Mechanistic Role of Methanolic Extract of Taraxacum officinale Roots as Cardioprotective Against Ischemia-Reperfusion Injury-Induced Myocardial Infarction in Rats.

Taraxacum officinale play an important role in the prophylaxis and treatment of cardiovascular disease (CVD). Taraxacum officinale is proven as promising antioxidant in earlier studies and one of its constituent "cichoric acid" is shown to have vasorelaxant property. Therefore, present study mainly designed to investigate the cardioprotective effects of Taraxacum officinale against ischemia-reperfusion injury (I/R injury)-induced myocardial dysfunction in rats. This study not only explored the overall cardioprotective potential but also tried to explore its molecular mechanism using pharmacological inhibition via L-NAME and glibenclamide. Pretreatment of methanolic extract of Taraxacum officinale significantly attenuated (p < 0.001) increased levels of lactate dehydrogenase (LDH), creatine kinase (CK), infarct size, and thiobarbituric acid reactive substance (TBARS), while it increased the reduced levels of protein content, glutathione (GSH), and catalase (CAT) activity. Results showed that pretreatment with methanolic extract of Taraxacum officinale provides cardioprotection against I/R induced myocardial dysfunction, at least, may be mediated through the endogenous release of nitric oxide.

High Doses of Inhaled Nitric Oxide as an Innovative Antimicrobial Strategy for Lung Infections.

Since the designation of nitric oxide as “Molecule of the Year” in 1992, the scientific and clinical discoveries concerning this biomolecule have been greatly expanding. Currently, therapies enhancing the release of endogenous nitric oxide or the direct delivery of the exogenous compound are recognized as valuable pharmacological treatments in several disorders. In particular, the administration of inhaled nitric oxide is routinely used to treat patients with pulmonary hypertension or refractory hypoxemia. More recently, inhaled nitric oxide has been studied as a promising antimicrobial treatment strategy against a range of pathogens, including resistant bacterial and fungal infections of the respiratory system. Pre-clinical and clinical findings have demonstrated that, at doses greater than 160 ppm, nitric oxide has antimicrobial properties and can be used to kill a broad range of infectious microorganisms. This review focused on the mechanism of action and current evidence from in vitro studies, animal models and human clinical trials of inhaled high-dose nitric oxide as an innovative antimicrobial therapy for lung infections.

The responses of sulforaphane synthesis and endogenous NO to Penicillium chrysogenum elicitor in Broccoli cells

In this study, we investigated the effects Penicillium chrysogenum elicitor and NO synthesis inhibitor on phenylalanine ammonia lyase (PAL) activity, cell viability, endogenous nitric oxide (NO) and NO synthase (NOS) and nitrate reductase (NR), secondary metabolite sulforaphane accumulation. In addition, the link of between endogenous nitric oxide (NO) and sulforaphane, as well as between NR and NOS pathway was also assessed. The results showed that all the concentrations of Penicillium chrysogenum elicitor induced NO synthesis, and the synthesis reaction time was earlier under high concentrations of Penicillium chrysogenum elicitor, while the released rate of NO was relatively slower under low concentration of Penicillium chrysogenum elicitor. In addition, NO scavenger carboxy-PTIO (c-PTIO) blocked the effect of Penicillium chrysogenum elicitor on the promotion that sulforaphane accumulation and the activation of PAL activity, and decrease cell vitality induced by Penicillium chrysogenum elicitor. The content of sulforaphane was related with NO accumulation, and the accumulation of sulforaphane was increased when endogenous NO release was excited, while sulforaphane accumulation amount decreased when NO synthesis was inhibited. Under the induction of Penicillium chrysogenum elicitor, the release of NO from in vitro cells of broccoli was mainly through NOS and NR pathways. When NR pathway was inhibited by high concentrations of NR inhibitor Tungstate, NOS activity was activated via high concentration of K+ from Tungstate. However, NR pathway could not be activated by high concentration of K+ when the NOS pathway was inhibited by NOS inhibitor L-NAME.

Polydopamine-Modified Copper-Doped Titanium Dioxide Nanotube Arrays for Copper-Catalyzed Controlled Endogenous Nitric Oxide Release and Improved Re-Endothelialization.

The controllable release is necessary for ideal drug delivery technologies. Because of their high specific surface area and high porosity, titanium dioxide nanotubes (TNTs) have been widely used as drug carriers in medical devices. By loading copper as the catalyst, nitric oxide (NO) generation was facilitated by catalyzing the decomposition of renewable endogenous NO donors in vivo. Herein, the long-term controllable release profile of NO is highlighted owing to the multilayer polydopamine (PDA) cap structure. Different layers of PDA are used to adjust the NO release behavior, and the results show that three layers of PDA can not only effectively prevent the burst release of NO but also maintain long-term stable release of copper ion and NO. The bioactivity of the NO generated from three-layer PDA-modified copper-loaded TNTs (PDA-3L-NTCu2) and unmodified copper-loaded TNTs (NTCu2) are verified by our work, indicating effective inhibition of platelet activation, thrombosis, inflammation, and intimal hyperplasia. Importantly, the PDA-3L-NTCu2 show selectively promote the growth of endothelial cells in vitro and outstanding re-endothelialization for 4 weeks in vivo, as compared to NTCu2, TNTs, and 316L stain steel. This study suggests that copper-loaded with PDA modification helps us achieve controlled long-term stable local NO release with well-retained bioactivity and enhanced re-endothelialization.

Low shear stress damages endothelial function through STAT1 in endothelial cells (ECs).

Low shear stress (LSS) occurs in areas where atherosclerosis is prevalent. Many studies have revealed that signal transducer and activator of transcription 1 (STAT1) plays a significant role in cardiovascular disease. Nonetheless, the mechanism underlying the connection between STAT1 and LSS is not fully understood. The purpose of this study was to investigate the link between LSS and STAT1 in endothelial cells (ECs). Monolayer endothelial cells were stimulated or not stimulated by LSS. Protein expression and phosphorylation levels were determined by western blotting. Immunofluorescence was used to compare the protein expression differences in bifurcated and non-bifurcated human coronary arteries. Endothelial function was assessed by using a dihydroethidium assay, real-time PCR, western blotting and nitric oxide (NO)-sensitive fluorophore. Results showed that STAT1 played a key role in LSS-induced endothelium damage. Firstly, LSS activated STAT1, as evidenced by LSS-induced STAT1 (Tyr701) phosphorylation in ECs in vitro and the increased intimal STAT1 expression at bifurcation of human coronary arteries. Secondly, LSS-induced STAT1 phosphorylation was positively regulated by inhibitor of nuclear factor kappa-B kinase ε (IKKε). Additionally, LSS-promoted inflammatory factor expression was markedly reversed by silencing STAT1 (siSTAT1). LSS also increased reactive oxygen species (ROS) level and decreased endogenous NO release: however, siSTAT1 reversed these adverse effects through upregulating the antioxidant gene heme oxygenase-1(HO-1) and downregulating endothelial nitric oxide synthase (eNOS) Thr495 phosphorylation. According to our results, LSS-mediated EC injury may be associated with the activation of STAT1. Strategies designed to reduce STAT1 expression or inhibit STAT1 activation may be effective approaches for reducing the incidence of atherosclerosis.

S-Nitrosylation of Mixed Lineage Kinase 3 Contributes to Its Activation after Cerebral Ischemia

Previous studies in our laboratory have shown that mixed lineage kinase 3 (MLK3) can be activated following global ischemia. In addition, other laboratories have reported that the activation of MLK3 may be linked to the accumulation of free radicals. However, the mechanism of MLK3 activation remains incompletely understood. We report here that MLK3, overexpressed in HEK293 cells, is S-nitrosylated (forming SNO-MLK3) via a reaction with S-nitrosoglutathione, an exogenous nitric oxide (NO) donor, at one critical cysteine residue (Cys-688). We further show that the S-nitrosylation of MLK3 contributes to its dimerization and activation. We also investigated whether the activation of MLK3 is associated with S-nitrosylation following rat brain ischemia/reperfusion. Our results show that the administration of 7-nitroindazole, an inhibitor of neuronal NO synthase (nNOS), or nNOS antisense oligodeoxynucleotides diminished the S-nitrosylation of MLK3 and inhibited its activation induced by cerebral ischemia/reperfusion. In contrast, 2-amino-5,6-dihydro-6-methyl-4H-1,3-thiazine (an inhibitor of inducible NO synthase) or nNOS missense oligodeoxynucleotides did not affect the S-nitrosylation of MLK3. In addition, treatment with sodium nitroprusside (an exogenous NO donor) and S-nitrosoglutathione or MK801, an antagonist of the N-methyl-D-aspartate receptor, also diminished the S-nitrosylation and activation of MLK3 induced by cerebral ischemia/reperfusion. The activation of MLK3 facilitated its downstream protein kinase kinase 4/7 (MKK4/7)-JNK signaling module and both nuclear and non-nuclear apoptosis pathways. These data suggest that the activation of MLK3 during the early stages of ischemia/reperfusion is modulated by S-nitrosylation and provides a potential new approach for stroke therapy whereby the post-translational modification machinery is targeted.

Renal oxygenation changes during water loading as evaluated by BOLD MRI: Effect of NOS inhibition

To demonstrate a possible role for endogenous release of nitric oxide in determining the response of water loading on intrarenal oxygenation as evaluated by blood oxygenation level-dependent (BOLD) magnetic resonance imaging (MRI). Twelve Sprague Dawley rats (weight 344.9 ± 40.6 g) were equally divided into two groups, A and B. Water loading was implemented by continuous infusion of hypotonic saline containing glucose (0.25% NaCl, 0.5% glucose). Rats in group A were subject to water loading alone, while group B rats were dosed with N-nitro-L-arginine methyl ester, (L-NAME) (10.0 mg/kg) prior to water loading. T(2) *-weighted images of the kidneys were obtained on a Siemens 3T Verio MRI scanner using a multiple gradient recalled echo (mGRE) sequence. Consistent with previous reports, group A exhibited a significant decrease in medullary R(2) * during water loading (40.64 ± 1.10 s(-1) to 34.68 ± 1.49 s(-1) , P < 0.05). On the other hand, in group B there was no decrease in R(2) * during water loading (48.11 ± 2.38 s(-1) to 51.06 ± 2.18 s(-1) ). The increased prewater loading R(2) * is due to the pretreatment with L-NAME (40.82 ± 3.23 s(-1) to 48.11 ± 2.38 s(-1) , P < 0.05). Our data suggest for the first time a role for endogenous nitric oxide in determining the response of renal medullary oxygenation to water loading.

To the Editors: A variety of isoforms of nitric oxide (NO) synthases are constitutively expressed in human airway and vascular endothelial cells continuously generating NO. NO plays an important role in regulating lung function in health and disease including modulation of pulmonary vascular resistance, airway calibre and host defence. Production of NO and its consumption by fluid-phase reactions can be detected and monitored in the exhaled air, providing an important window to assess the dynamics of NO metabolism in health and inflammatory lung conditions, asthma in particular 1. A series of milestone studies uncovered a relative deficiency of pulmonary NO availability in cystic fibrosis (CF), a severe chronic inflammatory lung disease with studies generally showing reduced exhaled NO 2, 3. However, there remains controversy regarding the anatomical localisation and mechanisms of NO deficiency, and its relationship with CF disease progression. By using extended exhaled NO analysis at different expiratory flow rates, indirect studies have attempted to distinguish NO production into bronchial and alveolar compartments based on basic models 4–7. However, the fate of NO delivered directly to the alveoli has not been demonstrated in these patients. Real-Time in Vivo Simultaneous Measurements of Nitric Oxide and Oxygen Using an Amperometric Dual Microsensor

This paper reports a real-time study of the codynamical changes in the release of endogenous nitric oxide (NO) and oxygen (O(2)) consumption in a rat neocortex in vivo upon electrical stimulation using an amperometric NO/O(2) dual microsensor. Electrical stimulation induced transient cerebral hypoxia due to the increased metabolic demands that were not met by the blood volume inside the stimulated cortical region. A NO/O(2) dual microsensor was successfully used to monitor the pair of real-time dynamic changes in the tissue NO and O(2) contents. At the onset of electrical stimulation, there was an immediate decrease in the cortical tissue O(2) followed by a subsequent increase in the cortical tissue NO content. The averages of the maximum normalized concentration changes induced by the stimulation were a 0.41 (±0.04)-fold decrease in the O(2) and a 3.6 (±0.9)-fold increase in the NO concentrations when compared with the corresponding normalized basal levels. The peak increase in NO was always preceded by the peak decrease in O(2) in all animals (n = 11). The delay between the maximum decrease in O(2) and the maximum increase in NO varied from 3.1 to 54.8 s. This rather wide variation in the temporal associations was presumably attributed to the sparse distribution of NOS-containing neurons and the individual animal's differences in brain vasculatures, which suggests that a sensor with fine spatial resolution is needed to measure the location-specific real-time NO and O(2) contents. In summary, the developed NO/O(2) dual microsensor is effective for measuring the NO and O(2) contents in vivo. This study provides direct support for the dynamic role of NO in regulating the cerebral hemodynamics, particularly related to the tissue oxygenation.

Nitric Oxide Is Involved in the Response of the Isolated Intact and Epithelium-Denuded Rat Trachea to the β2 Adrenergic Receptor Agonist Salbutamol

Background: The involvement of the airway smooth muscle mediator nitric oxide (NO) in the actions of the β₂ agonist salbutamol (Sal), a well- known bronchodilator, is very poorly understood. Objectives: To determine if endogenous NO release is a major factor in the Sal-induced relaxation of the carbachol- and electrical field-stimulated rat trachea and determine the role of the tracheal epithelium as the possible source of NO involved in these effects. Methods: Isolated carbachol- or electric field-stimulated pre-contracted in vitro male Sprague Dawley rat tracheas (with epithelium intact or denuded) were relaxed with incremental or discrete concentrations of Sal in the presence and absence of the NO synthesis inhibitor L-NAME. Results: Epithelium-denuded tracheas showed a reduced relaxation response to Sal. L-NAME (1 mM) similarly decreased the sensitivity of the rat tracheas to Sal in both epithelium-intact and -denuded conditions. In the presence of L-NAME, high concentrations of Sal induced an unexpectedly large relaxation response in carbachol-stimulated rat tracheas with both intact and denuded epithelium. Sal relaxation was also affected by L-NAME in electrical field-stimulated epithelium-intact and -denuded tracheas. Conclusion: The results suggest that NO derived from sources other than the epithelium is an important mediator of the Sal-induced relaxation in rat tracheas.

Nitric oxide can acutely modulate its biosynthesis through a negative feedback mechanism on l-arginine transport in cardiac myocytes

Nitric oxide (NO) plays a central role as a cellular signaling molecule in health and disease. In the heart, NO decreases the rate of spontaneous beating and the velocity and extent of shortening and accelerates the velocity of relengthening. Since the cationic amino acid l-arginine (l-Arg) is the substrate for NO production by NO synthases (NOS), we tested whether the transporters that mediate l-Arg import in cardiac muscle cells represent an intervention point in the regulation of NO synthesis. Electrical currents activated by l-Arg with low apparent affinity in whole cell voltage-clamped rat cardiomyocytes were found to be rapidly and reversibly inhibited by NO donors. Radiotracer uptake studies performed on cardiac sarcolemmal vesicles revealed the presence of high-affinity/low-capacity and low-affinity/high-capacity components of cationic amino acid transport that were inhibited by the NO donor S-nitroso-N-acetyl-dl-penicillamine. NO inhibited uptake in a noncompetitive manner with K(i) values of 275 and 827 nM for the high- and low-affinity component, respectively. Fluorescence spectroscopy experiments showed that millimolar concentrations of l-Arg initially promoted and then inhibited the release of endogenous NO in cardiomyocytes. Likewise, l-Arg currents measured in cardiac myocytes voltage clamped in the presence of 460 nM free intracellular Ca(2+), a condition in which a Ca-CaM complex should activate endogenous NO production, showed fast activation followed by inhibition of l-Arg transport. The NOS inhibitor N-nitro-l-arginine methyl ester, but not blockers of downstream reactions, specifically removed this inhibitory component. These results demonstrate that NO acutely regulates its own biosynthesis by modulating the availability of l-Arg via cationic amino acid transporters.

Role of Nitric Oxide in Ginsenoside Rg1-Induced Protection against Left Ventricular Hypertrophy Produced by Abdominal Aorta Coarctation in Rats

Ginsenoside Rg(1) (Rg(1)), one of the active components of Panax ginseng, has been reported to promote endogenous nitric oxide (NO) production in some tissues, and to inhibit left ventricular (LV) hypertrophy in rats. This study aimed to investigate whether Rg(1)-induced inhibition of rat LV hypertrophy is mediated by NO-production. Rat LV hypertrophy was induced by abdominal aorta coarctation. Rg(1) 15 mg/kg/d, L-arginine 200 mg/kg/d, and the nitric oxide synthase (NOS) inhibitor N(G)-nitro-L-arginine-methyl ester (L-NAME) 100 mg/kg/d used with the same dose of L-arginine or Rg(1) were given starting from 1 d after surgery for 21 consecutive days. LV hypertrophy was evidenced by determining LV weight and mRNA expression of atrial natriuretic peptide, a marker of cardiac hypertrophic response, as well as by histopathology. Rg(1) and L-arginine administration significantly reduced the elevated LV hypertrophic parameters independent of LV systolic pressure changing, and ameliorated the histopathology of LV myocardium and LV diastolic function. All the beneficial effects of Rg(1) and L-arginine were abolished or blunted by L-NAME. Further to examine the role of NO in Rg(1) inhibition on LV hypertrophy, expression of endothelial NOS was determined at the transcript levels. In our experimental conditions endothelial NOS mRNA expression in LV tissue was lowered by abdominal aorta coarctation, and upregulated by Rg(1) administration. These results demonstrate that Rg(1)-induced protection against LV hypertrophy elicited by abdominal aorta coarctation in rats is mediated, at least in part, via endogenous NO production and release.

Phospholamban S-nitrosylation modulates Starling response in fish heart

The Frank-Starling mechanism is a fundamental property of the vertebrate heart, which allows the myocardium to respond to increased filling pressure with a more vigorous contraction of its lengthened fibres. In mammals, myocardial stretch increases cardiac nitric oxide (NO) release from both vascular endothelium and cardiomyocytes. This facilitates myocardial relaxation and ventricular diastolic distensibility, thus influencing the Frank-Starling mechanism. In the in vitro working heart of the eel Anguilla anguilla, we previously showed that an endogenous NO release affects the Frank-Starling response making the heart more sensitive to preload. Using the same bioassay, we now demonstrate that this effect is confirmed in the presence of the exogenous NO donor S-nitroso-N-acetyl penicillamine, is independent from endocardial endothelium and guanylate cyclase/cGMP/protein kinase G and cAMP/protein kinase A pathways, involves a PI(3)kinase-mediated activation of endothelial NO synthase and a modulation of the SR-CA(2+)ATPase (SERCA2a) pumps. Furthermore, we show that NO influences cardiac response to preload through S-nitrosylation of phospholamban and consequent activation of SERCA2a. This suggests that in the fish heart NO modulates the Frank-Starling response through a beat-to-beat regulation of calcium reuptake and thus of myocardial relaxation. We propose that this mechanism represents an important evolutionary step for the stretch-induced intrinsic regulation of the vertebrate heart, providing, at the same time, a stimulus for mammalian-oriented studies.

Modulation of GABA release by nitric oxide in the chick retina: Different effects of nitric oxide depending on the cell population

γ-Aminobutyric acid (GABA) is considered to be the most important inhibitory neurotransmitter in the central nervous system, including the retina. It has been shown that nitric oxide (NO) can influence the physiology of all retinal neuronal types, by mechanisms including modulation of GABA release. However, until now, there have been no data concerning the effects on endogenous GABA release of NO produced by cells in the intact retina. In the present study, we have investigated how NO production induced by drugs influences the release of endogenous GABA in cells of the intact retina of mature chicken. Retinas were exposed to different drugs that affect NO production, and GABA remaining in the tissue was detected by immunohistochemical procedures. A specific nNOS inhibitor (7-NI) reduced the number of GABA + amacrine cells and cells in the ganglion cell layer (GCL) by 33% and 41%, respectively. A GABA transporter inhibitor blocked this effect. L-arginine (100 μM), the precursor of NO, induced increases of 62% and 34% in the number of GABA + amacrine cells and GCL cells, respectively. A sodium (Na +)-free solution, 7-NI and a PKG inhibitor prevented the effect of L-arginine (100 μM). However, a higher concentration of L-arginine (1 mM) induced a 35% reduction in the number of GABA + cells by a Na +-dependent mechanism that was restricted to the GCL population. NMDA, which stimulates NO production, increased GABA release as indicated by 53% and 38% reductions in the number of GABA + amacrine cells and GCL cells, respectively. This effect was blocked by 7-NI only in GCL cells. We conclude that basal NO production and moderate NO production (possibly induced by L-arginine; 100 μM) inhibit basal GABA release from amacrine cells and GCL cells. However, NMDA or L-arginine (1 mM) induce a NO-dependent increase in GABA release in GCL cells, possibly by stimulating higher NO production.

Comparative Relaxing Effects of Sildenafil, Vardenafil, and Tadalafil in Human Corpus Cavernosum: Contribution of Endogenous Nitric Oxide Release

To compare the direct relaxant activity of sildenafil, vardenafil, and tadalafil in the human corpus cavernosum (HCC) and to investigate their modulatory effects on nitric oxide (NO)-mediated responses. Phosphodiesterase (PDE)-5 inhibitors cause cavernosal smooth muscle relaxation and penile erection. HCC strips were mounted in 10-mL organ baths containing Krebs solution and connected to force-displacement transducers. The changes in isometric force were recorded using the Powerlab 400 data acquisition system. Corporeal smooth muscle was precontracted submaximally with phenylephrine (10 micromol/L). All PDE-5 inhibitors tested (0.001-10 micromol/L) relaxed phenylephrine-precontracted HCC with similar values of potency in a concentration-dependent manner. However, the maximal relaxations induced by tadalafil (83% +/- 4%) were significantly lower compared with sildenafil (107% +/- 5%) and vardenafil (111% +/- 3%). The NO synthesis inhibitor N-nitro-l-arginine methyl ester (100 micromol/L) caused significant rightward shifts in the concentration-response curves for sildenafil (4.0-fold), vardenafil (4.6-fold), and tadalafil (3.2-fold) in HCC tissue. The guanylyl cyclase inhibitor 1H-[1,2,4]oxadiazolo[4,3-a]quinoxalin-1-one (10 micromol/L) also produced similar rightward shifts for these PDE-5 inhibitors. The cavernosal relaxations evoked by either acetylcholine or the NO donor glyceryl trinitrate were markedly potentiated by sildenafil, vardenafil, and tadalafil (0.1 micromol/L each). All PDE-5 inhibitors significantly increased the duration of electrical field stimulation-induced relaxations (8 Hz). Our findings have shown that sildenafil, vardenafil, and tadalafil relax HCC tissues in a concentration-dependent manner, but the maximal relaxation obtained with tadalafil was significantly lower than that obtained with sildenafil and vardenafil. Moreover, the PDE-5 inhibitors interacted with endogenous and exogenous NO, amplifying its HCC relaxation.

Exogenous nitric oxide regulates activity and synthesis of vascular endothelial nitric oxide synthase

Nitric oxide (NO) - a major signalling molecule of the vascular system - is constitutively produced in endothelial cells (EC) by the endothelial NO synthase (eNOS). Since a reduced NO synthesis is an early sign of endothelial dysfunction and NO delivering drugs are used to substitute the impaired endothelial NO production, we addressed the effect of exogenous NO on eNOS in human umbilical venous endothelial cell cultures. The synthetic NO donor DETA/NO (trade name, but in the following we refer to detNO), that releases NO in a strictly first order reaction with a half life of 20 h, was used in our experiments. Short-term (20-30 min) detNO treatment of EC increases the Ser(1177) phosphorylation of the constitutively expressed endothelial NOS and the production of endogenous NO generated by eNOS from [(3)H]arginine. The phosphorylation of eNOS is Akt-dependent and completely reverted by the phosphatidylinositol-3 kinase (PI-3K) inhibitor LY294002. A prolonged continuous exposure of EC to detNO 150 micromol L(-1) over a period of 24-48 h causes a reversible cell cycle arrest at G(1)-phase associated with a larger cell volume and increased cell protein content (hypertrophic phenotype of EC). The eNOS protein and mRNA of the hypertrophic cells and the generation of endogenous NO are reduced but eNOS phosphorylation could still be elevated by stimulation with vascular endothelial growth factor. Our data explain clinical studies describing a short-term but not a long-term benefit of NO treatment for patients with cardiovascular risk factors. The results could be a rational approach to develop a generation of NO donors accomplishing a retarded release from NO donors that mimic the low continuous pulsatile stress-induced release of endogenous NO.

Effects of Shock Waves on Tenocyte Proliferation and Extracellular Matrix Metabolism

The shock wave is an effective noninvasive modality for resolving various tendon pathologies. However, scientific rationale and mechanism of shock wave therapy remains limited. This study aims to investigate the effects of shock waves and their biochemical mechanisms on tenocyte proliferation and collagen synthesis. Tenocytes harvested from Achilles tendons of Sprague-Dawley rats were used in this study. Cell viability was assayed by trypan blue exclusion methods. The colorimetric assay was determined to evaluate the mitochondria activity of the tenocytes after shock wave exposure. Synthesis of collagen, nitric oxide (NO) and transforming growth factor-β1 (TGF-β1) were determined and their gene expression was also studied. The results showed that there was a dose-dependent impairment of cell viability observed in 0.36 mJ/mm 2 and 0.68 mJ/mm 2 stimulation. In the proliferation assay, low energy level with low impulses (0.36 mJ/mm 2 with 50 and 100 impulses) showed positive stimulatory effects, whereas the high energy level with high impulses (0.68 mJ/mm 2 with 250 and 500 impulses) had significant inhibitory effects. At 0.36 mJ/mm 2, 100 impulse shock waves treatment, up-regulation of proliferating cell nuclear antigen (PCNA) (at 6 and 24 h) and collagen type I, collagen type III and TGF-β1 gene expression (at 24 h) were observed; these were followed by the increases in NO production (at 24 h), TGF-β1 release (at 48 and 96 h) and collagen synthesis (at the 7th day). This study revealed that shock waves can stimulate tenocyte proliferation and collagen synthesis. The associated tenocyte proliferation is mediated by early up-regulation of PCNA and TGF-β1 gene expression, endogenous NO release and synthesis and TGF-β1 protein and then collagen synthesis. The spontaneous mechanical activity of the circular smooth muscle of the rabbit colon in vitro

The spontaneous mechanical activity of the proximal, middle and distal colon of the rabbit shows in vitro two types of contractions: phasic contractions with low amplitude and high frequency, giant contractions (GCs) with high amplitude and low frequency. Both patterns of contractions did not present differences according to the region. Investigations on the neural control of giant contractions in the middle colon gave the following results. (1) GCs are insensitive to muscarinic antagonism by atropine and ganglionic blockade by hexamethonium; (2) GCs are converted into phasic contractions following the inhibition of acetylcholinesterase by neostigmine, and are abolished for a short period by dimethyl-phenyl-piperazinium, a ganglionic nicotinic receptor agonist; (3) application of l-arginine, the substrate of nitric oxide (NO) synthase prolonged the duration of GCs without affecting their amplitude; sodium nitroprusside, a donor of NO, reduced both the amplitude and frequency of GCs; (4) inhibition of guanylate cyclase by methylene blue converted GCs into phasic contractions; (5) blockade of K + channels with the non-selective blocker, tetraethylammonium, or with the more selective apamin-sensitive Ca 2+-dependent K + channels blocker, dequaliniun, increased the resting tone and decreased the amplitude of contractions; whereas opening of ATP-sensitive K + channels by diazoxide abolished any rhythmic contractile activity. These data taken together suggest that the amplitude and frequency of GCs are controlled by the endogenous release of NO which activates guanylate cyclase, the subsequent formation of cGMP activates in turn the opening of Ca 2+-dependent K + channels. The cholinergic input seems to be responsible of the resting tone, and an increase of this tone is prone to impose the phasic contractions pattern to the tissue.

Endothelin Receptor Antagonists for Pulmonary Arterial Hypertension

The last decade has seen significant advances in the understanding and treatment of pulmonary arterial hypertension (PAH). Three main pathways, involving endothelin, nitric oxide, and prostacyclin, have been identified in its pathogenesis and these have all led to the development of therapies in current use. While the nitric oxide and prostacyclin pathways require augmentation, the endothelin system is overactive in PAH, with increased endothelin synthesis and receptor expression and, therefore, requires blockade. There are two known endothelin receptors. The type A receptor, expressed in pulmonary artery media, mediates vasoconstriction and remodeling, whereas the function of the type B receptor is more complex. Like the type A receptor, the type B receptor mediates vasoconstriction and remodeling effects when expressed on smooth muscle cells and (myo)fibroblasts, yet functions to clear endothelin from the circulation and induce release of endogenous nitric oxide and prostacyclin, when activated in the pulmonary artery endothelium. Consequently, it is not clear from in vitro data whether the optimal strategy is to block only the type A receptor or both receptors. Phase III clinical studies show clear short-term physiologic benefit with both dual and selective endothelin blockade in PAH. Longer-term experience with bosentan, a dual receptor antagonist, has shown improved outcomes compared with historic control data and comparable survival to intravenous prostacyclin therapy. The newer selective blockers, sitaxsentan and ambrisentan, appear to have similar short-term efficacy, but long-term data are as yet either lacking or unpublished. They may be less hepatotoxic than bosentan, although long-term follow-up of patients receiving bosentan has shown this is not a significant problem. On the basis of available evidence, the endothelin receptor antagonists have become first-line therapy for patients with PAH, except in the most severely affected who still require treatment with intravenous prostacyclin. Although their use as part of combination therapy with other agents is widespread, the evidence for this is not as robust, but appropriate investigation is underway.
