Outcomes of Strabismus Surgery Following Teprotumumab Therapy

PurposeTo investigate the results of patients undergoing surgical treatment for strabismic diplopia in thyroid eye disease (TED) following teprotumumab. DesignMulticenter, retrospective, case series. MethodsWe report 28 patients who underwent extraocular muscle surgery for strabismic diplopia after treatment with teprotumumab at 7 different academic centers. Elapsed time from last teprotumumab dose to the date of surgery, previous orbital decompression, primary preoperative horizontal and vertical deviation, surgical procedure, and 2-month postoperative results were collected from the patient records. ResultsSixteen (57%) patients were diplopia-free after one surgery. Three (11%) chose prism spectacles to correct residual diplopia, 2 (7%) used compensatory head posture to resolve diplopia, and 1 (4%) had intermittent diplopia and was functionally improved (choosing no prisms or further surgery). These were considered treatment successes. Three (11%) patients required reoperation, and all were diplopia-free after their second procedure. ConclusionsMost patients requiring surgery for strabismic diplopia following teprotumumab achieve good outcomes with success rates comparable to series published before the availability of teprotumumab.

Concurrent adjacent strabismus surgery with glaucoma drainage device placement in childhood glaucomas

BackgroundDetermine outcomes of concurrent strabismus surgery with placement of a glaucoma drainage device (GDD) in children.MethodsRetrospective review of pediatric patients who underwent simultaneous lateral rectus (LR) muscle surgery with superotemporal GDD placement. Strabismus and GDD success were defined as residual horizontal misalignment < 10 prism diopter (PD) and intraocular pressure (IOP) < 21 mmHg, no visually devastating complications, and no additional IOP-lowering surgeries.ResultsFifteen eyes of 13 patients (69% male) underwent LR surgery (14 recessions, 1 resection) for exotropia or esotropia simultaneous with GDD placement (13 Baerveldt, 2 Ahmed) at 8.34 ± 5.26 years. Preoperative visual acuity (VA) in operative eye (0.89 ± 0.54) was worse than non-operative eye (0.23 ± 0.44, p = 0.0032). Preoperative horizontal deviation was 38.3 ± 9.4 PD and LR recession was 7.4 ± 1.1 mm. At final follow-up, VA in operative eye (0.87 ± 0.52) was unchanged from preoperative (p = 0.4062). Final IOP was significantly decreased (12.4 ± 4.7 mmHg vs. 31.1 ± 11.4 mmHg, p = 0.0001) as was number of glaucoma medications (2.7 ± 1.7 vs. 1.1 ± 1.3, p = 0.0037). Five (38%) and 9 patients (69%) met criteria for strabismus and GDD success, respectively. Two eyes required tube revision and endoscopic cyclophotocoagulation and 2 eyes had additional strabismus surgery.ConclusionsConcurrent strabismus and GDD surgery decreased horizontal deviation and obtained IOP control. It is important to consider correction of strabismus at time of GDD placement to maximize visual development and improve cosmesis in children with glaucoma.

The effect of previous orbital decompression on outcomes of rectus muscle recession surgery in patients with thyroid ophthalmopathy

To evaluate the effect of previous orbital decompression on outcomes of rectus muscle recession surgery in patients with thyroid-associated ophthalmopathy. This retrospective study enrolled 55 eyes of 33 patients treated in our hospital for restrictive strabismus caused by thyroid-associated ophthalmopathy. We performed muscle recession for the obviously restricted extraocular muscles, with 6 weeks of follow-up. Surgical outcomes were compared between the orbital decompression group (DG, n=15) and non-orbital decompression group (NDG, n=18). A total of 33 patients with Graves' ophthalmopathy who underwent rectus muscle recession surgery were included. Of these, 15 patients had undergone orbital decompression prior to strabismus surgery, and 18 had not. The two groups did not differ in terms of the preoperative horizontal or vertical ocular deviation, degree of restriction of eye movement, degree of diplopia, or mean number of muscles that underwent surgery (P>0.05). There was no significant difference in the preoperative horizontal or vertical ocular deviation, level of eye movement restriction, degree of diplopia and the success rate of the surgery (P>0.05). Rectus muscle recession surgery in patients with thyroid-associated ophthalmopathy during the quiescent period could improve the ocular deviation and diplopia, and orbital decompression performed before strabismus surgery had no significant effect on surgical technique or outcomes of rectus muscle recession surgery.

Factors associated with changes in horizontal deviation after inferior oblique weakening surgery.

To identify the factors affecting postoperative horizontal deviation following isolated inferior oblique (IO) weakening surgery. The medical records of patients who had undergone isolated IO weakening surgery between February 2010 and September 2021, with a minimum follow-up period of 6 months, were retrospectively reviewed. Patients were divided into two groups, eso-shift, and non-eso-shift, based on the change in horizontal deviation at postoperative 6 months. Eso-shift or exo-shift was defined as a change of 2Δ or more. A total of 77 patients were included. Among them, 49 patients (63.6%) showed changes in horizontal deviation: 37 (48.0%) eso-shift, 12 (15.6%) exo-shift. Twenty-eight patients (36.4%) showed no change in alignment. The eso-shift group showed the following characteristics compared to the non-eso-shift group: older age (P < 0.001), higher proportion of patients with preoperative exo-deviation (P < 0.001), and greater preoperative hypertropia in primary gaze (P < 0.001), down gaze (P = 0.031), ipsilateral gaze (P < 0.001), and ipsilateral head tilt (P = 0.002). Regarding the postoperative changes in horizontal deviation based on prior horizontal strabismus, patients with preoperative exo-deviation showed a statistically significant eso-shift. The patients with prior eso-deviation tended toward exo-shift, while those without prior horizontal deviation rarely changed. Changes in postoperative horizontal deviation are associated with preoperative horizontal and vertical deviation, which might result from changes in the rectus muscle due to oblique muscle dysfunction. Also, this study suggests that isolated IO weakening surgery could effectively correct small-angle horizontal deviation in patients with inferior oblique overaction (IOOA) and horizontal strabismus.

Determinants of postoperative alignment in patients of infantile esotropia

Abstract Purpose: The purpose of this article is to study the factors affecting postoperative alignment in patients of infantile esotropia with deviation more than or equal to 30 prism diopter (PD) after 12 weeks of follow-up. Materials and Methods: A total of 23 patients with infantile esotropia were included in this study over a period of 18 months. All the patients were assessed preoperatively and postoperatively at day 1, 3rd week, 6th week, and 12th week. The patients underwent horizontal muscle surgery alone or vertical combined with horizontal muscle surgery wherever required. All surgeries were planned and performed according to W.E. Scott table by a limbus-based approach. Results: Mean age of our study population was 13.36 ± 8.0 years (1.5–30 years). Fifteen (65.2%) patients were hypermetropic, 5 (21.7%) patients were emmetropic and 3 (13.1%) were myopic. Among the study participants, amblyopia was present in 13 patients (56.6%). The mean preoperative horizontal deviation was 48.04 PD ± 13.3 PD (range = 30–70 PD). Eleven patients had preoperative esotropia of &gt;50 PD and 12 had &lt;50 PD. The mean postoperative horizontal deviation was 5.1 ± 3.1 PD (range = 2–14 PD). Twenty-one (91.30%) patients had orthophoria, 2 (8.70%) patients had residual esotropia and none had overcorrection. Conclusion: The postoperative alignment was not influenced by age at the time of surgery (P = 0.67) and magnitude of preoperative deviation (P = 0.67) Further, the postoperative outcome was independent of the laterality of surgery (unilateral vs. bilateral, P = 0.6) and extraocular muscles operated (horizontal vs. combined horizontal and vertical, P = 0.537).

Postoperative drift and dose-response of strabismus surgery in thyroid eye disease: predicting desired outcomes with intraoperative adjustable sutures

ObjectifPrésenter les résultats de la correction chirurgicale du strabisme secondaire à l'ophtalmopathie thyroïdienne réalisée par l'insertion de sutures de type « nœud papillon » (bow-knot) ajustables en cours de chirurgie sous anesthésie locale et concevoir des modèles de prédiction de la correction chirurgicale et de la déviation postopératoire en fonction de la relation dose-effet de l'intervention. MéthodesNous avons procédé à une analyse rétrospective des dossiers de patients opérés entre 2016 et 2021. Le résultat était jugé satisfaisant si la diplopie avait disparu et s'accompagnait d'un alignement vertical (< 5 DP) et horizontal (< 10 DP) stable en position primaire du regard. La correction de la diplopie subjective au moyen de prismes permettant un alignement moteur était considérée comme un résultat passable. Dans les autres cas, le résultat était jugé médiocre. RésultatsAu total, 73 patients ont été admis à cette étude (60 femmes, soit 82 %). Les déviations horizontales et verticales moyennes en période préopératoire se chiffraient à 26,9 DP (é.-t. : 19,1) et à 11,8 DP (é.-t. : 7,6), respectivement. La déviation horizontale moyenne était de –3,2 DP (é.-t. : 5,2), tandis que la déviation verticale moyenne était de –3,4 DP (é.-t. : 3,3). Une analyse de régression linéaire a permis d’établir une relation dose-effet de 2,37 DP/mm pour la récession du droit médial (p < 0,0001; r2 = 0,777) et de 3,75 DP/mm pour la récession unilatérale du droit inférieur (p < 0,0001; r2 = 0,922). Les taux de réussite se déclinaient comme suit : satisfaisant chez 62 patients (85 %), passable chez 7 patients (10 %) et médiocre chez 4 patients (5 %). ConclusionsLa récession musculaire réalisée au moyen de sutures ajustables pendant l'intervention est une technique efficace pour corriger le strabisme secondaire à l'ophtalmopathie thyroïdienne. Il est possible d'obtenir une relation dose-effet prévisible avec une tendance vers une surcorrection. Une sous-correction proportionnelle à l'ampleur de la chirurgie prévue devrait être la cible de choix en période postopératoire immédiate. On n'a pas pu établir de corrélation entre d'autres variables, notamment la décompression orbitaire et la chirurgie à la fois verticale et horizontale, et la relation dose-effet ou la déviation postopératoire. Cela dit, d'autres études devront être réalisées afin de valider nos résultats.

Long-term outcomes of adjustable strabismus surgery at a Pakistani university hospital.

Strabismus, whether congenital or acquired, is a common visual and cosmetic problem, especially for the young. Adjustable suture strabismus surgery is not in vogue in our country. This technique gives the surgeon a second attempt to provide a better outcome for the patients. Our objective was to assess the long-term success of adjustable strabismus surgery in terms of postoperative alignment. We carried out a prospective study utilizing the fornix approach for adjustable strabismus surgery, in mainly horizontal, but also vertical strabismus in adults and cooperative children, to enhance the postoperative outcomes. The patient characteristics, preoperative deviation, type and pattern of strabismus, were evaluated and analyzed. The postoperative alignment was evaluated at 1year and beyond, to assess the success of this adjustable surgery. This study recruited 50 adults and children with a female predominance of 39 (78%); and with the mean age being 18.34 ± 9.88years. Exotropia was the primary diagnosis in the majority with 21(42%) cases; with purely horizontal strabismus in 23(46%) cases. The mean preoperative horizontal deviation was 48.76 ± 20.35prism diopters (PD) and the mean postoperative horizontal deviation was 2.73 ± 3.63 PD. The mean preoperative vertical deviation was 4.8 ± 8.54 PD whereas the mean postoperative vertical deviation was 0.86 ± 1.73 PD. The Wilcoxon Signed Ranks test analyzed the difference between the two which was statistically significant (p = 0.000). Surgical success, defined as postoperative horizontal alignment within ≤ 10 PD of orthotropia at the end of one year or more of follow-up after surgery, was achieved in 49 (98%) cases. The average follow-up was 21.47 ± 8.7months. Adjustable strabismus surgery has very good long-term outcomes in terms of postoperative alignment and patient satisfaction.

Pattern Strabismus in a Tertiary Hospital in Southern China: A Retrospective Review.

Background and objectives: To analyze demographic and clinical features of pattern strabismus patients and assess the relationship among these clinical variables and risk factors. Materials and Methods: Medical records of pattern strabismus patients who had undergone strabismus surgery at our center between 2014 and 2019 were retrospectively reviewed. Data collected included gender, age at onset, age at surgery, refraction, Cobb angle, pre- and post-operative deviations in the primary position, up- and downgaze, angle of ocular torsion, type/amount of pattern, grade of oblique muscle function and presence/grade of binocular function. To verify the clinical significance of the Cobb angle, 666 patients who had undergone surgery within one week after ocular trauma between 2015 and 2021 were enrolled as controls. Results: Of the 8738 patients with horizontal strabismus, 905 (507 males and 398 females) had pattern strabismus, accounting for 10.36%. Among these 905 patients, 313 showed an A-pattern and 592 showed a V-pattern. The predominant subtype was V-exotropia, followed by A-exotropia, V-esotropia and A-esotropia. Over half of these patients (54.6%) manifested an A- or V-pattern in childhood. The overall mean ± SD Cobb angle was 5.03 ± 4.06° and the prevalence of thoracic scoliosis was 12.4%, both of which were higher than that observed in normal controls (4.26 ± 3.36° and 7.8%). Within A-pattern patients, 80.2% had SOOA and 81.5% an intorsion, while in V-pattern patients, 81.5% had IOOA and 73.4% an extorsion. Patients with binocular function showed decreases in all of these percent values. Only 126 (13.9%) had binocular function, while 11.8% of A-pattern and 15.1% of V-pattern patients still maintained binocular function. Pre-operative horizontal deviation was negatively correlated with binocular function (r = −0.223, p < 0.0001), while the grade of oblique muscle overaction was positively correlated with the amount of pattern (r = 0.768, p < 0.0001) and ocular torsion (r = 0.794, p < 0.0001). There were no significant correlations between the Cobb angle and any of the other clinical variables. There were 724 patients (80.0%) who had received an oblique muscle procedure and 181 (20.0%) who received horizontal rectus muscle surgery. The most commonly used procedure consisted of horizontal rectus surgery plus inferior oblique myectomy (n = 293, 32.4%), followed by isolated horizontal rectus surgery (n = 122, 13.4%). Reductions of pattern were 14.67 ± 6.93 PD in response to horizontal rectus surgery and 18.26 ± 7.49 PD following oblique muscle surgery. Post-operative deviations were less in V- versus A-pattern strabismus. Post-operative binocular function was obtained in 276 of these patients (30.5%), which represented a 16.6% increase over that of pre-operative levels. The number of patients with binocular function in V-pattern strabismus was greater than that of A-pattern strabismus (p = 0.048). Conclusions: Of patients receiving horizontal strabismus surgery, 10.36% showed pattern strabismus. In these patients, 54.6% manifested an A- or V-pattern in childhood, and V-exotropia was the most frequent subtype. Pattern strabismus patients showed a high risk for developing scoliosis. Cyclovertical muscle surgery was performed in 724 of these patients (80.0%), and horizontal rectus surgery was effective in correcting relatively small levels of patterns. Binocular function represented an important factor as being involved with affecting the occurrence and development of pattern strabismus.

Larger Than Larger: Huge Amounts of Recession-Resection Surgery for Third Nerve Palsy.

To describe the characteristics and management outcomes of strabismus surgery in patients with third cranial nerve palsy and identify factors associated with long-term outcomes. A 10-year retrospective study of patients with third cranial nerve palsy who underwent strabismus surgery was performed at Zhongshan Ophthalmic Center, Sun Yat-sen University, China, between August 2010 and January 2021. Clinical characteristics and factors associated with long-term outcomes were evaluated. Success of surgery was defined as esotropia of 15 prism diopters (PD) or less at postoperative day 1 (POD1) or any deviation of 10 PD or less at postoperative month 2 (POM2) or later. A total of 91 patients were included, with a mean age of 24.02 ± 15.38 years (range: 2 to 64 years). Forty-one patients (45.1%) were diagnosed as having complete palsy. Congenital palsy accounted for 40.7% (n = 37). Mean follow-up time was 2.50 ± 1.78 years (range: 8 months to 8 years). One-step surgery was performed in 78.0% of cases (n = 71), a staging procedure in 7.7% (n = 7), and reoperations in 14.3% (n = 13). Most patients had supra-maximal recession and resection of the horizontal rectus muscle with additional techniques, including horizontal rectus transposition, superior oblique transposition, traction sutures, and fixation of the globe. The mean surgical dosage performed on the horizontal rectus muscle was 18.52 ± 5.69 mm (range: 8 to 35 mm). Success rates at postoperative month 8 (POM8) were 69.0% (49 of 71) in the one-step surgery group, 57.1% (4 of 7) in the staging surgery group, and 46.2% (6 of 13) in the reoperation group. Preoperative horizontal deviation (odds ratio [OR]: 1.07, 95% CI: 1.02 to 1.13, P = .011), horizontal deviation at POD1 (OR: 0.86, 95% CI: 0.76 to 0.97, P = .016), and horizontal deviation at POM2 (OR: 1.53, 95% CI: 1.18 to 1.98, P = .001) were associated with the outcomes at POM8. Patients with partial palsy had a higher success rate than those with complete palsy (76.0% vs 51.2%, P = .014). Supra-maximal recession and resection with additional techniques yields satisfying surgical results in patients with third nerve palsy when a larger surgical dose is performed for an initial overcorrection for 15 to 25 PD at POD1. Isolated inferior oblique pareses.

To describe etiology, clinical characteristics, radiological features and management of isolated inferior oblique pareses. A diagnosis of inferior oblique paresis was made after a thorough strabismus examination and neuroimaging. The patients were managed surgically with adjustable strabismus surgery, or conservatively. Surgical success was defined as average horizontal deviation within ≤ 10 prism diopters [PD] post-operatively and for vertical deviation, it was ≤ 5 PD, at last follow-up. Seven cases were congenital, 6 cases were bilateral, with esotropia in 6 cases; 'A' pattern in 7 cases and hypotropia in 3 cases. The mean preoperative horizontal deviation was 52.5 PD, and the mean postoperative horizontal deviation was 2.37 PD (p = 0.028). The pre-operative vertical deviation was 18 PD and post-operative vertical deviation was 5 PD. MRI showed reduced IO muscle size; average area being 11.27mm2 in the affected eyes, with normal sized inferior recti (average: 24.63mm2) and medial recti muscles (average: 30.08mm2). Surgical success was seen in all six cases. Average follow-up was 265days. The Parks' three step test was not valid, except for one acquired unilateral case. Isolated pareses of inferior oblique muscle exhibit defective elevation in adduction of the affected eye, 'A' pattern and fundus intorsion, and is confirmed by neuroimaging. These can be successfully managed surgically to correct the deviation.

A comparative multicentric long-term study of un-augmented modified Nishida procedure vs augmentation in unilateral sixth nerve palsy.

To compare the effectiveness of three procedures: modified Nishida procedure alone vs modified Nishida procedure combined with medial rectus recession (MRc) vs modified Nishida procedure combined with MRc and botulinum toxin (BT) for severe unilateral sixth nerve palsy. Consecutive, interventional case series. The medical records of a consecutive series of patients with severe unilateral sixth nerve palsy who underwent modified Nishida procedure in multiple centres were reviewed. Surgical technique was decided preoperatively at the surgeon's discretion. The preoperative and postoperative findings were compared. Of the 43 patients with abducens palsy that received the procedure, 32 were included (mean age 38.6 ± 19.8 years). Mean preoperative deviation was 63.0 ± 27.3 prism dioptres (PD) and mean limitation of abduction -4.5 ± 1.2. Five patients underwent a modified Nishida procedure alone, 24 patients had an additional MRc and 3 patients were also injected with BT. Overall, the average correction of modified Nishida technique by itself was 29.4 ± 6.6 PD (range 20-36) and adding a MRc corrected 62.6 ± 23.8 PD (range 24-120). Modified Nishida procedure, MRc and BT altogether corrected 95.0 ± 18.0 PD (range 75-110). No postoperative complications were observed in any of the patients. Excellent outcomes with fewer complications are obtained with modified Nishida procedure alone. The need for additional procedures such as MRc and BT which increase the effect in primary position can be determined depending on passive duction and preoperative horizontal deviation.

Congenital ocular counter-roll: a review of cases treated exclusively by ophthalmologists.

To review the demographics, clinical manifestations, and surgical experiences of patients with congenital ocular counter-roll, whose treatments were performed exclusively by ophthalmologists. A retrospective review was conducted consisting of patients who received strabismus surgery between 2017 to 2019. Patients with obvious ocular counter-roll were included. A total of 7008 patients who received strabismus surgery, 28 (12 males, 16 females) were diagnosed as congenital ocular counter-roll, accounting for 0.40%. All patients were initially misdiagnosed: 21 patients were misdiagnosed as superior oblique palsy (SOP), 3 as inferior oblique overaction, 2 as dissociated vertical deviation (DVD), 1 as superior oblique overaction with A-pattern exotropia, and 1 as medial rectus palsy. The mean±SD age was 12.4±9.4y (range 2.5-36y). The most common clinical findings included ocular counter-roll, vertical deviation or vertical deviation combined with outward deviation and head tilt. At follow-up, an excellent surgical result was achieved in 20 patients. Preoperative horizontal deviation of 26±24 prism diopter (PD) and vertical deviation of 18±12 PD were reduced to 0±12 PD (P=0.0001) and 3±4 PD (P=0.001), respectively. Congenital ocular counter-roll is a rare supranuclear vertical strabismus caused by congenital abnormalities involving vestibule-ocular reflex pathways. In addition to ocular counter-roll, the most salient clinical features included, but are not limited to, hyperdeviation, outward deviation, overelevation in adduction and head tilt.

Strabismus Surgery Outcomes and Their Determinants in Patients with Chronic Sixth Nerve Palsy.

PURPOSE:Patients with chronic sixth nerve palsy (CSNP) comprise a heterogeneous population, and the optimal surgical solution remains uncertain. Here, we present the success rate and factors associated with the success of strabismus surgeries for CSNP.METHODS:This was a retrospective cohort study of patients with strabismus due to CSNP operated on between 2015 and 2019 in a tertiary eye hospital in central Saudi Arabia. Surgical success was defined as a horizontal deviation of ≤10 prism diopters (PDs) assessed at least 12 months after surgery. Differences between groups with respect to the primary outcome were assessed.RESULTS:Fifty-five patients were analyzed with a median follow-up of 24 (range 12–48) months. Superior rectus and inferior rectus transposition (34.5%) and medial rectus recession with lateral rectus resection (32.7%) were the main surgeries performed. The overall success rate was 67.3% (95% confidence interval 54.9–79.7). Bilateral CSNP (P = 0.05), a higher preoperative angle of deviation (P = 0.002), or a greater degree of preoperative limitation of abduction (P = 0.012), but not the type of surgery (P = 0.09), were more likely to result in an under-corrected outcome of >10 PD. The preoperative deviation angle showed a trend toward being associated with a poor outcome after surgery (P = 0.06). Six patients with high-angle deviation before surgery required second surgery.CONCLUSION:While the surgical procedure does not impact outcomes, the severity of preoperative horizontal deviation might impact surgical success and the need for reoperation. Patients with severe CSNP should be counseled appropriately about the chances of surgical success and the potential need for further interventions.

Surgical outcomes of augmented bilateral lateral rectus recession with conjunctival recession in patients of intermittent exotropia

ABSTRACT Purpose: To evaluate the surgical outcome of augmented BLR recession with conjunctival recession in cases of IXT. Method: A total of 15 patients of the basic type of IXT were included in this prospective study. The patients underwent augmented BLR recession with conjunctival recession and on the basis of postoperative deviation, the outcome was assessed in terms OF orthophoria/success (< or =10 PD exotropia and < or =5 PD esotropia), residual exotropia/undercorrection (>10 PD exotropia), and overcorrection (>5 PD esotropia). Results: Preoperative primary horizontal deviation was 30.80 ± 9.56 PD (16–42 PD) for near (0.3 m) and was 33.93 ± 9.97PD (15–45 PD) for distance (6 m). The mean postoperative primary horizontal deviation was 5.6 ± 1.5 PD (4–8 PD) for distance and 6.73 ± 2.15 PD (4–10 PD) for near at 6 months follow-up. After 12 weeks of surgery, 12 (80%) of the patients were orthophoric, 2 (13.3%) of the patients were overcorrected and 1 (6.7%) of the patient was undercorrected for near and all 15 (100.0%) patients were orthophoric for distance. This alignment was maintained till 6 months of follow-up. Conclusions: Although fornix-based incisions for squint surgery are preferred these days augmented BLR recession with conjunctival recession by limbal-based incision leads to good surgical outcome and prevents undercorrection and recurrence.

The management of large-angle esotropia in Graves ophthalmopathy with combined medial rectus recession and lateral rectus resection

To describe surgical management and outcomes for large-angle esotropia of ≥50Δ secondary to Graves ophthalmopathy using combined initial nonadjustable medial rectus recessions and lateral rectus resections. The medical records of consecutive patients undergoing strabismus surgery for large-angle esotropia secondary to Graves ophthalmopathy from 1995 to 2012 by a single surgeon at each of two institutions was performed. Patient characteristics, surgical technique, and pre- and postoperative measurements of ocular alignment were analyzed. A modified Gorman diplopia scale was used to assess outcome. Of 38 patients, 36 had bilateral nonadjustable medial rectus recessions and lateral rectus resections as initial treatment for esotropia, and 6 patients underwent simultaneous vertical muscle surgery. Mean preoperative horizontal deviation was 60Δ and mean preoperative vertical deviation was 10Δ. Of the 38 patients, 19 (50%) reached the primary outcome, including 5 of 6 (85%) who had no preoperative vertical strabismus. The indications for reoperation were vertical strabismus in 13 of 21 patients (62%), residual esotropia in 7 of 21 (33%), and consecutive exotropia in 1 of 21 (5%). With a median follow-up of 13.2months after first surgery, 32 of 38 patients (84%) reached the secondary outcome. Combining nonadjustable medial rectus recessions with lateral rectus resections can be a beneficial primary treatment for large-angle esotropia in patients with Graves ophthalmopathy, especially in those patients with small or no associated vertical strabismus.

Long term outcomes of strabismus surgery for third nerve palsy

PurposeTo report long-term outcomes of strabismus surgery for treatment of third nerve palsy. MethodsWe performed a 15-year retrospective study of patients who had undergone surgery. We analyzed preoperative mean deviation (at 6 months, 1 year after surgery, and at the end of follow-up), type of surgery, and factors predicting outcomes. A final deviation <10 prism diopters (pd) was considered a good esthetic outcome, and no diplopia in the primary position at the end of follow-up was considered a good functional outcome. ResultsSurgery was performed in 31 cases. Mean age was 36.27 years (women, 51.6%). Total and complete third nerve palsy was recorded in 22.6% and acquired palsy in 80.6%. Mean preoperative horizontal deviation (HD) in primary position was 40.24pd (near) and 44.29 (distance) and 14.33pd in vertical deviation (VD). Mean final HD was 8.94pd (near) and 11.35pd (distance), and mean final VD was 6.13. One surgery was performed in 68.9%. A favorable esthetic outcome was obtained in 64.5% and a successful functional outcome in 72.2%. Statistically significant differences were found between near HD (p=0.019) and distance HD (p=0.035) at 1 year compared with the end of follow-up and between VD at 6 months and 1 year after surgery (p=0.03). Mean follow-up was 6.05 years. No specific factors predicted a successful outcome. ConclusionsEsthetic and functional results were similar. No predictors of successful outcome were identified. HD was better 1 year after surgery, and VD improved during follow-up.

Bupivacaine injection combined with recession of antagonist rectus muscle to treat sensory strabismus

ABSTRACTPurpose: This study aims to evaluate the effect of bupivacaine injection at 1.5% combined with recession of the antagonist rectus muscle to treat sensory strabismus in adult patients.Methods: Nine patients with a horizontal deviation, visual acuity of <20/60 in the worst-seeing eye, and no previous strabismus surgery were selected. Patients with vertical strabismus were excluded. Lateral rectus (LR) recession was performed in patients with exotropia and medial rectus (MR) recession in patients with esotropia according to Park’s Table, as modified by Wright. Bupivacaine 1.5 mL at 1.5% was injected in the antagonist of the recessed muscle. The patients were evaluated after 1, 7, 30, 60, 90, and 180 days of the procedure. Successful motor alignment was defined as within 10 prism diopters (PD) of orthophoria. Dose-response was considered as the total amount of correction, divided by recession performed in millimeters, associated with bupivacaine injection in the antagonist muscle.Results: Of the patients, 44% (4/9) had a successful motor alignment. The mean correction for exotropia (n = 5) with a preoperative horizontal deviation of 50 ± 6.12 PD was 23 ± 14.4 PD with a dose-response of 2.6 PD/mm. For esotropia (n = 4), the mean correction of 21.25 ± 4.8 PD was achieved with a preoperative deviation of 28.7 ± 14.9 PD with a final dose-response of 5 PD/mm. All unsuccessful patients had a preoperative deviation of ≥45 PD.Conclusion: Bupivacaine injection in the antagonist of a recessed extraocular muscle (EOM) appears to add an effect to the standard dose-response of a muscle recession. For patients with esotropia (<25 PD), a single MR recession combined with bupivacaine in the LR can produce a successful motor alignment. For patients with large horizontal strabismus (>45 PD), larger bupivacaine doses (volume and concentration) combined to larger recessions may be needed.

Bilateral Symmetric and Asymmetric Superior Rectus Recession for Patients with Dissociated Vertical Deviation.

To evaluate the outcome of bilateral symmetric and asymmetric superior rectus (SR) recessions in patients with bilateral dissociated vertical deviation (DVD) without oblique dysfunction and determine factors that might influence the surgical outcome. Retrospective study. All patients who underwent bilateral SR recession for bilateral DVD from January 2012 to December 2016 at an eye hospital in New Delhi, India were included. Indication for surgery was decompensated DVD in 1 or both eyes. Symmetric SR recession was performed for symmetric DVD and asymmetric SR recession was performed for asymmetric DVD of 10 prism diopters (PD) or more. Patients with a minimum follow-up of 6 months were included. Success was defined as absence of manifest DVD in both eyes at the final postoperative visit. Medical records of 27 patients were analyzed. Their median age at surgery was 8 years [interquartile range (IQR), 6-10 years] and mean follow-up was 16.3 months (range, 6-48 months). Symmetric and asymmetric surgeries were performed in 19 and 8 patients, respectively. The amount of SR recession performed ranged from 5 to 10 mm. The median DVD reduced from 9 PD (IQR, 6-14 PD) to 5 PD (IQR, 3-8 PD) in the right eye (P = 0.015) and 9 PD (IQR, 7-12 PD) to 6 PD (IQR, 3-10 PD) in the left eye (P = 0.016) after surgery. Successful outcome was seen in 63% of patients. There was no difference in successful outcome with respect to age, sex, preoperative horizontal deviation, preoperative vertical deviation, postoperative residual horizontal deviation, surgical technique, asymmetry of SR recession, and magnitude of preoperative DVD. Patients with a preference for fixation were more likely to have a successful outcome. Bilateral SR recession resulted in a success rate of 63% after single operation for bilateral DVD without oblique dysfunction. We found that the probability of a successful outcome did not depend on age at surgery, sex, preoperative horizontal or vertical deviation, magnitude of preoperative DVD, symmetry of SR recession, or surgical technique.

Suture Fixation of Globe to the Orbital Wall for Complicated Strabismus.

The aim of this study was to observe the surgical outcomes of suture globe fixation to the orbital wall for patients with complicated strabismus. The medical records of patients with complicated strabismus who were treated at the Zhongshan Ophthalmic Center of Sun Yat-sen University, Guangzhou, China, from January 2010 to June 2015 were retrospectively reviewed. Routine strabismus surgery was not possible in these cases. Eyes were anchored to the orbital wall by simple strong anastomosis of the extraocular muscle and orbital wall tissues using silk suture, which resulted in slight overcorrection of the ultimate ocular position after anchoring. Eight patients with complicated strabismus received suture globe fixation to the orbital wall in our hospital, including 4 cases of strabismus that occurred after severe orbital injury and/or fracture repair, 2 cases of medial rectus rupture after endoscopic sinus surgery, 1 case of complete third nerve palsy, and 1 case of complete sixth nerve palsy. Five patients were male and 3 female, with age ranging from 20.0 to 57.0 years (mean = 38.1 years). The right eyes were involved in 7 cases and the left in 1 case. Five cases underwent globe fixation to the medial orbital wall, 2 cases to the lateral orbital wall, and 1 case to the inferior orbital wall. After at least 6 months follow-up, preoperative horizontal deviation of 60.625 ± 27.443 prism diopter (PD) (mean ± standard deviation) was reduced to 16.250 ± 12.464 PD. The vertical deviation was reduced from 15.000 ± 12.313 PD to 7.5 ± 5.244 PD. Two patients achieved orthophoria, and although the remaining 6 patients showed some residual strabismus (undercorrection), their appearance was significantly improved. No other surgical complications occurred in these cases. Suture fixation of the globe to the orbital wall by flesh-to-flesh anastomosis of the extraocular muscle and orbital wall tissues achieved a desirable surgical result for complicated strabismus.

Surgical Management of Strabismus in Patients With Orbital Fracture.

Till date, the surgical policy of strabismus in patients with orbital fracture is scarcely described in literature. The purpose of this study was to describe the clinical features and surgical experience in patients with strabismus and persistent diplopia after orbital fracture repair, or in patients who need not receive surgery for orbital fracture. Retrospective review was performed on patients with orbital fracture who received strabismus surgery between 2004 and 2016 in Eye Hospital, Zhongshan Ophthalimc Center, Sun Yat-sen University, China. The following clinical data were recorded from the patients' charts: the nature of fracture, age at surgery, preoperative motor alignment, stereoacuity, surgical methods, surgical outcomes, and complications. Deviations of 10 prism diopter (PD) or less of horizontal angle and deviation of 5 PD or less of vertical angle in the primary position were considered to be a successful outcome. A total of 29 patients (23 males and 6 females) with an average age of 32.4 ± 16.6 years (2-61 years) were recruited in this study. The strabismus was paralytic in 16 patients, restrictive in 2 patients, and both paralytic and restrictive in 11 patients. The surgical methods were quite various in each patient; however, all patients underwent either rectus recession or rectus recession plus resection. With at least 6 months of follow-up, preoperative horizontal deviation of 32.79 ± 22.84 PD was reduced to 6.69 ± 13.01 PD (P < 0.01). The vertical deviation was reduced from 22.76 ± 17.66 PD to 6.59 ± 7.98 PD (P < 0.01). Sixteen patients (55.2%) were considered as success, whereas a surgical undercorrection was observed in 44.8%. There were no surgical complications. The clinical manifestations of strabismus in patients with orbital fracture or after fracture repair are quite complex, often involving both paralytic and restrictive factors. The surgical treatment of each patient needs to be individualized. The rectus muscle recession and resection seems to be predictable, effective, and stable.
