Joint decision on contraceptive use and determinant factors among married women of reproductive ages (15–49) in Ethiopia: a multilevel analysis using Ethiopian Demographic and Health Survey; 2016 data

BackgroundContraceptive usage in Ethiopia is significantly influenced by the decision-maker at the household level. Joint decision-making involving both women and their husbands/partners is considered ideal for improving contraceptive uptake among women and for managing health outcomes related to contraceptive side effects. However, there is a lack of substantial evidence regarding the prevalence and impact of joint decision-making on contraceptive use in Ethiopia. Therefore, the current study aimed to assess the magnitude of joint decision-making on contraceptive use and its determinant factors among married, contraceptive-using, reproductive-age women in Ethiopia.MethodsThis study was conducted based on the Ethiopian Demographic and Health Survey (EDHS) 2016 data. A total weighted sample of 3,669 married, contraceptive-using, reproductive-age women were included in the study. Multilevel logistic regression was employed due to the hierarchical nature of the data. Variables with a p-value of ≤ 0.2 in the bivariate multilevel analysis were included in the multivariable multilevel analysis. The adjusted odds ratio (AOR) with a 95% confidence interval (CI) was used to determine the direction and strength of associations. Variables with a p-value of < 0.05 in the multivariable multilevel analysis were considered statistically significant for joint decision-making on contraceptive use.ResultsThe prevalence of joint decision-making on contraceptive use in Ethiopia was 78.81% [95% CI: 71.35-74.23%]. Several factors were found to be statistically significant in relation to joint decision-making on contraceptive use: Women educational level primary, secondary, and higher (Adjusted odds’ ratio (AOR = 1.5; CI 1.2–1.9), (AOR = 1.9; CI 1.3–2.9), and (AOR = 2.1; CI 1.2–3.5) respectively, protestant in religion (AOR = 1.7; CI 1.7–2.5), wealth status rich (AOR = 1.4; CI 1.1–1.9), media exposure (AOR = 1.4; CI 1.1–1.9), and community poverty high (AOR = 0.6; CI 0.6–0.9).ConclusionsIn Ethiopia, the majority of contraceptive users are married, reproductive-age women, and their decision to use contraceptives is typically made jointly with their husbands or partners. Factors positively associated with joint decision-making on contraceptive use include women’s educational level (primary, secondary, and higher), being Protestant, having a higher wealth status, and media exposure. Conversely, women living in communities with high poverty levels are less likely to make joint decisions about contraceptive use. Health care providers, health care programmers and policy makers should be focused on these determinant factors to enhance joint decision-making in women’s contraceptive use in Ethiopia.

Knowledge, Attitude, Practice, and Associated Factors of Implant Use in Women, Ethiopia.

The world is in a dramatic expansion of population that resulted from the continued high birth rates in developing countries. At present, only a handful of countries have reduced fertility rates. That is enough to make gains as a result of reduced fertility or to ensure that their population will stabilize unforeseeable future. Ethiopia is the second largest country by population in Africa, but contraceptive use in Ethiopia is still low. The main objective of the study was to assess knowledge, attitude, and practice and associated factors towards implant use among married reproductive age group women. An institutional-based cross-sectional study design was used to collect the data from 272 married reproductive age group women attending the family planning clinic at Ejere Health Center from October 9, 2021 to November 24, 2021. A systematic random sampling technique was used to select and approach the study subjects. The collected data was analyzed using SPSS window version 21, and bi variable, multivariable logistic regression was used to test the association between independent and dependent variables. A P value of less than 0.05 and 95% CI was considered to be statistically significant. Finally, the result was presented using tables and graphs. The finding shows that most of the respondents know implant (87.6%). Concerning the practice of implants, the overall prevalence of implants was 51 (20.6%), and in attitude towards implant utilization among study clients, most of them wrongly believed that using implant can result in irregular uterine bleeding and cause pain during insertion and removal. Mothers who attended primary school 2.580 times (AOR = 2.580, 95% CI: 1.255-5.305, P value =0.01) had more positive attitudes than those who could not read and write. Mothers having positive attitude were 2.066 times (AOR = 2.066, 95% CI: 1.185-3.600, P value =0.01) knowledgeable about implant than those with negative attitudes. Even if most of the respondents were knowledgeable, practice and attitude towards implant utilization were low. To overcome this gap, we recommended adequate early counseling, on possible side effects, and creating awareness on implant contraceptive services and refresher courses for health workers regarding implant contraception should be given regularly regarding reproductive issues.

Trend, geographical distribution, and determinants of modern contraceptive use among married reproductive-age women, based on the 2000, 2005, 2011, and 2016 Ethiopian demographic and health survey

BackgroundThe most common family planning method is modern contraception. It is a cost-effective way to reduce maternal and neonatal morbidity and mortality and enable women to make informed choices about their reproductive and sexual health. The trend of modern contraceptive utilization has shown drastic change in Ethiopia, and identifying the major factors contributing to such a drastic change is vital to improving plans and strategies for family planning programs. Therefore, this study analyzed the trend, geographical distribution, and determinants of modern contraceptive use among married reproductive-age women in Ethiopia.MethodThis study used secondary data from the EDHS 2000–2016, collected from a population-based cross-sectional study by the Central Statistical Agency, focusing on married reproductive-age women aged 15–49. The study analyzed the modern contraceptive use trends through descriptive analyses conducted in three phases: 2000–2005, 2005–2011, and 2011–2016. The study utilized bivariable and multivariable logistic regression analyses to identify determinant factors, with significant variables declared using a P-value of 0.05 and an adjusted OR with 95% confidence interval. Analysis was conducted using STATA.14 and R. Spatial analysis was done using ArcGIS version 10.8 and SatScan™ version 9.6.ResultA weighted total of 33,478 women are included in the study, with a mean age of 31.4 years (8.6 SD). There was a significant increase in the trend of modern contraceptive use among married women over the study period, from 2000 to 2016, from 7.2% to 2000 to 15.7% in 2005, to 30% in 2011, and to 39.5% in 2016. The maximum increase was seen in the second phase (2005–2011), with a 14.3% increase. Factors like age of respondents, educational status, religion, residence, region, wealth index, number of living children, husbands’ desire to have more children, and media exposure were found to be predictors for modern contraceptive utilization.ConclusionThe prevalence of modern contraceptive use is below 50%, and there is also evidence of wide geographical variation in modern contraceptive use in Ethiopia. Thus, policymakers, high institutions, and other stakeholders must work collaboratively with the government in order to improve awareness about modern contraceptive use.

The effect of maternal educational status, antenatal care and resumption of menses on postpartum contraceptive use in Ethiopia: systematic review and meta-analysis

The postpartum period is a crucial starting point for the delivery of family planning services. To date, there are numerous primary studies in Ethiopia on postpartum contraceptive use and related factors. However, the results of key variables are inconsistent, making it difficult to use the results to advance the service dimensions of postpartum contraceptive use in the country. Therefore, this systematic review and meta-analysis was required to summarize this inconsistency and compile the best available evidence on the impact of maternal educational status, antenatal care and menstrual resumption on postpartum contraceptive use in Ethiopia. PubMed, Google Scholar, Scopus, Science Direct, and the repositories of online research institutes were searched. Data were extracted with Microsoft Excel and analyzed with the statistical software STATA (version 14). Data on the study area, design, population, sample size, and observed frequency were extracted using the Joanna Briggs Institute tool. To obtain the pooled effect size, a meta-analysis was performed using a weighted inverse variance random effects model. Cochran's Q X2 test, and I2 statistics were used to test for heterogeneity, estimate the total quantity, and measure the variability attributed to heterogeneity. A mixed-effects meta-regression analysis was performed to identify possible sources of heterogeneity. To examine publication bias, the Eggers regression test and the Beggs correlation test were used at a p-value threshold of 0.001. Of the 654 articles reviewed, 18 studies met the inclusion criteria and were included in this meta-analysis. Overall, the final analysis includes 11,263 study participants. In Ethiopia, postpartum contraceptive use correlated significantly with maternal educational status (OR = 3.121:95% CI 2.127–4.115), antenatal care follow-up (OR = 3.286; 95% CI 2.353–4.220), and return of the mother's menses (OR = 3.492; 95% CI 1.843–6.615). A uniform meta-regression was performed based on publication year (p = 0.821), sample size (p = 0.989), and city of residence (p = 0.104), which revealed that none of these factors are significant. The use of postpartum contraceptives was found to be better among mothers who are educated, attended antenatal appointments, and resumed their menstrual cycle. Based on our research, we strongly recommended that antenatal care use and maternal educational accessibility need to improve. For family planning professionals, removing barriers to menstruation resumption should be a key priority.

Spatial variations and multi-level determinants of modern contraceptive utilization among young women (15–24 years) in Ethiopia: spatial and multi-level analysis of mini-EDHS 2019

IntroductionThere is tremendous regional inequalities and low uptake of modern contraceptives particularly among young women (15–24 years), characterized by high fertility but high unmet need for contraceptives in Ethiopia. Hence, the present study aimed at exploring the spatial distribution and the multi-level determinants of modern contraceptive use among young women in Ethiopia.MethodsThis study was conducted using the 2019 Ethiopian demographic and health survey data on a weighted sample of 3,379 young women. STATA version 14 for the multi-level, and ArcGIS 10.7 and Sat Scan 9.6 for the spatial analysis were used. Spatial analysis was done to identify the hotspot areas of modern contraceptive use in Ethiopia. Multi-variable multi-level logistic regression was used for identifying determinants of modern contraceptive use and variables with a p-value < 0.05 were considered to be significant determinants.ResultThe overall prevalence of modern contraceptive use among young women in Ethiopia was 17.23% (95% CI: 10.98, 23.47). The hotspots areas for modern contraceptive use were detected in the central and south-western Amhara, western and central Oromia, and western SNNPR regions. Whereas the Somali region, Dire dawa, and Harari cities were cold spot areas for modern contraceptive use.Being married (AOR = 18.5; 95% CI: 12.66, 27.27), parity (AOR = 4.82; 95% CI: 1.27, 18.32), having television (AOR = 2.39; 95%CI: 1.43, 3.99), having radio (AOR = 1.43; 95%CI: 1.05, 1.94) had higher odds of using modern contraceptives compared to their counterparts. Besides, family size of above five (AOR = 0.46; 95% CI: 0.34, 0.62) and living in Somali region (AOR = 0.05; 95% CI: 0.01, 0.32) were associated with decreased odds of using modern contraceptives among young women in Ethiopia.ConclusionThe modern contraceptive use was low among young women and considerably varied across regions in Ethiopia. A remarkably low rate of modern contraceptive use (cold spot) area was detected in Somali region-Ethiopia. Taking in to account a geographic perspective and key factors identified in this study would be vital for efficient resource allocation, targeted interventions, and informed decision-making to enhance contraceptive uptake in Ethiopia.

Applying a gender lens to social norms, couple communication and decision making to increase modern contraceptive use in Ethiopia, a mixed methods study

BackgroundEthiopia, sub-Saharan Africa’s second most populous country has seen improvements in women’s reproductive health. The study objectives are (1) using mixed methods research, to identify determinants of contraceptive use in four regions of Ethiopia, and (2) to explore the relationship between social norms, gender equitable norms, couple communication and contraceptive use.MethodsThe study includes both quantitative and qualitative methods. Researchers interviewed a total of 2770 women of reproductive age (15–49 years) in 2016 using a structured survey covering six health areas. Eligible households were identified using a multi-stage cluster-sampling technique. Using probability proportionate to size sampling, the researchers selected 10% of the proposed target woredas (24 of 240 woredas). The qualitative study included 8 rapid assessments, 16 in-depth interviews, 24 key informant interviews, and 16 focus group discussions. Qualitative data were analyzed using NVivo version 8.ResultsAdjusted odds ratios were estimated for current modern family planning use among married women with logistic regression. The primary influencing factors for contraceptive use are gender equitable norms, high self-efficacy, and weekly exposure to the radio. Qualitative data indicate that the timing of contraceptive use is linked to the social norm of the desired family size of 4–5 children. Gender inequity is evident in couple communication with men controlling decision making even if women initiated conversations on family planning. A key finding based on an inductive analysis of qualitative data indicates that the micro-processes of couple communication and decision making are often dictated by male advantage. The study identified six micro-processes that lead to gender inequity which need to be further examined and researched.ConclusionsBarriers to contraceptive use include unequal couple communication and compromised decision making. Inequitable gender norms are also barriers to modern contraceptive use. The study recommends using a gender lens to study couple communication and decision making, with the goal of making both processes more equitable to accelerate the adoption of modern family planning methods in Ethiopia.

Male involvement in family planning use and its determinants in Ethiopia: a systematic review and meta-analysis protocol

BackgroundThe need to include males who require joint spousal decisions is critical in achieving key reproductive health indicators. Low involvement of males in family planning use is one of the contributing factors for low contraceptive use in Ethiopia. Despite this, there are inconsistent findings on the prevalence and determinants of male involvement in family planning use in Ethiopia. Thus, this systematic review and meta-analysis aimed to determine the pooled prevalence of male involvement in family planning use and its determinants in Ethiopia.MethodsThe Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines will be used to develop the protocol. The online databases PubMed, CINAHL, Google Scholar, and unpublished gray literature will be searched to retrieve available articles from April 10 to August 11, 2021. The two authors will conduct selection of studies, data extraction, and quality assessment. The quality of the studies will be assessed using the Joanna Briggs Institute checklist. The chi-squared test and I-squared statistic will be used to examine heterogeneity among studies. Sources of heterogeneity will be investigated using subgroup analysis and meta-regression based on regions and residence (urban and rural). Publication bias will be examined by observation using funnel plots and statistically by Begg’s and Egger’s tests. A random-effects model will be used to estimate the pooled prevalence and its determinants of male involvement in family planning use.DiscussionThe role of males in family planning and participation in contraceptive use improves women’s uptake and continuity of family planning use. Although there are studies on male involvement in family planning use, there are no synthesis research findings on the pooled prevalence of male involvement in family planning use and its determinants in Ethiopia. Therefore, the findings from this systematic review and meta-analysis will help the national health sector transformational plane emphasize the pooled prevalence and its determinants that drive low male involvement in family planning use in Ethiopia.

Long-acting reversible contraception utilization and associated factors among women in extended postpartum period in southern Ethiopia

BackgroundPostpartum long-acting reversible contraceptive is important to prevent unintended and closed spaced pregnancy following the first 12 months of childbirth. Few data were available on postpartum long-acting reversible contraceptive use in Ethiopia. So, this study aimed to assess the long-acting reversible contraception use and associated factors among women who gave birth in the last 12 months.MethodsA community-based cross-sectional study was conducted from October 1st to November 2019. Systematic random sampling was applied to recruit a total of 416 study participants. Data were collected using a structured questionnaire. Data were entered using Epidata 4.6 and exported to SPSS version 25 for analysis. P-value < 0.05 with 95% confidence interval (CI) used to declare statistical significance.ResultIn this study, long acting contraceptive utilization among women in the extended postpartum period was 22.6%. Maternal age ≤ 24 years (AOR = 3.7, 95% CI: 1.5, 8.9), being married (AOR =3.5 95% CI: (1.17–10.28)), menses resumption (AOR = 4.9 95% CI: (2.92, 8.20)), sexual intercourse resumption (AOR = 7.1 95% CI: (4.03, 12.56)) and received postpartum family planning counseling (AOR = 3.2 95% (1.95, 5.28)) were the factors associated with Long-acting reversible contraception use.ConclusionThis finding showed postpartum women’s long-acting reversible contraceptive use during the extended postpartum period was low. The factors significantly associated with extended postpartum modern contraceptive use were women’s age, being married, menses resumption, sexual intercourse resumption, and got postnatal family planning counseling. Strengthening Antenatal and postnatal counseling of postpartum family planning would improve long-acting reversible contraception use.

Contraceptive use among sexually active female adolescents in Ethiopia: trends and determinants from national demographic and health surveys

BackgroundSexual and reproductive health and right of adolescents is a global priority as the reproductive choices made by them have a massive impact on their health, wellbeing, education, and economy. Teenage pregnancy is a public health issue and a demographic challenge in Ethiopia. Increasing access to contraceptive services for sexually active adolescents will prevent pregnancies and related complications. However, little is known about the trends in contraceptive use and its determinants among adolescent girls in Ethiopia. Therefore, this study was designed to examine the trends and factors associated with contraceptive use among sexually active girls aged 15–19 years in Ethiopia by using Ethiopian demographic and health survey data.MethodsFour Ethiopian demographic and health survey data were used to examine trends of contraceptive methods use. To identify factors associated with contraceptive use, the 2016 Ethiopian demographic and health survey data were used. The data was downloaded from the demographic and health survey program database and extracted for sexually active adolescent girls. Data were weighted for analysis and analyzed using SPSS version 21. Descriptive analysis was used to describe the independent variables of the study. A multivariable logistic regression model was used to identify factors associated with contraceptive use and adjusted odds ratios with 95% confidence interval were presented for significant variables. Variables with a p-value less than 0.05 were considered as significantly associated with contraceptive use.ResultsContraceptive method use had increased significantly from 6.9% in 2000 to 39.6% in 2016 among sexually active adolescent girls in Ethiopia. The odds of contraceptive use were lower among female adolescents who had no formal education (AOR 0.044; 95% CI 0.008–0.231) and attended primary education (AOR 0.101; 95% CI 0.024–0.414). But the odds were higher among adolescents from a wealthy background (AOR 3.662; 95% CI 1.353–9.913) and those who have visited health facilities and were informed about family planning (AOR 3.115; 95% CI 1.385–7.007).ConclusionThere is an increment in the trend of contraceptive use among sexually active female adolescents in Ethiopia between 2000 and 2016. Significant variations in the use of modern contraception by wealth status, educational level and visited a health facility, and being informed about family planning were observed. Improving the economic and educational status of young women, and provision of information may help in improving contraceptive use in Ethiopia.

Factors Affecting Contraceptive Use in Ethiopian: A Generalized Linear Mixed Effect Model

Ethiopia is the second most populous nations in Africa. Family planning is a viable solution to control such fast-growing population. This study aimed to assess the prevalence of contraceptive use and its predictors in Ethiopia. About 4,563 women were drawn randomly by Central Statistics Agency from its master sampling frame. The survey was conducted from January, 2014 to March, 2016 within six months' interval for the study period. The study was conducted using secondary data collected by PMA2020/Ethiopia project. Negative Binomial regression model was employed for data analysis. The model was selected using information criterion. Predictors like easy access of health service, residence area, level of health institutions, regions, availability of community health volunteers, experience sharing, support from husbands, level of education and employment status of women as well as residence area significantly affected the performance of contraceptive use in Ethiopia. From the interaction effects of health centers with region and health post with number of opening days per a week were significant predictors of the contraceptive use. The performance of contraceptive use was different from one individual to another because of their experience sharing, support from their husbands, employment status and education level. A woman who got encouragement to use birth control from her husband had good performance to be effective for her contraceptive use. There should be an experience sharing/orientation, about use of birth control to protect women from unwanted pregnancy. Hence, rural women should get experience from urban women.

Contraceptive use and associated factors among sexually active reproductive age HIV positive women attending ART clinic at Felege Hiwot Referral Hospital, Northwest Ethiopia: A cross-sectional study

BackgroundContraception helps prevent unplanned pregnancies and mother to child Human Immune Virus (HIV) transmission among human immune virus positive women. Ethiopia has made remarkable progress in increasing contraceptive use rate but there is still a disparity of contraceptive use within the country. Although there were some studies about contraceptive use in Ethiopia, evidences about contraceptive use among sexually active HIV positive women was limited. Understanding the extent of and barriers in Ethiopia is important for learning how to best improve level of contraceptive use. Therefore, this study aimed to assess contraceptives use and associated factors among HIV positive sexually active women at anti-retroviral therapy clinic at Felege Hiwot Referral Hospital ART clinic in Bahir Dar, Ethiopia. MethodA facility-based cross-sectional study was conducted from June 01–30, 2018 among (n = 308) randomly selected HIV positive women at Felege Hiwot Referral Hospital. Data were collected using a pretested interviewer-administered questionnaire and analyzed using SPSS version 20. Survey logistic regression analysis was employed to identify determinants of contraceptive use. Statistical significance was declared at p-value <0.05. ResultsOut of three hundred eight participants, 118 (38.3%) reported contraceptive use at the time of the study. Injectable is the most preferred (43.5%) contraceptive method. Participants with age 15–34 years (AOR = 3.09, 95%CI: 1.59–5.99), disclosed their status to sex partner, (AOR = 2.7, 95%CI: 1.14–6.66), had history of contraception use; (AOR = 3.36, 95%CI: 1.68–6.74), were sexually active (AOR = 5.45, 95%CI: 2.72–10.91) had higher odds of contraceptive use. However, participants who had drinking habit (AOR = 4.35, 95%CI: 1.82–10.38) had lower odds of contraceptive use. ConclusionA significant proportion of HIV positive women had low level of contraceptive use that was lower than the national recommended level. Participants with younger age (15–34years), who disclosed HIV status to sex partner, had history of contraceptive use, and who were sexually active six months prior to the study were more likely to use contraception. However, participants who had drinking habit were less likely to use contraception. These results suggest that multi-sectorial and multi-disciplinary approaches are needed to increase contraceptive use in the HIV positive women.

Effect of integrating postpartum family planning into the health extension program in Ethiopia on postpartum adoption of modern contraception

Background Ethiopia has large unmet need for contraception among postpartum women. Community-level services may improve postpartum contraceptive use in Ethiopia and other contexts where home childbirth is common. This study estimated the additional effect of systematically integrating messages on postpartum family planning (PPFP) into community contacts with pregnant and postpartum women, on top of integrated facility contacts. Methods The quasi-experimental study was conducted in two districts in Oromia Regional State and used a controlled trial design with random assignment of primary health care units—a health center and surrounding health posts—to intervention and comparison arms. We used the log-rank test and fitted a multivariate Cox proportional hazards regression model to estimate the adjusted hazard ratio (adjHR) and 95% confidence interval (95% CI) for differences in contraceptive uptake by arm. Time from delivery to uptake of modern contraception was the outcome variable. We separately analyzed subsets of women by place of delivery, as this variable interacts with the study arm. Results A total of 772 pregnant women were enrolled February–March 2017 and re-interviewed 15 months later (May 2018). Loss to follow-up was 10% in intervention, 7% in comparison areas. Among women who delivered at home, there was higher PPFP adoption by one year postpartum in the intervention arm (35.2%, 95% CI: 28.8–42.4%) versus comparison arm (27.8%, 95% CI: 22.2–34.4%). In the adjusted Cox regression model, women who delivered at home in the intervention arm were 45% more likely to adopt contraception (adjHR1.45, CI: 1.01–2.07). There was no difference by arm for women who delivered in a facility. Conclusions Integrating PPFP into community-level services for pregnant and postpartum women and infants may have additional benefit on top of PPFP services at facilities. The intervention benefited women who delivered at home, an important target population in countries like Ethiopia, where many women do not deliver in a facility. This study, implemented under real world conditions, informs the PPFP body of evidence and fills a gap in research on the contribution of community-based PPFP in contexts where services are integrated within maternal, newborn, and child health care in facilities.

Factors associated with long-acting and short-acting reversible contraceptive use in Ethiopia: an analysis of the 2016 Demographic and Health Survey

Purpose This study sought to examine different factors associated with use of contraceptive types among Ethiopian women, as well as to update the literature on general contraceptive use. These analyses can be used to tailor and improve family planning policy and long-acting contraceptive use. Methods The Ethiopia Demographic and Health Survey 2016 conducted in-person interviews with 15,683 women aged 15–49. Participants were asked about contraceptive use, family planning attitudes, personal and household characteristics, and lifestyle. We used bivariate and multivariate logistic regression to analyse factors associated with general contraceptive use and long-acting contraceptive use. Results Our study confirms many previously documented determinants of general contraceptive use including marital status, age, religion, decision-making ability, and occupation. Factors associated with long- versus short-term reversible contraceptive use were religion, region, age group, highest level of education, and discussing family planning at a health facility. Conclusion This study outlines crucial differences between long- and short-acting contraceptive users. Further family planning policy efforts taking into account regional, religious and other personal and socio-economic factors would effectively augment ongoing efforts. Additionally, the role played by discussion with health professionals in supporting long-term contraceptive use reflects the success of the ongoing effort to drive use in Ethiopia. KEY MESSAGE Global and national health actors are focused on improving access to long-acting contraceptives, however there is little evidence on factors that facilitate their uptake compared to the traditionally popular, short-acting contraceptives. Our study found significant religious and regional differences in long-acting contraceptive use which policy makers can use to guide their efforts.. Community health extension workers play a key role in long-term contraceptive access through tailored person-person education, consultation, and provision.

Women's Decision on Contraceptive Use in Ethiopia: Multinomial Analysis of Evidence From Ethiopian Demographic and Health Survey.

Background:Women are left out of the conversation on contraceptive use due to a variety of reasons. One of the reasons women have reported for their nonuse of family planning method is that they do not decide to use or not to use it. This study aimed to assess the women’s decision-making on contraceptive use and identify its associated factors.Methods:Data for this study were extracted from the national representative 2016 Ethiopian Demographic and Health Survey. Data were collected using 2-stage cluster design, in which enumeration areas forming the first stage and households making the second stage. The analysis was done using multinomial logistic regression using STATA software version 14.Results:The study revealed that one-fourth (24.3%) 95% CI (23.7%-25.1%) decision was made by women. The multinomial analysis demonstrated women’s decision-making on contraceptive use was influenced by the age of women 15 to 19 years (adjusted odd ratio [AOR] = 0.327, 95% CI: 0.175-0.613), 20 to 24 years (AOR = 0.510, 95% CI: 0.390-0.666), and 25 to 29 (AOR = 0.557 95% CI: 0.460-0.675); place of residence (urban; AOR = 1.637, 95% CI: 1.331-2.015) and region in which the women dwell and husbands education; occupation of both woman and her husband; and number of children ever born were the factors significantly associated with the outcome variable.Conclusions:Women’s decision-making on contraceptive utilization was low. It was influenced by age, place of residence and region, education, occupation, and number of children ever born.

Assessment of Socio Cultural Factors Affecting the Utilization of Family Planning Services: The Case of Haramaya town Bate Kebele

In Ethiopia socio cultural factors are highly determine the utilization of family planning services. There are many socio cultural factors that contributes to low contraceptive use, high fertility rate, un meet need of family planning services among these: religion, culture, misconception and social norm as a barriers to universal untaken of contraceptive use in Ethiopia. The objective of the study was to assess socio cultural factors affecting the utilization of family planning services: The case of Haramaya town Bate kebele . This study was used different research methodology like, cross-sectional studies, with in research approach, the study was used mixed research approach, and also the study was obtained information from primary and secondary data sources. In addition to this, study was employed Interview and Survey methods. The study was used non-probability sampling techniques, with these techniques particularly purposive (Judgmental), and through this technique the researcher was selected 98 samples and also the researcher was analyzed the qualitative and quantitative data, that get from interview and questionnaire through narration, percentages and frequency. The Findings of this study was religious fundamentalists who are not take family planning services, and also they condemned individuals who used family planning services and Culture give much value to children. Family planning challenged by tradition it means the mainstream culture toward family planning, society perception, society perceive the concept family planning in a negative sense, it lead to imbalance of ministration, sterilization, over fatty and other negative explanation are given to the services. Both governmental and non-governmental health care institutions should strongly attach each other for the mitigating of socio-cultural factors affecting on family planning services utilization. Individual and community level associates of contraceptive use in Ethiopia: a multilevel mixed effects analysis

BackgroundFamily planning is one of the four pillars of safe motherhood initiative to reduce maternal death in developing countries. Despite progress in contraceptive use, unmet needs are wide open and fertility remains high. Ethiopia have a higher fertility rate which contributes to maternal and child health destitution, putting pressure on the already weak health system. This study examined individual and community-level factors associated with contraceptive use in Ethiopia.MethodsData from Ethiopian Demographic and Health Survey 2016 were used to identify individual and community level associated factors among reproductive-age women. Non-pregnant, fecund and sexually active women aged 15–49 were included. Six hundred forty-two communities and 6854 women were involved from this two-stage cluster sampled data. The analysis was done using two-level mixed-effects logistic regression to determine fixed effects of individual and community-level factors and random intercept of between characteristics.ResultsFrom the total eligible women for contraceptive use 2393 (34.9%) of them were users. Injectables were the commonest of all contraceptive methods. Various individual-level variables were associated with contraceptive use. Household wealth index, women’s age, number of living children, husband’s occupation, ever experience of a terminated pregnancy, current working status of the women, number of births in the last 3 years, and hearing of FP messages through different media were significantly associated individual-level variables after adjusting other factors. Community characteristics like region, place of residence, religion, and community-level wealth were the factors associated with contraceptive use.ConclusionBoth individual and community-level characteristics were significant predictors of use of contraceptives in Ethiopian women. Besides the individual-level factors, interventions should also consider community-level associates.

Determinants of Long-Acting Reversible Contraceptive Method Utilization among Married Women in Assosa Town in Western Ethiopia: A Case-Control Study

Background & aim: The prevalence rate of contraceptive use in Ethiopia is 42%; however, only 5% of Ethiopians use the long-acting contraceptive methods. This rate is 4.8% in Benishangul Gumuz Region in this country. Regarding this, the present study aimed to assess the determinants of long-acting reversible contraceptive method use among married women in Assosa town, Ethiopia. Methods: This community-based unmatched case-control study was conducted on 384 married women from April 26 to May 10, 2016. The study population consisted of the users (case group; n=128) and none-users (control group; n=256) of one of the long-acting methods. Multivariate logistic regression analysis was performed to identify the variables having a significant association. Results: A total of 382 women participated in this study with a response rate of 99.5%. The determinants of long-acting contraceptive use were identified as the lack of decision-making power (OR=0.29, 95% CI: 0.11-0.80), poor attitude toward such birth control methods (OR=0.13, 95% CI: 0.06-0.29), wrong expectation regarding method availability (OR=0.23, 95% CI: 0.07-0.73), low level of knowledge about long-acting contraceptive methods (OR=0.19, 95% CI: 0.08-0.47), future intention to get pregnant (OR=0.39, 95% CI: 0.18-0.82), and walking distance of more than 30 min from associated health facilities (OR=0.36, 95% CI: 0.15-0.85). Conclusion: Women’s decision-making power, knowledge and attitude regarding contraceptives, expectation regarding method availability, distance from associated facilities, and intention to get pregnant in the future were identified as the independent predictors of long-acting contraceptive method use.

Intentions on contraception use and its associated factors among postpartum women in Aksum town, Tigray region, northern Ethiopia: a community-based cross- sectional study

BackgroundIncreased access to contraceptive methods has been established as a cost-effective strategy for developing countries to reduce maternal and child mortality. Intentions to contraceptive uptake appear to be best predictors of actual contraceptive practice than the unmet need. However, intention to contraceptive use in Ethiopia particularly among postpartum women is not well assessed. Therefore, the objective of this study was to assess intention to use modern contraceptive and to identify factors associated among postpartum women in Aksum town.MethodsA community –based analytical cross-sectional study design was done to collect the data from 604 postpartum mothers using a structured questionnaire. The data was collected from March 25 to April 24, 2015. A multivariable logistic regression was conducted to assess factors associated with intentions to use contraceptive methods. Factors influencing intentions on contraceptive methods use were assessed by computing adjusted odds ratios (AOR) at 95% confidence interval (CI) with statistical significant p- value < 0.05.ResultsIntention to use modern contraceptive was 84.3%. Resumed sexual intercourse (AOR = 1.78; 95% CI: 1.34, 3.92) and women whose their husband approved family planning to use (AOR = 1.57; 95% CI: 2.02, 5.57) were more likely to have intention on contraceptive use. In addition, those women who knew at least one method of modern contraceptive (AOR = 5.17; 95% CI: 1.69, 15.82) were more likely to had intention to use modern contraceptive during extended postpartum period compared to their counterparts.Conclusion and recommendationMore than eight in ten study participants have intention to use contraceptive in the Aksum town. Resumed sexual intercourse, husband’s approval of family planning and knew at least one method of contraceptive are the three major predictors to be an areas when considering interventions to increase of intention on contraceptive. Therefore, this study highlighted that; in order to increase intention and adoption of contraceptive, the family planning services providers and programmers should continue the promotion of partner involvement and increasing family planning knowledge through printed media and mass media.
