Critical risk factors for implementing building information modelling (BIM): a Delphi-based survey

Building information modelling (BIM) is one of the new technologies that, despite its perceived benefits and positive impacts towards the project objectives, has a very low level of adoption. The main problem with this issue may be attributed to several potential risk factors that disrupt the implementation of this technology. Previous research studies have identified various significant risk factors for implementing BIM technology, however, the relationships between these risk factors have not been evaluated and analyzed. This paper aims to identify and evaluate the critical risk factors (CRFs) for BIM adoption via several rounds of Delphi surveys. A total of 52 potential risk factors were identified and classified by an extensive desktop literature review. The analysis of Delphi questionnaires, which were distributed and responded by a panel of BIM experts in three rounds, identified 36 major factors as CRFs of BIM. Then the relationships between these 36 CRFs were determined and assessed by using the decision-making trial and evaluation laboratory method (DEMATEL). The results showed that the CRFs such as: lack of knowledge of BIM and need for software training, resistance to change, and lack of skilled BIM architects/engineers have the most profound impact and interaction with other risk factors. The identification and prioritization of the CRFs can enable BIM users to conduct a systematic risk management and analysis and develop appropriate effective strategies for mitigating the potential risks associated with BIM implementation in a proactive manner.

Identifying behavior change techniques for inclusion in a complex intervention targeting antipsychotic prescribing to nursing home residents with dementia.

Nursing home residents with dementia are commonly prescribed antipsychotics despite the associated increased risk of harms. Interventions to optimize prescribing practice have been found to be effective in the short term, but there is a lack of evidence to support sustainability of effects, along with a lack of theory, public involvement, and transparency in the intervention development process. Using theory has been advocated as a means of improving intervention sustainability. The aim of this study was, therefore, to identify behavior change techniques (BCTs) for inclusion in a complex intervention targeting antipsychotic prescribing to nursing home residents with dementia. A comprehensive approach to identifying a long list of all potential BCTs from three different sources was undertaken. The most appropriate BCTs were then selected through a two-round Delphi consensus survey with a broad range of experts (n = 18 panelists). Advisory groups of people with dementia, family carers, and professional stakeholders provided feedback on the final BCTs included. After two Delphi survey rounds, agreement was reached on 22 BCTs. Further refinement of the selected BCTs based on advisory group and panelists' feedback, along with use of the APEASE criteria (Affordability, Practicability, Effectiveness, Acceptability, Side effects/safety, and Equity) resulted in a final list of 16 BCTs. The next step in intervention development will be to identify the most appropriate mode of delivery of the 16 BCTs identified for inclusion. The study provides a case example of a systematic approach to incorporating evidence with stakeholder views in the identification of appropriate BCTs.

Design and development of a web-based registry for Coronavirus (COVID-19) disease.

Background: The 2019 coronavirus (COVID-19) is a highly contagious disease associated with a high morbidity and mortality worldwide. The accumulation of data through a prospective clinical registry enables public health authorities to make informed decisions based on real evidence obtained from surveillance of COVID-19. This registry is also fundamental to providing robust infrastructure for future research surveys. The purpose of this study was to design a registry and its minimum data set (MDS), as a valid and reliable data source for reporting and benchmarking COVID-19. Methods: This cross sectional and descriptive study provides a template for the required MDS to be included in COVID-19 registry. This was done by an extensive literature review and 2 round Delphi survey to validate the content, which resulted in a web-based registry created by Visual Studio 2019 and a database designed by Structured Query Language (SQL). Results: The MDS of COVID-19 registry was categorized into the administrative part with 3 sections, including 30 data elements, and the clinical part with 4 sections, including 26 data elements. Furthermore, a web-based registry with modular and layered architecture was designed based on final data classes and elements. Conclusion: To the best of our knowledge, COVID-19 registry is the first designed instrument from information management perspectives in Iran and can become a homogenous and reliable infrastructure for collecting data on COVID-19. We hope this approach will facilitate epidemiological surveys and support policymakers to better plan for monitoring patients with COVID-19.

Development of Surgical Scrubbing, Gowning and Gloving Checklist using the Delphi Method

Background Competency in surgical scrubbing, gowning and gloving (scrubbing-in) procedures is necessary to participate in surgery, but difficult for trainees to learn due to the fast pace of the operating room and limited availability of experts to provide training. Current methods of scrubbing-in procedures are antiquated. Simulation-based education aligns with the paradigm shift occurring in medical education as we transition from passive models. This study utilized the Delphi technique to identify necessary items to include in a scrubbing-in checklist that can be used in future simulation and assessment tools.MethodsSurgeon experts participated in three rounds of a Delphi process. The panel consisted of surgical specialty residents, fellows and attending surgeons from gynecology, urology, general surgery, plastic, orthopedic and neurosurgery.ResultsFrom January 2018 to September 2018, 39 experts from 6 different surgical subspecialties participated in three Delphi survey rounds. Of the 43 items identified during the first round, 30 (70%) resulted with an importance rating > 4.0. During the final round, a consensus of > 85% was reached for 22 total items (73%).ConclusionWe developed a 22-step checklist for scrubbing-in procedures utilizing the Delphi technique. This checklist will enhance undergraduate medical education simulation curriculums and assessments.

Coping and wellbeing in bereavement: two core outcomes for evaluating bereavement support in palliative care

BackgroundBereavement support is a core part of palliative care. However, the evidence base is limited by a lack of consistency in the outcomes used to evaluate services and models of support, which makes it difficult to compare approaches. Core Outcome Sets (COS) represent the minimum that should be measured in research into specific conditions or services. The aim of this study was to use a stakeholders’ perspective to develop a COS for evaluating bereavement support for adults in adult palliative care settings.MethodsA list of outcomes relevant to bereavement support was created following a systematic review of the quantitative and qualitative literature. At an expert workshop 21 stakeholders discussed their views on the most important outcomes and compared these to and critiqued the lists constructed from the review. These lists and discussions informed a two round international DELPHI survey (n = 240) designed to reach consensus on which outcomes/outcome dimensions should be included in the COS. To prioritise and validate the items emerging from the survey, participants at a subsequent consensus day ranked the relative importance of these items (n = 23). A final feedback exercise with these consensus day participants was conducted to confirm the selection of outcomes and dimensions.Results‘Ability to cope with grief’ and ‘Quality of life and mental wellbeing’ were selected as two core outcomes. Twenty-one different dimensions to explore when assessing these outcomes were also identified. The coping related dimensions have been categorised as: Negative and overwhelming grief; Communication and connectedness; Understanding, accepting and finding meaning in grief; Finding balance between grief and life going forwards; Accessing appropriate support. Those relating to quality of life and wellbeing have been categorised as; Participation in work and/or regular activities; Relationships and social functioning; Positive mental wellbeing and Negative mental and emotional state.ConclusionThis COS outlines a more consistent way forward for bereavement researchers and practitioners, whilst also orientating towards public health and resilience-based approaches to bereavement care. Further work is planned to identify and develop measures which are specific to this core outcome set, and which will facilitate the future comparability of bereavement services and interventions.

Determine what to measure and how to measure in clinical trials for the treatment of pressure injury: A protocol for the development of a core outcome set.

Background:A core outcome set (COS) is an agreed minimum set of outcomes that should be reported in all clinical trials in specific areas of health care. A considerable amount of trials did not report essential outcomes or outcomes measurement methods, which makes it challenging to evaluate the efficacy and safety of treatment strategies for pressure injury (PI) and produced significant heterogeneity of reported outcomes. It is necessary to develop a COS, which can be used for clinical trials in PI treatment.Methods/Design:The development of this COS will be guided by an advisory group composed of clinicians, senior nurses, patients, and methodologists. We will search six databases and 2 registry platforms to identify currently reported PI treatment outcomes and outcome measurement instruments in randomized controlled trials, meta-analysis, and systematic reviews. We will also conduct a semi-structured interview with clinicians, nurses, and adult PI patients to collect their opinions on important outcomes. Each outcome of the initial list generated from systematic review and interviews will be scored and reach a consensus through two rounds of international Delphi survey with all key stakeholders. A face-to-face consensus meeting with key stakeholders will be conducted to finish a final COS and recommend measurement instruments for each outcome.Results:We will develop a COS that should be reported in future clinical trials to evaluate the effectiveness of PI treatment.Discussion:The COS will follow current guidance to develop a high-quality COS in the field of PI treatment to reduce heterogeneity in trial reporting, facilitate valid comparisons of new therapies, and improve the quality of clinical trials.

Third party logistics providers: sustainability performance measurement framework

Over the past two decades, the concept of outsourcing has driven the growth of third party logistics (3PL). Intensive competition led to the necessity of sustainability for 3PL firms. Although there are studies on 3PL sustainability; there is no comprehensive framework for measuring 3PL sustainability performance. Based on triple bottom line (TBL) framework, the purpose of this paper is to develop a sustainability performance measurement framework for 3PL, which can be used as a guide for the development of sustainable 3PL businesses. Related research and academic papers were reviewed; content analysis was used. Subsequently, two rounds of Delphi survey were carried out to derive the set of measures and dimensions of a sustainability performance measurement framework. This study shows that in addition to the TBL framework, two additional dimensions emerge. Sustainability performance measurement framework for 3PL includes five dimensions: economic performance, environmental performance, social performance, operational excellence and service capabilities.

How to Optimise Cognitive Behaviour Therapy (CBT) for People with Autism Spectrum Disorders (ASD): A Delphi Study

Children and adults with autism spectrum disorders (ASD) can benefit from cognitive behaviour therapy (CBT), yet the prevailing opinion is that this requires adaptations to accommodate commonly experienced socio-communication and neuropsychological impairments. There are, however, no empirically-derived guidelines about how best to adapt standard practice. In a three round Delphi survey, we asked expert clinicians and clinical-researchers, based in England, about how to optimise the design, delivery and evaluation of CBT for people with ASD. Of 50 people approached, 18 consented to take part in Round 1, nine in Round 2 and eight in Round 3. Using a five-point scale, participants rated the degree to which 221 statements—pertaining to the referral process, assessment, engagement, formulation, goal setting, therapy structure, interventions and techniques, homework, outcome measurement, managing endings and therapist attributes—were integral to CBT. The consensus was that 155 statements represented essential or important components of CBT. Adaptations to the structure and process of therapy were consistently endorsed, and an individualised formulation-derived approach was favoured when deciding upon which interventions and techniques to offer. Further studies are needed to clarify if adapted CBT is associated with improved treatment outcomes and acceptability.

Standardised criteria for classifying the International Classification of Activities for Time-use Statistics (ICATUS) activity groups into sleep, sedentary behaviour, and physical activity

BackgroundGlobally, the International Classification of Activities for Time-Use Statistics (ICATUS) is one of the most widely used time-use classifications to identify time spent in various activities. Comprehensive 24-h activities that can be extracted from ICATUS provide possible implications for the use of time-use data in relation to activity-health associations; however, these activities are not classified in a way that makes such analysis feasible. This study, therefore, aimed to develop criteria for classifying ICATUS activities into sleep, sedentary behaviour (SB), light physical activity (LPA), and moderate-to-vigorous physical activity (MVPA), based on expert assessment.MethodWe classified activities from the Trial ICATUS 2005 and final ICATUS 2016. One author assigned METs and codes for wakefulness status and posture, to all subclass activities in the Trial ICATUS 2005. Once coded, one author matched the most detailed level of activities from the ICATUS 2016 with the corresponding activities in the Trial ICATUS 2005, where applicable. The assessment and harmonisation of each ICATUS activity were reviewed independently and anonymously by four experts, as part of a Delphi process. Given a large number of ICATUS activities, four separate Delphi panels were formed for this purpose. A series of Delphi survey rounds were repeated until a consensus among all experts was reached.ResultsConsensus about harmonisation and classification of ICATUS activities was reached by the third round of the Delphi survey in all four panels. A total of 542 activities were classified into sleep, SB, LPA, and MVPA categories. Of these, 390 activities were from the Trial ICATUS 2005 and 152 activities were from the final ICATUS 2016. The majority of ICATUS 2016 activities were harmonised into the ICATUS activity groups (n = 143).ConclusionsBased on expert consensus, we developed a classification system that enables ICATUS-based time-use data to be classified into sleep, SB, LPA, and MVPA categories. Adoption and consistent use of this classification system will facilitate standardisation of time-use data processing for the purpose of sleep, SB and physical activity research, and improve between-study comparability. Future studies should test the applicability of the classification system by applying it to empirical data.

The prioritization of technologies and public R&D roles between the manufacturing and service industries in the fourth industrial revolution

PurposeThe purpose of this study is to reveal the similarities and differences between the manufacturing and service industries in their prioritization of technologies and public research and development (R&D) roles, along with the complementation of properties of technology and public R&D role in the context of Fourth Industrial Revolution.Design/methodology/approachTwo rounds of Delphi surveys were designed to meet the purpose of this study, which used rigorous triangulation techniques. The Delphi method was combined with the brainstorming method in the first-round Delphi survey, while the second-round Delphi survey focused on experts’ judgments. Finally, language network analysis was performed on the properties of technology and public R&D roles to complement the data analyses regarding prioritization.FindingsThis study identifies different prioritizations of five similar key technologies in each industry, so that it can note different technological impacts to the two industries in the Fourth Industrial Revolution. Smart factory technology is the first priority in the manufacturing industry, whereas artificial intelligence is the first priority in the service industry. The properties of the three common technologies: artificial intelligence, big data and Internet of things in both industries are summarized in hyper-intelligence on hyper-connectivity. Moreover, it is found that different technological priorities in the service and manufacturing industries require different approaches to public R&D roles, while public R&D roles cover market failure, system failure and government failure. The highest priority public R&D role for the service industry is the emphasis of non-R&D roles. Public R&D role to solve dy-functions, focus basic technologies and support challenging areas of R&D is prioritized at the highest for the manufacturing industry.Originality/valueThis study of the different prioritizations of technologies in the manufacturing and service industries offers practical lessons for executive officers, managers and policy-makers. They, by noting the different technological impacts in the manufacturing and service industries, can prepare for current actions and establish the priority of technology for R&D influencing the future paths of their industries in the context of the Fourth Industrial Revolution. While managers in the service industry should pay greater attention to the technological content of hyper-intelligence and hyper-connectivity, managers in the manufacturing industry should consider smart factory and robot technology.

How to evaluate the clinical outcome of joint-preserving treatment for osteonecrosis of the femoral head: development of a core outcome set

BackgroundThis study aimed to develop a core outcome set (COS) for clinical trials of joint-preserving treatment for osteonecrosis of the femoral head (ONFH), that is, to define a minimal set of outcomes that should be reported in such trials.MethodsA mixed research method was adopted in this study. First, clinical trials of hip preservation therapy were systematically researched and analyzed. Second, a three-round Delphi survey involving both doctors and patients was carried out to obtain the core outcome indicators. Round 1 was a modified Delphi questionnaire for doctors and patients to determine which outcomes are important to these stakeholders, round 2 determined what clinical evaluation core outcomes should be included for the joint-preserving treatment of ONFH, and round 3 determined how core outcomes should be measured. Finally, a consensus meeting was held to discuss and vote on the established COS.ResultsThe results of the systematic review showed that 42 outcome indicators were classified according to common signs and symptoms, quality of life, long-term outcomes, radiological evaluation, blood biochemistry, and indexes of safety. The three rounds of Delphi surveys completed the selection of indicators for the COS and the determination of the corresponding measurements.A total of 73 orthopedic doctors and 103 patients participated in round 1, and the top 10 indicators selected were basically the same. In round 2, 32 experts identified the following indicators: pain, range of motion (ROM) of hip flexion, walking distance, and stable rating of X-ray images. In round 3, 35 experts defined the measurement of each indicator. Finally, the consensus meeting identified the four indicators aforementioned that constituted the COS.The scores for pain, ROM of hip flexion, and walking distance are from 0 to 10; 0 represents the best scores, while 10 represents the most serious impairment. The stable rating of X-ray images is determined by the morphology of the femoral head and the change in the density of the necrotic area.ConclusionsWe established a COS for hip-preserving treatment of ONFH that includes four indicators: pain, ROM of hip flexion, walking distance, and stable rating of X-ray images.

Assigning Weights for Modified Project Quarter Back Rating Based Construction Project Performance Model

The Indian construction industry is facing challenges due to performance shortfalls. The construction projects are highly complex, distinctive, fragmented and do not have of well-established performance assessment models to evaluate their project success. Assessing overall project success is not possible to measure by single factor. To address this limitation, the Modified Project Quarter Back Rating (PQR) model was developed for the Indian construction industry. Modified PQR model’s output is a project score based on performance areas affecting project success and outcome. The model integrates ten performance areas that have identified through literature review. Performance areas included in the model are; (i) Cost (ii) Schedule (iii) Stakeholder satisfaction (iv) Safety (v) Quality (vi) Finance (vii) Environment (viii) Communication and collaboration (ix) Customer relation and (x) Productivity. These performance areas are measured through different performance metrics; i.e. performance metrics scores are aggregated to compute performance area scores. The model gives a single score that will help in comparing overall performance for different projects. This paper attempted to highlight the importance of performance metrics in modified Project Quarter Back Rating (PQR) based construction project performance assessment model for the Indian construction industry. The validity of the model needs assigning appropriate weights to the performance metrics as the weightage determines the relevance. Appropriate weights were determined using two round Delphi survey with 12 experts. Findings reveal that two performance metrics; return business from customer relation and OSHA recordable from safety have high weights. This modified PQR model will help key participants of the industry to compare the performance of various projects and to monitor performance areas that impact project performance rating.

The development of Nursing Quality Care Process Metrics and Indicators for use in Older Persons Care Settings: A Delphi-Consensus Study.

To develop a suite of nursing quality care process metrics and indicators for older persons care settings in Ireland. Regulatory investigations of health system failures highlight non-adherence to clinical guidelines and standards resulting in deficiencies in nursing care delivery. Limited attention has been paid to measuring nursing care processes particularly in the care of older people. Quality care process metrics can facilitate measurement of nurse-sensitive measures of care. A scoping literature review and modified Delphi-Consensus Technique. A scoping review of literature published between January 2007 - January 2017 was conducted to identify nursing process metrics and indicators. The Delphi Consensus phase incorporated a four-round electronic survey of 404 nurses and a consensus meeting with 13 stakeholders working in Older Persons Care Settings in Ireland. From the review, 33 potential metrics were identified. After all Delphi survey rounds, 20 metrics and 90 associated indicators were selected by the nurses. Following the consensus meeting, 19 metrics and 80 indicators were included in the final suite of nursing quality care process metrics and indicators. Developing this suite of nursing quality care process metrics and indicators for use in older persons care settings provided consensus on what nursing processes should be measured to improve the quality and safety of care delivery. The nursing processes identified, provide a framework for future research and educational programmes in the care of older persons. Although conducted in the Irish healthcare system, there is potential for adoption or adaption in other healthcare settings.

The doctor in my pocket: examining mobile approaches to personal wellbeing.

Today's democratized media access, online engagement, and digital clout offer global opportunities for individual influence and customized messaging strategies. As such, public health initiatives are dramatically changing as target audiences decide where to place their time and attention regarding health information. This article reports on ways in which health professionals and mobile techies are teaming up to create pocket platforms for personal wellbeing. The following work also considers how healthcare digitization and mobilization operate in relation to public wellbeing. Using a double round Delphi survey, nine industry experts were asked about the impact of new media tools on health initiatives and subsequent healthcare behaviors. The panel also addressed the impact and efficacy of the mobile health movement. Expert testimony identified ways in which millennial media can encourage (and sometimes prevent) improved patient, community, and population health outcomes. Industry leaders further addressed strategic moves healthcare professionals are making to ensure personal healthcare communication remains relevant, accessible, and attainable for the general public. Clear themes emerged throughout the iterative Delphi process, resulting in the overarching categories of information access, information literacy, patient privacy, patient accountability, physician engagement, and healthcare quality. This work connects theory with practice by examining developing health communication strategies through compliance-gaining choice models and modern marketing research. Findings delineate best practices for health communication and administration in the digital age, providing both a critical account of and practical approach to healthcare messaging within the millennial context. Results call attention to the impact of media platforms on information distribution, adaptive communication strategies, and overall health communication processes. The study invites further discussion regarding digital shifts in healthcare messaging and subsequent influences on patient/consumer compliance.

Significant Sustainable Facilities Management (SFM) Practices in the Health Care (HC) Sector

Health Care (HC) is identified as one of the most polluting industries. HC is recognised as the second energy intensive sector emitting 8% out of total 40% of CO2. HC is also ranked among the top 4 sources in spreading harmful substances in affecting health and wellbeing of the society. Facilities Managers are identified to be in a unique position to manage the building’s operations and hence integrating sustainability in the current practice of Facilities Management (FM) in HC sector. However, identification of sustainable FM practices is challenging as FM practice differ in terms of facility type, building engineering system, organisation’s business sector and culture. In this regard, the paper investigates the significant FM services and practices in integrating sustainability in HC sector.The study used a two round Delphi survey involving 10 experts who are specialised in FM and sustainable practices in the HC sector to identify the most significant FM services and sustainability practices. The results identified 8 FM services and 44 sustainability practices as significant. Building services (BS) was identified as the most significant FM service, whist practices such as “identifying applications for energy saving measures” and “ensure appropriate onsite, off-site storage, transport of wastes” were identified as the most important sustainability practices. The results of the study will be further carried forward to a questionnaire survey to rank and assign scores for each significant sustainability practices in order to develop an assessment framework to evaluate the SFM practice in HC sector.

Development of quality evaluation indicator system of midwifery care in Shanghai

Objective To develop a quality evaluation indicator system of midwifery care in line with the modern service concept in Shanghai area under the background of over-medicalization trend of midwifery services, and to guide the improvement direction of midwifery quality. Methods Guided by the evidence-based framework of midwifery quality and indicator selection principles, the indicators for evaluating midwifery quality were preliminarily selected by literature review and theoretical analysis. Two rounds of Delphi survey among 53 experts from Shanghai were conducted to decide the final quality evaluation indicator system of midwifery care. Analytic hierarchy process (AHP) and the importance′ sum average method were adopted to calculate weights coefficients of the indicators. Results 45 indicators for evaluating midwifery quality were preliminarily selected. The response rate of two rounds of Delphi survey were 76% and 100%, the authority coefficients were 0.90 and 0.89, expert opinion coordinate coefficients were 0.190 and 0.257, respectively. The final version of the indicator system consists of 11 structure quality indicators, 13 process quality indicators and 19 outcome quality indicators. The weight values of the three first-level indicators were 0.195 8, 0.310 8 and 0.493 4 respectively. The weight values of the 43 second-level indicators ranged from 0.016 1 to 0.028 6. Conclusions The quality evaluation indicator system of midwifery care developed fits evidence-based midwifery philosophy. It provides a scientific tool for midwifery quality evaluation in Shanghai, which can guide the improvement of midwifery quality. LO43: Simulation curricular content in postgraduate emergency medicine: a multicenter delphi study

Introduction: There is increasing evidence to support the integration of simulation into medical training; however, no national emergency medicine (EM) simulation curriculum currently exists. Using Delphi methodology, we aimed to identify and establish content validity evidence for EM curricular content best suited for simulation-based training to inform national postgraduate EM training. Methods: A national panel of experts in EM simulation-related education iteratively rated potential curricular topics, on a 4-point scale, to determine those best suited for simulation-based training. After each round, responses were analyzed and topics scoring <2/4 were removed. Remaining topics were resent to the panel for further ratings until consensus was achieved, defined as Cronbach α ≥ 0.95. At conclusion of the Delphi process, topics that were rated ≥3.5/4 were considered core curricular topics, while those rated 3.0-3.5 were considered extended curricular topics. Results: Forty-four experts from 13 Canadian centres participated. Two hundred and eighty potential curricular topics, in 29 domains, were generated from a systematic review of the literature, analysis of relevant educational documents and a survey of Delphi panelists. Three rounds of Delphi surveys were completed before consensus was achieved, with response rates ranging from 93-100%. Twenty-eight topics, in 8 domains, reached consensus as core curricular topics. An additional 35 topics, in 14 domains, reached consensus as extended curricular topics. Conclusion: Delphi methodology allowed for achievement of expert consensus and content validation of EM curricular content best suited for simulation-based training. These results provide a foundation for improved integration of simulation into postgraduate EM training and can be used to inform a national simulation curriculum to supplement clinical training and optimize learning.
