The proposition currently uses the same technologies to optimize logistics and ensure patient information. Projection of future gastric cancer incidence and health-care service demand by geographic area in Kanagawa, Japan.

Projections of future gastric cancer incidence and the demand for health-care services for gastric cancer patients by geographic area will assist local authorities in determining health-care needs, allocating medical resources, and planning services. This study aims to project the future incidence of gastric cancer, estimate the number of patients per medical institution, and decompose the net changes in cases to assess the impact of population aging by geographic area. Our projections are based on population-based cancer registry data, census data from 2000 to 2020, and the projected population for 2025-2045 in Kanagawa, Japan. We classified Kanagawa into urban, town, outer city, and rural areas based on geographic and population features. The number of medical institutions providing gastric cancer treatment was used to estimate the number of patients per medical institution. We projected a decrease of 25%, 52%, and 5% in gastric cancer cases in towns, outer cities, and rural areas from 2020 to 2045, respectively. However, cases are expected to increase by 9% in urban areas, primarily due to population aging. The annual number of gastric cancer patients per medical institution in urban areas is expected to increase from 54 to 59, while numbers in other areas are predicted to decline from 2020 to 2045. Our long-term projections indicate that the number of older gastric cancer patients will continue to increase in urban areas. While current measures effectively reduce gastric cancer risk, they need to be revised to address the impact of population aging.

Ageing in China: Comparison of Healthcare Modes for the Elderly Population

Abstract: As China strives to meet the challenges of an ageing population, the need for an effective healthcare system for the elderly is becoming increasingly urgent. The traditional family-centred mode of care, deeply rooted in Chinese filial piety, has become inadequate in the face of modern social change. With a rapid ageing population and a rising incidence of chronic diseases, the demand for healthcare services is expected to grow significantly. This article explores the limitations of the current family-centred mode of ageing care, including the strain placed on families and the lack of adequate healthcare resources, especially in rural areas. The article argues for a shift to a community-based caring mode that provides more comprehensive and sustainable solutions by integrating specialised healthcare services in local communities. Comparing the situation in China to the community healthcare system successfully implemented in Japan, this paper highlights the potential benefits of adopting a similar strategy while taking into account China's unique demographic and regional challenges. Finally, the article argues for a more equitable and efficient healthcare system in China to meet the needs of its rapidly ageing population.

A reinforcement learning approach for the online dynamic home health care scheduling problem.

Over recent years, home health care has gained significant attention as an efficient solution to the increasing demand for healthcare services. Home health care scheduling is a challenging problem involving multiple complicated assignments and routing decisions subject to various constraints. The problem becomes even more challenging when considered on a rolling horizon with stochastic patient requests. This paper discusses the Online Dynamic Home Health Care Scheduling Problem (ODHHCSP), in which a home health care agency has to decide whether to accept or reject a patient request and determine the visit schedule and routes in case of acceptance. The objective of the problem is to maximize the number of patients served, given the limited resources. When the agency receives a patient's request, a decision must be made on the spot, which poses many challenges, such as stochastic future requests or a limited time budget for decision-making. In this paper, we model the problem as a Markov decision process and propose a reinforcement learning (RL) approach. The experimental results show that the proposed approach outperforms other algorithms in the literature in terms of solution quality. In addition, a constant runtime of less than 0.001 seconds for each decision makes the approach especially suitable for an online setting like our problem.

Capacity Planning in Paediatric Palliative and Hospice Care: An Italian Case

Although the growing demand for healthcare services has enhanced the occurrence of inefficiencies in care provision, healthcare systems should ensure safe, timely and equitable access to care. In particular, the long waiting lists, the lack of beds, and the shortage of healthcare workforce have highlighted the necessity to effectively manage and strategically plan the available resources. Capacity Planning (CP) contributes to determine what, when, where and how much product or service to deliver, with the goal of analysing and optimizing the use and allocation of human and material resources (e.g., physical spaces, equipment, beds, workforce) based on the demand. CP demonstrates to be extremely relevant for hospital units to schedule appointments, diagnostics, surgeries and admissions. Complex contexts and broader healthcare networks could benefit even more from this approach, helping to coordinate multiple care providers and aligning the available resources with the demand pace. The current research presents the implementation of CP in the context of Paediatric Palliative and Hospice Care (PPHC), delivered in an Italian facility. PPHC provides long-term, interdisciplinary and coordinated care between territorial, ambulatory and hospice services. It encompasses follow-up activities, parental training, end-of-life and respite care. In the Italian case analysed, the shortage of beds affects the quality of care delivered to these patients, in terms of accessibility to the service, effectiveness and timeliness of care. Hence, a CP analysis has been performed to pursue a twofold objective: a) to understand if the current capacity of the facility meets the demand for care; b) to improve productivity through an optimized allocation of resources, particularly the available beds and the healthcare workforce. The expected results involve a more effective use of resources, an increased bed occupancy rate and a reduction in rescheduled admissions due to inadequate planning. Optimizing capacity in this specific setting can serve as a pilot project for other similar contexts, aiming to improve patients’ access to healthcare services, their quality of life and supporting the workforce involved in PPHC. This research offers new insights from both the academic and managerial point of view, providing evidence about the implementation of CP in complex healthcare settings.

New colleague or gimmick hurdle? A user-centric scoping review of the barriers and facilitators of robots in hospitals.

Healthcare systems are confronted with a multitude of challenges, including the imperative to enhance accessibility, efficiency, cost-effectiveness, and the quality of healthcare delivery. These challenges are exacerbated by current healthcare personnel shortages, prospects of future shortfalls, insufficient recruitment efforts, increasing prevalence of chronic diseases, global viral concerns, and ageing populations. To address this escalating demand for healthcare services, healthcare systems are increasingly adopting robotic technology and artificial intelligence (AI), which promise to optimise costs, improve working conditions, and increase the quality of care. This article focuses on deepening our understanding of the barriers and facilitators associated with integrating robotic technologies in hospital environments. To this end, we conducted a scoping literature review to consolidate emerging themes pertaining to the experiences, viewpoints perspectives, and behaviours of hospital employees as professional users of robots in hospitals. Through screening 501 original research articles from Web-of-Science, we identified and reviewed in full-text 40 pertinent user-centric studies of the integration of robots into hospitals. Our review revealed and analysed 14 themes in-depth, of which we identified seven as barriers and seven as facilitators. Through a structuring of the barriers and facilitators, we reveal a notable misalignment between these barriers and facilitators: Finding that organisational aspects are at the core of most barriers, we suggest that future research should investigate the dynamics between hospital employees as professional users and the procedures and workflows of the hospitals as institutions, as well as the ambivalent role of anthropomorphisation of hospital robots, and emerging issues of privacy and confidentiality raised by increasingly communicative robots. Ultimately, this perspective on the integration of robots in hospitals transcends debates on the capabilities and limits of the robotic technology itself, shedding light on the complexity of integrating new technologies into hospital environments and contributing to an understanding of possible futures in healthcare innovation.

This article delves into the intricacies of talent management in healthcare, from an operational standpoint, aiming to capture how the frontline workforce perceives talent management. It is hypothesized that by implementing comprehensive talent management strategies, healthcare institutions would be able to address the current challenges posed by the worldwide workforce shortages. The increasing demand for healthcare services has prompted organizations and governments to proactively approach talent management to attract and retain the right individuals with the right skills to remain competitive and even functional. Employee engagement initiatives, along with a robust culture of continuous learning and phycological safety, where individuals can be at optimal performance and positively impact patient experience and outcome, significantly reduce turnover rates and ensure workforce stability (Dyrbye et al., 2017).

Are random price generators useful for health policy processes and analysis? effectiveness of random price generators in health policy processes and analysis

This paper discusses why using random price generators in an economic model for medical markets may be useful. It summarizes the different steps for the development of the model and describes an original contribution of the physicians’ choice model with random drug prices applied to Type 2 diabetes in the US. Pharmaceutical markets have been supply-driven to boost life science and medical technologies; however, with the widening inequalities inside national health systems and the global agenda for universal health coverage, more economic research is being done to strengthen the analysis of demand for healthcare services. Research, especially, examines more disaggregated levels of demand systems to understand the heterogeneity of physicians’ choices and better capture patient needs. It can also be an approach to calibrate supply and demand adjustments in medical markets. This paper argues that choice modeling is particularly relevant, using random price generators in a structural model where a demand approach is useful. If validated by additional experimental studies, this first study by Professors Huttin & Hausman (2021) could be used in advanced value assessment frameworks. Random price generators on medical markets could also be tested with additional models that fit oligopolistic market structures (e.g., models for differentiated product markets such as the Berry–Levinsohn–Pakes model). It may also help with a policy analysis process that addresses major disruptive transformations of market dynamics, evolving boundaries in science, fast digitalization, and artificial intelligence-based information systems.

The Economic Impact of Post-Traumatic Stress Disorder Among Patients with Systemic Autoimmune Diseases During the COVID-19 Pandemic.

The present work aimed to estimate the economic impact of PTSD following COVID-19 in a population of patients affected by systemic autoimmune disease (SAD) using a cost-of-illness approach and accounting for the perspective of society. Considering data collected from SAD patients enrolled in a specialized outpatient clinic in the Tuscany region, Italy, generalized linear models and LASSO logistic regression were used to evaluate the impact of PTSD on costs and its relevance as a possible predictor of being a high-cost patient, respectively. Considering 301 SAD patients, 161 (51.2%) of whom were diagnosed with PTSD, the overall costs were EUR 3670 [890; 40,529] per patient/year among patients with PTSD and EUR 2736.7 [283; 21,078] per patient/year among those without PTSD (p-value < 0.001), with differences mainly attributable to significantly greater direct non-healthcare and indirect costs. PTSD was estimated to increase overall costs (β = 0.296 (0.140), p-value = 0.035), direct non-healthcare (β = 1.193 (0.392), p-value = 0.002), and indirect costs (β = 3.741 (1.136), p-value = 0.001). PTSD diagnosis was also significantly associated with the likelihood of being a high-cost patient. Findings from the present study offer a novel perspective on the economic impact of COVID-19 and provide valuable data for policymakers to better understand the demand for healthcare services and associated costs.

Increased Emergency Department Medical Imaging: Association with Short-Term Exposures to Ambient Heat and Particulate Air Pollution.

Background Climate change adversely affects human health, resulting in higher demand for health care services. However, the impact of climate-related environmental exposures on medical imaging utilization is currently unknown. Purpose To determine associations of short-term exposures to ambient heat and particulate air pollution with utilization of emergency department medical imaging. Materials and Methods In this retrospective time-stratified case-crossover study, daily imaging utilization counts from four emergency departments were linked to local daily environmental data-including fine particulate matter with 2.5-µm or smaller aerodynamic diameter (PM2.5) and ambient temperature-over 10 years (January 2013 to December 2022). Conditional Poisson regression models were used to evaluate the associations between daily imaging utilization and environmental exposures on the same day and each of the 7 days preceding imaging, lag days 0-7, controlling for day of the week, month, and year. Moving averages of mean daily PM2.5 and temperature were calculated to account for lagged exposure effects. Imaging counts were also stratified by modality (CT, radiography, US, and MRI). Results In an analysis of 1 666 420 emergency department imaging studies, a rise of 10 °C in the 2-day moving average of mean daily temperature and a rise of 10 μg/m3 in the 3-day moving average of mean daily PM2.5 were associated with overall imaging utilization increases of 5.1% (incidence rate ratio [IRR], 1.051; 95% CI: 1.045, 1.056) and 4.0% (IRR, 1.040; 95% CI: 1.035, 1.046), respectively. Heat exposure days (mean temperature >20 °C) and air pollution exposure days (mean PM2.5 >12 μg/m3) were associated with same-day excess absolute risk of 5.5 and 6.4 imaging studies per 1 million people at risk per day, respectively. Heat exposure days and air pollution exposure days were associated with increased utilization of radiography (excess relative risk, 2.7% [P < .001] and 2.1% [P < .001], respectively) and CT (excess relative risk, 2.0% [P = .001] and 2.7% [P < .001]) but not US (P = .14 and P = .14) or MRI (P = .70 and P = .65). Conclusion Short-term exposures to ambient heat and particulate air pollution were associated with increased utilization of radiography and CT but not US or MRI. © RSNA, 2024 Supplemental material is available for this article. Integrating Smart Computility for Subflow Orchestration in Remote Virtual Services.

The burgeoning domain of the metaverse has sparked significant interest from a diverse array of industries, including healthcare services. However, the metaverse and its associated applications present various challenges to existing networks. First, to meet the increasing demands of the metaverse, there is a need for enhanced bandwidth, reduced latency, and improved packet loss control. Furthermore, the transmission mechanism should exhibit flexibility to automatically adapt to the diverse hybrid needs of different healthcare services. In this article, a multipath transmission-based paradigm tailored for the metaverse-based healthcare services is developed. Significantly, we devise an orchestration framework to reconcile edge-side subflow management with diverse healthcare applications. Using machine learning techniques, the framework can produce near-optimal subflow adjustment strategies for client nodes and miscellaneous services. Comprehensive experiments are performed on applications with diverse requirements to validate the adaptability of the framework to the application needs. The experimental results demonstrate that the proposed method enables the network to autonomously adapt to changing network conditions and service requirements. This includes applications' preferences for high throughput, low delay, and high stability. Moreover, the test results show that the proposed approach can notably decrease the occurrences of network quality falling below the minimum requirement. Given its adaptability and impact on network quality, this work paves the way for future metaverse-based healthcare services.

Interrelationships Among Individual Factors, Family Factors, and Quality of Life in Older Chinese Adults: Cross-Sectional Study Using Structural Equation Modeling.

China's rapidly aging population necessitates effective strategies for ensuring older adults' quality of life (QOL). While individual factors (IF) and family factors (FF) are known to influence QOL, existing research often examines these factors in isolation or focuses on specific subpopulations, overlooking potential interactions and mediating pathways. This study aims to examine both direct and indirect pathways connecting IF and FF to older adults' QOL, focusing on the mediating roles of health risks (HR) and health care service demand (HSD). This study uses structural equation modeling (SEM) to analyze cross-sectional data from 8600 older participants in the 2015 China Health and Retirement Longitudinal Study (CHARLS), a nationally representative study using a multistage probability proportional to size sampling method. Among the 8600 participants, the majority (5586/8502, 65.7%) were aged 60-70 years, with a near-equal distribution of males and females at around 50%. The average PCS score was 76.77, while the MCS score averaged 59.70. Both IF (β=0.165, P<.001) and FF (β=0.189, P<.001) had a direct positive effect on QOL. Furthermore, the indirect effects of IF (β=0.186, P<.001) and FF (β=0.211, P<.001) through HR and HSD were also significant. In the direct model, IF and FF had a greater impact on MCS (β=0.841) than on PCS (β=0.639). However, after including the 2 mediating factors, HR and HSD, the influence of IF and FF on MCS (β=0.739) became consistent with that on PCS (β=0.728). Subgroup analyses revealed that the direct effect of IF on QOL was significant in the 60-70 age group (β=0.151, P<.001) but not in those over 70 years old (β=0.122, P=.074). Comorbidity status significantly influenced the pathway from HR to HSD, with older adults having 2 or more chronic diseases (β=0.363) showing a greater impact compared to those with fewer than 2 chronic diseases (β=0.358). Both IF (education, per capita disposable income, and endowment insurance) and FF (satisfaction with a spouse and children) directly impact the QOL in older people. Meanwhile, IF and FF have equal influence on QOL through the mediating role of HR and HSD. Recognizing the interplay among these factors is crucial for targeted interventions to enhance the well-being of older adults in China.

Ready to act? Factors associated with nurses’ desire to work in Primary Health Care in Italy

Abstract Background The rapid increase in demand for primary healthcare (PHC) services after the pandemic requires reconsidering nursing staff planning and availability. The objectives of the study were to i) identify knowledge, attitudes, practices and work expectations of future nurses in primary care; ii) estimate the prevalence of nursing students who desire to be employed in PHC and Family and Community Health Nursing (FCHN) services after graduation. Methods A cross-sectional study is being conducted on a sample of nursing students enrolled in universities in the Lazio Region. A structured instrument was built based on the literature and national regulations and validated (Cronbach’s alpha=0.81). Results In this first phase, 163 nursing students were enrolled. The majority of the sample was female (N = 125, 76.7%), with a mean age of 22 (SD ± 5) years and were enrolled in the last year of degree program (N = 85, 52.1%). A total of 49 students (30%) reported adequate knowledge of PHC service reorganization policies and nursing practice in this setting. While 74% (N = 122) of the sample intended to undertake a PHC curricular internship during their degree program, only seven students (4%) participated in one. 52 individuals (30%) expressed their intention to be employed in PHCs after graduation, and 29 (17.8%) indicated their desire to work as FCHN practitioners. The main factors contributing to the undesirability of employment in PHC reported by students were: i) preferring the hospital settings (N = 53, 48%); ii) not having adequate skills to be employed in the PHC (N = 21, 19%); and iii) believing that the PHC does not offer attractive services for nursing practice (N = 14, 13%). Conclusions A multivariable analysis will be conducted to identify predictors of nursing students’ desire to work in PHCs after graduation. Preliminary results of the study showed that only one-third of nursing students indicated a desire to work in PHCs. Key messages • Only one-third of nursing students express a desire to work in PHC and in Family and Community Health Nursing (FCHN) services post-graduation. • Collaboration between the PHC workforce, academics, stakeholders, and policymakers is needed to create tailored initiatives that increase the attractiveness of PHC.

Ethical issues after the earthquake in Turkey: A qualitative study on nurses' perspectives.

After natural disasters, the urgent need for survival and rescue operations for those trapped in the rubble results in a heightened demand for healthcare services. Nurses are pivotal in addressing the consequences of such calamities. Nurses operating in critical conditions encounter numerous challenges, including safety issues and ethical quandaries. The ethical considerations primarily revolve around the limitations, duties, and modified norms of care administered by nurses during disasters, presenting both significant and complex challenges. The study was designed to elucidate the nature and essence of nurses' care experiences and to offer an in-depth depiction of the ethical dilemmas while attending to individuals injured in the earthquake. A descriptive phenomenological design was used in this research. A total of 16 nurses who worked for at least 7 days in field hospitals in any of the earthquake regions after the 2023 earthquake in Turkey participated in the research. The research was conducted between March 13 and March 20, 2023. Data were collected by face-to-face interview method with a semistructured form. The steps of Colaizzi's phenomenological method were used to analyze the qualitative data. The study involved a group of 16 Turkish nurses, aged between 20 and 36 years. Through data analysis, four primary themes, 14 subthemes, and 25 codes were identified. The principal themes derived from the interviews included "humanitarian duties," "non-adherence to ethical principles," "respect for the injured," and "medical errors." In this intricate process, it becomes paramount to enhance support for nurses in the domains of management, education, and psychosocial assistance, thereby nurturing their individual and professional acumen, commitment, and ethical sensitivity. Refining the guidelines and professional standards that encapsulate the principles to be adhered to by nurses in disaster scenarios, coupled with providing comprehensive training in both medical and ethical competencies, represents a significant avenue for advancement. Focused training on crisis management will enable nurses to render ethically informed decisions and bolster their professional proficiency. Natural disasters, including earthquakes, pose significant ethical challenges for healthcare professionals, especially nurses who play a pivotal role in delivering care during these emergencies. Issues such as resource allocation and triage, the duty of care and ensuring continuity of care, informed consent and communication, cultural sensitivity, and providing psychosocial support are critical ethical considerations for nursing policies in earthquake situations. Addressing these ethical issues in nursing policies related to earthquakes is crucial for maintaining the integrity of healthcare delivery, preserving public trust, and ensuring the well-being of both the injured and healthcare providers during times of crisis.

A Relationship between Physical Infrastructure and Patient Satisfaction: A Case Study of Kantipur Dental Hospital

This study investigates the impact of physical infrastructure on patient satisfaction at Kantipur Dental Hospital, a prominent dental care facility in Nepal. With the increasing demand for high-quality healthcare services, understanding the factors influencing patient satisfaction is crucial for healthcare providers. The research aims to explore how various aspects of the hospital’s infrastructure—specifically, the use of state-of-the-art equipment, cleanliness and hygiene, sterilization practices, and convenient location—affect patients' perceptions of care quality. A quantitative research design was employed, utilizing a structured questionnaire distributed to 117 patients who had received treatment at Kantipur Dental Hospital within the last six months. The questionnaire measured patients' perceptions of the clinic's infrastructure and their overall satisfaction, with a focus on two key dimensions: confidence after treatment and the acceptability of service fees. Data analysis was conducted using Spearman’s rho correlation to assess the relationships between the perceived infrastructure variables and patient satisfaction outcomes. The findings reveal significant positive correlations between perceptions of the hospital's physical infrastructure and patient satisfaction. Specifically, the use of modern equipment and a clean, hygienic environment were strongly associated with increased patient confidence post-treatment. Furthermore, the accessibility of the hospital location was also found to influence perceptions of service fee acceptability. Notably, while sterilization practices contributed positively to patient confidence, they did not significantly impact fee perceptions, suggesting that patients prioritize cleanliness and modernity in their overall evaluation of care. These results underscore the critical role of physical infrastructure in enhancing patient satisfaction at Kantipur Dental Hospital. The study recommends that healthcare facilities continue to invest in modern technology and maintain high standards of cleanliness to foster patient confidence and satisfaction. This research provides valuable insights for clinic management and policy-makers, emphasizing that improving physical infrastructure is essential for delivering quality healthcare services and ensuring positive patient experiences.

The public health challenges of female migration: the Venezuelan diaspora in Andean countries

BackgroundVenezuelan migration has experienced an unprecedented increase in the last decade, with approximately 7.7 million Venezuelan-born individuals residing in other countries as of 2024. Our study aims to identify the potential and actual demand for healthcare services (SRH) in the Venezuelan diaspora’s four primary destinations within the Andean Countries: Colombia, Ecuador, Peru, and Chile.MethodsUsing official data from administrative records, censuses, and sample surveys reported by the host countries and international agencies, we estimate the annual evolution of Venezuelan-born women of reproductive age (WRA) and their offspring. Additionally, we conduct two case studies focusing on Colombia and Chile to analyse the groups most vulnerable to unmet health needs.ResultsThe population of WRA has increased to between 5 and 6.8%, and births have risen to approximately 3–8% in host countries due to Venezuelan migration. Yet, we found a general decrease in health coverage for certain age groups of Venezuelan female migrants in host countries for the period 2017–2022, particularly in Chile. By 2022, an estimated 20% of healthcare needs remained unmet among children, girls, and younger Venezuelan women, contributing to greater health inequalities between Venezuelan-born adolescents and those from other countries of birth.ConclusionsOur findings highlight the escalating demand for and limited access to healthcare services among Venezuelan WRA in their destinations. Unmet healthcare needs are particularly prevalent among younger women by 2022, underscoring the urgency for health system to incorporate gender-responsive, equitable interventions and ensuring health rights for high-risk migrant groups such as infants, children, adolescents, and younger women. Addressing these challenges remains a critical task for the regional public health agenda in Latin America.

Uncovering perspectives on physical activity in nursing homes: a qualitative exploration of the experiences of healthcare professionals and family caregivers

BackgroundThe ageing population has increased the demand for healthcare services. In Norway, community-based long-term care are prioritised, leading to fewer nursing home places. As a result, nursing home residents are now older and have more complex needs. Nearly 92% of nursing home residents are affected by cognitive impairments accompanied by neuropsychiatric symptoms (NPS) that affect their daily activity, physical function, cognition, and behaviour. Traditionally, pharmacological therapy has been the prevailing treatment for NPS. However, emerging evidence suggests that physical activity can serve as an alternative treatment approach. Physical activity has the potential to maintain physical independence and enhance the quality of life (QoL) for the residents. Despite these benefits, institutionalisation in a nursing home often restricts activity levels of residents. This study explores facilitators and barriers to physical activity in nursing homes through the experiences of healthcare professionals and family caregivers. The goal is to enhance our understanding of how to promote and support physical activity for nursing home residents by identifying essential factors for successfully implementing daily physical activity initiatives.MethodsSeven focus groups were conducted with a total of 31 participants. Participants included healthcare professionals (physiotherapists, nurses, unit- and department managers, assistant occupational therapists, and assistant nurses) and family caregivers of residents at nursing homes. Data were analysed using Braun and Clarke’s reflexive thematic analysis, underpinned by hermeneutic phenomenology.ResultsThree main themes related to facilitators and barriers to physical activity in nursing homes were identified: inconsistency in task prioritisation; need for improved interprofessional collaboration; and need for improved utilisation of external resources. The participants experienced task prioritisation and lack of interdisciplinary collaboration as barriers to physical activity. The involvement of external societal resources was considered as both a facilitator and a necessity for obtaining physical activity in nursing homes.ConclusionsThis study highlights the need for a consensus in task prioritisation, enhanced competence among healthcare professionals, and better interdisciplinary collaboration to facilitate physical activity in nursing homes. Involving external societal resources could be a strategic approach to address barriers and support physical activity initiatives. Future research should focus on developing effective strategies for interdisciplinary collaboration that prioritises and promotes physical activity in nursing homes.

The Effects of Health Insurance on Demand for Healthcare Services in Tanzania

ABSTRACT: Consistent healthcare financing systems within a particular nation or community have been regarded as vital for making universal healthcare access. This study intended to examine the influence of health insurance on the demand for healthcare services and the influence of socio-economic, demographic and hospital-related factors on demand for health care services in Tanzania context. The study employed secondary data, which were analysed by descriptive statistics and the results were presented in tables and percentages. Econometric analysis was also done using binary probit regression model to determine the factors influencing demand for health care services in Tanzania. The instrumental variable was applied to address the problem of endogeneity of health insurance. The regression results revealed that gender, education level, household size, income, treatment cost, perceived health quality and health insurance type were significant and positively linked with demand for health care services. However, marital status was also significant but negatively associated with the demand for healthcare services. The study concluded that, despite of the efforts made by the government and other stakeholders on emphasis over having health insurance, the enrolment to health insurance schemes is still low although it has been noted to significantly influence the demand for health care services. Lastly, the paper recommends that the government and other stakeholders are required to formulate or strengthen the formulated policy in practice, purposively to improve access, affordability and the quality of health care services. Governmental and non-governmental organizations (NGO`s) and other stakeholders should put more effective strategies to improve coverage but also reduce the financial burden caused by illness. Also, the government needs to subsidize the healthcare sector, especially to the poor and provide subsidies to private healthcare facilities, improvement of insurance type, specifically, the NHIF which needs to increase coverage. Furthermore, health care managers should educate other people and make a clear observation on factors that make individuals demand for health care because they have a great role in the health care sector as well as strategies to improve household`s income to meet the health needs in Tanzania.

Hospitalizations and emergency attendance averted by influenza vaccination in Victoria, Australia, 2017 - 2019.

Seasonal influenza epidemics result in high levels of healthcare utilization. Vaccination is an effective strategy to reduce the influenza-related burden of disease. However, reporting vaccine effectiveness does not convey the population impacts of influenza vaccination. We aimed to calculate the burden of influenza-related hospitalizations and emergency department (ED) attendance averted by influenza vaccination in Victoria, Australia, from 2017 to 2019, and associated economic savings. We applied a compartmental model to hospitalizations and ED attendances with influenza-specific, and pneumonia and influenza (P&I) with the International Classification of Diseases, 10th Revision, Australian Modification (ICD-10-AM) diagnostic codes of J09-J11 and J09-J18, respectively. We estimated an annual average of 7657 (120 per 100000 population) hospitalizations and 20560 (322 per 100000 population) ED attendances over the study period, associated with A$85 million hospital expenditure. We estimated that influenza vaccination averted an annual average of 1182 [range: 556 - 2277] hospitalizations and 3286 [range: 1554 - 6257] ED attendances and reduced the demand for healthcare services at the influenza season peak. This equated to approximately A13 [range: A6 - A25] million of savings over the study period. Calculating the burden averted is feasible in Australia and auseful approach to demonstrate the health and economic benefits of influenza vaccination.
