Lacrimal sac diverticulum presenting as a lower eyelid mass with a secreting fistula

Lacrimal sac diverticula are outpouchings of the lacrimal sac. Their various clinical symptoms complicate differential diagnosis. Here we report two patients of distinctive type of lacrimal sac diverticula. Each of them presented with a mass inferior to the medial canthus with a secreting fistula. Irrigation demonstrated lacrimal drainage system patency. The orificium fistula was located at lower palpebral conjunctiva. This distinctive type of lacrimal diverticulum had never been reported in the literature as we know. The mass had no apparent connection with lacrimal sac according to the results of computed tomography dacryocystography and B-scan ultrasonography. All patients underwent surgical exploration and mass removal under general anesthesia. The mass was confirmed as lacrimal sac diverticulum by pathological examination.

Atypical Bilateral Orbital Metastases of Lobular Breast Carcinoma

Breast carcinoma metastasizing to the orbit is a well-recognized entity. The authors present a 50-year-old woman with bilateral upper and lower eyelid masses and generalized dysmotility. MRI showed gadolinium-enhancing homogenous soft tissue masses filling both orbits from the eyelids to the apex and a biopsy that confirmed metastatic breast carcinoma. To the authors' knowledge, orbital metastasis of breast carcinoma has never presented in this manner.

Multilobular Lacrimal Sac Diverticulum Presenting as a Lower Eyelid Mass

Lacrimal sac diverticulum is a rare condition, and its various symptoms complicate differential diagnosis. We present cases of a peculiar type of lacrimal diverticulum. A 5-year-old girl and a 50-year-old woman presented with a protruding mass inferior to the medial canthus. Each lacrimal system was patent to irrigation. The masses compressed and distorted the lacrimal passage and had no apparent connection with the lacrimal sac in dacryocystography or computed tomography. Surgical exploration and complete excision of the masses were completed. Each patient had an inverted Y- and an inverted V-shaped multilobular cystic mass that was pathologically confirmed as a lacrimal sac diverticulum. Lacrimal sac diverticula may rarely take the form of a multilobular cyst and can present as a lower lid mass. We speculate that an abnormality in lacrimal embryogenesis resulted in multiple blind pouches, a peculiar type of lacrimal sac diverticulum.

Phakomatous choristoma: A rare orbital tumor presenting as an eyelid mass with obstruction of the nasolacrimal duct

We describe a case of phakomatous choristoma presenting as a lower eyelid mass with nasolacrimal duct obstruction. The literature is reviewed and the unique aspects of the case are discussed.

Simultaneous Presentation of Kimura Disease and Angiolymphoid Hyperplasia With Eosinophilia

Kimura disease and angiolymphoid hyperplasia with eosinophilia are rare disease entities that can manifest as subcutaneous nodules, plaques, or papules of the head and neck, and less commonly the orbit and ocular adnexa. The merits of each entity have been debated with regard to whether they are truly individual or are separate points on a single spectrum. Current opinion favors the former. This clinicopathologic report of a patient who presented with a right lower eyelid mass with pathologic features consistent with Kimura disease and a left conjunctival mass with features consistent with angiolymphoid hyperplasia with eosinophilia poses a challenge to the notion that these are distinct conditions.

Docetaxel-induced Meibomian duct inflammation and blockage leading to chalazion formation

We report a 71-year male with castration-resistant metastatic prostate cancer who was treated with weekly docetaxel for 12 weeks and developed significant eye irritation and dryness during treatment. Subsequently, the patient presented with a lower eyelid mass, which on excision was demonstrated to be a chalazion. Docetaxel induced Meibomian duct inflammation and blockage is the likely cause of this presentation in a patient with no history of eyelid masses in the past.

Malignant Proliferating Trichilemmal Tumor of the Lower Eyelid

To report on the first case of malignant proliferating trichilemmal tumor (PTT) on the eyelid, and to describe the clinical and histopathologic findings. Interventional case report and literature review. A 42-year-old woman presented with a 2-year history of a firm, 3 x 2 cm lower eyelid mass. The mass was excised with the frozen section control. The defect in the lower eyelid was reconstructed with modified Hughes tarsoconjunctival flap and skin graft from the left postauricular area. Histopathologic examination revealed the proliferation of outer hair sheath epithelium with multiple central areas of trichilemmal keratinization. The presence of marked cellular atypia and frequent mitoses indicated the malignant transformation. Malignant PTT has not yet been reported on the eyelid. Malignant PTT should be included in the differential diagnosis for eyelid tumors.

Recurrent Solitary Reticulohistiocytoma of the Eyelid

A healthy 13-year-old female presented with a 5-month history of an enlarging red-brown, firm lower eyelid mass associated with loss of eyelashes. An excisional biopsy revealed a well-circumscribed nodule composed of coalescing nests of epithelioid cells, rare multinucleated cells, and vacuolated macrophages and spindle cells. Additional studies revealed a positive vimentin and alpha(1)-antitrypsin, and negative S100 protein and Factor XIIIa immunophenotype. This constellation of clinicopathologic features is diagnostic of solitary reticulohistiocytoma, but also shows overlap with xanthogranuloma, a common childhood histiocytosis. Reticulohistiocytoma should be considered in the differential diagnosis of eyelid tumors and has the potential for local recurrence. Diffuse cutaneous reticulohistiocytosis or multicentric reticulohistiocytosis should be considered if similar cutaneous lesions with or without symptoms of arthritis are present, respectively.

Sentinel lymphadenectomy on a rare eyelid malignant blue nevus case

Lymphatic mapping and sentinel lymphadenectomy on a rare malignant blue nevus of the eyelid are reported. The treatment was wide excision and reconstruction. A 72-year-old male was referred with a progressively growing right lower eyelid mass on a blue flat lesion. Incisional biopsy confirmed malignancy. After wide tumor excision the resulting defect was reconstructed with Mustarde's cheek flap and a chondromucosal graft. A submandibular lymph node was found with intraoperative lymphatic mapping and sentinel lymphadenectomy technique. Histopathologic examination revealed a malignant blue nevus with sentinel lymph node involvement. Additional surgery or chemotherapy was not possible because of the patient's unfavorable general health. He was followed for 1 year without local or distant metastasis. Sentinel lymphadenectomy should be in the armamentarium of surgeons. This method provides important information about regional lymph node status and staging of patients as in malignant melanoma.

Solitary metastasis of breast carcinoma to the eyelid A clinicopathological case report

Metastatic tumors to the eyelid are relatively infrequent and may masquerade as many benign and malignant lesions in the absence of a known primary malignancy. A 50-year-old woman with a suspicious breast lump who developed a diffuse nontender left lower eyelid mass in three months is reported. Simultaneous biopsies revealed infiltrating lobular breast carcinoma. There was no other metastasis. The eyelid tumor responded nicely to chemotherapy. Magnetic resonance imaging was of help in defining the exact extent of the tumor in the anterior orbit. Metastasis should be considered in the differential diagnosis of ill-defined painless indurations of the eyelid in middle-aged women.
