Professor Liberato J.A. DiDio - A great anatomist of the 20th century and an advocate of medicine without borders

Liberato J. A. DiDio (1920?2004) was one of the most prominent figures of anatomy belonging to the 20th century?s second half and an open-minded man. In 1984, during the era of communism in Yugoslavia, he opened the doors of the Medical College of Ohio (MCO) in Toledo, OH, USA, to a Serbian doctor. During the troubled times for people and anatomists in Serbia in 1994, he saved their association from being expelled from the International Federation of Anatomical Associations. In 1999, only a few months after the bombing of Yugoslavia, he helped them to get the organization of the XVIII International Symposium on Morphological Sciences in 2005, the meeting of the leaders in the field. Serbian anatomists and clinicians proved that he was right when considering them on a par with their peers in the international anatomical and medical community. Professor DiDio first showed talent with Gold Medal ? top graduate at his high school, and La Royale Award (Graduation Golden Ring) ? top graduate MD. He was trained in Brazil, Italy, and the US. He was the Founding Chairman ? Department of Topographical Anatomy, Faculdade de Ciencias Medicas, Belo Horizonte, Minas Gerais, Brazil; Head of Gross and Surgical Anatomy, Northwestern University Medical, Dental, and Graduate Schools; Founding Chairman, Department of Anatomy, MCO; Professor Emeritus at the age of 70 (1990), Assistant to the President of the MCO, Consultant to the President and the Emeritus Dean (1992?2004). He was a member of editorial boards of 34 journals, academic adviser in 92 M.S. and Ph.D. theses.

The field of gastroenterology is divided into 12 sections that are reviewed once a year. He graduated from the Santa Casa de Misericordia School of Medicine in São Paulo, Brazil, and moved to the Cleveland Clinic Foundation, in Cleveland, Ohio, USA, where he received additional training in internal medicine, gastroenterology and basic immunology. Dr Fiocchi is currently Professor of Molecular Medicine, Cleveland Clinic Lerner College of Medicine, Cleveland, Ohio and Professor of Pathology and Pediatrics at Case Western Reserve University School of Medicine, Cleveland, Ohio, USA. In addition, he is a Staff Member in the Departments of Pathobiology, Lerner Research Institute, and Gastroenterology & Hepatology, Digestive Disease and Surgery Institute, Cleveland Clinic Foundation. Dr Fiocchi has published numerous papers and has served or serves on the editorial boards of several journals, including Gastroenterology, American Journal of Physiology, Inflammatory Bowel Diseases, Journal of Crohn's & Colitis, Current Opinion in Gastroenterology, and Digestive and Liver Disease. He has been or is a member of numerous study sections of the National Institutes of Health and the Crohn's & Colitis Foundation of America. He received the prestigious MERIT Award from the National Institutes of Health, and the Clifford and Jane Anthony Chair in Digestive Disease Research and Education. In addition, he is the recipient of the Premier Physician Award and the Henry Janowitz Lifetime Achievement Award in Inflammatory Bowel Disease from the Crohn's & Colitis Foundation of America, and the International Herbert Falk Award, Germany. He is also Honorary Foreign Member of the Brazilian Academy of Medicine. Dr Fiocchi's research is focused on the pathogenesis of inflammatory bowel disease. Studies include the identification of mechanisms of mucosal T-cell death and survival in gut inflammation, investigation of tolerance of mucosal T-cells for the autologous enteric flora and its possible loss in IBD, characterization of the interactions between mucosal T-cells and endothelial and stromal cells, and the study of immune-driven angiogenesis and fibrogenesis in intestinal inflammation. Stuart J. SpechlerStuart J. SpechlerStuart J. Spechler, MD is Chief of the Division of Gastroenterology at the Dallas VA Medical Center, Professor of Medicine, and holder of the Berta M. and Cecil O. Patterson Chair in Gastroenterology at The University of Texas Southwestern Medical Center at Dallas, USA. He is a graduate of Boston University School of Medicine, USA, and he spent many years working in Boston, both at Boston University and at the Beth Israel Deaconess Medical Center, USA, and Harvard Medical School, USA, where he was the Director of the Center for Swallowing Disorders from 1990 to 1997. He has been in Dallas on the faculty of UT Southwestern Medical Center since 1997. Dr Spechler's research has focused primarily on disorders of the esophagus, especially gastroesophageal reflux disease and its complications. He chaired the VA Cooperative Study on gastroesophageal reflux disease that was conducted in the 1980 s, and he now chairs a new VA Cooperative Study on the treatment of GERD that is refractory to treatment with proton pump inhibitors. Dr Spechler has published more than 300 scientific reports, editorials, review articles, and book chapters on esophageal disorders. He is perhaps best known for his work in the areas of GERD, Barrett's esophagus and, most recently on eosinophilic esophagitis. Dr Spechler has served on the editorial boards of numerous journals including Gastroenterology, Gut, Alimentary Pharmacology and Therapeutics, and Diseases of the Esophagus, and he has served as Chair of the Gastroenterology Teaching Project and the Esophago-gastro-duodenal Section of the American Gastroenterological Association.

David A. Margolin, M.D.

Dr. David A. Margolin is a Staff Colon and Rectal Surgeon and Director of Colorectal Surgery Research at the Ochsner Clinic Foundation in New Orleans, LA. Born in Cleveland, Ohio, David received an M.D. degree from the Medical College of Ohio, Toledo. His general surgery training was conducted at Case Western University in Cleveland, which included 2 years of research. He then performed a fellowship in Colon and Rectal Surgery at the Ochsner Clinic in New Orleans, LA. After his fellowship, he joined the staff at Henry Ford Hospital in Detroit, MI. In January 2003, David was recruited to the staff at the Ochsner Clinic. Dr. Margolin serves as a reviewer for Diseases of the Colon and Rectum. He has published more than 20 peer-reviewed articles and multiple book chapters on colorectal subjects. He is also an editor of the forthcoming textbook, Improved Outcomes in Colon and Rectal Surgery. He is a member of multiple medical societies and has served as the Chairman of the American Society of Colon and Rectal Surgeons Socioeconomic Committee. Dr. Margolin maintains a very busy clinical practice in which laparoscopy and the management of diverticular disease play an integral role. He is an active researcher, but still finds time to educate medical students, surgical residents, and colorectal fellows. David and his wife, Jeanne Rademacher, M.D., a practicing pediatrician, live with their two children (Elizabeth, age 13 and Eric, age 10) in Metairie, a suburb of New Orleans. When not working and raising children, David attends local sporting events, enjoys the unique cultural activities of New Orleans such as Jazzfest and Mardi Gras, and supports the local world class eateries. He also strives to become an avid golfer. On behalf of the readers and staff of Clinics in Colon and Rectal Surgery, I thank Dr. Margolin for serving as Guest Editor and along with his contributors for providing us with a superb issue on diverticular disease.

Regulation of the Dynamics of hsp90 Action on the Glucocorticoid Receptor by Acetylation/Deacetylation of the Chaperone

It is known that inhibition of histone deacetylases (HDACs) leads to acetylation of the abundant protein chaperone hsp90. In a recent study, we have shown that knockdown of HDAC6 by a specific small interfering RNA leads to hyperacetylation of hsp90 and that the glucocorticoid receptor (GR), an established hsp90 "client" protein, is defective in ligand binding, nuclear translocation, and gene activation in HDAC6-deficient cells (Kovacs, J. J., Murphy, P. J. M., Gaillard, S., Zhao, X., Wu, J-T., Nicchitta, C. V., Yoshida, M., Toft, D. O., Pratt, W. B., and Yao, T-P. (2005) Mol. Cell 18, 601-607). Using human embryonic kidney wild-type and HDAC6 (small interfering RNA) knockdown cells transiently expressing the mouse GR, we show here that the intrinsic properties of the receptor protein itself are not affected by HDAC6 knockdown, but the knockdown cytosol has a markedly decreased ability to assemble stable GR.hsp90 heterocomplexes and generate stable steroid binding activity under cell-free conditions. HDAC6 knockdown cytosol has the same ability to carry out dynamic GR.hsp90 heterocomplex assembly as wild-type cytosol. Addition of purified hsp90 to HDAC6 knockdown cytosol restores stable GR.hsp90 heterocomplex assembly to the level of wild-type cytosol. hsp90 from HDAC6 knockdown cytosol has decreased ATP-binding affinity, and it does not assemble stable GR.hsp90 heterocomplexes when it is a component of a purified five-protein assembly system. Incubation of knockdown cell hsp90 with purified HDAC6 converts the hsp90 to wild-type behavior. Thus, acetylation of hsp90 results in dynamic GR.hsp90 heterocomplex assembly/disassembly, and this is manifest in the cell as a approximately 100-fold shift to the right in the steroid dose response for gene activation.

RATS THAT CAN'T RUN CAN'T HIDE FROM RISK FACTORS

At a time when transgenic organisms are all the rage in biological research, it's nice to know that some hypotheses can still be tested with experimental animals created the old-fashioned way: through selective breeding. Since 1996, Steven Britton and Lauren Koch of the University of Michigan have used rats bred for high and low aerobic exercise capacity to address numerous questions about exercise performance, cardiac function and cardiovascular regulation. Their latest study of these rats - a collaboration with colleagues at the Norwegian University of Science and Technology, the Medical College of Ohio, and Williams College (Massachusetts) - probes the apparent link between aerobic capacity and cardiovascular and metabolic diseases.Aerobic capacity, abbreviated V̇O2max, is an animal's maximum rate of oxygen consumption per unit time. It is often measured by having an animal run on a treadmill that gets progressively steeper until the animal becomes exhausted. At the cellular level, V̇O2max reflects the ability of the mitochondria to use oxygen for ATP production. Individuals with a low V̇O2maxtypically have a lower-than-normal density of mitochondria in their muscle cells, the chief oxygen consumers during intense exercise.Beginning with a mixed population of rats, the researchers mated the best treadmill runners with each other and did likewise for the worst runners,testing the offspring's running ability and selecting the extreme individuals for the next round of mating. After 11 generations of this selective breeding,the population of high-capacity runners could run about 350% farther than the low-capacity runners during a progressive treadmill test. The team found that the low-capacity runners exhibited a variety of symptoms associated with an increased risk of cardiovascular and metabolic diseases. These included high blood pressure, impaired relaxation of blood vessels, insulin resistance,impaired cardiac pumping ability, and elevated triglyceride and free fatty acid levels in the blood. To add insult to injury, the low-capacity runners were also fatter than the high-capacity runners.These data add new weight to previous suggestions that a low V̇O2maxper se can lead to the development of symptoms like those listed above. If there is indeed a cause-and-effect relationship, it may well involve the mitochondria; several transcription factors and enzymes essential for mitochondrial function were 60-90% lower in the low-capacity runners' muscle than in the high-capacity runners' muscle. However, the possible connections between these proteins and cardiovascular variables such as blood pressure remain open to speculation at present.To what extent can exercise training protect individuals from the risks associated with low V̇O2max? When the team trained low-capacity runners on treadmills for six weeks, they noticed that several measures of cardiac function improved along with V̇O2max. However,they did not quantify the effects of training on the other above-mentioned risk factors; presumably, the team will examine these in a subsequent report. Aerobically impaired rats and humans will undoubtedly await these results with interest.

Prem C. Chandnani, MD

born on November 24, 1924, in Kotri, Sind, in prepartition India. He passed away on September 10, 2004, in Toledo, Ohio, following complications of a stroke incurred in May 2004. Dr Chandnani attended the DJ Sind College in Karachi, Pakistan, where he earned a BS degree. He then attended the G. S. Medical College in Mumbai, India, where he attained his MD degree in 1950. After completing medical school, he pursued further medical specialization in India and attained his MSurg degree. Dr Chandnani chose to subspecialize in the field of neurosurgery. He wanted to obtain the highest level of knowledge and the best education possible. Consequently, he went to Oslo, Norway, from 1959 to 1960, for advanced clinical training in neurosurgery. He returned to India and practiced neurosurgery in Mumbai and Ahmedabad, India. After moving to the United States in 1967, he changed his medical specialty to radiology and underwent training in that field from 1968 to 1971. In 1971, Dr Chandnani joined the Department of Radiology at the Medical College of Ohio in Toledo as an assistant professor. He advanced his credentials and went on to become a professor of radiology. From 1982 to 1984, he was acting chairman of the Department of Radiology. He was board certified in both radiology and nuclear medicine. After his retirement in 1995, the Board of Trustees at the Medical College of Ohio conferred on him the title of professor emeritus. He was the first person from the Department of Radiology at the Medical College of Ohio to be given that honor. Dr Chandnani was a follower of the principles of Mahatma Gandhi. He was optimistic, generous, modest, and spiritual. Because of his gentle and kind-hearted demeanor, as well as his professional skills, he has positively touched and influenced the lives of many with whom his path crossed. His main hobby was investing, specifically, the challenge of applying his analytic skills to an area that is ostensibly random. He is survived by his wife, Shilloo, a pathologist; his son, Vijay, a radiologist; and his daughter, Seema, a lawyer.

Student-run medical clinics in the United States: A national survey

Study objectives: By serving indigent populations, student-run medical clinics expose medical students to administrative, clinical, and public health issues similar to those of a typical emergency department. This study describes operational and functional characteristics of student-run medical clinics on a national level. Methods: This prospective, anonymous, mailed survey of student-run medical clinics was conducted at the Medical College of Ohio. Surveys were mailed to all identified student-run medical clinics in the United States. Results: To date, data have been collected on 33 programs. Of these programs, clinics were initiated between 1968 and 2003. Participants report a mean 607.4 patient visits per year (range 50 to 2500), or 11.8 patients per session. Clinics use a mean 11.8 volunteer employees (range 4 to 48) and 0.7 paid employees (range 0 to 5). The mean annual operating budget was $17,352 (range $200 to $100,000). Most clinics routinely care for uninsured patients (60%), and 33% specifically provide services to homeless patients. Of specialty services, adult medicine (97%), obstetrics and gynecology (76%), and pediatrics (76%) were most common. Services identified as most commonly available onsite include urine dipstick analysis (available in 91% of student-run medical clinics), glucose (91%), Papanicolaou test (88%), urine or serum pregnancy test (85%), urinalysis (82%), serum chemistries (82%), CBC count (82%), serum lipid levels (79%), gonorrhea and chlamydia testing (76%), guaiac (73%), streptococcal antigen (73%), prostate specific antigen (70%), radiography (67%), cultures (64%), stress testing (64%), mammography (61%), and oxygen saturation (61%). Conclusion: Student-run medical clinics provide numerous important services to patients and often serve uninsured and homeless patients. Considerable variation exists in services available to patients.
