Comparative analysis of different filling materials in deciduous teeth

Dental caries in pediatric patients presents a significant challenge in maintaining oral health, making the choice of filling materials for deciduous teeth crucial for long-term success. Various restorative materials, including composite resins, glass ionomer cements (GICs), and resin-modified glass ionomers (RMGICs), have been used, each with distinct properties regarding durability, biocompatibility, and aesthetic outcomes. Composite resins are well-regarded for their ability to closely match the natural color of teeth, making them ideal for anterior restorations. However, they are technique-sensitive and may not provide sufficient durability in posterior regions subjected to high stress. Glass ionomer cements, known for their ease of placement and fluoride release, offer functional benefits in high-caries-risk patients, despite their lower aesthetic quality and susceptibility to wear over time. Resin-modified glass ionomers attempt to combine the strengths of both composites and GICs by improving durability and aesthetics while maintaining the fluoride-releasing property. The safety and biocompatibility of these materials also play a pivotal role in pediatric dentistry. While composite resins may release small amounts of bisphenol A (BPA), which has raised concerns about its potential health risks, GICs and RMGICs are generally considered safer due to their simpler chemical composition and fluoride release. In terms of functional longevity, composite resins offer better wear resistance compared to GICs, though resin-modified glass ionomers provide a compromise with improved strength and ease of use. Ultimately, the choice of restorative material should consider the specific clinical circumstances, including the child’s caries risk, tooth location, and the balance between aesthetic and functional needs. No single material is universally ideal for all cases, and a tailored approach is necessary to optimize outcomes in pediatric dental restorations.

Advanced Material Strategy for Restoring Damaged Endodontically Treated Teeth: A Comprehensive Review.

The restoration of endodontically treated teeth (ETT) remains a significant challenge in modern dentistry. These teeth often suffer from substantial structural damage due to both the original pathology and the invasive nature of endodontic procedures. Consequently, ETT are more susceptible to fractures compared to vital teeth, necessitating restorative strategies that can effectively restore both function and aesthetics while minimizing the risk of failure. In recent years, advances in adhesive dentistry and the development of high-strength ceramics have further expanded the restorative options for ETT. Bonded restorations have gained popularity as they preserve more tooth structure and enhance the overall strenght of the tooth-restoration complex. The choice of restorative material and technique is influenced by numerous factors, including the amount of remaining tooth structure, the functional requirements of the tooth, and the aesthetic demands of the patient. Despite the plethora of available materials and techniques, the optimal approach to restoring ETT remains a topic of ongoing research and debate. In this comprehensive review, the current state of and recent advances in restoring damaged endodontically treated teeth are explored. Numerous therapeutic options exist, involving a wide range of materials. This article aims to present the biomaterial advancements of the past decade and their applications, offering alternative approaches to treating damaged ETT with the goal of prolonging their retention on the dental arch and serving as a valuable resource for dental practitioners who face this issue daily.

In Vitro Evaluation of Mechanical Properties of Cention N and Its Comparison with Resin Modified Glass Ionomer Cement (RMGIC) Restorative Material as Used in Primary Teeth.

22 specimens prepared with Cention and RMGIC were embedded in primary teeth mounted in acrylic for analysing shear bond strength. Shear bond strength was analysed using a universal testing machine. The modes of failure in samples were observed under a stereomicroscope and scanning electron microscope. 22 customised samples of Cention N and RMGIC were prepared and categorised as group A and group B, respectively. The flexural and compressive strengths of these samples were evaluated using a universal testing machine. The shear bond strength of RMGIC was higher than that of Cention N, whereas the compressive and flexural strengths of Cention N were significantly higher than those of RMGIC. The modes of failure were predominantly adhesive followed by mixed failures. The results of this study suggest that Cention N demonstrated superior mechanical properties compared with RMGIC and can therefore be recommended for restorations in primary posterior teeth. Cention N being a smart, esthetic, self-cured, or dual-cured material with better mechanical properties offers a wide range of applicability in primary teeth.

Survival of Endodontically Treated Maxillary Anterior Teeth with Aesthetic Post-Restoration: A Prospective Intervention Study

Introduction: There is much debate over the best choice of restorative material and the placement method for the most successful treatment. Many teeth that have undergone endodontic treatment may not have enough structure to support a restoration or crown. To improve retention for the restorative material, ferrule may be used. Adhesive techniques have transformed the way we restore teeth after root canal treatment, starting with the shift from using cast post-and-core to using fibre post. The use of fibre-reinforced posts has been found to prevent irreparable root fractures thereby improving the longevity of the treatment. Aim: To compare two different types of aesthetic fibre posts for 12 months. Materials and Methods: A prospective interventional study was carried out in the Department of Prosthodontics, Manav Rachna Dental College (Faridabad) for a period of 12 months from Nov 2022- Oct 2023. Forty participants with single rooted, endodontically treated, maxillary anterior teeth in the age bracket of 15-55 years, both males and females of the north Indian population were included in the study. Teeth were selected where a post-retained crown was indicated and post space was prepared using a piezo reamer drill at 5000 rpm. At least 4 mm of obturation is left in the canal to maintain the apical seal. Furthermore, Zircon-enriched silicon fibre and glass fibrereinforced posts were cemented into the prepared post space. Metal-ceramic crowns were cemented and baseline periapical radiographs were obtained. At the interval of one month, three months, six months and one year, the patients were evaluated. The Fischer’s exact test was utilised to establish the association between categorical variables and the level of significance (p-value <0.05). Results: One year follow-up revealed no failures in crown movement when evaluated under finger pressure, recurrent caries, or root fractures. One case of glass fibre-reinforced post showed a fracture of the post and another case of glass fibre post showed periapical radiolucency at 6-month intervals on clinical and radiographical examination which was significant when compared to Zircon-enriched silicon fibre post group (p-value=0.02). Conclusion: Both aesthetic fibre-reinforced post systems used in single-rooted upper anterior teeth demonstrated similar success rates in restoring root canal treated teeth.

The impact of occlusal forces on the longevity of restorations

The longevity of dental restorations like fillings, crowns, and bridges is critically influenced by occlusal forces exerted during biting and chewing. These forces vary among individuals based on diet, masticatory habits, and the structural soundness of the dental arch and can range from 11 to over 450 Newtons, depending on circumstances. The choice of restorative material is important for withstanding these forces. While amalgam is durable, modern dentistry has shifted towards aesthetic restoratives such as composite resins and ceramics. However, these materials respond differently to occlusal forces. Ceramics might fracture under excess load, whereas composite resins resist wear but can degrade over time due to other factors. Precision in restoration is crucial to preventing undue occlusal forces that might lead to restoration failure. Tools like T-scan assist in this precision. The manner and distribution of occlusal forces, along with the health of the periodontal ligament, are critical for restoration durability. Bruxism, a parafunctional habit, can significantly impact restorations, necessitating protective measures like occlusal splints and patient education. The interplay of occlusal forces and the chosen restorative material greatly affects the success and lifespan of dental restorations.

Strength of implant-supported lithium disilicate and polymer-infiltrated ceramic network restorations after thermomechanical aging.

To evaluate the fracture load and type of failure of two different monolithic restorative materials bonded to standardized titanium bases and fabricated by two different procedures regarding the bonding interface. All screw-retained implant crown specimens (n = 40), subjected to fatigue by thermomechanical loading, differed in the restorative material (lithium disilicate [LDS] or polymer-infiltrated ceramic network [PICN], referred to as 'hybrid ceramic' [HYC]) and the interface type between the restorative material and the titanium base abutment (prefabricated ex-factory or produced during a CAM-milling procedure). This resulted in the following groups (n = 10/group): 1) LDS-M: lithium disilicate crown with a CAM-milled interface; 2) LDS-P: lithium disilicate crown with a prefabricated interface; 3) HYC-M: PICN crown with a CAM-milled interface; and 4) HYC-P: PICN crown with a prefabricated interface. Aged specimens underwent static fracture load testing. The load (N) at which the initial crack occurred was denoted as Finitial, and the maximal load (N) at which the restorations fractured as Fmax. All specimens were examined under a stereomicroscope to determine the failure mode. The median Finitial values were 180 N for LDS-M, 343 N for LDS-P, 340 N for HYC-M, and 190 N for HYC-P. The median Fmax values were 1822 N for LDS-M, 2039 N for LDS-P, 1454 N for HYC-M, and 1581 N for HYC-P. The intergroup differences were significant for Finitial (KW: P = 0.0042) and for Fmax (KW: P = 0.0010). The failure types also showed differences between the restorative groups. The choice of restorative material had a stronger influence on the fracture load than the abutment interface workflow. Lithium disilicate showed the highest load for initial crack appearance (Finitial) and for complete fracture of the restoration (Fmax).

Rehabilitation with Table-Top Restoration Post Bicuspidisation: An Interdisicplinary Case Report

The first teeth to erupt in the oral cavity are the mandibular molars which are more prone to endodontic and periodontal involvement. This may lead to significant issues like an increase in pocket depth, furcation involvement, loss of clinical attachment and mobility of teeth ultimately resulting in tooth loss. Thus, it necessitates cautious oral hygiene measures. Advances in dentistry have led to treatment modalities for preserving teeth which were earlier indicated for extraction. Hence, periodontally compromised tooth with severe furcation involvement, which may not be amenable for regeneration, may be retained by bicuspidisation. Bicuspidisation is a surgical technique which involves separation of mesial and distal roots of mandibular molars along with its crown portion, where both segments are then retained individually, followed by rehabilitation with fixed prosthesis of the individual segments. This not only eliminates furcation involvement but also facilitates effective oral hygiene maintenance. In the present case report, a 50-yearold male patient reported with the chief complaint of pain, sensitivity and food lodgement in left mandibular first molar. Clinical and radiographic examination revealed grade III furcation involvement, significant pocket depths, delayed electric pulp test response and bone loss in furcation area. Bicuspidisation was opted as the suitable treatment modality. Root canal treatment was done, followed by bicuspidisation and subsequent rehabilitation with novel prosthetic technique. The bicuspids were restored using combination of deep margin elevation, and non splinted Lithium Disilicate table-top restorations. Six month follow-up showed good healing of hard and soft tissues around the bicuspidized sections, which was indicative of a successful treatment outcome, and can be attributed to a careful case selection, choice of restorative materials and techniques as well as good oral hygiene compliance.

Relevant optical properties for gingiva-colored resin-based composites

ObjectivesTo evaluate the optical properties of gingiva-colored resin-based composites (GCRBCs). MethodsFive discs (8 mm diameter x 1mm height) of 17 shades of GCRBCs were prepared. Diffuse reflectance was measured against white and black backgrounds using a calibrated spectroradiometer, CIE D65 illuminant and the CIE 45⁰/0⁰ geometry. Relative translucency parameter was calculated using ΔE00 (RTP00). Translucency differences were evaluated using published data of 50:50% translucency perceptibility (TPT00) and acceptability (TAT00) thresholds. Scattering (S) and absorption (K) coefficients and transmittance (T%) were calculated using Kubelka–Munk's equations. Data were statistically analyzed using Kruskal–Wallis, Mann–Whitney tests, and VAF coefficient. ResultsThe RTP00 values of the 17 evaluated shades ranged from 8.69 to 21.34. There were perceptible translucency differences (TPT00=0.62) between different shades of the same brand and between composites designated with the same shade of different brands. Spectral distributions of S, K and T were wavelength-dependent. Although the spectral behavior of the S and K coefficients and T% were similar for all the gingival composites evaluated, the values of these parameters presented statistically significant differences between shades, which would justify the differences found in the relative translucency parameter. ConclusionsThe optical properties S, K and T% of GCRBCs were significantly different, resulting in perceptible translucency differences between the same shade of different commercial brands and between different shades of the same brand. Clinical significanceTranslucency differences of gingiva-colored composites may significantly influence their masking ability affecting the clinician's choice of restorative material.

A Systematic Study of Restorative Crown-Materials Combinations for Dental Implants: Characterization of Mechanical Properties under Dynamic Loads.

This study aimed to find the optimum mechanical characteristics of the restorative materials for the manufacture of implant crowns subjected to impact loading when different combinations of materials are used for the inner and outer crown. Several combinations of external–internal crown restorative materials were analyzed. The dynamic stresses at eight different zones of a dental implant subjected to an impact load and the influence of several mechanical properties, such as the Young’s modulus, Poisson’s ratio, density, and initial velocity, were analyzed and compared. A detailed 3D model was created, including the crown, the retention screw, the implant, and a mandible section. The model was then built by importing the 3D geometries from CAD software. The whole 3D model was carefully created in order to guarantee a finite element mesh that produced results adjusted to physical reality. Then, we conducted a numerical simulation using the finite element method (FEM). The results of the FEM analysis allowed for evaluating the effect that different combinations of restorative materials and mechanical properties had on the stress distribution in various regions of the implant. The choice of restorative material is a factor to be considered in order to preserve the integrity of osseointegration. Restorative materials transfer more or less stress to the dental implant and surrounding bone, depending on their stiffness. Therefore, an inadequate Young’s modulus of the rehabilitation material can affect the survival of the implant over time. Eight interactive graphics were provided on a web-based surface platform to help clinical dentists, researchers, and manufacturers to select the best restorative materials combination for the crown.

Choice of Restorative Materials by Dental Practitioners and Students in Saudi Arabia for Endodontically Treated Teeth: A Systematic Review

Background: There are several elements to consider when determining the optimal procedure for restoring teeth that have undergone endodontic treatment, including the choice and requirement of a post, the kind of coronal restoration, how much coronal structure is still there, and the kind of cementing agent. The ideal restoration procedure for teeth that have undergone endodontic treatment is yet unknown. The goal of this systematic review was to evaluate the restorative material choices and factors that affect the decision-making on endodontically treated teeth (ETT) among dental professionals and students in the Kingdom of Saudi Arabia. Methods: Studies that evaluated the choice or preference of dental professionals for restorative materials for teeth that had undergone endodontic treatment were chosen from surveys that were published between 2012 and 2022 in English. Selected keyword searches were conducted in the Science Direct, PubMed, and Google Scholar databases. The data (theme-related questions and response rates for each survey question) were extracted using a standardized outline, and the likelihood of bias was evaluated. The information gathered was subjected to descriptive analysis. Results: There were 11 articles in total, encompassing 2192 respondents. The response rates ranged from 30% to 98%. The study population consisted of general dental practitioners (n = 1018), followed by dental students (n = 598), specialists (n = 475), and 101 dental surgeons. The years of experience range from 1 to > 30 years, with the majority claiming to have fewer than 10 years. Moreover, 55% of the studies preferred prefabricated posts, such as fiber posts following metal posts. Most of the respondents preferred resin-based cementing agents (22% to 82%). Over 60% of respondents chose composite resin over amalgam as the material for the core foundation Conclusion: The study results from Saudi Arabian data showed that dentists favored using prefabricated posts, such as fiber and metal-based posts, to restore ETTs and resin-based cementing agents. The preferred material for the core foundation built for ETT by regular dentists, specialists, and students is regarded as composite. The amount of coronal tooth structure that is still present is crucial when choosing the post type and restorative material.

The prevalence, intensity of dental caries and the need for treatment of permanent immature teeth remain high among the child population of the Republic of Belarus. The choice of restorative material and treatment regimen for caries of permanent teeth in children are relevant at the present. The aim of the study was to evaluate the clinical effectiveness of «Admira Fusion» composite material for the restoration of permanent teeth in children. Material and methods. 143 permanent teeth in 52 children and adolescents (average age 13.4 years) were treated using universal nanohybrid «Admira Fusion» composite. Restorations assessment was carried out in 12 and 24 months according to Ryge criteria. Results. In has found that caries experience of permanent teeth in children and adolescents participating in the study according to the DMFT index is 4.3±1.4, which corresponds to the moderate level of caries intensity. The level of oral hygiene according to the OHI-S index is 0.7±0.1, which is assessed as satisfactory hygiene. It has been analyzed that in children and adolescents 51.05 % caries affected teeth were the first permanent molars (73 teeth) and 27.27 % – the second permanent molars (39 teeth). In 11.89 % of clinical cases caries was detected in the first premolars (17 teeth) and in 9.79 % (14 teeth) – in the incisors of the upper jaw. The analysis of lesions localization on the surfaces of permanent teeth has indicated that in children and adolescents most often in 60.46 % treated teeth caries was detected on the occlusal surfaces (I class by Black). In 27.9 % of the restored teeth, combined lesions of the occlusal and proximal surfaces (Black, class II) were recorded: 11.53 % on the distal-occlusal surfaces and 16.27 % on the medial-occlusal surfaces. Isolated lesions of the medial (14.65 %) and vestibular (6.98 %) surfaces (Black, class V) were noted rarely. It has been found high effectiveness of the proposed treatment using universal nanohybrid material «Admira Fusion» for restoration in two years. 100 % retention of restorations were detected. There were no found complaints of toothache or loss of fillings in 100 % clinical cases. 100 % of the restorations were noted with good edge adaptation and anatomical shape. There were no signs of secondary caries in all clinical cases. Evaluation of the shade adaptation after 2 years has shown that 93.01±2.13 % composite restorations was impossible to recognize without a mirror, and 6.99±2.13 % (10) of the restorations had color deviations in contrast to the tooth tissues, but no more than a change in their shade or transparency which is within the normal color and transparency of the tooth. The color deviations were more often registered in the occlusal-proximal and the occlusal restoration (II Black class, 10.3±4.83 % and Black I class, 5.8±2.51 % accordingly). Color assessment of the filling’ edge after 2 years showed no change in the transition from the restoration to the tooth tissues during visual inspection with a mirror in 89.51±2.56 % of the nanohybrid restorations. Discoloration in the area of the filling-tooth junction not extending in depth was detected in 10.49±2.56 % (15) fillings. Most often, edge discoloration was observed in class II restorations by Black (11 teeth) and less often in class I restorations by Black (4 teeth). The evaluation of the surface quality of the fillings has shown, that 2 years after the treatment, 100 % of the restorations were with a shine and smooth surface, did not require correction or re-treatment, and also had no signs of irritation of the surrounding soft tissues. Conclusion. The use of «Admira Fusion» composite for the restoration of permanent teeth in children provides high clinical efficacy in 89–93 % of clinical cases. Thus, the «Admira Fusion» universal nanohybrid material is characterized by great working properties with good adaptation to the tooth hard tissues, retains its aesthetic characteristics in the oral cavity for 2 years. Convenience in work and technical ability to use any adhesive systems make it the material of choice in pediatric dentistry. The Diagnosis & Management of Non-Carious Cervical Lesions by Dental Practitioners from the South Yorkshire and Bassetlaw Regions of England

A survey-based study, utilising qualitative and quantitative questions was used to investigate the diagnosis and the management of non-carious cervical lesions (NCCLs), including an overview to the working and gender demographics of primary dental care practitioners in South Yorkshire. To establish the contributing factors associated with NCCLs; to identify the most common approach used by dentists on how to manage NCCLs, and to identify what restorative materials dentists used and why. Objectives: To investigate, by the use of a questionnaire, the diagnosis & management of non-carious cervical lesions by dental practitioners. Materials and Methods: A sample size calculation yielded a survey size of 149, following initial returns (non-responders) the questionnaire was posted to 300 randomly chosen participants. The questionnaire contained an initial demographic section to ascertain; gender, time since graduation and working times (8 questions), and the second section focused 6 questions upon; NCCL diagnosis, management and treatment options. Likert scale (5 point responses: greatly agree to greatly disagree), freetext boxes, closed-ended and multiple-choice questions were used to collect data from respondents. The response rate was (53.3%). Results: Abrasion, particularly tooth brushing, was believed to be the main cause of NCCLs (87%). An initial monitor/ preventative (fluoride application) approach was taken first, with the rationale to restore being based upon patient reported symptoms, first, and then followed by any aesthetic concerns/ preservation of tooth tissue. When NCCLs were being treated there was a wide range of numbers: modal value of 2, and median value of 5 NCCLs being restored per week. Composite was the aesthetic material of choice to restore NCCLs (57%), followed by (30%) for glass polyalkenoate (GI cements) restorations. Conclusion: Our demographic data indicated that the survey cohort could be considered as a representative sample of primary care dental practitioners in terms; of gender, time since qualification (as a proxy of age), and working practice (NHS vs. Private), and sessions worked per week. There was a strong agreement between dentists that tooth brushing abrasion is the main causes of NCCLs., and while the majority of NCCLs are managed conservatively, composite is the most frequent restorative material used by dentists to restore NCCLs followed by GIC. Dentists are more likely to restore NCCLs to improve sensitivity or to preserve remaining tooth structure, than to improve patients’ aesthetics. The size of the cavity, the anatomical position of the tooth, the nature of the cause, the aesthetic consideration and the materials’ technical properties had an effect on deciding the choice of restorative material.

Choice of Restorative Materials for Direct Posterior Restorations among Undergraduate Saudi College Students

Background: The most common restorative materials used in dentistry are amalgam and composite. Amalgam is a controversial material owing to its mercury toxicity. With recent advances in the properties of composite materials, there has been a shift towards its use. Objectives: The aim of this study was to understand the perceptions of undergraduate dental students in a northern Saudi Arabian dental school about the choice of restorative materials for restoring posterior teeth. Methodology: The study included undergraduate students studying in 4th and 5th year dental program in College of Dentistry, Jouf University. A four-item questionnaire with 18 close-ended questions was developed by the investigators, which were hand delivered to all the students. Data analysis is presented through tables and descriptive methods. Results: A total of 98 (out of 131) undergraduate students participated in this study. Overall, the students reported a significantly strong influence of the type of restorative materials in relation to the cavity size and margin of the restorations along with the esthetics factor(p<0.05). There was slight influence on the student’s choice because of the instructor's influence, whereas the students felt they were knowledgeable and had appropriate training to use either amalgam or composite. The study also found that patient’s preference had a strong influence on choosing composite material. The students were mostly not influenced while choosing the material as far as the safety of it was concerned. However, the patient’s influence was slight when it came to the choice of the material. A significant difference was noted among the students when it came to pregnancy-related safety concerns where the choice of material was not influenced by either amalgam or composite (p=.002). Conclusion: The undergraduate dental students at College of Dentistry, Jouf University are comfortable in using both amalgam and composite as a posterior restorative material. They are knowledgeable about both the materials and are adequately trained to use either one.

Our systematic review is made in view of the absence of studies dealing with failures and reoperations which encompassed all types of dento-supported fixed prosthetic restorations and aims to study prosthetic failures that could be encountered in fixed dento-supported prosthesis and the solutions to remedyOver the past two centuries, considerable progress has been made under the leadership of many scientists who have developed materials and techniques, with the aim of improving the quality of care and improving ergonomics.In short, in fixed prosthesis, failures have particularly serious consequences and can occur at all levels involved for these restorations. They can happen at any time.This is how we were able to note the influence of certaine variable on the success and durability of the treatments, which did not depend only on the skill and precision used during prosthetic development, such as the location, arch, preparation design, number of abutments, type of material used, bonding material, surface treatment and technologies used.So the choice of restorative materials was a decisive factor that had to be the result of a careful study of the different materials available depending on the clinical situation, in order to increase their longevity and therefore reduce the risk of failure.  Â

Survival of primary molars with pulpotomy interventions: public oral health practice-based study in Helsinki

Objectives To assess long-term survival of pulpotomized primary molars and factors related to the survival of the teeth. Materials and methods This retrospective longitudinal study was based on electronic dental health records of children who attended public health service. Eligible children were <12 years and had a pulpotomy procedure code in their dental treatment history. Data were obtained in the period between 2002 and 2016. Chi-squared test, t-test, and one-way ANOVA were used to assess for statistical differences. Kaplan–Meier analyses were performed to create survival estimates, and the log-rank test was performed to compare differences in survival distributions. A multivariate Cox regression was conducted, and hazard ratios were calculated to estimate the hazard rate for failure. Results Of interventions, 40% were registered in children 3–6 years of age; 56% were boys. Mean estimated survival of such molars was 82 months (SEM = 0.6) and differed by dental operator’s work setting and choice of restorative materials used after pulpotomy intervention. Using a stainless steel crown (SSC) to restore pulp-intervened primary molars had the strongest effect on survival (111 months with SEM = 1.8), followed by if the intervention was performed by a specialist or specializing dentist in paediatric dentistry (99 months with SEM = 2.6). Conclusions The estimated survival time of pulpotomized primary molars is affected by dental operator-related factors, which should be considered in management of extensively carious primary molars of paediatric patients, both at the clinical and organizational decision-making levels. An increased focus on the use of SSC is recommended to be in the learning objectives of dental education programs.

Introduction: The effectiveness of caries treatment depends on many factors, such as the condition of dental tissues, the stage of root development, the period of dentition, the quality of preparation and isolation, the optimal choice of material for restoration. The problem of choosing the filling material is acute in each treatment of caries in children with mixed and permanent dentition. The aim: Todetermine and statistically substantiate parameters for choosing restorative material for permanent teeth in children with different stages of root development. Materials and methods: 248 children aged 7 to 11 (122 boys and 126 girls) and 246 children aged 12 to 17 (120 boys and 126 girls),students of Uzhhorod Secondary School №20 were examined. Restoration materials -"Polofil Supra" (VOCO, Germany) and "DyracteXtra" (Densply, USA). The following criteria were investigated: saliva pH, area of destruction of the occlusal surface of masticatory teeth, stage of root development, depth of the lesion and cavity topography, the acid resistance of the enamel. For the purpose of statistical justification of the selected parameters that pointed as significant in the choice of restorative materials, and for the analysis of the interrelations of the parameters studied, the Spearman correlation coefficient (r) was determined. The correlation coefficient was considered statistically significant when p &lt;0.05. Results: For children aged 7-17 years to restore permanent teeth during the period of mixed and permanent dentition, the best materials in use were: "Polofil Supra" (VOCO, Germany) and "DyracteXtra" (Dentsply, USA). Conclusion: To restore permanent teeth in any stage of root development, it is rational to use a compomer material "DyracteXtra" (r = 0.80), and a composite material "Polofil Supra" -under the conditions of the formed root, at a pH level of more than 6.2, a high degree acid resistance of the enamel and the area of destruction of the occlusal surface less than 0.55 (r = 0.72).

THE UTILIZATION PATTERNS OF RESTORATIVE MATERIALS IN PRIMARY MOLARS AMONG PEDIATRIC DENTISTS.

Background: Many changes have occurred in development and availability of dental restorative materials for primary teeth. To date, there has been no emphasis to identify the approaches and materials used by pediatric dentists for primary teeth. The present study was conducted to assess the preferred choice of restorative materials among pediatric dentists for restoring primary teeth in children.&#x0D; Methodology: A self-administered questionnaire was utilized to obtain demographic information. Six hypothetical clinical scenarios were presented and clinicians choice of material for each of these scenario was evaluated.&#x0D; Statistical Analysis: Statistical analysis was done based on chi-square test. Results were displayed as percentages.&#x0D; Results: A total of 62 pediatric dentists completed the questionnaire, of which 97% preferred tooth-coloured materials over amalgam. Respondents reported the most influencing factors in the choice of restorative materials as age, behaviour of child and caries experience.&#x0D; Conclusion: Responses to various hypothetical clinical scenarios showed preference of tooth coloured materials and uniform choice of materials was noted among pediatric dentists irrespective of age or years of clinical experience.&#x0D; Keywords: restorative materials, primary teeth, pediatric dentists

Clinical performance of Equia Forte: A glass hybrid GIC versus Tetric N Ceram; a bulk fill composite in class II carious primary molars: A 12 month split mouth clinical trial

The present study aims to clinically evaluate the performance of a glass hybrid glass ionomer cement (GIC), Equia forte (GC India) and a bulk fill composite, Tetric N Ceram (Ivoclar Vivadent) in restoration of class II carious lesions in primary molars for a period of 12 months. A total of 39 patients of 5-9 years with occlusoproximal carious lesion on primary maxillary or mandibular molars were selected and randomly allotted into two groups i.e Group A (Equia Forte) and Group B (Tetric N Ceram). A total of 78 restorations (39 teeth for each group) was placed using Equia Forte and Tetric N Ceram using a split mouth design and were evaluated for a period of 12 months at regular time intervals using FDI criteria by two blinded examiners for esthetic, functional and biological properties. The data were analyzed using one way ANOVA for intragroup comparison and student t test for intergroup comparison. There was no statistically significant difference seen at baseline, 3, 6, 9 and 12 months for esthetic, functional and biological properties within the group. On inter group comparison, there was no statistically significant result observed except for functional property at 6 months with a p value of 0.02. The current study suggests that both Equia Forte and Tetric N Ceram can be a choice of restorative material in class II carious lesions in primary molars.

Clinical efficacy of treatment of immature permanent teeth in children with different risk of caries

Relevance. Nowadays the choice of restorative material and treatment plan for caries of immature permanent teeth in children remains of current interest. Purpose. To evaluate the clinical efficacy of modified glass-ionomer cement for restoration of permanent immature teeth in children with different risk of dental caries. Materials and methods. The results of the treatment of immature permanent teeth in children with different risk of caries are proposed in this article. 100 permanent immature teeth in children with a low, moderate and high risk of dental caries with acute and chronic process were treated using modified glass ionomer cement. Remineralizing therapy was carried out in children with acute caries for 2 weeks before restoration. The duration of remineralizing therapy in children with a high risk and chronic caries was 2 weeks, in the acute caries – 4 weeks. Remineralizing therapy included application of calcium and phosphate containing agents. Assessment of the fillings was conducted in 12 and 24 months in accordance with Ryge criteria which take into account the anatomical shape, marginal adaptation, marginal pigmentation, color stability, and surface roughness. Results. It has been found the high effectiveness of the proposed method of treatment after two years. 100% retention of restorations were revealed. 80-95% fillings were of satisfactory quality, 5-20% of fillings were of acceptable quality and do not require correction or re-treatment.Conclusions. The modified glass-ionomer cement restorations with previous remineralizing therapy provide the high clinical efficacy for the treatment of caries in permanent immature teeth in children with different risk caries in 85-90% of clinical cases.

How to Intervene in the Root Caries Process? Systematic Review and Meta-Analyses

The aim of this review as part of the preparation for a workshop organized by the European Federation of Conservative Dentistry (EFCD) in conjunction with the European Organisation for Caries Research (ORCA) was to systematically analyze available evidence of non-, micro- as well as invasive interventions for root caries lesions (RCLs). For each treatment strategy, a separate systematic review was either performed (micro-invasive and choice of restorative material) or updated (non-invasive and excavation technique) each of them following PRISMA guidelines, and if possible meta-analyses were performed. Besides the general advice to improve tooth brushing with fluoride toothpaste main findings for non-invasive interventions in RCLs, the use of dentifrices containing 5,000 ppm F^– as well as professionally applied chlorhexidine varnish or silver diamine fluoride seemed to be more efficacious to arrest root caries compared to conventional fluoride toothpaste or placebo respectively. However, this conclusion is based only on a few randomized clinical trials. For micro-invasive treatments, only 2 studies focusing on sealants were available without clear conclusions. A recent review on the comparison of atraumatic restorative treatment compared with conventional treatment concluded that there is insufficient data to clearly rule out if any difference with regard to restoration longevity between both techniques exists. When restoring coventionally, composites performed better than resin-modified and glass ionomer cements. However, all materials showed rather high annual failure rates in the majority of the studies and evidence is based on a low number of prospective studies with a rather high risk of bias.
