Comparative study of holmium laser versus cold-knife optical internal urethrotomy in urethral stricture

Objective: One of the most difficult urological ailments is to cure the urethral stricture adequately. There are various methods employed for the treatment of urethral stricture. This analysis was conducted to compare the efficacy and safety of holmium laser versus cold-knife optical urethrotomy for operative time, complications, and recurrence. Methods: The study design is a comparative study. The study participants were enrolled after procuring voluntary informed consent. Fifty-six patients were included in the study from the Urology department Super Speciality Hospital, GMC, Jammu from 1 November 2017 to 31 October 2018 and were divided into Group 1 (holmium group) and Group 2 (cold-knife group). Patients were catheterised with 14 Fr Foley’s catheter after the procedure and the catheter was removed on the third postoperative day. Patients were followed for 6 months. At each follow-up physical examination and uroflowmetry were performed. On the 90th post-operative day, a retrograde urethrogram was performed. Results: Both the cold knife and holmium laser have been safe and effective in providing immediate relief to patients with single and short-segment (< 1 cm long) urethral strictures. The cold knife offers a significant advance over the holmium laser in terms of the operative time, which was shorter in the cold-knife group. The success rates for laser and cold knife were similar. Conclusion: The success rates for laser and cold knife were very similar, with the groups having a similar trend in success rates at different follow-up time points. The present findings may benefit from long-term follow-up analysis. Level of evidence: 4

Abstract Background Urethral stricture is a narrowing of the urethra due to many causes including, trauma, ischaemia, spongiofibrosis, inflammation, and sexually transmitted gonococcal infections, resulting in tissue scaring in the corpus spongiosum, It can occur at any point on the male urethra. Regarding urethral stricture management, many researchers and surgeons have proposed and evaluated several methods, including urethral dilation, direct vision internal urethrotomy, laser urethrotomy and urethroplasty. Aim to compare the outcomes of Holmium: yttrium-aluminium-garnet (YAG) laser urethrotomy with the conventional cold-knife technique as regards treatment outcome, efficacy and complications in cases of urethral stricture disease. Methods This prospective randomized clinical trial was conducted at Ain Shams University hospitals and Nile hospital for insurance. This study was conducted on 30 male patients with urethral stricture assigned into two equal groups; Group A: received urethrotomy with conventional cold knife. Group B received urethrotomy with Holmium: YAG laser. Results Holmium Laser modality was associated with significantly lower postoperative postvoid residual urine volume and International Prostate Symptom Score (IPSS) at 1 & 3 months postoperative and shorter operative time but there was insignificant difference as regards peak flow rate(PFR). Complications in both procedures were minor and infrequent Conclusion Holmium Laser and cold knife internal urethrotomy are effective surgical options for the treatment of anterior urethral stricture with a promising outcome after 3- months follow-up with a better success rate using the Holmium laser. Comparative analysis of holmium: YAG laser internal urethrotomy versus Cold-Knife optical internal urethrotomy in the management of urethral stricture - a systematic review and meta-analysis.

Urethral stricture management has posed enduring challenges in urology, demanding innovative and effective treatment modalities. This study addresses the persistent issues associated with urethral strictures, focusing on the comparative evaluation of two contemporary treatment modalities: Ho: YAG Laser Internal Urethrotomy (HIU) and Cold Knife Optical Internal Urethrotomy (CIU). A comprehensive search of PubMed, Embase, Web of Science, Cochrane Library, and ResearchGate was conducted to identify relevant studies up to November 2023. Inclusion criteria encompassed comparative studies evaluating HIU and CIU in patients with urethral stricture. Data extraction, quality assessment, and subgroup analyses were performed using standardized methods. Outcome measures included preoperative and postoperative mean the maximum urine flow rate (Qmax), mean surgical time, recurrence rates, and perioperative complications. Nine articles met the inclusion criteria, and their data were analyzed using RevMan 5.4.1. Forest plots were generated for preoperative and postoperative mean Qmax, mean surgical time, recurrence rates, and perioperative complications. While postoperative mean Qmax (MD -0.06; 95% CI: -0.28-0.16; P =0.60, I2 =75%) and mean surgical time (MD 2.16; 95% CI: -1.66-5.99; P =0.27, I2 =98%) showed no significant differences between HIU and CIU, a trend towards lower recurrence rates (RR 0.71; 95% CI: 0.48-1.06; P =0.09, I2 =46%) was observed with HIU but without statistical significance. Perioperative complications, particularly bleeding (RR 0.21; 95% CI: 0.08-0.53; P =0.001, I2 =0%), favored HIU over CIU. The subanalysis indicates that for the treatment of complex urethral strictures, the two surgical methods differ in terms of mean Qmax at 6 months postoperatively (MD -2.51; 95% CI: -4.10--0.91; P =0.002, I2 =59%) and 12 months postoperatively (MD 2.62; 95% CI: 0.93-4.30; P =0.002, I2 =0%). The HIU group shows a significant decrease in recurrence rate at 12 months postoperatively (RR 0.44; 95% CI: 0.21-0.92; P =0.03, I2 =0%). For short-segment urethral strictures with a length ≤1.5cm, CIU had a shorter operative time compared to the HIU (MD 4.49; 95% CI: 3.87-5.10; P <0.00001, I2 =44%). Overall, both interventions demonstrated similar efficacy in improving postoperative mean Qmax, mean surgical time, and recurrence rates. However, subanalysis indicates that in the short-term, CIU is more effective than HIU in improving Qmax after complex urethral stricture surgery. In long-term Qmax follow-up after surgery, HIU is significantly higher than CIU, and HIU has a low recurrence rate. In addition, for short-segment urethral stricture (≤1.5cm), CIU requires less time. In terms of complications, HIU has a lower risk of bleeding.

Outcome of Urethroplasty in Management of Urethral Stricture: Experience at Institute of Kidney Diseases Hayatabad Peshawar

Objective: To determine the frequency of successful outcomes of urethroplasty in the management of urethral stricture Methods: It is a descriptive study, conducted in the department of Urology at the Institute of Kidney diseases Peshawar from April 2016 to December 2019. A total number of 85 patients for urethroplasty were included in the study by non-probability convenient sampling. The entire pre-operative, per-operative, and postoperative data were collected on structured proforma and analyzed on SPSS®, version 20.0. Results: The mean age of the patient in this study was 41 years. Regarding etiology, External trauma was the major cause in the form of road traffic accidents in 38.82% while straddle urethral injury in 29.41% of patients, other less frequent causes included iatrogenic urethral trauma and infective stricture. The bulbar urethral stricture was found in 55.29% of cases, with proximal penile and bulboperineal accounting for 23.52% and 21.17% respectively. Significant past surgical history found was suprapubic catheterization in 100%, a failed primary attempt of internal optical urethrotomy (IOU) in 29.41%, multiple recurrences in 48.23%, and re-do urethroplasty in 10.58% of cases. The most common procedure performed was end-to-end anastomotic urethroplasty in 81%, other procedures included penile flap urethroplasty, pull-through procedure, and 10.58% of cases required pubectomy. Urethroplasty was found highly successful in 94% of mean follow-ups 15 months. The cause of failure was wound infection in 1, UTI in 2, and necrosis of penile skin flap in 2 patients. Conclusion: Our initial results conclude that anastomotic urethroplasty is an effective and mainstay treatment modality in the management of urethral stricture disease. COMPARISON OF RECURRENCE OF URETHRAL STRICTURE AFTER INTERNAL OPTICAL URETHROTOMY WITH AND WITHOUT POST-OPERATIVE ORAL STEROIDS IN DISTRICT DERA ISMAIL KHAN

Urethral stricture is one of the most common diseases in urology. The urethral structure is the narrowing of the urethral lumen due to spongiofibrosis. It is notorious for its recurrence and accounts for the major burden on the Urology department. Internal optical urethrotomy is considered the first line of treatment for urethral stricture. When combined with clean intermittent self-catheterization, and Intralesional injections, better results could be obtained. This study compares the recurrence of urethral stricture after internal optical urethrotomy with and without post-operative oral steroids in the population of Dera Ismail Khan. The study was a non-randomized control trial done in the Urology Department, DHQ/MTI-TH, Dera Ismail Khan, Pakistan, from January 2021 to June 2021. The patients from the urology outdoor and emergency of DHQ Teaching Hospital were included in the study. A total of 152 patients were divided by lottery method into two groups’ an experimental group and Control group, each containing 76 patients. After losing follow-up, the remaining patients who completed the study were 128. The patients in both groups underwent internal optical urethrotomy with a cold knife. The experimental group of patients received oral steroids for 4 weeks. The control group didn’t receive steroids after the procedure. The mean age in the experimental group was 41 ± 09 years, while in the control group, it was 36 ± 13 years. After 6 months, the recurrence was observed in 12 patients (18.75%) in the experimental group and 25 in the control group (39.06%). These results showed a statistically significant difference in recurrence rate between these two groups with a p-value of 0.0123. Based on the results, it can be concluded that the use of oral steroids after internal optical urethrotomy decreases the risk of recurrence of urethral stricture significantly.

Minimally invasive techniques of urethral stricture treatment and their use in patients of pediatric age (literature review)

Urethralstricture remains one of the most discussed problemsin pediatric urology. Despite a significant number of articles and the existence of a wide variety of methods for treating urethral strictures and their modifications, every new generation of urologists encounters this problem.Treatment of urethralstricture often takes a long time and requires multiple surgical interventions. Treatment techniques continuously improve, therefore it is appropriate to regularly summarize new data. Such methods as bougienage, balloon dilation and urethralstenting are still being used, although as early asin the beginning of the 19th century the first types of such modern method as optical internal urethrotomy were developed. In relation to optical internal urethrotomy, discussions on indications, techniques, duration of catheterization, necessity of combination with intermittent catheterization, use of hormonal drugs continue. The question of the number of attempts to perform urethrotomy also remains controversial.In this review, we tried to clarify the problem of use of minimally invasive techniques for urethral stricture treatment in children in the context of current scientific data and historical aspects.

Outcome of Oral Steroids After Optical Internal Urethrotomy (OIU) In Comparison eith OIU Only

Objectives: To evaluate the effects of Oral Steroid as Tablet Deflazacort(6mg) in the prevention of recurrence of short segment anterior urethral stricture after optical internal urethrotomy compare with OIU only. Material and methods: This present Randomized clinical comparative study was designed to observe the efficacy of Oral Steroid as Tablet Deflazacort (6mg) after optical internal urethrotomy. All male patients of age between 18-45 years, who were diagnosed as short segment anterior urethral stricture(&lt;1.5cm) and planned for optical internal urethrotomy, were divided into two groups after fulfilling selection criteria. After informed consent and random allocation by lottery, patients under group A were given oral steroid as Tab. Deflazacort (6mg) twice daily for 2weeks then once daily for another 2weeks after catheter removal on 7th post-operative day following optical internal urethrotomy, while patients under group B were managed by optical internal urethrotomy only. 26 patients were enrolled in group A and 25 patients in group B. In group A, 4 patients and group B, 3 patients were lost during follow up. Hence total of 44 patients completed the study with 22 patients in each group, during the period of January 2018 to December 2018. Result: In present study, there were no significant differences found in age, preoperative American urological association (AUA) score, preoperative maximum urinary flow rate, and preoperative PVR between two groups. Postoperative comparison of AUA score at 6th month follow up between two groups showed the decrease in AUA score in group A in compare to group B and the difference was statistically significant (5.8±5.1 versus 10.9±7.1, p=0.009). Maximum flow rates at 3rd month (23.7±5.8 ml/sec versus 20.0±2.9 ml/sec( p= 0.010) and 6th month (20.0±5.2 ml/sec versus 15.9±5.8 ml/sec, p=0.018) follow up were significantly higher in group A than group B. Post void residual urine were significantly decrease in group A than group B (21.5±26.5 ml versus 42.0±30.9 ml, P=0.023). Study also showed a significant reduction in recurrence of anterior urethral stricture of 3(13.6 %) in the group A versus 9 (40.9% ) in the group B (p=0.042). Conclusion: Use of the Oral steroid after optical internal urethrotomy in preventing recurrence of short segment anterior urethral strictures were effective. Bangladesh J. Urol. 2023; 26(1): 22-27

Effect of Clean Intermittent Self-Catheterization by Frequency After Optical Internal Urethrotomy For Primary Bulbar Urethral Stricture

Background: The concept of clean intermittent self-catheterization (CISC) after optical internal urethrotomy has been regarded as preventive management for bulbar urethral stricture recurrence. Starting date of CISC, total duration of CISC, catheter size and material, and lubricating preparations for the catheter of CISC after OIU have been investigated in several studies to correlate and modify stricture recurrence. But doing CISC by an optimal frequency is still an unsettled issue. Some studies recommended doing this preventive procedure more frequently and some preferred a less intense course of frequency. But patients may find it difficult to follow a daily self-catheterization frequency. Moreover, effect and complications of doing clean intermittent self-catheterization after optical internal urethrotomy has been investigated insufficiently for primary bulbar urethral stricture in existing literatures. So, this study investigated whether CISC biweekly was more beneficial than daily after OIU for primary bulbar urethral stricture.Objective: To compare the effect of doing clean intermittent self-catheterization twice weekly and daily after optical internal urethrotomy for primary bulbar urethral stricture.Materials and methods: This was a hospital based quasi-experimental study among study samples who were selected for optical internal urethrotomy with diagnosed primary bulbar urethral stricture of ≤1.5 cm in length and conducted from November, 2020 to February, 2022 in the Department of Urology, Bangabandhu Sheikh Mujib Medical University. Fifty four study subjects were selected by purposive sampling. After first postoperative week of optical internal urethrotomy, patients attended to learn clean intermittent self-catheterization with a 14 Fr Nelaton Polyvinyl chloride catheter on outdoor basis. They were divided into two groups: Group-A did CISC twice weekly and Group-B practiced CISC daily. Both groups were instructed to do CISC for 6 months. Any patient reported complications associated with CISC (UTI, epididymo-orchitis, per-urethral bleeding, hematuria, urethral pain) were evaluated and treated during this period. After completion of 6 months, both groups had stopped doing CISC. Out-patient follow–up had been performed in both groups at 3 and 7 months to compare International Prostate Symptom Score (IPSS), maximum urinary flow rate (Qmax) by uroflowmetry, post void residual urine (PVR) by ultrasonography and stricture recurrence by retrograde urethrography. Four patients were lost during follow up. Finally, 50 patients were analyzed for this study.Results: There was no statistically significant difference in mean age, stricture etiology, preoperative stricture length, preoperative IPSS, PVR, and Qmax between groups. There were statistically non-significant findings regarding IPSS and Qmax measured in the 3rd and 7th month but the change in PVR was significant (22.24±25.26 vs 41.13±48.11 ml, p=0.02). UTI occurred significantly higher in group B (23.1% vs 0%, p=0.01) during follow-up in 3rd month. There was no statistically significant difference regarding stricture recurrence between the two groups. A significantly higher proportion (50%) of patients had shown stricture recurrence with stricture length between 1.1-1.5 cm during follow-up. Conclusion: CISC twice weekly after OIU is equally effective regarding stricture recurrence and complications in comparison to the patients doing CISC daily for short-segment stricture. Bangladesh J. Role of Steroids in Reducing Recurrence of Urethral Stricture after Direct Vision Internal Urethrotomy

Background: Recurrence of urethral strictures is the most frequent and bothersome complication, with majority of stricturesrecurring after optical internal urethrotomy. The objective of this study was to establish the role of oral steroid therapy after direct vision internal urethrotomy (DVIU) in minimizing recurrence of urethral stricture.&#x0D; Methodology: It was a randomized controlled trial carried out at Armed Forces Institute of Urology, Rawalpindi from January 2018 to March 2021. A total of 180 male patients with urethral stricture of ≤2 cm were included. Patients with traumatic stricture, post-anastomotic urethroplasty strictures, neurogenic bladder and history of steroid intake were excluded. Patients were randomly categorized into two groups i.e. Group A patients were given oral prednisolone (6 mg tablet twice daily for 4 weeks) while group B was control group. Patients were followed up to 12 months for recurrence of stricture.&#x0D; Results: The mean age of patients in group A was 44.38 ± 8.03 years while in group B was 46.64 ± 6.66 years. The mean length of stricture in group A was 0.93 ± 0.26 cm and in group B was 0.95 ± 0.27 cm. Recurrence of stricture was seen in 18 (20.0%) patients in group A (oral steroids group) and 42 (46.67%) patients in group B (no oral steroids) with p-value of 0.0001.&#x0D; Conclusion: Oral prednisolone therapy after internal urethrotomy waseffective in decreasing the rate of recurrence of urethral stricture.&#x0D; Keywords: Oral, Recurrence, Steroid, Urethral Stricture.

Male Urethral Stricture: Epidemiological, Clinical, and Therapeutic Aspects in Kara

Background: Urethral stricture is a pathology frequently encountered in urological practice. Management is often surgical, with possible recurrences. What about this pathology in Kara, a semi-urban city? Objective: Assess the management of male urethral stricture in Kara. Patients and Methods: This was a descriptive study with retrospective data collection. The study took place in the urology department of the teaching hospital of Kara, from December 2020 to December 2022. All cases of male urethral stricture, surgically treated at the teaching hospital of Kara, were listed. The inclusion criteria were as follows: any patient who had been treated surgically for male urethral stricture in the urology department of the teaching Hospital of Kara. The operating theater register and hospital records were used to collect the data. The diagnosis of urethral stricture had been made with retrograde urethrogram. A total of 24 patients were treated for male urethral stricture during the study period. The following variables were studied: age, reason for consultation, location, length, and etiology of the stricture; the type of treatment received: optical internal urethrotomy, or anastomotic urethroplasty, and the results. The result was considered good if, after removal of the urethral catheter, the patient regained his micturition without the need for dilatation; the result was considered average if, after removal of the urethral catheter, the patient needed one or more dilatation sessions to regain urination; the result was considered poor if, after removal of the catheter, the patient did not regain good micturition despite the urethral dilatation sessions. Microsoft excel and epi info 7 software were used for data processing. Results: The average age of our patients was 43.7 years ± 10.18 with extremes ranging from 27 to 70 years. The most represented age groups were that of 40 to 50 years, with 37.5% of cases; and that of 30 to 40 years with 33.3% of cases. The patients had consulted for urine retention in 66.6% of cases; the location of urethral stricture was bulbar in 45.8% of cases. The most found etiology was infectious in 58.3% of cases. Among our patients, 58.3% had received optical internal urethrotomy as treatment, while 41.6% of our patients had received anastomotic urethroplasty as treatment. Postoperatively, after removal of the urinary catheter, 87.5% of patients had benefited from one or repeated dilatation. In terms of results, we had a good result in 20.8% of patients; the result was average in 45.8% of patients, and poor in 33.3% of patients. The average duration of follow-up was 14.3 +/- 7.2 months (3-27). Conclusion: Male urethral stricture mainly affects young adults in Kara. Surgical management is done by optical internal urethrotomy and/or anastomotic urethroplasty.

Aim: To compare internal optical urethrotomy with end-to-end urethroplasty in treatment of anterior urethral stricture (&lt;1.5cm) in terms of recurrence. Study design: Cross-sectional Study. Place and duration of study: Mayo hospital, department of urology. June 2016 to July 2018 Methodology: Only 80 patients (40 individuals in each class) were included in the study with nonprobability convenience sampling. Urethral stricture of the same length was treated by class-A and class-B, but with different methods. 3 months after the initial diagnosis, the recurrence rate was examined. SPSS-22 was used to enter and analyse the data. Result: Patients' recurrences three months later indicated a significant difference (P=0.002) as a P value less than 0.05. A recurrence rate of 20% was found in class A end-to-end urethroplasty patients, whereas a stricture-free rate of 80% was found in the other 32. Stricture recurrence was seen in 52.5% of the patients in class B (internal optical urotomy), compared to 47.5% in class A. Conclusion: A urethroplasty is the best therapy for urethral strictures of 1.5 cm or more, according to this research. For the most part, the fundamental distinction between the two procedures is that urethroplasty has a lower rate of repeat surgeries than that of the other procedure. Outcome of Internal Optical Urethrotomy in Combination with Corticosteroids Injection in Recurrent Anterior Urethral Strictures

Background: Urethral stricture is a common urological disease and can occur mostly as a result of recurrent infections, inflammation, trauma or unknown factors. Internal optical urethrotomy is a useful treatment modality for the management of urethral strictures. Several adjuvant interventions are suggested to decrease the recurrence rate of urethral strictures. Objectives: To report the success rate of internal optical urethrotomy in combination with corticosteroids in recurrent anterior urethral strictures. Materials and Methods: This prospective clinical randomized control trial was performed on 70 patients from 1st January 2017 to 31st Dec 2018. Patients fulfilling the inclusion criterion were randomly divided into two groups. The experimental group comprises 34 patients who received a corticosteroid injection in combination with internal optical urethrotomy while the control group consisting of 36 patients who received distilled water in combination with internal optical urethrotomy. All the patients were advised not to do clean intermittent self-catheterization (CISC) and were followed for 24 months. Results: The mean follow-up time was 9 ± 5.40 months (6-24 months). Complications occurred in 14 patients. In the experimental group recurrence of stricture occurred in 11 patients, extravasation into perispongeal space in 3 patients (8.82%), infection in 1(2.94%) and bleeding in 2(5.8%) patients. In the control group recurrence of stricture occurred in 16 patients, Infection occurred in 3 patients and extravasation and bleedings in 2 and 3 patients respectively. Moreover, the difference between the recurrence in the experimental and control groups is also not significant (p.0.05). However, the stricture-free duration that is the time of recurrence was significantly delayed in the experimental group (8.07 ± 5.51 versus 3.8 ± 1.53 months). Conclusion: It is concluded that internal optical urethrotomy in combination with corticosteroid injection delays the recurrence rate of urethral stricture significantly. It can also be a feasible procedure for those who do not want urethroplasty or those with high risk for general anesthesia.

Unravelling the Role of Internal Optical Urethrotomy in Male Urethral Stricture Management

Background and aim: Male urethral stricture is still regarded as one of the most common and perplexing urological problems. Dilation, endoscopic urethrotomy, and urethroplasty are all options for treatment, however internal optical urethrotomy has a faster recovery time, less scarring, and a decreased risk of infection. The purpose of this study was to see how effective internal optical urethrotomy was in treating patients with urethral stricture. Materials and Methods: Between August 2020 and May 2021, the author at National institute of urology and nephrology, Cairo, Egypt, conducted this research. Internal ocular urethrotomy was used to treat a group of 18 male patients aged 22 to 51 who had urethral strictures. The follow-up time was between one and two months. The outcome was given a good, fair, or bad rating. Results: 44.44 % of the 18 patients were between the ages of 45 and 51. Trauma was the most common cause of stricture in 8 (44.4%) of patients, and poor urination was the most common presenting symptom in 6 (33.3%). The bulbous urethra was found to be the most common region of restriction in 9 (50%), followed by the penile urethra in 6 (33.3%) cases. The overall response rate was good in 12 individuals (66.7%). Only urethral hemorrhage was observed in 2 (11.1%) of the patients after surgery, with no additional significant problems. Conclusion: Internal optical urethrotomy is a dependable and effective procedure in treating urethral stricture.
