Lesion and lesion network localization of dysnomia after epilepsy surgery.

Temporal lobe (TL) epilepsy surgery is an effective treatment option for patients with drug-resistant epilepsy. However, neurosurgery poses a risk for cognitive deficits - up to one third of patients have a decline in naming ability following TL surgery. In this study, we aimed to better understand the neural correlates associated with reduced naming performance after TL surgery, with the goal of informing surgical planning strategies to mitigate the risk of dysnomia. We retrospectively identified 85 patients who underwent temporal lobe (TL) resective surgery (49 left TL, 36 right TL) for whom naming ability was assessed before and >3 months post-surgery using the Boston Naming Test (BNT). We used multivariate lesion-symptom mapping to identify resection sites associated with naming decline, and we used lesion-network mapping to evaluate the broader functional and structural connectivity profiles of resection sites associated with naming decline. We validated our findings in an independent cohort of 59 individuals with left temporal lobectomy, along with repeating all analyses after combining the cohorts. Lesion laterality and location were important predictors of post-surgical naming performance. Naming performance significantly improved after right temporal lobectomy (P = 0.015) while a decrement in performance was observed following left temporal lobectomy (P = 0.002). Declines in naming performance were associated with surgical resection of the left anterior middle temporal gyrus (Brodmann area 21, r =0.41, P = <.001), along with a previously implicated basal temporal language area. Resection sites linked to naming decline showed a functional connectivity profile featuring a left-lateralized network closely resembling the extended semantic \ default mode network, and a structural connectivity profile featuring major temporo-frontal association white matter tracts coursing through the temporal stem. This extends prior work by implicating the left anterior middle temporal gyrus in naming decline and provides additional support for the role of the previously identified basal temporal language area in naming decline. Importantly, the structural and functional connectivity profiles of these regions suggest they are key nodes of a broader extended semantic network. Together these regional and network findings may help in surgical planning and discussions of prognosis.

Conversational production and comprehension: fMRI-evidence reminiscent of but deviant from the classical Broca-Wernicke model.

A key question in research on the neurobiology of language is to which extent the language production and comprehension systems share neural infrastructure, but this question has not been addressed in the context of conversation. We utilized a public fMRI dataset where 24 participants engaged in unscripted conversations with a confederate outside the scanner, via an audio-video link. We provide evidence indicating that the two systems share neural infrastructure in the left-lateralized perisylvian language network, but diverge regarding the level of activation in regions within the network. Activity in the left inferior frontal gyrus was stronger in production compared to comprehension, while comprehension showed stronger recruitment of the left anterior middle temporal gyrus and superior temporal sulcus, compared to production. Although our results are reminiscent of the classical Broca-Wernicke model, the anterior (rather than posterior) temporal activation is a notable difference from that model. This is one of the findings that may be a consequence of the conversational setting, another being that conversational production activated what we interpret as higher-level socio-pragmatic processes. In conclusion, we present evidence for partial overlap and functional asymmetry of the neural infrastructure of production and comprehension, in the above-mentioned frontal vs temporal regions during conversation.

Word-producing brain: Contribution of the left anterior middle temporal gyrus to word production patterns in spoken language

Vocabulary is based on semantic knowledge. The anterior temporal lobe (ATL) has been considered an essential region for processing semantic knowledge; nonetheless, the association between word production patterns and the structural and functional characteristics of the ATL remains unclear. To examine this, we analyzed over one million words from group conversations among community-dwelling older adults and their multimodal magnetic resonance imaging data. A quantitative index for the word production patterns, namely the exponent β of Heaps’ law, positively correlated with the left anterior middle temporal gyrus volume. Moreover, β negatively correlated with its resting-state functional connectivity with the precuneus. There was no significant correlation with the diffusion tensor imaging metrics in any fiber. These findings suggest that the vocabulary richness in spoken language depends on the brain status characterized by the semantic knowledge-related brain structure and its activation dissimilarity with the precuneus, a core region of the default mode network.

Functional connectivity of brain networks during semantic processing in older adults.

The neural systems underlying semantic processing have been characterized with functional neuroimaging in young adults. Whether the integrity of these systems degrade with advanced age remains unresolved. The current study examined functional connectivity during abstract and concrete word processing. Thirty-eight adults, aged 55-91, engaged in semantic association decision tasks during a mixed event-related block functional magnetic resonance imaging (fMRI) paradigm. During the semantic trials, the task required participants to make a judgment as to whether pairs were semantically associated. During the rhyme trials, the task required participants to determine if non-word pairs rhymed. Seeds were placed in putative semantic hubs of the left anterior middle temporal gyrus (aMTG) and the angular gyrus (AG), and also in the left inferior frontal gyrus (IFG), an area considered important for semantic control. Greater connectivity between aMTG, AG, and IFG and multiple cortical areas occurred during semantic processing. Connectivity from the three seeds differed during semantic processing: the left AG and aMTG were strongly connected with frontal, parietal, and occipital areas bilaterally, whereas the IFG was most strongly connected with other frontal cortical areas and the AG in the ipsilateral left hemisphere. Notably, the strength and extent of connectivity differed for abstract and concrete semantic processing; connectivity from the left aMTG and AG to bilateral cortical areas was greater during abstract processing, whereas IFG connectivity with left cortical areas was greater during concrete processing. With advanced age, greater connectivity occurred only between the left AG and supramarginal gyrus during the processing of concrete word-pairs, but not abstract word-pairs. Among older adults, robust functional connectivity of the aMTG, AG, and IFG to widely distributed bilateral cortical areas occurs during abstract and concrete semantic processing in a manner consistent with reports from past studies of young adults. There was not a significant degradation of functional connectivity during semantic processing between the ages of 55 and 85 years. As the study focused on semantic functioning in older adults, a comparison group of young adults was not included, limiting generalizability. Future longitudinal neuroimaging studies that compare functional connectivity of young and older adults under different semantic demands will be valuable.

Dissociating action and abstract verb comprehension post-stroke

The neural bases of action and abstract concept representations remain a topic of debate. While several lines of research provide evidence for grounding of action-related conceptual content into sensory-motor systems, results of traditional lesion-deficit studies have been somewhat inconsistent. Further, few studies have directly compared the neural substrates of action and relatively abstract verb comprehension post-stroke. Here, we investigated the impact of the disruption of two neural networks on comprehension of action and relatively abstract verbs in 48 unilateral left-hemisphere stroke patients using two methodologies: 1) lesion-deficit association and 2) resting-state functional connectivity (RSFC) analyses. Disruption of RSFC between the left inferior frontal gyrus and right hemisphere primary and secondary sensory-motor areas predicted greater relative impairment of action semantics. Voxel-based lesion-symptom mapping revealed that damage to frontal white matter, extending towards the inferior frontal gyrus, also predicted greater relative impairment of action semantics. On the other hand, damage to the left anterior middle temporal gyrus significantly impaired the more abstract category relative to action. These findings support the view that action and non-action/abstract semantic processing rely on partially dissociable brain networks, with action concepts relying more heavily on sensory-motor areas. The results also have wider implications for lesion-deficit association studies and show how the contralateral hemisphere can play a compensatory role following unilateral stroke.

Neural correlates of acute apraxia: Evidence from lesion data and functional MRI in stroke patients

Behavioral deficits after stroke like apraxia can be related to structural lesions and to a functional state of the underlying network – three factors, reciprocally influencing each other. Combining lesion data, behavioral performance and passive functional activation of the network-of-interest, this study aims to disentangle those mutual influences and to identify 1) activation patterns associated with the presence or absence of acute apraxia in tool-associated actions and 2) the specific impact of lesion location on those activation patterns.Brain activity of 48 patients (63.31 ± 13.68 years, 35 male) was assessed in a fMRI paradigm with observation of tool-related actions during the acute phase after first-ever left-hemispheric stroke (4.83 ± 2.04 days). Behavioral assessment of apraxia in tool-related tasks was obtained independently. Brain activation was compared between patients versus healthy controls and between patient with versus without apraxia. Interaction effects of lesion location (frontal vs parietal) and behavioral performance (apraxia vs no apraxia) were assessed in a 2 × 2 factorial design.Action observation activated the ventro-dorsal parts of the network for cognitive motor function; activation was globally downregulated after stroke. Apraxic compared to non-apraxic patients showed relatively increased activity in bilateral posterior middle temporal gyrus and middle frontal gyrus/superior frontal sulcus. Altered activation occurred in regions for tool-related cognition, corroborating known functions of the ventro-dorsal and ventral streams for praxis, and comprised domain-general areas, functionally related to cognitive control. The interaction analyses revealed different levels of activation in the left anterior middle temporal gyrus in the ventral stream in apraxic patients with frontal compared to parietal lesions, suggesting a modulation of network activation in relation to behavioral performance and lesion location as separate factors.By detecting apraxia-specific activation patterns modulated by lesion location, this study underlines the necessity to combine structural lesion information, behavioral parameters and functional activation to comprehensively examine cognitive functions in acute stroke patients.

RTMS evidence for a dissociation in short-term memory for spoken words and nonwords

Differing patterns of verbal short-term memory (STM) impairment have provided unique insights into the relationship between STM and broader language function. Lexicality effects (i.e., better recall for words than nonwords) are larger in patients with phonological deficits following left temporoparietal lesions, and smaller in patients with semantic impairment and anterior temporal damage, supporting linguistic accounts of STM. However, interpretation of these patient dissociations are complicated by (i) non-focal damage and (ii) confounding factors and secondary impairments. This study addressed these issues by examining the impact of inhibitory transcranial magnetic stimulation (TMS) on auditory-verbal STM performance in healthy individuals. We compared the effects of TMS to left anterior supramarginal gyrus (SMG) and left anterior middle temporal gyrus (ATL) on STM for lists of nonwords and random words. SMG stimulation disrupted nonword recall, in a pattern analogous to that observed in patients, compatible with a role for this site in processing speech sounds without support from long-term lexical-semantic representations. Stimulation of ATL, a semantic site, disrupted the recall of words but not nonwords. A visual pattern memory task indicated that these effects of TMS were restricted to the verbal domain. These data provide convergent evidence for the conclusions of neuropsychological studies that support linguistic accounts of verbal STM.

Ultra-high-field fMRI insights on insight: Neural correlates of the Aha!-moment.

Finding creative solutions to difficult problems is a fundamental aspect of human culture and a skill highly needed. However, the exact neural processes underlying creative problem solving remain unclear. Insightful problem solving tasks were shown to be a valid method for investigating one subcomponent of creativity: the Aha!‐moment. Finding insightful solutions during a remote associates task (RAT) was found to elicit specific cortical activity changes. Considering the strong affective components of Aha!‐moments, as manifested in the subjectively experienced feeling of relief following the sudden emergence of the solution of the problem without any conscious forewarning, we hypothesized the subcortical dopaminergic reward network to be critically engaged during Aha. To investigate those subcortical contributions to insight, we employed ultra‐high‐field 7 T fMRI during a German Version of the RAT. During this task, subjects were exposed to word triplets and instructed to find a solution word being associated with all the three given words. They were supposed to press a button as soon as they felt confident about their solution without further revision, allowing us to capture the exact event of Aha!‐moment. Besides the finding on cortical involvement of the left anterior middle temporal gyrus (aMTG), here we showed for the first time robust subcortical activity changes related to insightful problem solving in the bilateral thalamus, hippocampus, and the dopaminergic midbrain comprising ventral tegmental area (VTA), nucleus accumbens (NAcc), and caudate nucleus. These results shed new light on the affective neural mechanisms underlying insightful problem solving.

Dismissing Attachment Characteristics Dynamically Modulate Brain Networks Subserving Social Aversion.

Attachment patterns influence actions, thoughts and feeling through a person’s “inner working model”. Speech charged with attachment-dependent content was proposed to modulate the activation of cognitive-emotional schemata in listeners. We performed a 7 Tesla rest-task-rest functional magnetic resonance imaging (fMRI)-experiment, presenting auditory narratives prototypical of dismissing attachment representations to investigate their effect on 23 healthy males. We then examined effects of participants’ attachment style and childhood trauma on brain state changes using seed-based functional connectivity (FC) analyses, and finally tested whether subjective differences in responsivity to narratives could be predicted by baseline network states. In comparison to a baseline state, we observed increased FC in a previously described “social aversion network” including dorsal anterior cingulated cortex (dACC) and left anterior middle temporal gyrus (aMTG) specifically after exposure to insecure-dismissing attachment narratives. Increased dACC-seeded FC within the social aversion network was positively related to the participants’ avoidant attachment style and presence of a history of childhood trauma. Anxious attachment style on the other hand was positively correlated with FC between the dACC and a region outside of the “social aversion network”, namely the dorsolateral prefrontal cortex, which suggests decreased network segregation as a function of anxious attachment. Finally, the extent of subjective experience of friendliness towards the dismissing narrative was predicted by low baseline FC-values between hippocampus and inferior parietal lobule (IPL). Taken together, our study demonstrates an activation of networks related to social aversion in terms of increased connectivity after listening to insecure-dismissing attachment narratives. A causal interrelation of brain state changes and subsequent changes in social reactivity was further supported by our observation of direct prediction of neuronal responses by individual attachment and trauma characteristics and reversely prediction of subjective experience by intrinsic functional connections. We consider these findings of activation of within-network and between-network connectivity modulated by inter-individual differences as substantial for the understanding of interpersonal processes, particularly in clinical settings.

Capturing multidimensionality in stroke aphasia: mapping principal behavioural components to neural structures.

Stroke aphasia is a multidimensional disorder in which patient profiles reflect variation along multiple behavioural continua. We present a novel approach to separating the principal aspects of chronic aphasic performance and isolating their neural bases. Principal components analysis was used to extract core factors underlying performance of 31 participants with chronic stroke aphasia on a large, detailed battery of behavioural assessments. The rotated principle components analysis revealed three key factors, which we labelled as phonology, semantic and executive/cognition on the basis of the common elements in the tests that loaded most strongly on each component. The phonology factor explained the most variance, followed by the semantic factor and then the executive-cognition factor. The use of principle components analysis rendered participants' scores on these three factors orthogonal and therefore ideal for use as simultaneous continuous predictors in a voxel-based correlational methodology analysis of high resolution structural scans. Phonological processing ability was uniquely related to left posterior perisylvian regions including Heschl's gyrus, posterior middle and superior temporal gyri and superior temporal sulcus, as well as the white matter underlying the posterior superior temporal gyrus. The semantic factor was uniquely related to left anterior middle temporal gyrus and the underlying temporal stem. The executive-cognition factor was not correlated selectively with the structural integrity of any particular region, as might be expected in light of the widely-distributed and multi-functional nature of the regions that support executive functions. The identified phonological and semantic areas align well with those highlighted by other methodologies such as functional neuroimaging and neurostimulation. The use of principle components analysis allowed us to characterize the neural bases of participants' behavioural performance more robustly and selectively than the use of raw assessment scores or diagnostic classifications because principle components analysis extracts statistically unique, orthogonal behavioural components of interest. As such, in addition to improving our understanding of lesion-symptom mapping in stroke aphasia, the same approach could be used to clarify brain-behaviour relationships in other neurological disorders.

An fMRI study of concreteness effects in spoken word recognition

BackgroundEvidence for the brain mechanisms recruited when processing concrete versus abstract concepts has been largely derived from studies employing visual stimuli. The tasks and baseline contrasts used have also involved varying degrees of lexical processing. This study investigated the neural basis of the concreteness effect during spoken word recognition and employed a lexical decision task with a novel pseudoword condition.MethodsThe participants were seventeen healthy young adults (9 females). The stimuli consisted of (a) concrete, high imageability nouns, (b) abstract, low imageability nouns and (c) opaque legal pseudowords presented in a pseudorandomised, event-related design. Activation for the concrete, abstract and pseudoword conditions was analysed using anatomical regions of interest derived from previous findings of concrete and abstract word processing.ResultsBehaviourally, lexical decision reaction times for the concrete condition were significantly faster than both abstract and pseudoword conditions and the abstract condition was significantly faster than the pseudoword condition (p < 0.05). The region of interest analysis showed significantly greater activity for concrete versus abstract conditions in the left dorsolateral prefrontal cortex, posterior cingulate and bilaterally in the angular gyrus. There were no significant differences between abstract and concrete conditions in the left superior temporal gyrus or inferior frontal gyrus.ConclusionsThese findings confirm the involvement of the bilateral angular gyrus, left posterior cingulate and left dorsolateral prefrontal cortex in retrieving concrete versus abstract concepts during spoken word recognition. Significant activity was also elicited by concrete words relative to pseudowords in the left fusiform and left anterior middle temporal gyrus. These findings confirm the involvement of a widely distributed network of brain regions that are activated in response to the spoken recognition of concrete but not abstract words. Our findings are consistent with the proposal that distinct brain regions are engaged as convergence zones and enable the binding of supramodal input.

The role of the left anterior temporal lobe in exception word reading: Reconciling patient and neuroimaging findings

Semantic dementia (SD) is a neurodegenerative disease that occurs following the atrophy of the anterior temporal lobes (ATLs). It is characterised by the degradation of semantic knowledge and difficulties in reading exception words (surface dyslexia). This disease has highlighted the role of the ATLs in the process of exception word reading. However, imaging studies in healthy subjects have failed to detect activation of the ATLs during exception word reading. The aim of the present study was to test whether the functional brain regions that mediate exception word reading in normal readers overlap those brain regions atrophied in SD. In Study One, we map the brain regions of grey matter atrophy in AF, a patient with mild SD and surface dyslexia profile. In Study Two, we map the activation pattern associated with exception word compared to pseudoword reading in young, healthy participants using fMRI. The results revealed areas of significant activation in healthy subjects engaged in the exception word reading task in the left anterior middle temporal gyrus, in a region observed to be atrophic in the patient AF. These results reconcile neuropsychological and functional imaging data, revealing the critical role of the left ATL in exception word reading.

Broca's area and sentence comprehension: A relationship parasitic on dependency, displacement or predictability?

The current rapid event-related fMRI study used optional parasitic-gap constructions, such as [Which paper] did the tired student submit [gap--] after reviewing [p-gap--/it]?, to test 3 potential roles for Broca's area in sentence processing. These 3 functional options are: I. any intra-sentential Dependency relation activates Broca's area. II. This region specifically processes syntactic Displacement or movement. III. Broca's area handles any dependency relation, as long as it is predictable at an early stage of processing. Broca's area was only activated by the contrast that tested predictability within BA 45, as determined by its overlap with cytoarchitectonic probability maps. These results imply that an alternative or modified functional account of Broca's area, from those presently available, is required. Constraints on either a displacement account to movements that are not parasitic or a Working Memory one to predicted dependencies that cross verbal arguments or noun phrases would achieve the necessary consistency. Further, the results from the minimal contrasts investigating displacement and dependency have implications to potential language regions outside of Broca's area. The minimal contrast investigating displacement activated the left anterior Middle Temporal Gyrus, which has more recently been claimed to play a role in syntactic operations.

Self-referential reflective activity and its relationship with rest: a PET study

This study used positron emission tomography (PET) to identify the brain substrate of self-referential reflective activity and to investigate its relationship with brain areas that are active during the resting state. Thirteen healthy volunteers performed reflective tasks pertaining to three different matters (the self, another person, and social issues) while they were scanned. Rest scans were also acquired, in which subjects were asked to simply relax and not think in a systematic way. The mental activity experienced during each scan was assessed with rating scales. The results showed that, although self-referential thoughts were most frequent during the self-referential task, some self-referential reflective activity also occurred during rest. Compared to rest, performing the reflective tasks was associated with increased blood flow in the dorsomedial prefrontal cortex, the left anterior middle temporal gyrus, the temporal pole bilaterally, and the right cerebellum; there was a decrease of blood flow in right prefrontal regions and in medial and right lateral parietal regions. In addition, the ventromedial prefrontal cortex (VMPFC) (1) was more active during the self-referential reflective task than during the other two reflective tasks, (2) showed common activation during rest and the self-referential task, and (3) showed a correlation between cerebral metabolism and the amount of self-referential processing. It is suggested that the VMPFC is crucial for representing knowledge pertaining to the self and that this is an important function of the resting state.

Identification of famous faces and buildings: a functional neuroimaging study of semantically unique items.

Several functional imaging experiments have clearly established that the fusiform gyri are preferentially responsive to faces, whereas the parahippocampal/lingual gyri are more responsive to buildings. Other studies have demonstrated that famous faces additionally activate the anterior temporal cortex relative to unfamiliar faces, animals, tools, body parts and maps. One explanation for this apparent specialization for known people could be that famous faces are 'semantically unique items'. In other words, they carry unique semantic associations that are not shared by other perceptually similar category members. If this hypothesis is correct, the anterior temporal cortex should also respond to other semantically unique items, such as famous buildings. In this PET study, we investigated the effect of fame (famous relative to non-famous) on activation elicited by famous and non-famous faces and buildings during a same-different matching task. We found that, when the task was held constant, category-specific activations in the fusiform and parahippocampal/lingual areas were not modulated by fame. In contrast, in the left anterior middle temporal gyrus there was an effect of fame that was common to faces and buildings. These results suggest that the identification of famous faces and buildings involves category-specific perceptual processing in the fusiform and parahippocampal/lingual regions, respectively, and shared analysis of unique semantic attributes in the left anterior temporal cortex.

The neural systems sustaining face and proper-name processing.

This PET study has revealed the neural system involved in implicit face, proper-name and object name processing during an explicit visual 'same' versus 'different' matching task. Within the identified system, some areas were equally active irrespective of modality (faces or names) or type of stimuli (famous and non-famous) while other areas exhibited differential effects. Our findings support the hypothesis that faces and names involve differential pre-semantic processing prior to accessing a common neural system of stored knowledge of personal identity which overlaps with the one associated with object knowledge. The areas specialized for the perceptual analysis of faces (irrespective of whether they are famous or non-famous) are the right lingual and bilateral fusiform gyri, while the areas specialized for famous stimuli (irrespective of whether they are faces or names) spread from the left anterior temporal to the left temporoparietal regions. One specific area, the more lateral portion of the left anterior middle temporal gyrus, showed increased activation for famous faces relative to famous proper names and for famous proper names relative to common names. The differential responsiveness of this region when processing familiar people suggests functional segregation of either personal attributes or, more likely, the demands placed on processes that retrieve stored knowledge when stimuli have highly similar visual features but unique semantic associations.

Functional anatomy of a common semantic system for words and pictures.

The relationship between the semantic processing of words and of pictures is a matter of debate among cognitive scientists. We studied the functional anatomy of such processing by using positron-emission tomography (PET). We contrasted activity during two semantic tasks (probing knowledge of associations between concepts, and knowledge of the visual attributes of these concepts) and a baseline task (discrimination of physical stimulus size), performed either with words or with pictures. Modality-specific activations unrelated to semantic processing occurred in the left inferior parietal lobule for words, and the right middle occipital gyrus for pictures. A semantic network common to both words and pictures extended from the left superior occipital gyrus through the middle and inferior temporal cortex to the inferior frontal gyrus. A picture-specific activation related to semantic tasks occurred in the left posterior inferior temporal sulcus, and word-specific activations related to semantic tasks were localized to the left superior temporal sulcus, left anterior middle temporal gyrus, and left inferior frontal sulcus. Thus semantic tasks activate a distributed semantic processing system shared by both words and pictures, with a few specific areas differentially active for either words or pictures.
