Diabetes mellitus (DM) is a globally growing chronic disease and is associated with numerous complications. In recent years, its potential impact on cognitive functions has gained increasing attention. This study was conducted to investigate the relationship between cognitive performance and micronutrient status in non-geriatric individuals with type 2 DM (T2DM). This case-control study included 120 patients with T2DM aged 18-65 years. Patients with cognitive impairment constituted the case group, while age-matched patients without cognitive impairment served as controls, based on Montreal Cognitive Assessment scores. Individuals with malabsorption syndromes, severe acute or chronic illnesses, prior gastrointestinal surgery, dementia, Parkinson's disease, cerebrovascular events, psychiatric disorders, use of cognition-affecting medications, or micronutrient supplementation were excluded. Fasting blood glucose (FBG), HbA1c, renal and liver function tests, total protein, albumin, iron, zinc, magnesium, calcium, vitamin B12, folate, and vitamins A, C, D, and E were measured and compared between groups. 75 females and 45 males, a total of 120 patients were included in the study. The mean age was 53.0 ± 8.4 years. Cognitive dysfunction was present in 50% of the patients. There were no statistically significant differences in the levels of FBG, HbA1c, urea, creatinine, aspartate aminotransferase (AST), alanine aminotransferase (ALT), total protein, folic acid, vitamin A, vitamin C, vitamin E according to age, gender, and cognitive dysfunction variables. Albumin, iron, zinc, magnesium, calcium, vitamin B12, vitamin D levels were significantly lower in the group with cognitive dysfunction than in the group without cognitive dysfunction. Albumin, iron, zinc, magnesium, calcium, vitamin B12, vitamin D levels in univariate model and iron, magnesium, calcium, vitamin B12 levels in multivariate model were effective in differentiating patients with and without cognitive dysfunction. This study shows that there is a significant relationship between cognitive functions and micronutrient levels in non-geriatric patients with DM. Screening of micronutrient levels and appropriate replacement of deficiencies during diabetes management is important for the protection and improvement of cognitive functions.

To evaluate cannabidiol (CBD) in pediatric patients with autism spectrum disorder (ASD), fluent verbal language and an estimated full-scale IQ of 80 or above. Preliminary evidence suggests CBD may ameliorate challenges associated with ASD. Whether CBD benefits pediatric ASD without accompanying intellectual or language impairment remains unknown. We enrolled 23 participants, ages 7.8-17.8 (Mean [M] = 11.3 ± 2.8) with ASD and IQ ≥ 80 in a 6-week Phase-2 open-label trial of ≥98% CBD (Epidiolex®, 100 mg/mL) at 3, 6, or 9 mg/kg/day using a Bayesian optimal interval dosing design. The primary endpoint was the CBD dose associated with the highest response rate (i.e., Clinical Global Impression Scale-Improvement [CGI-I] score = 1 or 2) on a target symptom domain designated individually based on informant report, standardized scales, and clinical observation. Secondary endpoints were effect sizes of changes from baseline in measures assessing ASD core and associated symptoms, and global functioning. Adverse events (AEs) were assessed weekly. Plasma CBD levels and clinical labs were obtained at the final visit. All 23 enrolled participants completed the trial. The highest response (62%) was observed at 9 mg/kg/day (N = 8). Overall, 10 of 23 patients (44%) were responders. Response varied across domains: Irritability/Tantrums (N = 5, 60%), Social (N = 8, 50%), Anxiety (N = 5, 40%), Restricted, Repetitive Behaviors (N = 4, 25%), and Sleep Problems (N = 1, 0%). Dose correlated with individualized CGI-I (r = -0.42, N = 23, p = 0.04).Individualized domain CGI-Severity scores improved from pre-(M = 4.6 ± 0.5) to post-(M = 3.9 ± 0.8) treatment, t(22) = -4.36, p ≤ 0.001; d = 0.91; 95%CI [0.37,1.03]. Social Responsiveness Scale-2 Total-Score (SRS2-T) had the largest effect size (p ≤ 0.001; d = 1.36, 95% CI [0.78-1.93]).Of 222 reported AEs, 27 unique AEs were considered treatment-related. Most AEs (93%) were mild and expected (82%); none was severe. The most frequent related AEs were increased salivation (30%), increased sleep duration (39%), sleepiness/sedation (26%), increased dream activity (35%), and polyuria (22%). Vital signs, physical exams, weight, liver function tests, and complete blood counts were unaffected. CBD plasma levels did not correlate with response. In this preliminary study, CBD was well tolerated; AEs were mild-moderate. Mean SRS2-T and subscores decreased significantly with large effect sizes, shifting from the severe to the moderate range. This study aimed to verify the efficacy and safety of balloon-occluded alternative infusion of cisplatin solution and gelatin particles of transarterial chemoembolization (BOAI-TACE) for hepatocellular carcinoma (HCC) refractory or intolerant to atezolizumab plus bevacizumab combination therapy. This prospective, single-arm study was performed at five hospitals from May 2021 to December 2024. The primary endpoint was objective response ratio (ORR) for BOAI-TACE at eight weeks after treatment. To evaluate safety, adverse events and changes in liver function were monitored. A total of ten patients were enrolled in this study with one exclusion for medical reasons. The results were complete response (CR), partial response (PR), stable disease (SD), and progressive disease (PD) for 5, 1, 1 and 2 patients, respectively. ORR (CR+PR) was 60% (95% confidence interval=26.2%-87.8%), meeting the primary endpoint. One patient suffered acute cholecystitis requiring surgery but recovered and was discharged after surgery. Another patient suffered right diaphragm paralysis after treatment but only mild shortness of breath on exertion remained. There were no statistically significant changes in liver function before and after BOAI-TACE. All nine patients were alive six months after treatment. BOAI-TACE is safe and effective in patients with HCC, even those with refractory or intolerant to immune checkpoint inhibitor therapy.
