Patients with small cell lung cancer (SCLC) may develop heart failure after chemotherapy; therefore, it is crucial to explore the factors that affect the occurrence of heart failure. The present study retrospectively collected data from 550 patients with SCLC who received chemotherapy. Analysis of differences between groups (patients with and without heart failure, as well as patients with different severities of heart failure) was performed on demographic characteristics and cardiac biomarkers, followed by multivariate and multinomial logistic regression analyses on factors that were found to be significantly different in the intergroup analysis, to screen for factors that significantly influence heart failure. Finally, receiver operating characteristic (ROC) curve analysis was performed to assess the predictive ability of combined indicators for the occurrence and severity of heart failure. Multivariate logistic regression analysis initially identified the cumulative dose of chemotherapy drugs, cardiac troponin I (cTnI), N-terminal pro-B-type natriuretic peptide, left ventricular ejection fraction (LVEF) and creatine kinase-myocardial band as independent prognostic factors of heart failure. Multinomial logistic regression analysis showed that both cTnI and LVEF had significant effects in predicting the severity of heart failure. ROC curve analysis demonstrated that a cTnI level ≥0.044 ng/ml and LVEF ≤39.083% could predict the occurrence of heart failure, whereas a cTnI level ≥0.059 ng/ml and LVEF ≤37.216% were associated with moderate-to-severe heart failure. The standardized combination indicators (cTnI and LVEF) also showed strong predictive ability (AUC >0.75). In conclusion, the present study demonstrated that cTnI, LVEF and their combination may serve as independent prognostic factors for predicting the occurrence and severity of heart failure in patients with SCLC after chemotherapy. These findings suggest that cTnI and LVEF should be routinely monitored during the early stages of chemotherapy for identifying patients at high risk for heart failure and early intervention, ultimately improving patient prognosis.

Heart failure (HF) is a major public health challenge, contributing to high morbidity, mortality, and significant economic burden worldwide. Early diagnosis and accurate risk stratification are critical for improving patient outcomes and guiding clinical decisions. N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a well-established blood biomarker of ventricular overload, widely used for diagnosing HF and assessing its severity. Beyond diagnosis, NT-proBNP provides valuable prognostic insights into treatment response and can predict adverse cardiovascular events, such as HF-related hospitalizations and mortality, thus playing a key role in personalized care and therapeutic management. Despite its clinical significance, current CE/FDA-certified methods for NT-proBNP measurement face critical limitations, particularly in point-of-care settings and for ongoing, minimally or non-invasive monitoring. These constraints hinder the ability to perform remote (e.g. community medical clinic) monitoring-a crucial capability for revolutionizing heart failure management. To address this challenge, we report the development and proof-of-concept validation of an ultrasensitive (compared to gold standard enzyme-linked immunosorbent assays), miniaturized electrochemical biosensor for fast and minute sample detection of NT-proBNP in tears from a well-characterized cohort of heart failure patients. Correlation analysis of NT-proBNP concentrations in tear fluid and blood plasma/serum, together with key clinical parameters, demonstrates that tear fluid is a promising non-invasive matrix for heart failure diagnosis and continuous monitoring in point-of-care settings. This approach provides a transformative pathway to advance heart failure management by enabling remote, point-of-care diagnostic monitoring outside of centralized hospitals and laboratories.

Atrial fibrillation (AF) has been associated with cognitive impairment. Impaired cerebral perfusion is a proposed mechanism, though evidence remains inconclusive, particularly in patients with concomitant heart failure (HF). We conducted a systematic review in accordance with PRISMA 2020 guidelines. PubMed and EMBASE were searched from database inception to May 9, 2025 for adult studies comparing cerebral perfusion in AF and sinus rhythm (SR). Risk of bias was assessed using ROBINS-E and ROBINS-I; certainty of evidence was graded using GRADE. Twenty-three studies were included (11 interventional, 1 observational, 11 cross-sectional). Measurement methods and effect sizes were heterogeneous. Most studies had serious/critical risk of bias, and certainty of evidence was low. Twenty-two studies concluded that AF may impair cerebral perfusion, while one found no such effect. Nine studies reported cognitive testing, and five demonstrated impaired performance in AF or improvement following SR restoration. In HF subgroup analyses (2 studies), perfusion was lower in HF patients with AF than those in SR. A third study found higher pulsatile index in HF patients with AF than those without HF, but no difference in peak, mean or diastolic cerebral blood flow velocities. Current clinical evidence suggests that AF may impair cerebral perfusion and contribute to cognitive decline. However, methodological limitations and heterogeneity limit the conclusions, and further longitudinal studies with standardized perfusion metrics are needed, including in concomitant HF where data are scarce.

Right ventricular dysfunction is a critical predictor of mortality, often progressing to right heart failure, contributing to 3%-9% of heart failure admissions. Acute right heart failure is the primary cause of death in conditions such as right ventricular myocardial infarction and massive pulmonary embolism. Mechanical circulatory support with right ventricular assist devices (RVADs) has become essential in managing both acute and chronic right heart failure. Such devices provide hemodynamic support by assisting the failing right ventricle. This review outlines the mechanisms, clinical indications, and complications of these RVADs. Chest radiography and echocardiography are the primary modalities used for confirming the correct positioning of these devices, and CT is useful for evaluating intra- and extracardiac complications. These imaging modalities are vital for identifying complications and ensuring optimal device performance. As the use of RVADs grows, radiology's role in optimizing outcomes will continue to expand.
