International distance education and the transition from ISDN to high-bandwidth Internet connectivity

summary The Thailand-Hawaii Assessment of Interactive Healthcare Initiative (THAI-HI) is an international distance-education project between two teaching hospitals in Honolulu and Bangkok that uses videoconferencing over three ISDN lines. A ‘morning report’ format is used to discuss clinical cases primarily covering infectious disease and critical-care topics. An audience response system is used at both sites to add interactivity. From July 2001 to May 2002, 816 health-care providers attended 20 clinical conferences. Audiences rated the conferences as highly relevant and as having high training value. Since the ISDN connection is expensive, we plan to convert the telecommunications to a high-bandwidth Internet connection. The Honolulu site will use a 45 Mbit/s commercial connection to the Hawaii Intranetwork Consortium, which links to the Abilene Network on the US mainland. The Bangkok hospital will use a 155 Mbit/s wireless optical connection to UNINET Thailand, which has a 45 Mbit/s circuit to Abilene.

Infection Highlights 2000-01

This 98-page paperback in the Fast Facts series contains concise updates on a diverse group of topics in infectious diseases. Chapters cover newly recognized and emerging infectious diseases problems including Escherichia coli O157:H7, Helicobacter pylori, and Acinetobacter. Other chapters provide therapeutic updates of a range of problems including exacerbations of chronic obstructive pulmonary disease, HIV infection, and onychomycosis. Most reviews are relevant to the clinician with the exception of discussions on alternative treatments for methicillin-resistant Staphylococcus aureus (MRSA) and antibiotic-resistance genes in plants. There are 12 chapters, and most of the authors are recognized authorities in their areas.

Modulation of immunodominant sites in influenza hemagglutinin compromise antigenic variation and select receptor-binding variant viruses.

The regions of antigenic variation in influenza hemagglutinin (HA) are located on surface-accessible regions in the three-dimensional structure of the HA1 monomer. The aim of this study was to establish whether a novel variant virus, IMUT4, in which we had mutated specific amino acid residues (HA1 63, 144, 158, and 193) in these regions, previously shown to be immunodominant for CBA/Ca mice, would either (a) establish holes in the antibody (ab) repertoire or (b) preclude further antigenic variation in IMUT4. CBA/Ca mice were able to mount a neutralizing ab response to IMUT4 infection and molecular recognition sites were established by sequencing of the HA genes of monoclonal antibody (mAb)-selected laboratory variants of wild-type X31 virus (HA1 131, 145, 155, and 196). However, each of these mAbs failed to select further antigenic variants of IMUT4, in ovo, but rather a receptor binding mutant (HA1 190 Glu-->Asp; 226 Leu-->Gln) that was still recognized by the selecting mAb, specific for HA1 155 of X31 virus. The facility for antigenic variation in influenza would appear to be compromised, therefore, by targeted mutation of immunodominant sites, as initially proposed by S. It is interesting to note that recent isolates of the H3 subtype, (e.g., A/Beijing/92) obtained between 1991 and 1993, contain the same substitutions at HA1 190 and 226, which may indicate similar constraints to immune evasion and the relevance of our findings to antigenic variation in the human population.

ABSTRACT As a pediatrician in practice initially at an urban health center and now at a university-based program, I have found that the most common diagnostic and treatment issues requiring my retreat to the reference shelf involve infectious diseases and their manifestations in children. With my practice now largely devoted to caring for children infected with the human immunodeficiency virus (HIV), this is even more the case.Although there are several outstanding general pediatric textbooks available that cover most infectious disease topics, I found the breadth and depth of information in them insufficient. In contrast, in the 12 years since the first edition of this text appeared, I have found it to be the best single reference on pediatric infectious diseases. A bibliography and comments on the use of statistical models in epidemiology in the 1980s

This paper reviews developments in statistical modelling in epidemiology in the 1980's, with emphasis on cohort and case-control studies. The central roles of the logistic and proportional hazard models are highlighted, and it is shown how these models lead to a deeper understanding of classical designs and methods of analysis as well as to efficient new designs and analytical procedures. The important area of model misspecification is discussed, including the problems of omitted latent structure, mis-modelling of available measurements, missing data and errors in measurements. Various designs motivated by the logistic model are illustrated numerically, and designs based on the proportional hazards model are discussed, as are papers on sample size determination. There are brief introductions to the literature on other topics, including attributable risk, disease clustering, family studies and genetics, analysis of disease incidence data, infectious disease, longitudinal data, screening and miscellaneous related topics in statistics. An extensive bibliography is indexed according to the outline of the paper.

This is the fourth volume in a series entitled Current Clinical Topics in Infectious Diseases. Like its predecessors, it is excellent. Practitioners from all areas of medical practice will find among the 16 chapters one or more of particular interest to them.Nelson, in a chapter called “A Critical Review of the Role of Oral Antibiotics in the Management of Hematogenous Osteomyelitis,” gives a detailed and thoughtful discussion of his own experience of some 400 cases, and reviews all of the published series of others. Useful tables clarify the text.“When Can the Infected Hospital Employee Return to Work?” includes constructive data relevant to the employee with active pulmonary tuberculosis, with streptococcal or staphylococcal infections, with Salmonella or Shigella infections, viral hepatitis or herpes simplex.The chapter by Munford on “Orbital Infections” is particularly well illustrated with anatomical drawings of the area, explaining the pathogenesis of these infections.

Infective Endocarditis and Other Intravascular Infections

The medical community owes much to Dr Freedman for his pioneering work more than a decade ago in developing an experimental animal model of endocarditis. Now Dr Freedman provides us with a clinically oriented book on endocarditis and related vascular infections (the fifth in a series of volumes devoted to current topics in infectious disease). Spanning the spectrum from the laboratory to the bedside, the author leaves few stones unturned. He carefully lays the groundwork by showing how vegetations form on endothelial surfaces, why only certain bacterial strains colonize the vegetations, and what the consequences of such processes are. With the insight of the dedicated student of endocarditis that he is, the author probes the subject from anatomy to zygomyces. We read about the controversy concerning potential benefits of addition of aminoglycosides to penicillinase-resistant penicillins for treatment of staphylococcal endocarditis. This is the third annual appearance of a multiauthored review of topics of interest in infectious diseases, edited by two distinguished professors. The subjects chosen for discussion are topical because of current clinical importance and changes in methods of diagnosis and treatment or represent in-depth reviews of somewhat neglected topics. The authors of the individual articles are recognized experts. The chapter by Owen, Sexually Transmitted Enteric Disease, reviews a topic that is of increasing current importance and interest. The broadening concept of sexually transmitted diseases, which includes enteric protozoa, metazoal viruses, and bacteria, is reviewed in a thorough and scholarly manner. The developing field of viral chemotherapy as typified by nucleoside derivatives and interferons as antiviral agents is ably reviewed by Hirsch and Hammer.
