Fragmentation within the healthcare system is a challenge to continuous care provision internationally. Patients with age-associated and chronic diseases with complex care requirements often face barriers to managing their care. Patient navigation interventions aim to support patients with their individual care coordination needs. Here, we investigate the feasibility and efficacy of a patient-oriented navigation program for stroke and lung cancer patients. Results of two randomized controlled trials that were part of a mixed methods study, are reported. 170 stroke patients and 64 lung cancer patients were randomized. The intervention group received the personal navigation for one year. The control group received a brochure with regional support offers. Two primary feasibility criteria were analyzed: uptake of initial navigation meetings (>70%) and drop-out of intervention (<40%). Further, we investigated the program's efficacy regarding satisfaction and trust with medical care. Both primary feasibility criteria were met as 96.9% (95%CI: 90.8%-100.0%) of lung cancer patients and 86.9% (95%CI: 79.9%-94.1%) of stroke patients of the intervention group received a first navigation meeting and only 10.5% (95%CI: 0%-24.3%) of lung cancer patients and 8.6% (7 of 81 patients, 95%CI: 2.5%-14.8%) of stroke patients dropped out of the study. Overall, no significantly higher satisfaction levels with medical care were detected for the intervention group with control group as reference (lung cancer: OR (95%CI): 1.40 (0.44-4.46); stroke: OR (95%CI): 1.16 (0.64-2.11), values > 1 indicate higher satisfaction in intervention group). In exploratory subgroup analyses, stroke patients without partner in the intervention group reported higher levels of trust with medical care compared to controls (OR (95%CI): 3.53 (1.14-10.09)). Our findings suggest feasibility of a patient-oriented navigation program focused on care coordination and support with social care questions regarding its uptake. Identifying patient subgroups with higher navigational need should be focussed on for efficient program implementation.

Neoadjuvant chemotherapy (NAC) represents a cornerstone in the management of locally advanced gastric cancer (GC); however, variability in therapeutic effect underscores the need for additional approaches to support post-operative risk stratification. The present study investigated circulating microRNA (miRNA)-30a as a potential post-treatment, response-associated indicator of therapeutic outcome in patients with GC undergoing NAC. Circulating miRNA-30a expression was measured in serum samples using quantitative real-time polymerase chain reaction (RT-qPCR). NAC-treated patients were stratified into responders (TRG 0-2) and non-responders (TRG 3-5) according to Mandards criteria, while chemotherapy-naive (CT-naive) patients were included as a control group. Circulating miRNA-30a levels were higher in responders compared to non-responders. Elevated miRNA-30a expression was associated with reduced pathological tumor burden, including lower T stage and absence of nodal involvement. ROC analysis demonstrated strong discriminatory performance for distinguishing treatment response. In both univariate and multivariable logistic regression analyses, increased miRNA-30a expression remained independently associated with treatment response. Furthermore, higher miRNA-30a levels were associated with improved overall survival. In conclusion, circulating miRNA-30a is associated with favorable therapeutic outcome in GC and may complement conventional assessment strategies in the post-treatment setting as a response-associated indicator. Further validation in larger, prospective cohorts is needed.

Brain-derived neurotrophic factor (BDNF) is a core biomarker involved in the brain-muscle axis. This study aimed to investigate the associations of plasma BDNF with sarcopenia and multidimensional gait characteristics in older adults. We enrolled 646 adults aged 60 years and older (mean age = 70.46 years; 56.97% females), among whom 65 (10.06%) were diagnosed with sarcopenia according to the Asian Working Group for Sarcopenia 2019. All participants underwent body composition analysis, comprehensive physical function assessments (handgrip strength, 6-m walk and five-time sit-to-stand test), and wearable sensor-based gait evaluation (including 14 parameters). Plasma levels of BDNF, along with multiple inflammatory and oxidative stress markers, were quantitatively measured. Multivariable regressions were conducted to examine the association of BDNF with sarcopenia and gait parameters, with adjustments for demographic characteristics, lifestyle factors, activities of daily living, cognitive function, depression and comorbidities. Plasma BDNF levels were significantly lower in older adults with sarcopenia compared to those without [1.82 (0.62, 4.95) μg/L versus 4.12 (0.81, 11.46) μg/L, p = 0.003]. BDNF was significantly correlated with appendicular skeletal muscle mass index (r = 0.11, p = 0.018), handgrip strength (r = 0.27, p < 0.001), gait speed (r = 0.32, p < 0.001) and sit-to-stand time (r = -0.34, p < 0.001). After full adjustment for covariates, the odds ratio (OR) for sarcopenia of ln-BDNF was 0.71 (95% CI: 0.63-0.81, p = 0.012). Ln-BDNF was negatively associated with swing time (β = -7.833, p = 0.011), step time (β = -12.769, p = 0.016), and stride time (β = -0.026, p = 0.012); and positively associated with thigh acceleration (β = 0.066, p = 0.001), thigh swing work (β = 0.039, p < 0.001), ground reaction force (β = 0.083, p < 0.001), foot landing control force (β = 0.224, p < 0.001), toe-off angle (β = 2.061, p < 0.001), step frequency (β = 1.615, p < 0.001) and stride length (β = 0.013, p < 0.001). Interleukin-6, interleukin-1β, superoxide dismutase and glutathione reductase were concurrently correlated with BDNF levels, sarcopenia status and gait parameters. Mediation analysis demonstrated that interleukin-1β mediated 14.65% of the relationship between BDNF and sarcopenia. Higher BDNF levels are associated with a lower prevalence of sarcopenia and superior kinetic and spatiotemporal gait performance in older adults. Future experimental studies are needed to explore the potential mechanism by which BDNF regulates sarcopenia via neuroinflammation.
