Analysis of the Impact of Interventions Based on Orem's Self-Care Model on the Balance and Motor Function of Patients with Parkinson's Disease.

Background and Purpose: The study investigates the effects of Orem's self-care model on balance and motor function in patients with Parkinson's disease (PD), aiming to enhance self-management abilities. Methods: Sixty-one PD patients were selected and divided into an experimental group (31 participants) receiving Orem's self-care interventions and a control group (30 participants) receiving standard care. The experimental group received Orem's self-care intervention, whereas the control group received standard care. The Berg Balance Scale (BBS) and the third part of the Unified Parkinson's Disease Rating Scale (UPDRS-III) scores were recorded and analyzed for both groups before and after the nursing intervention. Multiple linear regression was used to analyze the relationship between pre- and postintervention changes in BBS and UPDRS-III scores in the intervention group, considering factors, such as marital status, education level, place of residence, disease duration, and rehabilitation exercise participation. Results: After the intervention, the experimental group demonstrated significantly improved scores, with a BBS score of 34.81 ± 3.24 and a UPDRS-III score of 25.16 ± 5.49, compared with the control group, which had a BBS score of 29.54 ± 3.20 and a UPDRS-III score of 31.60 ± 7.69. The differences were statistically significant (t = 6.391, p < .001; t = 3.775, p < .001). Age and disease duration were negatively correlated with changes in BBS (ΔBBS) and UPDRS-III (ΔUPDRS-III), whereas education level showed a positive correlation with these changes. Implications for Practice: Incorporating Orem's self-care model into the nursing care of PD patients can lead to significant improvements in their balance and motor function.

Steroids Static Postural Balance Changes After Exercise Intervention Correlate with Steroidome in Elderly Female

Background: Aging is associated with the development of various disorders, including postural imbalance, which increases the risk of falls and related health complications. This study examines changes in static postural balance after a 4-week intervention involving appropriate exercise and an optimized daily regimen. Additionally, it explores the relationship between these changes and the steroidome. Methods: The study was conducted on a clinically homogeneous group of 41 females around their sixth decade, diagnosed with anxiety-depressive syndrome and treated with selective serotonin reuptake inhibitors (SSRIs). Postural balance was assessed using the dual-scales method (DLLL-DSM), which estimates postural imbalance by evaluating differences in the lower limb load in the standing position. Correlations between initial DLLL-DSM values, age, BMI, and the baseline levels of nine serum steroids, as well as post-intervention changes in five steroids, were analyzed using multivariate regression (OPLS model). Results: A significant reduction in lower limb load differences (-ΔDLLL-DSM), indicating improved postural balance, was observed. The -ΔDLLL-DSM strongly correlated with initial DLLL-DSM values, age, BMI, initial levels of nine serum steroids, and post-intervention changes in five steroids (R = 0.892, p &lt; 0.001). Furthermore, initial DLLL-DSM values negatively correlated with adrenal androgen androstenediol sulfate and various sulfated 5α/β-reduced androgen metabolites (R = 0.323, p &lt; 0.05), suggesting that the activity of steroid sulfotransferase (SULT2A1) and C17-hydroxylase-C17,20-lyase (CYP17A1) at the lyase step is negatively associated with postural imbalance in elderly females. Conclusions: The findings suggest that even severe postural imbalance can be effectively and relatively rapidly improved through an appropriate exercise-based intervention and an optimized daily regimen, provided that initial adrenal activity is not significantly impaired. Additionally, the identified associations between steroid levels and postural balance provide new insights into the hormonal mechanisms regulating balance control in older individuals.

The Acute Effects of Antagonist Static Stretching on Agonist Performance

Stretching the antagonist muscle to enhance agonist performance has gained considerable attention. However, most studies have focused on one stretching duration. Hence, the aim of this study was to compare varying durations (40-, 80-, and 120-seconds) of antagonist (dorsiflexors) static stretching (SS) on agonist (plantar flexors: PF) muscle performance. In this randomized crossover study, 16 participants (six females) underwent four sessions (40-, 80-, 120-s dorsiflexors SS and control), with pre- and post-intervention measurements of slow (600/s) and fast (2400/s) PF isokinetic, isometric peak torque, total work, stiff leg drop jump (SDJ) performance (height, reactive strength index (RSI) and peak power), and tibialis anterior and soleus electromyography (EMG). Dorsiflexors SS protocol involved 1x40-s (40-s), 2x40-s (80-s), and 3x40-s (120-s). There were no significant pre- to post-intervention changes in any parameter. A main effect for time demonstrated an overall decrease in fast (p &lt; 0.0001, 5.9%, d = 0.24) and slow (p = 0.05, 6.6%, d = 0.24) isokinetic peak torque, total work (p = 0.02, 5.1%, d = 0.20) and all SDJ measures (SDJ height: p = 0.02, 2.7%, d = 0.15, RSI: p &lt; 0.0001, 5.9%, d = 0.23, peak power: p = 0.002, 4.4%, d = 0.22). Soleus EMG decreased from pre- to post-SS after 120-s in both isometric (p = 0.002, 13.6%, d = 0.73) and slow isokinetic (p = 0.002, 12.3%, d = 0.91) peak torque as well as 80-s with slow isokinetic peak torque (p = 0.02, 6.6%, d = 0.75). In summary, different durations of dorsiflexors SS (40-s, 80-s, 120-s) did not significantly influence PF performance. However, deficits associated with a main effect for time suggested possible testing effects as detected with the control condition.

Effects of Reimbursement Add-On Policies on Trastuzumab and Bevacizumab Biosimilar Adoption in a Japanese Oncology Setting.

The Japanese government has introduced reimbursement incentive policies designed to promote biosimilar adoption for oncology biologics, including the provision of a financial incentive of ¥1500 (approximately $10) per prescription to physicians who prescribe biosimilars. This study aimed to evaluate the impact of Japan's reimbursement policies on the uptake of oncology biosimilars, specifically trastuzumab and bevacizumab, in the outpatient setting. We conducted an interrupted time series analysis using the health insurance claims database to assess the impact of the policy on biosimilar uptake, comparing trends before and after the policy intervention. The analysis incorporated 22 440 trastuzumab and 14 022 bevacizumab claims. For trastuzumab, interrupted time series analysis showed a significant slope change after policy implementation, with a slope change of 0.33% per month (95% CI 0.07-0.58). In contrast, bevacizumab did not exhibit significant changes in slope or level postintervention; however, its biosimilar uptake steadily increased over the entire study period. This study suggests that reimbursement incentives have significantly enhanced the uptake of trastuzumab biosimilars in Japan, whereas bevacizumab has not exhibited a similar response. Given these findings, it is crucial for policy makers to design reimbursement strategies that are specifically tailored to the characteristics of each biosimilar to optimize their effectiveness.

OPTIMIZED GUT BARRIER PROTEOMIC EXPRESSION AND IMPROVEMENTS IN DISEASE- AND BONE-RELATED BIOMARKERS: OUTCOMES FROM A 180-DAY INTERVENTIONAL TRIAL IN ULCERATIVE COLITIS PATIENTS

Abstract INTRODUCTION The long-term objective of our research is to ascertain whether, and to what extent, a multimineral product (Aquamin) can have a beneficial impact on individuals with ulcerative colitis (UC). In pursuit of this goal, we conducted a 180-day biomarker trial involving UC patients who were either in remission or at a mild-to-moderate stage of the disease. APPROACH A total of 28 subjects participated in the study. They were randomized to receive either Aquamin for 180 days or a placebo (maltodextrin) for 90 days while continuing their UC therapy. On day 90, subjects in the placebo group crossed over to receive Aquamin for the remaining 90 days. Serum samples were collected at baseline (day 0), mid-point (day 90), and study end (day 180) to measure C-reactive protein (CRP), alkaline phosphatase (ALP), intestine-specific ALP (ALPI), and biomarkers of bone turnover (osteocalcin, TRAP5b, and bone-specific ALP [BALP]). Stool specimens were evaluated for fecal calprotectin at the same time points, and colon biopsies were examined histologically for Geboes scoring. Dual-energy X-ray absorptiometry (DEXA) scans were performed at baseline and study end (day 0 and day 180). Additionally, a mass spectrometry-based proteomic assessment of colon biopsy specimens was conducted at each time point. RESULTS Subjects who received Aquamin for the full 180-day period (12 subjects) demonstrated improvements in UC disease-related biomarkers; these improvements were not observed in the placebo group (16 subjects). Subjects who received Aquamin for 90 days exhibited intermediate responses. Additionally, subjects receiving Aquamin for 180 days showed increased bone mineral density (BMD) and bone mineral content (BMC), resulting in a statistically significant increase in the hip strength index over the treatment period. This was accompanied by increases in osteocalcin and TRAP5b, and a decrease in BALP. The proteomic screening revealed upregulation of multiple gut barrier proteins, cell surface transporter molecules, and certain proteins with anti-inflammatory potential in response to Aquamin. Aquamin treatment also led to the downregulation of several proteins associated with the pro-inflammatory state. CONCLUSION The results presented here demonstrate that a multimineral intervention can enhance the gut barrier and improve disease-related biomarkers in patients with UC. An additional benefit includes improvements in bone health. These findings suggest the potential value of the multimineral intervention as a low-cost, non-toxic adjuvant therapy for mild to moderate UC or for those in remission. A Comparison of Bilateral vs. Unilateral Flywheel Strength Training on Physical Performance in Youth Male Basketball Players.

Background/objectives: This study aimed to compare the effects of bilateral and unilateral flywheel training programs on leg strength, sprint performance, jumping, and change of direction ability in young basketball players. Methods: Twenty-two youth male basketball players were randomly assigned to two groups: the unilateral group (UG; n = 11; age = 15.5 ± 0.5 years) and the bilateral group (BG; n = 11; age = 15.2 ± 0.4 years). Both groups participated in a six-week flywheel training intervention (UG: split squat; BG: half squat) alongside their regular basketball activities. Performance measures included change of direction ability (5-0-5 test), knee extension 60 degrees/s leg strength (EX60), bilateral and unilateral countermovement jump heights (CMJ, CMJL, and CMJD), reactive strength index (RSI), and 5 m and 20 m sprint times (SPR5m and SPR20m). A 2 × 2 ANOVA was used to evaluate pre- to post-intervention changes. Results: Significant interaction effects were observed for the 5-0-5 test (F = 13.27; p = 0.02), with pre-post improvements of 8.4% and 13.3% for the BG and UG, respectively. Both groups showed significant CMJ improvements (11.4%, ES = 0.69 for the BG; 14.6%, ES = 1.4 for the UG). The UG demonstrated greater unilateral jump improvements compared to the BG. Significant RSI improvements were found for both groups (BG: 19.6%, ES = 0.95; UG: 19.6%, ES = 0.77). Both groups improved on sprint performance, with the UG showing slightly larger effect sizes. Conclusions: Unilateral flywheel strength training appeared to be a more effective strategy than bilateral training for enhancing strength, sprinting, jumping, and change of direction ability in youth basketball players.

Brain connectivity correlates of the impact of a digital intervention for individuals with subjective cognitive decline on depression and IL-18

Late-life depression represents a significant health concern, linked to disruptions in brain connectivity and immune functioning, mood regulation, and cognitive function. This pilot study explores a digital intervention targeting mental health, brain health, and immune functioning in individuals aged 55–60 with subjective cognitive decline, elevated stress and depressive symptoms. Seventeen participants engaged in a two-week intervention comprising spatial cognition, psychological techniques based on mindfulness, attention-training exercises, and cognitive behavioral therapy. Pre-and post-intervention changes in resting-state functional connectivity, inflammation, and psychological health were evaluated. Key findings include: (1) Reduced self-reported depression with a large effect size, (2) Decreased connectivity within the default mode network (DMN), (3) Enhanced anticorrelation between the DMN-Salience networks that was associated with improved depression scores (4) Reduced salivary IL-18 concentration with a medium effect size, correlated with decreased DMN-amygdala connectivity. There was a trend towards reduced anxiety, with no significant changes in quality of life. To our knowledge, this is the first study to investigate the effect of digital intervention on immune markers, clinical behavioral outcomes, and brain function, demonstrating positive synergistic potential across all three levels. These preliminary findings, which need replication in larger, controlled studies, have important implications for basic science and scalable digital interventions.

Sex differences in absolute and relative changes in muscle size following resistance training in healthy adults: a systematic review with Bayesian meta-analysis.

Muscle hypertrophy may be influenced by biological differences between males and females. This meta-analysis investigated absolute and relative changes in muscle size following resistance training (RT) between males and females and whether measures of muscle size, body region assessed, muscle fibre type, and RT experience moderate the results. Studies were included if male and female participants were healthy (18-45 years old) adults that completed the same RT intervention, and a measure of pre- to post-intervention changes in muscle size was included. Out of 2,720 screened studies, 29 studies were included in the statistical analysis. Bayesian methods were used to estimate a standardised mean difference (SMD), log response ratio (lnRR) with exponentiated percentage change (Exp. % Change of lnRR), and probability of direction (pd) for each outcome. Absolute increases in muscle size slightly favoured males compared to females (SMD = 0.19 (95% HDI: 0.11 to 0.28); pd = 100%), however, relative increases in muscle size were similar between sexes (Exp. % Change of lnRR = 0.69% (95% HDI: -1.50% to 2.88%)). Outcomes were minimally influenced by the measure of muscle size and not influenced by RT experience of participants. Absolute hypertrophy of upper-body but not lower-body regions was favoured in males. Type I muscle fibre hypertrophy slightly favoured males, but Type II muscle fibre hypertrophy was similar between sexes. Our findings strengthen the understanding that females have a similar potential to induce muscle hypertrophy as males (particularly when considering relative increases in muscle size from baseline) and findings of our secondary analyses should inform future research that investigates sex differences in highly trained participants and muscle fibre type-specific hypertrophy.

Effects of dry cupping on exercise, autonomic activity and sleep in baseball players during preseason and in-season conditioning.

Cupping therapy has been shown to alleviate muscle fatigue, sustain exercise capacity, enhance post-exercise recovery of autonomic activity, and improves sleep quality. However, variations in athletes' training intensity, competition pressure, and fatigue levels throughout a sports season remain underexplored. Few studies have investigated whether the health benefits of cupping differ across various phases of a sports season. This study aimed to examine the effects of short-term cupping on athletes during preseason conditioning (PSC) and in-season conditioning (ISC). Forty university baseball players were recruited and randomly assigned to either the cupping (dry cupping at - 400 mmHg) or sham (dry cupping at - 100 mmHg) group. Cupping was applied to the upper back and shoulders for 15 minutes, twice a week for 8 consecutive weeks (4 weeks each during PSC and ISC). Cupping had no significant effect on upper-extremity function during either PSC or ISC. Exercise tests during PSC and ISC revealed no postintervention changes in peak power, peak oxygen consumption, or anaerobic threshold. However, cupping during PSC improved postexercise recovery of low-frequency power (LF; P = .013; a component of heart rate variability) and that during ISC improved recovery of the LF/high-frequency power ratio (P = .004) and LF% (P = .037). Additionally, cupping during PSC notably enhanced daytime function, as measured by the Pittsburgh sleep quality index (P = .026). The benefits of cupping therapy vary between PSC and ISC. Cupping during PSC and ISC notably improved the postexercise recovery of autonomic and sympathetic activities, respectively. However, improvements in sleep quality were only observed during PSC.

Effect of transcranial direct current stimulation on paroxysmal sympathetic hyperexcitability with acquired brain injury and cortical excitability: a randomized, double-blind, sham-controlled pilot study

Paroxysmal sympathetic hyperexcitation (PSH) refers to a clinical syndrome characterized by a sudden increase in sympathetic excitability caused by severe brain injury. This study aims to investigate the effectiveness and practicality of combining transcranial direct current stimulation (tDCS) with medication to treat PSH and employ non-linear electroencephalography (EEG) to assess changes in cortical activation post-intervention. 40 PSH patients were randomly assigned to receive either active tDCS or sham tDCS treatment over an 8-week period. The tDCS stimulation targeted the prefrontal area, left frontal-temporal-parietal cortex, right frontal-temporal-parietal cortex, and left dorsolateral prefrontal cortex. Both patient groups also underwent medication and other conventional therapies. The Paroxysmal Sympathetic Hyperactivity Assessment Measure (PSH-AM), Coma Recovery Scale-Revised (CRS-R), medication dosage, and approximate entropy (ApEn) index were assessed before and after treatment. The active tDCS group exhibited more substantial improvements in changes of PSH-AM, changes of CRS-R, and medication reduction ratios compared to the sham tDCS group after the treatment. After treatment and during follow-up, a significantly greater number of patients in the active tDCS group demonstrated clinically important differences compared to the sham tDCS group. The active tDCS group showed significantly higher ApEn indices in the less affected frontal lobe compared to the control group. No significant differences in ApEn indices were noted in the sham tDCS group before and after treatment. Regression analysis revealed that the group (active tDCS/sham tDCS) was the primary factor associated with improving PSH-AM. Therefore, we believe that in patients with PSH, combining tDCS with medication therapy demonstrated superior clinical efficacy compared to medication therapy alone. Electrophysiological results also indicated enhanced cortical excitability. Therefore, this single-center pilot study suggests that multi-target, multi-session tDCS combined with medication may be an effective treatment protocol for PSH.

A secondary analysis of gait after a 4-week postural intervention for older adults with hyperkyphosis

BackgroundThoracic hyperkyphosis (HK), common in older adults, has been linked to impairments in physical function, mobility, balance, gait, and falls. Our pilot study used a novel 4-week manual therapy and exercise intervention for HK and showed improved posture and function. This secondary analysis aims to explore 1) the changes in gait parameters after a novel intervention for HK, 2) the correlations between posture and gait variables at baseline, and 3) pre- to post intervention.MethodsThis secondary analysis uses data from a quasi-experimental, single group pilot study. Participants with HK underwent pre- and post intervention measurements in posture, function, and unique to this secondary analysis, gait parametrics of velocity (V), step length (SL), double limb support (DLS), and step width (SW) using the GAITRite® electronic walkway. Paired t-tests compared pre- and post intervention gait parameters. Pearson correlation coefficients were utilized to investigate correlations between all variables at baseline and in pre- and post intervention change values.ResultsFourteen women and 8 men (aged 65.9 years ±9.2; range 52 - 90) completed 12 treatments (3 times/week for 4-weeks). Statistically significant improvement (p≤.001) occurred pre- to post for postural measures: height (M=0.73cm ±0.54), Kyphotic index (-2.41 ±2.96), Block (-1.17cm ±1.22), Acromion to table (ATT) (-1.85cm ±1.42), and 3 gait measures: V (M=0.087m/s ±0.09), SL (2.34cm ±2.55), and DLS (- 0.031sec ±0.04). SW improvement was not statistically significant.Block and ATT measures were moderately correlated with V, SL, SW (Block only), and DLS (ATT only) at baseline. Strong correlations were found among V, SL, and DLS at baseline and in pre- to post change scores, but no correlation between change scores of posture and gait.ConclusionsThis study shows that a clinically practical 4-week PT intervention may benefit older adults with HK by demonstrating improved posture and gait parameters. Family-involved interventions may offer unique benefits for Veterans with posttraumatic stress disorder (PTSD) experiencing high family strain.

Family strain, which reflects the overall function of the family system, could play an important role in routine posttraumatic stress disorder (PTSD) care. We examined how high/low family strain influences perceptions of a family-involved intervention for Veterans with PTSD to inform the design and implementation of family-centered interventions in routine PTSD care. Sequential explanatory mixed methods study that analyzed qualitative exit interview data from a three-session family-involved pilot study stratified by Veteran baseline scores of family strain (Veteran n = 16; support partner [SP] n = 12). Themes were identified via rapid qualitative analysis of interview data categorized by high/low strain. Eleven Veterans (nine associated SPs) were classified as experiencing high and five Veterans (three associated SPs) were classified as experiencing low family strain. While nearly 50% of Veterans reported belonging to a non-White racial group, all of the Veterans were men. Themes reflected preintervention expectations, intervention experiences, and postintervention changes. Differences in intervention experiences were observed with different family strain levels. Participants in the high strain group articulated concrete goals for the intervention and provided more details about benefits, including an enhanced understanding of PTSD, improved family communication, and increased social support. Participants in the high strain group reported more discomfort with the intervention process, especially the conjoint sessions. Family-involved interventions for Veterans with high family strain are feasible and may have even greater benefits than for Veterans reporting low family strain. PROMIS-9 UE physical function demonstrates moderate responsiveness for patients following upper limb prosthesis intervention

BackgroundUpper extremity physical function is an essential health domain in the rehabilitation care for patients with upper limb amputation or absence. The PROMIS-9 UE Physical Function short form is a recently established instrument designed for individuals with upper limb amputation or absence. The instrument’s responsiveness to changes after receiving a prosthesis has not been investigated. The current study aimed to evaluate the ability of the PROMIS-9 UE to detect changes in bimanual (two-handed) functional capacity after patients’ receipt of a prosthesis.MethodologyA retrospective chart review was conducted on the longitudinal PROMIS-9 UE outcome measure scores completed between April 2016 and February 2024. Participants included individuals with an outcome collected before and after prosthesis receipt.ResultsThe final sample size included 124 individuals (91 male, 33 female; 43.4 ± 15.0 years old, 34.4 ± 103.0 months since amputation, and 62.9% injury etiology). Analyses found significant improvement across all patients in the PROMIS-9 UE scores from baseline to post prosthesis intervention (baseline: 25.3 ± 8.6, post: 29.5 ± 9.6; p < 0.001). The PROMIS-9 UE demonstrated a moderate responsiveness (Standardized Response Mean = 0.6) to prosthetic intervention. This was a directional change consistent with subjective patient reports of increased functionality. Furthermore, findings from the linear mixed model demonstrated significant responsiveness for the PROMIS-9 UE instrument to detect post-intervention changes after controlling for potential confounding effects (p < 0.001).ConclusionsThese findings suggest the PROMIS-9 UE Physical Function instrument demonstrates moderate clinical utility in capturing patient progress following upper limb prosthesis intervention.

Incorporating family support presents a promising approach to enhancing facilitators and addressing barriers to effective diabetes self-management, particularly in the Chinese context where familial relationships and collective family responsibility play a central role in health-related decision-making and daily life. We aimed to validate a family support-based self-management behavioral intervention program, Systematic evaluation, core education, self-directed planning, triple feedback, and develop a habit program (SeCe-STRIVE). The program comprises 5 modules: systematic evaluation, core education, self-directed planning, triple feedback, and habit development. We conducted a community-based, single-center, parallel randomized controlled trial in the Xietu community of Xuhui District, Shanghai. Eligible patients were randomly assigned to either the intervention or control group. Participants in the intervention group received the SeCe-STRIVE health management program based on family support, while those in the control group received the community-based follow-up management program for type 2 diabetes. The primary outcome measured was the total self-management behavior score and its changes across various dimensions from baseline. Secondary outcomes included family support, family function, diabetes knowledge, quality of life, self-efficacy, and biochemical markers. Post-intervention changes in both primary and secondary outcomes relative to baseline were analyzed using analysis of covariance. The study ultimately included 113 participants in the intervention group and 112 in the control group. The intervention group's total self-management behavior score improved by 12.74 (95% CI = 10.07, 15.40, P < .001) points more than the control group. Across all dimensions of self-management behaviors, the intervention group showed greater improvement, including diet, exercise, blood glucose monitoring, foot care, and medication adherence. Participants with low family support exhibited greater improvements in self-management behaviors compared to those with high family support. The intervention group demonstrated significant improvements in family support, family function, diabetes knowledge, and self-efficacy. Although quality of life and biochemical indicators of glycated hemoglobin, fasting blood glucose, and blood lipids showed a decreasing trend, the differences were not statistically significant. Compared to the control group, patients with type 2 diabetes who received the SeCe-STRIVE program intervention showed significant improvements in self-management behaviors, accompanied by enhancements in family support, family functioning, and self-efficacy.
