Novel metrics for tracking blood pressure changes incontinuous cuffless blood pressure estimations

Recent studies revealed the importance of tracking continuous blood pressure (BP) changes in monitoring and controlling hypertension and diagnosing cardiovascular diseases. However, current evaluation protocols utilize distance measures as primary metrics, which cannot properly evaluate the ability of the estimation model to track BP changes. This paper proposes a comprehensive evaluation framework which evaluates the distance and trend similarity metrics, and the composite metric of both between the reference and estimated BPs. The results of applying both widely used conventional metrics and the new proposed metrics for BP estimations are demonstrated in an example of comparing the reference with a set of different BP estimations. Then, the metrics are applied to BP estimations obtained using state-of-the-art (SOTA) algorithms. It is shown that even though SOTA algorithms have a low mean and standard deviation of absolute difference, they are not capable of tracking short-term blood pressure changes. Additionally, the proposed metrics are normalized metrics and range from -1 to 1, making them intuitively interpretable, similar to well-known correlation coefficients. Therefore, we suggest that the proposed evaluation framework should be used regularly in evaluating continuous BP monitoring systems.

OVAR-BPnet: A General Pulse Wave Deep Learning Approach for Cuffless Blood Pressure Measurement.

Pulse wave analysis, a non-invasive and cuff-less approach, holds promise for blood pressure (BP) measurement in precision medicine. In recent years, pulse wave learning for BP estimation has undergone extensive scrutiny. However, prevailing methods still encounter challenges in grasping comprehensive features from pulse waves and generalizing these insights for precise BP estimation. In this study, we propose a general pulse wave deep learning (PWDL) approach for BP estimation, introduc-ing the OVAR-BPnet model to powerfully capture intricate pulse wave features and showcasing its effectiveness on multiple types of pulse waves. The approach involves constructing population pulse waves and employing a model comprising an omni-scale convolution subnet, a Vision Transformer subnet, and a multilayer perceptron subnet. This design enables the learning of both single-period and multi-period waveform features from multiple subjects. Additionally, the approach employs a data augmentation strategy to enhance the morphological features of pulse waves and devise a label sequence regularization strategy to strengthen the intrinsic relationship of the subnets' output. Notably, this is the first study to validate the performance of the deep learning approach of BP estimation on three types of pulse waves: photoplethysmography, forehead imaging photoplethysmography, and radial artery pulse pressure waveform. Experiments show that the OVAR-BPnet model has achieved advanced levels in both evaluation indicators and international evaluation criteria, demonstrating its excellent competitiveness and generalizability. The PWDL approach has the potential for widespread application in convenient and continuous BP monitoring systems.

Clinical Validation of Wearable Piezoelectric Blood Pressure Sensor for Continuous Health Monitoring.

Wearable blood pressure sensors have recently attracted attention as healthcare devices for continuous non-invasive arterial pressure (CNAP) monitoring. However, the accuracy of wearable blood pressure (BP) monitoring devices has been controversial due to the low signal quality of sensors, the absence of an accurate transfer function to convert the sensor signals into BP values, and the lack of clinical validation regarding measurement precision. Here, we report a wearable piezoelectric blood pressure sensor (WPBPS), which achieved a high normalized sensitivity (0.062 kPa-1 ), fast response time (23ms) for CNAP monitoring. The transfer function of a linear regression model was designed, offering a simple solution to convert the flexible piezoelectric sensor signals into BP values. In order to verify the measurement accuracy of WPBPS, clinical trials were performed on 35 subjects aged from 20s to 80s after screening. The mean difference between the WPBPS and a commercial sphygmomanometer of 175 BP data pairs was -0.89 ± 6.19mmHg and -0.32 ± 5.28mmHg for systolic blood pressure (SBP) and diastolic blood pressure (DBP), respectively. By building a WPBPS-embedded wristwatch, we demonstrated the potentially promising use of a convenient, portable continuous BP monitoring system for cardiovascular disease diagnosis in this study. Remotely Programmable Deep Brain Stimulator Combined with an Invasive Blood Pressure Monitoring System for a Non-Tethered Rat Model in Hypertension Research.

The control circuits of blood pressure have a strong neural regulatory element important in the pathogenesis of essential drug-resistant hypertension. Targeting lower medullary neural control mechanisms of blood pressure by electrical stimulation could be beneficial, and therefore, a novel device is needed. This paper presents a remotely programmable deep brain stimulator with an invasive continuous blood pressure monitoring system in a non-tethered rat model. The device is designed for lower medullary deep brain stimulation research with minimal interference to a daily animal routine. Electrodes were implanted in the caudal ventrolateral medulla. Animal survivability, catheter patency rates, and device data drift were evaluated. Eight out of ten rats survived the surgery and testing period with no or mild temporary neurological compromise. The study revealed that carotid catheters filled with heparinized glycerol ensure better catheter patency rates and blood pressure transduction. There was no significant drift in the device's pressure sensitivity during the experiment. To our knowledge, this is the first experimental study to show considerable animal survival after lower medullary implantation. Combining the ability to measure and monitor invasive blood pressure with a closed-loop brain pulse generator in a single device could be of potential value in future hemodynamic animal research.

Overview of the verticalization of technology: history of the occurrence and relevance of application

Verticalization is a mobilization technique that is aimed at preventing and treating impaired gravitational gradient in patients who are on bed rest for 24 h, regardless of their motor and cognitive status, and is conducted actively and passively. Passive verticalization as a rehabilitation technique is recommended for patients who cannot stand up independently and keep themselves in an upright position because of the severity of the general condition.
 In the preparation of this review, mainly literature sources from highly rated publications were used. Preference was given to sources published in the last 10 years. Review articles and materials prepared by experts in the field of writing/editing scientific publications, including review articles, were selected for the review.
 In the aggregate of the verticalization methods analyzed, the common recommendations for all are the early start of rehabilitation measures, continuity, and careful monitoring of hemodynamic characteristics. The technique itself today has more than 50 years of experience, and the combination of verticalization technologies with a mechanotherapeutic simulator with electric motor support is a modern solution for their application. This equipment can be used in both intensive care in addition to the equipment of continuous respiratory support and rehabilitation and other wards.
 The verticalization procedure, if possible, should be applied at the earliest possible time after the traumatic event to improve the rehabilitation potential. The verticalization of patients with chronic impairment of consciousness has physiological features, and hemodynamic adaptation is recommended to reduce the risk of orthostatic cerebral ischemia. For patients with disorders of the central nervous system, preliminary adaptation should be conducted to subsequent verticalization by gradually lifting on a multifunctional bed under hemodynamic control, including the CNAP continuous blood pressure monitoring system (Drger systems).

Piezoelectric Dynamics of Arterial Pulse for Wearable Continuous Blood Pressure Monitoring

Piezoelectric arterial pulse wave dynamics are traditionally considered to be similar to those of typical blood pressure waves. However, achieving accurate continuous blood pressure wave monitoring based on arterial pulse waves remains challenging, because the correlation between piezoelectric pulse waves and their related blood pressure waves is unclear. To address this, the correlation between piezoelectric pulse waves and blood pressure waves is first elucidated via theoretical, simulation, and experimental analysis of these dynamics. Based on this correlation, the authors develop a wireless wearable continuous blood pressure monitoring system, with better portability than conventional systems that are based on the pulse wave velocity between multiple sensors. They explore the feasibility of achieving wearable continuous blood pressure monitoring without motion artifacts, using a single piezoelectric sensor. These findings eliminate the controversy over the arterial pulse wave piezoelectric response, and can potentially be used to develop a portable wearable continuous blood pressure monitoring device for the early prevention and daily control of hypertension.

Blood Pressure Monitoring System Using a Two-Channel Ballistocardiogram and Convolutional Neural Networks.

Hypertension is a chronic disease that kills 7.6 million people worldwide annually. A continuous blood pressure monitoring system is required to accurately diagnose hypertension. Here, a chair-shaped ballistocardiogram (BCG)-based blood pressure estimation system was developed with no sensors attached to users. Two experimental sessions were conducted with 30 subjects. In the first session, two-channel BCG and blood pressure data were recorded for each subject. In the second session, the two-channel BCG and blood pressure data were recorded after running on a treadmill and then resting on the newly developed system. The empirical mode decomposition algorithm was used to remove noise in the two-channel BCG, and the instantaneous phase was calculated by applying a Hilbert transform to the first intrinsic mode functions. After training a convolutional neural network regression model that predicts the systolic and diastolic blood pressures (SBP and DBP) from the two-channel BCG phase, the results of the first session (rest) and second session (recovery) were compared. The results confirmed that the proposed model accurately estimates the rapidly rising blood pressure in the recovery state. Results from the rest sessions satisfied the Association for the Advancement of Medical Instrumentation (AAMI) international standards. The standard deviation of the SBP results in the recovery session exceeded 0.7.

Anesthetic management of modified electroconvulsive therapy for a patient with coronary aneurysms: a case report

BackgroundModified electroconvulsive therapy (m-ECT) is utilized worldwide as an effective treatment for drug-resistant psychiatric disorders. However, during m-ECT, treatment of hypotension and hypertension in response to rapid hemodynamic changes is required. We used noninvasive continuous blood pressure monitoring system for continuous hemodynamic measurement during m-ECT.Case presentationThe patient was a 77-year-old man with depression complicated by coronary artery aneurysms (CAAs). We managed general anesthesia during m-ECT by using the ClearSight™ system (Edwards Lifesciences Corp, Irvine, CA, USA) for hemodynamic measurement. As a result, we performed a total of 10 m-ECTs. No rupture of CAAs or myocardial ischemia occurred and depressive symptoms improved.ConclusionWe successfully managed the anesthesia in m-ECT for a depressed patient with CAAs without complications by using the ClearSight™ system, which was used for the effective management of circulatory fluctuations.

Non-invasive continuous blood pressure monitoring systems: current and proposed technology issues and challenges.

High blood pressure (BP) or hypertension is the single most crucial adjustable risk factor for cardiovascular diseases (CVDs) and monitoring the arterial blood pressure (ABP) is an efficient way to detect and control the prevalence of the cardiovascular health of patients. Therefore, monitoring the regulation of BP during patients' daily life plays a critical role in the ambulatory setting and the latest mobile health technology. In recent years, many studies have been conducted to explore the feasibility and performance of such techniques in the health care system. The ultimate aim of these studies is to find and develop an alternative to conventional BP monitoring by using cuff-less, easy-to-use, fast, and cost-effective devices for controlling and lowering the physical harm of CVDs to the human body. However, most of the current studies are at the prototype phase and face a range of issues and challenges to meet clinical standards. This review focuses on the description and analysis of the latest continuous and cuff-less methods along with their key challenges and barriers. Particularly, most advanced and standard technologies including pulse transit time (PTT), ultrasound, pulse arrival time (PAT), and machine learning are investigated. The accuracy, portability, and comfort of use of these technologies, and the ability to integrate to the wearable healthcare system are discussed. Finally, the future directions for further study are suggested.

InstaBP: Cuff-less Blood Pressure Monitoring on Smartphone using Single PPG Sensor.

Cuff-less Blood Pressure (BP) monitoring has gained interest of the research community in recent years, due to its importance in continuous and non-invasive monitoring of BP for early detection of hypertension, thereby reducing mortality. Several approaches that involve photoplethysmography (PPG) and Pulse Transit Time (PTT) have been explored with promising results; however the requirement of two sensors makes them obtrusive for continuous use. Single PPG sensor approaches using machine learning have also been attempted, but there are certain deficiencies in these methods as they go for a one-size-fits-all approach. In this work, we develop an ensemble of BP prediction models based on demographic and physiological partitioning. Also, we incorporate a set of unique PPG features into our models, which results in test accuracies of 5 mmHg Mean Absolute Error (MAE) for Diastolic BP, and 6.9 mmHg MAE for Systolic BP. Given our marked improvement over ubiquitous models (18% for Diastolic BP and 11.5% for Systolic BP), this approach opens up avenues where single PPG sensor based methods can predict BP with a high degree of accuracy. This is a big step towards developing continuous BP monitoring systems, and can help in better management of cardiac health.

Cuff-Less and Continuous Blood Pressure Monitoring: A Methodological Review

Blood pressure (BP) is one of the most important monitoring parameters in clinical medicine. For years, the cuff-based sphygmomanometer and the arterial invasive line have been the gold standards for care professionals to assess BP. During the past few decades, the wide spread of the oscillometry-based BP arm or wrist cuffs have made home-based BP assessment more convenient and accessible. However, the discontinuous nature, the inability to interface with mobile applications, the relative inaccuracy with movement, and the need for calibration have rendered those BP oscillometry devices inadequate for next-generation healthcare infrastructure where integration and continuous data acquisition and communication are required. Recently, the indirect approach to obtain BP values has been intensively investigated, where BP is mathematically derived through the “Time Delay” in propagation of pressure waves in the vascular system. This holds promise for the realization of cuffless and continuous BP monitoring systems, for both patients and healthy populations in both inpatient and outpatient settings. This review highlights recent efforts in developing these next-generation blood pressure monitoring devices and compares various mathematical models. The unmet challenges and further developments that are crucial to develop “Time Delay”-based BP devices are also discussed.

Cuff-less PPG based continuous blood pressure monitoring: a smartphone based approach.

Cuff-less estimation of systolic (SBP) and diastolic (DBP) blood pressure is an efficient approach for non-invasive and continuous monitoring of an individual's vitals. Although pulse transit time (PTT) based approaches have been successful in estimating the systolic and diastolic blood pressures to a reasonable degree of accuracy, there is still scope for improvement in terms of accuracies. Moreover, PTT approach requires data from sensors placed at two different locations along with individual calibration of physiological parameters for deriving correct estimation of systolic and diastolic blood pressure (BP) and hence is not suitable for smartphone deployment. Heart Rate Variability is one of the extensively used non-invasive parameters to assess cardiovascular autonomic nervous system and is known to be associated with SBP and DBP indirectly. In this work, we propose a novel method to extract a comprehensive set of features by combining PPG signal based and Heart Rate Variability (HRV) related features using a single PPG sensor. Further, these features are fed into a DBP feedback based combinatorial neural network model to arrive at a common weighted average output of DBP and subsequently SBP. Our results show that using this current approach, an accuracy of ±6.8 mmHg for SBP and ±4.7 mmHg for DBP is achievable on 1,750,000 pulses extracted from a public database (comprising 3000 people). Since most of the smartphones are now equipped with PPG sensor, a mobile based cuff-less BP estimation will enable the user to monitor their BP as a vital parameter on demand. This will open new avenues towards development of pervasive and continuous BP monitoring systems leading to an early detection and prevention of cardiovascular diseases.

Separate estimation of long- and short-term systolic blood pressure variability from photoplethysmograph.

This paper proposed a method to monitor systolic blood pressure (BP) variability without using a cuff during the daytime. In this method, BP variability of long-term and short-term were separated and estimated respectively from features of phoplethysmograph (PPG) through the use of a frequency filter. Then, total variability was obtained from the combination of long-term and short-term. BP by using a cuff (ground truth) and PPG of nine healthy young subjects were measured during the daytime; then BP variability was estimated from PPG to verify the validity of our method. As a result, the correlation coefficients between measured BP variability and estimated BP variability was improved from r = 0.35 in previous method to r = 0.41 in proposed method. In particular, the estimation results in short-term BP variability showed good accuracy (r = 0.67). This method of estimating BP variability has the potential to be a simple and continuous BP monitoring system during the daytime.

Evaluation of Baroreflex Sensitivity by the Sequence Method Using Blood Pressure Oscillations and R–R Interval Changes during Deep Respiration

Background: Baroreflex sensitivity assessments have been considered to be important to evaluate cardiac autonomic neuropathy. The phenylephrine method, Valsalva maneuver or sequence method at rest caused several problems. We evaluated the usefulness of the sequence method during deep respiration. Method: Baroreflex sensitivity was evaluated in 20 normal volunteers and 50 patients with Parkinson’s disease. R–R intervals and systolic blood pressures were obtained by electrocardiogram and tonometry using a continuous blood pressure monitoring system. The sequence method is an evaluation of baroreflex sensitivity using sequences of 3 or more consecutive beats for 4 min. Baroreflex sensitivity was also assessed by the Valsalva maneuver at 5 beats before the peak systolic blood pressure of phase IV. The slope of the linear interrelationship between systolic blood pressure and the following R–R interval, i.e. baroreflex sensitivity (ms/mm Hg), was calculated with a correlation coefficient greater than 0.8. Result: The mean value of baroreflex sensitivity obtained by the Valsalva maneuver was 7.91 in normal volunteers and 5.35 in patients with Parkinson’s disease; the one obtained by the sequence method at rest was 9.10 in normal volunteers and 8.42 in patients with Parkinson’s disease, and the one obtained by the sequence method during deep respiration was 10.23 in normal volunteers and 6.73 in patients with Parkinson’s disease. In some cases with Parkinson’s disease, baroreflex sensitivities could not be found, whereas in all patients with Parkinson’s disease, the sequence method during deep respiration could be used for evaluations. Significant correlations were found among the baroreflex sensitivities obtained by the Valsalva maneuver, and the sequence method at rest or during deep respiration in normal volunteers and patients with Parkinson’s disease. Conclusions: The baroreflex sensitivity obtained by the sequence method during deep respiration could be investigated noninvasively in all cases with PD, being thus a useful method for clinical evaluation of baroreflex sensitivity.

An evaluation of a new continuous blood pressure monitoring system in critically ill patients

Study Objective: To determine if the DxTek monitor, which is a device that measures blood pressure (BP) noninvasively and continuously by means of pulse velocity and wave shapes derived from the pulse oximeter optical plethysmograph and electrocardiogram is as accurate as an oscillometric cuff device when compared with intraarterial BP measurement. Design: Prospective, comparative study. Setting: University Medical Center. Patients: 28 intensive care unit patients. Interventions: Blood pressures were reported every minute by intraarterial catheters and DxTek and every 10 minutes by an oscillometric monitor for 2 to 5 hours. DxTek calibration was performed initially and when specified patient manipulations by caretakers were performed (on average, every 100 minutes). Comparisons with intraarterial pressure included: 1) DxTek calibrated with arterial catheter pressure, 2) DxTek calibrated with oscillometric pressure, and 3) oscillometric pressure. Measurements and Main Results: When comparing oscillometric pressure to intraarterial pressure, the averages of the mean differences (bias) were −4.0 mmHg for systolic (SBP) and < 1.5 mmHg for diastolic (DBP) and mean (MAP) pressures. The averages of the standard deviation of the differences (precisions) were 9.6, 6.4, and 6.3 mmHg, respectively. With the DxTek device calibrated to intraarterial pressure, comparison of the DxTek pressure to intraarterial pressure resulted in a bias ≤0.5 mmHg for all three pressures and an average precision of 10.1 mmHg for SBP, 6.0 mmHg for DBP, and 6.7 mmHg for MAP. With the DxTek device calibrated to the oscillometric pressure, the DxTek pressure compared to the intraarterial pressure resulted in average biases of −5.1, −0.8, and −2.2 mmHg and average precisions of 11.1, 7.7, and 8.1 mmHg for SBP, DBP, and MAP, respectively. Conclusions: The DxTek monitor provides continuous, noninvasive BP measurements with an accuracy comparable to oscillometric devices.

Seven elderly edentulous patients without cardiovascular disorders were used in this study, and changes in the blood pressure were examined during impression taking for complete dentures. Blood pressure was measured continuously both during border molding and during final impression using a uninvasive continuous blood pressure monitoring system based on arterial tonometry.1. During border molding, systolic blood pressure increased in some subjects (elevation group) but not in others (non-elevation group).2. There was no difference in changes in the blood pressure among various sites of border molding.3. Elevation of the blood pressure was greater in all subjects during final impression than during border molding. The mean increasing rate was 24.1%.It was suggested that impression taking greatly affects blood pressure in elderly edentulous patients.

Instantaneous orthostatic hypotension in children and adolescents: a new entity of orthostatic intolerance.

We are the first to report clinical characteristics and circulatory and catecholamine responses to postural change in 44 children with instantaneous orthostatic hypotension (INOH). The symptoms include chronic fatigue, orthostatic dizziness, weakness, sleep disturbance, syncope or near syncope, headache, and loss of appetite. We divided the patients into two groups: group I (30 patients) had either a recovery time for mean arterial pressure of >25 s or a recovery time of >20 s with a 60% or greater decrease in mean arterial pressure at the initial decrease; group II (14 patients) had a prolonged reduction in systolic arterial pressure of > 15% during the later stage of standing (3-7 min) in addition to the criteria for group I. INOH was characterized by a marked reduction in blood pressure at the initial decrease (mean, -55/-27 mm Hg systolic/diastolic). Delayed recovery time of >60 s was found in 21 of 44 patients and orthostatic tachycardia (>35 beats per minute) in 20 of 44. Plasma noradrenaline responses were significantly lower in group I and II than in controls at 1 min of standing and were lower in group II at 5 min of standing. These results suggest that mechanisms responsible for INOH may depend on insufficient sympathetic activation during standing, possibly due to centrally mediated sympathetic inhibition, thus causing impairment of quality of life including school absenteeism. INOH is an important pathologic condition in children with complaints of orthostatic intolerance and can be an unrecognized cause of chronic fatigue. This condition can be identified by using a noninvasive beat-to-beat continuous blood pressure monitoring system.
