A Post-Implanto-Prosthetic Rehabilitation Study Regarding the Degree of Improvement in Patients' Quality of Life: A Before-After Study.

Implant-prosthetic rehabilitation has the ability to improve the quality of life of patients, because, in addition to the role of restoring masticatory function, they also have many other benefits, such as restoring aesthetics or improving speech. This study aimed to analyze whether patients' quality of life was improved by implanto-prosthetic rehabilitation and which were the most important aspects. In this before-after study, we applied the Oral Health Impact Profile (OHIP-14) questionnaire to analyze the degree to which complex implanto-prosthetic rehabilitation led or not to an increase in patients' quality of life. The present study was carried out at the level of a private medical center in a city in the central region of Romania and included patients who visited this medical center between January and June 2022 and who benefited from a complex implanto-prosthetic rehabilitation, with the total number of patients eligible for inclusion in the study being 116. Overall, an improvement in quality of life after implant-prosthetic rehabilitation was found. Patients' gender, age, or educational level did not significantly influence their responses. The network analysis offered an overview (intuitive visual representation) of the similarities but also the differences in the OHIP-14 item relationships in both situations: before and after oral rehabilitation. A better understanding of how patients perceive implanto-prosthetic rehabilitation and the aspects that influence this perception can lead to an improvement in their quality of life, increasing the addressability of this type of medical procedure.

Dental and Implantoprosthetic Rehabilitation of the Vertical Dimension in an Elderly Patient with Severe Parafunction

Tooth wear can have several etiological factors. When they occur in combination, the clinical result can be quite devastating, with severe tooth destruction. The purpose of this article is to present the case of a elderly patient with severe tooth wear caused by parafunctional habits (bruxism), with a marked reduction in the vertical dimension of the lower third of the face. Dental and implantoprosthetic rehabilitation was carried out, restoring the vertical dimension.

Guided Bone Regeneration with BoneHealTM Membrane after Explantation of Fractured Implant: Case Report

Implantoprosthetic rehabilitation has become the main method to reestablish totally or partially edentulous patients, with a high success rate. However, some complications can cause implant loss. The main one is periimplantitis, with bone loss around the osseointegrated implant. However, implant fractures or incorrect implant position may compromise implant rehabilitation and require implant removal. Several techniques can be used for implant explantation, and the dental surgeon should opt for the easiest and most conservative technique, ensuring the greatest preservation of remaining bone tissue and aiming at the future reinstallation of another osseointegrated implant. Sometimes, associated Guided Bone Regeneration techniques may be necessary in conjunction with future implant installation, using various procedures or biomaterials. The purpose of this article is to present the case of a patient who underwent explantation of a fractured implant. A polypropylene membrane (BoneHealTM) was used and an implant was reinstalled 8 months after explantation.

The importance of implant-supported overdentures in a bimaxillary complete edentulous patient – case report

Abstract Introduction. Complete edentulism is escorted by various comorbidities, which affect an individual. The management of edentulous patients was approached since the early days of dentistry. The aim of this case report was to present the implanto-prosthetic rehabilitation of a bimaxillary complete edentulous patient with implant-supported overdentures, by using Straumann dental implants. Case presentation. This case report presents the applied treatment to a bimaxillary complete edentulous patient, where the predictable osseointegration and implant stability after the placement of implants was possible, with the purpose to support the overdentures. Conclusions. The oral rehabilitation of complete bimaxillary edentulism through implant-supported overdentures is a procedure with a predictive treatment that presents beneficial aspects in the quality of life of the patients.

Implantoprosthetic rehabilitation of a patient with severe form of hemophilia B: a case report

AbstractThe preparation of patients with hemophilia before surgical operations and dental procedures constitutes a significant clinical challenge. This article presents the implantoprosthetic rehabilitation of a patient with severe hemophilia B (factor IX activity <1%). The patient was prepared for the surgical procedure with recombinant factor IX concentrate (Rixubis) during the clinical surgery study. Tooth extraction and the implantation of four dental implants in the mandible were planned: one dental implant of 3.7 mm diameter and 10 mm length in the place of tooth 35, and another of 3.2 mm diameter and 10 mm length in the place of tooth 37. The next two implants were implemented 1 month later: one implant 3.7 mm in diameter and 10 mm in length in the place of tooth 46, and another implant 3.2 mm in diameter and 10 mm in length in the area of tooth 44. Appropriate substitution of the missing coagulation factor, together with the use of local hemostatic therapy, allowed dental implantation to be performed without excessive blood loss in this patient with severe hemophilia B.

Interrelations Between Anesthetic Substances, General Condition of the Patient and Implant Rehabilitation Procedure

The purpose of this study is to quantify the existing interrelationship between the anesthetic substances used in the implanto-prosthetic rehabilitation procedures with the general state of the patient, as well as the specific implant rehabilitation work.The analyzed lot brought together 110 patients, candidates for implanto-prosthetic rehabilitation, subjected to a general and local evaluation protocol for choosing general anesthesia in accordance with the general state and complexity of the implanto-prosthetic algorithm. Regarding connection between therapeutic approaches in oral implantation and types of anesthesia , we obtained correlative aspects between the use of inhalosedation and implantation algorithm in 80% clinical cases and augmentation with implantation was found in 20%.General anesthesia with Midazolam has been correlated with implantation and augmentation in 77% clinical cases and sinus lift and implantation was found in 23% clinical cases.General anesthesia with Midazolam was used in 64% clinical cases with implantation and sinus lift and 36% clinical cases with implantation.Careful monitoring of the patient and proper dental surgery where the intervention is performed are the key for a therapeutic success regarding sedation and anesthesia.

Non-Invasive Implanto Prosthetic Rehabilitation in the Lower Arch Subsequent to Ameloblastoma Removal. A Case Report

Aim: Describing the surgical/therapeutic and the prosthetic/functional approaches for the treatment and rehabilitation of a patient with an ameloblastoma in the posterior area of mandible. Materials and Methods: Enucleation of the lesion took place by surgical resection with bone margin extending to more than 1 cm beyond the lesion's macroscopic margins, without performing guided bone regeneration. A year after surgery, a panoramic radiograph (OPG) was performed and seven implants were inserted to rehabilitate the lower arch. The upper arch was rehabilitated with implant-prosthetic full-arch structure (flat-one-bridge) and immediate functional loading within 72 hours. Results and Conclusions: A 30 month follow-up shows good peri-implant tropism with bone margins continuity and osteointegration to the bone previously compromised by the lesion. The non-invasive treatment of lesions of maxillary bones is very useful to maintain the interarch occlusal relationship since it accelerates bone healing and optimizes the soft tissues morphology.

Long term follow-up in inferior alveolar nerve transposition: our experience.

Introduction. Inferior alveolar nerve transposition (IANT) is a surgical technique used in implantoprosthetic rehabilitation of the atrophic lower jaw which has not been well embraced because of the high risk of damage to the inferior alveolar nerve (IAN). There are cases in which this method is essential to obtain good morphologic and functional rebalancing of the jaw. In this paper, the authors present their experience with IANT, analyzing the various situations in which IANT is the only surgical preprosthetic option. Methods. Between 2003 and 2011, 35 patients underwent surgical IANT at our center. Thermal and physical sensitivity were evaluated in each patient during follow-up. The follow-up ranged from 14 to 101 months. Results and Conclusion. Based on our experience, absolute indications of IANT are as follows: (1) class IV, V, or VI of Cawood and Howell with extrusion of the antagonist tooth and reduced prosthetic free space; (2) class V or VI of Cawood and Howell with presence of interforaminal teeth; (3) class V or VI of Cawood and Howell if patient desires fast implantoprosthetic rehabilitation with predictable outcomes; (4) class VI of Cawood and Howell when mandibular height increase with inlay grafts is advisable.

Mechanical resistance of zirconium implant abutments: A review of the literature

The increase of aesthetic demands, together with the successful outcome of current implants, has renewed interest in the search for new materials with enough mechanical properties and better aesthetic qualities than the materials customarily used in implanto-prosthetic rehabilitation. Among these materials, zirconium has been used in different types of implants, including prosthetic abutments. The aim of the present review is to analyse current scientific evidence supporting the use of this material for the above mentioned purposes. We carried out the review of the literature published in the last ten years (2000 through 2010) of in vitro trials of dynamic and static loading of zirconium abutments found in the databases of Medline and Cochrane using the key words zirconium abutment, fracture resistance, fracture strength, cyclic loading. Although we have found a wide variability of values among the different studies, abutments show favourable clinical behaviour for the rehabilitation of single implants in the anterior area. Such variability may be explained by the difficulty to simulate daily mastication under in vitro conditions. The clinical evidence, as found in our study, does not recommend the use of implanto-prosthetic zirconium abutments in the molar area. Morphofunctional and clinical study on mandibular alveolar distraction osteogenesis.

Alveolar Distraction Osteogenesis (ADO) is a process which forms new alveolar bone to correct alveolar deformities in ridge height and width. This work aims (a). to verify the predictability of the augmentation of height of atrophic alveolar ridges using an extra-alveolar distraction device and (b). to study the bone processes in order to optimize implanto-prosthetic rehabilitation. ADO was performed on 10 patients with ridge deformities to obtain the required ridge augmentation. Clinical and radiological (OPT and CT with densitometric assay) evaluations were carried out during the following 12 weeks, before implant insertion. Biopsies at 40, 60 and 88 days were studied after general, specific and histochemical staining of slides; microradiographs were analyzed to evaluate the Trabecular Bone Volume. Forty days after the end of distraction, soft callus indicated the start of ossification. Sixty days after the end of distraction, the soft callus was largely converted into a network of trabecular woven bone; osteogenic activity was high and TBV was about 50%. Eighty-eight days after the end of distraction, the amount of bone appeared reduced, with a more ordered structure; bone formation activity and TBV also diminished, whereas osteoclast erosion was active. The densitometric assay shows values increasing from the end of distraction, particularly after implant insertion. Histological results show a regression in bone deposition processes 88 days after the end of distraction culminating in a virtual steady-state after a certain time. The results suggest that early implant insertion may be desirable to avoid bone loss due to mechanical unloading.
