Abstract Background In inflammatory bowel disease (IBD), an acute clinical flare may be due to either disease exacerbation or an enteric infection. Thus, it is recommended to exclude infections before escalating IBD therapy. In this setting, conventional tests identify only a minimal proportion of infections (i.e. 10%). Whether nucleic acid multiplex tests can expand the range of detected pathogens and increase the overall prevalence is still a matter of debate. Methods We performed a single-center transversal prospective study at Fondazione IRCCS Policlinico Ca’ Granda. Stool samples from consecutive ulcerative colitis (UC) patients experiencing a disease flare and a group of patients in clinical remission were enrolled between February and October 2023 and tested with QIAstat-Dx Gastrointestinal Panel 2 (Qiagen GmbH, Hilden, Germany) for simultaneous qualitative detection and identification of nucleic acids from multiple viruses, bacteria, and parasites. Clinically active disease was defined as a Partial Mayo Score (PMS) of ≥2. Results Overall, 85 UC patients were enrolled. Demographics and clinical features are displayed in Table 1. Median age was 42 (18-79), 52% were males. The two most common treatment regimens were mesalamine (54%) and biologic/small molecule treatment (35%). 66 (78%) patients were clinically active, while 19 (22%) were in remission. In the active group, median bowel movements were 5 (3-15), 58 (88%) patients had loose stool, 32 (49%) reported abdominal pain, 40 (61%) urgency, 10 (15%) overnight symptoms, 6 (9%) fever. Median PMS was 5 (2-8). Median hemoglobin was 13.8 (8.2-16.4) g/dL, fecal calprotectin 508 (<5-4980) ug/g. Prevalence of gastrointestinal infections with QIAstat-Dx was 30 vs. 10% (p=0.13) in active vs. remission group, respectively. The most common infections were: C.Difficile infection (CDI) (4 cases), Cytomegalovirus (CMV) (4 cases) and enteropathogenic E.Coli (EPEC) (4 cases). Regarding viral and E. Coli infections, clinical remission (PMS <2) occurred spontaneously. As for CMV, Campylobacter spp., and CDI, all patients except one achieved remission following specific antimicrobial treatment. In one patient CMV infection led to hospital admission (circulating CMV DNA 35,395 IU/mL). The remaining patient did not receive specific antiviral therapy since sigmoidoscopy only detected isolated CMV+ cells, and remission was achieved with UC treatment escalation. Conclusion In UC patients experiencing a clinical flare, prevalence of gastrointestinal infections with QIAstat-Dx® Gastrointestinal Panel 2 is numerically higher than patients in remission. Further studies are needed to investigate whether increased detection could influence the clinical outcomes of IBD patients and time and costs for the healthcare system.

Associations between gastrointestinal infection and urinary phthalate metabolite concentrations in US children and adolescents from NHANES 2005-2016.

The purpose of this study was to evaluate at the link between gastrointestinal illness and urine phthalate metabolite concentrations in children and adolescents in the United States between 2005 and 2016. A total of 4008 National Health and Nutrition Examination Survey (NHANES) participants had urine samples obtained during the survey and self-reported their gastrointestinal functional status over the previous week. High performance liquid chromatography/tandem mass spectrometry (HPLC/MS-MS) was used to identify twelve phthalate metabolites in urine samples. The link between PAE concentrations and gastrointestinal illnesses was investigated using logistic regression, which was controlled for possible confounders. The combined and independent effects of PAEs on gastrointestinal illnesses were investigated using Bayesian Kernel Machine Regression (BKMR) and quantile-based g-computation (qgcomp). In children and adolescents, the prevalence of gastrointestinal infection was 9.0%. One log-unit increase in urinary concentrations was associated with an increased risk of gastrointestinal infection for monocarboxyoctyl phthalate (MCOP) (adjusted odd ratio (aOR) = 1.36, 95 percent confidence interval (95%ci): 1.08, 1.62), mono(2-ethylhexyl) phthalate (MEHP) (aOR = 1.18, 95 percent CI: 1.05, 1.32) and mono(2-eth The mixed exposure model findings revealed that the combined effect of PAEs was substantially linked with gastrointestinal infection; exposure to the combination of PAEs was positively associated with the risk of gastrointestinal infection. In the BKMR model, the exposure to the mixture of PAEs was positively associated with the risk of gastrointestinal infection. In qgcomp, a substantial positive correlation between PAEs and gastrointestinal illnesses was identified (OR = 1.16, 95 percent CI: 1.05, 1.28). MCOP and MEHP may be the major contributors after controlling for other PAE homologs. These associations were more pronounced in overweight and obese children and adolescents. Mixed exposure to phthalates (PAEs) in children and adolescents was significantly associated with gastrointestinal infections, with MCOP and MEHP accounting for the major proportions. These associations were more pronounced in overweight and obese children and adolescents.

Gastrointestinal helminth and protozoan infections of wild mammals in four major national parks in Sri Lanka

A cross-sectional, coprological survey of gastrointestinal (GI) parasites of wild mammals in four major National Parks in Sri Lanka: Wilpattu, Udawalawe, Wasgamuwa, and Horton Plains was carried out during November 2016 to August 2017. Fresh fecal samples were collected and analyzed using sedimentation technique, iodine & saline smears, and Sheather’s sucrose flotation for morphological identification parasite eggs, cysts, and larvae. A modified salt flotation was carried out for egg counts. Seventy samples from 10 mammal species: Asian Elephant, Spotted Deer, Water Buffalo, Sambar, Indian Hare, Asian Palm Civet, Sloth Bear, Wild Boar, Grey Langur, Leopard, and four unknown mammals (two carnivores, one herbivore and one omnivore) were analyzed. Most were infected (94.3%) with more than one GI parasites. The highest prevalence of infection was recorded in Horton Plains (100%), followed by Wasgamuwa (92.8%), Wilpattu (90.4%) and Udawalawe (75.0%) with a significant difference among four parks (Chi square test; χ2=35.435; df=3; p<0.001). Nineteen species of GI parasites were recorded, of which Entamoeba, Isospora, Balantidium, Fasciola, Moniezia, Dipylidium, strongyles, Toxocara, Trichiurus and hookworms were the most common. Strongyles (62.1%) and Entamoeba (80.3%) were the most prevalent helminth and protozoan infections, respectively. Overall, there was no difference in the prevalence of protozoans (84.3%) and helminths (87.1%; χ2=1.0; df=1; p=0.317). In carnivores, Entamoeba, Balantidium, Moniezia, strongyles and Strongyloides were common and in herbivores, Entamoeba, strongyles, Strongyloides and Toxocara were common. The quantitative analysis showed strongyles (17.639 EPG) and Isospora (18,743 OPG) having the highest infection intensity among helminthes and protozoans, respectively. This study provides baseline information of GI parasites and their distribution in wild mammals in the four national parks. Although the prevalence of GI infections was high, their intensity shows that they could be incidental infections. When the prevalence of an infection is high but the intensity is low, it is unlikely to be a major health problem leading to the endangerment of a species. Parasitic diseases can not only affect conservation efforts, but they are also natural selection agents and drive biological diversification, through influencing host reproductive isolation and speciation.

Prevalence of gastrointestinal parasitic infections in sheep of a coastal belt under Noakhali district, Bangladesh

Gastrointestinal (GI) parasitic infection is considered one of the major concerns in sheep production in Bangladesh. The current study was undertaken to determine the GI parasitic infections in sheep reared in bathan area and semi-intensive conditions of six (6) randomly selected areas of Subarnachar upazila under Noakhali costal belt. A total of 200 fecal samples were collected and examined to detect the ova of GI parasites using routine coproscopical tests (e.g. direct smear, sedimentation and flotation). Results demonstrated that the overall prevalence of GI infections in sheep was found 79.50%, where infections caused by trematodes were the highest in comparison to cestode and nematodes. The frequency of GI parasites were more common in all the study areas where the higher prevalence was recorded in Char Clark, Char Jabbar and Char Bata which ranged around 82-84% compared to other areas. Further, sheep reared in bathan areas (82.66%) were found to be more susceptible to GI parasitic infections compared to sheep kept on semi-intensive condition (77.60%). Furthermore, the occurrence of GI parasites was significantly higher in female sheep compared to male. The present study documented the occurrence of GI parasites which might reduce the production performance of the animals of that coastal belt. We recommend further extensive study for molecular detection of such parasitic infections including growing awareness among sheep owners to prevent and control of those infections.

Intestinal parasites in the slum-dwelling population in Naya Bazar, Kaski, Nepal

Background and Objectives: Gastrointestinal infection is a major cause of diarrhea, anemia, malnutrition and responsible for reducing physical and mental development especially in children. This study aimed to investigate the prevalence of the intestinal pathogens in the slum-dwelling population in Kaski, Nepal.Material and Methods: A cross-sectional study was conducted in August-September 2012 in slum-dwelling community. Overall 166 human faecal samples were collected and examined using formal ether sedimentation method.Results: The prevalence of gastrointestinal infections in slum-dwelling population was found to be 24.1%. The magnitude of infection was higher (26.9%) among females compared to males. The difference was not statistically significant (P > 0.05). The prevalence of infection was higher (37.0%, P > 0.05) in Dalit group. Children (≤10 years) had higher rate of infection than older people. 22% subjects had single and 2.4% had multiple infections. In the overall population, 12.7% had G. lamblia followed by 5.4% Ascaris lumbricoides, 4.8% Hymenolepsis nana and 3.6% Trichuris trichuria. Conclusion: Gastrointestinal infections were common in the slum-dwelling populations. Lack of health education and safe drinking water contributed higher infection rate in the community. Increased exposure time to the contaminated water and gender disparity had influenced the rate of infection. Due to the semi-urban area with absence of moist soil, protozoan infection was prevalent than helminth infections.

Gastrointestinal parasites of domesticated and Feral Horses (Equus Caballus) in Sri Lanka

Horses are not native to Sri Lanka. They are imported from neighbouring countries to be used for police work, sporting or riding and esthetic purposes. An isolated population of about 500 feral horses lives in the Delft Island in northern Sri Lanka. These feral horses served hundred years for western conquests and when they left Sri Lanka, horses were left behind in the Delft Island. There are no records of gastrointestinal (GI) parasites of horses in Sri Lanka and the present study was carried out to determine the GI parasites in the domesticated (free grazing and stabled) and feral horses. Fresh faecal samples were collected from horses and analysed qualitatively and quantitatively to determine the types of GI parasites and their prevalence and intensity. Qualitative analyses using direct saline and iodine mounts, simple test tube flotation, Sheather’s modified sucrose flotation and sedimentation technique were carried out followed by McMaster counting technique for the positive samples. Identification of parasites involved morphological, morphometric and molecular methods. A total of 73 horses were sampled. All the feral horses (n=14) were infected. Among the domesticated horses, free grazers had a significantly higher prevalence of GI infections (46.7%) than the stabled ones (18.2%; Chi square test χ 2 = 4.787, df = 1, p = 0.029). There was no significant difference between helminth and protozoan infections among the three groups of horses (Chi square test, χ 2 = 1.453, df = 2, p = 0.484). Six species of parasites: Anoplocephala sp., Parascaris equorum , Strongylus sp. Isospora sp. Entamoeba sp. and Giardia sp. were recorded. Strongylus infections were recorded in all the feral horses with a high intensity but not in any of the domesticated animals. Strongylus is a highly pathogenic and the most damaging parasite of horses worldwide. It is important to investigate whether Strongylus infection is responsible for the high mortality recorded in the feral horses in the Delft Island especially because the Department of Wildlife Conservation is to declare Delft Island a National Park to conserve the feral horses.

Interactions between gastrointestinal parasitism and pneumonia in Nigerian goats.

This study was carried out to determine the burden and interactions between parasitism and pneumonia in Nigerian goats. A total of 430 goats were examined for gastrointestinal (GIT) parasites by faecal analysis using sedimentation by gravity, Modified Winconsin sugar floatation method, McMaster egg counting techniques and for lung consolidation, pathology and morphometry of pneumonia were evaluated by standard techniques. The results from the faecal examination revealed the presence of nine (9) gastrointestinal parasites namely Eimeria, Trichuris, Oesophagostomum, Strongyloides, Moniezia, Haemonchus, Bunostomum, Trichostrongylus, and Nematodirus species and at an overall high prevalence of 74%. Eimeria species was found to have the highest prevalence of 26% for single infection, while co-infection of Eimeria and Trichuris species 21% was highest amongst other parasites, and of the breeds of goats examined for GIT parasites, the Red Sokoto (RS) breed had the highest infection rate 37%. The results from the lungs analysis showed 59% prevalence with the highest consolidation in RS (48.2%), followed by WAD (34.2%). The age of the goats was key in the infection rate of both parasites and pneumonia, which was seen to be highest in younger goats, with a significant (2.10±0.4×103) GI parasite burden in >1-2years group. The prevalence of GIT infection with lung consolidation in the goats, was observed to influence their Body condition scores (BCS) (p<0.05). This study showed that body condition score, age and breed of goats may be predisposing factors to gastrointestinal parasites and pneumonia in Nigerian goats. There was a strong correlation (+0.243, p=0.00) of occurrence of gastrointestinal parasitism and pneumonia in this study. Possibility of combined prophylactic measures should be considered in the nearest future.

Relationship between socioeconomic status and gastrointestinal infections in developed countries: a systematic review protocol.

BackgroundThe association between low socioeconomic status (SES) and poor health is well documented in the existing literature. Nonetheless, evidence on the relationship between SES and gastrointestinal (GI) infections is limited, and the mechanisms underlying this relationship are not well understood with published studies pointing to conflicting results. This review aims to identify studies that investigate the relationship between SES and GI infections in developed countries, in order to assess the direction of the association and explore possible explanations for any differences in the risk, incidence or prevalence of GI infections across socioeconomic groups.MethodsThree systematic methods will be used to identify relevant literature: electronic database, reference list and grey literature searching. The databases MEDLINE, Scopus and Web of Science Core Collection will be searched using a broad range of search terms. Screening of the results will be performed by two reviewers using pre-defined inclusion and exclusion criteria. The reference lists of included studies will be searched, and Google will be used to identify grey literature. Observational studies reporting quantitative results on the prevalence or incidence of any symptomatic GI infections by SES, in a representative population sample from a member country of the Organisation for Economic Co-operation and Development (OECD), will be included. Data will be extracted using a standardised form. Study quality will be assessed using the Liverpool University Quality Assessment Tools (LQAT). A narrative synthesis will be performed including tabulation of studies for comparison.DiscussionThis systematic review will consolidate the existing knowledge on the relationship between SES and GI infections. The results will help to identify gaps in the literature and will therefore provide an evidence base for future empirical studies to deepen the understanding of the relationship, including effective study design and appropriate data analysis methods. Gastrointestinal Helminth Parasites of the fish Synodontis clarias (Siluriformes: Mochokidae) from Lekki lagoon, Lagos, Nigeria

A total of 362 specimens of the fish Synodontis clarias were randomly selected and subjected to parasitological examination for helminth parasites. They were collected over a period of one year from Lekki lagoon, Lagos, Nigeria. The prevalence of gastrointestinal infection was 38.7%. The helminth worms found include two cestodes, Proteocephalus spp., Wenyonia acuminata, and a nematode species, Raphidascaroides. Male specimens (196) presented a higher rate of infection (37.8%) than female specimens (166) which showed a rate of 23.5%. The overall worm burden was high (678) and it was independent of fish sex and fish size.

Prevalence of Gastrointestinal Infection among International Travellers Returning to Canada

To describe the prevalence of parasitic and bacterial gastrointestinal infection (excluding enterotoxigenic Escherichia coli) among international travellers attending the International Travel Clinic at The University of Calgary. Data were abstracted from the records of the first visit after travel of all persons making a post travel visit between January 1, 1986 and March 31, 1990. Data were available for 886 first visits (840 persons). Stools were submitted by 692 travellers. The frequency of stool submission varied by the duration of travel abroad, and the frequency of diarrhea either during or after the trip was greater among those who had submitted a stool specimen. The prevalence of stools positive for ova, parasites or pathogenic bacteria was 41.2%. When only pathogenic organisms were considered, the prevalence of infection was 19.4%. The most commonly isolated pathogenic parasites were Dientamoeba fragilis, Giardia lamblia, and Entamoeba histolytica. The most commonly isolated bacteria were Campylobacter species and Salmonella species. Although the prevalence of positive stool screens among returned travellers in this population was high, only about one-fifth of persons tested were positive for pathogens.
