Investigation of Occupational Accidents in Western Lignite Corporation by Using the Efficiency Assessment Approach

The mining sector, which has an important place in terms of raw material and energy supply, is referred as a sector with one of the highest fatal accidents in comparison with other sectors. In this century, where occupational health and safety awareness has increased, it is important to review the past accident statistics and to determine the current deficiencies by using the information in the inventory. In this study, the Malmquist total factor productivity index (MPI) has been utilized to investigate the changes in the work accidents occurring in the Western Lignite Corporation (Turkey), where underground and surface production methods are applied, and to determine the measures to be taken. According to the results obtained, both the decrease in production amount and the increase in the number of accidents caused inefficiencies during the period in concern due to the expected 10-year economic life of the mechanized system operating in the underground production panel that came to an end in 2007 and the flooding took place in early 2017. Compared with the underground operations, it has been determined that production can be carried out at the appropriate scale despite the heavy working conditions encountered due to the changing climatic conditions and the progress of production in the surface operations.

Quality evaluation of official accident reports conducted by Labour Authorities in Andalusia (Spain).

A public accident investigation is carried out when the consequences of the incident are significant or the accident has occurred in unusual circumstances. We evaluated the quality of the official accident investigations being conducted by Safety Specialists of the Labour Authorities in Andalusia. To achieve this objective, we analysed 98 occupational accident investigations conducted by the Labour Authorities in Andalusia in the last quarter of 2014. Various phases in the accident investigation process were examined, such as the use of the Eurostat variables within European Statistics on Accidents at Work (ESAW), detection of causes, determination of preventive measures, cost analysis of the accidents, identification of noncompliance with legal requirements or the investigation method used. The results of this study show that 77% of the official occupational accident investigation reports analysed were conducted in accordance with all the quality criteria recommended in the literature. To enhance glogal learning, and optimize allocation of resources, we propose the development of a harmonized European model for the public investigation of occupational accidents. Further it would be advisable to create a common classification and coding system for the causes of accidents for all European Union Member States.

An investigation of occupational accidents and safety risks in policing: Views of employees

Policing is one of the riskiest and dangerous professions by its nature. Police officers face a range of risks at work: homicide, assaults, attacks, communicable diseases, car crashes or explosions. The risks vary according to the task being undertaken such as arresting offenders, attending street disturbances or performing traffic duties. These risks, having the characteristics of occupational accident in a way, have institutional losses like compensation, loss of manpower and reputation besides individual results like injury, death, mutilation, and posttraumatic stress disorder, exposure to psychological disorders or decrease in quality of life. Opinions and suggestions of 1066 employees currently working at different ranks and units in Turkish National Police in regards with reducing the risks of occupational accidents and safety risks were studied in this research. Suggestions of the participants were reviewed under total nine headings (themes) consisting of training, physical fitness and health, security measures, institutional policies and procedures, managerial policies, working conditions, equipment, uniforms, and patrol cars.

Ergonomic Criteria in the Investigation of Indirect Causes of Accidents

Guidelines on ergonomic deficiencies may be derived from accident reports which commonly name employees as being directly at fault (direct causes of accident). The root causes of accidents found of significance for prevention purposes include the misalignment of workstations with the psychophysical capabilities of workers. Such causes lead to the engagement of ergonomists. Accident analysis tools and detailed specifications of direct accident causes have been used to develop a concept of a method for identifying ergonomic deficiencies. The accident investigation methods and tools used to classify accident causes by means of specified criteria make it possible to identify ergonomic deficiencies at each stage of assessment. The author has additionally described a concept of an expert system which supports the investigation of occupational accidents from the ergonomic standpoint. The accident investigation methods identified in this paper are TOL, Job Safety Analysis (JSA), “What if …”, FMEA, STEP, OARU (Occupational Accident Research Unit), FTA, the Ishikawa Diagram, the energy transfer method, “4xwhy”, MORT, KIK, WAIT (Work Accident Investigation Technique), as well as the ILCI and TRIPOD models. A concept has also been offered for using network methods to establish a hierarchy of ergonomic incompliances.

Analysis of investigation reports on occupational accidents

The investigation of accidents is an occupational safety analytical tool aimed at discovering the causes of an accident. Conducting these investigations properly is essential to obtain useful information that helps avoid these accidents in the future.To prepare this study we analysed 567 investigations, conducted by OHS technical advisors, on occupational accidents occurring in Spain from 2009 to 2012 in industries such as construction, manufacturing, agriculture and services, in order to obtain information to improve the use of this technique. In this study we analysed how accident investigation reports are made identifying main flaws and omissions. Accident investigations lack details as they often do not consider the variables in the ESAW (European Statistics on Accidents at Work) Project. Likewise, they lack depth in determining the causes associated to active faults, preferably to latent faults, and to the company management and organisation systems. Similarly, they do not comply with the standards recommended by experts.Finally, in the conclusions we recommend two priorities: having a harmonised European model to conduct occupational accident investigations, as well as being able to access databases that collect accident investigation reports of this kind.

Framework for the use of official occupational accident investigations as a learning tool: analysis of a public programme for accident investigation in the manufacturing sector

Accident investigation is a useful tool for safety science. The use of accident investigation as a learning tool at the macro level necessitates specific requirements quite different from the usual needs at the company level. A proper codification system and information on the accident scenarios are needed to let safety practitioners identify if the information is useful for the organisations they are assessing. In the same way epidemiological tools have been applied to the analysis of circumstances of accidents, the epidemiological statistical tools can help to draw conclusions for a set of investigated accidents. Our case study includes the description of the dataset used in a programme for learning from accidents in Andalusia: ‘Pudo haberse evitado’ (this can be translated as ‘This could have been prevented’). This paper covers the requisites for a macro-level learning programme and the possibilities of analysis of the dataset in a ‘research to practice approach’.

What fatal occupational accident investigators can learn from fatal aircraft accident investigations

Our aim is to provide an overview of what international aircraft accident investigation practises could teach fatal occupational accident (FOA) investigators. The main purpose of an accident investigation is to prevent future accidents. This aim is easier to achieve if investigations are independent; biases and external pressures can place undue influence on particular recommendations. So far, independence has been easier to achieve in aviation investigations than in fatal occupational accident investigations. In FOAs, tensions often arise between different stakeholders including but not limited to industry regulators, police agencies, industry bodies, trades unions, insurance companies, etc. There are also a host of practical problems that complicate key activities in FOA investigations, including the need to prevent the contamination of evidence in the immediate aftermath of an accident. We argue that greater emphasis needs to be placed on the independence of FOA investigations and that this will help move the emphasis of recommendations from the worker level (sharp end) to the workplace and community system level (blunt end).

Occupational Injury and Fatality Investigations

The forensic evaluation of trauma in occupational injuries and fatalities can provide the benefit of a more thorough analysis of incident causation. Forensic nursing science applied during workplace investigations can assist investigators to determine otherwise unknown crucial aspects of the incident circumstances that are important to event reconstruction, the enforcement of occupational health and safety requirements, and the direction of workplace prevention initiatives. Currently, a medical and forensic medical knowledge gap exists in the subject-matter expertise associated with occupational accident investigations. This gap can be bridged with the integration of forensic nursing in the investigation of workplace fatalities and serious injuries.

Investigation of Occupational Accidents Induce Sever Injury in Fars, Iran from 2005 to 2007

Occupational accidents and incidents are increased in parallel of growth and expansion of industries. Implementation of preventive measures requires detailed analysis of accidents. So, the major goal of this study was analyze data on occupational accidents induced sever injury in Fars province, in the period 2005-2007 in order to use clues that would support political definition of priorities and strategies for prevention.Data were collected from 200 accidents registered in the Fars Labor Organization. Inclusion criteria were having an occupational accident during 2005-2007 and availability of the events records.Analysis was done using SPSS software (version 16.0) and descriptive analysis was used.The majority of injured workers (98%) were male. Of the accidents, 79% were occurred in Fars’ capital. The most frequent occupational accidents and injuries occurred in private industries (97.5%). Most commonly affected workers were aged group 21 - 35. Results also shown that,84% of workers had low education under diplomalevel. Workers with experience less than one year (12 month) had more accident (52.5%) than others. The most frequent occupational accidents occurred in construction activities (construction building). Of the accident, 24.13% were occurred due to neglect of regulations and lack of supervision. Unsafe act and unsafe condition were causes of 21.68% accidents.Accidents analysis and assessment could be used to identify component causing accidents and to give recommendations for accident prevention. Some considerations should be regarding for preventive measures which included: implementing and evaluating training program and establishment of OHS system, as well as insurance position

Eyewitness testimony in occupational accident investigations: towards a research agenda.

Accident investigation is frequently cited as the cornerstone of an effective occupational health and safety program. We suggest that the literature on accident investigation is based on a model of witnesses as neutral and accurate recording devices. The literature on eyewitness testimony and criminal investigation offers strikingly different conclusions. We review these findings and point to their implication for research on accident investigation in occupational health and safety contexts.

Reducing risks by deviation control—a retrospection into a research strategy

A conceptual model for practical investigation of occupational accidents within industry was presented in 1981. The model has served as a theoretical framework for research into safety management tools including methods for accident investigation, safety audits and risk analysis and computerized safety information systems. The original model is presented and the subsequent theoretical and applied research is reviewed. The significance of the model to the field of occupational accident research in general and to safety practice is discussed. It is concluded that all aspects of the original model have not survived the test of time. The model and the subsequent research has contributed to a converging trend as regards accident theories and models. The contribution from the model mainly lies in an improved understanding of the accident process and of the deviation concept as a common element in many theories and models of accidents.

Investigating accidents and reducing risks — A dynamic approach

A conceptual model for practical investigation of occupational accidents within the industrial company is presented. The model comprises two levels of investigation. On the accident sequence level, deviations or chains of deviations are mapped. On the second, determining factors level, the production process conditions related to the accident are highlighted. A preliminary version of the model has been used in five pilot studies within different branches of Swedish industry. Results show that information existing within the company on accident sequences and determining factors, but not included in the accident reports used by the local safety organisation, could be tapped by applying the model in the investigative procedure. The authors propose to use the model in intervention projects within Swedish companies aimed at establishing new methods in local accident prevention. The analysis of accidents and the development of counter-measures will take place in a structured group setting including workers, management and staff.
