Limbal stem cell deficiency following topical mitomycin c treatment of Conjunctival-Corneal intraepithelial neoplasia

Purpose To report a case of conjunctival-corneal intraepithelial neoplasia (CCIN) in an elderly African American patient treated with topical mitomycin C and the subsequent complication of limbal stem cell deficiency. Design Interventional case report. Methods A 92-year-old African American woman was diagnosed with CCIN in the right eye. Following incisional biopsy, the patient received five 1-week courses of 0.04% mitomycin C and was followed over a period of 10 months. Results The CCIN regressed completely following mitomycin C therapy. Three months later, the patient developed recurrent nonhealing epithelial defects in the right cornea. Conclusions Conjunctival-corneal intraepithelial neoplasia may occur in the African American population. Although MMC is effective in eradicating CCIN, a limbal stem cell deficiency may complicate the treatment.

Uso de mitomicina C tópico no tratamento da neoplasia intra-epitelial córneo-conjuntival e carcinoma espinocelular conjuntival: resultados preliminares

OBJETIVO: Avaliar a segurança, a eficácia e a recidiva tumoral após o uso de mitomicina C (MMC) tópica no tratamento de neoplasias intra-epiteliais (NIC) primárias e recidivadas e carcinomas espinocelulares (CEC) recidivados. MÉTODOS: Dois grupos de pacientes foram tratados. Grupo 1 com diagnóstico de NIC primário ou recidivado fez uso do colírio de MMC 0,02%, 4 vezes ao dia por 28 dias. O grupo 2 com diagnóstico de CEC recidivado fez uso do colírio de MMC 0,02%, 4 vezes ao dia por 21 a 28 dias. Após o término do tratamento era feita citologia exfoliativa para controle ou biópsia nos casos de lesão residual. RESULTADOS: Oito pacientes com NIC e 1 com CEC mostraram regressão total da lesão e citologia exfoliativa de controle negativa para células neoplásicas. Em 1 paciente com NIC e 2 com CEC, houve regressão parcial das lesões, tendo sido necessária exérese cirúrgica da lesão residual, com exame anatomopatológico negativo para neoplasia. Os efeitos colaterais foram transitórios e desapareceram após a suspensão do colírio. Não houve recidiva tumoral num seguimento médio de 24,9 meses. CONCLUSÃO: Os resultados preliminares desse estudo su-gerem que o uso da MMC tópica é opção segura e eficaz como tratamento único de NIC primária ou recidivada e no tratamento do CEC recidivado, porém não evitando a cirurgia na maioria dos pacientes com CEC. Não foram observadas recidivas tumorais durante o segmento. Mais estudos são necessários, com maior número de pacientes e maior tempo de seguimento para confirmar esses resultados.

Mitomycin C Treatment for Conjunctival—Corneal Intraepithelial Neoplasia: A Multicenter Experience

The purpose of the study is to evaluate the efficacy and risks of topical mitomycin C (MMC) for conjunctival-corneal intraepithelial neoplasia (CCIN). The study design was a clinical case series of CCIN. Seventeen patients, 16 with biopsy-confirmed CCIN and 1 with invasive squamous cell carcinoma (SCC), were included in the study. Patients received topical drops of MMC 0.02% to 0.04% four times daily from 7 to 28 days. Retreatment was done in cases of lesion recurrence. The size of the CCIN before and after the treatment and ocular complications post-MMC application were evaluated. Ten patients remained disease-free after one course of MMC application. In one case, residual CCIN remained very small without regrowth. In the one patient with invasive SCC and in five patients with CCIN, regrowth occurred within 6 months of the first treatment. After retreatment, invasive SCC and CCIN in an additional two patients were eradicated. In two cases, although the size of the lesions decreased after two and three applications of MMC, regrowth occurred, and the CCIN returned to its original size. In the final case, limited recurrence has occurred and no retreatment has been done. The complications of MMC use included mild-to-moderate conjunctival hyperemia and mild allergy, which resolved after discontinuation of the treatment. Severe pain manifested when treatment was longer than 14 days. Application of topical MMC is an efficient treatment for most but not all cases of CCIN.

Late Recurrences and the Necessity for Long-term Follow-up in Corneal and Conjunctival Intraepithelial Neoplasia

The purpose of the study was to elucidate the natural history of corneal-conjunctival intraepithelial neoplasia (CIN) and suggest treatment and follow-up guidelines. The records of all histologically proven cases of CIN at the Royal Victorian Eye and Ear Hospital between 1979 and 1994 were reviewed. Seventy-nine eyes of 76 patients had a pathologic diagnosis of CIN and were observed for up to 15 years. The lesion recurred in 31 eyes (39%) overall. There was no statistical difference shown in the likelihood of recurrence based on histologic classification. Complete excision was attempted in each case. In 18 eyes, dysplastic cells were evident at the excision margin. Ten (56%) of these tumors recurred, compared to a 33% recurrence rate in completely excised lesions. The time to the first recurrence ranged from 33.0 days to 11.5 years between the first and second surgeries, with 11 lesions recurring after more than 4 years. Incompletely excised lesions reappeared more rapidly (average, 2.5 years) than did those with clear surgical margins (average, 3.8 years). Seven cases progressed to invasive squamous cell carcinoma and four cases led to blindness or removal of the eye. This study suggests that excision margin at the time of surgery is the most important factor in predicting recurrence. The slow growth of the recurrent lesions combined with the ever-present malignant potential leads the authors to suggest that all patients with a history of CIN warrant annual follow-up for the remainder of their lives.
