Orbital fractures that require surgical intervention pose challenging problems to the trauma surgeon. Many options have been described to gain access to the subperiosteal plane orbit of the orbit. The approach to be adopted should be tailored to the site and extent of the fractures, and the experience of the surgeon. We prefer approaches that utilize hidden incisions as they provide safe surgical access but heal without visible scarring and are esthetically more acceptable to patients. We describe approaches to repair orbital fractures via hidden incisions in the caruncle, and the inferior conjunctival fornix.
