The Quality of Care Is Unmeasurable: An Equation, a Paralogism, a Paraphrasis, a Quackery... and a Reply-Reply

In Reply.— I have read Dr Sinnreich's letter many times and have come to surprising conclusion that he is accusing me of conspiring with Drs Ellwood, Relman, and Roper to impose on medical community notion that quality medical care can be measured only by referring to outcome. Dr Sinnreich's last paragraph implies that the respectability of their [Drs Ellwood, Relman, and Roper] pursuits depends on the theoretical rationale of my argument. I am honored to be associated with endeavors of these renowned physicians, but I urge Dr Sinnreich to accept my disclaimer that any conspiracy exists. If these physicians find or fail to find some value in my article, it will not be because of some collusion between us, but because they have read with open minds my independent attempt to clarify notion of quality medical care. When Should One Ligate the Umbilical Cord?

Having observed and photographed obstetrical techniques and practices on six continents in 1971, I am struck by the differing views of renowned physicians in the various developed and developing countries as to the relative merits of early and late clamping of the umbilical cord. While specialists in the Netherlands, a country with a low incidence of infant mortality essentially equivalent to that of Sweden, stress the importance of placental transfusion as a normal part of the physiological sequence of birth which lessens the likelihood of maternal hemorrhage and anemia in the growing child, Yao et al. in their recent paper published in the December 1971 issue of PEDIATRICS, suggest that placental transfusion causes "overloading" of the vascular bed and therefore should be avoided by early clamping.
