Enhancing awareness of breast self-examination and pap smears for early detection of breast and cervical cancer

Reproductive health problems, especially breast cancer and cervical cancer are the most common cancers diagnosed in women in Indonesia. Currently, several testing methods to detect breast and cervical cancer early, such as breast self-examination (BSE) and pap smears, have been used to reduce cancer risk factors. However, these two methods are still not widely used. Outreach activities have been proven to increase knowledge about the importance of BSE and pap smears in women. This community service program aims to increase knowledge about the causes and symptoms of breast and cervical cancer, as well as early detection using BSE and pap smears. This service activity plan consisted of preparation, implementation, and evaluation stages. The method used consists of delivering interactive material, discussion, and practice. This activity was attended by 38 participants, consisting of women from PKK cadres in the Andir area, Ciroyom District, the majority of whom were over 35 years old. Based on the results of the post-test, this community service activity was successful in increasing participants' knowledge, attitudes, and behavior towards breast and cervical cancer as well as the early detection methods of BSE and pap smears.

Influence of variables on breast self-examination: Potential barrier or enhancement of breast cancer prevention.

Breast self-examination (BSE) is an important tool for early detection of breast cancer and can contribute to increased awareness and familiarity with breast tissue. This study aimed to review factors that influenced BSE in preventing breast cancer. In the initial stage of searching for journal articles, 186 journals were obtained, then screening was carried out within a time span of 2019-2024 and using Indonesian and English 54 journals. Of the 12 journals reviewed, the results showed that 4 journals reviewed had conducted pre and post-BSE practices, and 8 journal reviews conducted BSE practices conducted by students and health workers. Several factors play an important role in implementing BSE, namely age, knowledge, history of exposure to information, family history, ethnicity, and attitude. Overall, the study shows that the need to create awareness and educate women, especially rural women, on the importance of BSE as a preventive measure for breast cancer is paramount. Factors influencing the implementation of BSE can be a barrier or an enhancement in breast cancer prevention.

This examination can easily detect signs of breast cancer. Conclusion: Breast cancer is a deadly cancer and affects many women in Indonesia and even around the world. The BSE examination is an early detection that is easy to carry out, and anxiety in breast cancer sufferers is normal. Patients who are afraid of undergoing surgery prefer complementary therapy, but this therapy takes longer to heal.

Delay in cancer management can be caused by several things, namely patient delay, doctor or medical personnel's ignorance (doctor delay) and pre-hospital delay (pre-hospital delay). Hospital delay is also a cause of delay in treatment in breast cancer patients, namely the time from the patient's arrival to the point of care until the start of the prescribed medical therapy guidelines. From some of these causes, it can also be grouped the factors that cause cancer delay in the form of predisposing factors, precipitation factors and supporting factors. The purpose of this study was to determine how the factors of delay in breast cancer treatment were treated at the Cut Meutia General Hospital, Aceh Utara. This type of research is descriptive analytic with a retrospective approach. The population in this study were inpatients and outpatients for breast cancer totaling 85 respondents using the Lemeshow formula. The results of the study showed that the value of knowledge was 37 people (43.5%), there was fear as many as 56 people (65.9%), low socioeconomic conditions as many as 40 people (47.1%), affordable health facilities were 64 people (75.3%), had insurance. As many as 77 people (90.6%), had visited alternative medicine as many as 49 people (57.6%) and had family support as many as 65 people (76.5%). The conclusion of this research is the importance of BSE (Breast Self-Examination) education to the public.

Pentingnya Pengetahuan Pemeriksaan Payudara Sendiri sebagai Deteksi Dini Kanker Payudara

Breast cancer is included in the five types of cancer that result in death. The reason is the delay in early detection so that survivors of this type of cancer are generally detected at an advanced stage. The community service entitled "PKM Knowledge and Early Detection of Breast Cancer in Women of Reproductive Age" aims to provide knowledge about the importance of breast self-examination (BSE) as an effort to detect breast cancer early. The method used is through online seminars via Zoom Meetings to 20 employees and their families at the Obstetrics and Gynecology Clinic, Muhammadiyah Asri Medical Center Hospital, Yogyakarta. The material provided was in the form of knowledge about women's health, breast cancer and its treatment, and training on BSE, which was preceded by a pretest to measure participants' knowledge before the seminar and a posttest at the end of the activity after receiving the seminar. The results of the pretest and posttest were analyzed using the Wilcoxon Signed-Rank test method. The results of the pretest showed that 17 participants (85.0%) had sufficient knowledge about BSE and 2 participants (10.0%) had less knowledge. The posttest results showed that all participants (100.0%) had knowledge in the good category about BSE. The test results showed a significance value of 0.001 (p 0.05), which means that there was a significant increase in participants' knowledge about BSE as early detection of breast cancer before and after the seminar.

Greek Women’s Knowledge and Perceptions Related to Breast Cancer Prevention

Background: Breast cancer is the most common form of cancer and the leading cause of cancer death in womenworldwide. The aim of this survey was to investigate Greece women’s knowledge and perceptions related tobreast cancer prevention.Methods: A reliable questionnaire from the literature was used which then was translated using the forward andbackward translation method. It contained 37 questions and the sample consisted of 650 women. The statisticalanalysis was completed using SPSS 25. The study found that the majority of the sample had knowledge regardingbreast cancer prevention, they knew the importance of breast self-examination and annual clinical breastexamination needed to be conducted as well as the age to undergo their first mammogram. Age, occupation,annual family income, education level, and marital status significantly influenced women’s knowledge andperceptions related to breast cancer prevention.Conclusions: The state, the media and schools contribute to raising awareness and the involvement of healthprofessionals is essential to better inform and educate adolescent girls and women on this issue.

Effect of health education on awareness and practices of breast self examination among females attending a charitable hospital at North Karachi.

To assess the effectiveness of health education as an intervention to promote breast self-examination in a low-resource setting. The quasi-experimental study was conducted from January to August 2018 in Karachi after approval by the Jinnah Sindh Medical University, and comprised women from a low resource locality and dividing them into intervention group A and control group B. Demographic data was collected using a pretested questionnaire filled through interviews. Subsequently, group A received health education regarding carcinoma of breast, importance of breast self-examination and monthly motivation through cell phone to perform self-examination. The questionnaire was again filled after 6 months of intervention. Data was analysed using SPSS 20. Group B was given the same health education sessions after the completion of the study. Of the 172 subjects, there were 86(50%) in each of the two groups. In terms of demographic data, the groups were similar (p>0.05). After the intervention, group A showed significant (p<0.001) improvement in knowledge and practice of breast self-examination. There was no change in group B (p>0.05). Being in the intervention group (p=0.001) and level of education (p=0.018) showed positive and negative associations with selfexamination practice. Upon adjusting for age, marital status, family history and education, group A (p=0.001) remained significant, while the level of education (p=0.116) became non-significant. Designed health promotion programmes with monthly reminders through cell phone improved knowledge and practice of breast self-examination.

Women's Awareness Level on Breast Self-Examination

Aim: The aim of this study was to investigate the level of awareness, regarding the importance of breast self examination (BSE), clinical breast examination (CBE) and mammography, and the effects of education on these issues among patients who applied to family medicine outpatient clinic for follow-up or treatment purposes.Methods: This study was done by establishing a questionnaire form, after review of necessary litetarure, to be applied in patients who applied for follow-up or treatment in a district polyclinic affiliated to the department of family medicine of a tertiary care center in Ankara.Results: A total of 434 women aged between 18-56 years participated in the study. The mean age of the participants was 36.62±9,21 years. Although 258 (59.4%) of the participants reported that they performed BSE. 176 participants (40.6%) stated that they did not perform a BSE. The 38.2% of the participants have undergone a CBE and the 24 % of women expressed that they underwent a mammography.The frequency of BSE, CBE and undergoing mammography has been shown to increase significantly as education level increases. The rates of having a CBE and a mammography were also found to be statistically significantly higher in patients who had a family member with breast cancer.Conclusion: Our results demonstrated that the level of education was important for awareness on the early diagnosis of breast cancer. Thanks to efforts to increase educational level, we can get promising achievements regarding the awareness of both BSE, CBE and mammography performances.

A Study to Assess the Knowledge and Expressed Practices regarding Breast Self-Examination among Women in a Selected Area of Haryana

Breast self-examination is a very simple screening tool for breast cancer. It requires minimal training to be imparted to women. They are instructed to detect any asymmetry with the other breast in terms of a lump, change in breast shape or of the overlying skin (puckering), any abnormal discharge from the nipples &amp;/or any breast lesion that does not heal easily. Teaching women about this importance of breast self-examination is a core subject of community nursing. It was therefore thought important to assess the background knowledge regarding the same amongst women of a selected area in Haryana. The aim of the study was to assess the knowledge and expressed practices regarding breast self-examination among women in a selected area of Haryana. A sample of 50 women was chosen. Data collection was done by using Google form questionnaire after assessing their willingness and ability to participate in this survey by filling up this online form. After assessing the knowledge level, it was found that 80% (n=40) were having adequate knowledge regarding breast self-examination and 20% (n=10) were having inadequate knowledge regarding breast self-examination. 52% of the women (n=26) were practicing breast self-examination and rest were not. Awareness regarding breast self-examination was adequate in the sample studied. However, the practice of performing the same was present in only about 50% of the women in the above-mentioned sample. The findings of the study highlights the need for educational programs to create awareness among women regarding regular breast screening behavior.

Assessment of knowledge and practice of breast self-examination among reproductive age women in Akatsi South district of Volta region of Ghana.

Breast cancer is the primary cause of cancer death among women globally, responsible for about 425,000 deaths in 2010. This study assessed the awareness, knowledge and practices of breast self-examination as a method of prevention and early diagnosis of breast cancer among reproductive aged women in Akatsi South district in Volta region of Ghana. This study was a cross-sectional study involving 385 women between the ages of 15-49 years. Data were collected with a structured questionnaire and variables included socio-demographic characteristics, breast cancer knowledge, breast self-examination knowledge and practice. Descriptive statistics were used to analyze and present the data and chi square test of significance was used to determine association between socio-demographic variable and practice of breast self-examination. The mean age of the women was 24.54±7.19. Only 3.1% of women had no formal education and 58.9% were single. Although 88.3% of the respondents were aware of breast cancer, 64.9% of the respondents had good or sufficient knowledge of breast cancer and only 94(37.6%) practice BSE. Over 50% of the respondents did not know how to perform BSE. There was a significant association between knowledge on breast cancer and practice of BSE (χ2 = 36.218 p = 0.000). The higher the age of a participant, the lower practice of breast self-examination and this was significant (χ2 = 11.324, p = 0.003). Breast self-examination is a key strategy to early detection of breast cancer and subsequently critical for effective treatment and cure of the disease. The findings in this study have shown significant low levels of awareness and practice of breast self-examination among women in Akatsi South district of the Volta region. This pattern may be similar to other rural communities across the region. The need to create awareness and to educate women, especially rural women, on importance of breast self-examination as preventive measure for breast cancer is paramount.

Behavior BSE (breast self-examination) is very important for adolescent girls. Breast self-examination is one way to detect a lump or tumor in the breast. If adolescent girls know the importance of breast self-examination can be as an effort to prevent breast cancer earlier. The purpose of this research is to find out determinant adolescent girls behavior conduct BSE in efforts to prevent breast cancer in SMA Negeri 5 Pekanbaru. The kind of analytic quantitativ research used the cross sectional design. The population in this study was a hight school students totaling 443 people. The sample in this study amounted to 107 adolescent girls using the random sampling technique. The results showed that knowledge was not related to breast self-examination with p value 0.234, attitude was not related to breast self-examination with p value 0.095, results for people closely related to breast self-examination with p value 0,008 and source of information related to breast self-examination with p value 0,002. A study of awareness and practices regarding breast cancer among women of self help groups in rural Puducherry.

Background: Breast cancer is a major cause of morbidity and mortality among females throughout the world. India, the cancers are the third important cause of increasing mortality rate. In India, the cases of breast cancer are steadily rising. Objective: To study the awareness and practices about breast cancer among women of SHGs in rural Puducherry. Materials and methods: The community based cross sectional study with both quantitative and qualitative technique, was conducted in four villages which come under the field practice area of Rural Health Training Centre (RHTC), department of Community Medicine. Information from the SHG women regarding socio-demographic characteristics, awareness regarding breast cancer and practice of Breast self examination (BSE) was collected Results: Nearly half of the SHG women were in the age group of 25-40 years while only 46.4% of women found to have finished their higher secondary education. The SHG women were aware regarding lump (33.9%), pain (20.5%) followed by change in the size of the breast (14.7%). Majority of SHG members (97.6%) had not heard about mammogram as a method of screening method of Breast cancer. The awareness of BSE was about (20.08 %). Majority (90.8%) of the study population have not received demonstration on BSE. Only 14.8% of SHG women knew the importance of BSE in early diagnosis. The awareness and practice of BSE among these SHG women was quite less in the present study. Conclusion: Awareness of breast cancer, screening methods and practice of BSE were found to be less in SHG member women. Keywords: Self help groups, women, breast cancer, BSE

Role of Technology (Breast Self Examination Animation Video) in Breast Cancer Screening in Indian Tribal Rural Women

Background: The Lifeline Express or Jeevan Rekha Express is World´s first hospital train run by the Impact India Foundation. It was developed in collaboration with the Indian Railways and Health Ministry and has been funded by Impact UK, international charitable sources, Indian corporate houses and individuals. It started on July 16, 1991; as of 2016 the service had completed almost 200 projects, benefiting over 900,000 rural Indians. Aim: This study aimed to study the role of superspeciality doctor (endocrine and breast surgeon) in this setting. Methods: The specialist surgeon was assigned a task screen and teach the importance of breast self examination in the rural setting. The women were divided into batches of 50 and were shown a PowerPoint presentation with a video on breast self examination. The screening was done by 2 female general surgeons and any kind of abnormal finding was consulted to the endocrine and breast surgeon. At the end of 15 minutes lecture on screening and breast self examination, the ladies were provided a pamphlet on breast self examination in their native language and also advised and requested to teach another 5 women in the community. Results: 1000 ladies were screened and taught them the importance of breast self examination. Each lady was requested to teach another 5 women in the community. Patients with breast cancer (6 ladies) were appropriately referred and treated in a tertiary referral hospital. 120 patients had mastalgia, treated with dietary measures and first line drugs evening primrose oil and vitamin E. 1 month telephonic follow-up revealed 60% ladies taught on a average 4 other ladies and was compliant. 2 patients who had abscesses was aspirated. Out of 6 patients with breast cancer 5 were locally advanced breast cancer (1 fungating breast cancer). The average duration of tumor was 34 ± 12 weeks. Other cancers like thyroid cancer and oral cancer were detected and treated appropriately. Conclusion: Technology with the intent to serve the rural community should be the motto to provide improved health care to the neglected rural women.

Abstract P4-17-05: A school-based breast health (BH) educational program to increase breast cancer awareness in a rural Mexican community: A qualitative comparative analysis of students', relatives' and teachers' perceptions

Abstract Background: In order to increase breast cancer awareness, we implemented a school-based educational program for female adolescents in a rural Mexican community, aimed at increasing knowledge and promoting intergenerational transmission of information to their female relatives. Here we report the perspective of the students, their female relatives and the school teachers on the program's impact on the adolescents and their community, as well as the major emergent themes related its implementation. Methods: Adolescents enrolled in a public rural Mexican middle school were invited to participate in the program. They received age-appropriate, culturally sensitive sessions from expert educators focused on transmitting BH knowledge in their household. The students' opinions of the program were evaluated using open ended questions immediately after its completion. The opinion of the students' female family members and of the school teachers were recorded four months after the intervention using open ended surveys and semi-structured interviews. The interviews were conducted at the school by two oncologists who designed and implemented the program and were recorded in video and audio formats. Data was coded using a narrative research approach, and a constructivist paradigm was used to explore emerging themes. NVivo software was used for qualitative data analysis. Results: The surveys were answered by 126 students, 185 family members and 18 teachers. Seven teachers then participated in semi-structured interviews. The following major themes were identified as related with the BH educational program by students: (1) the importance of breast self-examination; (2) the power obtained through knowledge and learning; and (3) the importance of disseminating the obtained information. The following major themes were identified by female relatives: (1) the importance of disease prevention; (2) knowledge to perform self-examination and seek medical attention and (3) empowerment through information. An analysis of the teachers' opinions identified the following major themes: (1) the program aided in discussing intimate topics; (2) the relevance of disseminating information in the community; (3) training other teachers and survivors for implementation; and (4) the importance of the acquisition of new knowledge. The relevance of obtaining and disseminating new knowledge was a common emerging theme among the three groups. Interestingly, both the students and their relatives highlighted the power of knowledge in empowering women and allowing them to prevent diseases and take care of their own health. Conclusions: Obtaining feedback from the relevant stakeholders and promoting active participation of the community is essential for the implementation of successful school-based education programs in cancer. We identified relevant themes related to BH education which could be useful for designing and implementing future programs. Providing adolescents from underserved populations with new knowledge and tools to take care of their health can empower them and aid them not only in preventing disease but in disseminating that knowledge in their communities. “Effectiveness of Planned Teaching Programme on Knowledge of Early Detection of Breast Cancer Among School Teachers”

Back ground of the study: Medical advances have shown that one-third of all cancers are preventable and further one third, if diagnosed sufficiently early, is potentially curable. This observation demands that cancer control should be of increasing priority in the healthcare programs of developing countries. One potentially important strategy in reducing breast cancer mortality is the use of screening to achieve earlier detection of cancer. The main methods of screening involve mammography, clinical breast examination (CBE), and breast self-examination (BSE). Despite the advent of modern screening methods, more than 90% of cases of cancers of the breast are detected by women themselves, stressing the importance of breast self examination. 11 Planned teaching program is one of the methods through which awareness can be created among women regarding early detection of breast cancer. Methodology: The aim of the study was to assess the knowledge of early detection of breast cancer among school teachers. An evaluative approach with pre-experimental (one group pre-test post-test) design was used for the study. The sample consisted of 60 school teachers selected by purposive sampling technique. Pre-test was conducted by administering a structured knowledge questionnaire. After the pre-test, the PTP was administered and on the eight day post test was conducted. The collected data was analyzed by using descriptive and inferential statistics (‘t’ test). Results: The mean post-test knowledge score (O2 =24.05) was higher than the mean pre-test knowledge score (O1=12.48). The computed‘t’ value (24.14) was higher than the table value (t (59) = 1.67) at 0.05 level of significance, suggesting that the PTP was effective in increasing the knowledge of female teachers on early detection of breast cancer. Interpretation: The result showed that the PTP was effective in increasing the level of knowledge of teachers on early detection of breast cancer. Conclusion: The findings of the study showed that the knowledge of teachers was average before the administration of the PTP. The post-test knowledge scores showed significant increase in the level of knowledge of teachers. Hence the PTP is an effective teaching method for providing information and improving the knowledge of teachers.

Practice of breast self-examination among rural women in Umuowa, Orlu government area, Imo state, Nigeria

Background: Regular practice of breast self- examination is the corner stone of the fight against breast cancer. Increasing breast health awareness would potentially allow for early detection of any anomalies and consequently result in early prognosis. This will in turn lead to increase in survival rate of breast cancer. This study was designed to assess practice of breast self- examination (BSE) among rural women. Methods: Descriptive survey design with the aid of a questionnaire which served as an interview schedule was conducted among 349 women in Umuowa Orlu L.G.A. The main outcome measures were the level of awareness and practice of BSE. Results: From a total of 349 women, a little above half (50%) of the respondents understood the meaning of BSE. Few of the respondents knew that BSE increases survival rate of breast cancer by early detection of abnormalities. Majority of the respondents did not know the abnormalities to check during BSE. Only a few of the respondents knew that BSE should be performed by every woman. Generally, the practice of BSE was inadequate for majority of the participants. There was a positive relationship between the respondents’ awareness of breast cancer and their practice of BSE (P-value = 0.000<0.05). Conclusions: The importance of BSE as a key factor in the early detection of breast cancer should be stressed with special emphasis on regular practice of BSE. The proper procedure of BSE should be taught to women in order to ensure accuracy of performance.

Assessing the Knowledge and Effect of Planned Teaching Regarding Breast Cancer and Breast Self-Examination (BSE) among Perimenopausal Women in Selected Urban Community of Mumbai

According to the American Cancer Society, more than 1, 93,000 cases of breast cancer are diagnosed every year with an estimated 40,000 deaths. It is thus very important that women themselves are made aware about the importance of breast self-examination, which is possible through adequate information and motivation by the health care provider itself. With the promise that the incidence of breast cancer is preventable and curable with proper awareness through nurses, an evaluator approach pre-test post-test study involving 50 perimenopausal womens was undertaken. The findings proved that planned teaching can improve the knowledge of the women to a great extent and improve the practice of breast self-examination. Thus, it is possible to reduce the breast cancer-related mortality and morbidity rate through self-examination of the breast.

Self-Care Education Programs Based on a Trans-Theoretical Model in Women Referring to Health Centers: Breast Self-Examination Behavior in Iran

Background:Breast cancer is one of the most common cancers and a major public health problem in developing countries. However, early detection and treatment may be achieved by breast self-examination (BSE). Despite the importance of BSE in reducing the incidence of breast cancer and esultant deaths, the disease continues to be the most common cause of cancer death among women in Iran. This study aimed to determine the effects of self-care education on performance of BSE among women referring to health centers in our country.Materials and Methods:This quasi-experimental interventional study with pretest/posttest control group design was conducted on 168 women referred to health centers. The data were collected using a validated researcher-made questionnaire including demographic variables and trans-theoretical model constructs as well as a checklist assessing BSE behavior. The instruments were administered to groups with and without self-care education before, a week after, and 10 weeks after the intervention. Then, the data were entered into the SPSS statistical software (version 19) and analyzed using independent sample t-tests, paired sample t-test, repeated measures ANOVA, Chi-square, and Friedman tests (p<0.05).Results:The results showed an increase in the intervention group’s mean scores of trans-theoretical model constructs (stages of change, self-efficacy, decisional balance, and processes of change) and BSE behavior compared to the control group (p<0.001).Conclusion:The study confirmed the effectiveness of aneducational intervention based ona trans-theoretical model in performing BSE. Therefore, designing educational interventions based on this model is recommended to improve women’s health and reduce deaths due to breast cancer.
