The condition of periodontal tissues and selected microorganisms presence around implants and natural teeth

Abstract Introduction Periodontitis is a disease of the tissues surrounding the tooth, leading to tooth loss. Missing teeth are often restored with fixed prostheses supported by implants. The aim of this study was to analyze subgingival microbiota in periodontal pockets and around implants, as well as to assess the correlation of periodontal parameters and risk factors for periodontitis with the presence of selected bacteria. Materials and Methods The study included 33 patients of both sexes, aged 39 to 79 years, having their own dentition and intraosseous implants with healthy periodontium or reversible gingivitis. The patients underwent periodontal examination and bacteriological analysis with the use of a PET RT-PCR kit (MIP Pharma, Germany). Results Less plaque was observed on implant surfaces than around natural teeth (28.7% vs. 39.8%). On the other hand, significantly deeper pockets (2.9 vs. 2.3 mm), and a slightly higher gingival bleeding index (27.9 vs. 22.6%) were noted around implants. Conclusions There were no significant differences in the overall presence and number of studied pathogens between the natural teeth and implants. In the case of natural teeth, the presence of Porphyromonas gingivalis, Treponema denticola, Tannerella forsythia and Prevotella intermedia showed correlation with clinical status: the amount of plaque (Td, Tf), bleeding (Td, Tf, Pi) and probing depth (Pg, Td, Tf, Pi). Such relationships were not observed in the case of implants. Bacteria from periodontal pockets also demonstrated more correlation with the general condition compared to bacteria residing in tissues around implants.

Porphyromonas Gingivalis in Different Peri-Implant Conditions: a Pilot Cross - Sectional Study.

To assess the potential correlation between clinical peri-implant parameters and the presence of Porphyromonas gingivalis in different peri-implant conditions. The study included 30 patients from the Department of Periodontology and Oral Medicine divided into three equal groups, defined according to the following diagnoses: peri-implantitis (PI), peri-implant mucositis (PM), and peri-implant health (HI). Clinical parameters such as peri-implant probing depth (PPD), bleeding on probing (BOP), suppuration on probing (SUP), and plaque index (PI) were recorded. The samples of peri-implant crevicular fluid were collected and relative levels of Porphyromonas gingivalis were analyzed using Real-Time PCR (Reverse transcriptase - real-time polymerase chain reaction). The mean patient age was 51.33 ± 12.61 years. The mean value for Porphyromonas ginigvalis relative level was higher in the PI group (14.80 ±31.51) compared to PM and HI group (0.48 ± 1.34 and 0.06 ± 0.12, respectively). This parameter significantly differed between PI and HI (p=0.012), as well as PI and PM (p=0.049). The mean whole mouth probing pocket depth (PPD) in the PI group (4.02mm ± 0.77mm) was also significantly greater compared to the other two groups (3.39mm ± 0.52mm in PM and 1.97mm ± 0.54mm in the HI group. The SUP in the PI (mean value of 18%) group was more frequent than in PM (mean value of 14%) while the SUP was not detected in the HI group. The correlation between clinical parameters and relative levels of Porphyromonas gingivalis was not significant. The findings of this research indicate higher relative levels of Porphyromonas gingivalis in peri-implant lesions, especially in peri-implantitis.

Microbial Diversity in the Peri-Implant Region of Patients with History of Periodontal Disease and Clinical Parameters

Although implant rehabilitations are successful, a small portion of patients may present peri-implant mucositis and peri-implantitis due to the bacteria biofilm formation. The purpose of the present study was to evaluate the presence of Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn) and Scardovia wiggsiae (Sw) in Peri-implant gingival Fluid (PGF) of patients who received implants 12 months ago and associated with periodontitis (PD) history and clinical data. Samples were collected from 44 volunteers and analyzed by PCR with specific primers. The results showed that Pg were detected in 34%, Fn and Pi, 81.8%, and Sw 75% of the volunteers. Pg was frequently detected in patients who had a history of PD (p<0.05, q=8.19), but no statistically significant differences for Fn, Pi, and Sw (p>0.05, q<0.61). Pg was detected in 58.3% of the volunteers who had teeth prior to implants and their presence was associated with periodontitis (p<0.05, q=5.31) and gingivitis (p<0.05, q=4.31). All the patients whose probing depths were greater than 3 mm had Fn in detectable levels (p<0.05, q=4.56). In conclusion, the involvement of Sw in peri-implant alterations was not evident in this work. The presence of Pi and Fn were not related to history of PD and mucositis. Fn was present in periodontal pockets above 3 mm, suggesting its involvement in peri-implant alterations. Pg was detected in patients with a history of PD, gingivitis, and periodontitis and in those who had teeth before the implant installation.

Systemic azithromycin as an adjunct to scaling and root planing in patients with stage III/IV periodontitis: 12-month results of a randomized controlled clinical trial.

To determine whether azithromycin (AZI) as an adjunct to scaling and root planing (SRP), when compared to placebo, decreases the number of sites demonstrating pocket depth (PD) ≥ 5 mm and bleeding on probing (BOP) 12 months post-treatment in stage III/IV periodontitis patients. In a double-blind randomized parallel-arm placebo-controlled trial, 40 stage III/IV periodontitis patients received steps 1 and 2 of periodontal treatment in two sessions within 7 days. Patients then received systemic antibiotic therapy (n = 20; AZI 500 mg/day, 3 days) or placebo (n = 20). Additional instrumentation of residual diseased sites (DS)-sites with PD ≥ 5 mm and BOP-was performed at the 3-, 6- and 9-month follow-ups. The primary outcome variable was the number of DS at the 12-month re-evaluation. Using a multivariate multilevel logistic regression model, the effects of gender, age, antibiotic therapy, presence of Porphyromonas gingivalis or Aggregatibacter actinomycetemcomitans, smoking, tooth being a molar and interdental location were evaluated. The number of DS after 12 months was similar in the test (median (Me) = 4, interquartile range (IQR) = 0-6) and control (Me = 3, IQR = 1-6.5) groups. Both groups showed substantial but equivalent improvements in periodontal parameters, with no intergroup differences at initially shallow or deep sites. The logistic regression showed a lower odds ratio (OR) for the healing of DS on molars (OR = 0.29; p < 0.001) and in smokers (OR = 0.36; p = 0.048). Stage III/IV periodontitis patients showed significant but comparable improvements in periodontal parameters and the number of residual DS at the 12-month revaluation regardless of treatment type. This may have been the result of the additional instrumentation received by patients at residual DS in both treatment groups. Treatment with AZI + SRP provided no additional benefits after 12 months in terms of periodontal parameters or the number of persisting sites with PD ≥ 5 mm + BOP as compared to SRP plus placebo. Anti-carbamylated protein and peptide antibodies as potential inflammatory joint biomarkers in the relatives of rheumatoid arthritis patients.

Antibodies against carbamylated proteins/peptide (CarP) have been associated with severity in rheumatoid arthritis (RA) patients. However, their role in risk groups, specific targets and relation with periodontal disease (PD) is uncertain yet. The aim of this study was evaluated the association between the levels of anti-CarP with clinical manifestation, human leukocyte antigen (HLA) alleles, periodontal activity markers, PD diagnosis, PD severity, and presence of Porphyromonas gingivalis (Pgingivalis) in relatives of patients with RA. One hundred and twenty-four individuals with a family history of RA in first-degree relatives (FDR) and 124 healthy individuals gender- and age-matched, RA activity was assessed. Antibodies against carbamylated protein anti-FCS-Carp and 2 carbamylated peptides of fibrinogen were selected (anti-Ca-Fib2, anti-Ca-Fib3). Anti-FCS-Carp-positive, anti-Ca-Fib2 and anti-Ca-Fib3 were more frequent in FDR than controls (25.0% vs 14.5%, 34.7% vs 15.3% and 33.1% vs 11.3%, respectively). Anti-FCS-CarP were associated with the HLA-DRB1-SE* 1402 allele (P = .035) and highly sensitive C-reactive protein levels (P = .016), the anti-Ca-Fib2 antibodies were associated with the HLA-DRB1-SE* 1501 allele (P = .03), with non-SE* 0901 allele (P = .01), the anti-Ca-Fib3 was associated with positive rheumatoid factor (P = .0012). The FDR condition was associated with the presence of anti-Ca-Fib3 (odds ratio [OR] =4.7; 95% CI=1.8-11.7; P = .001) and painful joints (OR=2.2; 95% CI=1.01-4.68; P = .045); we also detected an important trend toward the presence of P gingivalis (OR=1.9; 95% CI=0.9-3.7; P = .062). The presence of anti-FCS-Carp, anti-Ca-Fib3 and anti-Ca-Fib2 antibodies may have a role for these antibodies as early biomarkers in the development of RA, probably including additional mechanisms related with other non-SE alleles; the anti-peptide antibodies proposed in the present study may represent a simpler way to identify antibodies directed to a specific target.

Platelet plug formation in whole blood is enhanced in the presence of Porphyromonas gingivalis.

Porphyromonas gingivalis is a gram-negative anaerobic bacterium and an etiologic agent of adult periodontitis. By inducing a dysbiotic state within the host microbiota it contributes to a chronic inflammatory environment in the oral cavity. Under some circumstances, the oral bacteria may gain access to systemic circulation. While the most widely recognized function of platelets is to reduce hemorrhage in case of vascular damage, it is known that platelets are also involved in the hematologic responses to bacterial infections. Some pathogenic bacteria can interact with platelets, triggering their activation and aggregation. The aim of this study was to assess platelet responses to the presence of P. gingivalis in whole blood. Human whole blood was pretreated with P. gingivalis and then platelet plug formation was measured under high shear conditions using the PFA-100. In the presence of P. gingivalis, time for a platelet plug to occlude the aperture in the collagen/ADP cartridge was shortened in a manner dependent on bacterial concentration and the duration of bacterial preincubation of blood. P. gingivalis enhances thrombus forming potential of platelets in whole blood.

Contribution of fungi and viruses towards the etiopathogenesis peri-implantitis: A literature review of currently available evidence

• The aim was to review the contribution of fungi and viruses towards the progression of peri-implant diseases. • Indexed databases were searched using various keywords • Candida species particularly Candida albicans is associated with the etiology of peri-implantitis • Herpes- and Epstein-Barr virus exacerbate peri-implantitis diseases The aim of the present literature review was to evaluate the contribution of fungi and viruses towards the etiopathogenesis of peri-implant diseases. The focused question addressed was “Can fungi and viruses contribute towards the etiopathogenesis of peri-implant diseases?”. The databases, which were used for the literature search comprised of ISI Web of knowledge, OVID, PubMed/ Medline, EMBASE, Google-Scholar and Scopus. These databases were searched up to March 2020. Diverse groupings of the following key-indexing terms were done to identify the relevant studies: “ Candida ”, “fungi”, “yeast”, “crestal bone loss”, “microbe”, “peri-implant diseases”, “dental implant”, “etiopathogenesis”, “inflammation”, “peri-implant mucositis”, “peri-implantitis”, and “virus”. The pattern of the present literature review was customized to summarize the pertinent information. Five studies showed that in the presence of Porphyromonas gingivalis , herpes- and Epstein-Barr virus exacerbate peri-implantitis diseases. Seven studies associated oral yeasts (predominantly Candida species) with the etiology of peri-implantitis. Results from these studies showed that in the presence of periodonto-pathogenic bacteria, growth of Candida albicans is exacerbated that also may increase soft tissue inflammation and crestal bone loss around dental implants. Additional studies are needed to confirm the role of fungi and viruses towards the etiopathogenesis of peri-implant diseases. In the presence of periodontopathogenic bacteria, virulence of oral fungi and viruses seems to be increased. This in turn may worsen peri-implant inflammation in susceptible patients. However, a justification for this requires further studies.

Adipokines and periodontal markers as risk indicators of early rheumatoid arthritis: a cross-sectional study.

To establish the association between adipokine levels and markers of periodontal involvement as risk indicators of early stages of RA (eRA) and the interaction between the presence of markers of periodontal disease with adipokine in eRA individuals. Fifty-one patients with a diagnosis of eRA and 51 healthy controls matched for age and sex were studied. Clinical joint condition, clinical and serological markers of disease activity, serum adipokine levels (leptin, adiponectin, resistin, adipsin, vaspin, and IL-6), periodontal diagnosis, presence of Porphyromonas gingivalis, and related IgG1 and IgG2 antibodies were evaluated. Comparisons were made between eRA and healthy controls for periodontal indicators and adipokines. A subgroup analysis was realized with a non-conditional logistic regression to establish the association between the levels of leptin in individuals with eRA and controls according to the periodontal condition, presence of P. gingivalis, or high titers of IgG antibodies against P. gingivalis. The condition of overweight or obesity is associated with the diagnosis of eRA (p = 0.05), and these individuals also have higher levels of leptin (p = 0.001) and vaspin (p = 0.007). Higher frequency of P. gingivalis (p = 0.001) was found in the eRa group. Individuals with eRA with higher IgG2 titers against P. gingivalis had higher levels of leptin (OR: 1.66 (CI 95% 1.01-2.73)); however, individuals with periodontitis or P. gingivalis with eRA were associated with highest levels of leptin (OR: 1.86, CI 95% 1.19-24.3; and OR: 2.04, CI 95% 1.37-3 respectively). eRA individuals have high levels of leptin and vaspin. However, the presence of periodontitis and related-periodontal disease markers showed an effect only in leptin levels in eRA individuals. Emphasizing in personalized medicine, monitoring serum leptin levels and periodontitis markers can improve the early diagnosis of RA.

Probiotic strains of Lactobacillus brevis and Lactobacillus plantarum as adjunct to non-surgical periodontal therapy: 3-month results of a randomized controlled clinical trial

To determine if periodontitis patients benefit from treatment with Lactobacillus brevis and Lactobacillus plantarum strains, applied into periodontal pockets as gel and thereafter taken as lozenges, as an adjunct to scaling and root planing (SRP). In a double-blind, randomized, placebo-controlled trial, 40 patients received scaling and root planing (SRP) in two sessions within 7days. Patients then received either probiotic gel and lozenges (n = 20) or placebo (n = 20). The primary outcome variable was the number of diseased sites (DS: PD > 4mm + BOP) at the 3-month re-evaluation. The effects of gender, age, probiotic therapy, presence of Porphyromonas gingivalis or Aggregatibacter actinomycetemcomitans, smoking, tooth being a molar and interdental location were evaluated using a multivariate multilevel logistic regression model. The number of DS after 3months was similar in the test (Me = 8, IQR = 5-11) and control (Me = 5, IQR = 1-10) groups. Both groups showed substantial but equivalent improvements in periodontal parameters. The logistic regression showed higher odds for the healing of gingival bleeding (OR = 2.12, p = 0.048) and lower odds for the healing of DS (OR = 0.51; p < 0.001) in the probiotic group. Patients with periodontitis benefit from adjunctive use of probiotics containing L. brevis and L. plantarum in terms of reduction of gingival bleeding. However, adjunctive probiotics increase the number of persisting diseased sites with PD > 4mm and BOP. The adjunctive use of probiotics containing L. brevis and L. plantarum strains in treating chronic periodontitis results in a higher number of residual diseased sites when compared with SRP + placebo; its use is therefore unfounded.

Fluorescence image and microbiological analysis of biofilm retained around healthy and inflamed orthodontic miniscrews.

Peri-miniscrew inflammation is one of the causes of orthodontic miniscrew failure. The aim of this study was to correlate and quantify throughout autofluorescence images, PCR and microbiologic count of biofilm retained around orthodontic miniscrew and the presence of Porphyromonas gingivalis. Forty miniscrews used for orthodontic treatment were evaluated during orthodontic treatment, collected from patients and divided into two groups: healthy and inflamed miniscrews. To be considered inflamed, the samples should present: loss of stability checked by periotest®, clinical presence of mucositis, red aspect of the gum or bleeding around the miniscrew. Immediately after removal of the miniscrews, they were photographed using a macro 100 lens and a Pentax camera coupled to a fluorescent equipment - Qscan (AioBio - Korea) with 405 nm excitation wavelength and a blue band filter. A microbiologic sample was collected with a sterile microbrush scrubbed around the miniscrew tread. Fluorescent images were analyzed with ImageJ software to quantify fluorescent intensity and fluorescent area and microbiological samples were submitted to CFU count for total contamination and q-PCR assay to quantify Porphyromonas gingivalis. The results showed a good correlation between CFU count and fluorescent intensity and PCR/fluorescent area. The healthy miniscrews presented less fluorescent intensity and lower CFU count when compared to inflamed miniscrews. q-PCR analysis showed a higher number of P. gingivalis contamination around inflamed miniscrews. Quantification of biofilm retained by miniscrew by images of autofluorescence is a simple and reliable method with great potential for clinical use to monitory inflammation around miniscrew and risk of loss.

Porphyromonas Gingivalis Load is Balanced by 0.20% Chlorhexidine Gel. A Randomized, Double-Blind, Controlled, Microbiological and Immunohistochemical Human Study

Microbial contamination could compromise the stability of dental implants increasing the risk of inflammatory reactions in the surrounding soft tissues. In this human, randomized, double-blind, clinical study, the presence of Porphyromonas gingivalis on the healing abutment and the inflammatory infiltrate surrounding peri-implant soft tissues were investigated. Experiments were done in order to clarify the effect of 0.20% chlorhexidine (CHX) versus placebo, applied during each rehabilitation stage. Thirty patients (15 per group) were included. The load of adhering P. gingivalis on the healing screw were quantified by quantitative Polymerase Chain Reaction (qPCR) Taq-Man. Immunohistochemical analysis was carried out on the gingival biopsy. Moreover, clinical data were recorded. Analysis of variance and the Holm–Sidak test was used to evaluate differences between groups. The results showed a significant low presence of P. gingivalis load in healing abutments belonging to the 0.20% CHX group. Overall, the differences in terms of P. gingivalis DNA copy number between two groups were statistically significant (p < 0.01). All implants showed very low plaque and bleeding scores, but the placebo group appeared to have the highest expression of inflammation markers for T Lymphocytes, B Lymphocytes and macrophages Cluster definitions (CD3, CD20 and CD68). The use of 0.20% CHX could be recommended in all clinical procedures as it reduces significantly P. gingivalis load and host inflammatory response around implants.

Detection of Red complex bacteria, P. gingivalis, T. denticola and T. forsythia in infected root canals and their association with clinical signs and symptoms

Aim:This study aimed to investigate the association between endodontic clinical signs and symptoms and the presence of Porphyromonas gingivalis, Treponema denticola, and Tannerella forsythia employing polymerase chain reaction (PCR).Materials and Methods:Microbial samples were obtained from 60 cases with necrotic pulp with primary teeth infections. DNA extracted from samples were analyzed for endodontic pathogens by using species-specific primers.Results:P. gingivalis/T. denticola were detected in 15 symptomatic teeth associated with periapical lesions. T. forsythia/T. denticola were found in 16 symptomatic teeth associated with pain and swelling. P. gingivalis was detected in 9 teeth which were associated with pain, 2 with tenderness on percussion, and 15 with periapical lesions. Statistically significant associations were found between T. forsythia as well as T. denticola in relation to clinical findings of pain and swelling. (P < 0.05). Red complex bacteria showed no statistical significant association with the presence of signs and symptoms.Conclusion:Prevalence of P. gingivalis, T. denticola, and T. forsythia suggested association of these bacteria with symptomatic infected pulp and periradicular diseases.

Identification of subgingival periodontopathogen in subjects with chronic periodontitis

Abstract Objective The aim of this study was to identify the presence of subgingival periodontopathogen in subjects diagnosed with chronic periodontitis among patients attending International Islamic University Malaysia, Kulliyyah of Dentistry Polyclinic, Kuantan, State of Pahang. Materials and Methods: Fifty systemically healthy Malaysian subjects were enrolled in this study. Thirty of those who diagnosed with chronic periodontitis were recruited for the test group, and 20 subjects were considered the control group based on the absence of clinical attachment loss. The clinical periodontal parameters including full mouth plaque scores, bleeding on probing, pocket depth and clinical attachment loss were recorded. Subgingival plaque samples were collected using periodontal curette from deep pockets of each individual and pooled for analysis and DNA extraction. Preoperative serum immunoglobulin G and A antibodies to Porphyromonas gingivalis are potential serum biomarkers for the diagnosis and prognosis of esophageal squamous cell carcinoma

BackgroundThe key-stone-pathogen, Porphyromonas gingivalis associates not only with periodontal diseases but with a variety of other chronic diseases such as cancer. We previously reported an association between the presence of Porphyromonas gingivalis in esophageal squamous cell carcinoma (ESCC) and its progression. We now report the diagnostic and prognostic potential of serum immunoglobulin G and A antibodies (IgG/A) against Porphyromonas gingivalis for ESCC.MethodsAn enzyme-linked immunosorbent assay (ELISA) was used to determine the serum levels of Porphyromonas gingivalis IgG and IgA in 96 cases with ESCC, 50 cases with esophagitis and 80 healthy controls.ResultsThe median serum levels of IgG and IgA for P. gingivalis were significantly higher in ESCC patients than non-ESCC controls. P. gingivalis IgG and IgA in serum demonstrated sensitivities/specificities of 29.17%/96.90% and 52.10%/70.81%, respectively, and combination of IgG and IgA produced a sensitivity/specificity of 68.75%/68.46%. The diagnostic performance of serum P. gingivalis IgA for early ESCC was superior to that of IgG (54.54% vs. 20.45%). Furthermore, high serum levels of P. gingivalis IgG or IgA were associated with worse prognosis of ESCC patients, in particular for patients with stage 0-IIor negative lymphnode metastasis, and ESCC patients with high levels of both IgG and IgA had the worst prognosis. Multivariate analysis revealed that lymph node status, IgG and IgA were independent prognostic factors.ConclusionsThe IgG and IgA for P. gingivalis are potential serum biomarkers for ESCC and combination of IgG and IgA improves the diagnostic and prognostic performance. Furthermore, serum P. gingivalis IgG and IgA can detect early stage ESCC.

Estimation of Toll-like receptor 9 in gingival tissues of patients with chronic periodontitis with or without hyperlipidemia and its association with the presence of Porphyromonas gingivalis.

Background:Recent evidence suggests the interactions between bacterial DNA and nucleic acid receptors to play a role in inflammatory tissue destruction. The current study aims to evaluate the expression of Toll-like receptor 9 (TLR9) in periodontal disease associated with or without hyperlipidemia and to associate it with the presence of Porphyromonas gingivalis.Materials and Methods:Thirty participants in the age range of 25–50 years were randomly recruited and divided into three groups, i.e., healthy (Group I), chronic periodontitis without hyperlipidemia (Group II), and chronic periodontitis with hyperlipidemia (Group III). The gingival tissue samples were analyzed for TLR9 using immunohistochemistry, and plaque samples were analyzed for P. gingivalis using polymerase chain reaction.Results:The TLR9-positive cell ratio in gingival connective tissue for Group II and Group III was 0.95 ± 0.03 and 0.94 ± 0.03, respectively, which was significantly higher than that of Group I, with P < 0.001 (0.88 ± 0.04). These groups also demonstrated significantly higher presence of P. gingivalis as compared to Group I with P < 0.001. There was a positive association between TLR9 in gingival connective tissue and presence of P. gingivalis.Conclusion:The results of this study reveal a potential role of TLR9 in chronic periodontitis, in association with P. gingivalis. Furthermore, these variables do not show an appreciable change in hyperlipidemics suggesting a weak relation between TLR9 and lipid levels.

Preventive effects of the novel antimicrobial peptide Nal-P-113 in a rat Periodontitis model by limiting the growth of Porphyromonas gingivalis and modulating IL-1\u03b2 and TNF-\u03b1 production

BackgroundP-113 (AKRHHGYKRKFH-NH2) is a 12-amino-acid histidine-rich peptide derived from histatin 5 that is highly degradable in high salt concentrations and biological fluids such as serum, plasma and saliva. Nal-P-113, a novel antimicrobial peptide whose histidine residues are replaced by the bulky amino acids β-naphthylalanine, causes the antimicrobial peptide to retain its bactericidal activity even in physiological environments. This study evaluated the effect of the novel antimicrobial peptide Nal-P-113 in a rat periodontitis model and the mechanisms of action of Nal-P-113 for suppressing periodontitis.MethodsPeriodontitis was induced in mandibular first molars in rats receiving a ligature and infected with Porphyromonas gingivalis. Animals were randomly divided into six groups: a, P. gingivalis W83 alone; b, P. gingivalis W83 with 6.25 μg/mL of Nal-P-113; c, P. gingivalis W83 with 25 μg/mL of Nal-P-113; d, P. gingivalis W83 with 100 μg/mL of Nal-P-113; e, P. gingivalis W83 with 400 μg/mL of Nal-P-113; and f, control without P. gingivalis W83 or Nal-P-113. Morphometric analysis was used to evaluate alveolar bone loss. Microbiological assessment of the presence of Porphyromonas gingivalis and total bacteria was performed using absolute quantitative real-time PCR and scanning electron microscopy. Gingival tissue was collected for western blot and immunohistochemical assays of IL-1β and TNF-α levels.ResultsAlveolar bone loss was inhibited by 100 μg/mL or 400 μg/mL of Nal-P-113 compared to the control group (P < 0.05). Lower amounts of P. gingivalis and total bacteria were found in groups d and e compared with group a (P < 0.05). A decrease in the levels of IL-1β and TNF-α was detected in group d and group e compared to the control group (P < 0.05). The amount of P. gingivalis was positively correlated with IL-1β and TNF-α expression in periodontal tissue (P < 0.05).ConclusionsNal-P-113 exhibited protective effects on Porphyromonas gingivalis-induced periodontitis in rats by limiting the amount of bacteria and modulating IL-1β and TNF-α production. The use of Nal-P-113 in vivo might serve as a beneficial preventive or therapeutic approach for periodontitis.

Salivary ammonia levels and Tannerella forsythia are associated with rheumatoid arthritis: A cross sectional study.

The aim of this paper is to evaluate the relationship of salivary ammonium levels and the presence of bacteria with rheumatoid arthritis (RA) clinical disease activity in a cross‐sectional study of Mexican patients. From a periodontal and disease activity standpoint, 132 consecutive RA patients fulfilling clinical criteria were evaluated. Ammonia levels (including peptidyl arginine deiminase activity) were evaluated by colorimetric assay and the presence of Porphyromonas gingivalis, Tannerella forsythia, and Prevotella intermedia was evaluated by polymerase chain reaction (PCR) technique. After a multivariate analysis, adjusting for clinical and serological parameters, a significant association was only observed between severe periodontitis and probing depth with high RA disease activity. Additionally, in contrast to P. gingivalis, the presence of T. forsythia was significantly associated with high disease RA activity even after multivariable adjustment analysis. There was also a significant increase in ammonium levels in the high RA activity group and a significant correlation between salivary ammonia and RA disease activity but not with autoantibody titers. Similarly, we observed a significant increase in the ammonium levels derived from the cultures of P. gingivalis and T. forsythia, with respect to P. intermedia and S. gordonii cultures, or even healthy donors. These results suggest that RA activity is associated with severe periodontitis, high salivary ammonium levels and the presence of T. forsythia.

HUMORAL IMMUNE RESPONSE TO PORPHYROMONAS GINGIVALIS IN POSTPARTUM WOMEN AND NEWBORNS

Aims: The aim of this study was to evaluate the presence of Porphyromonas gingivalis (Pg) the in subgingival biofilm, as well as to compare the immune responses of postpartum women and their newborns to Porphyromonas gingivalis using antibody serum levels.Methods: A total of 43 postpartum women and 24 newborns were selected in the municipal maternity hospital of Alagoinhas, Bahia, Brazil between February and December 2003, for this cross-sectional study. The presence of Pg was verified using polymerase chain reaction. The immunoglobulin serum levels: IgG1, IgG2, IgG3, IgG4 and IgA, reactive to Pg ATCC33277, were tested using ELISA. Results: 39,3% of the postpartum women biofilm samples were positive for Pg. The incidence of the pathogen in the women with periodontitis (70%) was higher than in the group of women without periodontitis (30,30%). Among the newborns, high serum levels of IgG4 anti-Pg were observed (p&lt;0.001).Conclusion: Our findings confirm that postpartum women with periodontitis present remarkable incidence of Porphyromonas gingivalis which may influence systemic response, as represented by high levels of serum antibody levels against this pathogen in women and newborns. Furthermore, evidence suggests that some antibodies crossed the placental barrier in newborns.

Presence of Porphyromonas gingivalis in esophagus and its association with the clinicopathological characteristics and survival in patients with esophageal cancer.

BackgroundMounting evidence suggests a causal relationship between specific bacterial infections and the development of certain malignancies. However, the possible role of the keystone periodontal pathogen, Porphyromonas gingivalis, in esophageal squamous cell carcinoma (ESCC) remains unknown. Therefore, we examined the presence of P. gingivalis in esophageal mucosa, and the relationship between P. gingivalis infection and the diagnosis and prognosis of ESCC.MethodsThe presence of P. gingivalis in the esophageal tissues from ESCC patients and normal controls was examined by immunohistochemistry using antibodies targeting whole bacteria and its unique secreted protease, the gingipain Kgp. qRT-PCR was used as a confirmatory approach to detect P. gingivalis 16S rDNA. Clinicopathologic characteristics were collected to analyze the relationship between P. gingivalis infection and development of ESCC.ResultsP. gingivalis was detected immunohistochemically in 61 % of cancerous tissues, 12 % of adjacent tissues and was undetected in normal esophageal mucosa. A similar distribution of lysine-specific gingipain, a catalytic endoprotease uniquely secreted by P. gingivalis, and P. gingivalis 16S rDNA was also observed. Moreover, statistic correlations showed P. gingivalis infection was positively associated with multiple clinicopathologic characteristics, including differentiation status, metastasis, and overall survival rate.ConclusionThese findings demonstrate for the first time that P. gingivalis infects the epithelium of the esophagus of ESCC patients, establish an association between infection with P. gingivalis and the progression of ESCC, and suggest P. gingivalis infection could be a biomarker for this disease. Investigate the correlation between clinical sign and symptoms and the presence of P. gingivalis, T. denticola, and T. forsythia individually or as a "Red complex" by a multiplex PCR method.

Aim:The aim of this study was to investigate the correlation between endodontic clinical signs and symptoms and the presence of Porphyromonas gingivalis, Treponema denticola, and Tannerella forsythia or their association by Multiplex polymerase chain reaction assay.Materials and Methods:Microbial samples were taken from 30 cases with necrotic pulp tissues in primary infections. DNA was extracted from the samples, which were analyzed for the presence of three endodontic pathogens by using species-specific primers.Results:P. gingivalis, T. denticola, T. forsythia, and Red Complex were present in 11, 17, 4, and 2 canals, respectively. Clinical and statistically significant relationships were found between T. forsythia and mobility and between T. denticola and swelling. (P < 0.05). Presence of other Red complex bacteria shows clinical association with presence of signs and symptoms but no statistically significant relationship.Conclusion:The high prevalence of P. gingivalis, T. denticola, and T. forsythia in the examined samples suggests that these bacteria are related to the etiology of symptomatic periradicular diseases.
