Transvaginal Sonographic Findings of Isolated Intramural Uterine Choriocarcinoma Mimicking an Interstitial Pregnancy

The differential diagnosis of highly vascular, intramural lesions of the myometrium Includes arteriovenous malformation, gestational trophoblastic neoplasia, hemangiomata, sarcoma, and interstitial pregnancy. We present an unusual case in which a patient with irregular uterine hemorrhage in the presence of increasing serum β-human chorionic gonadotropin (β-hCG) levels exhibited transvaginal sonographic and color Doppler imaging findings considered consistent with an interstitial pregnancy. After laparoscopy and curettage yielded normal findings but with the continuously rising serum β-hCG levels and the concern for gestational trophoblastic neoplasia, the patient underwent vaginal hysterectomy. Histopathologic findings confirmed choriocarcinoma.

Color Doppler imaging of paragangliomas in the neck.

In this study, we describe the color Doppler imaging findings in carotid body tumors and vagal body tumors. B-mode and color Doppler imaging were performed on 17 patients who had a total of 25 previously diagnosed paragangliomas (14 carotid body tumors and 11 vagal body tumors). Nineteen of 25 tumors were depicted. Five small vagal body tumors in the region of the nodose ganglion and 1 carotid body tumor could not be depicted. With B-mode imaging, paragangliomas appeared as well-defined, solid, hypoechoic masses. With color Doppler imaging, hypervascularity with a low-resistance flow pattern was demonstrated in all but 1 of the 19 tumors. The use of color Doppler imaging in the workup of an ambiguous neck mass is advocated.

Color Doppler imaging findings were reviewed for 36 lymph nodes in 13 patients with oral cancer who underwent neck dissection. Seventeen lymph nodes were histologically diagnosed as metastatic and 19 as non-metastatic. Color flow was observed in 6 nodes, all of which were non-metastatic. None of the 17 metastatic lymph nodes had color flow. Al-though 13 of the 19 non-metastatic lymph nodes also had no detectable color flow, all of these nodes were less than 10mm in diameter.These results suggest that normal or inflammatory swollen lymph nodes have color flow detectable with color Doppler imaging, and that metastasis to these nodes leads to the loss of color flow. We conclude that color Doppler imaging facilitates the diagnosis of cervical lymph node matastasis.

Superficial masses with color flow Doppler imaging.

Color Doppler imaging findings and spectral waveforms were reviewed for 21 superficial masses in which color Doppler imaging revealed flow. This series included 5 benign hyperplastic lymph nodes, 5 malignant masses, 2 staphylococcus abscesses, and 9 pseudoaneurysms. Benign hyperplastic nodes had a vascular pattern consisting of prominent central flow with a branching radial pattern, reflecting the morphology of lymph nodes. One metastasis from melanoma demonstrated abundant peripheral flow and central avascularity, while the other 4 malignant masses and the 2 abscesses had sparse internal flow. All pseudoaneurysms had a to-and-fro imaging and spectral pattern (into the mass during systole, out during diastole), and/or a swirling pattern on real-time images (8 of 9 each). Doppler spectral analysis revealed continuous forward flow during diastole for all other masses and abscesses. Waveforms appeared similar for benign and malignant masses, and for abscesses. Conventional image-directed Doppler examinations performed prior to color Doppler studies failed to reveal flow in 2 partially thrombosed pseudoaneurysms, 1 malignant mass, and 1 abscess. Benign hyperplastic nodes, malignant masses, and pseudoaneurysms had distinctive color Doppler image appearances in this series, although demonstration of flow within a mass does not eliminate the possibility of abscess. Proper use of color and spectral Doppler analysis of superficial masses involves more than mere detection or exclusion of flow.

Color Doppler Imaging Provides Accurate Assessment of Orbital Blood Flow in Occlusive Carotid Artery Disease

Color Doppler imaging was used to evaluate the hemodynamics of the ophthalmic vasculature in a case of complete internal carotid artery occlusion. This procedure, which allows rapid, noninvasive imaging, showed a partial ophthalmic artery obstruction with absent flow in the central retinal artery, central retinal vein, and nasal posterior ciliary arteries. Although altered perfusion of the retinal vessels may be evaluated clinically, assessment of blood flow in the ophthalmic and ciliary arteries previously could be evaluated only indirectly by intravenous fluorescein angiography. The color Doppler imaging findings were confirmed by intravenous fluorescein angiography and carotid arteriography. Color Doppler imaging represents a noninvasive method to diagnose abnormal blood flow of the ophthalmic artery and its branches and to evaluate serial changes of the circulation in a noninvasive manner.

Iatrogenic femoral arteriovenous fistula: diagnosis with color Doppler imaging.

Color Doppler imaging was used to identify and localize arteriovenous fistulas in six patients after cardiac catheterization. In all six, there was a focal region of persistent intraarterial flow throughout the cardiac cycle at the site of the fistula, in contrast to normal arterial flow, which is forward only in systole. In all cases, turbulent pulsatile venous flow was present at the site of the fistula and nonturbulent pulsatile venous flow was present proximal to the fistula. In five cases a focal area of apparent extravascular tissue vibration was identified. In four cases the communication between the artery and vein was identified. Familiarity with these color Doppler imaging findings should allow for accurate noninvasive diagnosis of arteriovenous fistulas, at the patient's beside if necessary.
