Micronuclei analysis in mice peripheral blood exposed to polarized polychromatic noncoherent light (Bioptron® Light)

The increase in cancer in recent years suggests an inadvertent exposure to agents that cause genetic damage. The polarized polychromatic noncoherent light Bioptron® lamp is used to accelerate healing, among other therapeutic applications and its potential carcinogenic effects as a mitogenic agent have not been explored. The objective was to evaluate the genotoxicity of the Bioptron light therapy by means of the micronucleus assay in mouse erythrocytes. Male SKH1 hairless mice were randomly divided into six groups (5 mice/group); Group 1: negative control received ambient light; Group 2: positive control was exposed to ultraviolet light lamp A (UV-A) for 80 min; Experimental Groups 3–6 were exposed to the Bioptron lamp light for 10, 20, 40 and 80 min, respectively. Exposures in all groups were once a day for 4 days and blood smears were performed daily for 5 days and subsequently read with a microscope equipped with epifluorescence. The values of micronucleated erythrocytes (MNE), micronucleated polychromatic erythrocytes (MNPCE) and the proportion of polychromatic erythrocytes (PCE) were determined. The study group that received the UV-A light was the only one that increased MNE and MNPCE values, while in the groups exposed to the Bioptron lamp and the negative control did not show increases in any of the sampling days. In conclusion, under the conditions presented here, our results suggest that the light of the Bioptron lamp does not cause damage to the genetic material of SKH1 mice, by means of the micronucleus test in peripheral blood.

Polarized Polychromatic Noncoherent Light (Bioptron Light) as Adjunctive Treatment in Chronic Oral Mucosal Pain: A Pilot Study.

Objective: Aim of this study was to assess the efficacy of polarized polychromatic noncoherent light (Bioptron light) in the treatment of chronic oral mucosal pain (COMP). Patients and methods: Twenty-two patients affected by COMP were treated with standardized pharmacological protocols in association with Bioptron light (90 W; light wavelength = 480-3400 nm; degree of polarization = 95%; specific power density = 40 mW/cm2; energy density = 2.4 J/cm). The outcome measures were intensity of pain [measured by visual analog scale (VAS) score] and signs reduction (measured by Eisen score) recorded at baseline (t0), after 4 weeks (t1), and after 8 weeks (t2). Signs and symptoms scores were compared with those of a cohort of comparable patients selected from institutional medical record files. Results: Patients in pharmacological treatment associated with Bioptron showed a significant VAS score decrease at t1 and t2 (t0 = 6.9, t1 = 3.9, t2 = 1.8, p < 0.05), whereas the patients in exclusive pharmacological treatment showed a significant VAS score improvement only at t2. Comparing the VAS score at t1 and t2 in the two groups, a significant improvement was recorded in patients undergoing Bioptron adjunctive treatment (t1 = 3.9 vs. 5.9; p < 0.05 and t2 = 1.8 vs. 3.6; p < 0.05). In both groups Eisen score improved at t1 and t2, but in the Bioptron-treated patients the improvement was statistically better at t1 (1.9 vs. 0.8; p < 0.05) and at t2 (2.7 vs. 1.4; p < 0.05). Conclusions: In COMP patients, Bioptron use associated with pharmacological treatment allows a better and faster signs and symptoms reduction when compared with the exclusive pharmacological treatment. Further controlled studies are needed to establish the relative and absolute effectiveness of Bioptron in COMP management.

DN4 QUESTIONNAIRE IN FAMILY PRACTICE FOR EVALUATION OF CLINICAL MANIFESTATIONS OF NEUROPATHIC PAIN IN TYPE 2 DIABETES PATIENTS TREATED BY LIGHT THERAPY

Background. Peripheral diabetic polyneuropathy (DPN) is one of the most frequent neurological complications of diabetes mellitus (DM). Despite the large number of pharmacological agents, its treatment is not sufficiently effective, which necessitates the search for new therapies.Objective. The aim of the study was to increase the effectiveness of treatment of neuropathic pain in the patients with diabetic polyneuropathy by incorporating procedures using polarizing polychromatic non-coherent light (Bioptron light therapy) into the complex therapy of this disease.Methods. We examined 67 patients with type 2 diabetes complicated with diabetic polyneuropathy. Patients were divided into two groups: group 1 consisted of 32 patients, who received standard treatment; group 2 comprised 35 patients, who additionally underwent 12 light therapy treatments by means of the Bioptron Physiotherapy Unit. The evaluation of neuropathic pain intensity was performed using a modified questionnaire DN4. Results. A positive clinical effect of treatment was evidenced in both groups in 12 days of treatment. In 3 months, the intensity of complaints was significantly lower (p&lt;0.05) only in the group with additional use of polarizing light. In 6 months, the positive effect of the therapy was leveled in the patients of both groups.Conclusions. The use of the DN4 questionnaire with a modified scale for assessing the parameters of neuropathic pain can optimize its diagnosis. The light therapy procedures together with the standard complex therapy of diabetic polyneuropathy increase the clinical efficacy of neuropathic pain treatment and help to preserve the therapeutic effect within 3 months.

The effect of polarized polychromatic non-coherent light (Bioptron) therapy on patients with lower back pain

INTRODUCTION : Lately the polarized polychromatic non-coherent lower energy light emitted by the Bioptron lamp has become more widely used in the physiotherapeutic practice. AIM : To study the effect of polarized polychromatic incoherent low energy light on pain, motor activity and quality of life in patients with vertebrogenic lower back pain and to compare it with other methods used in practice. MATERIALS AND METHODS : Object of the study were 60 patients with lower back pain. A control group (N=30, age 46±11.5 y.) was treated with basic therapy and the Bioptron apparatus. Functional tests of the motor activity (test of Thomayer and test of Schober), evaluation of the pain using visual analogue scale (VAS) evaluation of the quality of life using the Roland-Morris test in the beginning, at the end and 1 month after the treatment were used to evaluate the effectiveness of treatment. RESULTS : The results have a statistical significance of (p<0.05) for both groups regarding the reduction of the pain syndrome, the functional tests for mobility in the lumbar region and the quality of life. There`s also a statistical significance in comparing the results of the test group (TG) and the control group (CG) (p<0.05). CONCLUSION : Both methods reduced pain, improved the mobility of the lumbar region and improved the quality of life of patients with lower back pain. The complex methodology incorporating basic treatment with the use of polarized polychromatic non-coherent low energy light yielded better results than just basic treatment.

Treatment of Carpal Tunnel Syndrome in pregnancy with Polarized Polychromatic Non-coherent Light (Bioptron Light): A Preliminary, Prospective, Open Clinical Trial.

The aim of this trial was to assess the efficacy of polarized polychromatic noncoherent light (Bioptron light) in the treatment of Carpal Tunnel Syndrome (CTS) in pregnancy. An uncontrolled experimental study was conducted in pregnant patients with CTS who visited our clinic from January 2006 to January 2010. Bioptron light (480-3400 nm; 95% polarization; 40 mW/cm2; and 2.4 J/cm2) was administered perpendicular to the carpal tunnel area. The irradiation time for each session was 6 min at an operating distance of 5-10 cm from the carpal tunnel area, twice each day, five days per week for 2 weeks. Pain and paraesthesia using a visual analogue scale (VAS) and finger pinch strength were evaluated at the end of treatment (week 2) and 1-month (week 6) after the end of treatment. The Student'sttest was used and p values < 0.05 were accepted as statistically significant. 46 patients participated in the study. The mean age of subjects was 27.6 years (range 22-37). The patient population had a mean duration of CTS of 2.3 months (range 1-4). All patients were in the third trimester. Pain and paraesthesia decreased at the end of treatment and at the 1-month follow-up, whereas the finger pinch strength increased at the end of treatment and at the 1-month follow-up. In conclusion, the results of the present study suggest that Bioptron light is a reliable, safe, and effective treatment option in pregnant patients with CTS. Controlled clinical trials are needed to establish the absolute and relative effectiveness of this intervention.

The effect of polarized polychromatic noncoherent light (bioptron) therapy on patients with carpal tunnel syndrome.

To study the effects of Polarized Polychromatic Noncoherent Light (Bioptron) therapy on patients with carpal tunnel syndrome (CTS). This study was designed as a randomized clinical trial. Forty four patients with mild or moderate CTS (confirmed by clinical and electrodiagnostic studies) were assigned randomly into two groups (intervention and control goups). At the beginning of the study, both groups received wrist splinting for 8 weeks. Bioptron light was applied for the intervention group (eight sessions, for 3/weeks). Bioptron was applied perpendicularly to the wrist from a 10 centimeter sdistance. Pain severity and electrodiagnostic measurements were compared from before to 8 weeks after initiating each treatment. Eight weeks after starting the treatments, the mean of pain severity based on Visual Analogue Scale (VAS) scores decreased significantly in both groups. Median Sensory Nerve Action Potential (SNAP) latency decreased significantly in both groups. However, other electrophysiological findings (median Compound Motor Action Potential (CMAP) latency and amplitude, also SNAP amplitude) did not change after the therapy in both groups. There was no meaningful difference between two groups regarding the changes in the pain severity. Bioptron with the above mentioned parameters led to therapeutic effects equal to splinting alone in patients with carpal tunnel syndrome. However, applying Bioptron with different therapeutic protocols and light parameters other than used in this study, perhaps longer duration of therapy and long term assessment may reveal different results favoring Bioptron therapy.

Electrophysiological parameters in patients with the «dry» form of age-related macular degeneration treated with polarized polychromatic non-coherent light and dynamic electromyostimulation

Aim. To study the electrophysiological parameters in patients with the «dry» form of age-related macular degeneration before and after combined treatment with polarized polychromatic non-coherent light and dynamic electromyostimulation. Methods. Two groups of patients with the «dry» form of age-related macular degeneration were examined. Patients in the main group (40 patients, 80 eyes) were treated with a combination of polarized polychromatic non-coherent light and dynamic electromyostimulation, patients in the control group (39 patients, 78 eyes) were treated conventionally. Mean age of the patients was 71.3±1.1 years. All patients underwent standard ophthalmic examination. To assess the functional status of the macula, electroretinography was performed using the «Neuro-MVP» device (Ivanovo, Russia). A-wave amplitude and latency on the red light stimulus were examined. Results. A-wave amplitude on macular electroretinogram was 11.9% lower and A-wave latency was 20.8% higher in patients with age-related macular degeneration compared to healthy subjects. A statistically significant improvement of the A-wave amplitude and latency was observed on macular electroretinogram after the combined treatment. A-wave amplitude increased from 13.22±1.36 to 17.17±1.59 µV (p 0.01) after 2 months and up to 17.80±1.60 µV (p 0.05) after 6 months of treatment. A-wave latency decreased from 19.32±0.41 to 18.51±0.36 ms (p 0.01) after 2 months and down to 18.31±0.45 ms (p 0.05) after 6 months of treatment. There was a relevant increase in A-wave amplitude from 13.15±1.09 to 16.3±1.09 µV (p 0.01) and decrease in A-wave latency from 19.23±0.30 to 18.64±0.30 ms (p 0.05) in patients of the control group 2 months after the end of treatment. Conclusion. There were relevant increase in A-wave amplitude and decrease in A-wave latency observed on macular electroretinography after treatment. The proposed treatment method does not cause any complications and can be used as a treatment option in patients with allergic reactions to standard drugs.

Change of electrical impedance measurements on brain in patients with the «dry» form of age-related macular degeneration treated with polarized polychromatic non-coherent light and dynamic electromyostimulation

Aim. To study the change of electrical impedance measurements in brain in patients with the «dry» form of age-related macular degeneration before and after combined treatment with polarized polychromatic non-coherent light and dynamic electromyostimulation. Methods. Two groups of patients with the «dry» form of age-related macular degeneration were examined. 40 patients (80 eyes) from the main group were treated using combined treatment and 39 patients (78 eyes) in the group of control who were treated conventionally. Results. Before the treatment, decreased blood flow in the internal carotid artery was found in 77.2%, and in the vertebral arteries in 82.2% of patients. Peak amplitude and the stress ratio index at rheoencephalography in the anterior cerebral circulation were 0.83±0.04 Ω and 27.46±0.40%, in the posterior cerebral circulation - 0.70±0.04 Ω and 23.78±0.35%. Peak amplitude values and the stress ratio index have improved significantly right after treatment and after 6 months in the main group compared to the control group. Mean visual acuity in the main group before treatment was 0.87±0.02, right after the treatment - 0.96±0.01 (p 0.001), 2 months after the treatment - 0.95±0.01 (p 0.001), 6 months after the treatment - 0.96±0.01 (p 0.001), 12 months after the treatment - 0.95±0.01 (p 0.001). In the control group mean visual acuity was 0.91±0.02 before the treatment, 0.95±0.02 (p 0.05) - right after the treatment. 2 months after the treatment - 0.94±0.02 (p 0.05), 6 months after the treatment - 0.92±0.02 (p 0.05). 12 months after the treatment mean visual acuity decreased and equaled 0.89±0.02. Decrease of anterior cerebral circulation was found in 77.2% of patients with the «dry» form of age-related macular degeneration, mean peak amplitude value was 0.83±0.04 Ω and the stress ratio index was 27.46±0.40%, decrease of posterior cerebral circulation was found in 82.2% of patients, mean peak amplitude value and the stress ratio index were 0.70±0.04 Ω and 23.78±0.35%. Conclusion. In patients with the «dry» form of age-related macular degeneration posterior cerebral circulation has improved significantly and visual acuity has improved 6 months after combined treatment with polarized polychromatic non-coherent light and dynamic electromyostimulation.

Blood antioxidant capacity change in patients with the «dry» form of age-related macular degeneration treated with polarized polychromatic non-coherent light and dynamic electromyostimulation

Aim. To assess the antioxidant capacity in patients with the «dry» form of age-related macular degeneration before and after combined treatment with polarized polychromatic non-coherent light and dynamic electromyostimulation. Methods. Two groups of patients with the «dry» form of age-related macular degeneration were examined. 40 patients (80 eyes) from the main group were treated using combined treatment and 39 patients (78 eyes) in the group of control who were treated conventionally. The integral blood antioxidant capacity was measured by galvanometry. Results. Blood antioxidant capacity in healthy subjects is 26.0 kC/l. In patients of the main group blood antioxidant capacity before the treatment was equal to 22.81±0.27 kC/l, compared to 22.17±0.20 kC/l in control group (total mean value 22.49±0.27 kC/l). There was a relevant elevation of the blood antioxidant capacity at the late stages after the treatment - up to 12 months, compared to only 2 months in the control group. Visual acuity in patients of the main group before the treatment was 0.87±0.02, after the treatment was finished - 0.96±0,01 (p 0.001), 2 months after the treatment - 0.95±0.01 (p 0.001), 6 months after the treatment - 0.96±0.01 (p 0.001), 12 months after the treatment - 0.95±0.01 (p 0.001). Visual acuity in patients of the control group before the treatment was 0.91±0.02, after the treatment was finished - 0.95±0.02 (p 0.05), 2 months after the treatment - 0.94±0.02 (p 0.05), 6 months after the treatment - 0.92±0.02 (p 0.05). 12 months after the treatment the visual acuity deteriorated compared to the treatment start and was measured as 0.89±0.02. Conclusion. There is a decrease of blood antioxidant capacity (22.49±0.27 kC/l) in patients with the «dry» form of age-related macular degeneration; a relevant increase of blood antioxidant capacity and visual acuity can be observed up to 12 months after the treatment with polarized polychromatic non-coherent light and dynamic electromyostimulation.

Low Level Light Could Work on Skin Inflammatory Disease: A Case Report on Refractory Acrodermatitis Continua

Low level laser or light treatment on the various clinical condition is getting considerable attention now. However, there has been no report about the clinical effect of low level polarized polychromatic noncoherent light (LPPL) on the inflammatory skin disease. We experienced a case of acrodermatitis continua in a pregnant woman refractory to any conventional treatment including the most potent topical steroid. She was successfully treated with LPPL. LPPL could be a possible treatment modality producing substantial clinical result in inflammatory skin condition without any side-effect.

Comparing the Effects of Exercise Program and Low-Level Laser Therapy with Exercise Program and Polarized Polychromatic Non-coherent Light (Bioptron Light) on the Treatment of Lateral Elbow Tendinopathy

The use of low-level laser therapy (LLLT) and polarized polychromatic non-coherent light as supplements to an exercise program has been recommended for the management of lateral elbow tendinopathy (LET). To investigate whether an exercise program supplemented with LLLT is more successful than an exercise program supplemented with polarized polychromatic non-coherent light in treating LET. Patients with unilateral LET for at least 4 wk were sequentially allocated to receive either an exercise program with LLLT or an exercise program with polarized polychromatic non-coherent light. The exercise program consisted of eccentric and static stretching exercises of wrist extensors. In the LLLT group a 904-nm Ga-As laser was used in continuous mode, and the power density was 130 mW/cm(2), and the dose was 0.585 J/point. In the group receiving polarized polychromatic non-coherent light the Bioptron 2 was used to administer the dose perpendicularly to the lateral epicondyle at three points at an operating distance of 5-10 cm for 6 min at each position. The outcome measures were pain and function and were evaluated at baseline, at the end of the treatment (week 4), and 3 mo after the end of treatment (week 16). Fifty patients met the inclusion criteria. At the end of treatment there was a decline in pain and a rise in function in both groups compared with baseline (p < 0.0005 on the paired t-test). There were no significant differences in the reduction of pain and the improvement of function between the groups at the end of treatment and at the 3-mo follow-up (p > 0.0005 on the independent t-test). The results suggest that the combination of an exercise program with LLLT or polarized polychromatic non-coherent light is an adequate treatment for patients with LET. Further research to establish the relative and absolute effectiveness of such a treatment approach is needed.

Comparison of effects of Cyriax physiotherapy, a supervised exercise programme and polarized polychromatic non-coherent light (Bioptron light) for the treatment of lateral epicondylitis

Objective: To compare the effectiveness of Cyriax physiotherapy, a supervised exercise programme, and polarized polychromatic non-coherent light (Bioptron light) in the treatment of lateral epicondylitis. Design: Controlled clinical trial. Setting: Rheumatology and rehabilitation centre. Subjects: This study was carried out with 75 patients who had lateral epicondylitis. They were allocated to three groups by sequential allocation. Interventions: Group A (n=25) was treated with Cyriax physiotherapy. A supervised exercise programme was given to group B (n=25). Group C (n=25) received polarized polychromatic non-coherent light (Bioptron light). All patients received three treatments per week for four weeks. Outcomes: Pain was evaluated using a visual analogue scale and function using a visual analogue scale and pain-free grip strength at the end of the four-week course of treatment (week 4), one month (week 8), three months (week 16) and six months (week 28) after the end of treatment. Results: The supervised exercise programme produced the largest effect in the reduction of pain and in the improvement of function at the end of the treatment (P<0.05) and at any of the follow-up time points (P<0.05). Conclusion: The supervised exercise programme should be the first treatment option for therapists when they manage lateral epicondylitis patients. If this is not possible, Cyriax physiotherapy and polarized polychromatic non-coherent light (Bioptron light) may be suitable.

Treatment of Carpal Tunnel Syndrome with Polarized Polychromatic Noncoherent Light (Bioptron Light): A Preliminary, Prospective, Open Clinical Trial

Our aim was to assess the efficacy of polarized polychromatic noncoherent light (Bioptron light) in the treatment of idiopathic carpal tunnel syndrome. Carpal tunnel syndrome is the most common compression neuropathy, but no satisfactory conservative treatment is available at present. An uncontrolled experimental study was conducted in patients who visited our clinic from mid-2001 to mid-2002. A total of 25 patients (22 women and three men) with unilateral idiopathic carpal tunnel syndrome, mild to moderate nocturnal pain, and paraesthesia lasting >3 months participated in the study. The average age of the patients was 47.4 years and the average duration of patients' symptoms was 5.2 months. Polarized polychromatic noncoherent light (Bioptron light) was administered perpendicular to the carpal tunnel area. The irradiation time for each session was 6 min at an operating distance of 5-10 cm from the carpal tunnel area, three times weekly for 4 weeks. Outcome measures used were the participants' global assessments of nocturnal pain and paraesthesia, respectively, at 4 weeks and 6 months. At 4 weeks, two patients (8%) had no change in nocturnal pain, six (24%) were in slightly less nocturnal pain, 12 (48%) were much better in regard to nocturnal pain and five (20%) were pain-free. At 6 months, three patients (12%) were slightly better in regard to nocturnal pain, 13 (52%) were much better regarding nocturnal pain, and nine patients (36%) were pain-free. At 4 weeks, four patients (16%) had no change in paraesthesia, five (20%) were slightly better, 13 patients (52%) were much better, and three patients (12%) were without paraesthesia. At 6 months, two patients (8%) had no change in paraesthesia, two (8%) were slightly better, 14 (56%) were much better, and seven (28%) were without paraesthesia. Nocturnal pain and paraesthesia associated with idiopathic carpal tunnel syndrome improved during polarized polychromatic noncoherent light (Bioptron light) treatment. Controlled clinical trials are needed to establish the absolute and relative effectiveness of this intervention.

The Use of Polarized Polychromatic Non-coherent Light as Therapy for Acute Tennis Elbow/Lateral Epicondylalgia: A Pilot Study

The aim of this study was to assess the efficacy of polarized, polychromatic, non-coherent, low energy light (Bioptron 2, Bioptron AG, Switzerland) in the treatment of acute tennis elbow. Tennis elbow, or lateral epicondylitis, is one of the most common lesions affecting the arm. A plethora of treatment regimes have been described for this condition, but no specific therapy has emerged as a gold standard. A pilot study was carried out with 25 patients who had acute tennis elbow. Bioptron 2 device was applied over lateral epicondyle three times per week for 4 weeks. Pain on VAS, function on VAS, and painfree grip strength were measured at the beginning (week 0) and at the end of the study (week 4). The pain on VAS was reduced at the end of treatment (t(24) = 3.84, p = 0.001). Function on VAS was increased at the end of treatment (t(24) = 4.23, p < 0.001). Pain-free grip strength was increased at the end of treatment (t(24) = 4.23, p < 0.004). Although the results suggested that the Bioptron 2 could reduce patients' symptoms with acute tennis elbow, future controlled studies are needed to establish the relative and absolute effectiveness of Bioptron 2.

The use of polarised polychromatic non-coherent light alone as a therapy for venous leg ulceration

This study assessed the effectiveness of polarised, polychromatic, non-coherent light therapy in the treatment of venous leg ulcers. Investigators in previous studies have advocated the use of light as an adjunct to other proven therapies or on its own. This was a pilot prospective case-series study. We enrolled 25 patients with venous leg ulcers. All were treated with light only. Phototherapy (light therapy) treatments were given once a day over four weeks. All ulcers except one (99%) had a positive value for the change in healing area at the end of the four weeks. The total number of 73 leg ulcers recorded at the beginning of the study was reduced to 51 at the end of the four weeks (p < 0.01). The decrease in wound surface area following the treatment was statistically significant (mean: 57.15%; SD: 31.87%; p < 0.01). Polarised, polychromatic light therapy applied as a monotherapy was associated with positive healing rates in patients with venous leg ulcers. It is a simple and non-invasive treatment. However, a well-designed randomised controlled study is needed to confirm the efficacy of this form of phototherapy and to objectively evaluate recommendations for its routine use in clinical practice.
