Comparative evaluation of Skeletal, Dental, and Soft Tissue Changes in Class II Division 1 Malocclusion Cases Treated with Twin Block and innovative Clear Block Appliance - A Prospective Interventional Study

The present study was undertaken to evaluate and compare the skeletal, dental, and soft tissue changes in skeletal Class II division 1 cases treated with Twin Block and Clear Block appliances using a cephalogram. A total of 40 patients of age between 12-14 years were randomly divided into two equal groups. Group 1: treated with Twin Block appliance and Group 2: treated with Clear Block appliance. The pre-treatment lateral cephalogram was taken and skeletal, dental, and soft tissue parameters were evaluated and the appliance was delivered. After 8 months, another lateral cephalogram of all the cases was taken and analyzed. The pre and post-treatment values were compared between the two groups. The pre-treatment cases were almost comparable in skeletal, dental, and soft tissue features in both groups. There was a significant change in mandibular growth by SNB angle. The retrusion and extrusion of maxillary incisors as well as a proclination and extrusion of mandibular incisors were seen in group 1 while no change was observed in group 2. Treatment with Clear Block appliances has shown significant and favorable Skeletal, Dental and Soft tissue changes which are similar to already proven by the Twin Block appliance. Clear Block provides an esthetic and less bulky option for growth modification with similar results as compared to conventional Twin Block with the additional benefit of preventing lower incisor proclination.

Separation of aligning and leveling stages to control mandibular incisor inclination: A randomized clinical trial

ABSTRACTObjective: To determine whether separating the alignment and leveling phases can reduce proclination of the mandibular incisors. Methods: Eligibility criteria included Class I subjects with an irregularity index of 3-5 mm, 3-4 mm curve-of-Spee (COS), and non-extraction treatment. Thirty adults were randomly allocated into two groups: (1) Control group was leveled and aligned simultaneously with flat archwires progressively to 0.016x0.022-in stainless-steel; (2) Experimental group was aligned first with 0.014-in-superelastic NiTi with mild accentuated COS, then leveled using 0.016x0.022-in beta-titanium accentuated COS archwires and gradually reduced the curve until flat. Mandibular incisor position and inclination were evaluated by cephalometric analysis. COS and irregularity index were evaluated in study models. Assessment was conducted twice after 0.016-in NiTi and after 0.016x0.022-in stainless-steel archwire placements. Dental changes from cephalograms and models were compared within group using paired t-test and between groups using independent t-test. Results: Control group: Round-wire-phase, mandibular incisors tipped labially (4.38° and 1 mm) with intrusion (-1.13 mm); Rectangular-wire-phase, mandibular incisors further intruded and proclined (-0.63 mm and 1.38°). Experimental group: During aligning with round accentuated COS archwires, mandibular incisors tipped very slightly labially (0.75° and 0.50 mm) with no significant intrusion; during leveling with rectangular archwires, incisors majorly intruded (1.75 mm) with slight proclination (1.81°). The experimental group had significant less incisor proclination (control: 5.76°, experimental: 2.56°) with more incisor intrusion (control: -1.75 mm, experimental: -2.13 mm). The COS in experimental group showed significant greater reduction (-2.88 mm) than that of the control group (-1.69 mm). Conclusion: In control group, mandibular incisor proclination was markedly observed in round archwires, with further proclination caused by rectangular archwires. In experimental group, minimal proclination was exhibited when accentuated COS round archwires were used for aligning. Leveling with rectangular archwires caused less proclination with more COS reduction.

Effects of Enlarged Adenoids on Craniofacial Growth in Bangalore Population – A Cross-Sectional Cephalometric Study

Background and Objectives: The purpose of the present cross-sectional study was to determine the effects of enlarged adenoids on the growth pattern of maxilla and mandible and to evaluate the relationship between airway size and lip strain. Methods: Pre-treatment lateral cephalograms of twenty-five children (10 boys, 15 girls) were taken. The children selected for the study were in the age group of 5 – 13 years (mean age of 10 years) during the year 2008- 11. All the tracings were made on 75µm lacquered polyester acetate tracing papers using a 0.05” lead pencil. This study assessed the associations of upper lip strain and upper pharynx size with selected cephalometric measures. The data was analyzed using SPSS (Statistical Package for Social Science, Ver.10.0.5) package. Results: The increased upper lip strain was associated with a forwardly placed maxilla, increased mandibular plane angle and upper pharynx size was associated with decrease in mandibular size (mandibular corpus size), increase in gonial angle (Ar-Go-Gn), decrease in middle third of face (N- ANS) and increase in lower jaw anterior face height (ANS- Me). Also, the dental effect of increase in the degree of mandibular incisor proclination relative to mandibular plane was observed. Conclusion: With an increase in upper-lip strain and decreased upper pharynx size, forward maxillary placement, retrognathic mandible, downward and backward rotation of the mandible, decreased middle third of face, increase in lower anterior face height and increase in degree of mandibular incisor proclination relative to mandibular plane were observed. It was inferred from the present study that a decreased upper airway affects the craniofacial skeletal growth pattern in children.

Three-dimensional evaluation of dental decompensation and mandibular symphysis remodeling on orthodontic-surgical treatment of Class III malocclusion

The purposes of this research were to identify the buccolingual inclinations of the mandibular teeth and the mandibular symphysis remodeling that result from the orthodontic decompensation movement. The sample consisted of 30 adults with Class III dentofacial deformity, who had presurgical orthodontic treatment. Three-dimensional images were generated by cone-beam computed tomography scans at 2 different times (initial and before orthognathic surgery). Three-dimensional virtual models were obtained and superimposed using automated voxel-based registration at the mandible to evaluate B-point displacement, mandibular molar and incisor decompensation movement, and symphysis inclination and thickness. The 3-dimensional displacements of landmarks at the symphysis were quantified and visualized with color-coded maps using 3D Slicer (version 4.0; www.slicer.org) software. The measurements showed high reproducibility. The patients presented mandibular incisor proclination, which was consistent with the movement of tooth decompensation caused by the presurgical orthodontic treatment. Statistically significant correlations were found between the inclination of the mandibular incisors, symphysis inclination, and B-point displacement. Regarding the thickness of the symphysis and the inclination of the incisors, no statistically significant correlation was found. The buccolingual orthodontic movement of the mandibular incisors with presurgical leveling is correlated with the inclination of the mandibular symphysis and repositioning of the B-point but not correlated to the thickness of the symphysis.

3D-analysis of unwanted tooth movements despite bonded orthodontic retainers: a pilot study

BackgroundRecently, reports of unwanted tooth movements despite intact orthodontic bonded retainers have increased. These movements are not subject to relapse but are classified as a new developed malocclusion. The aims of the present pilot study were to analyze the prevalence of unwanted tooth movements despite intact bonded cuspid-to-cuspid retainers and to identify possible predisposing factors.Materials and methodsPlaster casts of all patients finishing orthodontic treatment during three consecutive years were assessed before treatment (T0), after multibracket appliance debonding (T1) and after two years of retention (T2). After multibracket appliance treatment, all patients received a cuspid-to-cuspid flexible spiral wire retainer bonded to each tooth of the retained segment in the upper and lower jaw. The study group (SG) consisted of 44 patients (16 male, 28 female) with tooth movements (T1–T2) of the retained segment despite intact bonded cuspid-to-cuspid retainer and the control group (CG) of 43 patients (19 male, 24 female) without unwanted tooth movements. The casts of the SG were digitized, superimposed and measured. Using the Chi-square test, Fisher´s exact test and Mann–Whitney-U-test (p < 0.05), mandibular plane angle, incisor proclination, oral dysfunctions or habits (T0) and intercanine distance, overjet and interincisal relationship (T0, T1, T2) were compared between SG and CG.ResultsThe prevalence of patients with unwanted tooth movements in one or both jaws was 27.0%. Maxillary retainers were affected more often (20.9%) than mandibular retainers (14.1%). The median amount of tooth movements was 0 to 0.66 mm with large interindividual variations. Oral dysfunctions or habits at T0, such as a lack of interincisal contact at all time points, were associated with unwanted tooth movements.ConclusionUnwanted tooth movements occurred more often with maxillary than mandibular retainers. Patients with oral dysfunctions/habits and without interincisal contact had a higher prevalence of unwanted tooth movements.

Does overbite reduction affect smile esthetics?

To compare the effects of two common methods of overbite reduction on smile esthetics. A prospective clinical trial was conducted with 32 patients in whom overbite reduction was achieved using a maxillary incisor intrusion arch (18 patients) or flat anterior bite plate (14 patients). Clinical and cephalometric records were compared pretreatment (T1), after overbite reduction (T2), and posttreatment (T3). Both treatment groups experienced a reduction in overbite and maxillary and mandibular incisor proclination during treatment (T1-T3). The center of resistance of the maxillary incisor and the incisal edge was significantly intruded in the intrusion arch group during overbite reduction (T1-T2). However, most of the intrusion of the center of resistance was lost by the end of treatment (T2-T3). Both treatment groups experienced a reduction in maxillary incisor display and flattening of the smile arc during overbite reduction. Both overbite reduction methods caused a decrease in incisor display and flattening of the smile arc. Smiles were improved in some patients by the end of treatment. However, reduction in incisor display persisted. Clinicians should take precautions to prevent negative effects of overbite reduction.

Effects of orthopedic skeletal Class III correction in growing subjects using skeletal anchorage and Class III elastics

Aim of the study: The objective of this study was to evaluate the skeletal and dental effects of intermaxillary Class III elastics using bone anchored miniplates compared to an untreated Class III control group.Subjects and methods: 24 subjects (14 females and 10 males) having skeletal Class III malocclusion with maxillary deficiency with an age range of 8-11 years old were divided into two groups 12 patients each (n=12). Group A constituted treatment with Class III elastics anchored to 4 titanium miniplates, 2 anchored in the zygomatic buttress of the maxilla and 2 miniplates anchored in the symphyseal region of the mandible at the lateral/canine region. Group B (n=12) constituted untreated skeletal Class III cases (control group). The treatment and observation periods for both groups were 8 months. Lateral cephalometric radiographs were taken for the patients at start of treatment (T1) and at the end of the treatment/observation periods (T2). The radiographs were analyzed by performing lateral cephalometric analysis. Independent student t test was done to compare variables between the two groups.Results: There was correction of the skeletal Class III in the miniplates group with statistically significant increase in angle SNA (mean 3.50° ± 0.68), A-Nperp (mean 4.04mm ± 1.20), ANB (mean 4.06◦±0.95) and Witts appraisal (mean 4.91mm ± 1.76) .Slight mandibular retrusion occurred with a reduction in angle SNB (mean -0.56◦ ±0.57). Changes in B-Nperp were (-1.85mm±1.36).Overjet correction was obtained and no significant changes in the maxillary incisor angulation was observed. There was proclination of mandibular incisors by (mean 6.43◦±2.11).Conclusions: The use of intermaxillary Class III elastics and miniplates in the orthopedic treatment of skeletal Class III is a successful technique with more skeletal than dental effects.

Effectiveness of reverse twin block with lip pads-RME and face mask with RME in the early treatment of class III malocclusion

BackgroundThe use of face mask for early treatment of class III malocclusion has proven to be successful, but its compliance and related dental side effects have always been a problem. To overcome this, a new approach has been suggested. The purpose of the present study was to compare the effectiveness of reverse twin block with lip pads and fixed rapid maxillary expansion (RTBLP-RME) appliance and face mask with RME (FM-RME) appliance for early treatment of class III malocclusion.MethodsThe sample consisted of 39 patients with class III malocclusion in the age group of 6–12 years (mean 10.17). They were divided into 3 groups of 13 each: reverse twin block with lip pads-RME (RTBLP-RME), face mask with RME (FM-RME), and control group. Treatment time was 9 months. Lateral cephalograms were taken at the start of treatment (T1) and after 9 months (T2) (both groups).ResultsBoth appliances were effective in correction of class III malocclusion with significant (p < 0.01) changes in all the cephalometric variables except cranial base angulations as compared to the control group. Intergroup comparison showed nonsignificant but greater sagittal changes with RTBLP-RME as compared to the FM-RME group. For all vertical measurements, the RTBLP-RME group showed nonsignificant increase compared to the FM-RME group. Maxillary incisor proclination was less in the RTBLP-RME group than in the FM-RME group, while mandibular incisor proclination was more in the RTBLP-RME group. Condylar inclination was significantly (p < 0.01) different for both treatment groups. With the RTBLP-RME group, posterior inclination of the condyle was seen while the FM-RME group showed more forward positioning as compared to the control group.ConclusionBoth groups were effective in correcting the malocclusion, but RTBLP-RME appliance had nonsignificant but greater impact on maxillary advancement and more hold on the posterior positioning of the mandible with minimal dental compensation as compared to FM-RME appliance.

Evaluation of Sagittal Changes in Class II Div 2 Patients with Decelerating Phase of Growth by PowerScope Appliance: A Retrospective Cephalometric Investigation.

Background:Restricted mandibular growth is observed in Class II Div 2 malocclusions. The fixed functional appliances are used to effect the skeletal changes in patients with end stages of skeletal maturity.Aim:This cephalometric study aimed to distinguish the skeletal and dental corrections done in the mandible on skeletal Class II Div 2 subjects when treated with PowerScope appliance (American Orthodontics, Sheboygan, WI).Materials and Methods:This is a cephalometric study based on the retrospective data. The patients selected for this study were with Class II Div 2 malocclusion and assessed to be in decelerating phase of skeletal growth. A total of 12 patients were selected after applying inclusion and exclusion criteria. A PowerScope was placed after incisor correction and followed up till Class I molar relation was achieved. The initial values (T1) at the time of placement of the appliance and the final values at the end of the molar correction (T2) were noted. The cephalometric parameters at T1 and T2 were assessed using sagittal occlusal (SO) analysis by Pancherz. The paired Student’s t-test was used to analyze pre- and posttreatment changes.Results:There was sufficient lengthening of the mandible (4.38 mm). Both the dental and skeletal alterations were observed in the mandible. These changes were marked compared to the maxilla. The pre- and posttreatment changes were statistically significant for the mandibular skeletal changes (P < 0.001) as well as mandibular incisor proclination (P < 0.001).Conclusion:The PowerScope can be used as a propeller for sagittal mandibular correction in Class II Div 2 patients.

Post-orthodontic cephalometric variations in bimaxillary protrusion cases managed by premolar extraction - A retrospective study.

Bimaxillary protrusion is a common dentofacial condition associated with proclination of maxillary and mandibular incisors in relation to the dental and cranial bases resulting in soft tissue procumbency. The present retrospective study aimed to investigate dental and soft tissue profile changes using cephalometric analysis to evaluate bimaxillary protrusion patients after extraction of the first four premolars and subsequent retraction of the anterior teeth. Pre-treatment and post-treatment cephalometric radiographs of 46 Saudi patients (16 males and 30 females), 18-30 years of age with bimaxillary protrusion, were selected based on inclusion criteria. Dental and soft tissue landmarks were traced using the Dolphin® imaging software and statistically analyzed with SPSS® 21 software. The upper and lower incisors retroclined by a mean value of 9.6° and 9.65°, respectively, and an average distance of 4.1 mm. The level of maxillary incisor exposure was reduced by approximately 1.1 mm after treatment. A mean increase of 6.6° in the nasolabial angle was also observed. Multiple regression analysis showed that retraction of both upper and lower incisors by 1 mm would result in a 0.44 mm retraction of the upper and lower lips. A statistically significant increase in the nasolabial angle and upper lip length was found in relation to upper and lower incisor retraction and retroclination. A significant reduction was also evident in the post treatment upper incisor exposure, facial convexity angle and mentolabial sulcus depth.

Long-term side effects of sleep apnea treatment with oral appliances: nature, magnitude and predictors of long-term changes

ObjectivesOral appliances for the treatment of obstructive sleep apnea (OSA) reduce upper airway collapse by advancing the mandible (OAm) and associated soft tissues. OAm are well tolerated but have side effects, mainly dental movement. It is not yet clear whether there are irreversible skeletal changes associated with treatment. As oral appliance treatment for OSA is a life-long therapy, careful and extended follow-up of patients is required. The objectives of this study were to evaluate the magnitude and progression of the dental and skeletal changes associated with long-term treatment, in addition to determining the predictors of the changes. MethodsLateral cephalograms of adults treated for primary snoring or mild to severe OSA with a custom-made titratable OAm for a minimum of eight years were retrospectively studied. The magnitude and rate of progression of any changes over time was determined and initial patient and dental characteristics were investigated as possible predictors of the observed side effects. ResultsRecords of 62 patients with an average treatment time of 12.6 years (range:8–21 years) were included. Cephalometric analysis revealed significant (p < 0.001) maxillary incisor retroclination (mean of ≈6°) and mandibular incisor proclination (mean of ≈8°) over the observation period. Maxillary incisors demonstrated a constant rate of retroclination –0.5°/year, the rate of mandibular incisors proclination was variable. The number of treatment years was significantly associated with these variables (p < 0.001). A greater body mass index (BMI) and Subspinale, Nasion, Supramentale angle (ANB) were associated with more maxillary and mandibular incisor proclination respectively. Although statistically significant (p < 0.001) skeletal changes were noted over this extended observation period, the difference in the Sella, Nasion, Supramentale point B (SNB) and mandibular plane angles were approximately 1° and were deemed not clinically significant. ConclusionsThis study represents the longest observation period to date examining OAm side effects with up to 21 years of follow up for some patients. It confirms that there are significant and progressive dental changes with prolonged OAm use. Conversely, over the same time period skeletal or postural changes were negligible. Additionally, treatment duration was the predictor consistently associated with the magnitude of the observed side effects.

Evaluation of the miniplate-anchored Forsus Fatigue Resistant Device in skeletal Class II growing subjects: A randomized controlled trial.

To evaluate the use of direct miniplate anchorage in conjunction with the Forsus Fatigue Resistant Device (FFRD) in treatment of skeletal Class II malocclusion. Forty-eight females with skeletal Class II were randomly allocated to the Forsus plus miniplates (FMP) group (16 patients, age 12.5 ± 0.9 years), Forsus alone (FFRD; 16 patients, age 12.1 ± 0.9 years), or the untreated control group (16 subjects, age 12.1 ± 0.9 years). After leveling and alignment, miniplates were inserted in the mandibular symphysis in the FMP group. The FFRD was inserted directly on the miniplates in the FMP group and onto the mandibular archwires in the FFRD group. The appliances were removed after reaching an edge-to-edge incisor relationship. Data from 46 subjects were analyzed. The effective mandibular length significantly increased in the FMP group only (4.05 ± 0.78). The mandibular incisors showed a significant proclination in the FFRD group (9.17 ± 2.42) and a nonsignificant retroclination in the FMP group (-1.49 ± 4.70). The failure rate of the miniplates was reported to be 13.3%. The use of miniplates with the FFRD was successful in increasing the effective mandibular length in Class II malocclusion subjects in the short term. The miniplate-anchored FFRD eliminated the unfavorable mandibular incisor proclination in contrast to the conventional FFRD.

Cephalometric Evaluation of Dentoskeletal and Soft-tissue Changes with Powerscope Class II Corrector

Objectives: The objective of the study is to assess the skeletal, dental, and soft-tissue effects of PowerScope fixed functional appliance in Class II malocclusion. Materials and Methods: Fifteen late adolescent Class II subjects (CVMI Stages 5 and 6) were treated with fixed mechanotherapy and PowerScope fixed functional appliance. Student t -test was used to compare the cephalometric changes produced before insertion of appliance (T1) and 6 months after the treatment with this appliance (T2). Results: Maxillomandibular relationship improved with significant lengthening of the mandible. There was retroclination of maxillary incisors, proclination of mandibular incisors, extrusion, and sagittal displacement of mandibular molars and clockwise rotation of occlusal plane. Conclusion: PowerScope is effective in correcting Class II malocclusion with a combination of dentoalveolar (primarily) and skeletal changes.

Treatment outcomes of Class II malocclusion cases treated with miniscrew-anchored Forsus Fatigue Resistant Device: A randomized controlled trial.

To evaluate the skeletal, dental, and soft tissue effects of the Forsus Fatigue Resistant Device (FRD) used with miniscrew anchorage and compare them with those of the conventional Forsus FRD. This study was carried out on 38 patients. These patients were randomly allocated into three groups. The 14 patients in group 1 (aged 12.76 ± 1.0 years) were treated with the FRD appliance. In group 2, the 15 patients (aged 12.52 ± 1.12 years) received treatment with FRD using miniscrew anchorage, and the 9 patients in group 3 (aged 12.82 ± 0.9 years) received no treatment as a control group. Linear and angular measurements were made on lateral cephalograms before and immediately after Forsus treatment. Data were analyzed statistically using paired t-, ANOVA, and Tukey tests. Class I molar relationship and overjet correction were achieved in both treatment groups. Although mandibular growth was statistically nonsignificant, there was a significant headgear effect on the maxilla. Mandibular incisor proclination, maxillary incisor retroclination, and distalization of maxillary molars were significant in both treatment groups. However, no significant differences were found between the treatment groups. Class II correction was mainly dentoalveolar in both treatment groups. Use of miniscrews with Forsus did not enhance mandibular forward growth nor prevent labial tipping of the mandibular incisors.

Mandibular Incisor Position Changes do not Explain Increase in Keratinized Gingiva Height after Periodontally Accelerated Osteogenic Orthodontics

Purpose This study aimed to reexamine the relationship between mandibular incisor inclination, prominence and vertical changes in position, and keratinized gingival tissue (KT) height changes labial to the mandibular incisors after orthodontic treatment with and without augmented corticotomy (Cort). Materials and Methods Two orthodontically treated groups of 35 individuals each, with (Cort) and without (conventional [Conv]) alveolar decortication and augmentation bone grafting (periodontally accelerated osteogenic orthodontics [PAOO]), were matched for sample size, gender, mandibular premolar extractions, pretreatment age, posttreatment observation period, and pretreatment KT height in this case–controlled retrospective study. Pre- and post-treatment lateral cephalometric radiographs were evaluated for mandibular incisor vertical, inclination, and prominence changes. Results As reported previously (Wilcko 2015), KT height had significantly increased by 0.78 mm (P = 0.001) in the Cort group and decreased by 0.38 mm (P = 0.002) in Conv on an average of 1.5 years after completion of active orthodontic therapy. In this study, Cort mandibular incisors proclined and protruded significantly during therapy while Conv incisors did not. Changes in mandibular incisor inclination and prominence explained neither the decrease in keratinized gingiva height in Conv nor the KT height gain in Cort. Conclusions In spite of mandibular incisor proclination and protrusion, orthodontic therapy combined with alveolar decortication and augmentation bone grafting resulted in a clinically significant increase in keratinized gingiva height 1.5 years’ posttreatment.

Prevalence of gingival recession after orthodontic tooth movements.

This study was designed to evaluate the long-term prevalence of gingival recession after orthodontic tooth movements, focusing on the effects of mandibular incisor proclination and expansion of maxillary posterior teeth. Records of 205 patients (162 female, 43 male) were obtained from 2 private practice orthodontists. Using pretreatment (age, 14.0±5.9years) and posttreatment (age, 16.5±6.0years) lateral cephalograms and dental models, mandibular incisor proclination and maxillary arch widths were measured. Gingival recession was measured based on posttreatment and postretention (age, 32.3±8.5years) intraoral photographs and models. Associations between tooth movements and gingival recession were evaluated statistically. Only 5.8% of teeth exhibited recession at the end of orthodontic treatment (only 0.6% had recession>1mm). After retention, 41.7% of the teeth showed recession, but the severity was limited (only 7.0%>1mm). There was no relationship between mandibular incisor proclination during treatment and posttreatment gingival recession. Incisors that finished treatment angulated (IMPA) at 95° or greater did not show significantly more recession than did those that finished less than 95°. There were weak positive correlations (r=0.17-0.41) between maxillary arch width increases during treatment and posttreatment recession. Orthodontic treatment is not a major risk factor for the development of gingival recession. Although greater amounts of maxillary expansion during treatment increase the risks of posttreatment recession, the effects are minimal.

Comparison of Activator-Headgear and Twin Block Treatment Approaches in Class II Division 1 Malocclusion.

The purpose was to compare the treatment effects of functional appliances activator-headgear (AH) and Twin Block (TB) on skeletal, dental, and soft-tissue structures in class II division 1 malocclusion with normal growth changes in untreated subjects. The sample included 50 subjects (56% females) aged 8–13 years with class II division 1 malocclusion treated with either AH (n = 25) or TB (n = 25) appliances. Pre- and posttreatment lateral cephalograms were evaluated and compared to 50 untreated class II division 1 cases matched by age, gender, ANB angle, and skeletal maturity. A paired sample, independent samples tests and discriminant analysis were performed for intra- and intergroup analysis. Treatment with both appliances resulted in significant reduction of skeletal and soft-tissue facial convexity, the overjet, and the prominence of the upper lip in comparison to untreated individuals (p < 0.001). Retroclination of maxillary incisors and proclination of mandibular incisors were seen, the latter being significantly more evident in the TB group (p < 0.05). Increase of effective mandibular length was more pronounced in the TB group. In conclusion, both AH and TB appliances contributed successfully to the correction of class II division 1 malocclusion when compared to the untreated subjects with predominantly dentoalveolar changes.

Gingival biotype and its relation to incisors' inclination and dentopapillary complex: An in vivo study

Objectives: To study the gingival biotype and its relation to maxillary and mandibular incisor inclination and its relation to dentopapillary complex. Materials and Methods: This cross-sectional study included 150 consecutive patients seeking orthodontic treatment at JSS Dental College, Mysore. Gingival biotype was assessed for maxillary and mandibular incisors using a digital vernier caliper. Maxillary and mandibular incisors' inclination and position were measured using cephalometric analysis. Parameters of dentopapillary complex were recorded from the dental casts. Results: The prevalence of thin gingival biotype was 42.66% for maxillary and 39.33% for mandibular incisors. A significant association was found between mandibular incisor inclination and thin gingival biotype, whereas there was no association between maxillary incisor inclination and gingival biotype. There was a significant correlation between gingival biotype and crown length, area of papilla, area of crown, and papilla length with P = 0.001 each. Conclusion: Mandibular incisor proclination is associated with thin gingival biotype, whereas no association is found in the maxilla. The correlation between gingival biotypes and dentopapillary complex is confirmed in this study. Evaluation of gingival biotype is of paramount importance during treatment planning for orthodontic patients.

A randomized clinical trial comparing mandibular incisor proclination produced by fixed labial appliances and clear aligners.

To compare the mandibular incisor proclination produced by fixed labial appliances and third generation clear aligners. Patients underwent a course of orthodontic treatment using either fixed labial appliances or clear aligners (Invisalign). Mandibular incisor proclination was measured by comparing pretreatment and near-end treatment lateral cephalograms. Eligibility criteria included adult patients with mild mandibular incisor crowding (<4 mm) and Class I skeletal bases (ANB, 1-4°). The main outcome was the cephalometric change in mandibular incisor inclination to the mandibular plane at the end of treatment. Eligible patients picking a sealed opaque envelope, which indicated their group allocation, was used to achieve randomization. Data was analyzed using a Welch two-sample t-test. Forty-four patients (mean age, 26.4 ± 7.7 years) were randomized in a 1:1 ratio to either the fixed labial appliance or the clear aligner group. Baseline characteristics were similar for both groups: Fixed appliance mean crowding was 2.1 ± 1.3 mm vs clear aligner mean crowding, 2.5 ± 1.3 mm; pretreatment mean mandibular incisor inclination for the fixed appliance group was 90.8 ± 5.4° vs 91.6 ± 6.4° for the clear aligner group. Fixed appliances produced 5.3 ± 4.3° of mandibular incisor proclination. Clear aligners proclined the mandibular incisors by 3.4 ± 3.2°. The difference between the two groups was not statistically significant (P > .05). There was no difference in the amount of mandibular incisor proclination produced by clear aligners and fixed labial appliances in mild crowding cases.

Treatment of class II and class III maloccolusion by using churro jumper: an efficient, inexpensive and uncomplicated fixed flexible functional technique

Functional orthopedic treatment seeks to improve skeletal and dental relationship of the jaws. The challenging task is to correctly position jaws antero-posteriorly and vertically with correct overbite, overjet and Centric relation. The Churro Jumper is an efficient, inexpensive and uncomplicated fixed flexible functional appliance. It is used to evaluate the efficacy of the Churro Jumper appliance in treatment of skeletal Class II malocclusion with retrognathic mandible. Churro Jumper contributes in correction of Class II molar relationship by dento-alveolar effects on both jaws. There was up-righting of maxillary incisors and proclination of mandibular incisors. Churro Jumper is clinically efficient as well as effective appliance to correct skeletal class II malocclusion. The only problem with this appliance is its frequent breakage and oral hygiene maintenance causing inconvenience to both patient as well as operator.
