Prepackaged foods healthiness ranking index: developing front-of-pack labeling to facilitate the assessment and validation of healthiness rankings of prepackaged foods in China

BackgroundThe Medium and Long-Term Plan for the Prevention and Treatment of Chronic Diseases (2017–2025) in China has highlighted the importance of reducing fat, sodium, and sugar in foods. However, front-of-pack labeling, which enables consumers to assess the health levels of prepackaged foods, is lacking in China. In response to the Zhejiang Provincial Health Commission’s request, we sought to develop a method for efficiently evaluating the health level of prepackaged foods.MethodsThrough a comprehensive literature review, we established a systematic framework: the Prepackaged Foods Healthiness Ranking Index (PHRI). We determined specific threshold values and grading criteria and conducted two rounds of Delphi survey to refine the index. Using a measurement dataset, we aimed to determine the optimal limit values for assessing the health level of prepackaged foods in real-world settings.ResultsAfter two rounds of Delphi surveys, the PHRI underwent multiple revisions until consensus among experts was reached. The final decision regarding the upper limit values set 30% Nutrient Reference Values as the limit for solid foods and 15% Nutrient Reference Values for liquid foods. The calculation of PHRI values was successfully implemented with the dataset, in accord with current nutrition awareness and real-world conditions.ConclusionsThe PHRI provides a valuable tool for assessing the healthiness of prepackaged foods. This front-of-pack labeling system provides a convenient method for evaluating the nutritional quality of prepackaged foods. Ultimately, the PHRI has the potential to contribute to advancements in health policy, practice, and education in China.

Development of an emergency science popularization capacity index system for nurses in China-A Delphi study.

To construct a popularization of an emergency science capacity index system for nurses based on the iceberg model. A mixed-method research design incorporating quantitative and qualitative components was used. In this study, the first draft of the questionnaire was developed through literature analysis and semi-structured interviews. According to the Delphi expert inclusion criteria, 20 experts with rich experience in the field of science popularization were selected, and the system of emergency science popularization ability of nurses was ultimately developed through two rounds of consultation according to the Delphi method. The weights of various indicators were determined through hierarchical analysis. Following two rounds of Delphi surveys, a total of 3 primary, 6 secondary, and 26 tertiary indicators were identified. The indicators were found to be highly significant, with variation coefficients ranging from 0.063 to 0.140. The Kendall's harmonization coefficients for the primary, secondary, and tertiary indicators were 0.350, 0.341, and 0.146, respectively. The popularization of the emergency science capacity system has a certain scientific and practical significance, which is of guidance to the competency of nurses in public health emergencies. No patient or public contribution. This study did not involve patients, service users, caregivers, or members of the public.

Developing an assessment tool for the healthy lifestyles of the occupational population in China: a modified Delphi-analytic hierarchy process study

Lifestyle is an important factor affecting people's health. Evaluating and promoting healthy lifestyles among occupational population can not only improve work ability and efficiency, but also contribute to the prevention a variety of potential diseases. This study developed a preliminary index system based on literature review and group discussion. Two rounds of expert consultation were conducted on 12 experts using Delphi method. The questionnaire recovery rate of the two rounds of Delphi survey was over 85%, the expert authority coefficient was over 0.70, and Kendall W was 0.450 and 0.446, which were significant (P < 0.001). The weighting coefficient of each indicator was calculated using the hierarchical analysis method. Among them, the weighting coefficients of physical health lifestyle, mental health lifestyle, social adaptation lifestyle and occupational health lifestyle were 0.4133, 0.2922, 0.1078 and 0.1867, respectively. The consistency index CI = 0.024 and the consistency ratio CR = 0.027 of the first-level index judgment matrix. The weighting coefficient is acceptable and the indicators do not cause logical confusion. The healthy lifestyle assessment tool of the occupational population constructed in this study consists of 4 primary indicators, 13 secondary indicators and 45 tertiary indicators, which can provide a standardized and operable assessment tool for monitoring and evaluating the healthy lifestyle of the Chinese occupational population. At the same time, the weight analysis of various indicators through the analytic hierarchy process can also provide reference for the key areas of occupational health intervention.

Objectives The purpose of this study was to derive appropriate intervention strategies and considerations for difficult situations in which domestic school counselors find it challenging to cope during nonsuicidal self-injury counseling processes. Methods To achieve this, we recruited 10 school counselors with over 5 years of experience as expert panelists and conducted three rounds of Delphi surveys. CVR(Content Validity Ratio) was caculated for each situation, and consensus was reached on appropriate intervention methods and considerations. Results As a result, a total of 34 appropriate interventions and 25 considerations were derived for six situations, including (1) situations involving parental indifference or refusal to cooperate, (2) ethical dilemmas related to confidentiality, (3) dealing with passive, non-voluntary clients, (4) situations where clients reject prescribed medication, (5) situations where cooperation from school members is difficult, and (6) situations where you are contacted late at night without any boundaries. Conclusions We hope that our findings can be utilized in the education of school counselors related to nonsuicidal self-injury counseling, contributing to the prevention of burnout and enhancing the effectiveness of self-injury counseling through the sharing of appropriate intervention methods.

Development and Initial Validation of the Novel Scleroderma Clinical Trials Consortium Activity Index.

Accurate measurement of disease activity in systemic sclerosis (SSc) remains a significant clinical challenge. The Scleroderma Clinical Trials Consortium (SCTC) convened an Activity Index (AI) Working Group (WG) to develop a novel measure of disease activity (SCTC-AI). Using consensus methodology, we developed a conceptual definition of disease activity. Literature review and expert consensus generated provisional SCTC-AI items, which were reduced by Delphi survey. Provisional items were weighted against a combined endpoint of morbidity and mortality, using time-dependent Cox proportional hazards regression analysis of the Australian Scleroderma Cohort Study (ASCS) (n = 1,254). External validation of the SCTC-AI was performed using data collected from 1,103 Canadian Scleroderma Research Group Study participants. Disease activity in SSc was defined using consensus methodology as "aspects of disease that are reversible, or can be arrested, with time and, or effective therapy." One-hundred and forty-one provisional SCTC-AI items were generated and reduced using three rounds of Delphi survey and statistical reduction and weighting, against mortality and quality of life measures, yielding a final 24-item index with a maximum possible score of 140. Survival analysis in an external cohort showed a graded relationship between disease activity scores and survival (P < 0.01). We present a novel instrument to quantify the burden of disease activity in SSc. We have employed a rigorous consensus-based process in combination with data-driven methods to develop an instrument that has face, content, and criterion validity. Further work is required to fully validate and confirm the construct and discriminative validity of the SCTC-AI.

Development of the Technical Assistance Engagement Scale: a modified Delphi study

BackgroundTechnical assistance (TA) is a tailored approach to capacity building that is commonly used to support implementation of evidence-based interventions. Despite its widespread applications, measurement tools for assessing critical components of TA are scant. In particular, the field lacks an expert-informed measure for examining relationship quality between TA providers and recipients. TA relationships are central to TA and significantly associated with program implementation outcomes. The current study seeks to address the gap in TA measurement tools by providing a scale for assessing TA relationships.MethodsWe utilized a modified Delphi approach involving two rounds of Delphi surveys and a panel discussion with TA experts to garner feedback and consensus on the domains and items that compose the TA Engagement Scale.ResultsTA experts represented various U.S. organizations and TA roles (e.g., provider, recipient, researcher) with 25 respondents in the first survey and 26 respondents in the second survey. The modified Delphi process resulted in a scale composed of six domains and 22 items relevant and important to TA relationships between providers and recipients.ConclusionThe TA Engagement Scale is a formative evaluation tool intended to offer TA providers the ability to identify strengths and areas for growth in the provider-recipient relationship and to communicate about ongoing needs. As a standard measurement tool, it lends a step toward more systematic collection of TA data, the ability to generate a more coherent body of TA evidence, and enables comparisons of TA relationships across settings.

Developing a Research Agenda with Victim-Survivors to Improve Health Sector Responses to Domestic, Family and Sexual Violence in Australia

Abstract Purpose The World Health Organisation acknowledges the health sector as playing a key role in responding to those who experience and use domestic, family and sexual violence. Limited resources need to be well spent to ensure the best outcomes for victim-survivors and their families. This study was undertaken to explore: What do victim-survivors, researchers and practitioners see as the key research priorities for improving health sector responses to domestic, family and sexual violence in Australia? Methods A Delphi methodology was adopted, with input from those with lived experience. Victim-survivors, researchers, practitioners and policy makers (n = 35) were engaged in a workshop and two rounds of Delphi surveys (n = 72 and 69). Results Participants highly rated four themes of research priorities 1) embedding the voices victim-survivors of domestic and family violence in health sector reform; 2) improving outcomes for children, young people and young families: 3) strengthening response to victim-survivors and perpetrators; and 4) better meeting the needs of victim-survivors from diverse backgrounds (including those with a disability, mental health issues, or from culturally diverse or Indigenous backgrounds). Conclusions This study identifies a solid research agenda for organisations and individuals interested in conducting or funding research to improve health sector responses to victim-survivors in Australia. It also demonstrates one way in which victim-survivors can be engaged in a meaningful way to inform future research about domestic, family and sexual violence.

Assessing the Suitability of Interruption Intervention Strategies in Nursing Medication Administration: A Delphi Study

Purpose: This study explored the suitability of interventions for medication interruption and intervention preferences.Methods: Two rounds of Delphi surveys were conducted with 18 expert panels comprising staff (or charge) nurses, nursing managers, and Quality Improvement (QI) team nurses working in a tertiary general hospital. For 47 situations involving the location of interruption, medication step, and source of interruption, the suitability of three interventions (no-interruption zone, medication safety vest, and education) was evaluated using a 5-point scale.Results: A total of 51 interventions for each situation were found appropriate by satisfying the degree of convergence and consensus. Patients or caregivers, peer nurses, doctors, telephones, and call bells were sources of interruption and were identified as appropriate for the application of interventions. 'Responding to requests and inquiries' by patients or caregivers showed high overall suitability. The nurses' preferred color for the intervention design (no-interruption zone, medication safety vest) is blue text on a yellow background. The priority groups for education related to medication interruptions were patients or caregivers, nurses, and non-nursing staff, in that order.Conclusion: Effective implementation of tailored intervention strategies that consider the specific characteristics of medication interruptions is crucial for mitigating interruptions and enhancing patient safety. Comprehensive educational programs aimed at reducing medication interruptions by improving awareness are necessary. Moreover, future research should evaluate these strategies in clinical settings to ensure their effectiveness in enhancing patient safety.

The purpose of this study was to develop a self-report test to measure the multiple intelligences of elementary school students in grades 4-6, and to verify the reliability and validity of the test. For the study, domestic and international prior research related to measuring multiple intelligences was first reviewed. To operationally define multiple intelligences and develop test items, a panel of 13 experts underwent two rounds of Delphi surveys, resulting in 155 preliminary test items. A preliminary test was administered online to 375 participants, and after item analysis, 122 items were selected for the main test. The main test was administered to 1,062 participants, and after verifying item quality and considering test practicality through research team meetings, 97 final items were confirmed. The final developed test confirmed construct validity through confirmatory factor analysis, and the reliability of the eight multiple intelligence domains ranged from .811 to .878, which is satisfactory. To verify concurrent validity, 87 of the main test participants took the Career Development Competency Test for Elementary Students, confirming significant correlations between competencies measuring similar domains and intelligences.

Purpose: In the process of formulating, implementing, and evaluating policies at the national level, efforts to develop and manage indicators for monitoring and evaluation have intensified. This study aimed to establish a national special education indicator system policy performance model to manage the implementation and assess the outcomes of national special education policy goals. Method: To derive the core special education policy objectives and detailed indicators, available indicators and pertinent prior research were collected and analyzed from domestic special education policies, as well as from domestic and international statistics and surveys in the special education and related fields (e.g., special education statistics, special education status surveys, special education longitudinal studies, etc.). Results: A special education indicator system policy performance model was developed. Internal validation was carried out through two rounds of Delphi surveys involving 26 experts to ensure the appropriate extraction of the model's components and indicators. Ultimately, eight core policy goals and 20 indicators were identified: “Enhancement of the Special Education Delivery System” (3 indicators), “Strengthening Special Education Teacher Professionalism” (2 indicators), “Reinforcement of Individualized Special Education Support” (2 indicators), “Internalization of Disability Understanding (Human Rights Protection) Education” (4 indicators), “Substantiation of Inclusive Education” (3 indicators), “Fortification of Career and Vocational Education” (2 indicators), “Internalization of Education Support for Infants and Toddlers with Disabilities” (3 indicators), and “Expansion of Higher and Lifelong Education Opportunities” (2 indicators). Discussion: The study also discussed recommendations, implications, and future research directions based on these findings.

Factors Influencing Innovation of Female Managers on Their Business Performance

Management is a cornerstone of commercial corporations, often regarded as the executive organ of the business entity. The creativity and innovation of managers play a crucial role in the survival and success of any organiza- tion, without which firms risk stagnation and decline in the competitive business landscape. The aim of this study is to investigate the factors in- fluencing the innovation of female managers and its impact on business performance in companies located in Zanjan city. Employing the Delphi method, a qualitative-descriptive approach is adopted to conduct applied research and survey research methodologies. By gathering comments and feedback from female executives in Zanjan city companies through three rounds of Delphi surveys, and analyzing responses from 30 participants, six key factors influencing innovation were identified and validated using Mimac software. These factors, as perceived by female executives, include exhilaration, innovation, risk appetite, assiduity, availability of expert hu- man resources, and tension reduction. The study concludes with practical recommendations derived from the research findings, offering insights for managerial practice and organizational development.

Determining the significant contributing factors to the occurrence of human errors in the urban construction projects: A Delphi-SWARA study approach

The construction industry is believed to be more susceptible to human errors than other industries because of its unique characteristics, particularly when it comes to urban construction projects (UCP). Despite the considerable attention given to human errors in construction sector, there has been a lack of emphasis on analysing these errors in specific projects like construction in urban environments with distinct complexities. Hence, this paper seeks to determine and assess the critical factors influencing human errors associated with the UCP. In this vein, Three rounds of Delphi surveys were done with 17 specialists in safety and construction management. According to the Delphi survey results, 35 substantial factors that contribute to the incidence of human errors in the UCP were discovered. Then, an empirical questionnaire based on the 5-point Likert scale of measurement was developed and distributed among 37 construction experts to assess the level of impact that each factor on occurring human error in the UCP. The questionnaire had 35 influential factors related to human errors, categorised into five primary divisions (environmental, technological/information systems, individual (permanently related), individual (temporarily related), and organisational). Before the distribution, the reliability and validity of the questionnaire were evaluated and confirmed. The factors were ranked using the Step-wise Weight Assessment Ratio Analysis (SWARA) technique at this point. The research findings indicated that the criterion of “technological factors/information systems” is the most crucial, with the criterion of “individual factors (permanently related)” coming in second and the criterion of “environmental factors” coming in third. The sub-criterion “weak maintenance management systems” scores first in the general ranking of sub-criteria, indicating traditional network systems, the absence of appropriate tools and equipment, and a lack of understanding of required resources. The sub-criterion “defects in details and information and lack of design dynamism” is placed second, while the sub-criterion “violation of safety regulations (use of drugs, etc.)” is ranked third. The study results can help industry practitioners make more educated judgements to minimise and manage human errors in the UCP.

Establishing a course to train ICU nurses on prone position mechanical ventilation: A Delphi study.

To develop a comprehensive training course for training ICU nurses in prone positioning. A mixed study combining semi-structured interviews and two rounds of Delphi surveys. We constructed a questionnaire after collecting data through a literature review and semi-structured interviews. We used the Delphi expert correspondence method to conduct two rounds of research among 17 experts in the field of critical illness. Data collection took place between May and August 2022. The effective questionnaire recovery rate was 88.2%. The expert authority coefficient was 0.876; the Kendall coordination coefficient was 0.402; the average importance score for each index ranged from 4.00 to 4.93; and the coefficient of variation for each index ranged from 0.05 to 0.19. We established 13 second-level indicators and 41 third-level indicators on prone position ventilation training according to three aspects: training contents, training methods and training assessment. The training system of prone mechanical ventilation for ICU nurses established in this study will provide an effective framework for training and evaluating the practical ability of prone mechanical ventilation for ICU nurses.

The purpose of this study is to develop a verbal behavior assessment tool for asd children using ICF Core Set-Autism Children Brief and verify the validity. Two rounds of Delphi survey on validity of items from IICF Core Set-Autism Children Brief was undertaken by 9 panels who are professionals in applied behavior analysis. As a result, 22 items were identified to be suitable among 48 items. For developmental level suitability, pre-talking and watching were the most suitable items for pre-listener level and listener level and speaker level were found to share common items such as memory function, intellectual function, pay attention, overall psychosocial functions. We discussed about implications that verbal behavior assessment tool for asd children should consider based on the results.

Construction of Training Curriculum System for Venous Thromboembolism (VTE) Liaison Nurses

Objective To construct a training curriculum system for venous thromboembolism (VTE) liaison nurses and provide a basis for corresponding clinical training, evaluation and certification. Methods Analyzed the training needs of VTE liaison nurses. Conducted two rounds of Delphi surveys with 27 experts from Hubei, Sichuan, Guizhou, Guangxi and Guangdong provinces in China. Determined the weights of indicators at all levels using the Analytic Hierarchy Process (AHP) and equal weighting method. Results The age of the participants ranged from 38~59 (46.63±5.343), and the years of working ranged from 12~42 (25.48±7.439). 27 and 26 valid questionnaires were collected respectively from the first and second rounds of the surveys. The effective response rates of the two-round Delphi survey were 90.0% and 96.3%, respectively, and the experts’ authoritative coefficients were 0.884 and 0.906, respectively, and Kendall’s coefficient of concordance of experts’ opinions were 0.261 and 0.276, respectively (P &amp;lt;0.01). A training system with 5 first-level indicators and 26 second-level indicators was finally formed after two rounds of Delphi surveys. Conclusions The training system for VTE liaison nurses constructed in this study is highly scientific and practical, which can provide a basis for the training assessment and ability evaluation of VTE liaison nurses, and also provide a reference for VTE prophylaxis, treatment and nursing.

Enhancing asthma care in Kuwait: insights from a modified Delphi-based consensus on the Global Initiative for Asthma 2022 guidelines and management practices

Aim: Asthma represents a significant health burden in Kuwait, with high prevalence rates among adults and children. Most asthma patients rely on government healthcare facilities for management, so there is a pressing need to optimize asthma care and treatment strategies. A cross-sectional paper-based survey was conducted to gather insights from allergists and pulmonologists across various healthcare facilities in Kuwait. Methods: Twenty-six medical professionals participated, sharing their perspectives on asthma management practices and adherence to the Global Initiative for Asthma (GINA) 2022 guidelines through answering a modified single round Delphi survey. Results: A high level of consensus on the practicality of guideline changes and the importance of spirometry in diagnosis were reported. However, agreement varied regarding optimal management strategies and medication preferences, indicating areas of divergence among experts. Notably, while there was strong support for inhaled corticosteroid (ICS)-long-acting β2 agonist (LABA) therapy during exacerbations, opinions differed on the use of short-acting β2 agonist (SABA) inhalers as rescue medication. Additionally, the study highlighted challenges in achieving higher levels of agreement, particularly regarding the frequency of inhaler technique checks and specialist referrals for severe asthma cases. Conclusions: This study provided valuable insights into current asthma management practices in Kuwait and identified opportunities for consensus-driven strategies aligned with GINA guidelines. By incorporating diverse perspectives from expert allergists and pulmonologists, it contributed to the enhancement of asthma care and patient outcomes in Kuwaiti healthcare settings.
