This study developed a taxonomic approach based on 30 technical failure occurrences between 2002 and 2020. This framework extends the author's previous work on human-machine-environment-procedure (HMEP) taxonomy through the application of Rasmussen's Risk Management Framework (RRMF), which can serve as a common framework to aggregate or compare datasets collected by different organizations. This RRMF-based HMEP taxonomy was used to categorize the contributing factors of 30 technical failure occurrences into seven mutually exclusive categories and respective subcategories for statistical analysis and other risk management analysis. The RRMF provided a hierarchical structure for these first-layer categories, including government oversight, manufacturer deficiency, company management, company procedure and documentation, people and activity management, technical failure, and environment. The functional block diagram and failure modes and effects analysis (FMEA) were applied to translate the technical failures of 12 environment control system failure cases into FMEA tabular statements. Several failure modes had been identified for environmental control system failure occurrences, e.g., sense line leakage, flawed sensor input, duct crack and leakage, broken part (pin and spring), seal degradation, contamination, tube rupture, mechanical joining defect, poor contact, and unanticipated failure instances for electronic centralized aircraft monitor (ECAM). Each failure mode can provide diagnostic information to identify and fix the environmental control system failure problem more effectively. In conclusion, this RRMF-based HMEP taxonomy provided a mental model to guide the data collection during an accident investigation and subsequently derive accident patterns as the core for Safety Management System (SMS) implementation.

The amount of waste pharmaceuticals generated in households has increased steadily, due to societal and production changes. Therefore, managing household pharmaceutical waste (HPW) is an important challenge for our society. The study presents a comparative assessment of HPW management in Albania and practices in European countries, followed by recommendations on building a functional management system for HPW in Albania. Public and pharmacist perception on this issue is a key element in achieving sustainable waste management goals. The amount of waste pharmaceuticals stored remains low, with 86% respondents storing several tablets, primarily antipyretics (53). Regarding pharmacists, 86% responded that they don’t accept expired/unused products from patients, even though the demand from the latter is almost absent; 88% of the pharmacists refuse to cover the cost of waste generated by family members. The Albanian legislative and regulatory framework remains fragmented and incomplete, creating uncertainty regarding the responsibilities, competencies, and administrative regulatory roles of various actors.

Although tracheal gas insufflation (TGI) and aspiration of expiratory gas in the dead space (ASPIDS) have been shown to enhance CO₂ removal by washing out the anatomical dead space, clinical implementation has been limited due to complications including unintended airway pressure increases and mucosal damage, as well as their complexity. We developed a novel respiratory management system combining continuous intratracheal gas suctioning with pressure control ventilation, utilizing the leak compensation function of modern ventilators. The aim of this study was to document the preliminary physiological observations of this system in a proof-of-concept porcine model. Two female pigs weighing approximately 30kg were intubated with 7.5mm cuffed endotracheal tubes and paralyzed under general anesthesia. Each pig was studied under four different ventilatory conditions: two with healthy lungs and two with experimentally induced lung injury (created by repeated whole-lung lavage with normal saline and injurious ventilation until PaO₂/FIO₂ < 300 mmHg). A 10 Fr closed suction catheter was inserted into the trachea. Using the leak compensation function of a Puritan Bennett™ 840 ventilator, heated and humidified gas was automatically supplied at the same flow rate as the continuous suctioning flow (13-14L/min), maintaining constant circuit pressure. Pressure-controlled ventilation was applied, and arterial blood gas was analyzed before, during, and after continuous suctioning. PEEP was increased by 1 cmH₂O during suctioning to compensate for pressure loss through the endotracheal tube. A total of eight experimental runs were performed (two pigs × four conditions each: two healthy, two injured lung conditions). Baseline PaCO₂ ranged from 42.2 to 71.8 mmHg. During suctioning, PaCO₂ decreased in seven conditions (reductions: 0.9 to 10.1 mmHg; 1.8% to 14.1%) and increased in one condition (3.0 mmHg; 5.9%). The largest reductions occurred in conditions with higher baseline PaCO₂. After cessation of suctioning, PaCO₂ returned toward baseline levels. No ventilator alarms or operational problems occurred during the experiments. In a proof-of-concept study using a porcine model with both healthy and injured lungs, continuous intratracheal gas suctioning combined with ventilator leak-compensation may enhance CO₂ removal efficiency without increasing driving pressure. This technique has potential as a simple adjunct to conventional mechanical ventilation for enhancing CO₂ removal in patients with respiratory failure, while further investigation is required for safety and clinical applicability.

To develop and validate a method for assessing the performance and long-term constancy of Automatic Exposure Control (AEC) in interventional fluoroscopy using retrospectively collected clinical data from a Radiation Dose Management System (RDMS). The method uses the system-reported Patient Equivalent Thickness (PET) as a surrogate for PMMA attenuation, enabling continuous evaluation of the air kerma rate-thickness response under clinical conditions. Three angiographic systems from two different manufacturers were monitored over two years (>135,000 fluoroscopic and acquisition events in the RDMS). Air kerma-rate data were grouped in 1-cm PET bins and analysed quarterly. Temporal deviations in the median air kerma rate and interquartile range (IQR) were computed relative to a reference quarter, and exponential air kerma rate-thickness fits were used to derive compensation coefficients (λ) as indicators of AEC stability. PET was experimentally validated against PMMA and water thickness (R2>0.98). One of the systems showed stable AEC behaviour, with quarterly median deviations within±5% and λ variations within a proposed±0.02cm⁻1 tolerance. In the other system, a 72% deviation was observed due to a detector failure; normal performance was restored after detector replacement and protocol updates. Outside this event, all systems remained within EFOMP±20% constancy limits. RDMS-based PET analysis reliably reproduces phantom-derived ESAKR-thickness behaviour and enables continuous, clinically driven AEC surveillance. The method is sensitive to hardware faults and configuration changes, and provides a robust complement-not a replacement-to periodic phantom-based AEC testing.

Dinas Komunikasi dan Informatika, in supporting the achievement of the city’s development targets, carries out the good result procurement and services through the preparation of procurement documents by each division and the verification of payment documents by Sub Bagian Keuangan. However, the verification process still many challenges, including inconsistencies between documents, incomplete documentation according to regulations, delays in document revision, and the risk of document loss due to non-integrated document management. In addition, the lack of real-time monitoring and coordination among divisions, Sub Bagian Keuangan, and vendors or providers causes the verification process slowly and not efficient. Target this study to develop management information system with web-based for monitoring procurement activities that control and tracking of procurement status, so improve the effectiveness, transparency, and accountability of procurement management. This system used Waterfall method, including problem identification, requirements analysis, system design, and evaluation stages. The system is developed as a web-based application developed using the PHP programming language with MySQL as the database management system. The result of this study is a web-based procurement monitoring system application implemented at Sub Bagian Keuangan of Dinas Komunikasi dan Informatika. System evaluation through black-box testing shows that all five core functional scenarios passed as expected (5/5 valid).

The implementation of digital-based information systems in higher education aims to improve the effectiveness of academic and administrative activities. Universitas Islam Nahdlatul Ulama Jepara has implemented an Event and Meeting Management System application to support scheduling, agenda management, and documentation of institutional activities. This study aims to evaluate the usability level of the application using the System Usability Scale (SUS) method. The research employed a descriptive quantitative approach using the SUS questionnaire consisting of ten Likert-scale statements. A total of 200 active users, including university leaders, lecturers, administrative staff, and students, participated as respondents. The results show that the application obtained an average SUS score of 80.35, which is above the standard usability threshold of 68. These findings indicate that the Event and Meeting Management System application has a high level of usability and is easy to use by users. The results of this study are expected to provide valuable input for further system evaluation and development to enhance the effectiveness of academic and administrative management at the university.

This study aims to address issues in employee leave data management at the Deli Serdang District Agricultural Office, where leave records were previously processed manually using separate documents, resulting in data duplication, inaccuracies, and delays in preparing reports. To solve this problem, a web-based Employee Leave Record Management System was developed using the Waterfall model through stages of requirements analysis, system design, implementation, testing, and maintenance. System evaluation was conducted using Black-Box testing to verify functional performance. The system supports structured recording of employee leave information, employee profile data such as position and department, and provides real-time reporting features to support administrative decision-making. The application was developed using the CodeIgniter-based MVC architecture and MySQL database, which enhances data accuracy, information accessibility, and administrative efficiency. The results demonstrate that the system successfully minimizes recording errors and improves the effectiveness of leave data management. Future development opportunities include integrating automated online leave request and approval workflows, mobile access features, and notification services to maximize usability and system automation.

Urban sustainability challenges in many Global South cities occur within fragmented institutional environments where no single authority holds full legitimacy or capacity to govern. This study examines how pluralistic governance-characterized by overlapping mandates among public agencies, non-governmental organizations, landlords, and regulators-can still generate coordinated environmental performance. Drawing on stakeholder and dynamic-capabilities theories, it introduces relational capability as a sustainability-oriented governance mechanism that enables adaptive coordination and substitutes for weak formal structures. Findings from Nairobi's solid waste management system show that coordination arises not from technical capacity or centralized control but from distributed relational processes that align diverse actors around shared environmental goals. Relational capability thus functions as a meta-governance competence that transforms institutional incoherence into adaptive efficiency. The study highlights the importance of relational infrastructure-trust-building, joint planning, and shared accountability-in strengthening environmental governance across fragmented urban systems.

Waste generated in medical facilities poses chemical, toxic, carcinogenic, mutagenic, and radiation risks to the human body, alongside the risks of injury and infection. The microbial load in medical waste is one thousand times higher than in municipal solid waste. Rational and safe management of medical waste is one of the most pressing challenges in healthcare, as it directly impacts infection control and public health protection. An increasing number of countries worldwide are legislating medical waste management. By 2035, the volume of medical waste in Ukraine is expected to reach 141,000 tons, including 35,000 tons of epidemiologically infectious waste. Therefore, it is critically important to ensure sufficient capacities for its safe treatment. An analysis of the regulatory framework allows for the identification of existing differences in the medical waste management systems of Ukraine and the EU member states, as leading European countries have already established frameworks that Ukraine has only recently begun to implement. This review provides a comparative analysis of the regulatory framework for medical waste management in Ukraine and the European Union countries, while substantiating priority measures for reforming the national management system in accordance with European epidemiological safety practices. Based on theoretical, analytical, and comparative methods, the paper reviews current scientific publications and examines relevant regulatory and legal documents of Ukraine, the European Union, and individual member states in the field of medical waste management. The current state of medical waste management in Ukraine and the European Union, as well as the hazardous potential of this waste type for public health, was considered. The review assesses the legislative regulation of medical waste management in EU countries and the regulatory documents of Ukraine. It was established that the current state of legal support for waste management in Ukraine is hindered by a lack of efficient control and supervision over waste management entities and insufficient funding for relevant programs. The expediency of introducing amendments to the regulatory framework has been substantiated, particularly regarding the strengthening of state control over the activities of business entities and the implementation of European waste accounting standards. As a modern state governed by the rule of law, Ukraine has prioritized the development of medical waste management by aligning its legislation with the experience of EU member states. Given that the volume of hazardous waste continues to increase while comprehensive statistical records remain absent, the application of European law and best practices will facilitate the successful implementation of medical waste management programs. To achieve full alignment with EU standards, Ukraine must not only adopt relevant laws but also ensure their effective enforcement, develop modern infrastructure, and strengthen control in the field of medical waste management. The ultimate goal should be the creation of a comprehensive and sustainable management system that meets the best international practices and ensures the protection of human health and the environment.
