Use of a customized rigid clear matrix for fabricating provisional veneers.

The fabrication of provisional veneers is time consuming, and may be unpredictable, especially in cases of multiple veneer preparations; however, functional and esthetic provisional veneers, may be used as diagnostic adjuncts for the fabrication of the definitive restoration. This article presents, in a step-by-step procedure, the use of a customized rigid clear matrix with light-cured composite resin as a fast alternative for the fabrication of functional and esthetic provisional composite resin veneers. The use of a customized rigid clear matrix for the fabrication of light-cured composite resin provisional veneers has the following advantages: the shape and surface texture produced in the diagnostic wax-up are accurately transferred to the patient's mouth; the provisional restoration is fabricated without violation of the soft tissue and the margins of the preparations; use of the matrix can be repeated when needed; and esthetic and functional provisional veneers can be fabricated relatively quickly.

Torsional bond strength and failure pattern of ceramic brackets bonded to composite resin laminate veneers

The objectives of this study were to investigate the torsional bond strength of ceramic brackets bonded to composite resin veneer laminates and to human enamel. Microfilled resin veneers were bonded directly to prepared, etched bovine teeth embedded in epoxy resin. Brackets [Allure IV (NSB), Fascination, Starfire TMB, and Transcend 2000] were bonded to abraded, acid-etched resin veneers with a light-cured or a chemically-cured adhesive. Brackets were also bonded to human teeth with light-cured and chemically-cured adhesives for comparison purposes. After 24 hours storage in water, specimens were subjected to torsional stress and the maximum shear stress tau max, was calculated. The debonded brackets, the veneer, and enamel surfaces were examined under a stereomicroscope and a SEM to study the failure modes. Three-way ANOVA with a Tukey multiple comparisons test revealed significant differences in bond strengths among bracket types and bonding substrate at P = 0.05 level. Highest bond strength was observed in brackets with a combination of micromechanical retention and chemical adhesion. Significant interactions among bracket, substrate (enamel or resin) and mode of cure of adhesive were observed. Analysis of the failure pattern of brackets revealed adhesive and/or cohesive resin failures in all brackets studied, while cohesive bracket failures occurred in the single-crystal Starfire TMB bracket and the polycrystalline Transcend 2000 bracket. Debonding ceramic brackets under a steady torsional load caused no substrate surface alterations regardless of adhesive used.

Tooth stiffness with composite veneers: A strain gauge and finite element evaluation

Objectives. This study was conducted to determine the impact of composite veneer procedures on the functional properties of incisors. Methods. Ten extracted human maxillary central incisors were mounted in pairs in a nylon ring. One strain gauge was bonded along the long axis of each tooth on the center of the lingual surface. Each pair formed half of a Wheatstone bridge circuit and was wired to eliminate all but the voltage resulting from experimentally applied procedures. The teeth were ramp-loaded to 50 N near the incisal edge on the lingual surface. Loading was performed on the unaltered teeth, teeth with preparations and restored teeth. Two-dimensional finite element (FE) models were generated to evaluate each test condition. Relative stiffness, compared with the unaltered tooth, was calculated from measurements with the strain gauge steps and from the FE models. Results. A relative stiffness value of unity represents recovery of stiffness to the level of the unaltered tooth. Both methods of evaluation demonstrated a decrease in mean relative stiffness with each subsequent reduction in tooth structure. The composite restoration increased its mean relative stiffness compared to its corresponding preparation but never to the level of the unaltered tooth. Across all procedures, the two-dimensional FE model correlated well in both direction and magnitude with the experimental strain gauge method (R = 0.83). Significance. A resin composite veneer does not restore the stiffness to the level of an unaltered tooth.

Effect of an adhesive primer on the integrity of occlusal veneer-metal interface and wear of composite resin veneered restorations

This study examines the influence of a metal-resin bonding agent on the wear rate of composite resin veneer. The specimens were prepared with a silver-based casting alloy and a light-cured composite veneering material. After application of repeated loading through a three-body wear testing device, the amount of wear was measured. The results showed no significant difference in wear between bonded and nonbonded specimens on the 1.5 mm areas of the veneer-metal interface. However, the bonded specimens exhibited significantly lower wear values at the veneer-metal interface. The use of an adhesive primer effectively enhanced the integrity of the occlusal veneer-metal junction.

Shear strength of laboratory-processed composite resins bonded to a silane-coated nickel-chromium-beryllium alloy

The shear bond strengths of three commercial laboratory curing composite resin veneers bonded to a nickel-chromium-beryllium alloy treated with the Silicoater system were evaluated. Two light-cured resins and one heat- and pressure-cured resin were evaluated. No statistically significant difference in bond strengths among the three resins was found. Microscopic analysis of the fracture surfaces indicated that all failures were complex and cohesive in nature within the resin and composite. On the basis of the shear bond strengths measured, any of the composite resin veneers tested appear to be clinically acceptable.

The effect of bead attachment systems on casting patterns and resultant tensile bond strength of composite resin veneer cast restorations

This study compared the difference in tensile bond strength between the composite resin veneer and the cast Ni-Cr disk when different bead adhesives were used to make the laboratory patterns. Visio-Gem, cyanoacrylate, and shellac were the adhesives tested. Fifty-six composite resin bonded Ni-Cr disks were prepared and tested to tensile failure with the Kemper-Kilian device. All tested samples showed a complex failure pattern. The results showed that the mean tensile bond strength of the cyanoacrylate group was significantly higher than the other two groups. No significant difference in the mean tensile bond strength was observed between the Visio-Gem and shellac groups. The higher tensile bond strength in the cyanoacrylate group is thought to be attributed to the low rheological property of the adhesive that allowed greater exposure of the bead for retention. Using different adhesives in the fabrication of composite resin veneered-castings may affect the bond strength in the composite resin-metal interface.

Shear bond strengths of resin luting cements to laboratory-made composite resin veneers

Retention problems have been reported with the clinical use of indirect microfilled resin veneers. This study used 24-hour shear tests to assess the bond strengths of such a resin veneer compared with other veneer types. The effect of different resin luting cements and resin veneer surface treatments were analyzed to elucidate factors by which retention could be increased. The six resin luting systems investigated showed a range of bond values. Debonding occurred primarily at the veneer/cement interface. G-Cera material produced the weakest bonds to Isosit-N resin. Surface treatment of Isosit-N veneers resulted in bond strength changes. Sandblasting reduced the force required for bond failure; Special Bond resin increased it slightly. Improving retention of prefabricated resin veneers proved difficult. Etched hybrid resin veneers delivered higher bond strengths than microfilled resin veneers but not significantly. Etched porcelain veneers, however, provided consistently the strongest bond strengths with cohesive, as opposed to adhesive, bond failure.

Restoration of endodontically treated anterior teeth: An evaluation of coronal microleakage of glass ionomer and composite resin materials

A glass ionomer material was evaluated for coronal microleakage in permanent lingual access restorations of endodontically treated anterior teeth. The material was tested as a restoration, placed over a zinc oxide-eugenol base, and as a base with an acid-etched composite resin veneer and a dentinal bonding agent. Restored teeth were thermocycled, immersed in silver nitrate, developed, and sectioned to assess microleakage. Significant coronal leakage was observed with all materials used.

A simplified technique for fabricating esthetic cast metal occlusal surfaces for dentures

This article describes a simplified method for making esthetic cast metal occlusal surfaces. Monoplane posterior acrylic resin block teeth, east in a nickel-chrome alloy, are coated with silane and an esthetic composite resin veneer is applied to the buccal surface. These veneered posterior metal teeth are incorporated in the wax setup and the dentures are processed and finished. The dentist's routine technique for delivering dentures to the patient can be followed.

The use of a direct bonded microfilled composite resin veneer

This technique creates a direct bonded microfilled composite resin veneer that replicates the complex color distribution and surface texture of the natural tooth structure. Duplicating the complexity of natural tooth color can be repeated by recording the different layers of color used in their sequence of placement.

Bonding agent strengths with processed composite resin veneers

Intracoronal veneers of laboratory processed composite resin are currently being employed to produce esthetic restorations that do not involve the bulk or in situ polymerization problems of more traditional veneering systems. The retention of these materials depends on the bonds formed at the veneer/bonding agent interface and the bonding agent/enamel interface. The bond strength of the veneer/bonding agent interface was investigated using resin bonding agents differing in means of polymerization, filler content, and apparent viscosity. The bond strengths were better with materials that were light-cured and less highly filled.

Composite-veneered amalgam restorations

A procedure is described for the construction of composite resin veneers for large amalgam restorations. The resulting restoration combines the physical qualities of amalgam with the esthetic benefits of composite resins. Among the 14 restorations that have been studied for 2 years, none failed mechanically and minor surface discolorations have been satisfactorily corrected by polishing.

Simulation of White Enamel Discoloration in Composite Resin Veneer Restorations

T-■» he acid-etched enamel bonding technique and the wide variety of com­ posite resin materials have revolu­ tionized restorative dentistry for anterior teeth. Fractured incisors and those with developmental color or morphologic de­ fects can be conservatively and often im­ perceptibly restored. When performed with care, such restorations can have years of use before replacement or un­ complicated repair may be required. The visible light-cured fine, superfine, and micro-particle filled composite resins, which are based on Bowen’s1 bisphenol and glycidyl methacrylate (BIS-GMA) formula filled with colloidal silica, offer the best long-term solutions for restora­ tive problems that itivolve anterior teeth (R. E. Jordan, personal communication). Simonsen2 and Jordan and others3 de­ scribe techniques for correction of facial enamel defects. They identified step-bystep procedures for preparing affected teeth and esthetically restoring facial sur­ faces to normal color and form. In some cases, it may be desirable to characterize composite resin facial veneers to simulate enamel color defects. Such treatment may be required to create a homogeneous ap­ pearance of an entire facial surface, create visual harmony among adjacent teeth, or to add small white resin areas resembling enamel hypoplasia to mimic such defects in normal enamel surfaces. This paper describes a technique that was used to modify composite resin ve­ neer restorations to resemble enamel white discoloration on two maxillary cen­ tral incisors.

Important Considerations in the Diagnosis of Surgical Oral Lesions

Gingival recession causes not only aesthetic problems, but problems with oral hygiene, plaque accumulation, speech, and tooth sensitivity. Replacing the missing gingival tissue with composite resin, when indicated, can be a time- and cost-effective solution. Here we report the case of a 25-year-old female who presented with generalized gingival recession. Black triangles were present between the maxillary and mandibular anterior teeth due to loss of interdental tissues, caused by recent periodontal surgery. She also had slightly malposed maxillary anterior teeth. The patient elected to replace gingival tissue with pink composite resin and to alter the midline with composite resin veneers. The first treatment phase involved placement of pink gingival composite to restore the appearance of interdental papilla to her upper (16, 15, 14, 13, 12, 11, 21, 22, 23, and 24) and lower (34, 33, 32, 31, 41, 42, 43, and 44) teeth. Phase two was to place direct composite resin bonded veneers on her upper (16, 15, 14, 13, 12, 11, 21, 22, 23, and 24) teeth to alter the midline and achieve desired colour. The third treatment phase was to level the lower incisal edge shape by enameloplasty (31, 32, 41, and 42) to produce a more youthful and attractive smile. This case report and brief review attempt to describe the clinical obstacles and the current treatment options along with a suggested protocol. Use of contemporary materials such as gingival coloured composite to restore lost gingival tissue and improve aesthetics can be a simple and cost-effective way to manage patients affected by generalized aggressive periodontitis (AgP).
