Longevity of acrylic and cobalt-chromium removable partial dentures–A ten-year retrospective survival analysis of 1246 denture-wearing patients

Objectivesto assess the survival rates of removable partial dentures (RPDs) and identify factors impacting their longevity. Methodselectronic health records were retrieved of patients aged ≥18 who received RPDs between 2010 – 2021 with a follow-up of ≥ three months. Data extracted included demographics, medical history, dental charting, periodontal screening and recording scores, prostheses details and related interventions, including new dentures/denture remakes, and maintenance. Multivariate Mixed-Effect Cox regression was performed to identify potential RPD survival risk factors. Reduced model selection was reached using a backward step-down by comparing the performance of these multivariable models using the ANOVA test. Results1893 RPDs from 1246 patients were included, with a median follow-up of 21.8 months (range from 3 to 131.3 months). Three hundred and twelve patients received a maxillary RPD, 460 received a mandibular RPD, and the remaining 474 patients received both maxillary and mandibular RPDs. Metal-based RPDs had a median survival of 73 months (95%CI: 70 – 82) versus 45 months (95% CI: 37-67) for acrylic ones. Multivariable mixed effects Cox model showed that the lifespans of RPDs were longer amongst patients receiving more maintenance care within three months [Hazards Ratio (HR)=0.89 (0.83, 0.96)] and after three months [HR=0.53 (0.46, 0.61)] of denture delivery, patients wearing both maxillary and mandibular RPDs [HR=0.67 (0.52, 0.87)], and patients receiving metal-based RPDs [HR=0.31 (0.23, 0.42)]. ConclusionsMetal-based dentures, dual arch restoration, and increased maintenance positively impact the survival of RPDs. Clinical significanceAdapting consent and warranty practices is advised to reflect RPD performance variations.

Postinsertion adjustment appointments and their association with cast removable partial denture survival

Statement of problemSuccessful outcomes with cast removable partial dentures (RPDs) are dependent on adequate fabrication and adjustments at the delivery appointment. Evaluation of the number and frequency of postinsertion follow-up appointments helps determine whether the prosthesis continues to fit comfortably and whether function and esthetics continue to be satisfactory. Reports on the number of appointments and frequency and type of adjustments required for RPDs following insertion are sparse. PurposeThe purpose of this university-based population study was to determine the number of appointments and type of adjustments following RPD insertion and their association with patient demographics, type of RPD, and denture survival. Material and methodsThis retrospective clinical study examined the records of 257 patients at the University of Toronto, Faculty Dentistry, wearing 308 RPDs inserted between 2013 and 2014 with a 5-year follow-up. The outcome measures investigated included postinsertion appointments, type of adjustments, and denture survival. ResultsA total of 48.1% of the dentures (19.5% tissue supported; and 28.6% tooth supported) were maxillary, and 51.9% (34.7% tissue supported; and 17.2% tooth supported) mandibular. Most patients (68.9%) had 1 to 3 postinsertion appointments, with 78.6% having no major adjustments. Twenty-six dentures failed (failure rate 8.4%), with the failure-free time estimated at 4.58 years (95% confidence interval (CI) 4.42-4.73 years, Kaplan-Meier survival analysis). Failed dentures were significantly associated with more minor adjustments (Mean (M)=4.12, SD=3.90, Kruskal-Wallis (K-W) P=.027; OR=1.18; 95% CI 1.05-1.32, P=.006). More minor adjustments were needed for mandibular dentures (multivariable Poisson regression (MPR) P=.003) compared with maxillary dentures. More major adjustments were needed for maxillary dentures (MPR P=.030) compared with mandibular dentures. More minor and major adjustments were needed for dentures that were remade from within 5 years to beyond 10 years compared with first time denture wearers (MPR P<.001). Patients with musculoskeletal disorders required a significantly higher number of minor adjustments (M=3.67, MPR P<.001) and appointments (M=3.87, MPR P<.001) than those without these disorders. ConclusionsThe 5-year survival of RPDs following insertion was estimated at 91.6%. Most patients required 1 to 3 appointments after insertion. Mandibular RPDs required significantly more minor adjustments, and maxillary RPDs more major adjustments. More minor and major adjustments were needed for dentures that were remade at any time previously compared with first time denture wearers.

HIGH - GRADE OSTEOBLASTIC OSTEOSARCOMA OF THE MAXILLA: DIAGNOSIS, TREATMENT, AND BUCCOMAXILLOFACIAL REHABILITATION

Osteosarcoma is one of the most common malignant bone lesions; however, it comprises only 6%-8% of all osteosarcomas. It is a lesion commonly found between the third and fourth decade of life, in addition to having a similar proportion of appearing between the mandible and maxilla. This study aims to report the case of a 64-year-old Caucasian female patient, affected by a high-grade osteoblastic osteosarcoma. The patient presented an edema in the posterior region of the maxilla, paresthesia, and nasal obstruction. Panoramic radiography and CT scans, along with anatomopathological examination, were performed. Histopathologically, an intense volume of osteoid was found. Left radial maxillectomy was the chosen treatment. The patient was first rehabilitated with a provisional palatal obturator prosthesis in order to improve her nutritional status. Her rehabilitation was planned and accomplished with a mandibular removable partial denture and a maxillary removable partial obturator denture. Oral health and risk of cognitive disorders in older adults: A biannual longitudinal follow-up cohort.

Oral health may be associated with cognitive disorders such as mild cognitive impairment or dementia. This study elucidates the effects of oral health conditions on the progression of cognitive disorders. Data were collected from 153 participants of the Korean Longitudinal Study on Cognitive Aging and Dementia cohort who completed the longitudinal dental examinations and cognitive function assessments using the three-wave biannual survey. We analysed the relationship between dental factors and the conversion of cognitive function. The ratio of maxillary removable partial denture use (p = .03) was high in the converter and mild cognitive impairment/dementia groups. The low-grade ratio of posterior masticatory performance increased in the converter and mild cognitive impairment/dementia groups (modified Eichner index 2, p = .04). The mild cognitive impairment/dementia group had a higher rate of complete mandibular denture use (p < .001). The converter and mild cognitive impairment/dementia groups had fewer remaining teeth (p < .05) or removable prostheses (p < .01) than the normal group. Masticatory performance is associated with the conversion of cognitive disorders. Our findings suggest that oral health management can help delay the progression of cognitive disorders.

Removable partial denture - Accuracy in digital workflow

This study aimed to compare the fit and accuracy of metal frameworks for removable partial dentures (RPDs) made with conventional and digital fabrication techniques. On a representative model, maxillary RPD metal frameworks (N=5, n=2 each group) were created. A conventional casting of milled RPD patterns (C-M), and a conventional casting of printed RPD patterns (C-P), were all used to create RPDs. By creating replicas with silicone registration material and measuring under a digital microscope, the fit accuracy of RPD frameworks was examined.For the accuracy measurements (m), a total of 11 sites and 29 areas in the RPD metal frameworks were taken into account. Wilcoxon signed rank and the Friedman test were used in the statistical analysis of the data (p 0.05). Prior to finishing and polishing, the fit accuracy of the C-M approach (118 m) was significantly superior (P .001) when compared to other techniques. After finishing and polishing, the C-M approach demonstrated comparable fit accuracy to the C-P. When RPD precision is taken into account, it was observed that digital techniques were efficient and clinically acceptable.

Upgrading removable denture design by using strategic implants: A case report

Inserting a dental implant in a strategic position under a removable partial denture (RPD) can upgrade the RPD design by changing a mainly tissue-supported RPD to an implant-tissue-supported RPD or an implant-tooth-tissue-supported RPD with better retention, support, and stability. Case presentationThe patient (female, 77-year-old) was unsatisfied with her maxillary and mandibular clasp-retained removable dentures (RDs). Medical, social, and systemic health evaluations revealed that the patient was healthy. Dental history, intraoral and extraoral examinations, functional screening, and esthetic analyses revealed that the patient had a reduced vertical dimension, resulting in pseudo-class III malocclusion and angular cheilitis. Strategic implants were used to improve the retention, support, and stability of the new RDs. The maxillary canine 23 (World Dental Federation notation) and maxillary premolar 24 were used to retain the maxillary conical crown attachments. Three strategic mini-implants were inserted in the positions of teeth 12, 13, and 14 to support and retain the maxillary RPD. Two locators above the standard implants (33 and 43) were used to support and retain the mandibular overdenture. An improvement in the oral health-related quality of life and patient satisfaction with respect to the maxillary and mandibular RDs was observed immediately after using the new RDs. This improvement did not diminish after a year. ConclusionHerein, using strategic implants to support the maxillary RPD and two standard implants under the mandibular overdenture improved the oral health-related quality of life and patient satisfaction with respect to the maxillary and mandibular prostheses in terms of the retention, stability, support, eating ability, speaking ability, appearance, and cleanability.

Tooth-Supported Overdenture Retained with Ball Attachments and Custom-made Coping: A Case Report

&#x0D; Introduction: Tooth-supported overdentures is a simple and cost-efficient modality that helps preserve bone. This concept is completely supported by DeVan’s maxim: “Perpetual preservation of what is left is more important than the exact replacement of what is lost.” The preserved tooth structure beneath the denture base prevents alveolar ridge resorption, enhances proprioception, and improves denture support and stability. Additionally, using copings on the remaining teeth improves the denture’s retention.&#x0D; Case Report: A 49-year-old female patient reported having the chief complaint of difficulty in chewing food due to missing teeth in the upper and lower arch. An intraoral examination revealed a partially edentulous maxillary arch with denture-induced stomatitis as the patient had a faulty maxillary removable partial denture for five years. The mandibular arch was also partially edentulous, and only mandibular left second premolar and right first and second premolar were present. The patient was advised to stop using maxillary removable partial denture (RPD) altogether and was given an antifungal tablet as well as a multivitamin tab and topical clotrimazole. This case report describes prosthodontic rehabilitation of a mandibular’s partially edentulous arch with tooth-supported overdenture using ball attachments and custom-made coping.&#x0D; Conclusion: For edentulous patients with few remaining teeth, a mandibular tooth-supported overdenture is one of the best and most practical, feasible, and comfortable treatment alternatives. The patient was satisfied with the treatment outcome.

Effects of selective laser melting process parameters on the accuracy of the intaglio surface of maxillary removable partial denture frameworks

PurposeThis in vitro study aims to explore the effects of selective laser melting (SLM) process parameters on the accuracy of the intaglio surface of cobalt–chromium alloy (Co–Cr), commercially pure titanium (CP Ti) and titanium alloy (Ti–6Al–4V) maxillary removable partial denture (RPD) frameworks and optimize these process parameters.Design/methodology/approachMaxillary RPD framework specimens designed on a benchmark model were built. The process parameters, including contour scan speed and laser power, infill scan speed and laser power, hatch space, build orientation and metallic powder type, were arranged through the Taguchi design. Three-dimensional deviations of the clasps area, connector area and overall area of maxillary RPD frameworks were analyzed by using root mean square (RMS) as a metric. One-way analyses of variance with the above RMSs as the dependent variable were carried out (α = 0.05).FindingsMaxillary RPD frameworks built horizontally had a more accurate intaglio surface than those built at other orientation angles; CP Ti or Ti–6Al–4V maxillary RPD frameworks had a more accurate intaglio surface than Co–Cr ones; the Maxillary RPD framework built with a higher infill scan speed and lower infill laser power had the more accurate intaglio surface than the one built with other levels of these two process parameters.Originality/valueA novel benchmark model for evaluating the accuracy of the intaglio surface of maxillary RPD frameworks manufactured by SLM is proposed. The accuracy of the intaglio surface of maxillary RPD frameworks can be improved by adjusting SLM process parameters. The optimal setting of process parameters concerning the accuracy of the intaglio surface of maxillary RPD frameworks was given.

Strength of Polyether Ether Ketone Composite as a Major Connector Material for Removable Partial Dentures

PEEK composite was tested mechanically as a maxillary removable partial denture (RPD) framework material instead of cast cobalt chromium alloy. Methods: Partial edentulous upper jaw cast was scanned using structured-light 3D scanner, palatal strap (PS) designs for RPD were designed by a designing dental laboratory software. CAD/CAM machine fabricated PSs patterns using their designs’ STL software files. PSs were made from poly-ether-ether ketone (PEEK) reinforced by ceramic fillers using thermal injection press technique and Co-Cr alloy using centrifugal casting, each material group was subdivided into two subgroups according to storage conditions (stored and non-stored subgroups), storage took place in deionized water for about 180 days at 37.5 ± 2 o C. All specimens were subjected to fracture resistance test using universal testing machine then maximum compression load (MCL) result values were subjected to statistical analysis. PEEK composite specimens were scanned by field emission microscope (FEM) and energy dispersive spectroscopy. Storage water of PEEK composite was analyzed using atomic absorption spectroscopy (AAS). Results: In either stored or non-stored subgroups of PEEK composite straps they showed significant lower mean MCL values than corresponding alloy subgroups (p=0.0001). FEM scanning showed fillers agglomerations in non-stored PEEK composite and their nearly absence from stored PEEK composite specimens. AAS detected Al element in PEEK composite storage water. Conclusion: Mechanically thermally injected ceramic reinforced PEEK composite could not replace cast Co-Cr alloy as PSs material for maxillary RPD. Biocompatibility concerns raised in this study due to suspected Al leaching and ceramic fillers dissolution from PEEK composite matrix.

Rehabilitation of maxillary arch with attachment-retained reinforced single complete denture

Abstract Introduction: Fracture of maxillary complete denture is a common problem for dentists and denture wearers, occurring especially when it is opposing mandibular natural teeth or fixed and implant-sustained restorations; to prevent it, various methods and materials are available. Case presentation: This case is of a patient losing his old maxillary removable partial denture due to the mobility of the frontal abutments. Considering the age of the patient and the status of the mandibular arch, completely restored and having a mix of natural teeth, fixed and implant-sustained restorations, the functional and esthetic rehabilitation of the maxillary arch, and also the long-term resistance of the denture, are a serious prosthetic challenge. An association between ball attachments, metal-casted reinforcement and masking agents for both are described, underlying their role in the treatment’s succes. Conclusion: The age and health status of the patient and the situation of the mandibular arch imposed the construction of a functional, strong and stable maxillary denture; the technique described in this article offers a good functional and esthetical result and benefits the patient, even if the implant-prosthetic therapy is not available.

Palatal swelling in an 84-year-old woman

An 84-year-old woman initially went to her general dentist for evaluation of a “little bump on the roof of [her] mouth” that prevented her from wearing her maxillary removable partial denture. The denture was relined, but the lesion persisted and increased in size, causing significant pain. The patient was then referred to an oral and maxillofacial surgeon for evaluation. The patient’s medical history was significant for a heart murmur and multiple basal cell carcinomas of the skin. The review of systems was otherwise noncontributory, and results of the extraoral examination were within normal limits.

Pure titanium denture large-span frameworks additively manufactured with selective laser melting

Objective: To investigate the accuracy of pure titanium and cobalt-chromium alloy frameworks fabricated using the additive manufacturing (AM) of selective laser melting technology (SLM) for the mandibular implant-supported fixed prostheses and the maxillary removable partial denture (RPD), and to provide a reference for clinical application of SLM pure titanium frameworks. Methods: One edentulous mandibular model with implants and screw fixed abutments at bilateral canines and the first molars was selected and used as the mandibular full arch implant-supported model. At the same time, a Kennedy class Ⅰ maxillary dentition defect model was selected. The digital models were obtained by scanning the dental models, and the metal frameworks of the mandibular full arch implant-supported denture and the maxillary RPD (design model) were designed using the 3 Shape software. Meanwhile, 12 mandibular frameworks in the cobalt-chromium alloy and the pure titanium (6 in each group were treated with heat treatment, while the other 6 were not treated), and 7 maxillary frameworks in the cobalt-chromium alloy and the pure titanium were respectively made by SLM with the improved dual-laser metal printer. The axial direction of the printing powder accumulation was taken as the Z-axis. During the design process, the software (3Shape Dental System 2018) automatically generated the X-axis and Y-axis, X axis was the sagittal axis of the dental model and Y axis was the coronal axis of the dental model. The deviation of the interface center of the abutment of the digital model of the mandibular frameworks from the design model in the X, Y and Z axes was analyzed. As for the trueness of the mandibular framework, the larger the deviation data was, the worse the trueness was. The deviation of the whole maxillary framework and 7 measuring points (palatal plate center point and bilateral occlusal rests, I bars, proximal plates) were analyzed. The fitness of the whole maxillary framework to the design model was expressed by root mean square (RMS) of the deviation data, and the fitness of measuring points was expressed by the mean±standard deviation of the data. The trueness differences of each group before and after heat treatment of the mandibular framework and the fitness of the maxillary framework were compared. Results: The cobalt-chromium alloy frameworks showed lower trueness on the X, Y, Z-axes [(96.3±12.1), (86.3±11.4), (61.2±13.2) μm] than did the pure Ti frameworks [(82.3±11.2), (72.2±10.2), (51.2±11.6) μm] by SLM, and the heat treatment could reduce the discrepancy between the SLM frameworks and STL models, for pure titanium frameworks [(62.4±11.3), (55.2±13.2), (41.3±10.8) μm] and for cobalt-chromium alloy [(84.5±10.5), (72.3±11.2), (54.2±11.6) μm]. For the thin RPD major frameworks, pure titanium had better fitness [(121.3±17.0) μm] than cobalt-chromium alloy [(174.0±18.3) μm] by SLM, and the difference was statistically significant (P<0.05). Conclusions: Pure titanium frameworks fabricated by SLM additive manufacturing technology exhibited better fitness and trueness than did the Co-Cr frameworks after heat treatment respectively, and this satisfied the requirements of implant-supported fixed prostheses and RPD major metal frameworks.

GOHAI and OHIP-EDENT Evaluation in Removable Dental Prostheses Users: Factorial Analysis and Influence of Clinical and Prosthetic Variables.

To assess the degree of satisfaction and quality of life of patients with removable prostheses using the Geriatric Oral Health Assessment Index (GOHAI) and Oral Health Impact Profile in Edentulous Adults (OHIP-EDENT), in addition to evaluating the questionnaires for their validation. Eighty patients participated in the study after completing prosthetic rehabilitation treatment. Patients were evaluated using the OHIP-EDENT and GOHAI questionnaires to assess the criteria for quality of life, esthetics, phonetics, masticatory function, stability, comfort, and alteration of taste and pain related to the completed prostheses. The results were subjected to the chi-squared test, Fisher's exact test, and a multinomial logistic regression model, with a significance level of 5%. The most commonly used removable prosthesis was mandibular removable partial denture (RPD) (n = 43, 53.8%), followed by maxillary RPD (n = 42, 52.5%), maxillary complete denture (CD) (n = 33, 41.3%), and mandibular CD (n = 31, 38.8%). Most patients had to have one (n = 29, 36.3%), two (n = 21, 26.3%), or none (n = 18, 23.8%) adjustments in their prostheses. Five (6.3%) patients' prostheses required three adjustments, and six (7.5%) required four or more adjustments. All reported improvement in esthetics after using the prostheses (p < 0.05). The validation of the questionnaire proved to be more viable when completed without the presence of specific domains. The results of this study indicated that removable prostheses improved the quality of life of rehabilitated patients. The questionnaires used need to be answered and evaluated in their entirety for validation.

FREQUENCY OF TYPE OF COMMON MAJOR CONNECTOR DESIGN EMPLOYED IN REMOVABLE PARTIAL DENTURES PROVIDED TO PATIENT

Objective:&#x0D; To determine the frequency of the types of common major connector designs in removable partial dentures provided to patients.&#x0D; Materials and Methods:&#x0D; A cross-sectional study was done at Khyber College of Dentistry. Participants fulfilling the inclusion and exclusion criteria were invited to take part in the study. The information was collected using a single interviewer-administered questionnaire. The demographic information of the patient was recorded. Information related to the design of major connector wasrecorded after it was drawn on the cast, before it was sent to the laboratory. Data was analyzed using SPSS version 17.&#x0D; Results:&#x0D; Data from 207 subjects were collected over a period of 24 months. The mean age of subjects was 41.21 ± 9.13 (SD) years. Out of 207 subjects, 41.1% were males and 59.9% were females. Female to male ratio was 1.43:1. Out of total, 49.3% were maxillary and 50.7% were mandibular removable partial dentures.Removable partial dentures constructed from cobalt-chromium accounted for 152 of the observed cases.Of the 152 observed cases of removable partial dentures made of cobalt-chromium, 81 were in the mandibular arch and 71 were maxillary. The remaining 55 were made of acrylic resin, out of which 31 were maxillary and 24 belonged to the mandibular arch.The study concluded that the horse shoe shaped connector is the most common type of connector used in the maxillary removable partial dentures (both cast partial and acrylic RPD’s) and the lingual plate is the most frequent type of connector given in the mandibular removable partial dentures.&#x0D; Conclusion:&#x0D; The studyshowed that common maxillary cobalt-chromium major connectorwas the horseshoe shape, and that of mandibular cobalt-chromium major connector design was the lingual plate. Additionally, common acrylic RPD were horseshoe shape in maxilla and lingual plate in mandible.

Influence of Oral Morphology on Speech Production in Subjects Wearing Maxillary Removable Partial Dentures with Major Connectors

Background/Aims: Speech impairment during the initial phase of removable partial denture (RPD) treatment can prevent patient adaptation to RPDs. This study was undertaken to investigate the influence of oral morphology on speech production in subjects wearing RPDs with major connectors. Methods: Two types of connectors were fabricated for 17 subjects with normal dentitions: covering the middle palate (M-bar) and the anterior/posterior palate (AP-bar). Four target sounds ([∫i], [t∫i], [çi], and [ki]) were evaluated under 3 recording conditions: no connector, M-bar, and AP-bar. The mean appearance ratios of correct labels (MARCs) were calculated as parameters representing speech production accuracy with the speech evaluation system. Subgroup analysis was conducted based on palate height, dental arch width, and front space volume of the oral cavity. Results: Based on the multiple linear regression test, a significant association was found between the MARCs of [∫i] with M-bar and front space (p = 0.036). In the subgroup analysis, the AP-bar had a significant effect on the MARCs of [∫i] among subjects with high palate (p = 0.026), narrow arch (p = 0.004), and small front space (p = 0.014). Conclusion: RPDs with major connectors could disturb speech production among patients with high palates, narrow arches, and small front spaces.

Impending total airway collapse involving a maxillary removable partial denture: A clinical report

This article presents a rare incident of a bilateral maxillary removable partial denture as an airway foreign body. The removable partial denture and the surrounding soft tissue irritation caused partial obstruction of the airway. Impending airway collapse was avoided after successful removal of the foreign body. Removing a removable dental prostheses at night, not only to prevent soft tissue pathology but also to avoid ingestion or aspiration of the prosthesis, must be emphasized.

Maxillary removable partial denture combined with O’ring attachment implant: a case report

Oral rehabilitation with free-end removable partial denture (RPD) is one of the major challenges in prosthodontic dentistry. The absence of a distal abutment produces undesirable RPD movement during masticatory function due to the high resilience of the residual ridge, which limits the denture efficiency and may damage the abutment teeth and bone over time. The advent of dental implants made possible to substitute the missing teeth with fixed implant-supported dentures as the first choice treatment to overcome such inconveniences. However, this indication may not be suitable for all patients due to financial, anatomical or systemic health conditions. Nevertheless it is possible to improve free extension RPD by using implants in the posterior edentulous ridge to achieve biological, biomechanical, physiological and social benefits. This article aims to present a case report on oral rehabilitation in which a RPD was made combined with an implant for posterior support in a sizeable edentulous ridge. The patient appreciated the retention improvement and the aesthetic result. The combination of a RPD with a posterior implant is an alternative treatment for cases in which implant-supported fixed prosthesis is not indicated, reducing the displacement and minimizing the limitations of the free-end devices.

Objetivo: o objetivo deste estudo foi avaliar a influência do edentulismo parcial ou total na qualidade de vida de pacientes idosos. Material e Métodos: a amostra foi composta por 120 indivíduos, divididos em pacientes desdentados com prótese total bi-maxilar (CD) (n = 60) ou parcialmente desdentados com CD maxilar e prótese parcial removível mandibular (RPD) (n = 60). A qualidade de vida dos pacientes foi avaliada pelo questionário OHIP-EDENT-19 e os dados foram analisados ??no software SPSS. Resultados: os pacientes em uso de RPD apresentaram maior qualidade de vida em relação aos usuários de CD duplo, de acordo com os valores apresentados no OHIP-EDENT. Diferenças estatisticamente significantes foram encontradas quando comparados os grupos entre queixas relacionadas à mastigação (dificuldade de mastigar, desconforto ao comer, evitar comer, interrupção de refeições), desconforto psicológico e incapacidade (incapacidade de comer), incapacidade social (vida insatisfatória) e boca dolorosa desconforto (próteses inadequadas, dor na boca). Conclusão: o tipo de prótese utilizado pode influenciar a qualidade de vida autorreferida.

Prosthetic rehabilitation in a patient with Singleton Merten syndrome and acrylic resin hypersensitivity

Singleton Merten Syndrome is a rare disease characterized by the presence of the dental dysplasia phenotype, calcifications in the aorta, progressive wear and loss of bone protein (osteoporosis) in the hands and feet. Patients have muscle weakness, poor motor development, abnormal dentition, deformities of thefeet and hands, and skin lesions. Objective: This report describes the maxillomandibular rehabilitation of a patient with Singleton Merten Syndrome and an allergic reaction to the acrylic resin through maxillary overdenture and mandibular partial removable denture. Case report: Female patient, 18 years old, with clinical characteristics of Singleton Merten Syndrome and allergic reaction to acrylic resin, with complaints of loss of function and esthetics dueto the absence of several teeth, but with the presence of unerupted maxillary and mandibular tooth buds. Maxillary overdenture and mandibular removable partial denture were made of polyethylene. Conclusion: The rehabilitation treatment with maxillary overdenture and mandibular partial removable denture provided better facial muscle support, restoring masticatory function and facial esthetics. With this treatment, it was possible to reestablish patient’s satisfactionand self-esteem due to the correct construction of the prostheses, and minimize hypersensitivity reactions in the oral mucosa, which allowed the use of these prostheses by the patient without any complications.

Clinical and radiographic comparative study between two different types of clasps materials used in unilateral maxillary Kennedy class II removable partial denture cases

Objective: The objective of the present study was to compare between two different clasp materials for the treatment of maxillary class II Kennedy classification clinically, radiographically.Materials and methods: Twenty male patients were selected for this study. The patients were divided into two groups. Each group consists of 10 patients. For group I each patient have maxillary removable partial denture that was constructed and retained by cobalt–chromium RPI clasp on the maxillary first premolar with cross arch stabilization (using double Aker clasp) on the other side. For group II each patient has the same except that the retentive I bar of RPI clasp was made of acetal resin. Patient satisfaction, clinical, and radiographic evaluations were carried out for every patient at insertion, 3, 6, and 9 months, the data were collected, tabulated, and statistically analyzed using t-test.Results: Satisfaction for the treatment. Group II patients were more satisfied of the treatment regarding aspects of the patients' comfort and esthetics. No significant difference in the aspects of phonetics, the ability to eat, prosthesis loosening, the gingival health, general satisfaction and food impaction between the two groups were found, also there were insignificant increase in the gingival recession, pocket depth, and plaque index around the abutments tooth in both groups. Radiographically, there was significant decrease in the bone height around the abutments in both groups.Conclusion: Removable partial denture with acetal resin clasp is better for esthetic and patient comfort. Good periodontal condition of the abutment tooth of both groups is maintained. Acetal resin clasp is the clasp of choice when esthetic is of primary concern to the patient.
