Transtympanic sodium thiosulphate to prevent cisplatin-related hearing loss: a protocol for randomised controlled multicentre phase III trial, the SOUND trial.
  • Dec 3, 2025
  • BMJ open
  • Anouk V M Burger + 14 more

Cisplatin is a widely used chemotherapeutic anti-cancer drug. However, high-dose cisplatin is also known for its dose-limiting toxicities, including irreversible cisplatin-induced hearing loss (CIHL). Sodium thiosulphate (STS) can bind to cisplatin to form an inactive and harmless complex. A topical application is desired, allowing cisplatin to retain its systemic anti-cancer effect. The SOUND trial is an investigator-initiated randomised controlled multicentre phase III trial to study the efficacy of transtympanic administration of STS against CIHL in a cohort of 100 patients with head and neck cancer treated with cisplatin at a dose of ≥200 mg/m2. Each subject will receive transtympanic STS injections in one ear, chosen by randomisation, before each cisplatin infusion. The contralateral ear serves as an internal control. The primary objective is efficacy (ie, clinically relevant benefit) of transtympanic STS injections against CIHL, defined as a difference in threshold shift of ≥10 decibels between baseline and 3 months after treatment in favour of the STS-treated ear. Secondary objectives include the difference in mean threshold shifts on frequencies essential for speech and extended high frequencies, as well as the difference between both ears in the gradation of hearing loss as defined by ototoxicity grading scales. The medical ethics committee in the Netherlands approved the trial (Clinical Trials Information System (CTIS) 2023-503313-30-00). The results will be disseminated through the CTIS and peer-reviewed scientific journals. CTIS 2023-503313-30-00 approved by Medical Research Ethics Committee NedMec.

Padua Days on Muscle and Mobility Medicine, March 3-6, 2026: Call for oral presentations
  • Dec 2, 2025
  • European Journal of Translational Myology
  • Ugo Carraro

The Padova Muscle Days (PMD), an international congress on skeletal muscle, was founded in 1985 to provide consultancy on Translational Myology. Its goal has always been to translate the results of basic research into clinical trials. Indeed, function and mass of skeletal striated muscle are influenced by and affect central and peripheral neural pathologies, cardiac, pulmonary, hepatic, metabolic, and endocrine disorders, as well as lifestyle, aging, not to mention cancer. Over the years, PMD has led to innovative multidisciplinary results, so much so that it became natural to emphasize this with a neologism now included in the title of the congress series: "Padua Days on Muscle and Mobility Medicine (Pd3m)". Mobility Medicine is used to bring together knowledge currently dispersed across too many subspecialties. Medium-sized scientific conferences, held in hotels large enough to accommodate all participants, increase opportunities for constructive discussions during breaks and evenings, bringing together young and senior experts in basic sciences and clinical specialties. Extra-sessional discussions offer young scientists good opportunities for new collaborations and job offers. Since its inception, the PMD has offered these opportunities, leading to innovative multidisciplinary results published in high-level scientific journals. We hope that many more speakers will share our dreams and make them come true during the next conference in March 2026.

Influencing factors of psychological insulin resistance among patients with type 2 diabetes: a systematic review and meta-analysis.
  • Dec 1, 2025
  • Journal of diabetes and metabolic disorders
  • Hongzhan Jiang + 6 more

Diabetes, particularly type 2 diabetes, is a significant global health issue, with insulin therapy being crucial for glycemic control. Psychological insulin resistance (PIR) often hinders effective treatment, impacting treatment outcomes and patient quality of life. This systematic review and meta-analysis aimed to comprehensively analyze the influencing factors of PIR among patients with type 2 diabetes, providing insights for developing targeted interventions to enhance insulin therapy acceptance and improve diabetes management. Following PRISMA guidelines, we conducted a systematic search across multiple databases, including PubMed, the Cochrane library, CINAHL, Embase, APA PsycInfo, Web of Science, China National Knowledge Infrastructure (CNKI), Wanfang Database, China Science and Technology Journal Database (VIP), and Sinomed, from inception to July 2025. Studies were included if they focused on type 2 diabetes patients and PIR, with exclusion of non-original research and republished literature. Quality assessment was performed using Newcastle-Ottawa Scale for cohort and case-control studies, and AHRQ standards for cross-sectional studies. A total of 22 studies involving 5,965 patients were included. The prevalence of PIR ranged from 27.1 to 82.9%. Meta-analysis indicated that female gender (OR = 0.38, 95% CI: 0.20-0.72, P = 0.003), lack of diabetes education (OR = 1.85, 95% CI: 1.37-2.49, P < 0.0001), and negative attitudes toward insulin (OR = 1.36, 95% CI: 1.17-1.57, P < 0.0001) were significant predictors of PIR. Factors influencing PIR were categorized into five themes: patient characteristics, treatment history, patient attitudes, lifestyle and economic factors, and social-psychological factors. Understanding the multifaceted nature of PIR is essential for improving glycemic control and patient outcomes. This review provides valuable insights for healthcare providers and researchers, guiding the development of tailored interventions to address PIR and enhance diabetes management. Following the publication of this paper, it was drawn to the Editor's attention by a concerned reader that, in addition to the duplication of data comparing Fig. 4B with Fig. 6F (showing the results of Transwell assay experiments) and within Fig. 6C (showing the results of scratch‑wound assay experiments), a data panel in Fig. 2C was also featured in a paper written by different authors that had already been submitted to the journal Cancer Science. Owing to the fact that the contentious data in the above article were found to be strikingly similar to data that had already been submitted for publication elsewhere, the Editor of International Journal of Molecular Medicine has decided that this paper should be retracted from the Journal. The authors were asked for an explanation to account for these concerns, but the Editorial Office did not receive a satisfactory reply. The Editor apologizes to the readership for any inconvenience caused. [International Journal of Molecular Medicine 42: 2437‑2446, 2018; DOI: 10.3892/ijmm.2018.3862].

  • Jurnal Multidisiplin Madani
  • Maysarah Nasution + 1 more

This study aims to provide an in-depth explanation of the definition, functions, and objectives of the discipline of legal history. As a branch of knowledge that bridges the past with the present, legal history is often overlooked, even though it plays a crucial role in the formation, interpretation, and reform of law. This study uses normative legal research methods with a library research approach. Data were collected through the analysis of documents, textbooks, scientific journals, and other relevant literature. The results of the study indicate that (1) the understanding of legal history is not limited to the chronology of legal events, but is a critical study of the evolution of norms, institutions, and legal thought in its social, political, and economic context. (2) The functions of legal history include the educational function (providing understanding), the pragmatic function (assisting in interpretation and formation of law), and the critical function (revealing the ideology and power behind the law). The main goal of studying legal history is to understand the identity and character of a legal system, avoid past mistakes, and provide a strong foundation for future law reform. This study concludes that legal history is an essential analytical tool for academics, practitioners, and policymakers to understand the law in a comprehensive, dynamic, and contextual manner

Lemahnya Manajemen Lembaga PAUD: Antara Orientasi Operasional dan Pengabaian Mutu Pendidikan serta Solusi Penanggulangan
  • Nov 27, 2025
  • Jurnal Pendidikan Dirgantara
  • Dira Meri Andika

Early Childhood Education (PAUD) plays a fundamental role in shaping children's character, moral values, and intellectual potential from an early age. However, many PAUD institutions in Indonesia still face serious challenges in management, where attention is focused more on operational sustainability than on improving educational quality. This study aims to describe the actual condition of PAUD institutions, analyze the causes of weak management, and identify the impact of operational orientation on educational quality. The method used is a literature study through analysis of various sources such as books, scientific journals, research reports, and government policies related to PAUD implementation. The results of the study indicate that weak management is caused by limited human resources, low managerial competence, minimal funding, weak planning, and a lack of supervision and community support. The impact is seen in the decline in educational quality and low public trust in PAUD institutions. Therefore, it is necessary to strengthen quality-based management that emphasizes improving educator competence, community involvement, effective management of infrastructure, utilization of educational technology, and implementation of continuous evaluation. Professional and adaptive PAUD management is key to realizing superior, high-quality institutions that are relevant to the developmental needs of early childhood in the modern era.

THE ROLE OF ALPHA-FETOPROTEIN IN SUPPRESSING ANTITUMOR IMMUNE RESPONSE IN HEPATOCELLULAR CARCINOMA: A LITERATURE REVIEW
  • Nov 26, 2025
  • Oncologia i radiologia Kazakhstana
  • N Nurgaliyeva + 4 more

Relevance: Hepatocellular carcinoma (HCC) is one of the most aggressive and deadly malignant tumors, characterized by treatment complexity and high resistance to immunotherapy. Alpha-fetoprotein (AFP) is traditionally used as a diagnostic marker for HCC. Recent studies reveal its key role in carcinogenesis and in forming a tolerogenic microenvironment that suppresses the anti-tumor immune response. Despite numerous publications, contradictions remain in assessing AFP’s functions and prognostic value, necessitating an analytical review. This study aimed to summarize current data on AFP’s role in tumor immune evasion mechanisms and to evaluate its potential as a target for novel immunotherapeutic approaches for HCC treatment. Methods: A systematic search and analysis of publications on AFP and HCC were conducted in PubMed and Google Scholar, and several specialized medical and scientific journals (including Cancer Research) from 2017 to 2025. Keywords included: “alpha-fetoprotein”, “hepatocellular carcinoma”, “immunosuppression”, “tumor immune evasion”, “AFP immunomodulation”, “immune response in HCC”. Selected articles met relevance and novelty criteria. Results: The review revealed that AFP plays a central role in HCC progression through its immunosuppressive effects and activation of tumor immune evasion mechanisms. Contradictions in prognostic data reflect the complexity and multilayered biological functions of AFP. Conclusion: AFP is not only a key diagnostic marker for HCC but also plays an active role in establishing immune tolerance. Its ability to suppress antitumor immune response makes AFP a promising therapeutic target, especially in combined immunotherapeutic approaches aiming to improve treatment efficacy.

  • Nov 25, 2025
  • Journal of advanced nursing
  • Tingting Li + 2 more

To explore the competency of nursing graduates from the four dimensions of clinical practice, research ability, teaching ability and management ability, analyse its influencing factors and provide data support for improving the post ability of nurses with a master's degree. In September 2024, a convenience sampling method was used to administer a self-designed questionnaire regarding post-graduation post-competency to 330 nursing master's degree graduates from 68 tertiary hospitals and five medical universities across China. The average scores of clinical competence, research competence, teaching competence and management competence of nursing graduates were more than 7 (out of 10 points). Based on the Benner model, all the abilities of the participants were at the level of competent. Multivariate linear regression analysis indicated that marital status (p < 0.001), years since graduation (p < 0.001), major (p < 0.001) and etal., significantly influenced clinical competency. Furthermore, marital status, major, hospital rank and graduate type were key research competency factors. For teaching competency, major, training nature and professional title played a crucial role, whereas major, professional title, marital status and hospital rank were essential for management competency. Multivariate logistic regression analysis indicated that job position (χ2 = 11.375, p = 0.01) significantly influenced SCI publication, whereas the training nature and type of graduate school were independent factors affecting publication in Chinese core journals. Moreover, years since graduation and professional title were independent factors that influenced the publication of scientific core journals. The post-competency scores of nursing master's degree graduates in the four dimensions of clinical, research, teaching and management were moderate, indicating substantial potential for enhancement. Managers should develop personalised training programs based on different factors that influence the overall competency of nursing master's degree graduates, thereby improving nursing quality and ensuring patient safety.

  • Nov 25, 2025
  • European Journal of Psychology of Education
  • Majid Asadpour + 3 more

Abstract While various agents influence students' motivation to learn—such as teachers, parents, peers, and the learning environment—numerous studies indicate that teacher-related variables are among the most influential. This study aims to systematically review all qualified research on the motivational role of teachers and identify the full spectrum of teacher-related dimensions and factors that affect students' academic motivation. The systematic review followed the PRISMA guidelines and was conducted using data from 5 databases: Scopus, PubMed, Web of Science, PsycINFO, and Google Scholar. Original studies published in reputable scientific journals were included if they quantitatively examined the role of teachers in influencing school students’ academic motivation, from 1974 to the end of November 2024. After applying eligibility criteria, 38 peer-reviewed studies were selected from 1,591 records for data extraction. The studies included a total of 54,407 students (52.4% female, mean age: 12.08 years), 769 teachers, and 394 parents. Most studies reported that teacher interventions enhanced students' academic motivation. This study highlights the crucial role of teachers in shaping students' academic motivation, identifying 5 key dimensions and 14 factors that provide a comprehensive framework for understanding teachers’ motivational influence: Support (autonomy, relatedness, social); Relationship with Students (proximity, influence, care); Teaching Practices and Styles (feedback, structure, instruction, involvement, control); Motivation (educational interest, intrinsic motivation, self-efficacy); and Characteristics (students’ perceptions of their teachers, teacher gender, teacher boredom). Additionally, the review highlights findings on differences between gifted and typical students, gender interactions between teachers and students, age-related effects, and longitudinal influences.
