Management of functional constipation-associated halitosis: a retrospective study.

The features of functional constipation (FC)-associated halitosis were identified in the author's previous report. In this report, the author aimed to further investigate its treatment and efficacy. This retrospective study reviewed 100 FC patients, including 82 (82%) halitosis patients and 18 (18%) non-halitosis patients. They underwent the organoleptic test (OLT) to diagnose halitosis, and the organoleptic score (OLS) (0-5) was used to evaluated halitosis severity. The Cleveland Clinical Constipation Score (CCCS) (0-30) was used to evaluate FC severity. Patients were treated with the laxative polyethylene glycol electrolyte powder (PGEP) for four weeks. These tests were performed before and after treatment. The author found that, before treatment, the CCCS was 20.00 (18.00-23.00) for all patients, 21.00 (19.00-24.00) for halitosis patients, and 18.00 (17.00-18.25) for non-halitosis patients. A significant difference was observed between halitosis patients and non-halitosis patients (P < 0.001). The OLS for halitosis patients was 3.00 (3.00-4.00). A positive correlation (r = 0.814, 95% CI: 0.732-0.872, P < 0.001) was found between OLS and CCCS. A CCCS ≥18 predicted over 50% probability of halitosis. After treatment, the CCCS significantly decreased to 11.50 (6.00-14.75) (P < 0.001), and OLS significantly decreased to 1.00 (0.00-2.00) (P < 0.001). A positive correlation (r = 0.770, 95% CI: 0.673-0.841, P < 0.001) persisted between OLS and CCCS. A pre-treatment CCCS ≥21 predicted over 50% probability of post-treatment halitosis, while a post-treatment CCCS ≥12 predicted over 50% probability of post-treatment halitosis. The author concludes that the severity of FC parallels the severity of FC-associated halitosis, and can predict the probability of halitosis. Laxative treatment with PGEP is effective in improving FC-associated halitosis.

Efficacy of Polyethylene Glycol Electrolyte Powder Combined With Linaclotide for Colon Cleansing in Patients With Chronic Constipation Undergoing Colonoscopy: A Multicenter, Single-Blinded, Randomized Controlled Trial.

Constipation is an independent risk factor for poor bowel preparation. This study aimed to evaluate the bowel cleansing efficacy and safety of polyethylene glycol (PEG) combined with linaclotide (lin) for colonoscopy in patients with chronic constipation (CC). This single-blinded, randomized, controlled, and multicenter study was conducted from July 2021 to December 2022 at 7 hospitals. Patients with CC who underwent colonoscopies were enrolled and randomly assigned to 4 groups with split-PEG regimens: 4L-PEG group, 4L-PEG+1d-Lin group, 3L-PEG+1d-Lin group, and 3L-PEG+3d-Lin group. The primary outcome was rates of adequate bowel preparation, defined as a total BBPS score ≥6 and a score ≥2 for each segment. Secondary outcomes were adverse effects, sleep quality, willingness to repeat the colonoscopy, adenoma detection rate, and polyp detection rate. Five hundred two patients were enrolled. The rates of adequate bowel preparation (80.0% vs 60.3%, P < 0.001; 84.4% vs 60.3%, P < 0.001) and the total Boston Bowel Preparation Scale (BBPS) scores (6.90 ± 1.28 vs 6.00 ± 1.61, P < 0.001; 7.03 ± 1.24 vs 6.00 ± 1.61, P < 0.01) in the 4L-PEG+1d-Lin group and the 3L-PEG+3d-Lin group were superior to that in the 4L-PEG group. Compared with the 4L-PEG group, the 4L-PEG+1d-Lin group (66.7% vs 81.7%, P = 0.008) and the 3L-PEG+3d-Lin group (75.0% vs 81.7%, P = 0.224) had a lower percentage of mild adverse events. No statistically significant difference in willingness to repeat the colonoscopy, sleep quality, polyp detection rate, or adenoma detection rate was observed among groups. PEG combined with linaclotide might be an effective method for bowel preparation before colonoscopy in patients with CC.

The Effects of Combined Use of Linaclotide and Polyethylene Glycol Electrolyte Powder in Colonoscopy Preparation for Patients With Chronic Constipation.

The purpose of this study is to evaluate the safety and efficacy of linaclotide and polyethylene glycol (PEG) electrolyte powder in patients with chronic constipation undergoing colonoscopy preparation. We included 260 patients with chronic constipation who were scheduled to undergo a colonoscopy. They were equally divided into 4 groups using a random number table: 4L PEG, 3L PEG, 3L PEG+L, and 2L PEG+L. The 4 groups were compared based on their scores on the Boston Bowel Preparation Scale (BBPS) and Ottawa Bowel Preparation Quality Scale (OBPQS), adverse reactions during the bowel preparation procedure, colonoscope insertion time, colonoscope withdrawal time, detection rate of adenomas, and their willingness to repeat bowel preparation. In terms of the score of the right half of the colon, the score of the transverse colon, the total score using BBPS, and the total score using OBPQS, the 3L PEG (polyethylene glycol)+L group was superior to groups 3L PEG and 2L PEG+L (P<0.05), but comparable to the 4L PEG group (P>0.05). The incidence rate of vomiting was higher in the 4L PEG group than in the 2L PEG+L group (P<0.05). There was no statistically significant difference in the insertion time of the colonoscope between the 4 groups. The colonoscope withdrawal time in the 3L PEG+L group was shorter than in groups 4L PEG and 3L PEG (P<0.05) and comparable to that in the 4L PEG group (P>0.05). There was no statistically significant difference in the rate of adenoma detection among the 4 groups (P>0.05). The 4L PEG group was the least willing of the 4 groups to undergo repeated bowel preparation (P<0.05). The 3L PEG+L is optimal among the 4 procedures. It can facilitate high-quality bowel preparation, reduce the incidence of nausea during the bowel preparation procedure, and encourage patients to undertake repeated bowel preparation.

Preoperative bowel preparation promotes intestinal functional recovery after esophagectomy.

Patients are prone to intestinal dysfunction after esophagectomy. The value of preoperative bowel preparation before esophagectomy is controversial. There is a lack of evidence as to whether preoperative bowel preparation can help patients improve bowel function and shorten the recovery time of bowel function. The objectives of this study were to explore whether preoperative bowel preparation can promote the recovery of intestinal function after esophagectomy. We analysed 139 patients who underwent elective radical esophagectomy in the Department of Thoracic Surgery at the Second Affiliated Hospital of Xi'an Jiaotong University from May 2016 to December 2018. The enrolled patients were divided into the study group (bowel preparation group) and the control group (no bowel preparation group) of 71 cases and 68 cases. Patients in the study group were given dissolved polyethylene glycol electrolyte powder and a cleansing enema the day before surgery. Patients in the control group were neither given polyethylene glycol electrolyte powder nor cleansing enemas before surgery. The postoperative recovery of the two groups were compared. Postoperative bed rest time, bowel function recovery time and the time of first flatus and defecation after surgery were significantly shorter in patients with bowel preparation than in those without bowel preparation, and the differences were statistically significant. (P=0.038, P<0.001, P<0.001, P<0.001; respectively). Preoperative bowel preparation can promote the recovery of patients with esophageal cancer, especially the recovery of bowel function, which can reduce the pain caused by abdominal distension and improve the quality of life of patients.

Bowel preparation efficacy and safety of compound polyethylene glycol electrolyte powder combined with linaclotide for colonoscopy: A randomized controlled trial.

Adequate bowel preparation is essential for colonoscopy, which is important for detecting colon polyps and preventing colorectal cancer. Linaclotide is approved for irritable bowel syndrome with predominant constipation (IBS-C) symptoms. The main objective of this study was to explore the quality of bowel preparation by low-volume compound polyethylene glycol (PEG) combined with linaclotide. A total of 266 patients who underwent colonoscopy in Shangrao People's Hospital from June 2021 to June 2022 were randomized to 1 of 3 split PEG regimens: 4LPEG, 2LPEG, and 2LPEG + L (linaclotide). The primary end point was adequate bowel preparation (Boston Bowel Preparation Scale [BBPS] total score of ≥6, with each of three colonic segments subscores ≥2). Secondary outcomes were polyp detection rates and the incidence of adverse reactions. Over 12 months, 266 subjects were randomized into 2LPEG (n = 12), 4LPEG (n = 112), or 2LPEG + L (n = 142). There were no significant differences between the 4LPEG and 2LPEG + L groups in achieving adequate bowel preparation (P > 0.05). The mean BBPS score of the total colon, left hemi-colon, right hemi-colon, and transverse in the 2LPEG + L group was higher than that in the 2LPEG group (P < 0.001). Patient's sleeping quality and the incidence of adverse reactions of 2LPEG + L group were compatible with 2LPEG group, but it was significantly lower than that in 4LPEG group. There was no statistically significant difference in the detection rate of colon polyps between each group. The quality of bowel preparation of the compound polyethylene glycol electrolyte powder combined with linaclotide is approximately the same as that of 4LPEG, and it can reduce the adverse reactions in the process of bowel preparation, and its intestinal cleansing effect is also better than that of 2LPEG.

Risk factors for inadequate bowel preparation before colonoscopy: a retrospective cohort study

BackgroundColonoscopy is the standard and most effective screening tool for colonic diseases and the accuracy of colonoscopy depends on the quality of bowel preparation. The aim of this study was to analyze the risk factors for inadequate bowel preparation before colonoscopy.MethodsIn this retrospective study, patients who underwent colonoscopy in 2018 and received 3 L of Polyethylene Glycol Electrolytes powder were included. They were instructed to drink 1.5 L the night before the colonoscopy and 1.5 L 4–6 h before the procedure given in doses of 250 ml every 10 min with 30 ml of simethicone given 4–6 h before the colonoscopy. Patient- and procedure-related parameters were recorded. An adequate bowel preparation was defined as all 3 segments rated 2 or 3 on the Boston Bowel Preparation scale. Risk factors for inadequate bowel preparation were identified using multivariate logistic regression analysis.ResultsA total of 6720 patients were included in the present study. The mean age of these patients was 49.7 ± 13.0 years old. Inadequate bowel preparation was found in 233 (12.4%), 139 (6.4%), 131 (7%), 68 (8.6%) patients in spring, summer, autumn and winter respectively. On the multivariate analysis, male gender (OR: 1.295; 95% CI: 1.088–1.542; P = 0.005), inpatient status (OR: 1.377; 95% CI: 1.040–1.822; P = 0.025) and season (spring vs. winter, OR: 1.514; 95% CI: 1.139–2.012; P = 0.004) were the independent risk factors for inadequate bowel preparation.ConclusionsMale gender, inpatient status and spring season were the independent risk factors for inadequate bowel preparation. For patients with risk factors for inadequate bowel preparation, enhanced bowel preparation and instructions may help to optimize the quality of bowel preparation.

Effect of Compound Polyethylene Glycol Electrolyte Powder on the Quality of Gastrobowel Preparation before Enteroscopy Intervention.

Objective To study the effect of compound polyethylene glycol electrolyte powder (PGEP) on the quality of gastrobowel preparation before enteroscopy intervention. Methods From March 2021 to January 2022, among the patients who needed enteroscopy in our hospital, 280 patients who volunteered for this study were randomly selected as the research objects. All the subjects were randomly divided into the control group (140 cases) and the observation group (140 cases). Both groups received routine treatment before enteroscopy intervention. On this basis, patients in the control group were given 9 g of senna every day before operation, and 250 ml of 20% mannitol and 2500 ml of water were taken orally from 9:00 am to 11:00 am on the day of examination. Patients in the observation group took PGEP orally from 9:00 am to 11:00 am. The effective rate of bowel cleaning, the frequency of defecation and duration of diarrhea, the levels of blood electrolyte indexes such as Na+, K+, and Cl− before and after the intervention, and the incidence of adverse reactions were compared between the two groups. Results The effective rate of bowel cleaning in the observation group was significantly higher than that in the control group (P < 0.05). The frequency of defecation and duration of diarrhea in the observation group were significantly lower than those in the control group (P < 0.05). Compared with the control group, the levels of blood electrolyte indexes in the observation group after the intervention were not statistically significant (P > 0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group (P < 0.05). Conclusion Using PGEP for gastrobowel preparation before enteroscopy intervention can achieve high bowel cleaning efficiency, short bowel preparation time, and low incidence of adverse reactions, which does not affect the water-electrolyte balance of patients, and the psychological state of patients before enteroscopy intervention is more stable. This program is worthy of clinical promotion.

Influencing factors for the quality of bowel preparation with polyethylene glycol electrolyte powder combined with diet control before colonoscopy in children

To investigate the influencing factors for the quality of bowel preparation before colonoscopy in children and the association of the interval from the last administration of laxative to the start of colonoscopy (shortly referred to as waiting time) with the quality of bowel preparation. A retrospective analysis was performed for the children who were admitted to the Department of Gastroenterology, Children's Hospital of Nanjing Medical University, from January to November 2020, and received bowel preparation with polyethylene glycol electrolyte powder combined with diet control before colonoscopy. According to the score of Boston bowel preparation scale, they were divided into two groups: adequate bowel preparation group (n=337) and inadequate bowel preparation group (n=30). Related data were collected from the children in both groups, including general information, possible influencing factors for the quality of bowel preparation, adverse reactions associated with bowel preparation, duration of colonoscopy, and postoperative diagnosis. Univariate and multivariate analyses were used to explore the influencing factors for the quality of bowel preparation. The univariate analysis showed that age, body weight, and waiting time were associated with inadequate bowel preparation (P<0.05). The multivariate analysis showed that older age (OR=2.155, 95%CI: 1.087-4.273, P=0.028) and longer waiting time (OR=1.559, 95% CI: 1.191-2.041, P=0.001) were independent risk factors for inadequate bowel preparation. The receiver operating characteristic (ROC) curve analysis showed that the cut-off value of waiting time was 5.5 hours in determining whether bowel preparation was adequate or not, with a sensitivity of 90.0%, a specificity of 50.7%, and an area under the ROC curve of 0.708. After grouping based on waiting time, it was found that the incidence rate of inadequate bowel preparation in the ≥5.5 hours group was significantly higher than that in the <5.5 hours group [14.0% (27/193) vs 1.7% (3/174), P<0.001]. For children who use polyethylene glycol electrolyte powder combined with diet control for bowel preparation, older age is an independent risk factor for inadequate bowel preparation before colonoscopy, which may be associated with an insufficient dose of polyethylene glycol in older children. Longer waiting time is also an independent risk factor for inadequate bowel preparation, and it is recommended that the waiting time should not exceed 5.5 hours.

Effect of Combined Live Probiotics Alleviating the Gastrointestinal Symptoms of Functional Bowel Disorders.

Objective Changes of the gut microbiota are related to the pathogenesis of functional bowel disorders (FBDs), and probiotic supplementation may be an effective treatment option. Therefore, we aimed to investigate the effect of combined live probiotics on the gastrointestinal symptoms of FBDs via altering the gut microbiota. Methods Patients with the gastrointestinal symptoms of FBDs attending the Outpatient Department, from July to November 2019, were recruited. After the bowel preparation with polyethylene glycol electrolyte powder and colonoscopy, patients with normal result of colonoscopy were randomly divided into the probiotics group and control group. Patients in the probiotics group were prescribed with combined live Bacillus subtilis and Enterococcus faecium enteric-coated capsules for 4 weeks. Small intestinal bacteria overgrowth (SIBO) was measured by lactulose hydrogen breath test, and the microbial DNA was extracted from the fecal samples and the bacteria were classified by 16S rDNA gene amplicon sequencing. Results Twenty-five patients of each group were recruited, and there was no significant difference between the probiotics and control groups on baseline gastrointestinal symptom rating scale (GSRS), positive rate of SIBO, and relative abundances of the gut microbiota at the phylum level. After 4 weeks of treatment, the values of the probiotics and control groups were as follows: GSRS 1.4 ± 1.4 and 3.6 ± 1.6 and positive rate of SIBO 28.0% and 56.0%, respectively. The median relative abundances of the gut microbiota were 1.01% and 5.03% Actinobacteria and 43.80% and 35.17% Bacteroidetes at the phylum level; 0.76% and 3.29% Bifidobacterium, 0.13% and 0.89% Cillinsella, 0.03% and 0.01% Enterococcus, 0.18% and 0.36% Lachnospiraceae, 0.10% and 0.16% Ruminococcus torques group, 1.31% and 2.44% Blautia, and 0.83% and 2.02% Fusicatenibacter at the genus level (P < 0.05), respectively. Conclusion Combined live probiotic supplementation after the bowel preparation can alter the gut microbiota, decontaminate SIBO, and alleviate the gastrointestinal symptoms of FBDs. Tu1171 INCIDENCE OF ADVERSE DRUG REACTIONS OF POLYETHYLENE GLYCOL ELECTROLYTES POWDER: THE ANALYSIS OF REPORTS IN CHINA

Since 2018, the National Medical Products Administration of China has issued a notice on the Direct Reporting of Adverse Drug Reactions (ADRs) by Marketing Authorization Holders (MAH), which has ushered in a new era in the reporting of ADRs in China. Objective: Niflec (Polyethylene Glycol Electrolytes Powder, PEG-ELP) is one of the most widely used oral intestinal lavage preparation agents in China. The purpose of this study is to analyze the incidence of Niflec ADRs reported in the database of ADR monitoring system in the first six months after the MAH reporting policy, and to evaluate the clinical safety of Niflec. The reports of ADRs caused by Niflec were screened out from the database of ADR monitoring system, and they were analyzed by gender, age, primary disease, concomitant drugs, occurrence time, history of allergy, ADR types, causality, and severity. From December 15, 2018 to June 15, 2019, thirty-two ADR cases were reported, including one serious adverse event. The adverse reactions were more in patients aged 60 and over (50.0%) and more in female (71.9%). Only one patient had a clear history of drug allergy. The main adverse reactions occurred in the gastrointestinal system (53.1%) or skin and mucous membrane (34.4%). The top three manifestations were vomiting, nausea and rash. Most of the ADRs were observed within 30 minutes after the first use, accounting for 37.5%. Concomitant drugs were taken by 9.4% of the patients. The one serious adverse event was intestinal obstruction with shock. According to Pharmaceutical DataBase (PDB) data, the market size of Niflec in China was approximately 2 million bags in the first and second quarter of 2019. The number of drug users was estimated to be 679,896. Based on the above analysis, the overall incidence of ADR of Niflec was relatively low, less than 5 per 100,000 patients. The total incidence of serious ADR was less than 1.5 per million, which was very rare. All patients recovered or improved after proper treatments. The safety of PEG-ELP is confirmed by this study. However, it is still necessary to pay attention to the correct preparation and proper use. The bowel preparation personnel should be alert to monitor ADRs, especially on patients aged 60 and over, and take corresponding measures if any of these reactions are encountered.

Ultrasonic Chinese herbal medication penetration therapy at Tiānshū (天枢 ST25) for opioid-induced constipation with liver stagnation and spleen deficiency

ObjectivesTo observe the clinical effect on opioid-induced constipation of liver stagnation and spleen deficiency treated with the ultrasonic penetration of the No.1 constipation herbal formula at Tiānshū (天枢 ST25). MethodsA total of 126 patients that were in compliance with the inclusion criteria were randomized into an acupoint group (41 cases), a Chinese medication group (42 cases) and a western medication group (43 cases). In the acupoint group, the ultrasonic penetration of the No.1 constipation herbal formula at bilateral Tiānshū (天枢 ST25) were given. In the Chinese herbal medication group, the No.1 constipation herbal formula was prescribed for oral administration. In the western medication group, polyethylene glycol electrolyte powder was prescribed for oral administration. The treatment was given once a day, for 14 days totally. Before and after treatment, the constipation symptom score, the score of the patient assessment of constipation quality of life (PAC-QOL), the situation of nitric oxide synthetase (NOS) in serum were observed in each group. The safety indicators in the observation included diarrhea, allergic reaction, liver dysfunction, skin lesions, etc. ResultsA total of 122 cases accomplished the trial. The total effective rate was 82.5% in the acupoint group, 80.48% in the Chinese medication group and 78.04% in the western medication group. The constipation symptoms were all relieved in the three groups, indicating the statistical significance as compared with those before treatment (P < 0.01, P < 0.05). In the acupoint group and the western medication group, the improvements in the three symptom items, i.e. distention or scurrying pain in the chest, hypochondriac region or the bilateral sides of the abdomen, depression or irritability and poor appetite, were better than the western medication group (P < 0.01, P < 0.05). The improvements in all of the symptoms in the acupoint group were better than the Chinese medication group (all P < 0.05). The PAC-QOL score was improved in each group after treatment and it was different significantly in statistics as compared with that before treatment in each group. After treatment, the improvement of PAC-QOL score in the acupoint group and the Chinese medication group was better than that in the western medication group (both P < 0.05), and the improvement in the acupoint group was better than the Chinese medication group (P < 0.05). As to serum NOS, after treatment, it was different significantly as compared with that before treatment in each group (all P < 0.05), the improvement in the acupoint group was better than that of either the Chinese medication group or the western medication group, indicating the statistical significance (both P < 0.05). The incidence of adverse reaction in the acupoint group was less and different significantly as compared with the western medication group (P < 0.01), and it was different statistically as compared with the Chinese medication group (P < 0.05). ConclusionThe ultrasonic penetration of the No.1 constipation herbal formula at Tiānshū (天枢 ST25) is effective on opioid-induced constipation of liver stagnation and spleen deficiency. This therapy achieves the significant therapeutic effect and presents the good compliance in treatment. Hence, it deserves to be further promoted in clinical application.

Application of Time-by-Time Oral Method for the Colon Disease Patients Who Use Polyethylene Glycol Electrolytes Powder (II) Treatment Method in Electronic Colonoscopy

Objective: To evaluate application of Time - by - time oral method for the colon disease patients who use Polyethylene Glycol Electrolytes Powder (II) treatment method in electronic colonoscopy. Methods: 100 patients were invested to join our study from January 2019 to March 2019. They use Polyethylene Glycol Electrolytes Powder (II) treatment method in electronic colonoscopy process. They were randomly assigned to control group (n=50) and intervention group (n=50). In intervention group, the participants receive time - by - time oral method when they use PGEP2. The control group participants use the PGEP2 by the usual way. The collected data include patient characteristics, effective cleanliness index, adverse reaction and drug tolerance, those data from our recording. In research process, we use the special questionnaires which is Boston Bowel Preparation Scale (BBPS). Result: The two different group participants have similar patient characteristics and effective cleanliness in study result from patient characteristics research and BBPS. In addition, the intervention group has lower abdominal distension rate than that of control group in result, it is statistical significance [1 (2% vs 17 (34%), p < 0.001]. In addition, the intervention group patients have better drug tolerance, the result is statistical significance [50 (100%) vs 44 (88%), p=0.035]. Conclusion: the time - by - time oral method is good for reduce the rate of adverse reaction, such as nausea and abdominal distension. Additionally, it can improve the drug tolerance, it shows more patients can take medicine on time in the process of using a drug.

Objective To explore the effects of graphic communication and education on cleanliness of intestinal preparation and detection rate of colonic polyposis in patients. Methods Totally 96 patients receiving colonoscopy in He'nan Provincial People's Hospital from May 2016 to May 2018 were selected by convenient sampling, and divided into the control group (n=48) and the observation group (n=48) . Patients in the control group took Polyethylene Glycol Electrolytes Powder orally for conventional intestinal preparation, while patients in the observation group received graphic communication and health on the basis of conventional intestinal preparation. Intestinal cleanliness, duration of colonoscopy, acceptance rate of repeated intestinal preparation and detection rate of colonic polyposis were compared between the two groups, and the incidence of adverse effects was analyzed. Results The good rate of intestinal cleanliness in the observation group was 91.67%, while that in the control group was 75.00%, and the difference was statistical significant (P<0.05) ; the duration of colonoscopy in the observation group was (7.24±2.16) min, shorter than that in the control group, which was (9.01±3.54) min, and the difference was statistical significant (P<0.05) ; the acceptance rate of repeated intestinal preparation and the detection rate of colonic polyposis in the observation group were 4.17% and 29.17%, whereas those of the control group were 18.75% and 12.50%, respectively, and the difference was statistical significant (P<0.05) ; the rate of adverse effects in the observation group was 14.58%, lower than that of the control group, which was 52.08%, and its incidence rate of abdominal distention and stomachache was lower than that of the control group, and the differences were all statistical significant (P<0.05) . Conclusions The graphic communication and education can improve the intestinal cleanliness and the detection rate of colonic polyposis by colonoscopy in patients. Effects of individualized preoperative bowel preparation regimen on intestinal cleansing in patients with cervical cancer

Objective To investigate the effects of individualized preoperative bowel preparation regimen on intestinal cleansing in patients with cervical cancer. Methods Totally 140 patients who were admitted and diagnosed with cervical cancer in the Department of Oncology of Wuxi Maternal and Child Care Hospital from January 2016 to May 2017 were selected by convenient sampling and randomly divided into a control group and an experimental group by drawing method. Finally, totally 67 patients in each group completed the experiment. Routine preoperative bowel preparation was adopted for patients in the control group, while individualized bowel preparation regimen was adopted for patients in the experimental group, including evaluation before medication, activity guidance for patients with different ages, control of medication speed and response to untoward effects. The intestinal cleansing, untoward effects, time of first defecation and total time of defecation were compared between the patients in the two groups. The rank-sum test, χ2 test and t test were used to analyze the data for the patients in the two groups. Results There was statistically significant difference in the degree of intestinal cleansing between the patients in the two groups (Z=-2.732, P<0.05) ; there was statistically significant difference in the degree of nausea and vomiting between the patients in the two groups (Z=-2.065, P<0.05) ; there was statistically significant difference in the degree of abdominal distension between the patients in the two groups (Z=-2.526, P<0.05) ; and there was statistically significant difference in the incidence of stomachache between the patients in the two groups (χ2=5.835, P<0.05) . The time of first defecation and the total time of defecation of the patients in the experimental group was shorter than that of the patients in the control group (t=-3.901, -6.689; P<0.01) . Conclusions The individualized preoperative bowel preparation for patients with cervical cancer can improve the intestinal cleansing, shorten the time of defecation and reduce untoward effects such as abdominal distension and stomachache in these patients. Effects of adaptive biofeedback training on long-term efficacy and family maintenance treatment in patients with outlet obstruction constipation

Objective To explore the effects of adaptive biofeedback training (ABF) on long-term efficacy and family maintenance treatment in patients with outlet obstruction constipation (OOC). Methods From October 2010 to April 2013, a total of 114 patients with OOC met Rome Ⅲ criteria were enrolled in this study. After six weeks ABF treatment, patients were divided into family maintenance treatment group and simple follow-up group. And then the scores of clinical constipation symptoms, weekly dosage of polyethylene glycol electrolyte powder, total clinical efficacy and satisfaction rate were recorded. T test and chi-square test were performed for statistical analysis. Results A total of 95 patients with OOC were enrolled in the follow-up study eventually, 42 cases in family maintenance treatment group and 53 cases in simple follow-up group. After six weeks ABF treatment, the scores of clinical constipation symptoms and weekly dosage of polyethylene glycol electrolyte powder decreased in both two groups. At 36th month after six weeks ABF, in family maintenance treatment group, the score of clinical constipation symptoms and weekly dosage of polyethylene glycol electrolyte powder were 4.86±1.13 and (5.88±1.14) packs, respectively, which were both lower than those before treatment (11.05±3.26 and (15.28±4.12) packs, respectively), and the differences were statistically significant (t=2.520, P=0.041; t=3.080, P=0.042). At 36th month after six weeks ABF, in simple follow-up group, the score of clinical constipation symptoms and weekly dosage of polyethylene glycol electrolyte powder were (7.12±1.25) and (11.44±2.88) packs, compared with those before treatment (11.07±2.86 and (15.77±3.78) packs), the difference was not statistically significant (both P>0.05). At 36th month after six weeks ABF, in family maintenance treatment group, the total clinical efficacy and total satisfaction rates both were 71.4% (30/42), compared with those at the end of treatment (88.1% (37/42), 83.3% (35/42)), the difference was not statistically significant (both P>0.05). In simple follow-up group, at 12th month after six weeks ABF, the total clinical efficacy and satisfaction rates were 69.8% (37/53) and 66.0% (35/53), compared with those at the end of treatment (88.7%(47/53) and 84.9%(45/53)), the difference was not statistically significant (both P>0.05), at 24th months, the total clinical efficacy and satisfaction rates were 49.1% (26/53) and 47.2% (25/53), which were both lower than those at the end of treatment, and the differences were statistically significant (χ2 =1.762, P=0.036; χ2 =1.928, P=0.041). Conclusions The efficacy of ABF in the treatment of patients with OOC lasts less than two years. However, the family maintenance treatment can keep for three years, which may be an effective method to keep long-term efficacy in patients with OOC. Comparison of four different bowel preparation methods for colonoscopy

Objective To investigate the clinical effects of four different bowel preparation methods for colonoscopy. Methods 620 patients examined by coloscopy were randomized into four groups(group A, B, C and D), with each group in 155 cases.Group A received magnesium sulfate.Group B received compound polyethylene glycol.Group C received magnesium sulfate combined with simethicone.Group D received compound polyethylene glycol combined with simethicone.The quality and influencing factors, the detection of polyps and adverse events were compared between the four groups. Results The good bowel cleaning rate of group A, B, C and D were respectively 84.52%, 91.61%, 85.81% and 94.19%.The good bowel cleaning rates of group A and C were higher than those of group B and D (χ2=4.32, P<0.05; χ2=7.63, P<0.05; χ2=4.10, P<0.05; χ2=6.06, P<0.05.). The bubble free rate of group A, B, C and D were 86.45%, 87.74%, 92.26% and 90.96%.The bubble free rates of group C and D were higher than those of group A and B (χ2=4.17, P<0.05; χ2=4.76, P<0.05; χ2=5.02, P<0.05; χ2=4.23, P<0.05). The polyp detection rates of group A, B, C and D were 35.4%, 36.8%, 51.6% and 54.2%.Group C and D showed higher quality of foam eliminating and detection rate of polyps as compared with group A and B (χ2=8.24, P<0.05; χ2=11.04, P<0.05; χ2=6.95, P<0.05; χ2=9.53, P<0.05). The incidence of adverse reactions of group A and C (21.3% and 19.4%) were higher than group B and D (7.8% and 6.5%)(χ2=11.86, P<0.05; χ2=14.95, P<0.05; χ2=9.18, P<0.05; χ2=11.95, P<0.05). The frequency of colonoscopy, constipation, medication and examination time interval, bowel preparation methods and adverse events were the the influence factors of bowel preparation. Conclusion The quality of bowel preparation of compound polyethylene glycol electrolyte powder is better than magnesium sulfate with high security.Treatment of combination with simethicone can improve bowel preparation quality.Many factors can affect bowel preparation, which need to be paid attention to.
