Radiofrequency Ablation of Benign Thyroid Nodules and Recurrent Thyroid Cancers: Consensus Statement and Recommendations

Thermal ablation using radiofrequency is a new, minimally invasive modality employed as an alternative to surgery in patients with benign thyroid nodules and recurrent thyroid cancers. The Task Force Committee of the Korean Society of Thyroid Radiology has developed recommendations for the optimal use of radiofrequency ablation for thyroid nodules. These recommendations are based on a comprehensive analysis of the current literature, the results of multicenter studies, and expert consensus.

Program Description: The executive committee of the Board of Governors is proud to present its annual miniseminar. There are many topics that are at the forefront of the practice of otolaryngology. This miniseminar will present three topics that are exceptionally important at this time. Reimbursement for Emergency Department coverage, hospital/physician joint ventures, and an update on payments for EHR and meaningful use. Many hospitals are now reimbursing for emergency department (ED) coverage, many are not. Rick Love, MD, headed the BOG Legislative Committee task force on pay for ED call that resulted in the Academy position paper on this matter. He will give us recommendations on how to successfully merge the feeling of obligation to care for hospital emergency room patients in light of increasing overhead, liability, and decreasing physician reimbursement. Dr Love will shed light on how to be remunerated for hospital ED coverage and at the same time stay within OIG regulations. Lawrence Geller, vice president of consulting services for the group Medical Management Associates, is a sought after speaker and consultant on topics related to healthcare. As portions of the new healthcare law become implemented, hospital/physician joint ventures will be a common trend. Mr Geller will present on the implications of such alliances. The government has recommended that our country’s healthcare providers adopt electronic healthcare record keeping. There are multiple monetary incentives attached to meaningful use and 2011 is the first year slated for reimbursement. Michael Koriwchak, MD, an Atlanta-based otolaryngologist is a noted lecturer and consultant on matters of EHR design, implementation, and “meaningful use.” He also has a popular blog dedicated to topics related to EHR and physician practices- “The Wired EMR Practice.” Dr Koriwchak will update attendees on the progress of the EHR initiatives, how you can you stay or get compliant, and whether the government is keeping their end of the bargain. A Meta-study of the General Insurance Reserving Issues Taskforce and Reserving Oversight Committee Research in this area between 2004 and 2009

AbstractThe General Insurance Board of the Faculty and Institute of Actuaries responded to some of the criticisms raised in the Morris review of the Actuarial Profession and in the press by rating agencies and others, regarding the Actuarial Profession's approach to actuarial reserving in general insurance by setting up a taskforce known as the General Insurance Reserving Issues Taskforce (GRIT). The taskforce worked from 2004 to 2006 and produced a significant report, including some new professional content and recommendations for further areas of development and research. Since then, through the GRIT successor body: the Reserving Oversight Committee (ROC), many working parties have formed and many General Insurance Research Organisation (GIRO) presentations and papers have been forthcoming. One area that has been a recurring theme through the last five years is how the Profession models and communicates the uncertainty in the claims reserving process. In the context of recent events in global financial markets, the forthcoming new regulatory framework of Solvency II, and the developments in other professions globally through IFRS and other drivers, it is timely that actuaries take stock of the many changes in our practices over the last five years and consider the direction actuaries should take for the challenges that lie ahead. This paper is a meta-study of the output of GRIT and ROC on reserving and uncertainty, with the intention of meeting these objectives.

Guidelines for the diagnosis and treatment of primary aldosteronism -The Japan Endocrine Society 2009-

The Japan Endocrine Society (JES) attempted to develop guidelines for the diagnosis and treatment of primary aldosteronism (PA). The Task Force Committee (TFC) was composed of a chair, selected by the JES, and additional experts. Systematic reviews of available evidence for Japanese patients were used to recommend the key treatment and prevention. We have evaluated the methods of screening, confirmatory tests and imaging, plus adrenal vein sampling (AVS). Consensus was guided by systematic review of evidence and discussion during each annual meeting of the JES, plus its related meetings, and by e-mail communication. The drafts prepared by TFC were reviewed successively by the members of Research on Intractable Diseases provided by the Japanese Ministry of Health, Labour and Welfare, and in comments from the JES's councilors. At each stage of review, TFC received written comments and incorporated suggested changes. In conclusion, all patients with hypertension should be screened for PA, because of the high prevalence of cardiovascular disease and the current low case-detection rate in Japan. Case detection can be performed in hypertensive patients and those with hypokalemia by determining the aldosterone/renin ratio, and the diagnosis of PA can be confirmed by two of three confirmatory tests. The presence of a unilateral aldosterone-producing adenoma should be established/excluded by AVS by an experienced radiologist, optimally followed by laparoscopic adrenalectomy. In contrast, patients with bilateral adrenal hyperplasia, or those unsuitable for surgery, are optimally treated medically with mineralocorticoid receptor antagonists.

The “J Curve” Problem Revisited: Old and New Findings

This paper critically addresses the issue of the "J-curve" paradox--the finding described in studies performed about 30 years ago indicating that treatment-induced systolic blood pressure values below 120 or 125 mm Hg and diastolic blood pressure values below 75 mm Hg are characterized by an increase, rather than a reduction, in the incidence of coronary events. This paper focuses on four major subjects: 1) the benefits of a lower blood pressure target during treatment; 2) the historical background of the "J-curve" phenomenon; 3) the evidence collected in recent clinical trials regarding the existence of a "J-curve" in treated hypertensive patients; and 4) the recent recommendations by the Task Force Committee of the European Society of Hypertension on blood pressure goals to be achieved during treatment.

Bisphosphonate-related osteonecrosis of the jaw: position paper from the Allied Task Force Committee of Japanese Society for Bone and Mineral Research, Japan Osteoporosis Society, Japanese Society of Periodontology, Japanese Society for Oral and Maxillofacial Radiology, and Japanese Society of Oral and Maxillofacial Surgeons

Bisphosphonates (BPs) have been widely, efficiently, and safely used for the treatment of osteoporosis, malignant hypercalcemia, bone metastasis of solid cancers, and multiple myeloma bone diseases. Accumulating recent reports describe that surgical dental treatments in patients with cancer or osteoporosis who have been receiving intravenous or oral BPs are associated with osteonecrosis of the jaw (bisphosphonate-related osteonecrosis of the jaw, BRONJ). The accurate incidence, clinical backgrounds, and pathogenesis of BRONJ have been unclear and appropriate approaches for prevention and treatment have not been established to date. To address the current situation of BRONJ in Japan, the "Allied Task Force Committee of Bisphosphonate-Related Osteonecrosis of the Jaw," consisting of physicians specializing in bone biology, orthopedic surgery, rheumatology, obstetrics/gynecology, and medical oncology and dentists specializing in oral surgery, periodontology, dental radiology, and oral pathology, was organized. The committee attempted to propose a standard position paper for the treatment of BRONJ. The committee expects that this proposal will provide objective and correct scientific information on BRONJ and will serve as a reference for conducting dental procedures for patients receiving BPs and in designing prevention and treatment of BRONJ. However, because this position paper is not based on direct clinical evidence, it should be used as a reference, and a decision on treatment in each case should be made after an extensive discussion among physicians, dentists/oral surgeons, and the patients.

Approaches to Phase 1 Clinical Trial Design Focused on Safety, Efficiency, and Selected Patient Populations: A Report from the Clinical Trial Design Task Force of the National Cancer Institute Investigational Drug Steering Committee

The goals and objectives of phase 1 clinical trials are changing to include further evaluation of endpoints such as molecular targeted effects, in addition to dose-toxicity profile of the investigational agent. Because of these changes in focus, the National Cancer Institute and Investigational Drug Steering Committee's Task Force on Clinical Trial Design met to evaluate the most efficient ways to design and implement early clinical trials with novel therapeutics. Clinical approaches discussed included the conventional 3 + 3 cohort expansion phase 1 design, multi-institutional phase 1 studies, accelerated titration designs, continual reassessment methods, the study of specific target patient populations, and phase 0 studies. Each of these approaches uniquely contributes to some aspect of the phase 1 study, with all focused on dose and schedule determination, patient safety, and limited patient exposure to ineffective doses of investigational agent. The benefit of labor-intensive generation of preliminary biomarker evidence of target inhibition, as well as the value of molecular profiling of the study population, is considered. New drug development is expensive and the failure rate remains high. By identifying patient populations expected to respond to the study agent and tailoring the treatment with a novel drug, investigators will be one step closer to personalizing cancer treatment. The "fail early and fast" approach is acceptable if the appropriate patient population is evaluated in the phase 1 trial. The approaches outlined in this overview address the merits, advantages, disadvantages, and obstacles encountered during first in human studies.

Changing Times Require a Changing Middle Grades Research Agenda

Research Vincent A. Anfara, Jr. As I reflected upon theme this issue, Level Education in Changing Times, I reviewed many of topics that have been covered in What Research Says column. This column first appeared in Middle School Journal in May 1979. The list of topics includes, among others, teacher behavior; classroom management; parent involvement; advisory programs; single-sex education; courageous, collaborative leadership; core curriculum; lesson studies; grade configuration; and professional learning communities. As we focus on changing times, we likewise need to look at changing landscape of educational research as relates to middle schools and young adolescents. As part of this examination of middle grades research and its changing nature, history of National Middle School Association's efforts to promote middle grades research and to facilitate generation of knowledge needed to inform practice and policy are reviewed. This history will start in 1997 with publication of A 21st Century Research Agenda: Issues, Topics 8c Questions Guiding Inquiry into Middle Level Theory 8c Practice (National Middle School Association, 1997), travel through publications Research and Resources in Support of This We Believe (Anfara, et al., 2003) and R^sup 3^: Research, Rhetoric, and Reality: A Study of Studies (Hough, 2003), and conclude with an in-depth look at a current publication edited by Mertens, Anfara, and Roney (2009). A brief history of development of a middle grades research agenda In 1997, Blue Ribbon Research Agenda Task Force and Research Committee of National Middle School Association published A 21st Century Research Agenda: Issues, Topics & (Questions Guiding Inquiry into Middle Level Theory & Practice. The recommendations research contained in A 21st Century Research Agenda (NMSA, 1997) centered on 12 characteristics of developmentally responsive middle level schools, which were, at that point in time, delineated in 1995 edition of NMSA's This We Believe. It is essential to recognize that these same characteristics were identified by Association Supervision and Curriculum Development (1975) in its publication of The Middle School We Need, by National Association of Secondary School Principals (1985) with publication of An Agenda Excellence at Middle Level, and by Carnegie Task Force on Education of Young Adolescents' Turning Points: Preparing American Youth 21st Century (Carnegie Council on Adolescent Development, 1989) as essential to creating exemplary middle schools. The authors of A 21st Century Research Agenda (NMSA, 1997) noted that research yet been fully addressed (p. 3) and that for some time there has been a need substantive discussion among interested parties regarding vigorous and continued research that is based on a thorough and thoughtful research agenda middle level (p. 3). By stating that research yet been fully addressed, authors acknowledged that research foundation middle school movement was relatively new, since history of middle schools dates back only to 1960s. In years since inception of middle schools, middle level researchers and practitioners have been busy seeking answers to questions regarding effects of programs, policies, and pedagogical changes in middle schools across nation. The authors of A 21st Century Research Agenda (NMSA, 1997) commented that the most common questions relative to middle level education are unlike most common questions now being posed of all reform initiatives: Does work, and how do you know? (p. 5). They were careful to note that it is not a single construct but a complex set of interwoven, interconnected variables (p. 5), which we commonly refer to as middle school concept. …

Development of Agribiotechnology and Biosafety Regulations Used to Assess Safety of Genetically Modified Crops in Bangladesh

Bangladesh is on the verge of adopting genetically modified (GM) crops for commercial cultivation and consumption as feed and food. Most of the laboratories are engaged in tissue culture and molecular characterization on plants, whereas some have started living modified organism research with shortages of trained manpower, infrastructure, and funding. Nutritionally improved Golden Rice, biotech brinjal, and late blight-resistant potato are in contained trials in a greenhouse, and potato ring spot virus-resistant papaya is in the process of approval for a field trial. The government has taken some initiative in support of GM organism research, which include the formation of a Biotechnology Department in all institutes and the formation of the apex body, the National Task Force Committee on Biotechnology of Bangladesh under the chairpersonship of the Prime Minister. Biosafety policy guidelines and related aspects of biotechnology issues have been approved, and the laws are in the process of being promulgated. Being a party to the Cartagena Protocol, proper biosafety measures are regulated by the appropriate authority as stated. Although there are no laws made yet directly for biosafety of GM crops/foods, the relevant laws on agriculture, medicine, food, import, trade, environment, etc. may suffice and explain the situation.

Improved Neurologic Outcome After Implementing Evidence-Based Guidelines for Cardiac Surgery

A high incidence of neurologic complications was observed in the year 2001 in cardiac surgical patients in this department. This article attempts to show the impact of changing and optimizing management protocols on the incidence of neurologic morbidity after cardiac surgery. An observational study of cardiac surgical patients. University hospital. All cardiac surgical patients treated postoperatively in the cardiac surgical ICU in 2001 (n = 1,165, control group) and in 2003 (n = 1,222, intervention group) were evaluated. A quality improvement program started at the beginning of 2002, based on the Task Force Committee Guidelines, included surgical and cardiopulmonary bypass recommendations as well as peri- and postoperative care (eg, use of epiaortic echo and strict perfusion protocol, avoidance of hyperthermia and hyperglycemia, and minimization of cerebral edema). The number of neurologic complications decreased from 78 (6.7%) in 2001 to 33 (2.7%) in 2003 (p < 0.01), and corresponding numbers for ICU mortality were 44 (3.8%) and 24 (2.0%) (p < 0.01). The length of ICU stay also decreased (3.2 +/- 4.5 days in 2001 v 2.9 +/- 5.5 days in 2003, p < 0.001). In 2001, patients with neurologic complications consumed 853 ICU patient days (23% of all ICU patient days) and, in 2003, 549 (15% of all ICU patient days). According to logistic regression analysis that included 11 independent variables (treatment year, EuroSCORE, diabetes mellitus, history of stroke, and 7 different types of surgery), treatment in 2003 was independently associated with decreased risk for neurologic complications (odds ratio 0.30, 95% confidence intervals 0.19-0.47, p < 0.001). The occurrences of neurologic complications, mortality, and ICU resource consumption by this patient group decreased after implementation of an optimized management protocol and evidence-based guidelines.

Priorities Among Effective Clinical Preventive Services: Results of a Systematic Review and Analysis

Decision makers at multiple levels need information about which clinical preventive services matter the most so that they can prioritize their actions. This study was designed to produce comparable estimates of relative health impact and cost effectiveness for services considered effective by the U.S. Preventive Services Task Force and Advisory Committee on Immunization Practices. The National Commission on Prevention Priorities (NCPP) guided this update to a 2001 ranking of clinical preventive services. The NCPP used new preventive service recommendations up to December 2004, improved methods, and more complete and recent data and evidence. Each service received 1 to 5 points on each of two measures--clinically preventable burden and cost effectiveness--for a total score ranging from 2 to 10. Priorities for improving delivery rates were established by comparing the ranking with what is known of current delivery rates nationally. The three highest-ranking services each with a total score of 10 are discussing aspirin use with high-risk adults, immunizing children, and tobacco-use screening and brief intervention. High-ranking services (scores of 6 and above) with data indicating low current utilization rates (around 50% or lower) include: tobacco-use screening and brief intervention, screening adults aged 50 and older for colorectal cancer, immunizing adults aged 65 and older against pneumococcal disease, and screening young women for Chlamydia. This study identifies the most valuable clinical preventive services that can be offered in medical practice and should help decision-makers select which services to emphasize.

The Contemporary Presidency: Polarization and White House/Legislative Relations: Causes and Consequences of Elite-Level Conflict

Partisan polarization is like the cell phone of American politics--ubiquitous, occasionally annoying, and often the cause of public policy pileups when distracted lawmakers swerve into oncoming legislative traffic. Like the omnipresent technological device, partisanship is now one of the most dominant features of contemporary American politics. Trends and questions about Republican and Democratic voters sorting themselves into cohesive ideological camps is a popular and widely researched theme among political scientists (Abramowitz, Alexander, and Gunning 2005; Fiorina with Abrams and Pope 2005). Polarization Congress is a hot research topic among legislative scholars as well (Aldrich 1995; Bond and Fleisher 2000). Whether it is party in the or party in the Congress, partisanship now significantly affects voters, elections, and the lawmaking process a dramatic manner (Jacobson 2002). Experts might debate whether political parties offer policy alternatives, but there is little doubt they are responsible for much of the polarization today's public policy environment. How times have changed from the days when the American Political Science Association formed a task force (Committee on Political Parties 1950) promoting clearer distinctions and clarity between the Or, from 1968, when George Wallace said, There's not a dime's worth of difference between the two major parties. Dee and Tweedle Dumb similarities appear quaint compared to the choices offered, for example, the last presidential campaign between President Bush and Senator Kerry, or, perhaps even more strikingly, to the daily verbal arrows today's congressional leaders sling at each other. Yet despite extensive research and writing by academics and pundits about the impact of partisan polarization on Congress and the electorate, there is less known about how it affects the contemporary presidency (see Fleisher and Bond 1996; Sinclair 2000 for some notable exceptions). Specifically, how does growing polarization influence White House relations with the Congress and what way? This article explores the contours of polarization on Capitol Hill, how the White House navigates these shoals, and the impact of partisanship on the president's legislative strategies, tactics, and behavior. I argue polarization impacts the White House's relations with Congress a variety of ways, creating a unique set of challenges promoting an agenda and keeping cordial relations with lawmakers. I also posit that unified government may represent the political equivalent of polarization on steroids--amplifying already increasing trends toward partisanship. Finally, I ask whether increased conflict between Democratic congressional leaders and the White House is both a consequence of voter polarization and a cause of it. Partisan Polarization, Promoting an Agenda, and Fostering Relations with Lawmakers The combination of hotter partisan rhetoric from congressional leaders and a more polarized electorate had several implications for White House relations with lawmakers that came to a head the White House of George W. Bush 2001. The next section of the article outlines the consequences of blending unified government and political polarization on White House legislative relations over the past five years based on a series of interviews conducted with former White House staff serving Presidents George H. W. Bush, Bill Clinton, and George W. Bush. Presidential meetings with lawmakers take many forms. Congressional leadership meetings at the White House with the president's party only, as well as bipartisan groups, are common. So are meetings between the president's emissaries and lawmakers on the Hill. But the frequency, tone, and participants vary a great deal depending on factors such as divided versus unified government and the willingness of either the president or his opponents Congress to engage dialogue and cooperation. …
