Au carrefour d’une sociologie de la consommation culturelle, des intermédiaires et de la lecture, cet article examine la manière dont les lecteurs et lectrices de romans se saisissent des différentes formes de prescription auxquelles ils et elles sont exposé·es dans le lieu de la librairie indépendante, dans le but de faire leur choix parmi l’offre éditoriale. À partir d’une enquête par questionnaire et entretiens semi-directifs, il s’agit d’interroger l’influence du travail de prescription mené par les libraires ainsi que du travail éditorial réalisé en amont sur les œuvres et qui se donne à voir dans le péritexte éditorial. Si de nombreux travaux ont déjà démontré l’inégale répartition des pratiques de lecture ainsi que les modalités classantes de la pratique, l’apport de cet article est de montrer que le rapport que les lecteur·rices entretiennent avec les intermédiaires et la façon dont ils et elles mobilisent leur travail de prescription pour se retrouver dans le large éventail de choix qui leur y est offert est une modalité supplémentaire des pratiques de lecture qui reflète également de fortes disparités sociales.

Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) are beneficial in surgical settings, they may however lead to adverse drug reactions including decreased renal function, a risk, which is exacerbated by combination with other nephrotoxics, and particularly when NSAIDs are given as part of a so-called 'Triple Whammy' (TW) with diuretics and renin-angiotensin system blockers. The objective of this study was to identify the prevalence of TW-prescriptions in a surgical inpatient setting and to document the changes in renal function after pharmacist recommendations. A prospective, observational single centre pilot study was performed using a series of eleven weekly Point-Prevalence Analyses (PPA). Adult surgical inpatients were screened for a TW-prescription by a clinical pharmacist, who made one of two recommendations on identification of a TW: for patients with eGFR > 60 ml/min/1.73m² close monitoring of renal function; for patients with eGFR ≤ 60 ml/min/1.73m² discontinuation of NSAID. A TW was identified in 18 of 317 patients (prevalence 5.7%; mean age 75 years). NSAID discontinuation was recommended for 7/18 patients (39%) and implemented for six (33%). In cases where the NSAID was de-prescribed, an improvement in renal function until the time of discharge was observed, whilst in patients with eGFR > 60 ml/min/1.73m² for whom monitoring was recommended eGFR remained stable. TW-prescriptions were found to be a potential problem in the studied group of older surgical inpatients. Clinical pharmacists are well placed to identify patients who are prescribed a TW, and to advise on the management of these patients.

Screening Tool of Older Person's Prescriptions(STOPP)in Frail old patients(STOPP Frail)with limited life expectancy is a list of explicit criteria for potentially inappropriate medication(PIM)using in frail older adults.STOPP Frail criteria were initially created by the experts from Cork university hospital based on the clinical experience and literature appraisal and released in 2017 by an expert panel of Cork university hospital, Ireland.Using Delphi consensus methodology, the panelists ranked their agreement with each criterion on a 5-point Likert scale and provided a written feedback.Twenty-seven criteria with a median Likert response of 4/5(agree/strongly agree)and a 25th centile of ≥4 scores were included in the final list.STOPP Frail may assist physicians in deprescribing medications to reduce the PIM use in frail older adults.
