BPV value which was calculated as standard deviation (SD) and average real variability (ARV) was higher in the MACE group than in the non-MACE group. Systolic SD and systolic ARV in the MACE group were 13.28 ± 5.41 mmHg and 9.88 ± 3.81 mmHg respectively. In the non-MACE group, systolic SD and systolic ARV were 10.76 (4.59–26.17) mmHg and 8.65 (3.22–19.35) mmHg respectively. There was no significant association between BPV and MACE. However, there were significant differences between systolic SD and systolic ARV in patients with hypertension who experienced MACE and patients without hypertension who experienced MACE.ConclusionsThe BPV of AMI patients who experience MACE was higher than that of non-MACE AMI patients. There was no significant association between BPV ​​and MACE during the acute phase of AMI.

Sex differences in pre-hospital analgesia and outcomes in patients presenting with acute coronary syndromes

Abstract Background Opioid analgesia remains the mainstay of pain management in acute coronary syndromes (ACS). Significant sex differences persist in ACS presentation, management and outcomes, but the impact of sex-differences on pre-hospital pain management of ACS with opioids is unknown. There is increasing awareness of the importance of pre-hospital factors in ACS, as well as emerging concerns with opioid use impairing the gastrointestinal absorption of oral P2Y12 inhibitors. Purpose This study examined if sex-differences in pre-hospital pain scores, opioid administration and clinical outcomes exist in ACS patients. Methods Patients presenting with ACS via ambulance (2014–2018) that underwent percutaneous coronary intervention (PCI) were prospectively collected via the Victorian Cardiac Outcomes Registry (VCOR), the Melbourne Interventional Group (MIG), and linked to the Ambulance Victoria database. The primary outcome was 30-day major adverse cardiac events (MACE). Secondary outcomes were descriptive analyses of pre-hospital pain score, intravenous morphine equivalent analgesic dosing, plus predictors of MACE and Thrombolysis In Myocardial Infarction (TIMI) 0–1 flow pre-PCI using logistic regression. Results 10,547 patients were included (female: 2,775 [26.3%]). Opioids were administered to 1,585 (57%) females and 5,068 (65%) males (p&amp;lt;0.001). Adjusted 30-day MACE was similar between opioid groups in both sexes (female: OR 1.21, CI 0.82–1.79, p=0.34; male: OR 0.89, 0.68–1.16, p=0.40). Median pain score at presentation was 6 (IQR 4,8) for both sexes. Median opioid dose was 2.5 mg (IQR 0,10) in females and 5 mg (IQR 0,10) in males (p&amp;lt;0.001), with similar pain relief achieved. Adjusted rates of TIMI 0–1 pre-PCI were higher in patients administered opioids (female: OR 2.83, CI 2.14–3.56, p&amp;lt;0.001; male: OR 2.95, CI 2.49–3.49, p&amp;lt;0.001). Conclusions Female patients undergoing PCI received less opioid analgesia, but no sex-differences in pre-hospital pain scores were seen. Opioid administration was associated with impaired antegrade flow in the culprit artery in both sexes, but not short-term MACE. Randomised trials evaluating the clinical implications of opioid administration in ACS with sex subgroup analyses are needed to guide clinical practice. Telemedicine Improves the Short-Term Medical Care of Acute ST-Segment Elevation Myocardial Infarction After Primary Percutaneous Coronary Intervention.

Objectives: Telemedicine appears to be a promising tool for healthcare professionals to deliver remote care to patients with cardiovascular diseases especially during the COVID-19 pandemic. We aimed in this study to evaluate the value of telemedicine added to the short-term medical care of acute ST-segment elevation myocardial infarction (STEMI) after primary percutaneous coronary intervention (PCI).Methods: Two hundred acute STEMI patients after primary PCI were randomly divided into two groups. One hundred patients in group A (study group) received a monthly videoconferencing teleconsultation using a smartphone application for 3 months starting 1 week after discharge and at least a single face-to-face (F2F) clinic visit. We reviewed in each virtual visit the symptoms of patients, adherence to healthy lifestyle measures, medications, smoking cessation, and cardiac rehabilitation. Group B (control group) included 100 patients who received at least a single F2F clinic visit in the first 3 months after discharge. Both groups were interviewed after 4 months from discharge for major adverse cardiac events (MACE), adherence to medications, smoking cessation, and cardiac rehabilitation. A survey was done to measure the satisfaction of patients with telemedicine.Results: There was no significant difference between both groups in MACE and their adherence to aspirin, P2Y12 inhibitor, and beta-blockers. However, group A patients had better adherence to statins, angiotensin-converting enzyme inhibitors or angiotensin receptor blockers, smoking cessation, and cardiac rehabilitation. Sixty-one percent of patients stated that these videoconferencing teleconsultations were as good as the clinic visits, while 87% of patients were satisfied with telemedicine.Conclusions: Telemedicine may provide additional benefit to the short-term regular care after primary PCI to STEMI patients through videoconferencing teleconsultations by increasing their adherence to medications and healthy lifestyle measures without a significant difference in the short-term MACE. These virtual visits gained a high level of satisfaction among the patients.

Lack of prognostic significance for major adverse cardiac events of soluble suppression of tumorigenicity 2 levels in patients with ST-segment elevation myocardial infarction.

Elevation of soluble suppression of tumorigenicity 2 (sST2) is associated with cardiac fibrosis and hypertrophy. Under investigation herein, was whether sST2 level is associated with major adverse cardiac events (MACE) and left ventricular (LV) remodeling after primary percutaneous coronary intervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI). In total, this study included 184 patients who underwent successful primary PCI. A subsequent guideline-based medical follow-up was included (61.4 ± 11.8 years old, 85% male, 21% with Killip class ≥ I). sST2 concentration correlations with echocardiographic, angiographic, laboratory parameters, and clinical outcomes in STEMI patients were evaluated. The median sST2 level was 60.3 ng/mL; 6 (3.2%) deaths occurred within 1 year. The sST2 level correlated with LV ejection fraction (LVEF) changes from baseline to 6 months (r= -0.273; p = 0.006) after adjustment for echocardiographic parameters including wall motions score index (WMSI). Recovery of LVEF at 6 months was highest in the tertile 1 group (Δ6 months - baseline LVEF; tertile 1, p = 0.001; tertile 2, p = 0.319; tertile 3, p = 0.205). The decrease in WMSI at 6 months was greater in the tertiles 1 and 2 groups than in the tertile 3 group (Δ6 months - baseline WMSI; tertile 1, p = 0.001; tertile 2, p = 0.013; tertile 3, p = 0.055). There was no association between sST2 levels and short-term (log rank p = 0.598) and long-term (p = 0.596) MACE. sST2 concentration have predictive value for LV remodeling on echocardiography in patients with STEMI who underwent primary PCI. However, sST2 concentration was not associated with short-term and long-term MACE.

Usefulness of WBC Count to Mean Platelet Volume Ratio in Predicting Short-Term (30 Days) Major Adverse Cardiac Events in Patients Presenting with Acute Coronary Syndrome and its Comparison in Males and Females

Abstract Background: Among cardiovascular illnesses, acute coronary syndrome (ACS) events are associated with higher mortality and morbidity. The mechanism of inflammation involved in the ACS includes atherosclerotic plaque development, followed by development of plaque rupture and thrombosis. The present study is to determine whether white blood cell count to mean platelet volume ratio (WMR) can predict short-term (30 days) major adverse cardiac events (MACE) in ACS patients, and also compare WMR and MACE in males and females. Aim: To detect usefulness of white blood cell (WBC) count to WMR in predicting short term (30 days) MACE in patients presenting with ACS and compare values and MACE events in males and females. Material and Methods: The present study was conducted in a tertiary-care hospital from September 2020 to December 2020. A total of 60 patients, who presented with ACS and were undergoing percutaneous coronary intervention (PCI), fulfilled the selection criterion; hence, a total of 60 patients were selected for the study after taking informed consent. The clearance from the Institutional Ethical committee (IEC) was obtained prior to the initiation of the study. All the necessary investigations were done in patients who fulfilled the inclusion criteria. The results of the study were systematically selected and analysis done statistically. The study included all patients with clinical suspicion of ACS for more than 18 years and excluded non-ACS patients, patients with malignancy history, inflammatory diseases, autoimmune disorders, infections, and those who are immunosuppressed. Results: In the present study, receiver operating characteristic (ROC) curve showed cutoff value of WMR as 1059 with area under curve (AUC) of 0.6 (95% CI 0.4–0.9). MACE occurred in 12 patients and mortality occurred in 6 patients. Among MACE, recurrent myocardial infarction (REMI) occurred in 7 (35%) cases, arrhythmias occurred in 5 (25%), and cardiogenic shock occurred in 8 (40%) cases. WMR with a cutoff value of 1059 was significant and highly accurate in predicting MACE, with sensitivity of 91.6%, specificity of 87.5%, PPV of 64.7%, NPV of 97.6%, and diagnostic accuracy of 88.3%. Compared to males, females have less sensitivity to, high specificity to, and high diagnostic accuracy of WMR ratio in predicting MACE. MACE did not show any gender predilection. Conclusion: In patients, who presented with ACS, high WMR values were associated with worse short-term outcomes and independently predicted short-term MACE. Compared to males, females have less sensitivity to, high specificity to, and high diagnostic accuracy of WMR ratio in predicting MACE.

Predictive Value of C-Reactive Protein to Albumin Ratio in ST-Segment Elevation Myocardial Infarction Patients Treated With Primary Percutaneous Coronary Intervention.

The present study aimed to examine the association of C-reactive protein to albumin ratio (CAR) with short-term major adverse cardiac events (MACEs) in ST-segment elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary intervention (pPCI). We included 539 STEMI patient treated with pPCI in this study. Patients were divided into 2 groups according to MACE development. Patients with MACE had higher CAR than those without (1.18 [0.29-1.99] vs 0.21 [0.09-0.49], P < .001). Multivariate analysis showed that The Global Record for Acute Coronary Events score, Percutaneous Coronary Intervention with TAXus and cardiac surgery (SYNTAX) score, glucose and CAR (odds ratio:1.326, 95% CI: 1.212-1452, P < .001) were independent predictors of MACE. The CAR may be proven useful for risk stratification in STEMI patients undergoing pPCI.
