The article presents the results of the economic assessment of the digital health care market in the Russian Federation through interpretation of commercial relations in the field of telemedicine. The introduction of digital technologies is a fundamental development direction of all spheres of modern socio-economic system without any exception. The use of digital platform solutions actively affects health care system. The pandemic, which formed the objective imperatives of remote communications, became the trigger for intensification of digital transformations in health care. Telemedicine has become a significant manifestation of digital transformation of health care. The dominant part of its services is sold for money. In this regard, it is relevant to study the key dynamic and structural indicators of telemedicine market development, which will allow giving the answer about its economic capacity, investment attractiveness and potential commercial efficiency. The results of economic assessment of telemedicine market are important from the point of view of developing national mechanisms for its regulation and correction of competitive environment, since besides its commercial component there are also social effects that should be taken into account to manage the state of public opinion. The purpose of the study is to conduct economic assessment of the state and development of digital health care market in the Russian Federation. The research methodology was based on the use of the following methods: statistical, comparative, analytical, dialectical, graphical, and logical. The dynamics of cost capacity of telemedicine market, the volume of telemedicine services market per capita, the revenue of the leading companies in domestic telemedicine market, the level of investment activity, as well as the degree of popularity and economic accessibility of digital health care services were analyzed, as a part of economic assessment of digital health care market in Russia. The author's team reveals the essence, prerequisites and prospects of digital transformation of health care system, presents the results of analytical diagnostics of key dynamic and structural indicators of domestic telemedicine market functioning, and substantiates the potential of commercial development and investment attractiveness of the Russian digital health care market.

Regulation of Mesenchymal Stem Cell Morphology Using Hydrogel Substrates with Tunable Topography and Photoswitchable Stiffness.

Cell function can be directly influenced by the mechanical and structural properties of the extracellular environment. In particular, cell morphology and phenotype can be regulated via the modulation of both the stiffness and surface topography of cell culture substrates. Previous studies have highlighted the ability to design cell culture substrates to optimise cell function. Many such examples, however, employ photo-crosslinkable polymers with a terminal stiffness or surface profile. This study presents a system of polyacrylamide hydrogels, where the surface topography can be tailored and the matrix stiffness can be altered in situ with photoirradiation. The process allows for the temporal regulation of the extracellular environment. Specifically, the surface topography can be tailored via reticulation parameters to include creased features with control over the periodicity, length and branching. The matrix stiffness can also be dynamically tuned via exposure to an appropriate dosage and wavelength of light, thus, allowing for the temporal regulation of the extracellular environment. When cultured on the surface of the hydrogels, the morphology and alignment of immortalised human mesenchymal stem cells can be directly influenced through the tailoring of surface creases, while cell size can be altered via changes in matrix stiffness. This system offers a new platform to study cellular mechanosensing and the influence of extracellular cues on cell phenotype and function.

A Wireless, Regeneratable Cocaine Sensing Scheme Enabled by Allosteric Regulation of pH Sensitive Aptamers.

A key challenge for achieving continuous biosensing with existing technologies is the poor reusability of the biorecognition interface due to the difficulty in the dissociation of analytes from the bioreceptors upon surface saturation. In this work, we introduce a regeneratable biosensing scheme enabled by allosteric regulation of a re-engineered pH sensitive anti-cocaine aptamer. The aptamer can regain its affinity with target analytes due to proton-promoted duplex-to-triplex transition in DNA configuration followed by the release of adsorbed analytes. A Pd/PdHx electrode placed next to the sensor can enable the pH regulation of the local chemical environment via electrochemical reactions. Demonstration of a "flower-shaped", stretchable, and inductively coupled electronic system with sensing and energy harvesting capabilities provides a promising route to designing wireless devices in biointegrated forms. These advances have the potential for future development of electronic sensing platforms with on-chip regeneration capability for continuous, quantitative, and real-time monitoring of chemical and biological markers.

Harmful Information in Internet Media: The «Overton Window» and Destructive Network Content Interrelationships

The paper is devoted to the analysis of Internet media monitoring for the dissemination of destructive content and the comparison of the identified types of information of aggressive and (auto)destructive nature with legally fixed prohibitions and restrictions on the right to freedom of information dissemination. The author considers the issues of networking and decentralization of destructive communities in Runet and the development of radical movements using the Ingamasi technology. Taking into account the patterns of distribution of various types of destructive content revealed during the study, including the peculiarities of communication in network communities of different types, the refraction of the «Overton window» theory to the media field of the dissemination of destructive information in Internet media was scientifically justified.Despite the existence of a regulatory framework for ensuring the information security of the individual, the realities of the development of communication in the digital environment demonstrate the presence of many gray areas. New communication and content risks require a systemic transformation and updating of the legal regulation of the digital environment in order to ensure the information and ideological security of the individual. Given the trends in the spread of new types of aggressive and depressive-suicidal content discussed in the paper, the author concludes that it is necessary to normalize new types of prohibited and restricted information by making systemic changes to the Federal Law «On Information, Information Technologies and Information Protection», the Federal Law «On the Protection of Children from Information that causes Harm their health and development», the Law of the Russian Federation «On Mass Media», as well as the improvement of administrative and criminal legislation, including through the introduction of new elements of offenses (crimes).

Silica islands regulated external Ti‐site environment of TS‐1 for enhanced performance of 1‐hexene epoxidation

Abstract Rational regulation of the local environment of Ti‐sites in TS‐1 harbors tremendous industrial and scientific significance to epoxidation reactions. Herein, we report a facile and environment‐friendly strategy to boost catalytic selectivity by covering the Ti‐sites on an external surface of TS‐1 (ITS‐1) without sacrificing activity of internal Ti sites. By a quantitative analysis of d 3 ‐acetonitrile and quinoline‐DRIFTS, 1 H MAS NMR and XPS, it is found that the percentage of external Ti‐sites decreased from 11% to 6% after depositing surface silica islands. This successfully inhibits ring‐opening reaction of 1,2‐epoxyhexane on catalyst surface, as demonstrated by slower kinetic decomposition rate of 1,2‐epoxyhexane. Compared with conventional TS‐1 catalyst, selectivity of 1,2‐epoxyhexane over ITS‐1 catalyst significantly increased from 83.5% to 98.5% while maintaining high 1‐hexene conversion. Furthermore, overmuch surface silica coverage only leads to extremely low conversion (2%) due to inhibition of mass transfer. This work paves the way for rational construction of Ti‐containing catalysts for 1‐hexene epoxidation.

The ability to regulate the gut environment has resulted in remarkable great breakthroughs in the treatment of several diseases. Several studies have found that the regulation of the gut environment might provide relief from the symptoms of benign prostatic hyperplasia. However, the correlation between the gut microenvironment and the colon and prostate glands is still unknown. We found that ulcerative colitis (UC) induced an increase in prostate volumes that could be reversed by sodium butyrate (NaB) and fecal microbiota transplantation (FMT). The mechanism by which UC induced changes in the prostate gland was examined via RNA-Seq. The results show that the expression level of GPER was significantly lower in the prostate gland of UC mices than in normal mices. The expression of GPER could be increased via treatment with NaB or FMT. We found that prostate tissues exhibited higher butryic acid levels after they were treated with NaB or FMT. In experiments conducted in vitro, NaB or the fecal filtrate (FF) from healthy mice up-regulated of the expression of GPER, inhibited cell growth, and induced apoptosis in BPH-1 cells. These changes could be alleviated by treatment with the G15 or in GPER-silenced cells.

The accurate design and systematic engineering of MOFs is a large challenge due to the randomness of the synthesis process. Isoreticular chemistry provides a powerful approach for the regulation of pore environment in a more predictable and precise way to systematically control gas/vapor adsorption performances. Herein, utilizing an effective strategy of altering the "pillared" motifs of pillared layer structures, three isoreticular ultramicroporous MOFs were successfully constructed. Combined with the reported parent MOFs and two other recorded isoreticular MOFs modified with -NH2 and -CH3, gas and vapor uptake performances of this family of isoreticular pillared layer MOFs were systematically explored.

Policy of the transitıon from agricultural to high-tech export in conditions of permanent military threat: Israeli experience for Ukraine

The Russian-Ukrainian war causes the loss of a significant part of Ukraine's production and raw and food export potential, which creates the risk of economic insecurity in the long run. At the same time, the new challenges create favourable conditions and opportunities for restructuring the national economy in the direction of developing high-tech areas and increasing the science-intensiveness of traditional industries. Today, the question is to choose the most effective institutional and economic mechanism for such a transformation, as exemplified by the State of Israel. The article is devoted to the research of the Israeli practice of transition to high-tech production and the formation of an innovative export-oriented economy. The author aims to clarify the conditions, factors, mechanisms, and tools for the successful economic transformation of Israel in terms of the possibility of implementing the acquired historical experience in Ukrainian realities. The methodology is based on a systematic approach, which considers the economic transformation of Israel as a set of processes and systems of closely interrelated elements (historical conditions, institutional factors, public policy, available resources, economic potential, etc.). It is necessary to apply the historical-evolutionary approach to identify the features and trends of the country's transition from traditional spheres of production and export to high-tech. In this context, comparative-historical analyse is used. The precondition for the transition of the State of Israel to high-tech production and exports was the development of powerful science-intensive complexes of its own agricultural and defence production through targeted public and private investment, subsidies and loans to producers, stimulating education and research, a unique system of public-private partnership (so-called "gold triangle"). It is determined that the use of defence technologies in production of the double-purpose products (in space, aviation, cybersecurity spheres), the flow of intellectual capital, and the formed research base became the basis for the development of pharmaceuticals, electronics, microelectronics, computer hardware, and software, etc. The key role in the Israeli transition to high-tech production and exports was played by state support policy implemented by grant funding instruments for education and R&D, venture capital incentive programs, tax and credit benefits for small and medium businesses, an extensive system of institutional assistance, and effective legal regulation of the business environment for the protection of intellectual property rights, granting special privileges, simplifying the procedure of repatriation of profits, ensuring high-quality products, etc. The policy of "arms diplomacy", trade liberalization, tax, credit, and institutional support of exporters, the conclusion of a system of agreements on international trade, and economic and scientific-technical cooperation contributed to the formation of the competitive advantages of Israel in the markets of high-tech products

Engineering the Coordination Interface of Isolated Co Atomic Sites Anchored on N-Doped Carbon for Effective Hydrogen Evolution Reaction.

The regulation of the coordination environment of the central metal atom is considered as an alternative way to enhance the performance of single-atom catalysts (SACs). Herein, we design an electrocatalyst with active sites of isolated Co atoms coordinated with four sulfur atoms supported on N-doped carbon frameworks (Co1-S4/NC), confirmed by high-angle annular dark-field scanning transmission electron microscope (HADDF-STEM) and synchrotron-radiation-based X-ray absorption fine structure (XAFS) spectroscopy. The Co1-S4/NC possesses higher hydrogen evolution reaction (HER) catalytic activity than other Co species and exceptional stability, which exhibits a small Tafel slope of 60 mV dec-1 and a low overpotential of 114 mV at 10 mA cm-2 during the HER in 0.5 M H2SO4 solution. Furthermore, through in situ X-ray absorption spectrum tests and density functional theory (DFT) calculations, we reveal the catalytic mechanism of Co1-S4 moieties and find that the increasing number of sulfur atoms in the Co coordination environment leads to a substantial reduction of the theoretical HER overpotential. This work may point a new direction for the synthesis, performance regulation, and practical application of single-metal-atom catalysts.

Impact of gut microbiome on dyslipidemia in japanese adults: Assessment of the Shika-machi super preventive health examination results for causal inference.

Dyslipidemia (DL) is one of the most common lifestyle-related diseases. There are few reports showing the causal relationship between gut microbiota (GM) and DL. In the present study, we used a linear non-Gaussian acyclic model (LiNGAM) to evaluate the causal relationship between GM and DL. A total of 79 men and 82 women aged 40 years or older living in Shika-machi, Ishikawa Prefecture, Japan were included in the analysis, and their clinical information was investigated. DNA extracted from the GM was processed to sequence the 16S rRNA gene using next-generation sequencing. Participants were divided into four groups based on sex and lipid profile information. The results of one-way analysis of covariance, linear discriminant analysis effect size, and least absolute value reduction and selection operator logistic regression model indicated that several bacteria between men and women may be associated with DL. The LiNGAM showed a presumed causal relationship between different bacteria and lipid profiles in men and women. In men, Prevotella 9 and Bacteroides were shown to be potentially associated with changes in low- and high-density lipoprotein cholesterol levels. In women, the LiNGAM results showed two bacteria, Akkermansia and Escherichia/Shigella, had a presumptive causal relationship with lipid profiles. These results may provide a new sex-based strategy to reduce the risk of developing DL and to treat DL through the regulation of the intestinal environment using specific GM.

In the era of the COVID-19 Pandemic, medical waste is increasing in various hospitals, including UNS Hospital. The variables studied in the hazardous and toxic solid medical waste flow treatment. Primary data was obtained from an analysis of waste amount in 2021 and interviews with incinerator managers. Secondary data in the form of waste types, impacts, and incinerator residue were obtained from the literature study. This research aims to identify solid medical waste management with hazardous and toxic materials compared with Regulation of Environment and Forestry Minister of Republic Indonesia 56/2015. After analysis, it was found that there is dangerous and harmful solid medical waste flow management, distinguished by type. Particular colored medical waste is managed by reducing, sorting, storing, transporting, and destroying. Destruction is conducted with an incinerator. Arah Environmental Indonesia Company operates infectious medical waste. The potential impact of that waste can attack health, damage the environment quality, increase the degradation that has occurred, and pose a threat to microplastic. Completing waste management is done by tightening hospital regulations to produce output according to quality standards, limiting hazardous and toxic waste use, and selecting the waste.

Recent Advances in Targeted Nanotherapies for Ischemic Stroke

Ischemic stroke (IS) is a severe neurological disease caused by the narrowing or occlusion of cerebral blood vessels and is known for high morbidity, disability, and mortality rates. Clinically available treatments of stroke include the surgical removal of the thrombus and thrombolysis with tissue fibrinogen activator. Pharmaceuticals targeting IS are uncommon, and the development of new therapies is hindered by the low bioavailability and stability of many drugs. Nanomedicine provides new opportunities for the development of novel neuroprotective and thrombolytic strategies for the diagnosis and treatment of IS. Numerous nanotherapeutics with different physicochemical properties are currently being developed to facilitate drug delivery by accumulation and controlled release and to improve their restorative properties. In this review, we discuss recent developments in IS therapy, including assisted drug delivery and targeting, neuroprotection through regulation of the neuron environment, and sources of endogenous biomimetic specific targeting. In addition, we discuss the role and neurotoxic effects of inorganic metal nanoparticles in IS therapy. This study provides a theoretical basis for the utilization of nano-IS therapies that may contribute to the development of new strategies for a range of embolic diseases.
