Study Protocol: Autologous Platelet-Rich Plasma (PRP) Therapy in the Treatment of Pyoderma Gangrenosum: A Prospective, Open-Label, Randomized, Split-Ulcer Trial to Evaluate the Efficacy of Platelet-Rich Plasma Therapy in the Treatment of Chronic Pyoderma Gangrenosum

BACKGROUND Pyoderma gangrenosum (PG) presents as an ulceration of the lower extremities with deep, undermining, and violaceous borders. The disease is an orphan disease without any FDA-approved treatments. OBJECTIVE This study aims to evaluate the efficacy and safety of intralesional injectable and topical platelet-rich plasma (PRP) therapy in the treatment of chronic pyoderma gangrenosum ulcerations. METHODS Methodologically, this study will be conducted as a prospective, randomized, open-label split-ulcer study in patients with confirmed PG by PARACELSUS score. In each patient, up to three separate ulcerations will be randomized to receive either monthly intralesional PRP injections or a topical PRP solution at 0, 4, 8, and 12 weeks, while the third target lesion will receive standard wound care only. Patients will be followed up with at 16 weeks, one month after the completion of treatment. The primary endpoint for this study will be the composite proportion of the target ulcers achieving either complete resolution or a 50% reduction in the surface area at week 12. CONCLUSIONS This study intends to demonstrate the efficacy of an intralesional injectable or topical PRP therapy in the treatment of PG. If successful, PRP treatment may offer a new and effective option for patients with this debilitating disease.

A Comparative Study of the Efficacy of Platelet-rich Plasma Therapy versus Topical Application of 0.1% Tretinoin Gel in the Management of Striae Gravidarum

Abstract Aim: To determine the relative efficacy of platelet rich plasma therapy versus topical 0.1% tretinoin gel in the management of striae gravidarum. Objectives: To determine the degree of response to platelet rich plasma therapy in comparison with topical 0.1% tretinoin gel in striae gravidarum and to determine the treatment of choice amongst platelet rich plasma and topical 0.1% tretinoin gel. Material and Methods: Study design - A randomised control cross sectional comparative study among the female patients with striae gravidarum was carried out at department of Dermatology, Venereology & Leprosy, Base Hospital, Delhi Cantt, New Delhi. Results: There was early and fast clinical improvement in PRP group with continuous clinical improvement noticed in successive sittings although degree of improvement was lower in 5th and 6th sittings. There was also an earlier plateau of response, in tretinion group after 04 sittings with mild or little improvement in the last 02 sittings in comparison to the PRP group. Conclusion: Platelet Rich Plasma therapy was effective as compared to topical 0.1% Tretinoin gel in the management of Striae gravidarum. PRP had better treatment outcome as well as patient satisfaction.

Predictors of Effectiveness of Platelet-Rich Plasma Therapy for Knee Osteoarthritis: A Retrospective Cohort Study.

There has recently been growing interest worldwide in biological therapies such as platelet-rich plasma injection for the treatment of knee osteoarthritis. However, predicting the effectiveness of platelet-rich plasma therapy remains uncertain. Therefore, this retrospective cohort study was performed to assess a range of predictors for the effectiveness of platelet-rich plasma therapy in treating knee osteoarthritis. The study included 517 consecutive patients who underwent three injections of leucocyte-poor platelet-rich plasma therapy from 2016 to 2019 at a single institution. The treatment outcomes, including patient-oriented outcomes (visual analogue scale score and Knee Injury and Osteoarthritis Outcome Score), were analyzed and compared according to the severity of knee osteoarthritis based on Kellgren–Lawrence (KL) grading using standing plain radiographs. Fisher’s exact test, univariate regression, and multivariate regression were used for data analysis. Patient-oriented outcomes were significantly improved 6 and 12 months after platelet-rich plasma therapy. The overall responder rate in patients who met the Outcome Measures in Rheumatology (OMERACT)–Osteoarthritis Research Society International (OARSI) responder criteria was 62.1%. The responder rate was significantly lower in patients with severe knee osteoarthritis (KL4, 50.9%) than in those with mild (KL2, 75.2%) and moderate (KL3, 66.5%) knee osteoarthritis. The multivariate logistic regression analysis revealed that deterioration of the knee osteoarthritis grade (increased KL grade) was a significant predictor of a worse clinical outcome (odds ratio, 0.58; 95% confidence interval, 0.45–0.75; p < 0.001). The relative risk for non-responders in severe (KL4) KOA was 2.1 (95% CI, 1.5–3.0) at 6 months and 2.3 (1.6–3.2) at 12 months compared with mild-to-moderate (KL2-3) KOA. The efficacy of platelet-rich plasma therapy was not affected by age, sex, body weight, or platelet count. This study revealed that the effectiveness of platelet-rich plasma therapy for the treatment of knee osteoarthritis is approximately 60% and that the effectiveness depends on the severity of knee osteoarthritis. This observation is useful not only for physicians but also for patients with knee osteoarthritis.

Current Evidence on Using Platelet Rich Plasma as a Therapeutic Modality for Veterinary Orthopedic Conditions

This review evaluates the findings of available clinical literature on the use of platelet-rich plasma (PRP) to develop evidence-based recommendations for treating various musculoskeletal issues that arise in veterinary practice. The use of PRP-based treatments for various orthopedic conditions is rapidly evolving as a promising treatment modality; however, its true effectiveness has yet to be elucidated. Application of PRP has been reported in humans for a variety of orthopedic conditions. Although the majority of the veterinary literature on PRP reports its use in equine patients, there is a dearth of evidence addressing its use in canine patients. Nevertheless, evidence of the efficacy of PRP has appeared to be highly variable depending on its specific indication, particularly in musculoskeletal disorders. This review aims to present the available information on the efficacy of PRP therapy in veterinary orthopedic conditions and describes factors influencing its use, the limitations of PRP therapy, and future directions of PRP research and therapy.

A case report on efficacy of platelet-rich plasma therapy in avoiding surgical intervention for trapeziometacarpal joint osteoarthritis

Osteoarthritis of the first carpometacarpal joint also called as trapeziometacarpal joint is the most common of all five carpometacarpal joints to undergo degeneration. The patient suffers with chronic pain, tenderness, stiffness of joint, and sometimes, even deformity can be seen in long-standing cases. Due to limited treatment options, patients have to suffer with side effects of pain killers or may have to undergo surgical intervention, but intra-articular joint injections and platelet-rich plasma therapy have been proven efficacious in providing good pain relief.

Role of Platelet Rich Plasma (PRP) injection in treatment of rotator cuff tear

BackgroundOne of the commonest causes of shoulder pain and disability is partial thickness tear of the rotator cuff. Platelet-rich plasma (PRP) therapy is a volume of the plasma fraction of autologous blood that has platelet concentration above the baseline. It is theorized that PRP may help stimulate the development of normal-appearing histologic tissue characteristics at the repair site and improve clinical outcomes. Our purpose is to assess the efficacy of PRP therapy in treatment of rotator cuff tear. This prospective single-center study involved 30 patients (aged 31.7 ± 7.7 years, 63.3% males) complaining of symptomatic rotator cuff tear. Platelet-rich plasma (PRP) injection was done to all patients. Before and after injection of PRP, scoring was done using visual analog score, Shoulder Pain and Disability Index (SPADI), and musculoskeletal ultrasound scoring of the tear. Reassessment was done 4 weeks after the injections.ResultsThe pre-injection VAS pain score was 7.1 ± 0.99 after injection, and it became 3.7 ± 1.09. Also, SPADI score decreased from 78.03 ± 8.25 pre-injection to 42.1 ± 13.97 post-injection. There was a highly statistically significant difference in VAS pain score and SPADI score before and after PRP injection (p < 0.001). There was highly statistically significant positive correlation between the improvement of US grading score change and the improvement of VAS score change, and between it and SPADI improvement change (r = 0.695, p < 0.001 and r = 0.693, p < 0.001, respectively). There was a highly statistically significant difference between US grading score before and after PRP injection (p < 0.001).ConclusionPRP can be considered an effective method for treatment of rotator cuff tear and less invasive compared to surgical treatment. It improves the pain, shoulder function, and tear size.

The efficacy of platelet-rich plasma therapy in the treatment of androgenetic alopecia: a literature review

Androgenetic alopecia is a genetically determined disease characterized by gradual thinning of hair, which subsequently leads to hair loss, and thereby significantly affects a patients quality of life. The existing methods of disease management have a number of side effects and drawbacks. Thus, new treatment methods combining both high tolerance and efficacy are continuously determined. This current review presents an analysis of the effectiveness of single- and double-spin preparation methods of platelet-rich plasma for the management of patients with androgenetic alopecia.

A short-term comparative analysis of the efficacy of platelet-rich plasma therapy and corticosteroid injection in lateral epicondylitis

Background: Tennis Elbow, a common overuse syndrome, is a tendonitis of the extensor muscles of the forearm mainly due to intratendinous degeneration associated with aging, repetitive movement, and vascular compromise. Autologous platelet-rich plasma (PRP) injections represent a new therapeutic option for chronic tendinopathies including tennis elbow. The purpose of the present study was to assess the therapeutic effect, efficacy, and safety of a single injection of PRP and corticosteroid in improving pain and function in patients suffering from lateral epicondylitis over a short term. Materials and Methods: Sixty patients with lateral epicondylitis were randomized into two groups to receive one injection of PRP or corticosteroid and were followed for 3 months. The two groups were compared as regards pain and functional improvement using the visual analogue scale (VAS) and the patient-rated tennis elbow evaluation (PRTEE) Score, by assessing the patients three times postintervention. Results: VAS scale showed that both modalities were effective in pain reduction. PRTEE score represented variably improved symptoms, i.e., decrease in pain and better function along with improvement in daily activity after 3 months in both groups. Conclusion: As the degree of pain relief achieved by the respective modalities was almost similar, the most important difference between the intervention groups was the duration of their effectiveness. PRP is suggested to be superior in the duration of pain relief when compared to corticosteroids. We can propose that PRP can be administered less often and more conveniently as compared to corticosteroid for lateral epicondylitis.

Comparative study between the efficacy of platelet-rich plasma therapy and corticosteroid injection in the treatment of planter fasciitis

Background: Planter fasciitis is the common cause of heel pain and is the disabling disease in its chronic form. Various treatment modalities are available including medical therapies and surgical procedures. This study was designed to compare the efficacy of PRP therapy with corticosteroid injection in the treatment of chronic planter fasciitis. Methods: Total of 40 patients with chronic plantar fasciitis were included in the study and were divided into 2 groups. In the PRP group (n=20), 3ml PRP preparation was made from the patient’s blood and injected in a single dose. In the steroid group (n=20), a single dose of 2ml (40mg) methylprednisolone was given. Clinical evaluation was made using the American Foot and Ankle Score (AFAS) and the visual analog scale (VAS).Results: Both groups were similar in terms of age, gender and side involvement. Mean preinjection VAS, AOFAS scores in steroid and PRP group were 8.1±1.2, 68.1±9.5 and 7.9±1.2, 67.5±9.2 respectively. Post injection the scores improved in both the groups, with slightly more improvement of scores in PRP group than in steroid group, however no significant difference in scores of VAS and AOFAS could be detected between the two groups.Conclusion: The local injection with steroid or PRP preparation was equally effective in terms of pain and functional outcomes in the treatment of chronic planter fasciitis.Level of evidence: II Prospective randomized clinical trail.

Analysis of the clinical efficacy of platelet-rich plasma therapy in the treatment of patients with isolated facet-syndrome of the lumbar spine

To analyze clinical results of the surgical treatment of patients with isolated facet-syndrome of the lumbar spine using platelet-rich plasma (PRP) therapy for arcuate spine joints. The study included 49 patients, aged 39.5 (34; 45), who, after provocative tests, were diagnosed with isolated facet syndrome and operated on from 2015 to 2018 using PRP therapy. A visual analogue pain scale (VAS), Oswestry questionnaire (ODI), Macnab subjective satisfaction scale, and the presence of complications were used to evaluate clinical efficacy. Dynamic evaluation was made on average 18 months after surgery. PRP therapy is a highly effective method for treatment of patients with isolated facet syndrome caused by degenerative diseases of the facet joints. Clinical efficacy is confirmed by the persistent significant reduction of pain symptoms and restoration of functional status in the early and late postoperative periods with low risks of adverse outcomes.

PRP Therapy.

The need of treating the osteochondral lesions has not been yet met in the clinics. Thanks to being an autologous source of growth factors, interleukins, and other cytokines and relative ease of clinical application, i.e., during a single-step surgical procedure, the use of platelet-rich plasma is of great interest. The high theoretical potential of the role of platelet-rich plasma in the regeneration process of osteochondral lesions is known, and the efficiency needs to be confirmed by high-quality randomized controlled trials for a robust position in the treatments of osteochondral lesions in the clinics.

Utilization of Platelet-Rich Plasma for Musculoskeletal Injuries: An Analysis of Current Treatment Trends in the United States.

Background:Platelet-rich plasma (PRP) has emerged as a popular biologic treatment for musculoskeletal injuries and conditions. Despite numerous investigations on the efficacy of PRP therapy, current utilization of this treatment within the United States is not widely known.Purpose:To investigate the national utilization of PRP, including the incidence and conditions for which it is used in the clinical setting, and to determine the current charges associated with this treatment.Study Design:Descriptive epidemiology study.Methods:Using a national database (PearlDiver) of private insurance billing records, we conducted a comprehensive search using Current Procedural Terminology (CPT) codes to identify patients who received PRP injections over a 2-year period (2010-2011). Associated International Classification of Diseases, 9th Revision (ICD-9) codes were identified to determine the specific conditions the injection was used to treat. The aggregate patient data were analyzed by yearly quarter, practice setting, geographic region, and demographics. PRP therapy charges were calculated and reported as per-patient average charges (PPACs).Results:A total of 2571 patients who received PRP injections were identified; 51% were male and 75% were older than 35 years. The overall incidence ranged from 5.9 to 7.9 per 1000 patients over the study period. PRP was most commonly administered in hospitals (39%) and ambulatory surgical centers (37%) compared with in private offices (26%). The most common conditions treated were knee meniscus/plica disorders, followed by unspecified shoulder conditions, rotator cuff injuries, epicondylitis, and plantar fasciitis. Further evaluation revealed that 25% of all patients received injections for cartilage-related conditions, 25% meniscus, 25% unspecified, 12% tendon, 8% glenoid labrum, and 5% ligament. The PPAC for PRP treatment was US$1755 per injection.Conclusion:Despite a lack of consensus regarding PRP indications and efficacy, we observed widespread application of this treatment for a myriad of musculoskeletal injuries. Most treated patients were older than 35 years, and the most commonly treated conditions included cartilage and meniscus disorders. Given the current controversy surrounding this treatment, further studies are necessary to guide clinicians on the value of this therapy for each clinical diagnosis.
