Exploratory study on the mechanism of necrotic effect of nourishing cells in the context of genital tract infection in premature rupture of membranes.

To explore the mechanism of necrotic effect of nourishing cells in the context of genital tract infection in premature rupture of membranes (PROM). One hundred eight patients with PROM treated at our hospital from June 2020 to June 2022 were selected as the PROM group. Simultaneously, 108 cases of normal full-term pregnant women were chosen as the control group. Western blot analysis was performed to measure the relative expression levels of cysteinyl aspartate specific proteinase-1 (Caspase-1), cysteinyl aspartate specific proteinase-3 (Caspase-3), nucleotide-binding oligomerization domain-like receptor family pyrin domain containing 3 (NLRP3), and interleukin (IL)-1β proteins, which are associated with necrosis of placental nourishing cells, in the placenta of both groups. TUNEL staining was used to detect the number of apoptotic placental nourishing cells. The differences in necrotic factors of placental nourishing cells were analyzed between full-term and preterm cases in the PROM group, as well as among patients with different genital tract infections. The apoptotic count of placental nourishing cells in the PROM group was 58.46 ± 11.26 cells/field, which was markedly higher than that of the control group (P < .05). The relative expression levels of the necrotic factors Caspase-1, Caspase-3, NLRP3, and IL-1β proteins in placental nourishing cells of the PROM group were 1.32 ± 0.26, 1.19 ± 0.30, 1.29 ± 0.28, and 1.23 ± 0.24, respectively. These values were significantly higher than those of the control group (P < .05). The relative expression levels of the necrotic factors Caspase-1, Caspase-3, NLRP3, and IL-1β proteins in placental nourishing cells were compared between full-term and preterm patients in the PROM group (P > .05). The relative expression levels of the necrotic factors Caspase-1, Caspase-3, NLRP3, and IL-1β proteins in placental nourishing cells were higher in patients with multiple genital tract infections compared to those with single infections or no infections in the PROM group (P < .05). PROM is associated with a significant upregulation of placental nourishing cell apoptosis and necrotic factors, including Caspase-1, Caspase-3, NLRP3, and IL-1β proteins. This upregulation is correlated with the presence of genital tract infections.

Urinary tract infection in premature rupture of membrane (PROM): an academic hospital based study

Premature rupture of membrane (PROM) and preterm PROM (PPROM) are commonly related with poor maternal and perinatal outcomes. Urinary tract infection (UTI) has been known as one of its risk factors. The aim of study was to ascertain the frequency and pattern of urinary symptoms as well as the risk factors for UTI in PROM and PPROM. A retrospective study was conducted at Dr. Sardjito General Hospital, Yogyakarta, Indonesia. All pregnancy cases with the history of PROM/PPROM from January to December 2015 were included. The research subjects were obtained from medical records, using the format of basic data collection to identify the risk factors of UTI in pregnancy. One hundred cases of complicated pregnancy with either PROM or PPROM were obtained. The mean of maternal age, gestational age, and birth weight were 28 ± 5.99 yr; 34.05 ± 4:28 wk; 2170.79 ± 835.447 g; respectively. Urinalysis was done in 58 patients. The prevalence of bacteriuria was 55.17%. Symptomatic vs. asymptomatic bacteriuria showed statistically significant differences (p<0.001, OR = 0.409; CI = 0287-0584). In multivariate analysis using linear regression, maternal age, gestational age and parity were not directly related to the occurrence of UTI (p = 0.367; p = 0.697; p = 0.385; respectively). In conclusion, the proportions of symptomatic bacteriuria in pregnancy are significantly higher than asymptomatic. However, maternal age, gestational age, and parity are not directly related to the prevalence of UTI in pregnancy complicated with PROM and PPROM.

Concealed abruptio-placenta and disseminated intravascular coagulopathy: a near fatal management experience in a peripheral center

Disseminated Intravascular Coagulopathy (DIC) is a fatal complication. In pregnant mothers concealed abruptio placenta though less commonly occurring but more dangerous is a compounding factor for development of DIC. Ascending uterine infection in premature rupture of membranes and PIH are also compounding factors in causing DIC. However, the outcome is not very positive in majority of the cases. While managing such cases in peripheral setting, it is most pertinent to keep in mind the possibility of rapid deterioration of clinical condition of such patients, which might progress to fatality. Therefore, timely institution of resuscitative and supportive measures in such labile patient is of utmost necessity while delivering peri and post-operative care. The interesting case report discussed in this patient also was of life threatening severity but timely institution of whole blood transfusion, FFP and fluid support was immensely helpful in saving the patient in peripheral setup despite her undergoing three consecutive life saving surgeries and a peri-operative cardiac arrest.

Evaluation of a Universal Real-Time Polymerase Chain Reaction for Detection of Amniotic Fluid Infection in Premature Rupture of Membranes

Managing preterm rupture of membranes (PPROM) is a balance between benefits of prolonging gestation and the risks of perinatal infection. This study evaluates a real-time polymerase chain reaction (PCR) for the detection of amniotic fluid infection and neonatal complications by amniocentesis following PPROM. A total of 61 singleton pregnancies with PPROM were analyzed retrospectively, including histopathologic examination of the placenta and neonatal complications. The real-time PCR detects a highly conserved sequence of the bacterial 16S ribosomal DNA, and its efficacy was compared with standard tests including amniotic fluid glucose concentration, lactate dehydrogenase level, and maternal white cells count and C-reactive protein levels. Sensitivity and specificity were similar for PCR (64% and 85%) and standard tests (58% and 80%). However, the PCR technique has the additional advantage of possible identification of the bacteria through sequencing and has a much better positive predictive value in the occurrence of neonatal complications (60% for PCR versus 35% for standard tests). The latency period between premature rupture of the membranes and delivery was not related to the incidence of histopathologic signs of infection in the placenta or the umbilical cord and was inversely related to the incidence of neonatal complications.

Fetal breathing as a predictor of infection in premature rupture of the membranes.

The value of the presence or absence of fetal breathing in predicting infection was determined by a retrospective analysis of 130 patients with premature rupture of the membranes and no clinical signs of infection or labor. The last ultrasound examination performed within 48 hours of delivery was used for comparison to infection outcome, as reflected by the development of clinical amnionitis, possible neonatal sepsis, and neonatal sepsis. The sensitivity and specificity of fetal breathing in predicting infection in patients with premature rupture of the membranes were 91.6 and 64.8%, respectively. These data suggest that the presence of fetal breathing is a good predictor of noninfection outcome (negative predictive value 95.3%), whereas its absence does not necessarily indicate impending infection (positive predictive value 50%).

Among the causes of infant mortality, premature birth has been found to rank high. Many of these premature births are brought about by premature rupture of the membranes. On this basis, a study was undertaken to determine the cause of premature rupture.It seemed logical that an infectious process in the cervical canal might extend itself to the membranes overlying the internal os; these membranes, becoming inflamed and friable, might then give way under the duress of a strong Braxton Hicks contraction or other strain.The literature was searched for a similar approach to this problem, but we were unable to find such reports.
