ABSTRACT This study explored rural foster caregivers' perceptions of telemental health (TMH) services for children in foster care in a Southeastern state. Through fourteen Zoom interviews and thematic analysis, we identified key facilitators and barriers, guided by the Unified Theory of Acceptance and Use of Technology 2 framework. Facilitators included provider engagement, ease of technology use, convenience, foster parent advocacy, reliable tech access, child willingness, cost savings, and tech familiarity. Barriers encompassed therapeutic challenges, technology issues, privacy concerns, child's lack of motivation, and policy hurdles. Addressing these multilevel factors can enhance TMH access and well-being for rural children in foster care.
