Microvascular structural changes in esophageal squamous cell carcinoma pathology according to intrapapillary capillary loop types under magnifying endoscopy.

The intrapapillary capillary loop (IPCL) characteristics, visualized using magnifying endoscopy, are commonly assessed for preoperative evaluation of the infiltration depth of esophageal squamous cell carcinoma (ESCC). Japan Esophageal Society (JES) classification is the most widely used classification. Microvascular structural changes are evaluated by magnifying endoscopy for the presence or absence of each morphological factor: tortuosity, dilatation, irregular caliber, and different shapes. However, the pathological characteristics of IPCLs have not been thoroughly investigated, especially the microvascular structures corresponding to the deepest parts of the lesions' infiltration. To investigate differences in pathological microvascular structures of ESCC, which correspond to the deepest parts of the lesions' infiltration. Patients with ESCC and precancerous lesions diagnosed at Peking University Third Hospital were enrolled between January 2019 and April 2023. Patients first underwent magnified endoscopic examination, followed by endoscopic submucosal dissection or surgical treatment. Pathological images were scanned using a three-dimensional slice scanner, and the pathological structural differences in different types, according to the JES classification, were analyzed using nonparametric tests and t-tests. The 35 lesions were divided into four groups according to the JES classification: A, B1, B2, and B3. Statistical analyses revealed significant differences (a P < 0.05) in the short and long calibers, area, location, and density between types A and B. Notably, there were no significant differences in these parameters between types B1 and B2 and between types B2 and B3 (P > 0.05). However, significant differences in the short calibers, long calibers, and area of IPCL were observed between types B1 and B3 (a P < 0.05); no significant differences were found in the density or location (P > 0.05). Pathological structures of IPCLs in the deepest infiltrating regions differ among various IPCL types classified by the JES classification under magnifying endoscopy, especially between the types A and B.

The patterns and risk factors for relapse in oesophageal squamous cell cancers that achieve pathological complete response to neoadjuvant chemoradiotherapy.

The goal of this study was to investigate the patterns and risk factors for recurrence in patients with oesophageal squamous cell carcinoma with a pathological complete response (pCR) after neoadjuvant chemoradiotherapy (nCRT). Between January 2008 and December 2018, a total of 96 patients with pCR were enrolled in this study. Lymph nodes with a pCR [LN-ypCR response (+)] were defined as those lymph nodes without residual tumour but with the presence of treatment response to nCRT. Prognostic factors for recurrence-free survival (RFS) were analysed with Cox proportional hazards models and Fine-Gray competing risk models. Lymph node (LN) stations were counted according to the Japan Esophageal Society classification. The median follow-up time was 51.5 months. Recurrence occurred in 15 cases (15.6%) with a 9.9-month median time to recurrence and a 15.6-month median survival after recurrence. The majority of recurrent diseases developed within the first 2 years postoperatively. Distant recurrences were detected in 14 cases (14.6%), in which the most common recurrence sites were no.104 LN and the lung, followed by no.16 LN. The mean RFS in the whole cohort was 116.6 months. The LN-ypCR response (+) was identified as the independent prognostic factor for worse RFS in both the multivariate Cox model and the Fine-Gray competing risk model (P = 0.001 and P = 0.002, respectively). Relapse is not rare in oesophageal squamous cell carcinoma cases with pCR after nCRT. Distant recurrences, the predominant pattern of relapse, occur primarily within the first 2 years after oesophagectomy. Patients with pCR with an LN-ypCR response (+) have a higher risk for postoperative recurrence.

Diagnostic Accuracy of JES Classification for Prediction of Invasion Depth in Superficial Esophageal Squamous Carcinoma: Meta-Analysis.

Prediction of the depth of invasion in superficial oesophageal squamous carcinoma (SESC) is an important factor for choosing the treatment. Recently, the Japan Esophageal Society (JES) designed a magnifying endoscopy classification based on the Inoue and Arima classifications. The aim of this study was to conduct a meta-analysis of the published literature on JES classification. Meta-Disc version 1.4, Review Manager 5.4 as well as stata 14.0 were used. The analysis combined sensitivity and specificity with the respective 95% CI, to draw a summary receiver operating characteristic curve (SROC), and estimated the area under curve (AUC) for overdiagnosis and underdiagnosis for each type B. Eight studies were included in the meta-analysis comprising 1279 patients. Type B1 has high sensitivity (0.86, 95%CI: 0.83-0.88) and specificity (0.84, 95%CI: 0.81-0.88) for the prediction of EP/LPM classifications. The AUC was calculated to be 0.92 with a high proportion of underdiagnosis (17%). The sensitivity and specificity of type B2 were 0.66 (95% CI, 0.6-0.72) and 0.84 (95% CI, 0.82-0.86) respectively. The overdiagnosis and underdiagnosis of type B2 were 14% and 39%. Type B3, sensitivity was low (0.49, 95% CI: 0.41-0.56), with high specificity and AUC (specificity: 0.99; AUC: 0.95). JES classification is a useful and reliable modality for predicting the depth of invasion of SESC, but other modalities should be considered for additional assessment when type B2 is detected. Evaluating the usefulness of considering the size and morphological type of type B2 vessel area based on Japan Esophageal Society classification in estimating tumor invasion depth in superficial esophageal squamous cell carcinomas: study protocol for a prospective observational study (Japan BEES study)

BackgroundAccurate evaluation of tumor invasion depth is essential to determine the appropriate treatment strategy for patients with superficial esophageal cancer. The pretreatment tumor depth diagnosis currently relies on the magnifying endoscopic classification established by the Japan Esophageal Society (JES). However, the diagnostic accuracy of tumors involving the muscularis mucosa (MM) or those invading the upper third of the submucosal layer (SM1), which correspond to Type B2 vessels in the JES classification, remains insufficient. Previous retrospective studies have reported improved accuracy by considering additional findings, such as the size and macroscopic type of the Type B2 vessel area, in evaluating tumor invasion depth. Therefore, this study aimed to investigate whether incorporating the size and/or macroscopic type of the Type B2 vessel area improves the diagnostic accuracy of preoperative tumor invasion depth prediction based on the JES classification.MethodsThis multicenter prospective observational study will include patients diagnosed with MM/SM1 esophageal squamous cell carcinoma based on the Type B2 vessels of the JES classification. The tumor invasion depth will be evaluated using both the standard JES classification (standard-depth evaluation) and the JES classification with additional findings (hypothetical-depth evaluation) for the same set of patients. Data from both endoscopic depth evaluations will be electronically collected and stored in a cloud-based database before endoscopic resection or esophagectomy. This study’s primary endpoint is accuracy, defined as the proportion of cases in which the preoperative depth diagnosis matched the histological depth diagnosis after resection. Outcomes of standard- and hypothetical-depth evaluation will be compared.DiscussionCollecting reliable prospective data on the JES classification, explicitly concerning the B2 vessel category, has the potential to provide valuable insights. Incorporating additional findings into the in-depth evaluation process may guide clinical decision-making and promote evidence-based medicine practices in managing superficial esophageal cancer.Trial registrationThis trial was registered in the Clinical Trials Registry of the University Hospital Medical Information Network (UMIN-CTR) under the identifier UMIN000051145, registered on 23/5/2023.

Diagnostic Efficacy of Type B Vessels in the Japan Esophageal Society Classification for the Depth of Invasion of Superficial Esophageal Squamous Cell Carcinoma.

The preoperative diagnostic method for superficial esophageal squamous cell carcinoma (SESCC) invasion depth based on the Japan Esophageal Society (JES) classification has been promoted. However, there have been a few investigations into its diagnostic performance in clinical settings. Therefore, we aimed to elucidate the actual diagnostic performance of the JES classification using a single-center retrospective study design. We retrospectively analyzed the clinical data of 315 newly diagnosed SESCC patients who underwent narrow-band imaging magnifying endoscopy (NBI-ME) examination and received endoscopic submucosal dissection (ESD) or esophagectomy in our center during the past 5 years. To evaluate the diagnostic performance of JES classification in assessing the depth of invasion of SESCC, clinical data of these patients were collected, and the concordance between NBI-ME findings and postoperative pathology reports was analyzed. This study included a total of 338 lesions. The diagnostic accuracy of vascular morphology was 76.0%. The sensitivity (87.0%) and positive predictive value (PPV, 85.4%) of B1 vessels were high, but the specificity (42.0%) and negative predictive value (NPV, 45.3%) were low. The specificity (86.9% and 98.8%) and NPVs (87.5% and 96.8%) of B2 and B3 vessels were high, but the sensitivity (36.4% and 21.4%) and PPVs (35.1% and 42.9%) ware low. Furthermore, only a few lesions (n = 57) described avascular area, but the overall diagnostic accuracy was not ideal (21.1%). However, if lesions invading the superficial submucosa or shallower were included in the category of "suitable for ESD", the overall accuracy of the JES classification was found to be 95.6%. In actual clinical settings, the overall accuracy of the JES classification system decreases, but the diagnostic performance of each subtype retains its original characteristics. Additionally, this classification is appropriate for determining whether type 0-II SESCC lesions are suitable for ESD.

Gastrointestinal: Unusual pathology of an anorectal lesion mimicking a benign laterally spreading tumor.

A 70-year-old woman was referred to our institution for the diagnosis and treatment of an anorectal lesion detected on colonoscopy performed at a previous hospital for hematochezia. Colonoscopy revealed a 32-mm flat elevated lesion in the lower rectum with an anal component extending to the dentate line (Fig. 1a). Chromoendoscopy with indigo carmine spraying demonstrated a papillary surface (Fig. 1b). Magnifying endoscopy with narrowband imaging (NBI) displayed an irregular vessel resembling the intrapapillary capillary loop (IPCL) of the Japan Esophageal Society Classification Type B1 (Fig. 1c). Magnifying endoscopy with 0.05% crystal violet staining revealed the absence of discernible glandular ducts, hindering the evaluation of the pit pattern (Fig. 1d). The endoscopic appearance resembled a large adenoma or condyloma acuminatum; however, the absence of a pit pattern indicated that the lesion was covered with squamous epithelium and not columnar. Moreover, the irregular vessel pattern suggested a neoplastic lesion. Based on these observations, the lesion was diagnosed as a neoplasm originating from the squamous epithelium of the anal canal. Thus, we performed endoscopic submucosal dissection for complete elimination. Histological examination revealed a papillary growing squamous cell carcinoma (SCC) exhibiting low-grade and high-grade atypia and abundant dilated vessels (Fig. 2a). The tumor extended from the anal transitional zone (ATZ) to the rectum and entrapped the rectal crypts (Fig. 2b). Some tumor cells had a clear cytoplasm, indicative of koilocytosis (Fig. 2c), and positive p16 immunohistochemical staining suggested human papillomavirus (HPV) infection (Fig. 2d). The final diagnosis was HPV-associated SCC in situ originating from the ATZ with rectal extension. Anal canal cancer (ACC) is a rare disease that accounts for only 0.3% of all malignancies and is strongly associated with high-risk HPV (HPV-16 and HPV-18). This case was unusual because of the extensive infiltration of the columnar epithelium. Therefore, it might be misidentified as a rectal adenoma. Early ACC shows irregular vessel patterns reminiscent of esophageal SCC on magnified endoscopic NBI, which has been reported to be useful in diagnosing ACC. However, laterally spreading tumor (LST) can exhibit similar irregular capillary vessels. The difference in endoscopic appearance between ACC and LST can be explained by their different epithelial anatomies. ACC develops in the squamous cell epithelium and has no orifice or crypt. In contrast, LST originates from the columnar epithelium and has crypt openings. Condyloma acuminatum has a similar endoscopic appearance but with milder vascular dilatation and caliber changes. In conclusion, HPV-associated anal canal cancer can be suspected in papillary tumors with an irregular esophageal SCC-like vascular pattern and absence of glandular structures. Piecemeal endoscopic mucosal resection can be selected for large adenomas; conversely, en bloc resection is the preferred method for ACC to mitigate the risk of recurrence. To ensure the implementation of appropriate treatment, more attention should be paid to lesions extending along the anal canal to the rectum.

Prediction of depth of invasion and lymph node metastasis in superficial pharyngeal cancer by magnifying endoscopy using the Japan Esophageal Society classification.

BackgroundsThe pharynx has no muscularis mucosae, so it is unclear whether diagnostic techniques used for the esophagus can be applied to the pharynx. This study investigated the usefulness of magnifying endoscopy with narrowband imaging using the Japan Esophageal Society (JES) classification for predicting the depth of invasion and lymph node metastasis (LNM) in pharyngeal cancer.MethodsA total of 123 superficial pharyngeal carcinoma lesions that had been observed preoperatively with magnifying endoscopy with narrowband imaging between January 2014 and June 2021 were analyzed. Predictors of subepithelial invasion (SEP) and LNM were sought based on endoscopic findings, including microvascular morphology, using the JES classification.ResultsThe lesions were divided into carcinoma in situ (n = 41) and SEP (n = 82). Multivariate analysis identified B2–B3 vessels (odds ratio [OR] 6.54, 95% confidence interval [CI] 1.74–24.61, p = 0.005) and a middle/large avascular area (OR 4.15, 95% CI 1.18–14.62, p = 0.027) as independent predictors of SEP. Significant predictors of LNM were protruding type, B2–B3 vessels, middle/large avascular area, SEP, venous invasion, lymphatic invasion, and tumor thickness > 1000 μm. Median tumor thickness increased significantly in the order of B1 < B2 < B3 vessels (B1, 305 μm; B2, 1045 μm; B3, 4043 μm; p < 0.001). The LNM rates for B1, B2, and B3 vessels were 1.6% (1/63), 4.8% (2/42), and 55.6% (10/18), respectively (p < 0.001).ConclusionsMagnifying endoscopy with narrowband imaging using the JES classification could predict the depth of invasion in superficial pharyngeal carcinoma. The JES classification may contribute to the prediction of LNM, suggesting that it could serve as an alternative to tumor thickness.

Usefulness of the Japan Esophageal Society Classification of Barrett’s Esophagus for Diagnosing the Lateral Extent of Superficial Short-Segment Barrett’s Esophageal Cancer

Introduction: The Japanese guidelines for endoscopic submucosal dissection (ESD) of Barrett’s esophageal adenocarcinoma (BEA) recommend image-enhanced magnifying endoscopic examination for diagnosing the lateral extent of superficial esophageal adenocarcinoma. The Japan Esophageal Society Barrett’s Esophagus (JES-BE) classification is proposed recently and is useful in terms of diagnostic accuracy. In this study, we retrospectively examined the usefulness of the JES-BE classification for differential diagnosis and determination of the extent of BEA originating in short-segment Barrett’s esophagus. Methods: The study reviewed 51 lesions which underwent ESD for BEA. The circumference of the esophagogastric junction was divided into four parts, and the lesions were divided into those in the right anterior portion (RA group; n = 33) and those in other portions (non-RA group; n = 18). Clinicopathological characteristics and clinical outcomes were compared between the two groups. Results: JES-BE classification findings as “dysplasia” were seen in 48 out of 51 (94.1%) BEA lesions retrospectively. There was no significant difference in histological type, tumor depth, lymphovascular invasion, or the proportion of tumors with a positive or unknown horizontal or vertical margin status between the groups. The proportion of tumors with type 0-I morphology was significantly higher in the RA group (p = 0.023). The tumor size was significantly greater in the RA group (p = 0.034). According to the JES-BE classification, 31 lesions (93.9%) in the RA group and 17 lesions (94.4%) in the non-RA group were diagnosed as dysplasia. There was also no significant difference in the rate of consistency between the endoscopic and histopathological findings on the lateral extent of the lesion (90.9% vs. 83.3%; p = 0.612). Discussion/Conclusions: The JES-BE classification may be useful for determining the extent of BEA.

A prospective multicenter study of the magnifying endoscopic evaluation of the invasion depth of superficial esophageal cancers.

Treatment strategies for superficial esophageal squamous cell carcinoma (SESCC) are determined mainly on the basis of the invasion depth. The Japan Esophageal Society (JES) developed a simplified magnifying endoscopic classification for estimating the invasion depth of SESCC. We aimed to evaluate its accuracy. We prospectively applied the JES classification for estimating the invasion depth of SESCC to 204 consecutive lesions from 6 hospitals in Japan between April 2016 and October 2018. We analyzed the accuracy of the endoscopic diagnosis by adding the following two categories to the JES classification: ≥ 7mm lesion in B2 vessels (defined as B2 ≥ 7mm) and B2 vessels with inflammation (defined as B2i). After applying the exclusion criteria, 201 lesions remained in the analysis. The diagnostic value of type B1, B2, B3 vessels were as follows: sensitivity, 93.9%, 68.0%, 25.0%; specificity, 81.1%, 89.2%, 99.4%; positive predictive value (PPV), 95.6%, 47.2%, 75.0%; negative predictive value (NPV), 75.0%, 95.1%, 95.4%; and accuracy, 91.5%, 86.5%, 95.0%, respectively. A retrospective analysis showed that the diagnostic accuracy was higher in type B2 vessels (86.5% to 92.0%). An avascular area (AVA) was found in 55 (27%) of the 201 lesions, which tended to be associated with a deeper pathological diagnosis of each Type B vessel. In an additional analysis, B2 ≥ 7mm and B2i improved the diagnostic accuracy of type B2 vessels from 86.5% to 92.0%. The JES classification is useful for estimating the invasion depth of SESCC. The diagnostic accuracy for type B2 vessels was low, which may be improved by using B2 ≥ 7mm and B2i.

Relationship between the microvascular patterns observed by magnifying endoscopy with narrow-band imaging and the depth of invasion in superficial pharyngeal squamous cell carcinoma.

Prediction of the invasive depth is the objective of endoscopic observation for digestive cancer. In superficial esophageal cancer, a close relationship between microvascular patterns observed by magnifying endoscopy with narrow-band imaging (M-NBI) and pathological depth of invasion is well known. The ability of M-NBI to predict the invasion depth in superficial pharyngeal squamous cell carcinoma (SPSCC) has been seldom evaluated. This study aimed to clarify the relationship between the microvasculature patterns and pathological depth in SPSCC. SPSCC lesions evaluated with M-NBI followed by endoscopic resection were analyzed between April 2010 and March 2017. Endoscopic images were classified as microvasculature tumor types B1, B2, and B3 according to the Japan Esophageal Society classification. The pathological depth of invasion was described as either squamous cell carcinoma in situ (Tis) or invasive subepithelial cancer, and the tumor thickness of all lesions was examined. Data were analyzed using the unpaired t, χ2, or Mann-Whitney U test. Type B1 and type B2/B3 (35/3) microvessels were found in 180 lesions (82%) and 39 (18%), respectively. Of the flat lesions, 115 (83%) were classified as Tis and 23 (17%) as subepithelial cancer. Positive and negative predictive values of the B1 vessels were 77% and 82%, respectively. Additional analysis showed that the positive predictive value of the B1 vessels for the flat-type lesions was 87%; the negative predictive value for the elevated lesions was 93%. Microvascular patterns observed by M-NBI are an important factor in predicting the pathological depth of invasion.

Exploration of a new intrapapillary capillary loop classification in diagnosis of superficial esophageal squamous cell carcinoma staging: a meta-analysis and single-center study.

Though intrapapillary capillary loops (IPCL) can be used to diagnose the staging of superficial esophageal squamous cell carcinoma (SESCC), the accuracy still remains controversial. Moreover, which IPCL classification is more suitable for clinical application is unclear. The aim was to evaluate the diagnostic performance of different IPCL classifications and to explore a new classification. A meta-analysis was conducted to compare the diagnostic efficiency of the three IPCL classifications for SESCC staging. STATA 12.0 software was used and bivariate mixed effects-model was applied for the meta-analysis. A new classification was developed based on the meta-analysis result. Then a single-center study was conducted for further validation analysis. Of the 2310 citations, 14 studies fulfilled our criteria. For epithelium (EP) and lamina propria mucosa (LPM) staging tumors, IPCL showed high diagnostic accuracy (Inoue: 87.17%, Arima: 98.71%, JES: 86.70%). For muscularis mucosa (MM) and submucosa (SM1), 23.12% were underestimated and the Japan Esophageal Society classification (JES) was better than Inoue (Accuracy: 75.48% VS 58.67%, P < 0.05). For ≥ SM2, 38.86% were underdiagnosed and the diagnostic accuracy of Arima classification was significantly higher than the other two (P < 0.05). Our study showed that the new criteria had higher diagnostic sensitivity than the widely used classification (JES) for differentiating ≥ SM2 staging cancer from EP-SM1 (0.77 VS 0.50, P < 0.05). IPCL was valuable for diagnosing SESCC staging. However, ≥ MM would be underestimated easily. Our study revealed that the diagnostic sensitivity for ≥ SM2 staging tumors was significantly improved by the new standard.

Preoperative indicators of misdiagnosis in invasion depth staging of esophageal cancer: Pitfalls of magnifying endoscopy with narrow-band imaging.

The Japan Esophageal Society classification has been widely applied for predicting the invasion depth of superficial esophageal squamous cell carcinomas (SESCCs). Although Type B2 of the classification clinically corresponds to SESCCs with muscularis mucosa or slight submucosal invasion (MM/SM1), diagnostic yield based on Type B2 is insufficient. This study aimed to investigate risk factors for misdiagnosis in preoperative invasion depth staging. We included a total of 104 SESCCs in which Type B2 was observed by magnifying endoscopy. SESCCs were classified as either correct diagnosis (pMM/SM1, 39 lesions), overdiagnosis (epithelium or the lamina propria [pEP/LPM], 34 lesions), or underdiagnosis (deep invasion into the submucosa [pSM2-3], 31 lesions) based on pathological depth of invasion. The association between misdiagnosis and endoscopic features, including distinct features, was evaluated using logistic regression analysis. Distinct features were defined as nodular protrusion, thickness, and/or clearly depressed area. The diameter of type B2 area was endoscopically measured, and the cut-off value was determined using a receiver operating characteristic curve. Type B2 area <6mm (area under the curve, 0.776) and Type B2 vessels around erosion were significantly associated with overdiagnosis (odds ratio, 16.6 and 11.0, respectively), while distinct features were significantly associated with underdiagnosis (odds ratio, 8.7). Adjusted by these misdiagnosis factors, positive predictive value of Type B2 significantly improved from 38% to 65% (P<0.01). Lesions with a small Type B2 area (<6mm) and/or Type B2 vessels around erosion should be diagnosed as EP/LPM and lesions with distinct features as SM2-3.

Numeric pathologic lymph node classification shows prognostic superiority to topographic pN classification in esophageal squamous cell carcinoma

The current eighth tumor node metastasis lymph node category pathologic lymph node staging system for esophageal squamous cell carcinoma is based solely on the number of metastatic nodes and does not consider anatomic distribution. We aimed to assess the prognostic capability of the eighth tumor node metastasis pathologic lymph node staging system (numeric-based) compared with the 11th Japan Esophageal Society (topography-based) pathologic lymph node staging system in patients with esophageal squamous cell carcinoma. We retrospectively reviewed the clinical records of 289 patients with esophageal squamous cell carcinoma who underwent esophagectomy with extended lymph node dissection during the period from January 2006 through June 2016. We compared discrimination abilities for overall survival, recurrence-free survival, and cancer-specific survival between these 2 staging systems using C-statistics. The median number of dissected and metastatic nodes was 61 (25% to 75% quartile range, 45to 79) and 1 (25% to 75% quartile range, 0 to 3), respectively. The eighth tumor node metastasis pathologic lymph node staging system had a greater ability to accurately determine overall survival (C-statistics: tumor node metastasis classification, 0.69, 95% confidence interval, 0.62-0.76; Japan Esophageal Society classification; 0.65, 95% confidence interval, 0.58-0.71; P=.014) and cancer-specific survival (C-statistics: tumor node metastasis classification, 0.78, 95% confidence interval, 0.70-0.87; Japan Esophageal Society classification; 0.72, 95% confidence interval, 0.64-0.80; P=.018). Rates of total recurrence rose as the eighth tumor node metastasis pathologic lymph node stage increased, while stratification of patients according to the topography-based node classification system was not feasible. Numeric nodal staging is an essential tool for stratifying the oncologic outcomes of patients with esophageal squamous cell carcinoma even in the cohort in which adequate numbers of lymph nodes were harvested.

Prediction of the invasion depth of superficial squamous cell carcinoma based on microvessel morphology: magnifying endoscopic classification of the Japan Esophageal Society

Predicting invasion depth of superficial esophageal squamous cell carcinoma is crucial in determining the precise indication for endoscopic resection because the rate of lymph node metastasis increases in proportion to the invasion depth of the carcinoma. Previous studies have shown a close relationship between microvascular patterns observed by Narrow Band Imaging magnifying endoscopy and invasion depth of the superficial carcinoma. Thus, the Japan Esophageal Society (JES) developed a simplified magnifying endoscopic classification for estimating invasion depth of superficial esophageal squamous cell carcinomas. We conducted a prospective study to evaluate the diagnostic values of type B vessels in the pretreatment estimation of invasion depth of superficial esophageal squamous cell carcinomas utilizing JES classification, the criteria of which are based on the degree of irregularity in the microvascular morphology. Type A microvessels corresponded to noncancerous lesions and lack severe irregularity; type B, to cancerous lesions, and exhibit severe irregularity. Type B vessels were subclassified into B1, B2, and B3, diagnostic criteria for T1a-EP or T1a-LPM, T1a-MM or T1b-SM1, and T1b-SM2 tumors, respectively. We enrolled 211 patients with superficial esophageal squamous cell carcinoma. The overall accuracy of type B microvessels in estimating tumor invasion depth was 90.5 %. We propose that the newly developed JES magnifying endoscopic classification is useful in estimating the invasion depth of superficial esophageal squamous cell carcinoma.

The intrapapillary capillary loop (IPCL) changes in superficial esophageal lesions.

The aims of this study were to investigate intrapapillary capillary loops (IPCLs) of superficial esophageal lesions changes in different types classified by the Japan Esophageal Society classification. The calibers, areas, and densities of IPCLs were detected in 34 cases of esophageal lesions using immunohistochemical analysis. Statistically significant differences in calibers, areas, and densities of IPCLs were observed between type A, type B1/B2, and type B3 area (P < 0.001). In conclusion, the results of this observation showed the Japan Esophageal Society classification of IPCL would help endoscopists to diagnose the type and the invasion depth of lesion in esophagus, and decide the treatment strategy.

Although narrow band imaging (NBI) and endoscopic ultrasonography (EUS) are useful for determining superficial ESCC invasion depth, few reports have shown the risk factors of misdiagnosis by such modalities. Aim: This study aimed to clarify the usefulness and risk factors for misdiagnosing superficial ESCC invasion depth using NBI and EUS. Methods: Between October 2008 and April 2011, 47 patients (39 men; mean age, 67.4 years) with 52 superficial ESCCs who underwent NBI and EUS before ESD or surgery were included in this study. Patients who had received chemotherapy or radiotherapy were excluded. Three endoscopists reviewed the recorded endoscopic images collected using NBI and EUS and evaluated the invasion depth as EP/LPM, MM/SM1 (submucosal invasion 200 m), or SM2 (deeper than SM1). We used the Japan Esophageal Society classification of NBI composed of vessel shape and width, and avascular area. The diagnostic accuracies of NBI and EUS were compared with the pathology. Multivariate analysis was performed to clarify the risk factors of invasion depth misdiagnosis. The cut-off values of major diameter and circumferential extension were calculated using receiver operating characteristic curve analysis. Results: The average accuracies of NBI and EUS were 76.9% (40/ 52) and 67.3% (35/52), respectively. Through NBI, the following were correctly diagnosed: 31 of 32 (96.9%) EP/LPM lesions, 6 of 14 (42.9%) MM/SM1 lesions, and 3 of 6 (50.0%) SM2 lesions. Overestimation occurred in 8 lesions and underestimation occurred in 4 lesions. Through EUS, the following were correctly diagnosed: 31 of 34 (91.2%) EP/LPM lesions, 2 of 6 (33.3%) MM/SM1 lesions, and 2 of 12 (16.7%) SM2 lesions. Overestimation occurred in 11 lesions and underestimation occurred in 6 lesions. Multivariate analysis showed that major diameter (odds ratio [OR], 1.65; 95% confidence interval [CI], 1.03-2.64; p 0.04) and circumferential extension (OR, 1.49; 95% CI, 1.04-2.15; p 0.03) were the risk factors of NBI misdiagnosing invasion depth. In contrast, no significant differences were noted for EUS. The cut-off values calculated on major diameter and circumferential extension were 28 mm (95% CI, 0.56-0.86; area under the curve [AUC], 0.71) and 0.50 (95% CI, 0.57-0.87; AUC, 0.72), respectively. The average accuracies of NBI and EUS were 91.7% (22/24) and 79.2% (19/24) or 64.3% (18/28) and 57.1% (16/28), respectively, in the lesions 28 mm or 28 mm of the major diameter. Conclusion: Major diameter and circumferential extension are risk factors of misdiagnosing invasion depth for superficial ESCC using NBI endoscopy. NBI might be useful for diagnosing superficial ESCC invasion depth, especially in cases with major diameters 28 mm.

Tu1588 Usefulness of Japan Esophageal Society Classification of Magnified Endoscopy for the Diagnosis of Superficial Esophageal Squamous Cell Carcinoma

Grades of dysplasia can help predict risk of EA. Pathologists cannot distinguish regenerative changes from true dysplasia in up to 20% of patients (Indeterminate for dysplasia -IND). The rate of progression and clinical significance of IND are uncertain. Endoscopic surveillance and treatment patterns are also not standardized. Objectives: To understand the practice patterns and clinical outcomes for patients with BE IND. Methods: We conducted a retrospective study of patients with BE with IND from 2000-2010 at one university referral center. Statistical analysis was performed with SPSS (17.0). Results: 322 patients with BE were identified. 48 patients with IND on index EGD were identified. 24 patients were excluded since they had other degrees of dysplasia prior to or at the time of index EGD. All patients were non-Hispanic white (75% males, median age 53). 14 patients had a documented diagnosis of GERD (58%). 17 patients were on a PPI (71%). 14 patients (58%) had a recommendation for follow up after the first EGD (median 46 weeks, 6-104 weeks). The median actual follow up EGD was 40 weeks (range 12-108). Findings on repeat EGD included IND (23%), no dysplasia (46%), and low grade dysplasia (LGD) (31%). Of the patients who continued to have surveillance EGD, almost 50% had no dysplasia on subsequent biopsies. Endoscopic surveillance intervals ranged from every 2 months to every 16 months. Only 1 patient had high grade dysplasia (HGD) with an incidence of 14.3 patients per 1000 person-year. 11 patients had histologic evidence of esophagitis on the index EGD (46%). Patients without dysplasia on the 2nd EGD were more likely to have had histologic evidence of esophagitis on the 1st EGD. None of the patients with LGD on the 2nd EGD had histologic evidence of esophagitis on the 1st EGD. All patients with persistent IND or documented progression of dysplasia had a hiatal hernia. Patients who progressed to LGD or continued to have IND were more likely to be on a PPI. Immunohistochemical staining for biomarkers was performed on the initial biopsy of 12 out of 14 patients who had a second EGD. p53, -methylacylcoenzyme A racemase (AMACR), and beta-catenin were positive in 9 (75%), 7 (58%), and 6 (50%) patients. All 3 markers were positive in the patient who progressed to HGD. Re-examination of H&E slides by 2 pathologists confirmed indefinite of dysplasia in 12 out of 13 patients. Conclusions: Patients with IND may be at low risk of progression to HGD. Significant variation in endoscopic surveillance patterns for IND was found. Biomarkers may help predict progression of dysplasia. More research is needed to better understand this population and guide resource utilization in patients with BE.
