Case report: Unilateral optic nerve aplasia and developmental hemi-chiasmal dysplasia with VEP misrouting

PurposeTo describe the trans-occipital asymmetries of pattern and flash visual evoked potentials (VEPs), in an infant with MRI findings of unilateral optic nerve aplasia and hemi-chiasm dysplasia.MethodsA child with suspected left cystic microphthalmia, left microcornea, left unilateral optic nerve aplasia, and hemi-chiasm underwent a multi-channel VEP assessment with pattern reversal, pattern onset, and flash stimulation at the age of 16 weeks.ResultsThere was no VEP evidence of any post-retinal visual pathway activation from left eye with optic nerve aplasia. The VEP trans-occipital distribution from the functional right eye was skewed markedly across the midline, in keeping with significant misrouting of optic nerve fibres at the chiasm. This was supported by the anatomical trajectory of the optic chiasm and tracts seen on MRI.ConclusionThis infant has chiasmal misrouting in association with unilateral optic nerve aplasia and unilateral microphthalmos. Chiasmal misrouting has not been found in patients with microphthalmos or anophthalmos, but has been reported after early eye loss in animal models. Our findings contribute to our understanding of the discrepancy between the visual pathway physiology of human unilateral microphthalmia and animal models.

Multichannel visual evoked potentials in the assessment of visual pathways in children with marked brain abnormalities

To demonstrate how multichannel visual evoked potentials (VEPs) can provide quantitative measures of visual function in children with marked cortical anatomy abnormalities. Four children with marked brain pathology (2 holoprosencephaly, 2 giant interhemispheric cysts with hydrocephalus) underwent pattern reversal and flash VEP recordings from 16 equally distributed electrodes. Voltage maps of the major VEP components were constructed, and their distributions were compared to the magnetic resonance imaging (MRI) findings. No reproducible responses were evident in 1 case, and responses were present, but, as expected based on the MRI finding, not over the occipital electrodes in 3 cases. Thus, the standard clinical VEP electrode placement would not have detected responses. The distribution of responses during monocular testing obtained in 2 cases suggested normal decussation of the visual pathways at the chiasm, and voltage mapping indicated which part of the abnormally positioned brain tissue is functional visual cortex. In children with markedly abnormal brain anatomy, multichannel VEP recordings can provide quantifiable measures of visual pathway function detected in atypical locations. VEPs provide a quantifiable measure of visual function that could be used to assist in determining visual acuity levels, and offered a baseline for monitoring in the context of raised intracranial pressure. These recordings were also able to identify functional anatomical structures that were not apparent on MRI. In a clinical setting, the use of additional recordings from nonstandard electrode placement based on the MRI findings is suggested.

Multi-channel visual evoked potential as an ancillary tool to diagnose intraorbital optic nerve lesions

Multi-channel visual evoked potential (MVEP) recording method was used to assist in diagnosing a 4-month-old Chinese Albino rabbit with an intraorbital mass. Subcutaneous MVEP of its both eyes were recorded simultaneously using 16 electrodes (4 x 4) multi-channel array. Analysis of the cortical potential landscapes (CPL) showed that the conduction function of right eye was remarkably impaired in terms of decreased amplitudes and prolonged latencies. Specific side-dominant distribution asymmetry of the decreased MVEP amplitudes indicated that the temporal side of the optic nerve (ON) was severely involved. Overall prolonged latencies of the CPL without side differences suggested that the functional impairment could have been caused by the mechanical compression exerted by an intraorbital mass. Surgical removal procedures confirmed that the mass was located temporally to the ON. Pathological examination provided a final diagnosis of a giant polycystic mucocele. Beyond its significance as a standard tool to assess functional changes of the visual pathway, MVEP recordings might assist locating intraorbital lesions that involve the ON by careful analysis of abnormal CPLs.

Increasing the Diagnostic Value of Evoked Potentials in Multiple Sclerosis by Quantitative Topographic Analysis of Multichannel Recordings

This study presents a method to record and analyze multichannel visual-evoked potential (VEP) and somatosensory-evoked potential (SEP) in an objective, automatic, and quantitative manner. The intention of this study was to assess their diagnostic value in multiple sclerosis (MS). A 256-channel VEP and SEP were recorded in 44 healthy subjects, 26 patients with MS, and 20 patients with other neurologic diseases. Topographic pattern recognition methods were applied and a normative database was established. Z-score statistics allowed identifying the number of subjects with significant abnormal values in each group. These values were compared with conventional single-channel waveform analysis. The diagnostic value of the new measures for MS reached a sensitivity of 72% and a specificity of 100% for the VEP, which was significantly higher than the conventional analysis. For the SEP, the specificity was also high (93%) but the sensitivity remained low as in the conventional analysis (30%). The quantitative topographic analysis of multichannel VEP revealed high-diagnostic sensitivity and specificity for MS. Moreover, the method reliably identified the most dominant VEP and SEP components in the healthy subject group. The results indicate that objective topographic analysis of multichannel recordings increase the value of VEP as surrogate marker for MS.

Multi-Channel Noise Reduced Visual Evoked Potential Analysis

In this paper, Principal Component Analysis (PCA) is used to reduce noise from multi-channel Visual Evoked Potential (VEP) signals. PCA is applied to reduce noise from multi-channel VEP signals because VEP signals are more correlated from one channel to another as compared to noise during visual perception. Emulated VEP signals contaminated with noise are used to show the noise reduction ability of PCA. These noise reduced VEP signals are analysed in the gamma spectral band to classify alcoholics and non-alcoholics with a Fuzzy ARTMAP (FA) neural network. A zero phase Butterworth digital filter is used to extract gamma band power in spectral range of 30 to 50 Hz from these noise reduced VEP signals. The results using 800 VEP signals give an average FA classification of 92.50% with the application of PCA and 83.33% without the application of PCA. © 2003, The Institute of Electrical Engineers of Japan. Multichannel visual evoked potentials in migraine

Multichannel recordings of visual evoked potentials (VEPs) have proved to be useful in the evaluation of visual field defects. We studied the topographic distribution of transient VEPs in 15 migraine patients (8 with visual aura and 7 without) and 15 age-matched controls during the migraine-free interval. All the subjects included in the study had normal visual fields. VEPs were recorded from 9 electrodes placed on the posterior scalp. Stimuli were full-field and hemifield reversing square wave grating patterns of medium spatial frequency (4 c/deg). The groups did not show significant differences in latencies and amplitudes of the major components (N70, P100) recorded from the midline. However, migraine patients with visual hemianopic aura showed definite asymmetries in the VEP amplitude distribution. Significantly reduced, absent or polarity-invered VEP responses were recorded ipsilateral to the side of the prodromic visual symptoms. Direct comparison of affected and unaffected hemispheres by partial field stimulation confirmed these findings. According to the VEP cortical generator theory, these abnormalities suggest a functional anomaly consistent with the clinical syndrome and detectable also in the migraine-free interval. None of the migraine patients without aura or the controls showed VEP amplitude asymmetries. We conclude that multichannel VEP recordings may discriminate between different subtypes of migraine and contribute important physiopathological information to the study of this disease.

Topography of the evoked potential to spatial localization cues.

Visual tasks that are perceptually diverse might be expected to elicit unique evoked-potential waveforms that exhibit differing topographic maps. To investigate this possibility, multichannel visual-evoked potentials (VEPs) were recorded in response to several dot spatial localization stimuli that are physically similar yet produce different percepts (vernier offsets, steroscopic disparity, bisection, orientation, and relative displacement) to determine if the unique percepts arising from these stimuli reflect the activation of different cortical neural populations. The resulting evoked potentials were all similar in waveform, although the stereoscopic VEPs were relatively delayed. Topographic maps of the evoked-potential activity to each stimulus revealed a late major component with two independent foci: one 7 or more centimeters above the inion lateral to the midline, and the other at least 6 cm lateral to OZ. The scalp localization of both peaks was independent of both the position of the stimulus in the visual field and the particular stimulus cue presented. An asymmetric response to pattern appearance vs. disappearance indicated strong pattern specificity for each stimulus type except unreferenced motion. The timing of the VEP responses and relative insensitivity to retinal locus of stimulation suggest the involvement of higher cortical areas. The two map foci might be interpreted as activation of inferotemporal and parietal cortices whose roles are thought to be visual object interpretation and spatial attention and localization, respectively.
