Work Experience of Medosmotr39 LLC in Organizing and Conducting Medical Support for the Floating Staff of the Fishing and Scientific Fleet in the Subject of the Russian Federation

There is a problem in the country of comprehensive and competent pre-voyage medical examination of seafarers. Since the implementation of Federal Law no. 326 dated 11/29/2010 On Compulsory medical insurance in the Russian Federation, the issues of compulsory medical insurance in the field of marine medicine have not been resolved. Crew members with chronic diseases are sent on the flight without prior medical examinations. There is no medical worker on the ships, a member of the crew is responsible for the medical part without a license to provide medical care, who does not have the right to provide it, which in turn led to an increase in morbidity and deaths on the voyage. There is no single standardized form of laying first-aid kits on ships, which also affects the health of the crew. This situation prompted the medical organization Medical Examinations 39 to open a Marine Medicine Department at its base, which significantly improved the situation in the region. We also propose to legislate the issue of a single form of packing first-aid kits on flights, the composition of which will depend on the time of year, natural and social conditions of the area to which the sailors will go.

Oleg Konstantinovich Bumay (in memory of a teacher, colleague, comrade)

January 1, 2024 marked the 70th anniversary of the birth of the former head of the organization and tactics of the medical service of the fleet (with a course of tactics and combat weapons of the fleet) department of the S.M. Kirov Military Medical Academy, effective director for modern topics of the Federal State Budgetary Institution Research Institute of Industrial and Marine Medicine FMBA of Russia, candidate of medical sciences, associate professor Oleg Konstantinovich Bumay (1954–2022). O.K. Bumay was born in the village of Bogudenka, Belarus. He was alumnus the faculty of training doctors for the Navy of the S.M. Kirov Military Medical Academy. After graduating from the academy in 1977, for a decade he provided and organized medical support for nuclear submarines as the head of the ship’s medical service, the cheef of medical a submarine forces in the Northern Fleet, and subsequently served as a senior medical officer in the Baltic Fleet. Over the course of two decades, O.K. Bumay was associated with the organization and tactics of the naval medical service (with a course of tactics and combat weapons of the fleet) department of S.M. Kirov Military Medical Academy. In 1991, he entered the adjunct program at the department; upon completion of his studies, he was appointed as a teacher, subsequently as a senior teacher, and since 2009 — deputy head of the department. He received the rank of Philosophy doctor of Medical Sciences (1994), associate professor (1998). After leaving the Armed Forces of the Russian Federation in 2007, he continued his career as deputy director for special topics at the Federal State Budgetary Institution Research Institute of Industrial and Marine Medicine of the Federal Medical and Biological Agency of Russia. O.K. Bumay developed origins medical support system for the domestic nuclear submarine fleet. Was appointed on the post the head of the ship’s medical service and after that officer of the command-and-control body of an operational-tactical formation. Throughout their career he left a multifaceted scientific and practical legacy, which is used by new generations of naval doctors.

It emphasises the potential for qualified paramedics to pursue specialised training in marine paramedicine through postgraduate programmes, ultimately broadening their skill set.

A Comprehensive Review on the Role of Chemotype Marine Derived-Drug Discovery

Abstract: The global population's rapid expansion is a worldwide concern, which has led to higher medication and resource consumption. As a result, there is a tremendous need to seek out new means of producing reliable medications to meet the rising demand of a global populace suffering from a wide range of health problems. Various resources are available in marine habitats for the development of novel medications. Their life circumstances are radically different from those found in a terrestrial setting. In order for marine animals to thrive in the ocean, they produce a variety of secondary metabolites, which can possibly be life-saving bioactive compounds that come from an increasing variety of marine microorganisms. These metabolites have pharmacological properties that make them intriguing as a potential for human medications. Therefore, there has recently been a rise in interest in marine-derived biomolecules as potential treatments. Utilizing a wide range of screening methods, we can investigate the effects of these extracts and purified compounds from marine organisms in the medicinal industry, such as cancer prevention, inflammation reduction, virus and bacteria inhibition, ion channel/receptor modulation, and plant growth stimulation. The structures of bioactive substances will be determined after they have been isolated chromatographically. Marine-based bioactive compounds can be (semi) synthesized to make new derivatives, structural analogues, and copies that can be used to build new marine-based chemical catalogs and contribute as lead or hit molecules. This overview classifies FDA-approved marine-based drugs and provides information on their origins, chemical composition, manufacturing processes, and pharmacology. This paper outlines the supply dilemma in marine medicine development.

Role and place of maritime medicine

The article considers the role and place of maritime medicine as science, as a separate health care industry and as the academic discipline. Maritime medicine is at the crossroads of 2 vast areas of human knowledge – medicine and oceanology. As medical science, maritime medicine explores the impact of Ocean on human health in order to develop means and methods of preserving the Earth’s human potential, first of all, the one of maritime industry and coastal territories. Maritime medicine includes a set of private medical sciences, such as marine hygiene, marine epidemiology, medicine for ships, diving medicine, naval medicine, marine ergonomics, marine physiology and pathophysiology, marine therapy, marine surgery, marine toxicology, marine pharmacy, history of marine medicine, marine health care organizations and some other disciplines. The article discusses the relationship of maritime medicine with other fields of knowledge, sciences and academic disciplines.

Regulatory and legal bases for the establishment and functioning of seafarers' health protection centers in the Russian Federation

Introduction. Ensuring the national interests of the Russian Federation in the World Ocean presupposes the development of medical and sanitary support for maritime activities, which currently requires integrated approaches in implementing the requirements for the preservation of the health of employees of the Russian fleet established by international legal acts on health and medical care in maritime navigation, of which the Russian Federation is a participant. In this regard, harmonization of legislation and improvement of measures related to the organization and functioning of seafarers' health protection centers are of particular importance.
 The study aims to conduct comprehensive legal monitoring of national regulatory and methodological documents regulating the activities of seafarers' health protection centers in comparison with international standards.
 Materials and methods. The authors have studied and analyzed scientific publications for the period from 1980 to 2023 defining the problems of medical and sanitary provision of water transport workers, methodological approaches of the World Health Organization, the International Labor Organization, the International Maritime Organization in this area, as well as current international and Russian regulatory legal acts, information from official websites of executive authorities and organizations.
 Results. The study found that today the system of seafarers' health protection, which previously existed in our country, has suffered significantly, and its restoration requires an integrated approach and centralization of medical support for maritime activities in accordance with the Maritime Doctrine of the Russian Federation and the Concept of Development of the Russian Federation Marine medicine. To this end, in the structure of organizations subordinate to the FMBA of Russia, in large port cities, head and regional centers for the protection of seafarers' health were established, carrying out accounting and analytical, organizational and methodological, regulatory, therapeutic and preventive, expert, scientific and educational activities. An important direction in the work of these centers is the implementation of preventive measures and the identification of occupational and work-related diseases. For the successful development of a unified system of medical and sanitary support for employees of the marine and river fleet, it is advisable to assign the functions of regulatory regulation in this area to the FMBA of Russia.
 Conclusion. The restoration of a full-fledged domestic system of providing medical care to water transport workers is possible through the creation and regulatory consolidation of the activities of seafarers' health protection centers in the structure of the FMBA organizations of Russia, which is an essential condition for achieving compliance of the national maritime policy with international requirements in the field of health and safety of navigation while maintaining its strategic goals and interests in the field of maritime and river navigation.

Developing the marine medicine syllabus for medical sciences students: a multiphase design study

BackgroundMarine medicine is one of the medical fields that deals with the health and safety of people related to the sea but the marine medicine syllabus for education to the students is not specified yet. The present study aimed to develop the marine medicine syllabus to medical sciences students education.MethodsThis study was conducted in three phases. First, a literature review was conducted to find the concepts and topics related to marine medicine. Second, a content analysis research method was conducted. Data collection was done first by using semi-structured interviews with the 12 experts in marine medicine. Sampling was purposeful and continued until data saturation was reached. The information obtained from the interviews was analyzed by conventional content analysis with Geranheim's method. The found topics in the literature review and content analysis of interviews were combined and formed the initial draft of the marine medicine syllabus, which was validated with the Delphi method in the third phase. The Delphi was conducted in two rounds and the panel consisted of 18 experts in the field of marine medicine. After the completion of each round, the items that had less than 80% consensus among the participants were removed and the remaining topics after round two formed the final syllabus of the marine medicine.ResultsThe findings showed that the marine medicine syllabus should include an overview of marine medicine, health at sea, common physical diseases and injuries at sea, subsurface medicine and hyperbaric, safety action in marine incidents, medical care at sea, psychology at sea and medical examinations of people working at sea main topics and their sub topics.ConclusionsMarine medicine is an extent and specialized medical field which has been neglected and it is necessary to teach this lesson to medical sciences students with the syllabus found in the present study.

Integrated LC-MS and network pharmacology methods to screen quantitative indicators in the Hippocampus histrix Kaup and method transfer

Hippocampus histrix Kaup is a popular marine medicine with high medicinal and healthcare values. In this study, liquid chromatography-mass spectrometry (LC-MS) analysis combined with network pharmacological method was used to screen for suitable quantitative indicators for the quality control of H. histrix Kaup. Firstly, an LC-MS analytical method for the simultaneous determination of 12 nucleosides in extracts of H. histrix Kaup was established. And then, a network pharmacological method incorporated target prediction, protein-protein interaction network, components-targets network, and targets-pathways network was performed to screen for quantitative indicators. Finally, the developed LC-MS method was transferred to liquid chromatographs to improve the generalizability of the method. All 12 nucleotides were authenticated in extracts of H. histrix Kaup by comparing with the standards. The optimal chromatographic separation conditions are as follows: the chromatographic separation was achieved on an Acquire BEH-C18 column (2.1 mm * 100 mm, 1.7 µm) and gradient elution was performed using methanol solution and buffer (0.30% formic acid and 10 mmol/L ammonium acetate) as mobile phase at a flow rate of 0.15 mL/min and an acquisition wavelength of 260 nm. Network pharmacology results showed that adenosine, and uridine show excellent pharmacological activity. Integration the content, correlation, chromatographic separation, and pharmacological activity of each compound in H. histrix Kaup, uridine and adenosine were tentatively determined as quantitative indicators for quality control in H. histrix Kaup. The established LC-MS method was successfully transferred to liquid chromatographs, and the method is stable and reliable for the quality control of H. histrix Kaup. This developed integrated strategy was successfully used to screen quantitative indicators in the H. histrix Kaup.

Prevalence of arterial hypertension in Vietnamese seafarers aboard merchant vessels: a cross-sectional study.

Hypertension is one of the leading causes of morbidity and mortality globally. It is a major risk factor for major cardiovascular events such as stroke, myocardial infarction, heart failure, kidney failure, and blindness. The aim of this research is to assess the prevalence and some factors related to arterial hypertension on Vietnamese seafarers aboard merchant vessels. Seven hundred eight Vietnamese seafarers working aboard merchant ships were examined at the Institute of Marine Medicine before going to sea during the period from January 2022 to December 2022. It was a cross-sectional descriptive epidemiological study. The following parameters were measured: blood pressure, height, weight, waist circumference, buttock circumference to assess the prevalence of hypertension, overweight, and obesity. Seafarers we directly interviewed about workplace on ships and physical exercise, smoking tobacco, alcohol abuse, and anxiety symptoms to identify several factors associated with hypertension. The prevalence of hypertension in seafarers was 32.9%, prehypertension 26.4%, overweight 32.4%, obesity 13.3%, abdominal obesity 47.7%. Factors related to hypertension of seafarers included: job duration at sea > 10 years, odds ratio (OR) = 8.23 (95% confidence interval [CI] 4.34-17.27); non-officers, OR = 2.11 (95% CI 1.45-2.82); engine room crew, OR = 2.11 (95% CI 1.45-3.58); obesity, OR = 3.34 (95% CI 2.15-5.63); abdominal obesity, OR = 9.12 (95% CI 4.23-18.45); current smoking, OR = 1.32 (95% CI 1.02-1.99); irregular exercise, OR =1.43 (95% CI 1.03-2.18); anxiety symptoms, OR = 1.56 (95% CI 1.08-2.27). Hypertension is a health problem for Vietnamese seafarers. To minimise hypertension, seafarers need to adjust their lifestyle, increase regular exercise and improve psychological issues on board.

Bibliometric Analysis of Marine Traditional Chinese Medicine in Pharmacopoeia of the People's Republic of China: Development, Differences, and Trends Directions.

Marine traditional Chinese medicine (MTCM) is a class of traditional medicine that has antitumor, anti-inflammatory, and antiviral properties. Bibliometric approaches were used in this study to conduct systematic research in order to gain a complete picture of MTCM research around the world. CiteSpace and NoteExpress software were utilized as tools to examine the information about authors, sources, keywords, etc. Chinese publications were collected from the CNKI, VIP, and WANFANG databases; English publications were collected from the Web of Science database. A total of 10080 publications were screened, and the search volume of Chinese literature is greater than that of English literature; Nanjing University of Chinese Medicine, China, and Jeju National University, South Korea, published a greater number of articles than other institutions; the scholars Zhaohui-Zhang and Youjin-Jeon have published the highest number of articles in the world. MTCM of shells was often researched for inorganic elements, and data mining methods were applied frequently; MTCM of animals was commonly used for antifatigue and was taken authenticity identification owing to the scarcity of resources; scholars conducted the most research on MTCM of plants, this category usually for antitumor, anti-inflammatory, and antioxidant purposes, and the mechanisms of action were studied in depth. The Chinese literature has undertaken a multifaceted research study based on the theories of processing and the nature of TCM. In the English literature, in-depth studies have been done from the perspectives of the mechanism of action, the extraction and purification of active substances, etc. According to the analysis of keywords, different medicinal parts present their own special research directions, and different research hotspots have also emerged under different medical theories. The development of MTCM is moving in the direction of standardization and modernization, thanks to the development of cross-disciplinary research as well as the use of several new technologies and statistical techniques.

ОПЫТ ПРОФИЛАКТИКИ ЭМОЦИОНАЛЬНОГО СТРЕССА С ПОМОЩЬЮ МЕТОДИК МУЗЫКОТЕРАПИИ И МЕДЛЕННЫХ МАНИПУЛЯЦИОННЫХ ДВИЖЕНИЙ У РЫБАКОВ ВО ВРЕМЯ ДЛИТЕЛЬНОГО МОРСКОГО РЕЙСА

Under the effect of extreme factors, compensatory and adaptive mechanisms are triggered in the body that change the metabolism and functional state of organs and tissues. With short-term exposure to these factors, homeostasis practically does not change, and with prolonged and repeated effects, stress and corresponding pathology may occur. In marine medicine, the problem of stress is particularly relevant. The adaptation of the seafarers' body should be carried out to a significant number of adverse factors in a relatively short time with sharp changes in the navigation regions. There are many ways to improve or correct a person's condition. Among them, relatively simple and inexpensive ones are of interest. The aim of the study is to evaluate the results of the use of various methods of psychological relaxation for the prevention and reduction of emotional stress among fishermen during a long sea voyage. Materials and methods: seafarers were examined during a long fishing voyage for psychoemotional stress detecting. Seances of psychological and emotional relaxation – music therapy and exercises of the technique of slow manipulative movements were conducted among individuals with identified signs of reactive anxiety. Results of the study: upon completion of all full cycles of emotional and psychological unloading, a positive effect was revealed in most of the subjects. 6 days after the start of the music therapy cycle, the level of reactive anxiety normalized in half of the participants, and after another 4 days, almost all individuals achieved a decrease in anxiety to at least moderate. In an experiment with the technique of slow manipulative movements (counting rosaries), after a week, the normalization of the level of anxiety occurred in 20% of the subjects, and in more than 2/3 of the sailors it decreased to moderate. A week later, almost all the subjects' anxiety levels returned to normal.

Scientific research in the development of medical support for the nuclear fleet (to the 75th anniversary of the FMBA of Russia)

August 21, 2022 marks the 75th anniversary of the Federal Biomedical Agency, founded on the basis of the Third Main Directorate under the Ministry of Health of the USSR specifically «for the atomic project». The basis of the activities of the FMBA of Russia, as the world’s only scientific and practical community of scientists and specialists in various fields, are innovations and dual-use technologies aimed at preserving human life and health. The article discusses the main periods of formation and development of scientific research of the Research Institute of Industrial and Marine Medicine of the FMBA of Russia, aimed at ensuring radiation safety and medical support of the Russian nuclear fleet.

ПСИХОЛОГИЧЕСКАЯ И МЕДИЦИНСКАЯ БЕЗОПАСНОСТЬ ПЕРСОНАЛА НА МОРСКИХ СУДАХ И ПОДГОТОВКА СПЕЦИАЛИСТОВ ПО МОРСКОЙ МЕДИЦИНЕ

Abstract. Due to the significant strategic importance of the marine transport system, the issue of ensuring various types of security (especially medical, psychological) for personnel working in this branch of the national economy seems relevant. The Marine Doctrine of the Russian Federation and the corresponding Decree of the President of the Russian Federation No. 512 dated July 31, 2022 is the leading document regulating these issues. The article presents various groups of factors that affect the health of seafarers (genetic, medical support, living conditions and lifestyle, the state of the environment). The negative aspects of these factors that impair the health of the crew are listed: disorders of adaptive processes in the body, up to stressful conditions; the occurrence of diseases of the body systems (cardiovascular, endocrine, nervous, sensory, musculoskeletal system); the purchasing of various kinds of dependent states (alcohol and nicotine); decreased physical activity, eating habits, satisfaction from their work, psychological and emotional stress disorders social isolation, fatigue. The problem of the shortage of specialists in the marine medicine, despite the requirements of word standards, is emphasized. In this regard, the problem is raised about the training of specialized medical personnel (including in the field of psychological support) in medical educational organizations for prevention, sanitary, educational and hygienic work with personnel, monitoring the health of seafarers, timely identification of premorbid conditions, quality treatment and rehabilitation. To solve it, first of all, an integrated approach at the state level is needed. First of all, it is proposed to improve the legislative basis, and then the implementation of documents at all levels of administrative authority.

The maritime medicine in Morocco: reality and future prospects

INTRODUCTION: The maritime sector in Morocco constitutes an important economic and social lever of the country, in particular with the two maritime facades of the country. If the fishing sector is classically the dominant part of this system, the change experienced by the Moroccan economy over the past two decades and the development of the port infrastructure of the merchant marine have allowed the latter to regain its importance. This development of the marine sector has repercussions on seamen and calls into question the importance of the social issue in this area, particularly in relation to the medical care of this population.OBJECTIVES: To research the scope of medical care issues related to merchant marine workers.MATERIALS AND METHODS: This study was based on the collection of data: legislative texts on occupational health and safety; statistical data from administrations (the Ministry of Health and the Ministry of Marine Fisheries), the General Medical Council of Morocco, scientific societies and associations for occupational health and marine medicine. The analysis included socio-demographic and professional characteristics, training, evaluation of activities, limitations in its implementation and priority recommendations for improving the situation.RESULTS AND DISCUSSION: Morocco’s health system includes two areas: the commercial and fishing sectors. The study showed that there are no specialized medical centers in any of the sectors, and medical care for this population is at a low level. There is no Moroccan telemedicine system. Currently, Morocco has already defined a legal framework for this practice.CONCLUSION: This article sheds light on the health system of seamen in Morocco while highlighting its strengths and weaknesses as well as avenues for reflection for the improvement of existing anomalies.

Gum recession as a medical and social problem of marine medicine

Introduction. An important role in ensuring the aesthetics of the face and smile is played by the integrity of the teeth, dentition, the absence of dentoalveolar anomalies, as well as periodontal health, including the presence of gum recessions. If there are separate publications for inflammatory periodontal diseases in the crew, then there is no information on the occurrence of gum recessions in the crew.The aim of the study was to study the incidence of gingival recessions in the crew, to assess the features of their clinical course and the possibility of eliminating them during the voyage.Material and methods. To identify gingival recessions and the peculiarities of their clinical course after oral sanitation and before a long sea voyage, a dental examination was carried out on 156 men aged 22 to 52 years, who were divided into 2 groups: young (group 1, 105 people) and middle (group 2, 51 people) age. To determine the type of gum recession in the crew, the generally accepted Miller classification was used.Results and its discussion. The high incidence of gingival recessions among the ship crew was determined, amounting to 33,97%. It was found that gum recessions are more often detected in middle-aged sailors (72,55%) than in young sailors (15,24%). Regardless of the age group of sailors, most often, in 90,57% of cases, there were such clinical forms of gum recessions (I, II and III A classes), with surgical removal of which a favorable treatment result is noted.Conclusion. The recession of the gums in the ship’s crew is a medical and social problem, which can be solved during a long sea voyage with the presence of a specialist — a dentist-surgeon.

Astaxanthin alleviates neuropathic pain by inhibiting the MAPKs and NF-κB pathways

Neuropathic pain is a complex condition that usually lasts a lifetime and has a major negative impact on life after injury. Improving pain management is an important and unmet need. Astaxanthin (AST) is a natural marine medicine with effective antioxidant and anti-inflammatory properties and neuroprotective effects. However, few mechanisms can explain the role of AST in the treatment of neuropathic pain. In the present study, we examined its potential to eliminate spinal nerve ligation (SNL) damage by inhibiting the phosphorylation of extracellular signal-regulated kinase (ERK)1/2, phosphorylation of p38 mitogen-activated protein kinase (p38 MAPK), nuclear factor-κB (NF-κB) p65 and the inflammatory response. The results of behavior tests indicated the promising role of AST in analgesic effect in SNL mice. AST decreased the neuronal and non-neuronal activation, the levels of the inflammatory signaling mediators (p-ERK1/2 p-p38 MAPK and NF-κB p65) and inflammatory cytokine expression (interleukin [IL]-1, IL-17, IL-6, and tumor necrosis factor-α [TNF-α]. These results suggest that AST is a promising candidate to reduce nociceptive hypersensitization after SNL.
