The provision of abortion in Australia: service delivery as a bioethical concern.

Despite significant progress in the legalization and decriminalization of abortion in Australia over the past decade or more recent research and government reports have made it clear that problems with the provision of services remain. This essay examines such issues and sets forth the view that such issues can and should be seen as (bio)ethical concerns. Whilst conscientious objection-the right to opt-out of provision on the basis of clear ethical reservations-is a legally and morally permissible stance that healthcare professionals can adopt, this does not mean those working in healthcare can simply elect not to be providers absent a clear ethical rationale. Furthermore, simple non-provision would seem to contravene the basic tenants of medical professionalism as well as the oft raised claims of the healthcare professions to put the needs of patients first. Recognizing that much of the progress that has been made over the past three decades can be attributed to the efforts of dedicated healthcare professionals who have dedicated their careers to meeting the profession's collective responsibilities in this area of women's health and reproductive healthcare, this paper frames the matter as a collective ethical lapse on the part of healthcare professionals, the healthcare professions and those involved in the management of healthcare institutions. Whilst also acknowledging that a range of complex factors have led to the present situation, that a variety of steps need to be taken to ensure the proper delivery of services that are comprehensive, and that there has been an absence of critical commentary and analysis of this topic by bioethicists, I conclude that there is a need to (re)assess the provision of abortion in Australia at all levels of service delivery and for the healthcare professions and healthcare professionals to take lead in doing so. That this ought to be done is clearly implied by the healthcare profession's longstanding commitment to prioritizing the needs of patient over their own interests.

Self‑Assessment of the Intercultural Competences of Medical Doctors and Nurses in Poland in the Face of Current Refugee Challenges

Health care is one of the most important systems in both society and the economy. At the same time, it is a highly complex system that is subject to an increasing number of challenges in the contemporary world. For example, the health care system in Poland struggles primarily with inadequate numbers of nurses and medical doctors, as well as insufficient funding. A relatively new challenge is to provide for the health needs of refugees, whose number is increasing due to the war in Ukraine beyond Poland’s eastern border. This raises the question of the substantive preparation of nurses and medical doctors to work in a culturally diverse environment. Have they had the opportunity to train in intercultural competences? How do they assess their own intercultural competences? Do they see challenges in their interactions with foreigners? The aim of this paper is therefore to present a self‑assessment of the intercultural competences of medical doctors and nurses in Poland, in particular in the context of current refugee challenges. In order to achieve the assumed goal, a proprietary questionnaire form was created, and a nationwide survey was conducted among a group of professionally active medical doctors and nurses. The results of the research, in addition to providing valuable knowledge, were used to draw up specific recommendations for improving the continuing education of medical and nursing staff in the area of intercultural competences. Based on the research results, guidelines for better management in public health organisations were also created. This text may be of interest to researchers of management in public health care organisations, and in particular human resources management in health care institutions, as well as practitioners trying to deal with the current problems facing public health management.

The subsystem of management is tax management, namely optimization of taxation aimed at achieving better business results. The medical industry is increasingly turning into a business structure that functions according to the laws of a market economy, therefore the role of tax management in ensuring the successful operation of health care institutions is growing significantly. This article analyzes the regulatory framework on taxation and statistical data on the activities of health care institutions. The taxation systems provided for by the current legislation for medical institutions of various forms of ownership and the possibilities of their application with the most beneficial effect for the institution are considered. Peculiarities of the organization of tax management in health care institutions during martial law were studied.

FORMATION OF CLUSTERS OF MEDICAL INSTITUTIONS AS AN ADMINISTRATIVE TOOL OF MANAGEMENT IN THE INDUSTRY OF HEALTH CARE IN THE CONDITIONS OF DECENTRALIZATION

The health care management system involves the application of administrative methods of managing medical facilities, which are a tool of state regulation in the field of health care. The transformation of the health care system through medical reform is aimed at providing citizens of Ukraine with equal access to quality medical services and reorienting the system so that the patient is at its center. The article examines the management of health care institutions in the process of reform, finds out the relevant issues of the infrastructural stage of the medical reform and the effective functioning of medical institutions in conditions of decentralization. Attention is focused on the transformation of the health care system in the form of the formation of clusters of medical institutions, which is proposed by the Ministry of Health of Ukraine and aims to improve the quality and availability of medical care through the creation of a new network of medical institutions in accordance with the principle of hospital districts. The structure of the cluster distribution of medical institutions within the framework of the medical reform and the characteristics of a capable network of medical institutions in the Odesa region are considered. The significance of the division of territories into hospital clusters in the process of balancing the advantages and disadvantages of decentralization and the maximum approximation to the modern European system of providing medical care is emphasized. A promising direction of further research on this issue is to solve the problem of «community hospitals», which are not included in the capable network of the hospital district (region) and, due to uncertainty, create a risk of reducing the availability and quality of medical care for residents of territorial communities.

Experiences of family caregivers of people with spinal cord injury at the neurosurgical units of the Komfo Anokye Teaching Hospital, Ghana.

Spinal cord injury (SCI) often leaves affected persons with a lifelong demand for care. As a result, the effect of the condition goes beyond the injured person to affect family members and significant others who have to adjust their lives to provide care and support. However, little is known about the experiences of these family caregivers regarding the care of people with SCI in Ghana. Exploring their experiences would enhance the understanding of family caregiving of people with SCI and contribute to policy intervention. This qualitative descriptive phenomenology study used the purposive sampling method to select 10 family caregivers. Data were collected using a semi-structured interview guide through individual in-depth interviews. Written informed consent was obtained and interviews were audio-recorded and transcribed verbatim. Data were manually analysed following Colaizzi's method of data analysis. In all, 4 main themes emerged from the analysis of data (1) becoming a caregiver, (2) roles of the caregiver, (3) the burden of caregiving, and (4) coping strategies. The family caregivers provided vital assistance to their relatives with SCI and experienced physical and financial burdens as a result of the care. Due to the strains involved in the caregiving process, family caregivers adopted various strategies to cope with the situation. This study has provided evidence of the lived experiences of family caregivers of people with SCI in the Ghanaian context and further supports the findings of previous studies. Measures including training, counselling, and instituting social support services for family caregivers should be considered by the management of healthcare institutions to enhance the experiences of family caregivers.

Health Status and Management Practices of Home Isolated COVID-19 Adult Patients.

The worldwide containment strategy for COVID-19 outbreak includes laboratory-confirmed cases, and their isolation and management in health care institutions or at home. The spread of the COVID-19 virus has mandated home isolation for mild cases, as recommended by the Government of Nepal. Isolation is a situation that can have a substantial influence on physical and mental health of isolated people. This study is aimed to assess physical and mental well-being of COVID-19 home isolated patients, and their home management practices. A descriptive cross-sectional research using quantitative methods was carried out. Purposive sampling was used to select COVID-19 patients. Total 536 COVID-19 home isolated patients were included in this study. Telephonic interview was conducted to obtain the data. Descriptive analysis was done and interpreted. About 34 % of the participants were symptomatic. The most common symptoms experienced were fever (22.6%), followed by cough (19.4%) and cold (16.1%). About 20 % indicated having difficulties isolating at home due to lack of separate room. Furthermore, 4 percent of the participants didn't have anyone to take care of them at home. Also almost 2 percent of participants didn't get family support when infected. Moreover, majorities of individuals had normal stress, depression and anxiety level. Most of the participants' physical and mental health was found to be normal though some of them experienced difficulties for management during home isolation. Hence, Interventions should focus resilience building by improving communication to address fears and concerns, encouraging routines and physical activities, and taking measures to reduce loneliness.

Governance and management of health care institutions in Serbia: An overview of recent developments

In order to promote awareness of factors that affect social services, their quality, effectiveness and coverage, the term “governance” is frequently used. However, there is no agreement on definitions, frameworks and how it relates to the health sector. In this overview, two interrelated processes in Serbia will be analyzed: governance and management at the macro-, meso-, and micro level. Key messages are as follows: i) Continue decentralization and support to an effective national decision-making body (Health Council of Serbia) with all relevant stakeholders; ii) Reduce the well-known implementation gap and agree on a binding time frame for reforms, and; iii) Establish obligatory schemes for education and training of managers and support sustainability of state institutional capacity to teach, train and advise on a scientific basis.

When used successfully, this data aids in the creation of information that improves patient health and everyday organizational functioning, as well as the prevention of unfavorable public health scenarios such as the spread of infectious illnesses. This is where big data analytics comes in, providing rational methods for navigating enormous quantities of data to disclose knowledge that assists businesses and analysts in making faster and better decisions. Higher education, like healthcare, creates large amounts of heterogeneous data that hides useful knowledge. Economic and analytical diagnostics in the financial activity management system of a modern health care institution

Introduction. The concept of the development of the system of financial support in the field of health care outlines the need to introduce a new financial mechanism, which provides for the provision of a sufficient level of managerial and financial autonomy to medical institutions.The change in the nature of financial relations imposes new requirements on the management of financial activities in health care institutions. The functional component of managerial activity is economic and analytical diagnostics, the application of which will ensure the adaptability of financial management methods and models to modern realities and the integration of the finances of health care institutions into the system of competitive market relations.Purpose. The purpose of the study is the development of theoretical provisions of economic-analytical diagnostics and the development of methodological approaches for its application in the management of financial activities of health care institutions in the conditions of transformational changes.Methods. Achieving the set goal is achieved by using general scientific, special and empirical methods of learning processes and phenomena. The methods of analysis and synthesis, comparison, and generalization are used to understand economic and analytical diagnostics in the management system; scientific abstraction, structural-logical and tabular methods for determining the methodology of economic-analytical diagnostics in modern health care institutions.Results. It has been proven that the mechanism of management activity of state institutions with elements of entrepreneurial structures, which are currently medical institutions, requires the use of modern economic and analytical methods and diagnostic models and their adaptation to the specifics of the activities of health care institutions. The analysis of the main approaches of scientists to the definition and understanding of economic-analytical diagnostics as a functional component of managing the financial activities of a health care institution was carried out, and it was determined that the essence of economic-analytical diagnostics of health care institutions should be the system of management activities regarding decision-making on the investment of financial and other resources to provide quality services that will ensure the efficiency and competitiveness of the health care institution.At the same time, the main tasks are defined: analysis of the financial activity of medical institutions, assessment of production capacities and resource base and means of their improvement, which will ensure the financial and economic stability and stability of medical institutions with the approval of market positions and/or exit from the crisis state.It is substantiated that the indicators of the efficiency and stability of the functioning of the health care institution should form an optimal management model of an expedient and justified system of financing and provision of financial resources and the efficiency of their spending.The use of integral methods and models that combine internal and external factors of the functioning of economic entities is proposed to carry out economic and analytical diagnostics and determine the capabilities of health care institutions. The main approaches to the SWOT analysis methodology with the possibility of its adaptation and application in the management of health care institutions were studied.Discussion. In order to carry out economic and analytical diagnostics regarding the determination of the capabilities of health care institutions, including financial ones, in addition to SWOT analysis, it is advisable to use other integral methods and models that combine internal and external factors of the functioning of economic entities in the field of medicine, which may be the subject of further research.

The effect of internal marketing on employees' organizational commitment through their job satisfaction: The case of healthcare sector in Serbia

The purpose of this paper was to examine the link between internal marketing (IM) and employees' organizational commitment (OC) through their job satisfaction (JS). The data were collected through an on-line survey from 278 employees in various public healthcare institutions in Belgrade. In order to test the hypotheses, partial least squares structural equation modelling was used. The results of this research showed that employees' JS partially mediates the relationship between IM activities and employees' OC. These results are relevant for healthcare marketing professionals and managers of healthcare institutions since they show the significant role of IM in improving employees' OC and the importance of nurturing employees' JS as a job-related attitude. The obtained results are also important since they support decision makers in promoting the IM concept in healthcare sector.

Human Resource Management as an Area of Changes in a Healthcare Institution.

The underlying objective of the paper was to investigate the areas that require changes in human resource management in healthcare institutions. The practical objective of the study was to formulate recommendations targeted at the management staff of a healthcare institution, which allow to increase the adaptability and development capacity of medical personnel with the use of appropriate instruments and methods of human resource management. The quantitative research was conducted among 652 patients using the services of primary health care clinics located in the Silesian province in Poland, in 2019. The share of women in the research sample was 61%, and men - 39%. 11% of the respondents were students, 27% - people of retirement age, and 62% were economically active. An anonymous survey questionnaire was used. Calculations were performed using Statistica software. Mann-Whitney U-test and correlation analysis using Gamma (Γ) coefficients were used. The results of the empirical research showed that the respondents positively assessed most of the examined elements (over 70% of positive opinions). Negative opinions did not exceed the error threshold and amounted up to 5%. The presented results allowed to conclude that the expectations and preferences of patients should be constantly monitored, have an impact on the management of medical personnel and imply introducing changes in the weakest areas. The conducted research made it possible to identify the needs and expectations of patients and to develop possible solutions to improve human resource management in healthcare entities. In order to improve the operating conditions of the clinic, managers should constantly monitor patient satisfaction and, if necessary, introduce changes to raise service standards. This will allow early detection of shortcomings and the introduction of necessary changes in the clinic.

MODERN COMPETENCIES AND TASKS OF MANAGERS OF HEALTH CARE INSTITUTIONS

Purpose. The purpose of the article is the scientific and theoretical substantiation of modern competencies and tasks of health care managers. Tasks. Achievement of this goal necessitated the following tasks: to analyze the role of managers and their impact on the activities of health care institutions; to consider the scope of management activities of managers of modern medical institutions; to consider the main competencies of managers of health care institutions; to determine the current tasks of managers and their impact on the activities of health care institutions. Object. The object of the study is health care institutions. Methodology. The information base of the study was formed by legislative and regulatory documents, periodicals, electronic resources. The work uses general scientific and special methods: monographic research, generalization, systematization. This made it possible to consider the impact of modern environmental factors on the functions and responsibilities of managers in health care institutions. Also to explore the modern competencies and tasks of managers and their impact on the activities of health care institutions. Results. The role of managers and their influence on the activities of health care institutions are considered. The influence of modern environmental factors on the functions and responsibilities of managers in health care institutions is analyzed. The purpose of health care institutions is considered. Two spheres of management of managers are defined: external and internal. The main competencies of managers in health care institutions are considered. The necessity of performing additional duties by managers in today's unstable economic conditions is substantiated. The modern tasks of managers and their impact on the activities of health care institutions are analyzed. Value/originality. It is found that when managing health care institutions, managers must take into account endogenous and exogenous challenges to make effective management decisions and manage the quality of health care institutions. It is determined that the key competencies of an effective manager are conceptual, technical and interpersonal skills. It is concluded that in modern conditions the heads of medical institutions also face new challenges, which are determined by such modern trends as: management in difficult conditions, management in conditions of constant change, management in a creative way. Conclusion. In today's volatile economic environment, healthcare managers must develop critical new skills and competencies to ensure the effective development of healthcare institutions. Healthcare managers must also continuously improve them in accordance with market requirements. When managing health care institutions, managers must take into account endogenous and exogenous challenges to make effective management decisions and quality management of health care institutions. Constant changes in environmental factors lead to changes in the competence and responsibility of health care managers.

ORGANISATIONAL AND ECONOMIC MECHANISM FOR PREVENTING CORRUPTION IN PUBLIC AUTHORITIES IN THE CONTEXT OF ENSURING EFFECTIVE REGULATION OF THE HEALTHCARE SYSTEM

The article is devoted to the research of the impact of the phenomenon of “corruption” in the state authorities on the functioning of the healthcare system in Ukraine. The article considers not only the impact of corruption on the functioning of the healthcare system in general, but also such issues as state regulation of medical institutions, redistribution of management functions in the healthcare system, granting autonomous status to medical institutions (autonomization), and financing of the healthcare system in general. The article describes the Ukrainian legislative anti-corruption regulators, institutional levers of the Cabinet of Ministers of Ukraine, local administrations and executive authorities. The author presents an organizational and economic mechanism for preventing corruption in the healthcare sector. The mechanism is based on the system of institutional and legislative regulators, and also reveals their subordination and sequence of influence. The main elements of the organizational and economic mechanism for preventing corruption in the healthcare sector are anti-corruption legislation, legalization of the healthcare market, decentralization of the healthcare management system, autonomy of healthcare institutions, openness of financial and economic activities, and transparency of information and communication activities. The presented scheme of the mechanism reflects the elements of regulatory prevention of corruption, as well as the subordination and sequence of activation of elements in healthcare management. the proposed mechanism for preventing corruption in healthcare allows for effective and anti-corruption management of healthcare institutions at all levels of subordination to state and local authorities. The author formulates conclusions regarding the possibility of corruption’s impact on the functioning of the healthcare sector. The author analyses the possibility of creating and implementing a rational organizational and economic mechanism for preventing corruption in public authorities in the context of ensuring effective regulation of the healthcare system.

Artificial Intelligence Model for Risk Management in Healthcare Institutions: Towards Sustainable Development

This paper proposes an artificial intelligence model to manage risks in healthcare institutions. This model uses a trendy data source, social media, and employs users’ interactions to identify and assess potential risks. It employs natural language processing techniques to analyze the tweets of users and produce vivid insights into the types of risk and their magnitude. In addition, some big data analysis techniques, such as classification, are utilized to reduce the dimensionality of the data and manage the data effectively. The produced insights will help healthcare managers to make the best decisions for their institutions and patients, which can lead to a more sustainable environment. In addition, we build a mathematical model for the proposed model, and some closed-form relations for risk analysis, identification and assessment are derived. Moreover, a case study on the CVS institute of healthcare in the USA, and our subsequent findings, indicate that a quartile of patients’ tweets refer to risks in CVS services, such as operational, financial and technological risks, and the magnitude of these risks vary between high risk (19%), medium risk (80.4%) and low risk (0.6%). Further, several performance measures and a complexity analysis are given to show the validity of the proposed model.

The relational effects of perceived organizational support, fear of COVID-19, and work-related stress on the safety performance of healthcare workers.

This paper assesses the relational effects of perceived organizational support (POS), fear of COVID-19 (FOC-19), and work-related stress (WRS) on the safety performance of healthcare staff. The sample for this research was extracted from the University College Hospital (UCH) in the Oyo State of Nigeria. The participants were midwives, doctors, auxiliary services staff, and nurses who functioned in a COVID-19 hospital ward, fever or respiratory ICU, Auxiliary services, or outpatient clinics. This investigation espoused a clinical cross-sectional survey involving self-reported surveys. Of the 150 questionnaires distributed, 147 were fit for scrutiny and analyzed with Statistical Packages for the Social Sciences (SPSS version 28). This paper established a relationship between POS and safety performance (SP). Besides, it showed a significant positive correlation between FOC-19 and SP. It further noted that work-related stress negatively relates to safety performance. Moreover, this study showed the significant joint strong influence of POC, FOC-19, and WRS on the safety performance of healthcare workers. Hence, healthcare institutions are encouraged to create adequate support for healthcare workers, particularly during a global health crisis. Government and healthcare institutions should also develop an awareness program on the danger and consequences of getting infected by the virus or infecting other significant others. This will increase the fear of COVID-19 and, consequently, health workers’ safety performance. Besides, it is recommended that the management of healthcare institutions provides a proper work structure and schedule to help reduce workloads, consequently reducing WRS, as lowering it improves healthcare workers’ safety performance.

CONCEPTUAL PRINCIPLES OF THE MANAGEMENT SYSTEM IN HEALTHCARE INSTITUTIONS

The article reveals the conceptual principles of management in health care institutions. The purpose of the article is to determine the conceptual foundations of the management system in health care institutions in the context of the state's implementation of the sustainable development strategy. The legal framework that regulates the management process in health care institutions has been defined. Features of medical facilities are outlined. The definition of management in the field of health care has been clarified, taking into account the peculiarities of medical institutions. The subject and object of management in health care institutions are defined. The characteristics of the principles and functions of management are given. The principles on which the management system in health care institutions should be based are the principles of purposefulness; consideration of needs and interests; science; systematicity; interdependence; optimal combination of centralization and decentralization of management; unity; correct selection of personnel; economy The list of functions includes the functions of planning, organization, regulation, coordination, motivation, and control. The content of management methods of medical institutions is covered. Management methods are divided into economic, organizational-administrative and social-psychological. The types of management decisions made in a medical institution for its effective functioning are characterized. Management decisions should be classified according to their focus on the object of management into social, medical, organizational, resource, administrative and economic, management decisions on control. The stages of the management process are described. Four stages of the management process in health care institutions are distinguished: the first is obtaining information about the initial state of the health care institution, its processing and analysis, which allows to assess the current state of the system and make a forecast of the further development of the health care institution; the second is the development and adoption of management decisions; the third is the implementation of a management decision; the fourth is control over the implementation of the management decision. It was determined that management in a medical institution should be based on a systemic approach that will ensure the formation of an optimal management mechanism for the development and effective operation of a health care institution.

What Do Nurses Experience in Pandemics? A Scale Development Study

Aim: To develop a valid and reliable measurement tool to identify the challenges nurses face during pandemics.Method: The study has a cross-sectional and methodological design. Using a multi-method approach, it was conducted with 405 nurses in Istanbul between May 15 and August 15, 2020. Data were collected online via the Personal Information Form and Draft Scale. Descriptive statistics, validity reliability analysis, and total score averages were evaluated for data analysis using SPSS 21 and AMOS 22 programs.Results: 38 items were grouped into four dimensions. The variance rate was found as 66.88%. Goodness-of-fit statistics were appropriate. The total correlation scores of the items were between 0.42-0.82 (p<0.001). Cronbach's alpha value was 0.92. There was no significant difference between the test-retest mean scores (t:1.349, p:0.188), while a highly significant correlation was found between the measurements (r:0.88 p<0.001). The overall mean score of the scale was 3.50±0.62.Conclusions: The valid and reliable scale can assist nurses, managers of healthcare institutions, and policymakers in developing coping strategies for the challenges.
