A 58-year-old woman previously high functioning, independent, occupationally active with no psychiatric history and no family history of mental health disorders, was admitted to our Psychiatric Hospitalization Unit in August 2020 with her first depressive episode, presenting intense anguish, regressive behavior, restlessness, and continuous crying. Her son describes the beginning of symptoms after a complicated divorce, approximately two months earlier, with depressed mood, feelings of hopeless, decrease of appetite, insomnia and early-morning awakening, as well as continuous ruminations about her situation and future, and severe anxiety. Prior to being hospitalized, ambulatory treatment was tried, receiving lorazepam 3 mg daily and olanzapine 5 mg daily with no clinical improvement. Furthermore, sertraline 50 mg daily was initiated a few weeks earlier.
