The Impact of Venous Stenting for Patients with Idiopathic Intracranial Hypertension and Spontaneous Cerebrospinal Fluid Leak on Symptoms and Quality of Life

BACKGROUND: Elevated intracranial pressure can cause skull base defects and a spontaneous cerebrospinal fluid leak (CSF). Venous sinus stenting (VSS) has emerged as a promising treatment option for patients with a CSF leak in the setting of idiopathic intracranial hypertension (IIH). There is a lack of literature on symptomatology and quality of life (QOL) after VSS for IIH patients with a CSF leak. This study explores the effects of VSS on symptoms and QOL in IIH patients with a CSF leak. METHODS: This is a retrospective study on patients who have IIH complicated by a CSF leak and underwent VSS. A QOL questionnaire was developed from the migraine disability assessment test and the PROMIS-PI was given to patients included in this study. RESULTS: Ten patients were included in this study. Nine patients underwent endoscopic closure of CSF prior to stent placement and one patient was treated with VSS only. There was no evidence of CSF leak recurrence in this population following VSS. Headaches improved in 5/8, tinnitus in 5/6, and visual disturbance in 4/5 patients. Diamox was discontinued in 7 out of 8 patients after VSS. There was an improvement in headache (p=0.0140) specific questions and overall QOL (p=0.0061) on the QOL questionnaire. DISCUSSION: This preliminary study demonstrates that VSS is effective in alleviating many symptoms in IIH patients with a CSF leak, especially headaches. Diamox may be able to be discontinued in many patients following VSS. No CSF leak recurrence was noted in this patient population.

The Prevalence and Impact of Migraine Among Physicians in Saudi Arabia: A Cross-Sectional Study.

Background Migraine is a primary headache disorder and one of the most common causes of disability worldwide. Prior reports showed a higher prevalence of migraine among physicians. We conducted this study to estimate the prevalence of migraine among physicians in Saudi Arabia, as well as its impact on quality of life. Methods A cross-sectional study was conducted using a validated online questionnaire that was distributed among physicians in various medical and surgical specialties in Saudi Arabia from January to June 2023. The inclusion criteria involved physicians in all specialties, who worked in governmental or private hospitals in Saudi Arabia. Employed or healthcare workers other than physicians were excluded. To measure the impact of headache on a person's quality of life, the Migraine Disability Assessment (MIDAS) test was applied. Statistical analysis of the study was conducted using the Statistical Package for the Social Sciences (SPSS) Version 24 (IBM Corp., Armonk, NY). Results We had 387 respondents, of whom 362 fulfilled the inclusion criteria. The mean age was 33.02 ± 9.07 years, and genders were distributed equally. Most physicians worked in departments of internal medicine, family medicine, general practice, and general surgery. The prevalence of migraine was recorded in 17.1% (62 patients) of all the included participants, with a female predominance. MIDAS revealed a total mean score of 16.823±14.149. The severity grades were classified as moderate (37.1%), severe (35.5%), or little to no (21.0%). Stress and light were cited as the top factors that trigger or worsen migraine, while sleep, rest, and quiet and darknesswere cited as the top relieving factors. The average number of headache attacks (month) was 6.823. Most patients with migraine were board residents (27.4%), living in the western region (53.2%), and working in a governmental hospital (38.7%). Conclusion Migraine is a common disorder among physicians in Saudi Arabia, with significant moderate-to-severe disability. Stress is identified as a major trigger. Proper awareness, stress management programs, and therapeutic intervention may mitigate the untoward consequences of migraine.

The effect of fibromyalgia syndrome on female patients diagnosed with chronic migraine

ObjectiveTo compare pain, quality of life, sleep, anxiety and depression, central sensitization, and functionality between chronic migraine (CM) patients with comorbid fibromyalgia syndrome (FMS) and patients with CM alone. MethodThirty three female patients with CM and thirty three female patients with CM+FMS were enrolled in the study. Demographic and clinical characteristics of the patients were recorded. FM was diagnosed based on the 2016 American College of Rheumatology diagnostic criteria. All participants were evaluated with Allodynia Symptom Checklist, Short Form-36 (SF-36), Pittsburgh Sleep Quality Index (PSQI), Hospital Anxiety and Depression Scale (HADS), Migraine Disability Assessment (MIDAS) and Headache Impact Test (HIT-6) questionnaires, and Central Sensitization Inventory (CSI). FM patients were also evaluated with Fibromyalgia Impact Questionnaire (FIQ). ResultsThe average number of headache days was significantly higher in patients with CM+FMS (p = 0.006). Among migraine accompanying symptoms, the number of patients with phonophobia was significantly higher in patients with CM+FMS (p = 0.008). While CSI score was 39.0 ± 11.7 in CM patients, it was 52.2 ± 9.2 in CM+FMS patients. CSI scores were higher in CM+FMS patients (p < 0.001). SF-36 sub-cores, including physical function, energy/fatigue, emotional well-being, and general health scores, were lower in CM+FMS patients (p < 0.05). Sleep duration was significantly lower and use of medication to sleep was more common in same group (p < 0.05). FIQ score in CM+FMS patients was associated with quality of life scores, sleep quality, anxiety, and central sensitization scores (p < 0.05). ConclusionIn patients with chronic migraine, FMS comorbidity negatively affects the quality of life and significantly increases central sensitization.

The Neuroprotective Role of Indole-3-Propionic Acid in Migraine Pathophysiology.

Background and Objectives: Migraine is a leading cause of disability worldwide, with complex pathophysiological mechanisms involving oxidative and nitrosative stress. Recent research suggests that Indole-3-Propionic Acid (IPA) may have a neuroprotective role in reducing nitrosative stress. This study aims to elucidate the roles of IPA and nitrosative stress biomarkers in migraine patients, focusing on their potential as therapeutic targets. Materials and Methods: This cross-sectional, case-control study included 57 migraine patients and 30 healthy controls. Patients were categorized into episodic migraine (EM) and chronic migraine (CM) groups. Socio-demographic and clinical characteristics were documented through structured interviews. Validated scales such as the Visual Analog Score (VAS), Headache Impact Test 6 (HIT-6), Migraine Disability Assessment Test (MIDAS), Migraine 24 h Quality of Life Scale (24 h QoL), Mini-Mental State Examination (MMSE), and Migraine Attacks-Subjective Cognitive Impairments Scale (Mig-SCog) were administered. Venous blood samples were collected, and serum levels of IPA, Nitric Oxide (NO), Nitric Oxide Synthase (NOS), and Peroxynitrite (ONOO-) were measured using ELISA and spectrophotometric methods. Results: Significant differences in serum IPA and NO levels were observed between migraine patients and controls. Specifically, higher serum IPA levels were found in the EM group, while higher serum NO levels were observed in the CM group. Elevated NO levels correlated with increased migraine attack frequency. Conversely, serum IPA levels showed a negative correlation with attack frequency, suggesting a protective role. Specifically, NO levels were positively correlated with the number of painful days, NSAID usage, VAS scores, HIT-6 scores, and MIDAS scores, while negatively correlated with 24 h QoL scores. Conclusions: The study highlights the significant involvement of IPA and nitrosative stress in migraine pathophysiology. Elevated IPA levels, particularly in EM patients, suggest its potential neuroprotective role. These findings underscore the importance of targeting oxidative and nitrosative stress pathways in developing effective migraine therapies.

A Comparative Study of Cognitive Distortion among Mild and Severe Migraine Patients

Migraine is a kind of headache which induced various types of problems in patients. Cognitive distortion is one the worst impact of migraine. The term cognitive distortion refers the individual’s negative thoughts towards their environment. The aim of this research is to know the level of migraine and its impact on cognitive distortion. 62 migraine patients were selected from different hospitals of Ranchi. The Migraine Disability Assessment Test (MIDAS) was applied to know the severity level of migraine and Cognitive Distortion Scale (CDS) was applied to know the cognitive distortion in patients. After analysis of data it was found that the number of severe migraine patients was higher and they have higher level of cognitive distortion.

Effects of aerobic and resistance exercises on psychological and cognitive functions in patients with post-stroke migraine

ABSTRACT Objective To investigate the impact of a combination of aerobic and resistance exercises on the psychological and cognitive functions of post-stroke migraine patients. Methods This study recruited 100 patients suffering from post-stroke migraine pain who were admitted to the hospital, categorizing them into a control group (n = 50) and an intervention group (n = 50). The control group received conventional drug treatment, while the intervention group received the exercise-based intervention that combined aerobic exercise with resistance exercise. Results Before treatment, both groups displayed similar Patient Health Questionnaire-9 (PHQ-9), Generalized Anxiety Disorder-7 (GAD-7), Mini-mental State Examination (MMSE) and MoCA scores. However, after the intervention, the intervention group exhibited lower scores on these measures compared to the control group (all p < 0.05). Additionally, there were no discernible disparity in Migraine Disability Assessment (MIDAS) and Headache Impact Test (HIT-6) scores between the two cohorts of patients before treatment (p > 0.05), whereas the intervention group demonstrated significantly lower MIDAS and HIT-6 scores following the intervention (p < 0.05). Although there were no discernible distinctions in National Institute of Health stroke scale (NIHSS) and Stroke Specialized Quality of Life Scale (SS-QOL) measurements between the two patient groups before treatment (p > 0.05), the intervention group exhibited a significant decrease in NIHSS scores and a notable increase in SS-QOL scores after the intervention (p > 0.05). Moreover, the satisfaction rate and overall satisfaction rate were significantly higher in the intervention group (p < 0.05). Conclusion The combination of aerobic and resistance exercises demonstrated positive effects on the psychological well-being and overall quality of life for post-stroke migraine patients.

Reducing the Impact of Headache and Allodynia Score in Chronic Migraine: An Exploratory Analysis from the Real-World Effectiveness of Anti-CGRP Monoclonal Antibodies Compared to Onabotulinum Toxin A (RAMO) Study.

Chronic migraine (CM) is a disabling and hard-to-treat condition, associated with high disability and high cost. Among the preventive treatments, botulinum toxin A (BoNT-a) and monoclonal antibodies against the calcitonin gene-related protein (anti-CGRP mAbs) are the only disease-specific ones. The assessment of the disease burden is complex, and among others, tools such as the allodynia symptoms checklist (ASC-12) and headache impact test (HIT-6) are very useful. This exploratory study analysed the impact of these two therapies on migraine burden. The RAMO study was a multicentre, observational, retrospective investigation conducted in two headache centres: the Fondazione IRCCS Istituto Neurologico Carlo Besta (Milan) and the Fondazione Policlinico Campus Bio-Medico (Rome). This study involved patients with chronic migraine treated with mAbs or BoNT-A. We conducted a subgroup exploratory analysis on HIT-6 and ASC-12 scores in the two groups. The Wilcoxon rank-sum test, Fisher's exact test, and ANOVA were performed. Of 126 patients, 36 on mAbs and 90 on BoNT-A had at least one available follow-up. mAbs resulted in a mean reduction of -11.1 and -11.4 points, respectively, in the HIT-6 at 6 and 12 months, while BoNT-A was reduced -3.2 and -3.6 points, respectively; the mAbs arm resulted in mean reductions in ASC-12 at 6 and 12 months of follow-up of -5.2 and -6.0 points, respectively, while BoNT-A showed lesser mean changes of -0.5 and -0.9 points, respectively. The adjusted analysis confirmed our results. In this exploratory analysis, anti-CGRP mAbs showed superior effectiveness for HIT-6 and ASC12 compared to BoNT-A. Reductions in terms of month headache days (MHD), migraine disability assessment test (MIDAS), and migraine acute medications (MAM) were clinically relevant for both treatments.

Real-world effectiveness of Anti-CGRP monoclonal antibodies compared to OnabotulinumtoxinA (RAMO) in chronic migraine: a retrospective, observational, multicenter, cohort study

BackgroundChronic migraine (CM) is a disabling condition with high prevalence in the general population. Until the recent approval of monoclonal antibodies targeting the calcitonin gene-related peptide (Anti-CGRP mAbs), OnabotulinumtoxinA (BoNT-A) was the only treatment specifically approved for CM prophylaxis. Direct comparisons between the two treatments are not available so far.MethodsWe performed an observational, retrospective, multicenter study in Italy to compare the real-world effectiveness of Anti-CGRP mAbs and BoNT-A. Patients with CM who had received either treatment according to Italian prescribing regulations were extracted from available clinical databases. Efficacy outcomes included the change from baseline in monthly headache days (MHD), MIgraine Disability ASsessment test (MIDAS), and monthly acute medications (MAM) evaluated at 6 and 12 months of follow-up. The primary outcome was MHD change from baseline at 12 months. Safety outcomes included serious adverse events (SAE) and treatment discontinuation. Unadjusted and adjusted models were used for the analyses.ResultsTwo hundred sixteen potentially eligible patients were screened; 183 (86 Anti-CGRP mAbs; 97 BoNT-A) were included. One hundred seventy-one (80 Anti-CGRP mAbs; 91 BoNT-A) and 154 (69 Anti-CGRP mAbs; 85 BoNT-A) patients were included in the efficacy analysis at 6 and 12 months of follow-up, respectively. Anti-CGRP mAbs and BoNT-A both resulted in a mean MHD reduction at 6 (-11.5 and -7.2 days, respectively; unadjusted mean difference -4.3; 95%CI -6.6 to -2.0; p = 0.0003) and 12 months (-11.9 and -7.6, respectively; unadjusted mean difference -4.4; 95%CI -6.8 to -2.0; p = 0.0002) of follow-up. Similar results were observed after adjusting for baseline confounders. Anti-CGRP mAbs showed a significant MIDAS (-31.7 and -19.2 points, p = 0.0001 and p = 0.0296, respectively) and MAM reduction (-5.1 and -3.1 administrations, p = 0.0023 and p = 0.0574, respectively) compared to BoNT-A at 6 and 12 months. No SAEs were reported. One patient receiving fremanezumab discontinued treatment due to arthralgia. Treatment discontinuations, mainly for inefficacy, were comparable.ConclusionBoth Anti-CGRP mAbs and BoNT-A were effective in CM patients with Anti-CGRP mAbs presenting higher effect magnitude, with comparable safety. Still, BoNT-A remains a valuable option for CM patients with contraindications to Anti-CGRP mAbs or for frail categories who are candidates to local therapy with limited risk of systemic administration.Graphical

Comparison of pupil size in patients of migraine with photophobia versus normal subjects and correlation of the pupil size to severity of headache.

To compare the pupil size of patients having migraine with photophobia versus normal subjects and to correlate the pupil size to the severity of headache. This was a prospective case-control study. One hundred eyes of 50 patients who had migraine with photophobia were compared with same number of control subjects who had no history of headache. Pupil size was measured using the IOL Master 700 in all cases. Severity of headache was evaluated by the Migraine Disability Assessment (MIDAS) test questionnaire, and MIDAS grades from 1 to 4 ranging from minimal disability to severe disability were obtained in all cases. The pupil sizes in all grades were compared. The mean pupil size was 4.89 ± 0.9 mm (95% confidence interval [CI]) in patients of migraine with photophobia compared to 3.8 ± 0.49 mm (95% CI) in normal subjects, with a P value of <0.0001, which was statistically significant. Among the 50 patients with migraine, according to MIDAS grades, 20 had Grade 4 or severe disability, 15 had Grade 3 or moderate disability, 10 had Grade 2 or mild disability, and five had Grade 1 or minimal disability. Pupil size in Grade 4 category was the largest with a mean value of 5.6 ± 0.79 mm (95% CI). The pupil size in patients of migraine with photophobia is larger than in normal subjects and could be a trigger for the migraine attacks. The larger the pupil size, the more severe is the migraine disability.

The Effect of OnabotulinumtoxinA on Headache Intensity and Number of Monthly Headache Days in Individuals with Chronic Migraine with Different Levels of Neck Disability.

One of the treatment methods used in chronic migraine is OnabotulinumtoxinA. The effects of OnabotulinumtoxinA on headache intensity (HI) and number of monthly headache days (NMHD) in chronic migraine (CM) patients classified according to neck disability levels are unknown. Our aim was to investigate the effect of OnabotulinumtoxinA on the HI and the NMHD in individuals with CM with different levels of neck disability. One hundred sixteen patients were enrolled in the study. The OnabotulinumtoxinA protocol was administered as per Follow-the-Pain PREEMPT. The Neck Disability Index was used to evaluate neck disability. Primary outcome measures were headache intensity, assessed with the Visual Analogue Scale, and the number of monthly headache days recorded from patients' diaries. Secondary outcome measures were migraine disability, assessed with the Migraine Disability Assessment Test, and quality-of-life, assessed with the Headache Impact Test-6. All assessments were made at baseline and end of the treatment. The OnabotulinumtoxinA treatment showed a greater improvement effect in the number of monthly headache days (p = 0.000) and migraine disability (p = 0.000) parameters in the severe and complete disability groups. CM patients with complete and severe neck disability received the most benefit in reducing the NMHD at 3 months after OnabotulinumtoxinA treatment, but the HI decreased at a similar level in all neck disability groups.

Intramuscular Electrical Stimulation for the Treatment of Trigger Points in Patients with Chronic Migraine: A Protocol for a Pilot Study Using a Single-Case Experimental Design.

Background and Objectives: Trigger points (TrPs) are prevalent in patients with migraine headaches. Needling interventions targeting TrPs in migraine patients may reduce the intensity and frequency of headaches, yet systematic reviews reveal a lack of robust evidence. Intramuscular electrical stimulation (IMES) is a modality that delivers electrical current into muscles and TrPs, with recent studies suggesting it may amplify the therapeutic effects of dry needling peripherally and centrally. This could be advantageous for patients with migraine and symptomatic TrPs. Materials and Methods: This study will implement a multiple baseline single-case experimental design (SCED). In a clinical setting, a SCED study lends itself to conducting research with only a few patients that each serve as their own controls. In this SCED study, four participants with chronic migraine will be enrolled in a non-concurrent manner and randomized to one of four baseline measurement periods (4, 5, 6 or 7 weeks), leading to four potentially different start dates for each participant in the intervention phase. During the intervention phase, patients will receive five sessions of dry needling with IMES, one session per week for five weeks. The primary outcome measure will be headache frequency, i.e., the reduction in the number of headache days over a one-month period using electronic headache diary data from the Migraine Buddy smartphone application. Secondary outcome measures will be changes in mean migraine pain intensity using a numeric pain rating scale (NPRS), migraine disability using the Migraine Disability Assessment Test (MIDAS), the Headache Impact Test (HIT-6), and changes in selected cervical musculoskeletal impairments including pressure pain thresholds (PPTs) over TrPs, the craniocervical flexion test (CCFT), and cervical active range of motion (AROM). Primary and secondary outcome measures will be analyzed separately using both visual and statistical analyses. Results: Actively recruiting participants. This project was approved by the Mass General Brigham Institutional Review Board (protocol #2023P000931) and is registered with ClinicalTrials.gov (NCT05893914). Conclusions: This study will seek to determine the effects of a five-week intervention period of IMES to TrPs in the posterior cervical muscles of subjects with chronic migraine.

Headache in long COVID as disabling condition: A clinical approach.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can exacerbate previous headache disorders or change the type of pain experienced from headaches. This study aimed to investigate the clinical features of Long COVID headaches. This was a cross-sectional, descriptive, and analytical observational study that included 102 patients (with previous headache, n = 50; without previous headache, n = 52) with long COVID and headache complaints. The Migraine Disability Assessment Test and Visual Analog Pain Scale were used to collect participants' headache data according to a standardized protocol. The patients in this study who reported experiencing headaches before COVID-19 had longer headache duration in the long COVID phase than that in the pre-long COVID phase (p = 0.031), exhibited partial improvement in headache symptoms with analgesics (p = 0.045), and had a duration of long COVID of <1 year (p = 0.030). Patients with moderate or severe disability and those classified as having severe headaches in the long COVID phase were highly likely to develop chronic headaches. Hospital admission [odds ratio (OR) = 3.0082; 95% confidence interval (95% CI): 1.10-8.26], back pain (OR = 4.0017; 95% CI: 1.13-14.17), insomnia (OR = 3.1339; 95% CI: 1.39-7.06), and paraesthesia (OR = 2.7600; 95% CI: 1.20-6.33) were associated with headache in these patients. Headache is a disabling condition in patients with long COVID-19, exacerbating the conditions of those with headaches prior to contracting COVID-19.

Measuring Success in Headache Surgery: A Comparison of Different Outcomes Measures.

Studies of migraine surgery have relied on quantitative, patient-reported measures like the Migraine Headache Index (MHI) and validated surveys to study the outcomes and impact of headache surgery. It is unclear whether a single metric or a combination of outcomes assessments is best suited to do so. All patients who underwent headache surgery had an MHI calculated and completed the Headache Impact Test, the Migraine Disability Assessment Test, the Migraine-Specific Quality-of-Life Questionnaire, and an institutional ad hoc survey preoperatively and postoperatively. Twenty-seven patients (79%) experienced greater than or equal to 50% MHI reduction. MHI decreased significantly from a median of 210 preoperatively to 12.5 postoperatively (85%; P < 0.0001). Headache Impact Test scores improved from 67 to 61 (14%; P < 0.0001). Migraine Disability Assessment Test scores improved from 57 to 20 (67%; P = 0.0022). The Migraine-Specific Quality-of-Life Questionnaire demonstrated improvement in quality-of-life scores within all three of its domains ( P < 0.0001). The authors' ad hoc survey demonstrated that participants "strongly agreed" that (1) surgery helped their symptoms, (2) they would choose surgery again, and (3) they would recommend headache surgery to others. Regardless of how one measures it, headache surgery is effective. The authors demonstrate that surgery significantly improves patients' quality of life and decreases the effect of headaches on patients' functioning, but headaches can still be present to a substantial degree. The extent of improvement in migraine burden and quality of life in these patients may exceed the amount of improvement demonstrated by current measures.

Effect Of Exercise In Sync With Circadian Preference On Migraine Load In Migraineurs

Migraines are the most common cause of chronic pain. Effective, non-pharmacological strategies to reduce migraine load are needed. Exercise is an effective strategy, but it is unclear how exercise timing and temporal preference (chronotype)factors modulate the laudatory effects of exercise. PURPOSE: The purpose of this study was to determine the effects of exercising in-sync or out-of-sync with one’s chronotype on migraine load. METHODS: Participants were 7 sedentary individuals who experienced migraines 8+ times per month (age = 34 ± 11 yrs, 167 ± 8 cm, 99 ± 26 kg). Participants were categorized into morning- or evening-types based on the Morning/Eveningness Questionnaire (MEQ). Exercise consisted of 1 month of self-selected exercise in the morning and 1 month in the evening, 3 x per week of 30-min/session at 60-70% of estimated HRmax. In-sync (IS) refers to a morning-type exercising in the morning and an evening-type exercising in the evening. Out-of-sync (OOS) refers to a morning-type exercising in the evening and vice-versa. Before and after each month of exercise, participants completed questionnaires (Headache Impact Test [HIT-6], Migraine Disability Assessment Test [MIDAS]). We analyzed the data using a 2 (in sync, out of sync) x 2 (pre, post) repeated measures ANOVA with significance accepted at p < 0.05. RESULTS: We observed a trend towards an interaction effect in HIT-6 scores suggesting IS exercise produced a decrease in HIT-6 scores while no reduction was seen in OOS exercise (p = 0.11). The the number of migraine days experienced tended to increase in the month exercise was performed OOS (p = 0.12), and this is reflected in increasing MIDAS scores obtained OOS (p = 0.63). CONCLUSION: While no statistical changes were observed, trends in the data show that prescribing exercise in sync with migraineur's chronotype may be an accessible, non-pharmacological option to decreasing migraine load.

A clinical trial on preventive effects of licorice on propranolol-related dizziness, a side-effect in patients with migraine headache

Background/Aims: This trial aimed to evaluate Licorice preventive effects on dizziness, a side effect of propranolol in patients with migraine headaches (systolic blood pressure &lt; 120 mmHg). Methods: Forty patients (systolic blood pressure ˂120 mmHg) who received propranolol (prophylactic medicine) were subjected and divided into Licorice and placebo groups. Dizziness frequency and severity, systolic and diastolic blood pressure, and orthostatic hypotension were assessed after three months. Headache frequency and severity in groups were examined via the Migraine Disability Assessment Test (MIDAS) to evaluate the licorice impact on migraine and response to treatment. Results: Dizziness frequency and incidence and orthostatic hypotension incidence were significantly reduced in the Licorice group more than those of placebo, while dizziness severity was almost similar. Finally, systolic and diastolic blood pressure values increased in the Licorice group and decreased in the placebo. MIDAS and headache severity in both groups were the same. However, headache frequency in the Licorice group was lower than the placebo. Conclusion: Licorice maybe effective for the prevention of propranolol-induced dizziness and hypotension in patients with migraine headaches (systolic blood pressure &lt; 120 mmHg). Assessment of balance and functional capacity in fibromyalgia patients with and without migraine

ObjectiveTo evaluate the balance and functional capacity of fibromyalgia patients’ with and without migraine.MethodsObservational study with 60 women, between 20 and 60 years old (47±9 years), who were allocated into the following groups: 1. fibromyalgia and migraine (n=49); 2. only fibromyalgia (n=11). The following were used: Migraine Disability Assessment Test (MIDAS) to analyze the impact of migraine on quality of life; Revised Fibromyalgia Impact Questionnaire (FIQ-R) to assess the impact of fibromyalgia; six-minute walk test (6MWT)to assess functional capacity and Berg Balance Scale (BBE) for balance analysis.ResultsThere was no difference between groups 1 and 2 regarding functional capacity (TC6M: 308.7 m ±107.33 vs. 298.8 m ±153.54; p=0.55) and balance (BBE: 48± 14 vs. 47±16; p=0.68) as well as the impact on quality of life (FIQ: 78.8±14.77 vs. 74.4±13.99; p=0.28 and MIDAS: 31 ±24).ConclusionsOur study showed that there was no difference in functional capacity, balance and quality of life in fibromyalgia patients compared to patients with fibromyalgia associated with migraine.

Association of migraine with Gastritis in Pakistani young adults and its impact on their educational performance

Young adults worldwide are suffering from migraine as well as from gastritis problems. Reportedly, both of these situations has been occurring at the same time in young adults. For this purpose, a sample size of 30 subjects ranging from 10-24years, who were suffering from gastritis were selected in this study. To conduct the analysis, the subjects filled the questionnaires, in the presence of an investigator. This questionnaire comprised migraine differentiation questions, lifestyle factors, and the Migraine Disability Assessment test (MIDAS). MIDAS was used to estimate the impact of migraine on the class performance of young adults. With female to male ratio of 3.3:1, the average age of participants was 10-24. The prevalence of migraine associated with gastritis was found to be 53.3% significant. A significant relationship was identified between migraine, gastritis, and migraine in young adults. MIDAS test indicated that migraine had a significant impact on the class performance of young adults indicating that no impact, mild impact, moderate impact, and severe impact had a prevalence of 26.7%, 30%, 26.7%, and 16.7%, respectively. Hence, significant associations were found between migraine and gastritis in the young adults. Shocking alteration in the lifestyle of Pakistani community was seen, while conducting this study. Further studies can be conducted to understand the factors associated with day-by-day altering trends in the country.

Effectiveness and Safety of Memantine for Headache: A Meta-analysis of Randomized Controlled Studies.

The effectiveness and safety of memantine for headache are elusive, and this meta-analysis aimed to explore the influence of memantine versus placebo for headache. We searched PubMed, Embase, Web of science, EBSCO, and Cochrane library databases through May 2021 and included randomized controlled trials reporting memantine versus placebo for headache patients. This meta-analysis was performed using the random-effects model. Our meta-analysis included 4 randomized controlled trials and 229 patients. Compared with control group for headache, memantine treatment could substantially reduce headache days (mean difference [MD] = -3.10; 95% confidence interval [CI] = -5.46 to -0.75; P = 0.01), pain intensity (MD, -0.43; 95% CI, -0.85 to -0.01; P = 0.04), monthly attack frequency (MD, -2.14; 95% CI, -2.83 to -1.46; P < 0.00001), and Migraine Disability Assessment Test (MD, -5.63; 95% CI, -6.46 to -4.79; P < 0.00001) but revealed no significant influence on days for acute pain medications, adverse events, or nausea/vomiting. Memantine treatment is effective and safe to treat headache.

Real-world effectiveness of fremanezumab in migraine patients initiating treatment in the United States: results from a retrospective chart study

BackgroundThe efficacy and tolerability of fremanezumab, a fully humanized monoclonal antibody (IgG2Δa) that selectively targets calcitonin gene-related peptide (CGRP) and is approved for the preventive treatment of migraine in adults, have been demonstrated in randomized, double-blind, placebo-controlled trials. Real-world data can further support those clinical trial data and demonstrate the full clinical benefits of fremanezumab. This chart review assessed the effectiveness of fremanezumab for improving clinical outcomes in adult patients with migraine treated according to real-world clinical practice.MethodsThis retrospective, panel-based, online physician chart review study used electronic case report forms with US physicians. Patient inclusion criteria were a physician diagnosis of migraine, fremanezumab treatment initiation at ≥ 18 years of age after US Food and Drug Administration approval, ≥ 1 dose of fremanezumab treatment, and ≥ 2 assessments of monthly migraine days (MMD; 1 within 30 days before treatment initiation and ≥ 1 after initiation). Changes from baseline in MMD, monthly headache days (MHD), and Migraine Disability Assessment (MIDAS) and 6-item Headache Impact Test (HIT-6) scores were assessed over 6 months. These endpoints were evaluated in the overall population and subgroups divided by dosing schedule and number of prior migraine preventive treatment failures.ResultsThis study included data from 421 clinicians and 1003 patients. Mean age at fremanezumab initiation was 39.7 years, and most patients were female (75.8%). In the overall population, mean baseline MMD and MHD were 12.7 and 14.0, respectively. Mean (percent) reductions from baseline in MMD and MHD, respectively, were − 4.6 (36.2%) and − 4.7 (33.6%) at Month 1, − 6.7 (52.8%) and − 6.8 (48.6%) at Month 3, and − 9.2 (72.4%) and − 9.8 (70.0%) at Month 6. Mean (percent) reductions from baseline in MIDAS and HIT-6 scores also increased over the 6-month study period, from − 6.2 (21.6%) and − 8.4 (14.0%) at Month 1 to − 18.1 (63.1%) and − 16.2 (27.0%) at Month 6, respectively. Improvements in these outcomes over 6 months were observed across all evaluated subgroups.ConclusionsThis real-world study demonstrated effectiveness of fremanezumab treatment for up to 6 months, irrespective of dosing regimen or number of prior migraine preventive treatment failures, reflecting ongoing, clinically meaningful improvements in patient outcomes.

Real-world efficacy of galcanezumab for the treatment of migraine in Korean patients

We aimed to provide real-world data on the effectiveness of an anti-calcitonin gene-related peptide monoclonal antibody administered for treating migraine in Korean patients. We prospectively recruited patients with migraine who received galcanezumab treatment at a single university hospital from June 2020 to April 2021. The treatment response was assessed after three consecutive monthly injections. A 50% responder rate was evaluated based on ≥50% reduction in the number of moderate/severe headache days. Overall, 87 patients were included in the analysis. Most patients were women (83.9%). They had a mean age of 41.7 ± 12.3 years (range 17-72). Sixty-five patients (74.7%) had chronic migraine, 35 patients (40.2%) had a history of medication-overuse headache, and 32 patients (36.8%) were previously unresponsive to or found intolerable five classes of preventive medication. After three months of treatment, mean changes in numbers of monthly headache days, moderate/severe headache days, crystal clear days, and days of acute medication use were -7.2 ± 8.43, -4.3 ± 9.76, 7.3 ± 8.50, and -4.1 ± 7.93, respectively. The 50% responder rates were 58.3%, 44.2%, and 40.6% for patients with unsuccessful previous use of 0-1, 2-4, and 5 preventive medication classes, respectively. Headache Impact Test-6 and Migraine Disability Assessment Test scores also decreased (-4.4 ± 8.09 and -32.9 ± 77.04, respectively). In our cohort, the effectiveness and safety of galcanezumab were comparable with those reported in clinical trials, whereas a higher response rate was observed in the difficult-to-treat patient subset than that reported in trials. We provide real-world evidence of galcanezumab treatment benefits in Asian patients with migraine.
