Decellularized solid organ xenografts such as the kidney hold great promise due to the maintenance of structural integrity while removing all cellular components. However, acelluar kidney vasculature needs an endothelial cell lining to avoid blood clotting and thrombus formation when transplanted into a living recipient. We previously have demonstrated that re-endothelialization of entire vasculature of decellularized kidney scaffolds is possible. In this study, we aimed to evaluate the effectiveness of re-endothelialization in vivo through implantation in a porcine model. METHODS Acellular porcine kidney scaffolds seeded with endothelial cells (MS1) expressing GFP was cultured in a bioreactor system. After perfusion bioreactor culture, the cell-seeded scaffold was implanted into pigs by arterial and venous anastomosis. Doppler ultrasound and angiography were performed to confirm patency of vessels after implantation. Histomorphological analyses were performed to determine the levels of re-endothelialization of vasculatures in the renal scaffold. RESULTS Uniform seeding of endothelial cells on the decellularized porcine kidneys was confirmed by H&E staining and SEM analysis. Organized endothelial cells were shown on the artery, vein, and smaller blood vessels without apparent clogging of cells at the capillary structures in the cortex. Furthermore, MS1 seeded scaffold displayed significant inhibition of platelet adhesion on cell-attached surfaces as compared to the strong platelets deposition found on the unseeded scaffolds. Patency of both renal artery and vein was confirmed by Doppler ultrasound after implantation. Angiography of the harvested kidneys showed intact vascular trees, indicating of patent vasculature network. Histological examination confirmed that most of the vasculatures in the kidney parenchyma were homogeneously coated with endothelial cells with minimal platelet adhesion. CONCLUSIONS These results show that implantation of re-endothelialized porcine kidney scaffolds prevents the formation of blood clots and maintains flow after implantation.
